صفحه 2257مقدمهبا پیشروی بیماری پارکینسون، عوارض ناخواسته ای همچون نقص تعادل، کیفیت زندگی فرد را تحت تاثیر قرار می دهد. شواهد نشان داده اند که برنامه های مکمل فرایند درمان همچون فیزیوتراپی با وجود کاهش عوارض ناخواسته، مغز و امواج آن را به عنوان عامل اصلی بروز بیماری و کنترل حرکات بدن نادیده می گیرند. از این رو، هدف از این پژوهش بررسی تاثیر یک دوره ی تمرین های نوروفیدبک بر تعادل و سقوط در بیماران مبتلا به پارکینسون بود.روش هااین مطالعه در زمره ی پژوهش های نیمه تجربی با طرح پیش آزمون- پس آزمون بود که بر گروه های مورد و شاهد انجام شد. از بین داوطلبین، 16 بیمار بر اساس معیارهای تحقیق انتخاب و به طور تصادفی به دو گروه تجربی و شاهد تقسیم گردیدند. گروه تجربی در 8 جلسه تمرین های نوروفیدبک واقعی و گروه شاهد در 8 جلسه نوروفیدبک ساختگی شرکت نمودند. پیش و پس از دوره ی نوروفیدبک خطر سقوط و ثبات وضعیتی، اندازه گیری و مقایسه شد. برای ارزیابی خطر سقوط و ثبات وضعیتی از دستگاه بایودکس و به ترتیب از آزمون های خطر سقوط (Fall risk) و ثبات وضعیتی (Postural stability) در سطح 8 استفاده گردید. سپس نتایج پیش و پس آزمون مورد تجزیه و تحلیل آماری قرار گرفت.یافته هامیانگین نمرات خطر سقوط و ثبات وضعیتی گروه تجربی پس از انجام مداخله، بهبود معنی داری داشت (001/0 > P). میانگین نمرات خطر سقوط (450/0 = P) و ثبات وضعیتی (750/0 = P) گروه شاهد تغییر معنی داری نداشت.نتیجه گیریتمرین های نوروفیدبک می تواند در کنار روش های مرسوم پزشکی مانند دارودرمانی برای کنترل عوارض بیماری پارکینسون به خصوص حفظ وضعیت و جلوگیری از سقوط استفاده شود.

صفحه 2267مقدمهدر بیماران آلوده به HIV (Human immunodeficiency virus)، کاندیدیازیس دهانی عفونت قارچی رایج است. هدف از این مطالعه، تایپینگ مولکولی جدایه های کاندیدا آلبیکنس به دست آمده از بیماران آلوده به HIV مبتلا به کاندیدیازیس دهانی و مقایسه ی الگوی مولکولی جدایه های مقاوم و حساس به داروی فلوکونازول بود.روش هاتایپینگ مولکولی و تعیین الگوی DNA ژنومی 48 جدایه ی کاندیدا آلبیکنس به دست آمده از بیماران آلوده به HIV با روش RAPD-PCR (Random amplified polymorphic DNA-polymerase chain reaction) به انجام رسید. همچنین، حساسیت جدایه ها به فلوکونازول به روش میکرودایلوشن براث (Broth microdilution method) تعیین گردید.یافته هاجدایه های کاندیدا آلبیکنس در دو شاخه ی اصلی، 7 خوشه و 24 ژنوتایپ قرار گرفتند. تفاوت معنی داری بین الگوی جدایه های جدا شده از بیماران بستری، بیماران سرپایی و فرم بالینی عفونت کاندیدیازیس دهانی مشاهده نگردید. ارتباط نسبی بین جدایه های مقاوم و حساس به دارو در قرارگیری در خوشه ها وجود داشت.نتیجه گیریتعدد ژنوتایپ های کاندیدا آلبیکنس در بیماران آلوده بهHIV اقدامات درمانی و پیشگیرانه را با مشکل مواجه می کند. عدم وجود ارتباط بین ژنوتایپ ها و فرم بالینی عفونت کاندیدیازیس دهانی- حلقی، نشانگر اهمیت شرایط میزبان در پاتوژنیسیتی قارچ است. عدم وجود رابطه ی فیلوژنتیک بین جدایه های به دست آمده از بیماران بستری، تاییدی بر اندوژن می باشد و عامل بیماری است. نتایج نشان داد که بیمار آلوده به HIV شرایط مساعد رشد و تکثیر ژنوتایپ های مختلف کاندیدا آلبیکنس را دارد. در نتیجه، تایپینگ مولکولی با استفاده از RAPD-PCR ناهمگونی ژنومی در جدایه های بالینی کاندیدا آلبیکنس را در بیماران HIV نشان می دهد.

