    مقدمه
    اختلال بیش فعالی/ نقص توجه، شایع ترین اختلال عصبی- رفتاری دوران کودکی است که در سنین قبل از 7 سالگی تشخیص داده می شود و از آن جایی که خشم، بخشی از زندگی افراد است و باید به کودکان آموخت که چگونه با خشم خود کنار بیایند، این پژوهش با هدف تعیین اثربخشی بازی های کنترل خشم بر علایم رفتاری و خشم کودکان بیش فعال/ نقص توجه اجرا گردید.
    روش ها
    مطالعه ی حاضر از نوع مورد-شاهدی و نیمه تجربی بود که به صورت پیش آزمون- پس آزمون انجام شد. جامعه ی آماری شامل 40 نفر دانش آموز 13-7 ساله ی بیش فعال/ نقص توجه مراجعه کننده به کلینیک روان پزشکی کودک و نوجوان شهر اصفهان ساکن نجف آباد بود که به صورت در دسترس انتخاب شدند و بعد از تایید تشخیص توسط فوق تخصص روان پزشکی کودک و نوجوان به طور تصادفی در دو گروه مورد و شاهد قرار گرفتند. روش مداخله، شامل مجموعه بازی های کنترل خشم بود. ابزار پژوهش، شامل مجموعه ی پرسش نامه ی خشم در کودکان و پرسش نامه های توانایی ها و مشکلات والد و معلم بود. داده ها با استفاده از روش تحلیل آماری ANCOVA و نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفت.
    یافته ها
    بین آزمودنی ها در گروه های مورد و شاهد از لحاظ نمره ی خشم، تفاوت معنی داری وجود داشت (001/0 = P و 990/6 = F). میزان تاثیر یا تفاوت برابر با 159/0 بود. همچنین، بین آزمودنی ها در گروه مورد و شاهد از لحاظ نمرات هیجانی، مشکلات رفتاری، بیش فعالی، مشکل با همتایان و مشکلات اجتماعی بر اساس نظر والدین یا معلمین تفاوت معنی داری وجود داشت.
    نتیجه گیری
    بازی های کنترل خشم، بر خشم و علایم رفتاری، هیجانی و ارتباطی کودکان بیش فعال/ نقص توجه اثر داشت. استفاده از این بازی ها، به دلیل سهولت استفاده ی کودکان و هزینه ی پایین، به عنوان روش های کمکی نوین توصیه می گردد.

    مقدمه
    مطالعات گذشته، حاکی از آن هستند که دزهای بالای تیموکینون، قادر به متوقف نمودن حرکت اسپرم ها در محیط کشت است. هدف از این مطالعه بررسی تاثیر عصاره ی سیاه دانه و تیموکینون بر روی حرکت و آپوپتوز اسپرم در مردان نورموزواسپرم بود.
    روش ها
    برای هر کدام از غلظت های سیاه دانه )5 و 10 میلی گرم بر میلی لیتر) 10 نمونه ی شسته شده ی مایع منی مورد استفاده قرار گرفت. دو حجم 5/0 میلی لیتری از هر نمونه با یا بدون دز مورد نظر به عنوان نمونه ی مورد و شاهد انکوبه شدند. حرکت کلی اسپرم ها، بعد از یک ساعت تعیین گردید. به علاوه، آپوپتوز اسپرم ها به دنبال دریافت دز 10 میلی گرم بر میلی لیتر با روش رنگ آمیزی Annexin-Propidium iodide و به دنبال آن بررسی با روش فلوسایتومتری انجام پذیرفت.
    یافته ها
    هر دو دز عصاره ی سیاه دانه، به طور قابل توجهی سبب کاهش درصد کل حرکت، درصد اسپرم ها با حرکت پیش رونده ی سریع و کند و حرکت غیر پیش رونده می گردند. بعد از استفاده از عصاره ی سیاه دانه، درصد اسپرم های زنده، آپوپتوتیک و نکروتیک تغییر قابل ملاحظه ای نشان ندادند.
    نتیجه گیری
    دزهای 5 و 10 میلی گرم بر میلی لیتر عصاره ی سیاه دانه، به طور قابل ملاحظه ای درصد اسپرم های متحرک را در محیط کشت کاهش می دهند. به همین ترتیب، عصاره ی سیاه دانه با دز 10 میلی گرم بر میلی لیتر، درصد اسپرم های زنده را در محیط کشت تغییر نمی دهد.

