    مقدمه
    با توجه به جایگاه ازدواج در زندگی و اثر آن بر شیوه ی زندگی و سلامت عمومی، هدف از انجام این مطالعه، بررسی ارتباط وضعیت تاهل و سلامت عمومی در دانشجویان دانشگاه علوم پزشکی اصفهان بود.
    روش ها
    در این مطالعه ی مقطعی، 330 نفر از دانشجویان سال چهارم 9 رشته های مختلف علوم پزشکی در سال 1395 شرکت کردند. دانشجویان بر اساس نمونه گیری تصادفی انتخاب شدند و اطلاعات دموگرافیک آن ها پس از دریافت رضایت برای شرکت در مطالعه ثبت شد. وضعیت اقتصادی افراد و وضعیت سلامت روان با استفاده از پرسش نامه ی General health questionnaire-28 (GHQ-28) برای همه ی رشته ها ثبت گردید و با استفاده از نرم افزار SPSS واکاوی شد.
    یافته ها
    بیشتر افراد متاهل در این مطالعه، مونث بودند (001/0 > P) و بین افراد مجرد و متاهل، اختلاف معنی داری بر اساس رشته ی تحصیلی وجود داشت (030/0 = P) ، اما بین دو گروه اختلاف معنی داری بر اساس وضعیت اقتصادی- اجتماعی وجود نداشت (060/0 = P). بر اساس پرسش نامه ی GHQ-28، نمره ی اختلال در کارکرد اجتماعی در افراد مجرد به صورت معنی داری بالاتر بود (006/0 = P). همچنین، اختلاف معنی داری بین رشته های مختلف بر اساس نشانه های جسمانی، اضطراب و بی خوابی، افسردگی و اختلال در کارکرد اجتماعی وجود داشت (001/0 > P).
    نتیجه گیری
    دانشجویان متاهل رشته های علوم پزشکی در مقایسه با افراد مجرد، کارکرد اجتماعی بهتری داشتند و در سایر عوامل مربوط به سلامت روان نتایج مشابهی داشتند. از طرفی، سلامت روان در برخی رشته های پزشکی، دندان پزشکی، داروسازی و پرستاری و مامایی، مناسب نبود و باید سیاست های لازم جهت بهبود سلامت روان در دانشجویان این رشته ها اتخاذ گردد.

    مقدمه
    هدف از انجام این مطالعه، بررسی تاثیر استفاده از نرم افزار شبیه سازی جراحی آپاندکتومی (مبتنی بر بازی) بر میزان خودکارآمدی دانشجویان کارشناسی اتاق عمل بود.
    روش ها
    در این مطالعه ی نیمه تجربی یک سو کور، 70 دانشجوی کارشناسی اتاق عمل ترم 2 در شهر اصفهان به صورت نمونه گیری در دسترس انتخاب شدند و بعد از کسب رضایت در دو گروه مورد و شاهد قرار گرفتند. بعد از استفاده از نرم افزار شبیه سازی (مبتنی بر بازی) برای گروه مورد و برگزاری کلاس و سخنرانی برای گروه شاهد، داده ها با پرسش نامه ی محقق ساخته ی خود کارآمدی که روایی و پایایی آن تایید شده بود، جمع آوری گردید. سپس، داده ها با استفاده از نرم افزار SPSS و آزمون های توصیفی و تحلیلی با سطح معنی داری 05/0 > P مورد تجزیه و تحلیل قرار گرفتند.
    یافته ها
    متوسط نمره ی خود کارآمدی در گروه مورد (48/1 ± 97/62) از گروه شاهد (67/2 ± 63/48) بالاتر بود که این تفاوت، از نظر آماری معنی دار بود (05/0 > P). همچنین، نتایج نشان داد که بین مشخصات دموگرافیک با خودکارآمدی ارتباط معنی داری وجود نداشت (05/0 < P).
    نتیجه گیری
    نرم افزار شبیه سازی جراحی آپاندکتومی، می تواند منجر به ارتقای خود کارآمدی دانشجویان شود. از این رو، به کارگیری آن به عنوان یک روش کمک آموزشی برای آموزش دانشجویان اتاق عمل پیشنهاد می شود.