صفحه 2278مقدمههیپربیلی روبینمی به عنوان شایع ترین علت بستری مجدد در نوزادان سالم شناخته شده است. هدف از این تحقیق، تعیین خطر بروز زردی شدید در روزهای آینده در نوزادان ترم و نزدیک ترم ایرانی هنگام ترخیص از بیمارستان توسط نوموگرام بیلی روبین متری پوستی بود.روش هااین مطالعه ی کاربردی، از نوع توصیفی بود و به صورت تصادفی بر روی 450 نوزاد سالم انجام شد که سن حاملگی ≥ 35 هفته و وزن ≥ 2 کیلوگرم و سن بین 140-12 ساعت داشتند. بیلی روبین نوزادان قبل از ترخیص آن ها با دستگاه Bilichek که روی سینه و مابین دو Nipple قرار می گرفت، تعیین می شد و پس از ترخیص پیگیری می شدند. نوزادان بر اساس TCB (Transcutaneous bilirubin) هنگام ترخیص و سن نوزاد به ساعت و بر اساس نوموگرام بیلی روبین در 4 گروه (خطر کم، خطر متوسط پایین، خطر متوسط بالا و خطر بالا) تقسیم شدند. در پیگیری در صورتی که نوزاد دچار زردی شده بود، دوباره بیلی روبین وی اندازه گیری می شد و نوزادانی که دچار زردی شدید شده بودند و نیاز به فتوتراپی داشتند، مشخص می شدند.یافته هابر اساس TCB هنگام ترخیص و سن نوزاد به ساعت، 0/20 درصد در گروه با خطر کم، 1/33 درصد در گروه با خطر متوسط پایین، 2/32 درصد در گروه با خطر متوسط بالا و 7/14 درصد در گروه با خطر بالا طبقه بندی شدند. خطر بروز زردی شدید در پیگیری در گروه های با خطر کم تا زیاد به ترتیب 4/1، 9/6، 6/13 و 8/32 درصد بود. در طی این مدت، 9/11 درصد نوزادان دچار زردی شدید بالای 95 درصد شدند.نتیجه گیریبا تهیه ی نوموگرام بیلی روبین بر اساس TCB و بر حسب عمر نوزاد به ساعت، می توان شدت بروز زردی شدید در نوزادان سالم ایرانی را در آینده تخمین زد. استفاده از روش TCB با دستگاه بیلی چک به طور معمول، که روش غیر تهاجمی می باشد، در زمان ترخیص در تمام دنیا برای نوزادان پیشنهاد می شود.

صفحه 2287مقدمهکلوپیدوگرل یک داروی ضد پلاکتی است که در میان داروهای قلبی جایگاه مهمی یافته است. کاربرد اصلی این دارو در ترکیب با آسپیرین و در پیشگیری از انسداد در بیمارانی است که تحت استنت گذاری عروق کرونری قرار گرفته اند. هدف از این مطالعه، بررسی تاثیر مصرف کلوپیدوگرل بر خونریزی و ترانسفوزیون خون و فراورده های آن حین و بعد از عمل جراحی CABG (Coronary artery bypass graft) بود.روش هادر این مطالعه، 100 بیمار که تحت درمان با کلوپیدوگرل بودند، شرکت داشتند. بیماران با توجه به زمان قطع دارو در 4 دسته قرار می گرفتند. 2، 3، 6-4 و 8-7 روز قبل از عمل فواصلی بودند که هر گروه از بیماران مبادرت به قطع کلوپیدوگرل کردند. میزان خونریزی و ترانسفوزیون خون و فراورده های خونی حین و بعد از عمل جراحی، اندازه گیری و ثبت شد و ارتباط آن با زمان قطع دارو مورد ارزیابی قرار گرفت.یافته هامیانگین سن شرکت کنندگان 7/61 سال (محدوده ی 77-50 سال) بود. بین زمان قطع دارو قبل از عمل CABG و میزان خونریزی حین و بعد از عمل، ترانسفوزیون خون حین و بعد از عمل و ترانسفوزیون فراورده ی خونی حین و بعد از عمل ارتباط معکوس وجود داشت (050/0 > P و r به ترتیب 750/0-، 695/0-، 654/0-، 545/0-، 556/0- و 475/0- بود).نتیجه گیریبین زمان قطع کلوپیدوگرل قبل از عمل جراحی با خونریزی و ترانسفوزیون خون حین و بعد از عمل جراحی ارتباط معکوس وجود داشت. با توجه به نتایج این مطالعه می توان گفت که بین مصرف کلوپیدوگرل قبل از عمل CABG و خونریزی حین و بعد از عمل ارتباط وجود دارد.

