صفحات 1364-1369مقدمهکاندیدوری، یکی از شایع ترین عفونت ها در بیماران بستری در بیمارستان ها می باشد. این عارضه به دنبال کاندیدیازیس منتشر، مصرف بی رویه ی آنتی بیوتیک ها، ناهنجاری ها و مشکلات دستگاه ادراری و داشتن کاتتر ادراری به مدت طولانی ایجاد می شود و شیوع آن در زنان بیشتر از مردان است. هدف از انجام مطالعه ی حاضر، تعیین فراوانی گونه های کاندیدایی ایجاد کننده ی کاندیدوری در بیماران بستری در بخش نفرولوژی بیمارستان لبافی نژاد تهران بود.روش هااین پژوهش بر روی 53 بیمار انجام شد. شناسایی اولیه ی گونه ها با کشت بر روی محیط کروم آگار کاندیدا صورت گرفت. از کشت تازه ی مخمری، نمونه ی DNA به روش جوشاندن استخراج و نواحی ITS1-5.8s-ITS2 با استفاده از روش Polymerase chain reaction (PCR) تکثیر گردید. مراحل Restriction Fragment Length Polymorphism (RFLP) با کمک آنزیم MspI انجام شد. محصولات در ژل آگارز الکتروفورز گردید و ماهیت هر مخمر با توجه به الگوی الکتروفورتیک به دست آمده مورد شناسایی قرار گرفت.یافته هااز 53 بیمار مورد بررسی، 14 نفر مبتلا به کاندیدوری تشخیص داده شدند. میزان فراوانی کاندیدوری در مطالعه ی حاضر، 4/26 درصد گزارش گردید. 5 نفر (7/35 درصد) از بیماران را مردان و 9 نفر (3/64 درصد) را زنان تشکیل دادند. کاندیدا گلابراتا، بالاترین فراوانی (8/42 درصد) را به خود اختصاص داد. سایر گونه های جدا شده به ترتیب فراوانی شامل کاندیدا آلبیکنس و کاندیدا کروزئی هر کدام با 4/21 درصد و کاندیدا تروپیکالیس و کاندیدا پاراپسیلوزیس نیز هر کدام با 2/7 درصد بود.نتیجه گیریافزایش گونه های غیر آلبیکنس با توجه به مقاوم بودن برخی از آن ها به داروهای ضد قارچی و افزایش استفاده از داروهای سرکوبگر سیستم ایمنی، درمان این عارضه را با مشکلات بسیاری مواجه کرده است. بنابراین، شناسایی این عوامل به خصوص در بخش نفرولوژی بیمارستان ها، امری ضروری به نظر می رسد.کلیدواژگان: کاندیدیازیس، نفرولوژی، بیمارستان

صفحات 1370-1375مقدمهLegionella pneumophila، یکی از مهم ترین عوامل ایجاد عفونت های تنفسی در بیماران مبتلا به نقص سیستم ایمنی است که عوارض و مرگ و میر بالایی دارد. هدف از انجام مطالعه ی حاضر، ارزیابی اپیدمیولوژیک Legionella pneumophila در بیماران تب دار نوتروپنیک بود.روش هااین تحقیق به روش مقطعی طی سال های 95-1394 در بیمارستان سید الشهدای (ع) اصفهان انجام شد و در آن 36 بیمار تب دار نوتروپنیک مورد مطالعه قرار گرفتند. در تمام بیماران، نمونه ی ترشحات حلق و گلو با سواپ استریل جمع آوری گردید و برای تشخیص عفونت Legionella pneumophila، با روش Real-time polymerase chain reaction (Real-Time PCR) مورد آزمایش قرار گرفت و شیوع عفونت مذکور در این بیماران بررسی گردید.یافته هامیانگین سن بیماران مورد مطالعه، 6/7 ± 8/54 سال بود. 22 نفر از نمونه ها را مردان و 14 نفر آنان را زنان تشکیل دادند. لوسمی میلوئیدی حاد (Acute myeloid leukemia یا AML) با 3/33 درصد و لنفوم Non-Hodgkin با 4/19 درصد، شایع ترین بیماری های موجود بود. بر اساس نتایج به دست آمده از روش Real-Time PCR، هیچ مورد مثبتی از عفونت Legionella pneumophila مشاهده نگردید.