صفحات 470-475مقدمهتعبیه ی لید ضربان ساز سبب ایجاد آسیب در میوکارد می شود که می تواند با افزایش قطعه ی ST همراه باشد. تغییر در میزان قطعه ی ST، می تواند با تغییرات آستانه ی تحریک (Threshold)، حس (Sense)، میدان نوسان (Amplitude)، مقاومت ظاهری (Impedance) و حساسیت (Sensitivity) در ارتباط باشد. از طرفی، رابطه ی میزان عددی افزایش قطعه ی ST با عملکرد ضربان ساز روشن نیست.روش هامطالعه ی حاضر یک مطالعه ی هم گروهی آینده نگر بود که بر روی 83 بیمار مراجعه کننده به بخش قلب بیمارستان بوعلی سینای قزوین جهت تعبیه ی ضربان ساز دایمی (Permanent pacemaker یا PPM) در فاصله ی زمانی آذرماه 1390 تا اردیبهشت 1392 انجام شد. کلیه ی بیماران در یک مرکز و توسط یک الکتروفیزیولوژیست مورد تعبیه ی ضربان ساز دایمی قرار گرفتند. اطلاعات با مشاهده و ثبت داده های آنالیز ضربان ساز دایمی (تغییرات ST، آستانه ی تحریک، حس و مقاومت ظاهری) در حین کارگذاری با استفاده از پرسش نامه جمع آوری گردید و با استفاده از نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفت.یافته هامیانگین سنی بیماران 4/1 ± 7/71 سال بود. 1/48 درصد از بیماران مورد پژوهش مذکر و 8/51 درصد مونث بودند. برای 25 بیمار ضربان ساز دایمی تک حفره ای و 58 بیمار ضربان ساز دایمی دو حفره ای تعبیه شد. متوسط تغییرات ST لید دهلیزی 27/1 ± 95/3 میلی ولت و در لید بطنی 41/3 ± 71/16 میلی ولت بود. میزان تغییرات پارامترها (حس، مقاومت ظاهری و آستانه ی تحریک) به طور جداگانه در لید دهلیزی و بطنی به دست آمد و با تغییرات قطعه ی ST مقایسه شد. رابطه ی تغییرات ST فقط در دهلیز راست با تغییرات آستانه ی تحریک و حس دهلیزی ارتباط معنی دار داشت (003/0 = P).نتیجه گیریدر پژوهش حاضر، رابطه ی معنی داری بین تغییرات ST و سایر پارامترها دیده نشد. می توان چنین استنباط کرد که استفاده از تغییرات ST پیش بینی کننده ی مناسبی جهت عملکرد ضربان ساز دایمی در آینده نیست.کلیدواژگان: ضربان ساز مصنوعی، نوار قلب، حساسیت

صفحات 476-481مقدمهمطالعات پیشین نشان داده است که بخش های مراقبت های ویژه با میزان بروز بالای مرگ و میر مواجه هستند، اما عوامل موثر در مرگ بیماران بستری در این بخش، به طور کامل بررسی نشده است. از این رو، مطالعه ی حاضر با هدف تعیین ویژگی های دموگرافیک، اپیدمیولوژیک و علل بستری بیماران در بخش مراقبت های ویژه ی مرکز آموزشی- درمانی الزهرای (س) اصفهان در سال 1394 انجام شد.روش هاطی یک مطالعه ی مقطعی، 400 بیمار بستری در بخش مراقبت های ویژه انتخاب شدند و اطلاعات دموگرافیک و بالینی و بروز مرگ و میر و علت مرگ در آن ها تعیین و ثبت شد و نقش این عوامل در مرگ بیماران مورد بررسی قرار گرفت.یافته هااز 400 بیمار بستری شده در بخش مراقبت های ویژه در سال های 94-1390، 138 نفر (5/34 درصد) فوت کردند که 44 نفر (9/31 درصد) در روز اول بستری، 55 بیمار (9/39 درصد) در فاصله ی زمانی 6-1 روز، 28 نفر (3/20 درصد) در فاصله ی 12-7 روز و 11 نفر (0/8 درصد) در فاصله ی زمانی 24-13 روز از موقع ورود به بخش مراقبت های ویژه فوت نمودند. از بین عوامل مورد بررسی، سن بیمار، وضعیت علایم حیاتی در بدو ورود، ابتلا به بیماری زمینه ای، زمان ورود به بخش مراقبت های ویژه و برخی یافته های آزمایشگاهی در مرگ بیماران تاثیر معنی داری داشتند.نتیجه گیریمیزان مرگ و میر در بخش مراقبت های ویژه بالا می باشد که عوامل مختلفی در آن نقش دارد. از این رو، لازم است امکانات و تجهیزات بخش های مراقبت های ویژه با توجه به ویژگی های بیماران و علل شایع مرگ و میر در این بخش فراهم گردد.کلیدواژگان: بستری، مرگ و میر، بخش مراقبت های ویژه