    مقدمه
    خونریزی، مهم ترین عارضه ی ضربه ی مغزی می باشد و در صورتی که بتوان به نحوی جلوی افزایش حجم خونریزی را گرفت، نه تنها میزان ناخوشی و مرگ و میر کاهش می یابد، بلکه نیاز به دخالت جراحی نیز کمتر می شود. هدف از انجام این مطالعه، تعیین تاثیر داوری ترانگزامیک اسید بر انواع خونریزی مغزی در تروماهای مغزی بود.
    روش ها
    طی یک مطالعه ی کارآزمایی بالینی، 190 بیمار مبتلا به ترومای مغزی، به روش تصادفی در دو گروه 95 نفره توزیع شدند؛ گروه مورد، تحت تزریق ترانگزامیک اسید و گروه شاهد، تحت تزریق دارونما (نرمال سالین، قرار گرفتند و بیماران در قبل از تجویز دارو و 24 ساعت بعد، تحت Computed tomography scan (CT scan) قرار گرفتند و یافته های قبل و بعد از درمان، از نظر وجود نوع خونریزی داخل جمجمه و حجم آن، ایجاد خونریزی جدید و میزان افزایش حجم آن توسط پزشک رادیولوژیست تعیین و داده های به دست آمده با استفاده از نرم افزار SPSS بین دو گروه مقایسه شد.
    یافته ها
    میانگین حجم خونریزی اولیه در دو گروه مورد و شاهد به ترتیب 9/4 ± 7/4 و 2/4 ± 0/5 سی سی بود و بر حسب آزمون t، اختلاف معنی داری بین دو گروه وجود نداشت (640/0 = P). حجم خونریزی در 24 ساعت بعد از مداخله، در دو گروه مورد و شاهد به ترتیب 80/5 ± 69/4 و 85/4 ± 12/5 سی سی بود و تفاوت معنی داری بین دو گروه مشاهده نشد (590/0 = P). میانگین تغییرات حجم خونریزی در دو گروه مورد و شاهد نیز به ترتیب 71/3 ± 01/0- و 48/2 ± 13/0- بود؛ اما اختلاف معنی داری بین دو گروه مشاهده نشد (810/0 = P). بررسی حجم خونریزی بر حسب نوع و محل خونریزی، نشان داد که در بیماران کانتوژن، حجم خونریزی به طور معنی داری کاهش پیدا کرد.
    نتیجه گیری
    تاثیر تزریق ترانگزامیک اسید در بیماران دچار ترومای مغزی، بیشتر به محل و نوع ضایعه بستگی دارد و اثرات آن در بیماران دچار کانتوژن مشهودتر می باشد، اما برای مشخص شدن تاثیر کلی دارو، انجام مطالعات بیشتر توصیه می گردد.

    مقدمه
    Candidiasis منتشره، دیابت، حاملگی، مصرف بی رویه ی آنتی بیوتیک های وسیع الطیف، عوامل مستعد کننده ی Candidiasis دستگاه ادراری می باشند. شیوع عفونت با گونه های غیر Candida albicans رو به افزایش است. هدف از انجام این مطالعه، بررسی میزان شیوع گونه های Candida مسبب کاندیدوری در شهر اصفهان با استفاده از روش Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) بود.
    روش ها
    از کشت تازه ی مخمری، DNA مربوط به کمک روش Fast technology for analysis-card (FTA-card) استخراج گردید. نواحی ITS1-5.8s-ITS2 به روش PCR تکثیر و سپس تحت تاثیر آنزیم MspI طی مراحل RFLP برش داده شد. محصولات در ژل آگارز الکتروفورز و بر اساس تفاوت های باندهای ایجاد شده، گونه های Candida شناسایی گردید.
    یافته ها
    از تعداد کل 3200 نمونه ی ادرار بیماران، تعداد 80 نمونه شرایط ورود به مطالعه را داشتند؛ به این صورت، میزان شیوع کاندیدوری در مطالعه ی حاضر به میزان 5/2 درصد گزارش گردید. 4 نفر (5 درصد) از بیماران مورد مطالعه مرد و 76 نفر (95 درصد) زن بودند. عوامل زمینه ای شامل دیابت (5/47 درصد)، عفونت دستگاه ادراری (8/38 درصد)، سنگ کلیه (0/5 درصد) و سایر بیماری های زمینه ای (7 نفر معادل 8/8 درصد) بودند. 3/46 درصد نمونه ها، دارای 103 × 99-103 × 50 واحد کلنی در میلی لیتر ادرار (CFU/ml) بودند. Candida glabrata بالاترین فراوانی (3/41 درصد) را در بین انواع گونه ها به خود اختصاص داد.
    نتیجه گیری
    افزایش گونه های غیر Candida albicans و مقاومت ذاتی برخی گونه ها، درمان Candidiasis را با چالش هایی روبه رو کرده است. شناسایی و تعیین هویت دقیق گونه های Candida، به خصوص در افراد مبتلا به دیابت و با عفونت ادراری، به منظور درمان صحیح و موثر، امری لازم و حیاتی به نظر می رسد.