نتیجه گیریLegionella pneumophila نقش مهمی در ایجاد پنومونی در بیماران تب دار نوتروپنیک ایفا نمی کند. بنابراین، ضمن توصیه به مطالعات بیشتر، پیشنهاد می شود از سایر روش های تشخیصی در موارد وجود پنومونی های آتیپیک استفاده گردد.کلیدواژگان: Legionella pneumophila، تب، نوتروپنی، Real، time polymerase chain reaction

صفحات 1376-1381مقدمههدف از انجام مطالعه ی حاضر، بررسی شیوع و شدت یافته های خارج قلبی تصادفی در CT angiography عروق کرونر بود.روش هادر این پژوهش، بیماران مشکوک به بیماری عروق کرونری به دنبال تزریق ماده ی حاجب، تحت Multidetector computed tomography (MDCT) قرار گرفتند. تصاویر به دست آمده از نظر یافته های خارج قلبی در چهار دسته شامل «یافته های نیازمند درمان، یافته های نیازمند بررسی، یافته های نیازمند پیگیری و یافته های بدون نیاز به اقدام» طبقه بندی شد.یافته هایافته های خارج قلبی در 30/24 درصد از بیماران وجود داشت. بدین ترتیب، یافته های نیازمند درمان 94/1 درصد، یافته های نیازمند بررسی 02/8 درصد، یافته های نیازمند پیگیری 94/10 درصد و یافته های بدون نیاز به اقدام در 40/3 درصد بیماران مشاهده گردید.نتیجه گیریشیوع یافته های خارج قلبی در تصاویر CT angiography قابل توجه بود. بنابراین، توصیه می شود جهت یافتن پاتولوژی ها، تمام فیلدهای قابل مشاهده با دقت بررسی گردد.کلیدواژگان: CT angiography، عروق کرونر، یافته های تصادفی

صفحات 1382-1388مقدمهپژوهش حاضر با هدف ارزیابی میزان و محور آستیگماتیسم قرنیه ی قدامی (Anterior corneal astigmatism یا ACA)، خلفی (Posterior corneal astigmatism یا PCA) و کل (Total corneal astigmatism یا TCA) انجام گرفت.روش هادر این مطالعه ی گذشته نگر، نقشه های توپوگرافی پنتاکم 226 چشم (113 بیمار) از افراد که آستیگماتیسم Refractive بین 6-1 دیوپتر داشتند، مورد بررسی قرار گرفت. میزان آستیگماتیسم سطح قدامی، خلفی و کل قرنیه تعیین گردید. همچنین، بزرگی و محور آستیگماتیسم این سطوح با هم مقایسه شدند. 050/0 > P به عنوان سطح معنی داری در نظر گرفته شد.یافته هامیانگین سنی جمعیت مورد مطالعه، 74/6 ± 00/33 سال بود. میانگین و انحراف معیار میزان ACA برابر 99/0 ± 02/2، PCA 20/0 ± 44/0 و TCA 01/1 ± 98/1 دیوپتر بود. آستیگماتیسم قدامی موافق قاعده و آستیگماتیسم خلفی مخالف قاعده، شیوع بیشتری نسبت به سایر جهت های محورها داشتند. در گروه های ACA و TCA، بیشترین میانگین بزرگی مربوط به آستیگماتیسم With-the-rule (WTR) بود. مقدار PCA فقط در 8 درصد افراد از 75/0 دیوپتر تجاوز می کرد. ACA با PCA (83/0 = r) و TCA (98/0 = r) ارتباط مستقیم بارزی داشت. همچنین، بین PCA و TCA رابطه ی مستقیم و شدیدی وجود داشت (78/0 = r، 001/0 > P).نتیجه گیرینتایج این مطالعه، اطلاعاتی فراهم کرد که ممکن است در دستیابی به نتایج مطلوب در اصلاح آستیگماتیسم در جراحی انکساری یا طراحی لنزهای داخل چشمی جدید مفید باشد.کلیدواژگان: آستیگماتیسم، عیوب انکساری، چشم

