صفحه 1مقدمهدوکسوروبیسین در درمان انواع سرطان ها مصرف می گردد. اما سمیت قلبی، مصرف آن را محدود ساخته است. برخی مطالعات نشان داده اند که کورتیکواستروییدها باعث محافظت قلبی می گردند. در این مطالعه با استفاده از کاردیومیوسیت های مشتق از سلول های بنیادی جنینی ترانس ژنیک موش اثرات دگزامتازون در کاهش توکسیسیته ی ناشی از دوکسوروبیسین بررسی شد.روش هاسلول های قلبی خالص شده ی مشتق از سلول های بنیادی جنینی ترانس ژنیک موش (RB1-αMHC) با غلظت های مختلف دوکسوروبیسین به مدت 24 ساعت تیمار شدند، تا غلظتی که باعث کاهش 50 درصد حیات سلول های قلبی می شود برای استفاده در مراحل بعدی به دست بیاید. سلول ها در 3 گروه قرار گرفتند: گروه شاهد، گروه دوکسوروبیسین 5 میکرومولار به مدت 24 ساعت و گروه دگزامتازون 10 میکرومولار به مدت 24 ساعت قبل از دوکسوروبیسین 5 میکرومولار به مدت 24 ساعت. گروه ها از نظر ضربان با میکروسکوپ فاز کنتراست و درصد حیات با MTS Assay و از نظر بیان ژن با استفاده از تکنیک RT-PCR (Real time- polymerase chain reaction) ارزیابی شدند.یافته هاغلظت 5 میکرومولار دوکسوروبیسین به عنوان حداقل غلظتی که باعث مرگ 50 درصد از این سلول ها می گردد، به دست آمد. در گروه اول تیمارشده با دوکسوروبیسین و غلظت 10 میکرومولار دگزامتازون اثر محافظت کننده بر حیات سلول های و بیان ژن های قلبی دیده شد. در گروه دوم اثر محافظت کنندگی قابل توجهی مشاهده نشد.نتیجه گیریتیمار سلول های قلبی حاصل از سلول های بنیادی جنینی ترانس ژنیک موش با دگزامتازون 24 ساعت قبل از تیمار با دوکسوروبیسین باعث پیش گیری از مرگ سلول های قلبی ناشی از دوکسوروبیسین می گردد. نتایج این مطالعه مطابق نتایج مطالعه بر روی سلول های قلبی موش بود. بنابراین این سلول ها می توانند به عنوان مدل آزمایشگاهی مناسبی برای بررسی آثار درمانی و یا سمی داروها مورد استفاده قرار گیرند.

صفحه 9مقدمهشکستگی هیپ یکی از جدی ترین مسایل و دومین علت بستری در جمعیت مسن می باشد. این مطالعه با هدف ارزیابی شکستگی لگن در مبتلایان به دیابت و عوارض آن نسبت به افراد غیر مبتلا به این بیماری انجام شد.روش هااین مطالعه ی مقطعی روی تمامی بیمارانی که از سال 1387 تا سال 1391 با شکستگی هیپ در بیمارستان شهید بهشتی و یحیی نژاد بابل بستری شدند، انجام گرفت. چک لیست در بر دارنده ی اطلاعات دموگرافیک، محل شکستگی، نوع درمان، طول مدت بستری، عوارض حین و بعد از عمل جراحی برای همه ی بیماران تکمیل شد. اطلاعات به وسیله ی نرم افزار SPSS نسخه ی 20 و آزمون های آماری Student-t و 2χ تجزیه و تحلیل شد.یافته هااز 195 بیمار، 116 (5/59 درصد) مرد و 79 نفر (5/40 درصد) زن بودند. 155 نفر (4/79 درصد) غیر مبتلا به دیابت با میانگین سنی 53/17 ± 25/63 سال و 40 بیمار (5/20 درصد) مبتلا به دیابت با میانگین سنی 12/25 ± 67/67 سال بودند. در بین مبتلایان به دیابت 1 نفر (5/2 درصد) به دیابت نوع یک و 39 نفر (5/97 درصد) مبتلا به دیابت نوع دو مبتلا بودند. محل شکستگی در بیماران غیر مبتلا، 8/56 درصد گردن ران، 9/21 درصد اینترتروکانتریک، 1/18 درصد سر ران و در مبتلایان به دیابتی 50 درصد گردن ران، 35 درصد اینترتروکانتریک و 15 درصد سر ران بود. حجم خون تزریقی بعد از عمل در افراد مبتلا و غیر مبتلا به ترتیب 49/1 ± 30/3 و 52/1 ± 43/2 سی سی به صورت Packed Cell بود که به طور معنی داری در مبتلایان به دیابت بیشتر بود (029/0 = P).نتیجه گیریبا وجود شیوع بالای شکستگی هیپ و تاثیر دیابت در بروز شکستگی هیپ شاید بتوان با افزایش آگاهی و مراقبت از مبتلایان به دیابت به خصوص سالمندان از ایجاد این صدمات کاست.