    مقدمه
    در جامعه ی ما، مسمومیت عمدی در خانم های باردار مقوله ی شایعی نیست. با این وجود، در این مقاله، یک مورد مسمومیت یک خانم باردار با سم حشره کش گزارش می گردد.
    گزارش مورد: خانم 36 ساله خانه دار با حاملگی 24 هفته، به دنبال خوردن سم حشره کش با کاهش سطح هوشیاری، تاکی کاردی، استفراغ با تشخیص مسمومیت با سم حشره کش ارگانوکلره به مرکز مسمومیت ها اعزام گردید. 20 دقیقه پس از ورود، دچار تشنج تونیک- کلونیک ژنرالیزه و سیانوز شد. بیمار تحت درمان های لازم قرار گرفت. بعد از رفع مسمومیت، روز بعد بیمار به دلیل درد شکم و خونریزی واژینال به بیمارستان زنان اعزام شد. نوزاد نارس زنده متولد شد و پس از 25 روز زندگی در انکوباتور، فوت کرد. مادر، با حال عمومی خوب مرخص و به دلیل اقدام به خودکشی، مشاوره ی روان پزشکی شد.
    نتیجه گیری
    مسمومیت با سموم حشره کش به خصوص از نوع مسمومیت عمدی، می تواند برای مادر و جنین خطرناک باشد.

    Background
    Hyperactivity/attention deficit disorder is the most common neurobehavioral disorders in childhood usually diagnosed before the age of seven. Since anger is a part of the people's life, the children need to learn how to deal with their anger. This project was performed aiming to determine the effectiveness of anger management games on behavioral and anger symptoms of the children with hyperactivity/attention deficit disorder.
    Methods
    This study is a semi-experimental research with a pre-test and post-test design conducted by using a control group. The statistical population includes 40 students aged between 7 and 13 with hyperactivity/attention deficit disorder in Najafabad, Iran referred to child and adolescent psychiatric clinic in Isfahan city (Iran). They were selected as in-available and after verifying the diagnosis by a child and adolescent subspecialist psychiatrist and placed in two test and control groups, randomly. The intervention method included a collection of Anger Management Games. The research tool included Children׳s Inventory of Anger Questionnaire as well as Parent and Teacher Strengths and Difficulties Questionnaire. The data was analyzed through the statistical analysis method of covariance using SPSS software.
    Findings: The results showed that there is a significant difference between the subjects in test and control groups in terms of anger scores (F = 6. 99, P = 0, 001). The value of impact or difference was 0. 159. Moreover based on the parents’ or teachers’ views, there is a significant difference between subjects in test and control groups regarding emotional scores, behavioral problems, hyperactivity, problems with their peers, and social problems.
    Conclusion
    Anger management games were effective on anger, behavioral, emotional and relational symptoms of children with hyperactivity/attention deficit disorder; utilizing these games as a novel supplementary method by the children is recommended due to the ease of use and low cost.

