هپاتیت B در شهرستان گنبد کاووس: فراوانی نسبی، عوامل خطر و انتشار خانوادگیصفحه 222زمینه و هدفهپاتیت B شایعترین علت هپاتیت مزمن و سیروز در ایران می باشد. اکثر مطالعات انجام شده در مورد هپاتیت B بر روی بیماران بستری در بیمارستان یا اهداکنندگان خون صورت گرفته و کمتر در جمعیت طبیعی انجام شده است. هدف این مطالعه تعیین بررسی فراوانی نسبی آلودگی با ویروس هپاتیت B، عوامل خطر ابتلا به این ویروس و انتشار داخل خانوادگی آن در ساکنین شهرستان گنبدکاووس می باشد.روش بررسیاز طریق نمونه گیری خوشه ایسیستماتیک براساس شماره خانوار، 1035 نفر از ساکنین سه روستا و شهر گنبدکاووس که در فاز پایلوت مطالعه کوهورت سرطان مری گلستان انتخاب شده بودند وارد مطالعه شدند. HBsAg دوباره به فاصله شش ماه تعیین شد و کلیه افرادی که در خانه فرد HBsAg مثبت زندگی می کردند و سن بالاتر از 2 سال داشتند از نظر HBsAg و HBcAb مورد بررسی قرار گرفتند.یافته ها44 نفر (%4.25) از 1035 نفر هر دو بار HBsAg مثبت بودند. از 279 نفری که با افراد HBsAg مثبت در یک خانه زندگی می کردند، 17 نفر (%6) HBsAg مثبت بودند که همگی بالای 12 سال بودند و 20 نفر (%7) HBcAb مثبت داشتند. فراوانی نسبی عوامل خطر ابتلا به هپاتیت های B از طریق خون در افراد HBsAg مثبت و HBsAg منفی تفاوت معنی داری نداشت.نتیجه گیریآلودگی با هپاتیت B در شهر گنبدکاووس از برآورد متوسط آلودگی در ایران بالاتر است. عوامل خطر بررسی شده نقش مهمی در آلودگی با ویروس هپاتیت B در جمعیت طبیعی گنبدکاووس ندارند و انتشار داخل خانوادگی در حدی متوسط است. به نظر می رسد جمعیت زیر 12 سال که برای آنان واکسیناسیون حین تولد انجام شده است از مصونیت قابل ملاحظه ای برای آلودگی با این ویروس برخوردارند.

اثر فلوگستین در بیماران مبتلا به سندرم روده تحرک پذیر با غلبه درد و یبوستصفحه 226زمینه و هدفسندرم روده تحریک پذیر (IBS) شایعترین اختلال تشخیص داده شده توسط متخصصین گوارش است که به طور تقریبی در 40 - 50 درصد از بیماران تحت نظر یک متخصص دیده می شود. این بیماری های با مهارکننده های باز جذب سروتونین (SSRIs) درمان می شود، ولی در مطالعات بالینی، شواهد کافی مبنی بر اثربخشی این داروها وجود ندارد.روش بررسی44 فرد مبتلا به IBS تشخیص داده شده براساس معیارهای روم (Rome II) II، با غلبه علایم درد و یبوست در این مطالعه شرکت داشتند. علل عضوی به وسیله شرح کامل، معاینه بالینی، آزمونهای آزمایشگاهی و کولونوسکوپی رد شد. سپس شرکت کنندگان به طور تصادفی، به منظور دریافت فلوکستین یا دارونما به مدت 12 هفته، به دو گروه تقسیم شدند. این افراد با استفاده از یک سیستم رتبه بندی ساده براساس علایم (simple symptom score system) در طول درمان و 4 هفته پس از اتمام درمان تحت پیگیری قرار گرفتند.یافته هافلوکستین به طور واضحی نسبت به دارونما در کاهش ناراحتی شکمی، تخفیف احساس نفخ، افزایش تعداد حرکات روده ای و کاهش قوام مدفوع، موثرتر بود. رتبه کلی علایم (total symptom score) در گروهی که فلوکستین مصرف کرده بودند از 10.7 به 2.8 و در گروه شاهد از 10.5 به 6.7 کاهش یافت (p<0.001).نتیجه گیریفلوکستین یک درمان موثر و کوتاه مدت قابل تحمل برای بیماران مبتلا به IBS با غلبه درد و یبوست است.

