بررسی تاثیر متیل فنیدات بر اثر ضد دردی متادون در موش صحراییصفحه 1مقدمهاز مشکلات استفاده از اپیوئیدها، تحمل به اثرات ضد دردی است. یکی از راه های کاهش دوز مصرفی و در نتیجه، تحمل و وابستگی به اپیوئیدها اضافه نمودن عوامل غیر اپیوئیدی به رژیم دارویی است. از طرفی، مطالعات نشان داده است که استفاده ی مکرر از داروی متیل فنیدات، که از دسته آمفتامین ها است، سبب افزایش بیان گیرنده های اپیوئیدی مو و کاهش درد می شود. بنابراین، در این مطالعه به بررسی اثر متیل فنیدات بر اثر ضد دردی متادون پرداخته شد.روش هادر این مطالعه، از 9 گروه 8 تایی موش صحرایی نر نژاد ویستار در محدوده ی وزنی 20 ± 270 گرم استفاده شد. گروه ها شامل گروه شاهد (دریافت کننده ی نرمال سالین)، سه گروه دریافت کننده ی متیل فنیدات (5، 10 و 15 میلی گرم بر کیلوگرم)، سه گروه دریافت کننده ی متادون (5، 10 و 20 میلی گرم بر کیلوگرم) و دو گروه ترکیبی متادون (5 میلی گرم بر کیلوگرم) با متیل فنیدات (5 و 10 میلی گرم بر کیلوگرم) استفاده شد. تاخیر در احساس درد با استفاده از دستگاه درد حرارتی (آزمون پلانتار) بررسی شد.یافته هامصرف متیل فنیدات به صورت خوراکی فقط در دوز 15 میلی گرم بر کیلوگرم دارای اثر ضد دردی بود. همچنین، ترکیب دوزهای غیر موثر متیل فنیدات به همراه 5 میلی گرم بر کیلوگرم متادون، اثرات ضد دردی متادون را در دقیقه ی 60 پس از مصرف خوراکی به طور معنی داری افزایش داد.نتیجه گیریبه طور کلی، متیل فنیدات اثر ضد دردی متادون در مدل حیوانی را افزایش داد.

صفحه 2مقدمهپوسیدگی دندان از شایع ترین بیماری های عفونی می باشد که تمامی اقشار، سنین و طبقات جامعه را درگیر می کند. درمان پوسیدگی های دندانی مخارج سنگینی را در همه ی کشورها تحمیل می کند. با توجه به این که گیاهان از منابع طبیعی فراوان و سالم می باشند، همیشه به عنوان یکی از منابع مهم دارویی مطرح بوده اند. این مطالعه، با هدف بررسی اثر ضد باکتریایی عصاره ی اتانولی گل محمدی بر روی استرپتوکوک های عامل پوسیدگی دندان انجام گرفت.روش هادر این مطالعه، اثر ضد میکروبی عصاره ی اتانولی گل محمدی بر روی مهم ترین استرپتوکوک های عامل پوسیدگی دندان شامل استرپتوکوکوس موتانس، استرپتوکوکوس سوبرینوس، استرپتوکوکوس سالیواریوس و استرپتوکوکوس سنگوئیس به روش میکروتیتر پلیت برای تعیین حداقل غلظت ممانعت از رشد (Minimum inhibitory concentration یا MIC) و حداقل غلظت کشندگی (Minimum bactericidal concentration یا MBC) انجام گرفت. همچنین، تاثیر این عصاره روی اتصال این باکتری ها مورد بررسی قرار گرفت. نتایج با استفاده از آزمون واریانس یک طرفه، تجزیه و تحلیل شد.یافته هامیزان حداقل غلظت ممانعت از رشد برای استرپتوکوک های موتانس، سوبرینوس، سالیواریوس و سنگوئیس به ترتیب 5/ 7، 5/ 7، 0/ 15 و 0/ 15 میلی گرم بر میلی لیتر و حداقل غلظت کشندگی برای این باکتری ها به ترتیب 0/ 15، 5/ 7، 0/ 15 و 0/ 30 میلی گرم بر میلی لیتر به دست آمد. همچنین، نشان داده شد که رقت های مختلف این عصاره، باعث کاهش 43 تا 93 درصدی تشکیل بیوفیلم در این باکتری ها می شود.نتیجه گیریبا توجه به تاثیر ضد باکتریایی عصاره ی الکلی گل محمدی بر روی هر چهار سویه ی باکتری عامل پوسیدگی دندان، این عصاره می تواند در پیش گیری و کنترل پوسیدگی دندان مورد استفاده قرار گیرد.

