    مقدمه
    آژیتاسیون پس از جراحی قلب باز، یکی از عوارض شایع و تهدید کننده ی حیات است که شناخت عوامل موثر بر آن، سبب کاهش بروز این عارضه می شود. هدف از انجام مطالعه ی حاضر، بررسی فراوانی نسبی بروز آژیتاسیون بعد از عمل جراحی قلب باز و عوامل موثر بر آن بود.
    روش ها
    این مطالعه، بر روی 100 بیمار کاندیدای جراحی قلب باز انجام گرفت. اطلاعات دموگرافیک و فراوانی نسبی بروز آژیتاسیون بعد از عمل با استفاده از معیار Richmond agitation-sedation scale (RASS) و بررسی بیماران هر 24 ساعت یک بار تا 72 ساعت ثبت شد. برای مقایسه ی متغیرهای مورد مطالعه، از آزمون های Independent t، 2χ و Mann-Whitney استفاده شد.
    یافته ها
    شیوع کلی آژیتاسیون در بیماران تحت عمل جراحی قلب باز، 40 درصد بود. همچنین، مشخص شد که شیوع آژیتاسیون در بیماران مبتلا به دیابت (050/0 = P)، بیماران مبتلا به فشار خون بالا (001/0 > P)، بیماران مصرف کننده ی سیگار (001/0 > P) و مواد مخدر (002/0 = P) به صورت معنی داری بالاتر از سایر بیماران بود. همچنین، مشخص شد که بروز عوارض حین جراحی در بیماران آژیته به طور معنی داری بیشتر از بیماران دیگر بود (045/0 = P). همچنین، در این بیماران، میانگین مقدار White blood cell (WBC) قبل از جراحی کمتر و مدت زمان جراحی، میانگین مورفین، میدازولام، پتیدین و مدت زمان بستری در Intensive care unit (ICU) بیشتر بود (050/0 > P).
    نتیجه گیری
    در مطالعه ی حاضر، بروز آژیتاسیون با دیابت، ابتلا به فشار خون، مصرف سیگار، اعتیاد به مواد مخدر، عوارض حین جراحی، میانگین WBC قبل از جراحی، مدت زمان جراحی، میانگین مورفین، میدازولام و پتیدین دریافتی در 24 ساعت اول و مدت زمان بستری در ICU ارتباط معنی داری داشت.

    مقدمه
    لرز بعد از عمل، یک عارضه ی شایع در بی حسی نخاعی است که تا کنون روش های مختلفی برای کنترل آن ارایه شده است. این مطالعه، با هدف تعیین تاثیر تجویز 2 دز هیدروکورتیزون وریدی در مقایسه با گروه شاهد، بر پیش گیری از لرز بعد از بی حسی نخاعی در اعمال جراحی سزارین انتخابی انجام شد.
    روش ها
    در یک مطالعه ی کارآزمایی بالینی، 135 زن باردار کاندیدای سزارین، در سه گروه 45 نفره توزیع شدند. در گروه های اول و دوم، به ترتیب 1 و 2 میلی گرم بر کیلوگرم هیدروکورتیزون و در گروه سوم، نرمال سالین با حجم مشابه بعد از کلامپ بند ناف به شکل وریدی طی 5 دقیقه به بیماران تزریق شد و بروز لرز بعد از عمل در سه گروه تعیین و مقایسه گردید.
    یافته ها
    75 بیمار، دچار لرز بعد از عمل شدند که 23 نفر (8/54 درصد) از گروه هیدروکورتیزون 1 میلی گرم/کیلوگرم، 16 نفر (4/36 درصد) از گروه هیدروکورتیزون 2 میلی گرم/کیلوگرم و 36 نفر (7/85 درصد) از گروه شاهد بودند و بروز لرز بعد از عمل، در سه گروه تفاوت معنی داری داشت (001/0 > P).
    نتیجه گیری
    تزریق 2 میلی گرم/کیلوگرم هیدروکورتیزون وریدی در بیماران تحت عمل جراحی سزارین با بی حسی نخاعی، با کاهش بروز لرز بعد از عمل در بیماران همراه بود و به نظر می رسد استفاده از این دارو، عارضه ی جانبی خاصی را برای بیماران در پی ندارد. در عین حال، انجام مطالعات بیشتر به منظور تعیین تاثیر قطعی هیدروکورتیزون در پیش گیری از لرز بعد از عمل توصیه می گردد.