رابطه بین سطح آلانین آمینوترانسفراز سرمی و یافته های آسیب شناسی کبدی در مبتلایان به هپاتیت C مزمنصفحه 232زمینه و هدفدر اغلب بیماری های کبدی، افزایش سطح آلانین آمینوترانسفراز (ALT) می تواند نشانه ای از نکروز سلولی باشد؛ ولی هنوز مطالعه ای در این زمینه در ایران انجام نشده است.روش بررسیاین مطالعه جهت مقایسه درجه (grade)، مرحله (stage) و مقیاس فعالیت بافتی (HAI Score) در دو گروه از مبتلایان به هپاتیت مزمن C با آلانین آمینوترانسفراز سرمی طبیعی و افزایش یافته طراحی شده است. در این مطالعه 32 بیمار با آلانین آمینوترانسفراز سرمی طبیعی و 84 بیمار با آلانین آمینوترانسفراز سرمی افزایش یافته مورد بررسی قرار گرفته اند.یافته هامیانه درجه و مرحله در گروه با سطح طبیعی آنزیم به ترتیب 4 و 1 و در گروه با سطح افزایش یافته آنزیم به ترتیب 6.5 و 2 به دست آمد. مرحله بزرگتر از 2 به ترتیب در 5 بیمار (%15.6) با سطح طبیعی آنزیم و 36 بیمار (%42.9) با سطح افزایش یافته آنزیم مشاهده شد (p=0.006).نتیجه گیریاگرچه ضایعات کبدی در بین مبتلایان به هپاتیت مزمن C با سطح طبیعی آلانین آمینوترانسفراز سرمی خفیف ترند، لیکن نمونه برداری و بررسی آسیب شناسی کبدی به عنوان یک روش تشخیصی مفید در تمامی مبتلایان به هپاتیت مزمن C، صرف نظر از سطح سرمی آلانین آمینوترانسفراز توصیه می گردد.

آنتی بادی ضد ویروس هپاتیت A در ناقلین ویروس هپاتیت Bصفحه 237زمینه و هدفبا توجه به اهمیت ابتلا به هپاتیتA در مبتلایان به بیماری های مزمن کبدی، مطالعه حاضر با هدف تعیین شیوع هپاتیت A در بیماران مبتلا به هپاتیت B انجام شد.روش بررسیمطالعه پژوهشی حاضر به صورت مقطعی انجام شد. 283 بیمار مبتلا به هپاتیت B)شامل 118 نفر ناقل غیرفعال، 144 نفر مبتلا به هپاتیت مزمن و 21 نفر مبتلا به سیروز(از بین مراجعین به مرکز هپاتیت تهران در 3 ماه زمستان 1383 به صورت ترتیبی وارد مطالعه شدند. تمامی نمونه ها از نظر Anti HAV Ab، متغیرهای دموگرافیک، عوامل خطرساز بیماری های منتقله از راه خون، محل تولد، محل سکونت، سابقه زردی، سابقه بیماری هپاتیت B در خانواده، AST، ALT، پلاکت، درجه (grade)، امتیاز (score) و مرحله (stage) پاتولوژی مورد بررسی قرار گرفتند.یافته ها224 نفر (%79.2) از کل بیماران به هپاتیت A مبتلا بودند. در 89 نفر از ناقلین غیرفعال (%75.4)، 115 نفر از مبتلایان به هپاتیت مزمن (%79.9) و 20 نفر از مبتلایان به سیروز (%95.2)، HAV Ab مثبت بود. 47 نفر از بیماران زیر 30 سال (%65.3)، 123 نفر از بیماران 30 - 49 سال (%80.4) و 54 نفر از بیماران بیش از 50 سال (%93.1) به هپاتیت A مبتلا بودند (p=0.000).نتیجه گیریبرطبق مطالعه حاضر، %80 از بیماران مبتلا به هپاتیت B سابقه عفونت هپاتیت A را نشان می دهند و %20 باقی مانده نیز مستعد ابتلا به هپاتیت A می باشند. با افزایش سن بیماران، میزان ابتلای به عفونت هپاتیت A بیشتر می شود. با توجه به افزایش احتمال مرگ و میر بیماران مزمن کبدی به دنبال ابتلا به هپاتبت A، واکسیناسیون بر ضد ویروس هپاتیت A در این جمعیت توصیه می شود.