طراحی الگوریتم جایگاه لاپاروسکوپی در جراحی توده های تخمدانی بر مبنای نمره ی احتمال بدخیمیصفحه 3مقدمهکاربرد لاپاروسکوپی در جراحی های زنان و از جمله، توده های تخمدانی، در سال های اخیر وسعت یافته است. در این مطالعه، جایگاه لاپاروسکوپی در جراحی توده های تخمدانی مورد بررسی قرار گرفت.روش هاابتدا، با استفاده از مدل (OMPS1 (Ovarian Malignancy Prediction Score، بر مبنای یافته های مربوط به سن و سونوگرافی که حساسیت بالا دارد و سپس، با استفاده از مدل مدل اختصاصی تر OMPS2، آلگوریتم کاربرد و جایگاه لاپاروسکوپی در توده های تخمدانی طراحی شد.یافته هادر صورتی که جراح از مهارت سطح 3 لاپاروسکوپی برخوردار باشد، بیماران با OMPS1 کمتر از 3 /2 (رد بدخیمی) می توانند تحت لاپاروسکوپی قرار گیرند. در بقیه موارد، به شرط دسترسی به Frozen section، جراحی با لاپاروسکوپی شروع شود و در صورتی که جراح مهارت سطح چهارم لاپاروسکوپی را داشته باشد، تا در صورت نیاز به مرحله بندی کامل (با توجه به جواب پاتولوژی Frozen section) بتواند لاپاروسکوپی را ادامه دهد، تا انتهای جراحی رویکرد لاپاروسکوپی ادامه یابد.نتیجه گیریدر جراحی توده های آدنکس، نحوه ی اداره ی بیمار با توجه به Frozen section و بر مبنای راهنمای بالینی تعیین می شود و بسته به وجود مهارت لاپاروسکوپی، می توان از این رویکرد استفاده کرد.

صفحه 4مقدمهانسولین هورمون پروتئینی است که توسط سلول های بتای پانکراس ترشح می شود. به علت معایب تولید پروتئین های نوترکیب در میکروارگانیسم ها، هزینه به نسبت بالا، امکان آلودگی با پروتئین های سمی و مراحل هزینه بر خالص سازی، تولید پروتئین های نوترکیب در گیاهان امری قابل بررسی است. توسعه ی سیستم بیان گذرا بر پایه ی نوع حذف شده ی RNA-2 ویروس موزائیک لوبیا چشم بلبلی (Cowpea Mosaic Virus-Hyper Translatable یا CPMV-HT)، امکان تولید سریع و سطوح بالای پروتئین ها را بدون استفاده از همانندسازی ویروسی فراهم کرده است.روش هادر این مطالعه، سازه های (pBI121-Proinsulin-Zera (pBI-ProZ در بر دارنده ی توالی ژن پروانسولین انسانی و Zera (دومین انتهای N غنی از پرولین گاما- زئین ذرت) و pCAMBIA1304-Proinsulin-Extensin (pCAMBIA-ProE) در بر دارنده ی سیستم بیانی CPMV-HT، به منظور بهبود ترجمه ی ژن پروانسولین انسانی و توالی سیگنال پپتید اکستنسین هویج ساخته شد. این دو سازه، به واسطه ی باکتری Agrobacterium tumefaciens pv. C58، به صورت بیان گذرا به گیاهان کاهو و یونجه انتقال داده شدند. تحلیل آماری این پژوهش بر پایه ی آزمایش فاکتوریل، در قالب طرح به کلی تصادفی بر روی غلظت پروانسولین تولیدی در هر گرم برگ تراریخت صورت گرفت. بیان ژن پروانسولین در بافت گیاهی تراریخت در سطح رونویسی، با استفاده از واکنش Reverse (transcription polymerase chain reaction (RT-PCR و در سطح ترجمه، با استفاده از آزمون لکه گذاری نقطه ای و (Enzyme-linked immunosorbent assay (ELISA مورد تایید قرار گرفت.یافته هامیزان پروانسولین فعال تولیدی با سازه های pCAMBIA-ProE و pBI-ProZ، در برگ های یونجه تراریخت، به ترتیب 82/6 و 32/4 نانوگرم در هر گرم برگ تر و در برگ های کاهوی تراریخت، به ترتیب 6/6 و 8/3 نانوگرم در هر گرم برگ تر بود.نتیجه گیریطبق نتایج به دست آمده از این تحقیق، میزان بیان پروتئین پروانسولین در سازه ی pCAMBIA-ProE در بر دارنده ی خصوصیات سیستم بیانی CPMV-HT، بیشتر از سازه ی pBI-ProZ بود.

