صفحه 1مقدمهتاکنون فاکتورهای پیش آگهی دهنده ی متعددی برای سرطان پستان شناسایی شده است که گیرنده های هورمون های استروییدی و فاکتور رشد اپیدرمال انسانی 2 (Human epidermal growth factor receptor 2 یا HER2) و پروتئین P53 از جمله ی آن ها هستند. در این مطالعه، فراوانی بروز این فاکتورها و ارتباط آن ها با سن و مرحله ی بروز بیماری در زنان مبتلا در شهر اصفهان بررسی شد.روش هااین مطالعه به صورت توصیفی در مقطع زمانی بین سال های 1378 تا 1388 بر روی خانم های مبتلا به سرطان پستان با روش نمونه گیری سرشماری انجام گردید. اطلاعات بیماران از پرونده های آنان استخراج و در نرم افزار SPSS نسخه ی 16وارد شد. سپس این اطلاعات با کمک آمارهای توصیفی مانند فراوانی، میانگین، 2χ و آزمون های غیر پارامتری تجزیه و تحلیل گردید.یافته هادر این مطالعه 685 بیمار بررسی شدند که میانگین سنی آن ها 07/11 ± 39/47 سال بود. از میان بیماران بررسی شده در زمان مراجعه، 22 نفر در مرحله ی 1، 372 نفر در مرحله ی 2، 60 نفر در مرحله ی 3 و 38 نفر در مرحله ی 4 بیماری قرار داشتند. فراوانی گیرنده ی استروژن 4/39 درصد، گیرنده ی پروژسترون 9/36 درصد، پروتئین P53 9/49 درصد و HER2 3/23 درصد بود. بین مثبت یا منفی بودن گیرنده ی استروژن در مرحله ی 4 بیماری، گیرنده ی پروژسترون در مرحله ی 2 و 4 بیماری، P53 در تمام مراحل بیماری و HER2 در مرحله ی 2 بیماری، تفاوت معنی داری وجود داشت. فراوانی فاکتورها بین مراحل مختلف بیماری اختلاف معنی داری نداشت. به علاوه گیرنده های استروژن و پروژسترون بر خلاف HER2 و P53، با سن بیماران ارتباط داشت.نتیجه گیریبه نظر می رسد که بررسی اختصاصی وضعیت این فاکتورها در زمان تشخیص بیماری برای گرفتن تصمیم درمانی مناسب و پی گیری های بعدی در طول بیماری لازم است، اما در نظر گرفتن تفاوت های جمعیتی در این موضوع نیز ضروری می باشد.

یکی از مهم ترین مشکلات در مبتلایان به دیابت میکروآلبومینوری است. در واقع میکروآلبومینوری ساده ترین و حساس ترین فاکتور برای ارزیابی خطر و مشخص کردن بیماری کلیوی در مبتلایان به دیابت است و اگر در مراحل اولیه تشخیص داده شود، می توان از بروز و پیشرفت نفروپاتی دیابتی جلوگیری به عمل آورد. هلیکوباکتر پیلوری یکی از عوامل عفونت زا در کل جامعه و به خصوص در بیماران مبتلا به دیابت، به دلیل سطح ایمنی پایین تر، می باشد. بر مبنای بعضی از گزارش ها ژنوتیپ cagA مثبت از هلیکوباکتر پیلوری به عنوان عامل موثر در ایجاد میکروآلبومینوری قلمداد شده است. هدف مطالعه ی حاضر، بررسی ارتباط ژن ویرولانس cagA هلیکوباکتر پیلوری با میکروآلبومینوری در بیماران مبتلا به دیابت نوع دو بود.