صفحات 482-487مقدمههدف از انجام این مطالعه، بررسی تغییرات سطح پروتئین Occludin و ارتباط آن با دز مثانه در بیماران تحت پرتودرمانی خارجی پروستات بود.روش هادر این تحقیق، 64 بیمار از میان مردان مبتلا به سرطان پروستات که کاندیدای پرتودرمانی بودند، انتخاب شدند. برای پردازش اطلاعات طرح درمان بیماران، برنامه ای در محیط نرم افزار Matlab (version 2008a، MathWorks، USA) نوشته شد. با کمک این برنامه، میزان دز مثانه به صورت میانگین دز کل حجم مثانه به دست آمد. نمونه های ادرار بیماران، در فواصل زمانی مشخص و به منظور بررسی سطح پروتئین Occludin جمع آوری شد. الگوی زمانی انجام آزمایش ها، شامل قبل و بعد از هر 5 جلسه از پرتودرمانی تا اتمام درمان و سپس، هر 1 ماه از اتمام پرتودرمانی تا 3 ماه یا به عبارت دیگر، 3 مرتبه بعد از اتمام پرتودرمانی بود.یافته هاافزایش در بیان Occludin مثانه ی بیماران از جلسه ی پنجم درمان شروع شد (046/0 = P) و در جلسه ی پانزدهم به بیشترین میزان خود (تا 7/24 نانوگرم بر میلی لیتر) رسید. بر اساس روش محاسباتی دز میانگین مثانه، میزان دز برای ایجاد چنین آسیبی 5/25 گری محاسبه شد. علاوه بر این، تغییرات در بیان نشانگر Occludin در توافق خوبی با آسیب های تکرر ادرار، عجله در تخلیه ی ادرار و درد حین دفع ادرار قرار داشت (050/0 > P).نتیجه گیریافزایش معنی دار در بیان Occludin، می تواند به عنوان یک روش تشخیصی در بررسی آسیب ناخواسته ی مثانه ی بیماران تحت پرتودرمانی خارجی پروستات مورد استفاده قرار گیرد.کلیدواژگان: آسیب های مثانه، Occludin، دز، پرتودرمانی، سرطان مثانه

صفحات 488-495مقدمهانگ یا استیگما (Stigma)، یک برچسب منفی است که مردم اغلب به گروه ها و افرادی که در جنبه هایی مانند ظاهر، نژاد، سلامت جسم و روان با آن ها متفاوتند، نسبت می دهند. استیگما، تاثیرات مخرب زیادی بر تمام ابعاد زندگی افرادی که برچسب خوردند، می گذارد. از آن جایی که بیماران مبتلا به اختلالات روان پزشکی، گروهی از این افراد هستند و یکی از شایع ترین این بیماری ها افسردگی است، این پژوهش در جهت شناخت عوامل موثر بر روی استیگمای افسردگی در بین دانشجویان اجرا شد تا زمینه ی اطلاعاتی برای مداخلات احتمالی در آینده باشد.روش هامطالعه ی حاضر، یک پژوهش مقطعی بود. میزان استیگما در نگرش 553 نفر از دانشجویان دانشگاه اصفهان، دانشگاه علوم پزشکی اصفهان و دانشگاه هنر اصفهان نسبت به افسردگی سنجیده شد. استیگما، بر اساس ترجمه ی پرسش نامه ی Griffiths و همکاران سنجیده شد که شامل دو معیار استیگمای شخصی و استیگمای ادراک می باشد. داده ها با استفاده از روش های آماری توصیفی و استنباطی، تجزیه و تحلیل گردید.یافته هاتجربه ی شخصی مثبت افسردگی با کاهش استیگمای شخصی افسردگی، ارتباط معنی داری داشت (007/0 = P). دانشجویان متاهل، به طور معنی داری میزان استیگمای شخصی کمتری داشتند (030/0 = P) و دانشجویان دانشگاه اصفهان، نمره ی استیگمای ادراک شده ی بیشتری نسبت به دو دانشگاه دیگر کسب کردند که این اختلاف معنی دار بود (009/0 = P).نتیجه گیریسابقه ی مثبت ابتلا به افسردگی، متاهل بودن و دانشجوی دانشگاه علوم پزشکی و یا دانشگاه هنر بودن، از عوامل محافظت کننده در برابر استیگمای افسردگی در میان دانشجویان می باشد.کلیدواژگان: افسردگی، دانشجویان، استیگمای جمعی

صفحات 496-502مقدمهPseudomonas aeruginosa یکی از مهم ترین پاتوژن های عامل عفونت در بیماران سوختگی است. هدف از انجام این مطالعه، تعیین فراوانی ژن های exoU و exoS و الگوی حساسیت آنتی بیوتیکی Pseudomonas aeruginosa جدا شده از بیماران سوختگی در کرمانشاه بود.روش هادر این مطالعه ی توصیفی- مقطعی، تعداد 194 نمونه با استفاده از روش های استاندارد باکتری شناسی بررسی شدند. پس از سنجش حساسیت آنتی بیوتیکی ایزوله ها به روش انتشار از دیسک، از پرایمرهای اختصاصی جهت فراوانی ژن های exoU و exoS در میان ایزوله ها استفاده شد. داده های به دست آمده با استفاده از نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفت.یافته هااز مجموع 91 ایزوله ی Pseudomonas aeruginosa شناسایی شده، 72 مورد (1/79 درصد) آن ها، Multidrug resistance (MDR) داشتند. بیشترین مقاومت آنتی بیوتیکی در برابر جنتامایسین (1/79 درصد) و سفتازیدیم (7/74 درصد) و همچنین، بیشترین حساسیت نسبت به کلیستین (100 درصد) و پلی مکسین B (3/92 درصد) بود. فراوانی ژن های exoU و exoS برابر با 2/80 درصد و 1/68 درصد بود. بین فراوانی ژن exoU و مقاومت به سفتازیدیم (041/0 = P) و سفوتاکسیم (050/0 = P) از نظر آماری رابطه ی معنی داری وجود داشت.نتیجه گیریبا توجه به نقش باکتری Pseudomonas aeruginosa در ایجاد عفونت های زخم سوختگی و نقش ژن های سیتوتوکسین و ژن های مقاومت آنتی بیوتیکی در کلونیزاسیون و بقای این باکتری، پرهیز از درمان های تجربی، تشخیص دقیق نوع عوامل ویرولانس (Virulence) و تعیین مقاومت آنتی بیوتیکی جهت انتخاب آنتی بیوتیک مناسب و کارا به منظور جلوگیری از عفونت، امری ضروری به نظر می رسد.کلیدواژگان: Pseudomonas aeruginosa، اگزوآنزیم های U و S، مقاومت آنتی بیوتیکی

Pages 470-475BackgroundActive lead implantation can create injury in myocardium that may be associated with a ST-segment elevation. This ST-segment elevation may be correlated with changes in other parameters such as threshold, sense, amplitude, impedance, and sensitivity. But, the relationship between the numerical elevating of ST-segment with permanent pacemaker device performance is not clear.MethodsIn this prospective cohort study, census sampling method was used to enroll 83 patients referred to Bou Ali Sina hospital, Qazvin, Iran, for permanent pacemaker device implantation during December 2010 to May 2012. The procedure was conducted by an electrophisiologist. Data such as ST-segment elevation and device parameters (threshold, sense, and impedance) were collected via recording during active implantation of device and analyzed using SPSS software.
Findings: Patient's mean age was 71.7 ± 1.4 years; 48.1% were men and 51.8% were women. Single chamber device was implanted for 25 cases and dual chamber device for 58 others. The mean ST-segment elevation was 3.95 ± 1.27 and 16.71 ± 3.41 mv in atrial and ventricular leads, respectively. The changes in other parameters separately in the atrial and ventricular leads were obtained and compared considering ST-segment changes. ST changes in the right atrium were correlated with sense and threshold (P = 0.003).ConclusionSince we could not find significant correlation between pacemaker parameters and ST elevation, it can be concluded that ST elevation during lead implantation is not a predictor for device performance.Keywords: Pacemaker, Artificial, Electrocardiography, Sensitivity -
Pages 476-481BackgroundPrevious studies showed that mortality rate in intensive care units is high but etiology of this problem has not been studied completely. So, this study aimed to determine epidemiologic and demographic aspects and causes of hospitalization and mortality in patients admitted to intensive care units.MethodsIn a cross-sectional study, 400 patients hospitalized during 2011-2015 in intensive care units of Alzahra hospital, Isfahan, Iran, were studied. Demographic data, clinical findings, mortality, and causes of hospitalization were extracted from hospital records and role of the factors on the mortality were investigated.