صفحات 1389-1395مقدمهاین مطالعه با هدف تعیین تاثیر ژل پلاسمای غنی از پلاکت بر روی زخم دیابت به منظور یافتن موارد درمان جدید، بی خطر و موثر انجام شد.روش هادر این کارآزمایی بالینی تصادفی، 50 بیمار مبتلا به زخم دیابتی به طور تصادفی به دو گروه تقسیم شدند. در گروه مورد، ژل پلاسمای غنی از پلاکت سه نوبت به فاصله ی یک هفته به صورت موضعی در لبه های زخم تزریق شد و در گروه شاهد، درمان معمول ارایه گردید. سپس، کیفیت زندگی، نمره ی درد، وسعت زخم و بروز عفونت برای همه ی بیماران در 1، 2 و 3 ماه بعد از درمان بررسی و مقایسه شد.یافته هامیانگین نمره ی کیفیت زندگی پس از یک ماه در هر دو گروه مشابه بود؛ در حالی که بعد از 2 و 3 ماه در گروه شاهد (8/2 ± 88/31)، کمتر از گروه مورد (35/1 ± 13/39) بود (001/0 > P). میانگین نمره ی درد در گروه مورد (59/0± 4/2)، پس از 1 ماه به طور معنی داری کمتر از گروه شاهد (70/0 ± 05/3) بود که بعد از 3 ماه نیز این تفاوت همچنان باقی ماند (003/0 = P). وسعت زخم (طول و عرض) پس از 2 هفته در گروه مورد بیش از گروه شاهد بهبود یافته بود.نتیجه گیریبه نظر می رسد استفاده از ژل پلاسمای غنی از پلاکت در درمان پای دیابتی، موثر باشد و احتمال می رود این ترکیب بتواند به عنوان گزینه ی درمانی علاوه بر درمان استاندارد مورد استفاده قرار گرفته و در کنترل بهتر زخم و عفونت در کوتاه مدت موثر واقع شود. در عین حال، با توجه به محدودیت های این مطالعه، از جمله پایین بودن حجم نمونه، پیشنهاد می گردد مطالعات بیشتری در این زمینه انجام گیرد.کلیدواژگان: پلاسمای غنی از پلاکت، پای دیابتی، کیفیت زندگی

صفحات 1396-1404مقدمهوضعیت ثابت هنگام انجام Endoscopic retrograde cholangiopancreatography (ERCP) برای رسیدن به مقاصد درمانی و جلوگیری از عوارض، ضروری می باشد. بنابراین، ایجاد یک آرام سازی مناسب در بیمار اهمیت فراوانی دارد. رژیم معمول شامل یک بنزودیازپین به تنهایی یا با یک نارکوتیک وریدی مانند مپریدین است. در بیماران با آرام سازی دشوار مانند بیماران معتاد، می توان از بیهوشی عمومی استفاده نمود. در بین داروهای نارکوتیک، اغلب از مپریدین استفاده می شود که علت آن اثر انقباضی کمتر بر اسفنکتر ادی (Oddi) می باشد، اما این گونه به نظر می رسد که استفاده از مورفین در بیماران معتاد می تواند بدون ایجاد مشکلات جدی در ارتباط با انقباض اسفنکتر، موجب آرام سازی عمیق تر گردد.روش هامطالعه ی حاضر از نوع کارآزمایی بالینی بود که بر روی 88 بیمار معتاد کاندید انجام ERCP انجام شد. گروه مورد علاوه بر رژیم معمول، قبل از انجام ERCP مورفین دریافت کردند و گروه شاهد تنها تحت آرام سازی با رژیم معمول قرار گرفتند. سپس عمق آرام سازی، حرکات بیمار و وضعیت اسفنکتر حین پروسیجر مورد ارزیابی قرار گرفت.یافته ها44 بیمار در گروه مورد و 40 بیمار در گروه شاهد با میانگین سنی 11/1± 52/58 سال مورد بررسی قرار گرفتند. عمق آرام سازی در بیماران گروه مورد به صورت معنی داری بیشتر بود. همچنین، اسفنکتر ادی در گروه مورد انقباض بیشتری داشت، اما تنها در یک بیمار انجام پروسیجر با مشکل مواجه شد.نتیجه گیریتجویز مورفین در بیماران معتاد کاندید انجام ERCP، می تواند بدون تداخل در روند پروسیجر، موجب آرام سازی بهتر شودکلیدواژگان: آرام سازی عمیق، وابستگی به مورفین، کلانژیوپانکراتوگرافی آندوسکوپیک رتروگراد