سندرم متابولیک یکی از اختلالات رایج در کودکان است که با مجموعه ای از فاکتورهای خطر از قبیل چاقی، افزایش قند خون، تری گلیسرید و فشار خون و کاهش لیپوپروتئین با چگالی بالا (High-density lipoprotein یا HDL) همراه است. این اختلال چند عاملی به علت افزایش خطر ابتلا به دیابت نوع 2 و بیماری های قلبی- عروقی در بزرگ سالی، یکی از علل اصلی مرگ و میر در سراسر جهان است. مطالعات اخیر نشان داده است که این سندرم، شیوع بالایی در کودکان و نوجوانان چاق دارد. اتیولوژی سندرم متابولیک پیچیده است و نتیجه ای از برهم کنش عوامل ژنتیک و محیطی است که در پیشرفت چاقی، مقاومت به انسولین و چندین فرآیند التهابی تاثیر می گذارد. مطالعه روی جوانان ایرانی، شیوع نزدیک به 23 درصد سندرم متابولیک را نشان می دهد. در این مقاله ی مروری، به تعریف سندرم متابولیک، شیوع، علل ابتلا و مهم ترین ژن های درگیر در این اختلال اشاره می شود.

Effect of Methylphenidate on Methadone-Induced Analagesia in RatPage 1BackgroundA known problem of chronic administration of opioid is tolerance to its analgesic effect. Co-administration of a non-opioid analgesic may help reducing the opiate dose and consequently tolerance and dependency. It is reported that repeated use of methylphenidate, one of amphetamines, increased the expression of µ opioid receptor and had antinociception. Therefore, this study aimed to evaluate the effect of methylphenidate on methadone-induced analgesia.MethodsNine groups of 8 male rats weighting 270 ± 20 g were randomly selected; control group received 1 ml/kg oral normal saline, metylphenidate groups received 5, 10 and 15 mg/kg oral metylphenidate, methadone groups received 5, 10, 20 mg/kg oral methadone, and combined groups received 5 mg/kg methadone +5 or 15 mg/kg methylphenidate orally. Pain was measured using plantar test apparatus.FindingsOnly, 15 mg/kg methylphenidate produced significant analgesic effect. In addition, co-administration of non-effective doses of methylphenidate and 5 mg/ kg methadone significantly increased the analgesic effects of methadone at 60 minutes after gavage.ConclusionWe found that methylphenidate potentiate the analgesic effects of methadone in rats.Keywords: Pain, Methadone, Methylphenidate, Plantar test

Page 2BackgroundTooth decay is the most common infectious disease that involves all groups, ages and classes of the society. Treatment of dental caries imposes high costs in all countries. Since the plants are safe and abundant natural resources, they have always been one of the major sources of pharmaceuticals. This study aimed to evaluate the antibacterial effect of alcoholic extract of Damask rose on cariogenic streptococci.MethodsIn this study, the antimicrobial effect of ethanol extract of Rosa damascena on the cariogenic streptococci, including Streptococcus mutans, Streptococcus sobrinus, Streptococcus salivarius and Streptococcus sanguis was assessed via microtitre plate method for minimum inhibitor concentration (MIC) and minimum bactricidal concentration (MBC). The effect of the extract on attachment of these bacteria was surveyed. Results were analyzed using one-way analysis of variances.FindingsThe minimum inhibitor concentrations for the Streptococcus mutans, Streptococcus sobrinus, Streptococcus salivarius and Streptococcus sanguis were 7.5, 7.5, 15.0, and 15.0 mg/ml, respectively; and minimum bactricidal concentration for these bacteria were 15.0, 7.5, 15.0, 30.0 mg/ml, respectively. In addition, it was shown that different dilutions of Rosa damascena extract decreased the biofilm formation in these bacteria between 43 to 93 percent.ConclusionDue to the antibacterial effect of Rosa damascena on all four strains of cariogenic bacteria, the extract can be used for prevention and control of dental caries.Keywords: Dental caries, Oral streptococci, Rosa damascena, Antimicrobial effect