    مقدمه
    در این مطالعه، مقایسه ای بین انواع طراحی درمان های مختلف Intensity-modulated radiation therapy (IMRT) برای تومورهای ناحیه ی پیشانی جهت دستیابی به تکنیک بهینه ی درمان انجام گردید.
    روش ها
    طراحی و انجام کانتورینگ بر روی اسکن 20 بیمار، با نرم افزار TiGRT صورت گرفت. سه تکنیک مقایسه شدند. تکنیک اول چهار فیلد، تکنیک دوم پنج فیلد و تکنیک سوم شش فیلد IMRT بود. در نهایت، با مقایسه ی میانگین دز رسیده به هر عضو، با استفاده از نمودار Dose volume histogram (DVH) تکنیک بهینه معرفی گردید.
    یافته ها
    در هر سه تکنیک، بر اساس منحنی ایزودز، 30 درصد دز تجویزی از چشم می گذرد، اما نکته ی قابل توجه این است که با افزایش تعداد فیلدها به سمت پایین چشم کشیده می شود.
    نتیجه گیری
    مقدار دز دریافتی عدسی در تکنیک دوم، که پنج فیلد است و هیچ کدام از فیلدها به طور مستقیم از چشم نمی گذرد، کمتر است.

    مقدمه
    دیابت، منجر به اختلال در توانایی تولید مثل جنسی نر می شود. پودر بامیه، دارای اثرات ضد دیابتی، آنتی اکسیدانی و محافظتی می باشد. هدف از انجام این مطالعه، بررسی اثرات پودر بامیه بر تغییرات بافت موش های صحرایی مبتلا به دیابت شده توسط جیره ی پرچرب و استرپتوزوتوسین بود.
    روش ها
    25 سر موش صحرایی به صورت تصادفی به 5 گروه شامل گروه های شاهد، مبتلا به دیابت، مبتلا به دیابت/پودر بامیه (200 میلی گرم/کیلوگرم)، مبتلا به دیابت/متفورمین (200 میلی گرم/کیلوگرم) و پودر بامیه (200 میلی گرم/کیلوگرم) تقسیم شدند. دیابت، توسط رژیم غذایی پرچرب به مدت 4 هفته و تزریق استرپتوزوتوسین (35 میلی گرم/کیلوگرم) القا شد. گروه ها، به مدت 4 هفته مورد تیمار قرار گرفتند. پس از تیمار، میزان قند خون، میزان هورمون تستوسترون، پارامترهای اسپرمی و هیستومورفولوژی بیضه مورد بررسی قرار گرفت.
    یافته ها
    القای دیابت، موجب کاهش هورمون تستوسترون، حرکت، تعداد و زنده مانی اسپرم شد. کاهش معنی دار تعداد سلول های اسپرماتوگونی، اسپرماتوسیت اولیه، سرتولی و لیدیگ، قطر لوله های اسپرم ساز و ضریب اسپرمیوژنز در گروه مبتلا به دیابت در مقایسه با گروه شاهد مشاهده گردید (05/0 > P). در حالی که تیمار با پودر بامیه به صورت قابل توجهی اثرات مضر دیابت بر بافت بیضه و پارامترهای اسپرم در موش های صحرایی را بهبود بخشید.
    نتیجه گیری
    رژیم غذایی حاوی پودر بامیه در موش های صحرایی مبتلا به دیابت شده توسط جیره ی پرچرب و استرپتوزوتوسین موجب بهبود هورمون تستوسترون، شاخص های اسپرم و همچنین، بهبود اختلالات ساختاری ناشی از دیابت در سیستم تولید مثلی نر می شود.