صفحات 1405-1411مقدمهکنترل ناکافی درد، تاثیر مهمی در سلامت مادر و نوزاد در دوره ی بعد از زایمان دارد و شیر دادن را تحت تاثیر قرار می دهد و اثر منفی بر رشد طبیعی کودک می گذارد. مطالعه ی حاضر، با هدف بررسی اثر تزریق منیزیم سولفات در محل برش جراحی درکاهش درد پس از عمل انجام شد.روش هااین مطالعه، یک کار آزمایی دو سو کور تصادفی شده بود که جامعه ی آماری آن شامل 64 خانم باردار بود که بین سال های 95-1394 در بیمارستان های شهید بهشتی و الزهرای (س) اصفهان، کاندیدای جراحی سزارین بودند. بیماران به طور تصادفی به دو گروه 32 نفری تقسیم شدند. بیماران در گروه مورد، 1 گرم منیزیم سولفات در محل برش جراحی دریافت کردند. پتیدین به عنوان مخدر کاهنده ی درد استفاده شد. بیماران پس از عمل در زمان های مختلف، از نظر میانگین مصرف مخدر، شدت درد، تهوع، بروز استفراغ، فشار خون سیستول و دیاستول، ضربان قلب، نمره ی بی حسی و رضایت مندی، با استفاده ازآزمون های Repeated measures ANOVA و Man-Whitney مورد بررسی قرار گرفتند.یافته هامتوسط میزان مصرف مخدر در بین دو گروه، تفاوت معنی داری نداشت (850/0 = P). متوسط شدت درد در دو گروه به جز درد و زمان های 12 (024/0 = P) و 24 ساعت (007/0 = P) در سایر زمان ها تفاوت معنی داری با هم نداشت (130/0 = P). همچنین، سایر پارامترها نیز در دو گروه تفاوت معنی داری نشان نداد.نتیجه گیرینتایج این مطالعه فرضیه ی اثر تزریق پره امتیو سولفات منیزیم در محل برش جراحی در کاهش درد و کاهش میزان مصرف مخدر پس از عمل سزارین را تایید نکرد.کلیدواژگان: سزارین، منیزیم سولفات، درد، پتیدین

Pages 1364-1369BackgroundCandiduria is one of the most common infections in hospitalized patients, which is created by the administration of long-term antibiotic, disseminated candidiasis, urinary tract problems, and having long-term urine catheter. Prevalence of the candiduria in women is more than men. The aim of present study was to determine the frequency of the Candida species of candiduria in hospitalized patients in nephrology department, Labbafinejad hospital, Tehran, Iran.MethodsThis study was carried out on 53 patients. The early identification of species was done using CHROMagar Candida medium. The genomic DNA was extracted using boiling method. Polymerase chain reaction (PCR) was used to amplify ITS1-5.8s-ITS2 region. The polymerase chain reaction products were then digested with MspI restriction enzyme. Polymerase chain reaction and restriction fragment length polymorphism (RFLP) products were revealed on 1.5% and 2% agarose gel electrophoresis, respectively, to identify the yeast.