صفحه 15مقدمهکاربرد ماسک باینری ایده آل برای پردازش سیگنال گفتاری، بهبود قابل ملاحظه ای در وضوح گفتار هم در افراد با شنوایی طبیعی و هم افراد مبتلا به کم شنوایی نشان داده است. این ماسک به بخش های زمان- فرکانس سیگنال نویزی اعمال می گردد و بخش هایی از سیگنال پایین تر از سطح آستانه ی SNR (Signal-to-noise ratio) حذف می گردد و سایر بخش ها را عبور می دهد.روش هادر این مطالعه عوامل موثر بر روی الگوریتم ماسک باینری ایده آل مورد مطالعه و بررسی قرار گرفتند. تاثیر سطح آستانه SNR، سطح SNR ورودی، نوع ماسک کننده و تخمینگر نویز، بررسی و ارزیابی شد. تخمینگرهای جدیدی شامل وزنی Euclidean و COSH معرفی شدند. این تخمینگرها مبتنی بر درک سیستم شنوایی و ماسک شنیداری می باشند.یافته هاعملکرد بالای ماسک باینری در ناحیه ی 15-5 دسی بل مشاهده شد. یافته ها می تواند برای پیشرفت طراحی سمعک و پروتز کاشت حلزون مفید باشد.نتیجه گیرییافته های ما موید مطالعات پیشین در زمینه ی وضوح گفتار قابل توجه است؛ حتی زمانی که SNR، 10- دسی بل باشد. ارزیابی عملکرد این الگوریتم نشان داد که تخمینگرهای جدید در مقایسه با تخمینگر Wiener می توانند حذف نویز بهتری داشته باشند.

صفحه 25مقدمهندول های تیروییدی از شایع ترین بیماری های تیرویید هستند که در نیمی از جمعیت بالغین یافت می شوند. روش های تشخیصی متعددی برای ارزیابی بیماران به کار می رود. اسکن رادیونوکلیید غده ی تیرویید توسط 99mTcO4 فعالیت بافت ندول را مشخص می کند. معاینه ی فیزیکی گردن و علامت گذاری ندول با مارکر سربی پیش از انجام سنتی گرافی می تواند با تعیین مکان ندول در هنگام تفسیر سنتی گرام از اشتباهات در گزارش فعالیت ندول ها بکاهد.روش ها117 بیمار مبتلا به ندول تیروییدی که جهت اسکن رادیونوکلیید از غده ی تیرویید به مرکز پزشکی هسته ای سیدالشهدا (ع) اصفهان ارجاع شده بودند، در فاصله ی زمانی مهر ماه 1392-1391 مورد مطالعه قرار گرفتند. پس از اخذ شرح حال عدم مصرف داروها و مواد مداخله گر با اسکن، تکنسیوم رادیواکتیو به بیمار تزریق شد و بیمار مورد اسکن قرار گرفت. سپس غده ی تیرویید بیمار معاینه شد و به طور مجدد اسکن انجام شد. در مرحله ی سوم بعد از معاینه ی تیرویید بیمار، ندول لمس شده توسط مارکر سربی علامت گذاری شد و سپس اسکن تکرار شد. در آخر نتایج حاصل از تفسیر هر سه تصویربرداری مقایسه گردید.یافته هاطبق آزمون 2χ معاینه ی فیزیکی گردن و علامت گذاری ندول تیروییدی لمس شده با مارکر سربی، بر تفسیر نتیجه ی اسکن تیرویید موثر بود و باعث کاهش نتایج مثبت کاذب در ندول های گرم و سرد شد (05/0 > P). وطبق آزمون Wald استفاده از روش معاینه ی فیزیکی و نیز علامت گذاری ندول تیروییدی با مارکر سربی پس از معاینه، باعث کاهش خطای تشخیص در تفسیر نتیجه ی اسکن تیرویید می شود.نتیجه گیریاین مطالعه نشان داد که با افزودن معاینه ی فیزیکی که روشی ساده و مقرون به صرفه است، با اسکن رادیونوکلیید، می توان در راستای تشخیص ندول های تیروییدی سود جست.