Algorithm of Laparascopy Choice in Ovarian Mass Surgery According to Malignancy Probability ScorePage 3BackgroundUse of laparoscopy in gynecologic surgeries, including adnexal masses, has increased in recent years. The present research intended to study the role of laparoscopy in adnexal masses surgery.MethodsThe algorithm for the use and role of laparoscopy in adnexal masses surgery was designed via using first, the model of Ovarian Malignancy Prediction Score (OMPS1) according to findings related to age and ultrasound with high sensitivity and then, the model of OMPS2 (a more specific model).FindingsIn case, the surgeon possesses the skill for the level three of laparoscopy, the patients with OMPS1 of less than 2.3 (rule out of malignancy) may undergo laparoscopy. As concerns other cases, provided that frozen section is available, surgery starts with laparoscopy and shall continue up to the end with complete staging, in case the surgeon possesses the skill for the level four of laparoscopy.ConclusionIn adnexal masses surgery, management of the patients is determined via using frozen section and on the basis of the clinical guideline and this method may be used depending on the skill the surgeon possesses for laparoscopy.Keywords: Laparoscopy, Adnexal masses, Surgery

Page 4BackgroundInsulin is a hormone exclusively produced by pancreatic beta cells. The production of recombinant proteins in microorganisms has some disadvantages such as high cost, the possibility of contamination with toxic proteins, and costly purification steps; so, the production of recombinant proteins in plants can be investigated. Development of transient expression system based on a deleted version of Cowpea mosaic virus RNA-2, Cowpea Mosaic Virus-Hyper Translatable (CPMV-HT), has provided the extremely high-level and rapid production of proteins without viral replication.MethodsIn this study, two constructions were prepared; pBI121-Proinsulin-Zera (pBI-ProZ) containing human proinsulin gene and Zera (N-terminal proline-rich domain of γ-zein) and pCAMBIA1304-Proinsulin-Extensin (pCAMBIA-ProE) containing CPMV-HT expression system for improvement of translation of human proinsulin gene and carrot extensin signal peptide. Both structures were transiently transferred in to lettuce and alfalfa leaves using Agrobacterium tumefasiens pv. C58. Statistical analysis of this study was conducted on the concentration of produced proinsulin per each gram of transgenic leaf using factorial arrangement of treatment in a complete randomized design. Gene expression was confirmed in transcription level using reverse transcription polymerase chain reaction (RT-PCR) method and in translation level using enzyme-linked immunosorbent assay (ELISA) and Dot blot assay.FindingsProtein accumulation for pCAMBIA-ProE and pBI-ProZ constructs were 6.82 and 4.32 ng/g in recombinant alfalfa leaves and 6.6 and 3.8 ng/g in recombinant lettuce leaves, respectively.ConclusionResults showed that the expression of proinsulin in pCAMBIA-ProE contains CPMV-HT expression system was more than pBI-ProZ.Keywords: Human proinsulin gene, Transient expression, Cowpea Mosaic Virus, Hyper Translatable (CPMV, HT) expression system, Transgenic plant tissue

Metabolic syndrome (MetS) is a prevalent disorder in children and it is described as a group of risk factors including obesity, increased fasting blood sugar, increased triglyceride, high blood pressure and decreased high-density lipoprotein (HDL) levels. This multifactorial disorder has been considered as one of the leading causes of mortality throughout the world and has been considered as a major risk factor for type 2 diabetes mellitus and atherosclerotic cardiovascular diseases. Recent studies have shown that the syndrome is highly prevalent among overweight children and adolescents. The etiology of the metabolic syndrome is complex, and is determined by the interaction between genetic and environmental factors and their influence on the development of obesity, insulin resistance and various inflammatory processes. Recent findings on Iranian children and adolescents show the prevalent of 23%. In this review, we looked at the definition, prevalence etiology and the main determinant genes in the development of this disorder.