    مقدمه
    کارگذاری و برداشت کاتتر پورت اغلب با بی حسی موضعی و آرام بخشی انجام می شود. با توجه به مدت زمان انجام این جراحی، لازم است از داروها یا ترکیبی از داروها استفاده نمود که دارای شروع اثر و بازگشت هوشیاری سریع باشد و از آرام بخشی و ضد دردی کافی برخوردار باشد و بتواند عملکرد قلبی- عروقی و تنفسی، فراموشی و بی تحرکی مناسبی را ایجاد کنند.
    روش ها
    68 بیمار کاندیدای تعبیه یا خروج کاتتر به روش تخصیص تصادفی در دو گروه 34 نفره توزیع و وارد مطالعه شدند و در دو گروه دریافت کننده ی پرپووفول- کتامین (کتوفول) و پروپوفول- فنتانیل (فنوفول) تقسیم شدند. بیهوشی در گروه کتوفول با تزریق 1 میلی گرم/کیلوگرم پروپوفول و 5/0 میلی گرم/کیلوگرم کتامین و در گروه فنوفول با تزریق 1 میلی گرم/کیلوگرم پروپوفول و 5/1 میکروگرم/کیلوگرم فنتانیل صورت گرفت. در حین انجام فرایند، ارزیابی عمق آرام بخشی با معیار Ramsay و شدت درد بر اساس معیار دیداری سنجش درد (Visual analog scale یا VAS) صورت گرفت. بیماران از نظر تغییرات همودینامیک قبل، حین و پس از جراحی پایش شدند.
    یافته ها
    سطح آرام بخشی (94/0 = P) و درد (58/0 = P) و همچنین، تغییرات همودینامیک (05/0 < P برای همه) در گروه کتوفول بهتر، مدت زمان جراحی (04/0 = P) و ریکاوری (12/0 = P) در این گروه بیشتر و میزان حرکت حین عمل (54/0 = P) و نیاز به دز مجدد دارو (01/0 = P) در این گروه کمتر بود.
    نتیجه گیری
    تجویز کتوفول در بیماران تحت شیمی درمانی به لحاظ آرام بخشی و بی دردی توصیه می گردد.

    مقدمه
    بافت پوست، همواره در اثر عواملی مانند سوختگی، آسیب های فیزیکی و بیماری هایی مانند دیابت دچار آسیب می شود. عفونت و عدم التیام زخم های ناشی از سوختگی و عدم رگ زایی در زخم های دیابت، منجر به آسیب های وسیع و مرگ افراد می شود. از این رو، طراحی پانسمان های دارای قابلیت رهایش کنترل شده ی دارو در محل زخم، اهمیت به سزایی دارد.
    روش ها
    در ابتدا، پلیمر پلی گلیسرول سباکیت [Poly (glycerol sebacate) یا PGS] تهیه گردید و جهت الکتروریسی هم محور به وسیله ی پلی هیدروکسی بوتیرات (Polyhydroxybutyrate یا PHB) غلاف شد و داروی ضد التهاب و آنتی باکتریال به صورت مجزا بارگذاری گردید. ابتدا، جهت بررسی ویژگی های ریخت شناسی از میکروسکوپ الکترونی (Scanning electron microscope یا SEM) استفاده شد و سپس، به منظور ارزیابی های بیولوژیک پوشش زخم، از آزمون های بررسی خواص آنتی باکتریال زخم پوش، روند تکثیر و چسبندگی سلول های پوستی استفاده شد.
    یافته ها
    نتایج SEM نشان داد که الیاف زخم پوش یکنواخت و با قطری برابر با 6/40 ± 0/575 نانومتر است. در بررسی خاصیت آنتی باکتریال و سلول سازگاری زخم پوش مورد نظر مشاهده شد که وجود دارو در سامانه، علاوه بر این که خواص آنتی باکتریال مناسبی دارد؛ بلکه هیچ گونه اثر نامطلوبی بر سلول ها ندارد و تکثیر، رشد و چسبندگی سلول فیبروبلاست پوستی بر روی سطح نمونه مشاهده نشده است.
    نتیجه گیری
    با استفاده از روش الکتروریسی هم محور و قرار گرفتن دو پلیمر کنار هم و بارگذاری هم زمان و رهایش کنترل شده ی دو دارو، می توان به پوشش زخم با خواص ایده آلی با خواص آنتی باکتریال و سلول سازگاری دست یافت.