روش هادر این مطالعه 88 بیمار مبتلا به دیابت نوع دو مراجعه کننده به مرکز تحقیقات غدد و متابولیسم اصفهان بررسی شدند که 60 نفر از آن ها بیماران دچار دیابت نوع دو مبتلا به میکروآلبومینوری و 28 نفر بیماران مبتلا به دیابت نوع دو بدون میکروآلبومینوری بودند. نمونه های مدفوع از بیماران و افراد شاهد جمع آوری گردید و پس از استخراج DNA برای بررسی نمونه ها از لحاظ عفونت با هلیکوباکتر پیلوری Nested PCR و سپس ژنوتایپینگ با پرایمرهای اختصاصی cagA انجام شد.
یافته هااز کل نمونه ها 16 نفر(2/18 درصد) آلوده به هلیکوباکتر پیلوری بودند که 12 نفر (75 درصد) از گروه بیماران دچار دیابت نوع دو مبتلا به میکروآلبومینوری و 4 نفر (25 درصد) از گروه بیماران مبتلا به دیابت نوع دو بدون میکروآلبومینوری بودند. 3 نمونه (8/18 درصد) دارای ژن cagA بودند که هر سه نفر از گروه افراد دچار دیابت نوع دو مبتلا به میکروآلبومینوری بودند. ارتباط بین ژن cagA هلیکوباکتر پیلوری و میکروآلبومینوری در بیماران مبتلا به دیابت نوع دو از نظر آماری معنی دار نبود (08/0 = P).
نتیجه گیریدر این مطالعه اگر چه شیوع میکروآلبومینوری در افراد با ژنوتیپ cagA+ هلیکوباکترپیلوری بیش از افرادی بود که از لحاظ ژنوتیپ هلیکوباکتر پیلوری -cagA بودند، اما در بیماران مبتلا به دیابت نوع دو ارتباط معنی داری بین ژن cagA هلیکوباکتر پیلوری و میکروآلبومینوری نبود.

صفحه 3مقدمهآنزیم Taq پلیمراز به صورت گسترده در واکنش زنجیره ای پلیمراز (Polymerase chain reaction یا PCR) کاربرد دارد. یکی از قسمت های مهم دخیل در فعالیت این آنزیم، منطقه ی O-helix آنزیم می باشد. طی تحقیقات قبلی یک وکتور بیانی شامل موتاسیون Asn-666-Glu بر روی ژن آنزیم طراحی شده است. به منظور بررسی تاثیر این موتاسیون بر روی فعالیت آنزیم در ابتدا باید به خالص سازی آنزیم Taq پلیمراز پرداخت. هدف از این پروژه، جداسازی و خالص سازی آنزیم Taq پلیمراز نوترکیب از گونه ی باکتری اشرشیاکلی Bl21 بود.روش هادر این مطالعه پس از ترانسفورم کردن سلول های پذیرا با پلاسمید حاوی ژن Taq پلیمراز موتانت شده در ناحیه ی N666E و القای بیان آنزیم با ایزوپروپیل بتا تیوگالاکتوزید (Isopropyl β-D thiogalactopyranoside یا IPTG) از روش های خالص سازی Desai، پروتکل Desai بهینه سازی شده، رزین- نیکل (Ni-NTA His.Bind Resins)، تری کلرو استیک اسید (Trichloroacetic acid یا TCA) و رفولدینگ (Refolding) به منظور تخلیص آنزیم Taq پلیمراز استفاده گردید و در نهایت به مقایسه ی این روش ها با یکدیگر پرداخته شد.یافته هادر مقایسه ی نتایج با یکدیگر، استفاده از پروتکل Desai علاوه بر این که باندی شارپ در ناحیه ی مورد انتظار (94 کیلودالتون) ایجاد نمود، در سایر نواحی نیز باندهایی دیده شد؛ اما پس از بهینه سازی پروتکل Desai تنها باند مورد نظر قابل مشاهده بود. با استفاده از تری کلرو استیک اسید و رزین- نیکل باندهای دیده شده در ناحیه ی 94 کیلودالتون ضعیف بود و گاهی باندهای دیگری در سایر نواحی دیده شد. با رفولدینگ آنزیم، باند ایجاد شده در ناحیه ی 66 کیلودالتون مشاهده شد.نتیجه گیریروش خالص سازی Desai که با استفاده از RNase و DNase بهینه سازی شده انجام شد، باعث ایجاد باندی واضح و مناسب در ناحیه ی 94 کیلودالتون گردید. استفاده از تری کلرو استیک اسید به منظور رسوب پروتئین هایی که با حرارت از بین نرفته اند و استفاده از رزین- نیکل باعث حذف تقریبی باندهای ناخواسته شد، هر چند مقدار آنزیم مورد نظر را نیز در ناحیه ی 94 کیلودالتون کاهش داد. در مورد باند 66 کیلودالتون ایجاد شده در فرایند رفولدینگ آنزیم، این احتمال وجود دارد که در حین جداسازی اینکلوژن بادی ها از سلول، باقی ماندن آنزیم پروتئاز باعث شکست آنزیم در این ناحیه شده باشد.