صفحه 2295چندین دهه است که موضوع آنالوگ های اسید های نوکلئیک توجه برخی دانشمندان علم بیولوژی را به خود معطوف نموده است. برخی از این مولکول های صناعی به عنوان ابزارهای قدرتمندی در بیولوژی مولکولی و بیوتکنولوژی، مورد توجه ویژه قرار گرفته اند. چهار دسته ی عمده ی این آنالوگ های صناعی عبارت از زنونوکلئیک اسید (XNA یا Xeno nucleic acid)، مورفولینو (Morpholino) و اسید نوکلئیک قفل شده (LNA یا Locked nucleic acid) و نوکلئیک اسید پپتیدی (PNA یاPeptide nucleic acid) که دسته ی چهارم این آنالوگ ها بود و در این میان، جایگاه برجسته ای دارد. PNA، پلیمری است از واحدهای ان-آمینو اتیل گلایسین که به هر یک از این واحدها بازهای پورین/ پیریمیدین اتصال یافته است. این پلیمر، خصوصیات پپتیدها و اسیدهای نوکلئیک را توامان دارد و قادر است به طور اختصاصی به نوکلئیک اسیدهای طبیعی یا PNA های دیگر، هیبرید شود که این ویژگی و دیگر خصوصیات ویژه ی این مولکول باعث به کارگیری آن در بسیاری از روش های بیولوژی مولکولی در زمینه ی تشخیص و درمان بیماری ها شده است. در این مقاله، ساختار، خصوصیات بیوشیمیایی و کاربردهای زیست پزشکی PNA شرح داده می شود.

Page 2257BackgroundWhen Parkinson's disease progresses, complications such as impaired balance, can affect patient's quality of life. Results have indicated that complementary program processes such as physiotherapy can reduce these complications while the brain and its waves are ignored. This study aimed to verify the effect of neurofeedback training on postural stability and fall risk in patients with Parkinson's disease.MethodsThis was an experimental research with pre-, post-test method and control group. 16 patients were selected through purposive sampling and were randomly divided to two experimental and control groups. The experimental group participated in 8 sessions of actual neurofeedback training as well as control group participated in 8 sessions of sham neurofeedback training. Before and after the training, the pre- and post-test of postural stability and fall risk were administered using biodex system.FindingsThe mean scores of fall risk and the postural stability improved significantly after the intervention in experimental group (P < 0.001), while there were no significant changes among control group in fall risk (P = 0.450) and the postural stability (P = 0.750) scores.ConclusionOur results showed that the neurofeedback training can be used to control the symptoms of Parkinson's disease, especially postural stability and fall risk.Keywords: Parkinson's disease, Neurofeedback, Postural stability, Fall risk

Page 2267BackgroundIn human immunodeficiency virus HIV-infected patients, oropharyngeal candidiasis is a common infection. The aim of present study was molecular typing of Candida albicans isolates recovered from HIV-infected patients with oropharyngeal candidiasis and comparing the molecular patterns of fluconazole-resistant and susceptible isolates.MethodsThe molecular typing of genomic DNA of 48 Candida albicans isolated from HIV-infected patients were assessed by the random amplified polymorphic DNA-polymerase chain reaction (RAPD-PCR) fingerprinting. Sensitivity of strains to fluconazole was determined by the broth microdilution method.FindingsWe detected two main clusters and seven sub-clusters, comprising 24 genotypes of Candida albicans strains. No associations between the patterns of strains isolated from hospitalized patients and outpatients and clinical forms of oropharyngeal candidiasis were detected. There was also a relative correlation between fluconazole susceptibility and strain clustering.ConclusionThe presence of several genotypes of Candida albicans in HIV-infected patients would lead to problems in the treatment and prevention of oral candidiasis. The lack of correlation between genotypes and clinical forms of oropharyngeal candidiasis shows the importance pf host condition in the pathogenesis of candida infections. The lack of phylogenetic relationships among strains from hospitalized patients approves endogenous origin of candida infection. The results indicate that HIV-infected patients have favorable conditions for the growth and proliferation of different genotypes of Candida albicans. In conclusion, molecular typing using RAPD-PCR revealed a genomic heterogeneity in the Candida albicans clinical isolates studied in HIV-infected patients.Keywords: Oropharyngeal candidiasis, Candida albicans, Genotyping, Human immunodeficiency virus (HIV), infected patients