    مقدمه
    تمرینات بدنی، از راه تعدیل آنژیوژنز در بافت تومور نقش حمایتی را در درمان سرطان پروستات ایفا می کنند. عامل القا شونده به وسیله ی هیپوکسی 1 (Hypoxia-inducible factor-1 یا HIF-1) ، نقش مهمی در روند آنژیوژنز و رشد تومور دارد. آنتی ژن اختصاصی پروستات (Prostate-specific antigen یا PSA) ، قابل دسترس ترین نشانگر تومور برای تشخیص سریع سرطان پروستات است. هدف از انجام پژوهش حاضر، بررسی اثر تمرینات بدنی بر مقادیر پلاسمایی و بیان ژن HIF-1 و سطح سرمی PSA مردان مبتلا به سرطان پروستات بود.
    روش ها
    در این مطالعه ی کارآزمایی بالینی، با مراجعه به بخش پاتولوژی بیمارستان فوق تخصصی بقیه اله الاعظم (عج) تهران و بررسی مدارک پزشکی مردان مبتلا به سرطان پروستات در 6 ماه اول سال 1395، تعداد 20 مرد مبتلا به سرطان پروستات به طور تصادفی انتخاب و به دو گروه مساوی مورد (با سن 04/10 ± 7/63 سال، وزن 26/11 ± 00/78 کیلوگرم و شاخص توده ی بدنی 44/2 ± 27/28 کیلوگرم/مترمربع) و شاهد (با سن 69/4 ± 5/61 سال، وزن 26/11 ± 20/79 کیلوگرم و شاخص توده ی بدنی 83/2 ± 93/27 کیلوگرم/مترمربع) تقسیم شدند. گروه مورد، به مدت 8 هفته و هر هفته 3 جلسه ی60 دقیقه ای، تمرین ترکیبی مقاومتی و استقامتی در زمان برابر با شدت 75-60 درصد یک تکرار بیشینه و 75-60 درصد بیشینه ی ضربان قلب انجام دادند. گروه شاهد، هیچ مداخله ای دریافت نکردند. نمونه های خونی جهت بررسی PSA و HIF-1 به روش Enzyme-linked immunosorbent assay (ELISA) و Real-time polymerase chain reaction (Real-time PCR) و با آزمون آماری Dependent t و Independent t سنجیده شد.
    یافته ها
    تمرین ترکیبی، سبب کاهش معنی دار مقادیر پلاسمایی (007/0 = P) و بیان ژن HIF-1 (005/0 = P) و سطح سرمی PSA (005/0 = P) در گروه مورد شد.
    نتیجه گیری
    با توجه به اهمیت PSA در تشخیص سرطان پروستات و نقش HIF-1 در آنژیوژنز تومور، تمرین ترکیبی می تواند از راه تعدیل HIF-1 و PSA، نقش موثری در کاهش روند پیشرفت سرطان پروستات داشته باشد.

    Background
    Considering the role of marriage in life and its effect on lifestyle and general health, the purpose of this study was to determine the relationship between marital status and general health among the students of Isfahan University of Medical Sciences, Isfahan, Iran.
    Methods
    In this cross-sectional study, 330 students from the fourth year in 9 medical sciences fields of study in years 2015-2016 were participated. Students were selected on the basis of random sampling, and their demographic information was recorded after obtaining consent for participation in the study. Economic status of individuals and their mental health status [by General health questionnaire-28 (GHQ-28)] were evaluated, and the data were analyzed using SPSS software.
    Findings
    Most of the married participants were women in this study (P < 0.001). There was a significant difference between single and married subjects based on the field of study (P = 0.030). But there was no significant difference between the two groups based on socioeconomic status (P = 0.060). According to the GHQ28 questionnaire, the score of disorder in social function was significantly higher in single subjects (P = 0.006). There was also a significant difference between different fields of study based in physical symptoms, anxiety and insomnia, depression and social function disorder (P < 0.001).
    Conclusion
    Married students of medical sciences had better social function than single students, and had similar results in other mental health-related factors. On the other hand, mental health in some fields of medicine, dentistry, pharmacy, and nursing and midwifery was not appropriate, and policies should be developed to improve the mental health status of students in these fields.

    Background
    The purpose of this study was to investigate the effect of Appendectomy Surgical Simulation software (game-based) on the self-efficacy of undergraduate students in the operating room.
    Methods
    In this single-blind quasi-experimental study, 70 second semester undergraduate students in Isfahan City, Iran, were selected using convenience sampling method. After obtaining satisfaction, they were assigned to intervention and control groups randomly. Simulation software (game-based) and the holding practice and lecture class (routine method) were used in intervention and control groups, respectively. Then, the data were collected using a researcher-administered questionnaire of self-efficacy which validity and reliability were confirmed. Data were then analyzed using SPSS software and descriptive and analytical tests with significance level of less than 0.05.
    Findings
    The average score of self-efficacy in the intervention group (62.97 ± 1.48) was higher than the control group (48.63 ± 2.67), and the difference was statistically significant (P < 0.05). Moreover, there were no significant relationship between demographic characteristics and self-efficacy (P > 0.05).
    Conclusion
    Appendectomy Surgery Simulation software can promote students' self-efficacy. Therefore, it is recommended to be used as a supplementary training method for teaching the operating room students.