بیماری سلیاک در کشورهای در حال توسعه: خاورمیانه، هند و شمال آفریقاصفحه 242به دنبال معرفی آزمونهای سرولوژیک ساده برای تشخیص بیماری سلیاک (coeliac disease) در دهه 1980، به تدریج مشخص شد که شیوع بیماری سلیاک در کشورهای مختلف خاورمیانه، شمال آفریقا و هند تقریبا شبیه آنچه در کشورهای غربی است می باشد. شیوع بیماری سلیاک درجمعیت در معرض خطر در این نواحی بین 3 تا %20 و شیوع در جمعیت با دیابت نوع یک تقریبا %3 - 5 گزارش شده است. تظاهرات بالینی بیماری سلیاک به نحو مشخصی با سن بیمار، طول مدت و وسعت بیماری فرق می کند. مطالعات بالینی نشان داده اند که تظاهر بیماری با نشانه های غیر مشخص یا بدون نشانه، به همان اندازه اروپا در خاورمیانه نیز شایع است.
گندم به مدت چندین قرن در این نواحی غذای غالب بوده است و احتمال دارد که تماس مداوم و به مقدار زیاد با پروتیینهای گندم منجر به القای درجاتی از تحمل ایمنی شده باشد که باعث ایجاد نشانه های خفیف تری می شود که منجر به تشخیص اشتباه به عنوان سندرم روده تحریک پذیر یا بیماری های گوارشی توصیف نشده می شود. در تمام کشورهای در حال توسعه، باید ظن بالینی قوی برای بیماری سلیاک برای بیمارانی که مشکل آنها با اسهال مزمن یا کم خونی فقر آهن تظاهر می یابد، وجود داشته باشد. بهترین روش برای تشخیص بیماری سلیاک در بیماران دچار اسهال، پانل آزمونهای سرولوژیک سلیاک و به دنبال آن بیوپسی روده کوچک است. در غیاب دسترسی به رژیم فاقد گلوتن در کشورهای خاورمیانه، فراهم کردن این رژیم یک چالش واقعی، هم برای بیماران و هم برای پزشکان می باشد.

همزمانی لیپودیستروفی منتشر مادرزادی و پرفشاری پورت؛ گزارش یک مورد و مروری بر مقالات منتشر شدهصفحه 248لیپودیستروفی منتشر مادرزادی (congenital generalized lipodystrophy) بیماری بسیار نادر و پیشرونده ای می باشد 1) در 12 میلیون نفر(. مبتلایان به این بیماری با تظاهرات بالینی آتروفی بافت چربی، قیافه آکرومگالویید، آکانتوزیس نیگریکانس (acanthosis nigricans)، هیپرلیپیدمی (hyperlipidemia)، دیابت قندی یا اختلال تست تحمل گلوکز و به ندرت سیروز کبدی مراجعه می کنند.
بیشترین موارد گزارش شده در مقالات پزشکی 8 مورد از برزیل و 6 مورد از کشورهای اسکاندیناوی بوده است و بقیه موارد به صورت گزارشهای یک یا دو موردی بوده است. بالاترین سن بیماران گزارش شده 19 سال می باشد.
در این مقاله گزارش مورد، به معرفی یک مورد پسر 13 ساله مبتلا به نوع مادرزادی لیپودیستروفی با عارضه نادر سیروز کبدی می پردازیم.