Findings: Among 53 patients, 14 were diagnosed with candiduria. In our study, the frequency of candiduria was 26.4%. Separately, 5 patients (35.7%) were men and 9 (64.3%) were women. Candida glabrata (C. glabrata) had the highest frequency (42.8%). Other identified Candida species were C. albicans (21.4%), C. krusei (21.4%), C. tropicalis (7.2%), and C. parapsilosis (7.2%).ConclusionIncrease in non-albicans species, due to resistance to antifungal drugs and increased use of immunosuppressive drugs, have resulted in many problems for candiduria treatment. Therefore, identification and appropriate treatment of these elements, especially in nephrology ward of hospitals, seems to be necessary.Keywords: Candidiasis, Nephrology, Hospital -
Pages 1370-1375BackgroundLegionella pneumophila is considered as one of the most important etiologies of respiratory infection in immunocompromised patients, which causes high morbidity and mortality. The aim of this study was to evaluate the epidemiology of Legionella pneumophila in febrile neutropenic patients.MethodsThis cross-sectional study was performed on 36 patients with neutropenia admitted to Seyedoshohada hospital, Isfahan, Iran, during 2014-2015. All patients underwent throat sampling with a sterile swab. All samples were analyzed via quantitative real-time multiplex polymerase chain reaction (PCR) covering Legionella pneumophila. The prevalence of Legionella pneumophila in febrile neutropenic patients was then assessed.
Findings: Mean age of the patients (including 22 men and 14 women) was 54.8 ± 7.6 years. Most of the patients had acute myeloid leukemia (AML) (33.3%) and non-Hodgkin lymphoma (NHL) (19.4%). From 36 examined throat samples obtained from the patients with confirmed neutropenia, no positive result of Legionella pneumophila was detected via polymerase chain reaction.ConclusionOur study does not support the hypothesis that Legionella pneumophila plays an important role in the development of neutropenic fever. Thus, more studies and using additional diagnostic methods to detect Legionella pneumophila in atypical pneumonia are recommended.Keywords: Legionella pneumophila, Fever, Neutropenia, Real-time polymerase chain reaction -
Pages 1376-1381BackgroundThe aim of our study was to evaluate the prevalence of accidental extracardiac findings in multidetector computed tomography (MDCT) angiography of coronary arteries.MethodsPatients with suspected coronary artery disease were evaluated with multidetector computed tomography during injection of contrast material. Images were reviewed for extracardiac findings which were classified as therapy needed, further workup needed, follow-up needed, and no further action needed.
Findings: Extracardiac findings were detected in 24.30% of patients. The frequency of the findings classified as needed therapy, needed workup, needed follow-up, and no further action needed was 1.94%, 8.02%, 10.94%, and 3.40%, respectively.ConclusionThe prevalence of noncardiac findings on cardiac multidetector computed tomography was noticeable. To avoid missing clinically important findings, all the fields of cardiac multidetector computed tomography should be evaluated carefully.Keywords: Computed tomography angiography, Coronary vessels, Incidental findings -
Pages 1382-1388BackgroundThe aim of this study was to evaluate the magnitude and axis orientation of anterior, posterior, and total corneal astigmatism (ACA, PCA, and TCA, respectively).MethodsIn this retrospective study, pentacam topographical maps of 226 eyes (113 patients) with 1.0 to 6.0 diopters (D) of astigmatism were analyzed. Topographical anterior and posterior corneal surface astigmatism was determined. The magnitude and axis orientation of the anterior, posterior and total corneal astigmatism were compared.
Findings: The mean age of the study population was 33.00 ± 6.74 years. The mean magnitude was 2.02 ± 0.99, 0.44 ± 0.20, and 1.98 ± 1.01 diopters for anterior, posterior, and total corneal astigmatism, respectively. Most eyes had with-the-rule (WTR) anterior astigmatism and against-the-rule (ATR) posterior astigmatism. With-the-rule astigmatism had a higher mean magnitude compared to against-the-rule and oblique astigmatism in anterior and posterior corneal astigmatism groups. Posterior corneal astigmatism magnitude exceeded 0.75 diopters in only 8% of the subjects. Anterior corneal astigmatism had a significant positive correlation with posterior and total corneal astigmatism (PConclusionThe provided information of this study may be helpful in obtaining optimum results in astigmatism correction in refractive surgery or designing new intraocular lenses.Keywords: Astigmatism, Refractive error, Eye -
Pages 1389-1395BackgroundThe aim of this study was to evaluate the effects of platelet-rich plasma (PRP) gel on the diabetic wound in order to find a new safe and effective treatment method.MethodsIn this randomized clinical trial, 50 patients with diabetic wound were selected and randomly allocated into two groups. In the intervention group, 1 ml of platelet-rich plasma was administered topically at the wound edges, 3 times with one-week interval. The control group received routine treatment. Quality of life, pain score, wound extent, and incidence of infection were evaluated for all the patients within the first 3 months.