    Background
    Placement and removal of port catheter is often done by local anesthesia and sedation. Due to the duration of this surgery, it is necessary to use drugs or drugs combination. These drugs should have two distinct features, firstly, the onset of the effect and the return of alertness should be fast, and secondary it should have adequate sedation and analgesia, and can provide proper cardiovascular and respiratory function, amnesia, and inactivity.
    Methods
    Using convenience sampling method, and considering inclusion and exclusion criteria, 68 patients candidate for placement and removal of port catheter were entered randomly into two groups receiving propofol-ketamine (ketofol) and propofol-fentanyl (fenofol). Anesthetics induction was done in ketofol group with propofol (1 mg/kg) and ketamine (0.5 mg/kg), and in fenofol group with propofol (1 mg/kg) plus fentanyl(1.5 μg/kg). Then, the procedure was performed by the surgeon. During the procedure, depth of sedation was assessed using Ramsay scale, as well as pain intensity using visual analogue scale (VAS). Patients were monitored for hemodynamic changes before, during, and after the surgery.
    Findings
    Sedation (P = 0.94) and analgesia (P = 0.58), as well as hemodynamic changes (P > 0.05 for all) were reported better in ketofol group. The duration of surgery (P = 0.04) and recovery (P = 0.12) in the ketofol group was upper, but the amount of motion during operation (P = 0.54) and the need for a re-dose of drug (P = 0.01) was lower in this group.
    Conclusion
    The drug combination of propofol-ketamine is suggested to be used in patients under chemotherapy

    Background
    The skin is always damaged by burns, physical damages, and diseases such as diabetes mellitus. Infection and acute inflammation of wounds caused by burns and diabetes ulcers lead to extensive damage and death. Therefore, it is necessary the design polymeric dressings containing drugs with controlling ability to release the drugs in wound site.
    Methods
    Wound dressing of poly(glycerol sebacate) (PGS)/ polyhydroxybutyrate (PHB) was fabricated by coaxial electrospinning process. These fibers were characterized by scanning electron microscope (SEM) images. The anti-bacterial activity, cell viability (MTT) and cell adhesion were investigated.
    Findings
    The results of SEM showed that the wound fibers were uniform, and with a diameter of 575 ± 40.6 nm. The drugs loaded were separately in the core and shell of fibers. The samples containing drugs showed good bactericidal activity against both of Gram-positive and Gram-negative bacteria. Result of MTT did not show viability, attachment, and growth of cells in wound dressing with drugs.
    Conclusion
    It can be concluded that this wound dressing with the release of two drugs simultaneously is an ideal wound dressing to control the infection, and heal the wound.

    Background
    Physical exercises play a supportive role in the treatment of prostate cancer by modulating angiogenesis in tumor tissue. The hypoxia-inducible factor-1 (HIF-1) modulates angiogenesis and tumor growth. Prostate specific antigen (PSA) is the most widely available tumor marker for the rapid diagnosis of prostate cancer. The purpose of this study was to investigate the effect of physical exercises in the expression of HIF-1 gene levels in plasma, and the PSA level in the sera of men with prostate cancer.
    Methods
    In this clinical trial study, 20 men with prostate cancer were randomly selected from the pathology department of Baqiyatallah al-Azam subspecialty hospital in Tehran, Iran, during the middle 6 months of 2016. Patients were divided into two groups of experimental [age: 63 ± 10 years, weight: 78 ± 11 kg, body mass index (BMI): 28.5 ± 2.4 kg/m2] and control [age = 62 ± 4 years, weight: 79 ± 11 kg, BMI: 27.9 ± 2.9 kg/m2). The experimental group performed 8 weeks of 60 minutes per session training including 3 sessions per week. The workouts were the combination of exercises with resistance and endurance training per session as 60-75 percent of one repetition maximum, and 60-75 percent of maximum heart rate. The control group did not receive any intervention. Blood samples were tested for PSA and HIF-1 using enzyme-linked immunosorbent assay (ELISA) and real-time polymerase chain reaction (PCR) methods, respectively, and the data were analyzed using dependent and independent t test.
    Findings
    Combined exercise significantly decreased the plasma level (P = 0.007) and gene expression (P = 0.005) of HIF-1, as well as serum PSA level (P = 0.005) in the experimental group compared with control group.
    Conclusion
    Considering the importance of PSA in the diagnosis of prostate cancer, and the role of HIF-1 in tumor angiogenesis, the combined exercise can be effective in reducing the progression of prostate cancer by modifying HIF-1 and PSA.