در مطالعات قبلی میزان بهبودی کامل لوکمی لنفوبلاستیک حاد (Acute lymphocytic leukemia یا ALL) بالغین توسط رژیم Hyper-CVAD حدود 90 درصد عنوان شده است. مطالعه ی حاضر امکان سنجی تجویز رژیم Hyper-CVAD و تعیین بهبودی کامل (CR یا Complete remission) را در بیماران مبتلا به ALL در بیمارستان سید الشهدای (ع) اصفهان بررسی کرده است.
روش ها11 بیمار جدید تشخیص داده شده ی ALL، 8 دوره رژیم Hyper-CVAD را به طور متناوب همراه با حمایت فاکتور محرک گرانولوسیتی (Granulocyte colony-stimulating factor یا GCSF) و کموپروفیلاکسی داخل نخاعی دریافت کردند. سپس بیماران وارد درمان نگهدارنده ی شیمی درمانی شدند. میزان CR 2 و 4 هفته پس از اولین دوره ی شیمی درمانی بررسی شد.
یافته هامیانگین سنی بیماران 25 سال بود. در این مطالعه 9 بیمار (82 درصد) به فاز CR رفتند و 2 بیمار (22 درصد) عود داشتند. بقای کلی (Overall survival یا OS) شش و دوازده ماهه به ترتیب 91 و 80 درصد و بقای عاری از بیماری (Disease free survival یا DFS) شش و دوازده ماهه به ترتیب 91 و 60 درصد بود. با وجود تب و نوتروپنی در 88 درصد بیماران در فاز Induction هیچ مرگی دیده نشد.
نتیجه گیریالقا و تحکیم درمان با رژیم Hyper-CVAD در بیماران ALL امکان پذیر است و مرگ و میر پایین دارد.

اضافه وزن و چاقی به عنوان یک مشکل مهم در دنیا مطرح است که مرتبط با دیابت، سندرم متابولیک و بیماری های قلبی-عروقی می باشد. رژیم های غذایی پرپروتئین به عنوان یکی از راه های کاهش وزن، در بهبود عوامل خطر بیماری های قلبی-عروقی مورد توجه است. هدف مطالعه ی حاضر، مروری بر شواهد موجود در ارتباط با اثر رژیم های غذایی پرپروتئین بر عوامل خطر بیماری های قلبی-عروقی بود.
روش هاپس از جستجو در موتور PubMed و استفاده از واژگان کلیدی weight loss، lipid profile، triglyceride، high or low density lipoprotein، total cholesterol، cardiovascular risk،glucose level و high protein diet و محدود کردن آن ها به عنوان و نیز جستجو بین سال های 2002 تا اکتبر 2011، در کل 25 مقاله با طراحی کارآزمایی بالینی از نوع موازی و متقاطع مورد بررسی قرار گرفتند.