بررسی اثر کلسترول، ترکیبات محتوی آهن و ویتامین E، بر فراگمنتاسیون DNA و کمبود پروتامین اسپرم در خرگوشصفحه 34مقدمهنقش ویتامین E به عنوان یکی از آنتی اکسیدان ها و اثر ترکیبات محتوی آهن بر روی DNA به خوبی شناخته شده نیست. از این رو، مطالعه ی حاضر جهت تعیین نقش ویتامین E و آهن، به عنوان دو عامل موثر به صورت جداگانه و ترکیبی بر فراگمنتاسیون DNA و کمبود پروتامین اسپرم در خرگوش انجام گرفت.روش هاتعداد 42 عدد خرگوش نر بالغ به صورت تصادفی به هفت گروه 6 تایی تقسیم شدند و به مدت 6 هفته تحت رژیم غذایی مخصوص قرار گرفتند. برای گروه شاهد از غذای معمولی، گروه دوم از غذای معمولی حاوی ویتامین E (mg/kg 50)، گروه سوم از غذای معمولی حاوی آهن (mg/kg 50)، گروه چهارم از غذای معمولی حاوی آهن (mg/kg 50) و ویتامین E (mg/kg 50)، گروه پنجم از غذای حاوی 1 درصد کلسترول، گروه ششم از غذای حاوی 1 درصد کلسترول و ویتامین E (mg/kg 50) و گروه هفتم از غذای حاوی 1 درصد کلسترول و آهن (mg/kg 50) استفاده شد. پس از طی شدن زمان لازم، خرگوش ها بی هوش و قربانی شدند و مجاری اپیدیدیم و دفران آن ها تشریح شد و اسپرم های این مجاری به کمک محلول 10Hams f خارج گردید. میزان آسیب DNA اسپرم توسط رنگ آمیزی آکریدین اورنج و کمبود پروتامین اسپرم با رنگ آمیزی 3CMA (3Chromomycin A) مورد ارزیابی قرار گرفت.یافته هامصرف همزمان غذاهای محتوی کلسترول و آهن، آسیب معنی داری را در DNA اسپرم ایجاد نمود. به علاوه، مصرف ویتامین E به عنوان یک عامل آنتی اکسیدان، اثر قابل توجهی در کاهش این آسیب دارد. بررسی وضعیت کروماتین اسپرم نشان داد که مصرف ترکیبات محتوی آهن و کلسترول به تنهایی و یا به شکل همزمان، باعث افزایش قابل توجهی در میانگین درصد کمبود پروتامین اسپرم می شود. استفاده از ویتامین E، میانگین درصد کمبود پروتامین اسپرم را به صورت متفاوت در گروه ها تغییر دادنتیجه گیریمصرف ویتامین E به عنوان نوعی آنتی اکسیدان، آسیب DNA اسپرم ناشی از مصرف غذاهای پر کلسترول و آهن را کاهش می دهد.

Page 1BackgroundDoxorubicin (DOX) is a powerful chemotherapeutic agent used in the treatment of solid tumors and malignant hematological diseases. However, cardiac toxicity limits the clinical usefulness of this drug. Previous reports have shown that corticosteroids induce a cytoprotective effect on cardiomyocytes. Mouse transgenic embryonic stem cell-derived pure cardiomyocytes may be considered as a model for assessment pharmacological and toxicological effects of drugs in vitro.MethodsMouse transgenic embryonic stem cell-derived pure cardiomyocytes were treated by different concentrations of doxorubicin to determine median lethal dose (LD50). Pure cardiomyocytes were evaluated in two groups: treatment by 10 µM dexamethasone (DEX) 24 hours before or before and in continuation with doxorubicin. The percentage of cardiomyocyte viability by MTS assay, the percentage of beating, and quantitative real-time polymerase chain reaction (RT-PCR) for cardiac gene expression (β-MHC) were evaluated in each group.Findings5 µM doxorubicin was determined as drug concentration that leads to 50% cardiomyocyte mortality. Cardiotoxicity on mouse transgenic embryonic stem cell-derived pure cardiomyocytes could be ameliorated by treatment with dexamethasone (DEX) when administrated before doxorubicin. The effect of dexamethasone appeared to be mediated via glucocorticoid receptors. Dexamethasone increased cardiomyocyte gene expression and decreased apoptosis.ConclusionTransgenic embryonic stem cell-derived cardiomyocytes are a model for evaluation of doxorubicin toxicity. Additionally, this model provides us with a clinical suggestion, which proposes that the beneficial effect of dexamethasone is obtained when added only before doxorubicin. Page 9BackgroundHip fracture is one of the most serious problems and the second cause of hospitalization in elderly people. The aim of this study was evaluation of hip fracture in patients with and without diabetes mellitus and its complications.MethodsIn this cross-sectional study, all patients with hip fracture referred to Yahyanejad and Shahid Beheshti hospitals (Babol, Iran) were enrolled. A checklist including demographic data, site of fracture, type of treatment, duration of hospitalization, complications during and after surgery was filled for each patient. Data were analyzed using t and chi-square tests.FindingsFrom 195 patients, 116 (59.5%) were men and 79 (40.5%) were women. 155 patients (79.4%) were non-diabetic with a mean age of 53.17 ± 25.63 years and 40 (20.5%) were diabetics with a mean age of 67.67 ± 12.25 years. 1 patient (2.5%) had type I and 39 patients (97.5%) had type II diabetes mellitus. In patients without diabetes mellitus, the fracture location was neck of femur in 56.8%, intertrochanter in 21.9%, and head of femur in 18.1%. In patients with diabetes mellitus, 50% was in neck of femur, 35% in intertrochanter and 15% in head of femur. Blood transfusion volume was significantly higher in patients with diabetes mellitus (3.30 ± 1.30 units of packed cell) compared to those without diabetes mellitus (2.43 ± 1.52 units of packed cell) (P = 0.029).ConclusionAccording to the high incidence of hip fractures, awareness and care of the patients with diabetes mellitus, especially the elderly, would reduce the incidence of complications.Keywords: Hip fracture, Diabetes mellitus, Complications