Findings: Of 400 hospitalized patients in intensive care units during 2011-2014, 138 (34.5%) died, 44 (31.9%) of them in the first day, 55 (39.9%) during 1st-6th days, 28 (20.3%) during 7th-12th days, and 11 (8.0%) during 13th-24th days after hospitalization in intensive care units. Among factors that related to mortality, vital signs at the entrance to intensive care unit, underlying disease, time of hospitalization in intensive care unit, and some of laboratory findings had significant effect on mortality.ConclusionMortality rate in intensive care units of Alzahra hospital is high. Therefore, special care facilities considering patient characteristics and causes of mortality in this sector should be provided.Keywords: Hospitalization, Mortality, Intensive care unit -
Pages 482-487BackgroundThe aim of this study was to assess the effects of fractionated radiotherapy (RT) on expression of urothelial tight junction protein (occluding) in patients under prostate radiotherapy. Finding a correlation between occludin expression and bladder dose for patients undergoing prostate conformal external radiotherapy was the second aim.MethodsUrine samples of 64 patients under prostate radiotherapy were collected before and at regular intervals during and 3 months after radiotherapy. Occludin expression analysis of the collected urine samples was performed using chemical methods. The effective uniform dose to bladder was derived from the individually produced dose treatment plan of the patients.
Findings: There was a significant change in urine occludin level after initiation of the radiotherapy course compared to the baseline urine samples (P = 0.040). The threshold bladder dose that caused significant change in occludin level was 25.5 Gy (P = 0.046). This bladder dose was occurred after the 15th radiotherapy session. In addition, change in urine occludin level was compatible with frequency, urgency, and dysuria (PConclusionSignificant change in occludin expression occurred during prostate radiotherapy. Pages 488-495BackgroundStigma is a mark of disgrace in the society's attitude that discredits a person or a group because of an attribute such as a deformity, color, or mental illness. Stigma can adversely affect all aspects of life. Depression is one of the most common mental health problems. The goal of the current study was to investigate some factors associated with stigma of depression among university students in Isfahan City, Iran, in order to facilitate possible interventions.MethodsThis cross-sectional study was designed to evaluate the stigma of depression in students of University of Isfahan, Isfahan University of Medical Sciences, and Art University of Isfahan, and was conducted among 553 participants. We used a questionnaire designed by Griffiths et al. and translated it to Persian to evaluate the personal and perceived stigma.
Findings: Personal stigma was statistically lower in participants with positive history of depression (P = 0.007). It was also lower in married participants (P = 0.030). Perceived stigma was significantly higher in students of University of Isfahan comparing to other universities (P = 0.009).ConclusionPositive history of depression, being a married person, and being a student of Isfahan University of Medical Sciences or Art University of Isfahan are protective factors against the stigma of depression in university students in Isfahan City.Keywords: Depression, Social stigma, Students -
Pages 496-502BackgroundPseudomonas aeruginosa is one of the major pathogens causing burnt wound infection in hospitals. This study aimed to evaluate the prevalence of exoenzyme U (exoU) and exoenzyme S (exoS) genes and the antibiotic resistance pattern of Pseudomonas aeruginosa isolated from burn patient's samples in Kermanshah City, Iran.MethodsIn this cross-sectional study, 194 samples were tested with conventional bacteriological methods. After evaluation of antibiotic sensitivity with disc diffusion method, specific primers were deployed to assess the frequency of exoU and exoS genes among isolates. The retrieved data were analyzed using SPSS software.
Findings: From 91 isolates of Pseudomonas aeruginosa, 72 (79.1%) were multi-drug resistant (MDR) isolates. The most prevalent antibiotic resistances were against gentamicin (79.1%) and ceftazidime (74.7%); the most prevalent sensitivities were against colistin (100%) and polymixin B (92.3%). The frequency of exoU and exoS genes was 80.2% and 68.1%, respectively. There were significant relationships between the frequency of exoU gene and resistance to ceftazidime (P = 0.041) and cefotaxime (P = 0.050).ConclusionConsidering the role of Pseudomonas aeruginosa in burn wound infections and the role of cytotoxin and antibiotic resistance genes in colonization and survival of this bacteria, avoiding home remedies, accurate detection of virulence factors, and recognition of antibiotic resistance pattern among the isolates in order to choose the appropriate antibiotic regimen to prevent infection seem to be necessary.Keywords: Pseudomonas aeruginosa, exoenzyme S, exoenzyme U, Antibiotic resistance