Hepatitis B in Gonbad-Kavoos: Prevalence, Risk Factors and Intrafamilial SpreadingPage 222BackgroundHepatitis B is the most common cause of chronic hepatitis and liver cirrhosis in Iran. Most studies about HBV have been done among hospitalized patients and blood donors and scantly among normal population. The aim of this study was to determine prevalence, risk factors and intrafamilial spreading of HBV in general population of Gonad.Materials And MethodsA total of 1035 inhabitants of three villages and city of Gonbad who were recruited in "Golestan cohort study on esophageal cancer" underwent this study. HBsAg was checked twice in six-month intervals. All family members who lived with a HBsAg positive subject in a home and were older than 2 years underwent HBsAg and HBcAb checking.ResultsForty four out of the 1035 (4.25%) were found to be HBsAg positive twice. Seventy (6%) and 20(7%) out of the 279 family members were positive for HBsAg and HBcAb respectively. All HBsAg positive family members were older than 12 years. There was no significant difference for definite risk factors of HBV (through blood and sexual) between HBsAg positive and HBsAg negative subjects.ConclusionsPrevalence of HBsAg positive in Gonbad is much more than average rate of Iran. Intrafamilial spread is in medium range and defined risk factors for HBV did not have any role in our population. The Effect of Fluoxetine in Patients with Pain and Constipation Predominant Irritable Bowel SyndromePage 226BackgroundIrritable bowel syndrome (IBS) is the most common disorder diagnosed by gastroenterologists seen in about 40-50% of patients seeking care from a specialist. IBS has been treated with selective serotonin reuptake inhibitors, but there is not enough evidence from controlled trials for their effectiveness.Materials And MethodsThe aim of the study was to compare the effects of fluoxetine and placebo in the treatment of pain and constipationpredominant IBS in a double-blind randomized controlled trial. Forty four cases meeting Rome II criteria for IBS with predominance of pain and constipation were included in this study. Organic causes were ruled out by detailed history, physical examination, laboratory tests, and colonoscopy. Participants were then randomly assigned to receive either fluoxetine or placebo for 12 weeks. Subjects were followed using a simple symptom score system during treatment and four weeks after termination of treatment.ResultsFluoxetine was significantly more effective than placebo in decreasing abdominal discomfort, relieving feeling and sense of bloating, increasing frequency of bowel movements and decreasing consistency of stool. The total symptom score decreased from 10.7 to 2.8 in the fluoxetine group vs. 10.5 to 6.7 in controls (p‹0.001).ConclusionsFluoxetine is an effective and well-tolerated short-term treatment for pain and constipation-predominant IBS.Keywords: Intestinal disorders, Motility, nerve, gut interactions, Irritable bowel syndrom, Fluoxetine

The Relationship Between Serum Alanine Aminotransferase Level and Liver Biopsy Findings in Chronic Hepatitis CPage 232BackgroundAmong most of the hepatic diseases, elevation of alanine amino transferase (ALT) level is the hallmark of hepatocyte necrosis. But, no study has been performed in this field in Iran.Materials And MethodsThis study is designed to compare the grade, stage, and Histological Activity Index (HAI score) of liver pathology between patients with normal serum ALT and elevated serum ALT. It has been conducted on 32 patients with normal serum ALT and 84 patients with elevated ALT level, at our clinic.ResultsThe median grade and stage among patients with normal serum ALT level were 4 and 1, and among patients with elevated serum ALT they were 6.5 and 2, respectively. Stages more than 2 were detected in 5 (15.6%) and 36 (42.9%) of normal ALT and elevated ALT groups respectively (p= 0.006).ConclusionsAlthough liver lesions are milder among most of the patients with normal serum ALT, liver biopsy is considered to be useful in chronic hepatitis C regardless of serum ALT level.Keywords: Hepatitis C, Normal enzyme, Liver pathology

Anti HAV Antibody in HBV CarriersPage 237BackgroundWith respect to the importance of hepatitis A in patients with chronic liver disease, current study was conducted with the aim of determining the prevalence of previous hepatitis A infection in hepatitis B virus carriers with different stages of liver disease.Materials And MethodsThis was a cross sectional study. 283 HBsAg positive subjects (including 118 inactive carriers, 144 patients with chronic active hepatitis and 21 cirrhotic subjects)Â were selected from Tehran Hepatitis Center within 3 months period in 2004. All subjects were assessed for Anti HAV Ab, demographic data, risk factors of blood borne disease, place of birth, place of living, history of jaundice, history of hepatitis patients, ALT, AST, Platelet, Stage, Grade and Score.ResultsFrom all subjects, 224 patients (79.2%) were affected with HAV. 89 inactive carriers (75.4%), 115 patients with chronic active hepatitis (79.9%) and 20 cirrhotic patients (95.2%) were Anti HAV Ab positive. 47 patients under 30 year (65.3%), 123 those between 30 and 49 years (80.4%) and 54 over 50 years old subjects (93.1%) were positive for HAV infection (p=0.000).ConclusionsOur results indicated that 80% of patients with hepatitis B have previous infection with hepatitis A and 20% of them are susceptible to it. The frequency of infection rises with increasing age. Because of increased fatality of this infection in higher ages, vaccination against hepatitis A in this population is recommended.Keywords: Hepatitis A, Hepatitis B, Risk factors