Page 2278BackgroundHyperbilirubinemia is one of the most common causes of neonatal re-hospitalization; using transcutaneous bilirubin (TCB) measurement nomogram, the aim of this study was to determine the risk of severe hyperbilirobinemia in term and near-term Iranian neonates at discharge time.Methods450 healthy newborn infants, with the gestational age of ≥ 35 weeks, weight of ≥ 2 kg, and the age of 12 to 140 hours were enrolled in this randomized-descriptive practical study. Before being discharged, the infant’s bilirubin was measured using Bilichek that was placed on their chest and between two nipples. After being discharged, the newborn infants were followed up. According to the discharge time transcutaneous bilirubin level, the infant's age (in terms of hour), and based on bilirubin nomogram, the newborns were divided into four groups of low-risk, low-medium-risk, high-medium-risk, and high-risk. In following up the infants, if they were suffering hyperbilirobinemia, the bilirubin was measured again.FindingsAccording to discharge time transcutaneous bilirubin level and the infant's based-on-hour age, 20% of the subjects were included in the low-risk group, 33.1% in low-medium-risk group, 32.2% in high-medium-risk group, and 14.7% in high-risk group. The risk of severe hyperbilirobinemia in followed-up newborns was 1.4%, 6.9%, 13.6%, and 32.8%, in groups 1 to 4, respectively. During this time, 11.9% of the newborns were found to have sever hyperbilirubinemia (more than 95th percentile.ConclusionBy providing bilirubin nomogram, based on transcutaneous bilirubin level and the infant's age (in terms of hour), the severity of future hyperbilirobinemia can be estimated in Iranian healthy newborns. Page 2287BackgroundClopidogrel is an adenosine-5' diphosphate (ADP) receptor antagonist that eventually inhibits platelet aggregation. It is used concomitantly with percutaneous coronary interventions and in patients with acute coronary diseases. This study was conducted to determine the effects of preoperative clopidogrel administration on perioperative blood loss and transfusion requirements in patients undergoing coronary artery bypass graft (CABG) surgery.MethodsFor 100 patients who underwent nonurgent first-time coronary artery bypass grafting surgery participated in this study. clopidogrel was discontinued in 2, 3, 5-6 or 7-8 days before surgery. A cell salvage device was used during surgery and salvaged blood was recorded. After surgery, chest tube output and need for blood transfusion were measured and recorded.FindingsPatient's mean age was 61.7 ± 8.0 years (range: 50 to 77). There was a reverse relation between the time clopidogrel had been discontinued and blood loss and need for transfusion of packed cell and blood products during and after surgery (P < 0.05 for all).ConclusionPreoperative clopidogrel administration increases perioperative blood loss and need for blood transfusion in patients undergoing coronary artery bypass grafting surgery.Keywords: Clopidogrel, Coronary artery bypass grafting (CABG) surgery, Blood Loss, Transfusion

Page 2295For decades, nucleic acid analogues have been emerged in molecular biology. Some of these analogues are powerful bimolecular tools in biotechnology. Among the four major classes of these synthetic molecules (XNA: xenonucleic acid, LNA: locked nucleic acid, PNA: peptide nucleic acid and morpholino), PNA is the most important. In PNA, N-aminoethyle glycine units (to which purine/pyrimidine bases have been attached), are joined together to form an achiral and peptide-like structure that mimics the behavior of DNA and can hybridize to nucleic acid and to other PNA in sequence specific manner. Because of its unique physicochemical properties, PNA in many cases prefers over natural oligonucleotide and therefore, have been exploited in some strategies and methods in the field of diagnostics and therapy.