Page 15BackgroundThe application of the ideal binary mask (IBM) for speech signal processing provides remarkable intelligibility improvements in both normal-hearing and hearing-impaired listeners. Binary mask widely applies to the time-frequency (T–F) representation of a noisy signal and eliminates units of a signal below a signal-to-noise-ratio (SNR) threshold while retains others.MethodsThe factors underlying intelligibility of ideal binary-masked speech were examined and evaluated in the present study. The effects of the local SNR threshold, input SNR level, masker type, and ideal mask-estimator were examined. New estimators including weighted Euclidean and COSH were proposed in which, the human perceptual auditory masking effect and perceptual perception were incorporated.FindingsHigh-performance plateau for SNR thresholds ranging from −20 to 5 dB was observed. Findings could be used for hearing-aid and cochlear-implant designs.ConclusionIntelligibility of speech was high even at −10 dB SNR for all maskers tested. Performance assessment shows that our proposed estimators can achieve more significant noise estimation as compared to the Wiener estimator.Keywords: Speech enhancement, Binary masking, Speech intelligibility

Page 25BackgroundThyroid nodules are of the most common thyroid diseases found in about half of adults. Diagnosis of nodules is clinically important because of the risk of cancer. Thyroid gland radionuclide scanning with 99mTcO4 helps the physician to choose better approach to the disease, knowing the activity of the nodule. Neck physical examination before scintigraphy and using a piece of lead marker specifies the location of the nodule in the scintigram to give a more distinct interpretation of the report of the scan.Methods117 patients with thyroid nodules referred to Seyyed Alshohada Hospital Nuclear Medicine Center (Isfahan, Iran) during 2012-2013 were studied. The patients underwent scanning with 99mTcO4 once; the next time, physical examination of the neck was done and then, scanning was applied; and for the last time, we put a lead marker on the palpated nodule in physical examination, then did the radionuclide scanning. The reports of three scanning tests were compared together.FindingsRegarding chi-square test, physical examination of neck and using a lead marker on the palpated thyroid nodule affected the reporting of the scintigram and caused reduction of the false-positive results in both cold and warm nodules (P < 0.05). Also regarding to Wald test, using both mentioned methods resulted in less mistakes in interpretations of the reports of the thyroid scanning.ConclusionThis study showed that, as a simple cost effective method, physical examination of thyroid gland and using lead marker through scintigraphy may be useful in diagnosis of thyroid nodules.Keywords: Nodules, Thyroid scan, Lead marker

Effect of Cholesterol, Iron and Vitamin E on Protamine Deficiency and DNA Fragmentation of Male Rabbit SpermPage 34BackgroundThe role of vitamin E as an antioxidant and iron-containing compounds on DNA is not well defined. So, recent study was carried out to determine the role of vitamin E and iron, as both separate and combined effect on sperm protamine deficiency and DNA fragmentation in male rabbits.MethodsThis study included 42 male rabbits which were randomly divided into seven groups each containing six rabbits.They were all treated under special diets for 6 weeks. After this period the animals were anesthetized and sacrificed. The epididymis duct and vas deferens were dissected and their sperms were extracted from it by Ham's f10 solution. Sperm DNA fragmentation and protamine deficiency were evaluated by the acridine orange and chromomycin A3 staining, respectively.FindingsThe mean values of protamine deficiency and DNA fragmentation were significantly higher in group that concomitant use of cholesterol and iron. In addition, Vitamin E as an antioxidant agent has a significant effect in reducing these damages.ConclusionVitamin E as an antioxidant, decrease sperm DNA damage resulting from the consumption of foods high in cholesterol and iron.Keywords: Cholesterol_Iron_Vitamin E DNA fragmentation_Protamine deficiency_Sperm