Coeliac Disease in Developing Countries: Middle East, India and North AfricaPage 242Following the application of simple serological tests for the diagnosis of coeliac disease (CD) in the 1980s, it gradually became clear that the prevalence of CD in different countries in the Middle East, North Africa and India is almost the same as that in Western countries. The prevalence of CD in at-risk populations in these regions is reported to range between 3 and 20% and the prevalence in people with type 1 diabetes is approximately 3-5%. Clinical manifestations of CD vary markedly with age, the duration and the extent of disease. Clinical studies showed that presentation with nonspecific symptoms or no symptoms is as common in the Middle East as it is in Europe. Wheat has been the major staple food in these regions for many centuries and it is possible that the continuous and high level of exposure to wheat proteins has induced some degree of immune tolerance, leading to milder symptoms, which are misdiagnosed as irritable bowel syndrome or unexplained gastrointestinal disorders. A high index of suspicion for CD should be maintained in all developing countries for patients who present with chronic diarrhoea or iron deficiency anaemia. The best method for diagnosing CD in patients with diarrhoea is the panel of coeliac serological tests followed by small-bowel biopsy. In the absence of supplies for a gluten-free diet in Middle Eastern countries, maintaining this diet represents a real challenge to both patients and clinicians.Keywords: Coeliac disease, Middle East, India, North Africa

Congenital Generalized Lipodystrophy, Coincident with Portal Hypertension: A Case Report and Review of LiteraturePage 248Congenital generalized lipodystrophy (CGL) is a rare and progressive disorder (1 in 12 milion). Common clinical manifestations are lipoatrophy, acromegaloid feature, acanthosis nigricans, Hyperlipidemia, diabetes mellitus or abnormal glucose tolerance test. Rarely fatty liver and cirrhosis is seen. The highest number reported in literature is 8 cases from Brazil and 6 cases from Scandinavia, respectively. The oldest case was a 19-year-old girl. Our case is a 13-year-old male with all above manifestations, also he had liver cirrhosis. Assessment of Immunization Rate of Hepatitis B Vaccination among Health Care Personnel in TehranPage 266BackgroundHepatitis B is one of the most important etiologic factors of acute and chronic hepatitis, cirrhosis of the liver and hepatocellular carcinoma. The best preventive method is vaccination. The Aim of the study was to evaluate the immunization rate against hepatitis B in medical health personnel.Materials And MethodsThis study was conducted on 331 health care workers employed in public, semiprivate, and private hospitals that had received the complete dosage of vaccination with an average duration of 0-3 years since the last dose, and had not previously received any booster or HBV - Immunoglobulin. The Shariati (public), Imam Khomeini (public), Baqiyatallah (semi-private) and Day (private) hospitals were the sites of our research. Regarding the age and occupational status, persons were randomly selected and data relating to demographic aspects and serum anti-HBS titer were recorded in all individuals and then were evaluated.ResultsResults show that out of the 331 cases, 47 (14.2%) were non-responder (Anti-HBs titer ‹10 IU/L), 160 (54.4%) had low  response (10|£|Anti-HBs titer |£|100 IU/L), and 104 persons (31.4%) were found to have good response (Anti-HBs titer ›100 IU/liter). There was no significant relationship between the antibody titer and other variables including age, gender, smoking, body mass index (BMI) and duration since last dose of vaccine.ConclusionsWe suggest the assessment of Anti-HBs titer after complete vaccination in high-risk groups to find non- and low-responders in order to revaccinate them.Keywords: Hepatitis B, vaccination, Anti, HBs titer

Esophagitis Caused by Ciprofloxacin; A Case Report and Review of the LiteraturePage 272Ingestion of ciprofloxacin can be associated with esophagitis and esophageal ulcer. We report a 46-yearold man with odynophagia and dysphagia following ciprofloxacin ingestion, with a glance on diagnosis and treatment.