یافته هادریافت رژیم غذایی پرپروتئین (22 تا 35 درصد) منجر به کاهش وزن، کاهش سطح گلوکز خون و بهبود سطح لیپید های خون از جمله HDL (High density lipoprotein)، LDL (Low density lipoprotein)، کلسترول تام و تری گلیسیرید می گردد. به نظر می رسد، رژیم غذایی پرپروتئین نسبت به رژیم غذایی با پروتئین با مقدار استاندارد سبب کاهش وزن بیشتری گردد. احتمال دارد سطح تری گلیسیرید و کلسترول نیز در رژیم های غذایی پرپروتئین که از منابع حیوانی کم چرب و منابع گیاهی استفاده شده است، بهبود بیشتری یابد.
نتیجه گیریرژیم های غذایی پرپروتئین در بهبود عوامل خطر بیماری های قلبی-عروقی از جمله کاهش نمایه ی توده ی بدنی، بهبود سطح لیپید های سرم و گلوکز خون موثر است.

Page 1BackgroundBreast cancer is the most common cancer and the second cause of mortality in women. It is also the main cause of death among 45-55 year-old women. Several predictive and prognostic factors have been detected until now. Some of these factors, such as steroid hormone receptors and human epidermal growth factor receptor 2 (HER2) and P53, have been proved. The aim of this study was to investigate these factors in women with breast cancer in Isfahan, Iran.MethodsDuring 1999-2009, this descriptive study was conducted on patients with pathologically confirmed breast cancer who referred to Seyed-Al-Shohada Cancer Clinic and doctor's offices in Isfahan. Data was extracted from patient files. The collected data was then entered into SPSS16 and analyzed using descriptive statistics like frequencies and mean values, and chi-square and non-parametric tests.FindingsIn this study, 685 patients with breast cancer were investigated during 10 years. The mean age of patients was 47.39 ± 11.07 years. While 22 patients were in stage 1, 327, 60, and 38 subjects were in stages 2-4, respectively. In this study, the frequency of estrogen receptor, progesterone receptors, P53 protein, and HER2 was 39.4%, 36.9%, 49.9%, and 23.3%, respectively. Significant differences were detected between positive and negative estrogen receptor in stage 4, progesterone receptor in stages 2 and 4, P53 in all stages, and HER2 in stage 2. Stages of cancer were not significantly different in terms of frequency of these factors. A relation was found between estrogen and progesterone receptors and patient's age. However, such a relation could not be established in case of HER2 and P53.ConclusionAssessments of hormone receptors and HER2 and P53 after the diagnosis of breast cancer seem necessary in identifying the best treatment choice. Diabetes mellitus is the most prevalent metabolic disorder worldwide with many subsequent medical complications. An important complication of diabetes is microalbuminuria which is an important indication for development of diabetic nephropathy. The prevalence of diabetic nephropathy in patients with type 2 diabetes has been estimated to be 47%. Since the disorder initiates with microalbuminuria, early detection and prevention are of vital importance. Helicobacter pylori infection is more prevalent in diabetic patients due to their low immune tolerance. The cagA positive genotype is reported to be effective in microalbuminuria development in type 2 diabetic patients. In this study, we investigated the relation of H. pylori cagA virulence factor with microalbuminuria in patients with type 2 diabetes.
MethodsThis study included 88 patients with type 2 diabetes. Microalbuminuria was present in 60 subjects and absent in the other 28. Stool samples were collected from all patients and DNA was extracted using QIAamp DNA Stool Mini Kit. In order to detect H. pylori infection, a nested polymerase chain reaction (PCR) protocol was developed and all H. pylori positive samples were genotyped for cagA positivity.
FindingsOverall, 16 patients (18.2%) were detected to be H. pylori positive out of whom 12 had microalbuminuria (75%) and 4 (25%) did not. CagA positivity was detected in 3 subjects who belonged to the group with microalbuminuria.
ConclusionIn the population studied and the sample size used, no correlation was found between cagA gene and microalbuminuria in patients with type 2 diabetes.