    مقدمه
    Multiple Sclerosis (MS) یک عامل ناتوانی عصبی شایع در بزرگ سالان و یک بیماری خود ایمن با مکانیسم ناشناخته و پیچیده است که با آسیب آکسونی و دمیلینه شدن نورون های سیستم عصبی مرکزی مشخص می شود. یکی از مکانیسم های مطرح در بیماری زایی MS، عوامل رشد عصبی هستند. از طرفی، فعالیت بدنی می تواند یکی از مکانیسم های موثر در تنظیم عوامل رشد عصبی در افراد مبتلا به MS باشد. هدف از انجام این مطالعه ی مروری، بررسی نظام مند تاثیر فعالیت بدنی بر عوامل رشد عصبی در افراد مبتلا به MS است.
    روش ها
    مطالعه ی مروری نظام مند، بر اساس واژه های سیستم Medical subject headings در پایگاه های اطلاعاتی Pubmed، Googlescholar، Sciencedirect و Springer به زبان انگلیسی و پایگاه های اطلاعاتی Magiran و SID به زبان فارسی انجام شد. معیارهای انتخاب مطالعه، شامل فارسی یا انگلیسی بودن زبان مقاله، بازه ی زمانی 15 سال اخیر، تمرکز صرف مطالعه بر روی مبتلایان به MS، مداخله ی ورزشی یا فعالیت بدنی در مبتلایان به MS به همراه ارزیابی عوامل رشد عصبی بود. روش کار مطالعات، با استفاده از مقیاس Physiotherapy evidence database ارزیابی شد.
    یافته ها
    از بین 161 مطالعه ی مورد بررسی، 9 مطالعه شرایط مورد نظر را داشتند. میانگین شاخص Physiotherapy evidence database مطالعات مرور شده 55/6 بود. مطالعات اغلب از Brain-derived neurotrophic factor (8 مطالعه) و Nerve growth factor (4 مطالعه) استفاده کرده بودند. نتایج اغلب مطالعات نشان داد که فعالیت بدنی منجر به افزایش Brain-derived neurotrophic factor می شود، اما تغییرات معنی دار Nerve growth factor را به دنبال ندارد.
    نتیجه گیری
    فعالیت بدنی منظم، می تواند به عنوان یک روش درمانی کمکی در بیماری MS مورد توجه باشد و از طریق تنظیم عوامل رشد عصبی و به خصوص Brain-derived neurotrophic factor، بر روند بیماری MS موثر باشد؛ هر چند برای اثبات قطعی این یافته، انجام مطالعات بیشتری مورد نیاز است.

    Background
    Agitation after coronary artery bypass grafting (CABG) operation is a common and life-threatening complication. Recognizing factors affecting it will reduce the incidence of this complication. The aim of this study was to evaluate the incidence of agitation after CABG operation and the factors affecting it.
    Methods
    This study was conducted on 100 patients undergoing CABG operation. Demographics and incidence of postoperative agitation were recorded using Richmond Agitation-Sedation Scale (RASS) criteria for all patients every 24 hours to 72 hours. Analysis was done using t, chi-square, and Mann-Whitney tests.
    Findings: Overall prevalence of agitation in patients undergoing CABG was 40%. The prevalence of agitation in patients with diabetes (P = 0.050), hypertension (P
    Conclusion
    In our study, incidence of agitation had significant correlation with diabetes, hypertension, smoking, drug addiction, surgery complications, and mean WBC count before the surgery, and duration of surgery, and amount of received morphine, midazolam, and pethidine in the first 24 hours, and the length of stay in intensive care unit.