Findings: The mean score of quality of life after the first month in both groups was similar. However, the mean score of quality of life in the control group was lower than the intervention group after 2 and 3 months (31.88 ± 2.8 vs. 39.13 ± 1.35, respectively, PConclusionPlatelet-rich plasma gel seems to be effective in the treatment of diabetic foot. It is likely that this combination can be used as a treatment option in addition to standard treatment to be effective in controlling the wound and infection in short term. Pages 1396-1404BackgroundDeep sedation is essential during endoscopic retrograde cholangiopancreatography (ERCP) in order to achieve therapeutic goals and avoid complication. Therefore, providing a suitable sedation is critical for the patients. The most common sedation protocol includes an intravenous (IV) benzodiazepine along with an intravenous narcotic. For the difficult-to-sedate patients, like addicted patients, general anesthesia should be considered as an alternative. Meperidine is commonly used because of the traditional belief that other narcotic agents may be associated with contraction of Oddi. However, it seems that using morphine for addicted patients may cause deeper sedation without Oddi sphincter contraction.MethodsThis was a clinical trial study on 88 ERCP-candidate addicted patients assigned into two groups. The case group received morphine before starting ERCP and the control group only received the routine sedation regimen. The degree of sedation, patient movements, and the status of the sphincter of Oddi were evaluated during procedure time.
Findings: The data about 44 patients of the case and 40 patients of control groups were analyzed. The mean age was 58.52 ± 1.11 years. Evaluating the degree of sedation every 15 minutes during ERCP showed significant differences between the case and control group. In case group, the degree of sedation was significantly deeper. Furthermore, Oddi sphincter showed a significantly increased contraction in case group. However, only in one patient procedure failed.ConclusionAdministration of morphine in addicted patients can make deeper sedation during ERCP, without any significant problem related to contraction of the sphincter of Oddi.Keywords: Deep sedation, Morphine dependence, Endoscopic retrograde cholangiopancreatography -
Pages 1405-1411BackgroundInadequate pain control has a significant role in maternal and neonatal health in the early postpartum period. It interferes with breastfeeding and has a negative influence on child normal growth. This study aimed to evaluate the preemptive effect of intraincisional infiltration magnesium sulfate on postoperative pain control.MethodsThis was a double-blind randomized prospective clinical trial study involving 64 pregnant women underwent cesarean section, from 2015 to 2016, in Shahid Beheshti and Alzahra hospitals, Isfahan, Iran. Patients were randomly assigned to two equal groups. Patients in the case group received 1 mg of magnesium sulfate in the incisional site. Pethidine was a pain reliever in the follow-up examination. The obtained data, including mean pain intensity, nausea and vomiting, pethidine consumption, blood pressure, heart rate, sedation score, and patient satisfaction were collected and analyzed using repeated measures ANOVA and Man-Whitney tests.
Findings: Mean pethidine consumption was not statistically different between two groups (P = 0.850). Mean pain intensity was significantly different only 12 (P = 0.024) and 24 (P = 0.007) hours after the surgery and it was not significant at other time points (P = 0.130). Other parameters were also not statistically significant.ConclusionIt seems that preemptive intraincisional infiltration of magnesium sulfate could not have a significant effect on postoperative pain and reduction of pethidine use in cesarean section.Keywords: Cesarean section, Magnesium sulfate, Pain, Pethidine