Page 3BackgroundThe aim of this project was to isolate and purify the highly active recombinant Taq DNA polymerase from the strain of Escherichia coli BL21. This enzyme, with a molecular weight of about 94 kDa, is widely used in polymerase chain reaction (PCR). In PCR, the activity and fidelity of Taq polymerase can significantly influence the results. One of the active regions of Taq polymerase has been suggested to be the O-helix region. In previous studies, an expression vector containing mutated Asn 666 Glu Taq polymerase gene was designed. In order to investigate the effects of this mutation on the function of the enzyme, Taq polymerase needs to be purified first.MethodsIn this study, after transformation of competent cells, enzyme expression was induced by isopropyl β D thiogalactopyranoside (IPTG) method. Modified Desai protocol with DNase and RNase, nickel-nitrilotriacetic acid (Ni-NTA) resin, trichloroacetic acid (TCA), and refolding protocols were subsequently used for purification of this enzyme. These protocols were finally compared.FindingsUsing Desai protocol resulted in the production of a sharp band in the expected region (94 kDa) and several other visible bands. After further modification of Desai protocol, only the desirable band was observed. In protocols using TCA and Ni-NTA resin, the expected bands were weak. Refolding protocol caused a band in an undesirable region (66 kDa).ConclusionFrom the different purification techniques that were used in this study, the modified method of Desai containing RNase and DNase worked best. Addition of TCA can precipitate proteins that had not been affected by heat. Using Ni-NTA resin resulted in elimination of unwanted bands. However, the amount of Taq polymerase was also decreased. The extra band that was observed in refolding protocol was probably due to the presence of proteases that were isolated with inclusion body and could digest Taq polymerase.Keywords: Taq polymerase recombinant, Purification, Desai protocolOhelix mutation

Hyper cyclophosphamide, vincristine, Adriamycin, and dexamethasone (CVAD) regimen is considered as an effective regimen in treatment of adult acute lymphoblastic leukemia (ALL), with complete remission (CR) rate of 90% in previous studies. The aim of this research was to evaluate the feasibility of this regimen in ALL patients in Seyed-Al-Shohada Hospital, Isfahan, Iran.
MethodsEleven adults with newly diagnosed ALL received therapy with hyper-CVAD regimen consisting of 8 courses of alternative intensive chemotherapy with growth factor support and intrathecal chemotherapy prophylaxis followed by oral maintenance chemotherapy. CR was evaluated 2 and 4 weeks after the first courses of chemotherapy.
FindingsThe mean age of patients was 25 years. After the first course of treatment, 9 patients (82%) achieved CR while 2 patients (18%) relapsed. Overall survival (OS) at 6 and 12 months was 91% and 81%, respectively. Disease free survival (DFS) at 6 and 12 months was 91% and 60% respectively. Febrile neutropenia was seen in 88% of patients in Induction. No death was seen in Induction.
ConclusionInduction/Consolidation with hyper-CVAD in ALL patients was feasible in our institution and had low mortality.

Overweight and obesity are worldwide problems which are also associated with diabetes, metabolic syndrome, and cardiovascular disease risk factors. High protein diets are one of the approaches in weight loss that emphasize on the improvement of the cardiovascular risks. The aim of the present study was reviewing the evidences regarding the effects of high protein diet on cardiovascular disease risk factors.
MethodsWe searched in PubMed search engine from 2002 to October 2011, by limiting the title using the following key words: weight loss, lipid profile, triglyceride, high or low density lipoprotein, total cholesterol, cardiovascular risk, glucose level, and high protein diet. Totally, 25 clinical trials (parallel or crossover) were recruited in this review.
FindingsConsumption of high protein diet (22-35 percent) affects weight loss, reducing serum glucose level, and improving lipid profile [low density lipoprotein (LDL), high density lipoprotein (HDL), triglyceride (TG), and total cholesterol]. High protein diet may have useful effects on weight loss more than diet with the normal amount of protein. Serum triglyceride and total cholesterol levels may improve more in high protein diet with low fat animal protein sources and vegetable proteins.
ConclusionHigh protein diet positively improve cardiovascular disease risk factors such as body mass index (BMI), lipid profile, and serum glucose level.