    Background
    Postoperative shivering is one of the most prevalent complications that several methods were used for controlling it. This study aimed to determine the effect of two doses of intravenous hydrocortisone (1 or 2 mg/kg) on postoperative shivering in cesarean section with spinal anesthesia.
    Methods
    In a randomized controlled clinical trial study, 135 pregnant women candidate for cesarean were randomly divided in three equal groups. The groups were received 1 or 2 mg/kg hydrocortisone, or the same volume of normal saline (as control group) during the 5 minutes after umbilical clamping and postoperative shivering was determined and compared between the groups.
    Findings: Postoperative shivering was happened in 23 patients (54.8%) in 1 mg/kg hydrocortisone group, 16 patients (36.4%) in 2 mg/kg hydrocortisone group, and 36 patients (85.7%) in control group; the difference between the three groups was statistically significant (P
    Conclusion
    Intravenous injection of 2 mg/kg hydrocortisone in patients under cesarean with spinal anesthesia led to decrease postoperative shivering. It seems that using this dose does not lead to serious complications. But, more studies are recommended to determine real effect of this dose of hydrocortisone.

    Background
    In this study, a comparison was performed between various intensity-modulated radiation therapy (IMRT) techniques for treatment of frontal area tumors in order to achieve the optimal treatment technique considering absorbed dose to the eye lens.
    Methods
    Treatment planning and contouring were performed with TiGRT software for 20 patients with frontal area tumors. Three techniques were compared to each other. Finally, by comparing the dose volume histograms, the average dose received by each organ was determined and the best method was selected.
    Findings: In all three techniques, isodose curve of 30% passed through the eye, but the important point was that the curves were shifted towards the bottom of the eye with increasing the number of fields.
    Conclusion
    The dose in the second technique was less than the other techniques. In this technique, there are five fields, and the path of the field does not go through the eye directly.

    Background
    Diabetes tends to disorders in male reproductive system. Okra (Abelmoschus esculentus) has antidiabetic and anitoxidant effects. The aim of this study was to investigate the protective effects of okra powder on testis structure and sperm parameters in diabetic rats.
    Methods
    25 rats were divided randomly into five groups including control, diabetic, diabetic/okra (200 mg/kg), diabetic/metformin (200 mg/kg), and okra (200 mg/kg). Diabetes was induced by high-fat diet fed (HFD), and injection of streptozotocin (STZ) (35 mg/kg) for four weeks. Groups were treated for 4 weeks. After treatment, blood glucose levels, testosterone levels, sperm parameters, and histomorphological structure of testes were studied.
    Findings: Induction of diabetes decreased testosterone levels, and sperm movement, numbers, and survival. Number of spermatogonia, primary spermatocyte, sertoli, and leydig cells, diameter of seminiferous tubules, and spermiogenesis index decreased significantly in diabetic group as compared to control group (P
    Conclusion
    The results suggest that okra powder supplemented diet in HFD/STZ diabetic rats improves testosterone level and sperm parameters as well as ameliorates diabetes-induced structural disorders in rat male reproductive system.

    Background
    Multiple sclerosis is a common cause of neurological disability in adults and a disease with largely unknown and complex etiology that characterizes with axonal damage and central nervous system demyelination. One of the mechanisms involved in the pathogenesis of multiple sclerosis is nerve growth factors. On the other hand, physical activity can be one of the effective mechanisms in modulation of neural growth factors in patients with multiple sclerosis. The purpose of this review was the systematic investigation of the effect of physical activity on nerve growth factors in ppatients with multiple sclerosis.
    Methods
    A systematic review was made based on Medical Subject Headings system word on the databases of Pubmed, Googlescholar, Sciencedirect and Springer in English and the databases of Magiran and (SID) in Persian. The inclusion criteria were studies published in English or Persian, during a 15-year period, only in patients with multiple sclerosis, with sport or physical activity intervention, and assessment of nerve growth factors. Methodology assessed through Physiotherapy Evidence Database scale.
    Findings: Between 161 reviewed studies, 9 that had the criteria were selected. Physiotherapy Evidence Database scale mean was 6.55. Studies had mostly assessed brain-derived neurotrophic factor (BDNF) (8 studies) and nerve growth factor (NGF) (4 studies). Most of the results indicated that physical activity lead to an increase in brain-derived neurotrophic factor but is not associated with significant changes in nerve growth factor.
    Conclusion
    Although further studies are needed, regular physical activity can be considered as an adjuvant therapy in multiple sclerosis. It can affect multiple sclerosis disease process through regulation of nerve growth factors.