    Background
    Previous studies have revealed that high doses of Thymoquinone (TQ) is able to cease the movement of sperms in culture medium.The aim of this study was to investigate and evaluate whether Nigella sativa (NS) extract has similar effect on spermatozoa with TQ or not.
    Methods
    Ten washed semen samples were used for each concentration of NS (5 and 10 mg/ml). Two 0.5 mlaliquots of each sample were incubated with or without the considered dose as case and control samples. Total sperm motility was assessed after one hour. In addition, apoptosis of sperms receiving dose of 10 mg/ml were measured by Annexin-PI staining method followed by flow cytometry detection.
    Findings: Both doses of NS extract substantially reduce the percentage of total motility, fast and slow progressive and non-progressive sperms. After administration of NS extract, the percentages of alive, apoptotic and necrotic sperms did not show significant change.
    Conclusion
    5 and 10 mg/ml doses of NS extract considerably diminish the percentage of motile spermatozoa in culture medium. Similarly, NS extract with dose of 10 mg/ml does not change the percentage of viable spermatozoa in culture medium.

    Background
    Hematoma is the most serious complication of head trauma and if we are capable to prevent the increment in size of hematoma, not only the mortality and morbidity but also the need of surgical intervention will decrease in management of head trauma. The aim of this study was to evaluate the effect of Tranexamic acid on traumatic brain hematomas.
    Methods
    In a double blind clinical study, 190 head injured patients in whom the primary brain Computed tomography scan (CT scan) had confirmed the existence of one of the different kinds of hematoma were selected and randomly divided in two groups, the first group received Tranexamic acid and the second group received placebo (normal saline). For all patients another brain CT scan was performed 24 hour after receiving the medication for detecting new bleeding or a change in size of the previously existing hematoma and the CT scans were reported by an expert radiologist. The collected data was analyzed by SPSS software.
    Findings: Before the intervention the mean volume of the hematoma in the case and control group was 4.7 ± 4.9 and 5.0 ± 4.2, respectively and there was not statistically difference between the two groups (P = 0.64). The mean change in the volume of hematoma after intervention in the case and control group was -0.01 ± 3.71 and -0.13 ± 2.48, respectively and no statistically significant difference between the two groups were observed but considering the type of hematoma, the study showed meaningful decrease in the size of brain contusions in the case group.
    Conclusion
    The effect of Tranexamic acid in traumatic brain injuries depends on the type of the lesion so that its effect on brain contusions was more prominent, although further studies are recommended to determine the overall effect of medication.

    Background
    Disseminated candidiasis, diabetes mellitus, pregnancy, and consumption of widespread antibiotics are predisposing factors of urinary tract infections due to Candida species. The prevalence of non-albicans species is increasing. The aim of the present study is to identify Candida spp. isolated from candiduria patients in Isfahan, Iran using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method.
    Methods
    The genomic DNA was extracted by fast technology for analysis-card (FTA-card). Polymerase chain reaction (PCR) was used to amplify ITS1-5.8s-ITS2 region then PCR products were digested with MspI restriction enzyme. PCR and RFLP products were revealed on 1.5 and 2% agarose gel electrophoresis, respectively.
    Findings: Out of 3200 urine samples 80 samples with more than 104 CFU/ml were involved in this study. Patients including 4 (5%) were males and 76 patients (95%) were females. Predisposing factors were diabetes (47.5%), urinary tract infection (38.8%) and kidney stone (5.0%). The colony count of 46.3% of the urine samples were with 50×103-99×103 CFU/ml. Candida glabrata was the most prevalent species among isolates (41.3%).
    Conclusion
    Increasing of non-albicans Candida species and drug resistance of isolates are main challenges of Candida infection treatment particularly in patients with diabetes and urinary infections. Identification of Candida species is inevitable for better management of infection.

    Background
    Intentional poisoning in pregnant women is not common in our society. In this paper, we report a case of pesticide poisoning in a pregnant woman.
    Case Report: A 36-year-old housewife with 24 weeks pregnancy following ingestion of pesticide presented with low level of consciousness, tachycardia, and vomiting and was diagnosed as insecticide poisoning. 20 minutes after the arrival generalized tonic-colnic seizure and cyanosis was occurred. She recovered with supportive cares. Because of abdominal pain and vaginal bleeding she was referred to the gynecology department. The mother was discharged with good general condition, and due to suicide attempt underwent psychiatric consultation. However premature infant died after 25 days living in the incubator.
    Conclusion
    Pesticides poisoning, especially in a suicide attempt can be dangerous for mother and fetus.
