    مقدمه
    سزارین از آن دسته اعمال جراحی است که نقش مهمی در کاهش بروز برخی عوارض در مادر و جنین در زایمان های خاص دارد. اما باید توجه داشت که عوارض عفونی پس از آن از علل اساسی ناخوشی های مادر هستند و مصرف آنتی بیوتیک می تواند در کاهش این عوارض موثر باشد. لذا مطالعه حاضر برای تعیین مدت تجویز آنتی بیوتیک برای کاهش عوارض عفونی در عمل های جراحی سزارین انجام شد.
    روش کار
    این مطالعه کارآزمایی بالینی مورد شاهدی تصادفی شده در سال 94-1393 بر روی 204 نفر از بیماران داوطلب عمل جراحی سزارین غیر اورژانس در بیمارستان شهدای خلیج فارس بوشهر انجام شد. افراد به صورت تصادفی به دو گروه آزمون (دریافت کنندگان تک دوز آنتی بیوتیک پروفیلاکتیک پس از کلامپ بند ناف) و کنترل (دریافت کنندگان تک دوزآنتی بیوتیک همراه با آنتی بیوتیک 7 روزه پس از عمل) تقسیم شدند. بیماران در طی 48 ساعت بستری پس از سزارین از لحاظ علائم عفونت شامل تب، لرز، سلولیت محل برش جراحی، ترشحات محل برش جراحی و حساسیت رحمی مورد بررسی قرار گرفتند و همچنین یک هفته پس از ترخیص نیز از طریق مراجعه به درمانگاه یا بیمارستان پیگیری شدند. نوزادان آن ها نیز در طی مدت بستری مادر و هم چنین پس از مراجعه مجدد با مادر از نظر عوارض نوزادی از جمله اسهال، راش و کاندیدا دهانی نیز بررسی شدند. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS (نسخه 18) و آزمون های تی مستقل، کای دو و تست دقیق فیشر انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد.
    یافته ها
    بین گروه آزمون و کنترل تفاوت معناداری از نظر بروز عوارض مادری شامل تجزیه کامل ادرار (118/0=p)، کشت ادرار (180/0=p)، متریت (622/0=p) و عفونت محل زخم جراحی (076/0=p) وجود نداشت. بروز عوارض نوزادی شامل اسهال (621/0=p)، راش (1=p) و کاندیدای دهانی (1=p) نیز بین دو گروه تفاوت معناداری نداشت.
    نتیجه گیری
    در عمل های سزارین غیر اورژانس می توان از رژیم تک دوز آنتی بیوتیک به جای رژیم 7 روزه جهت کاهش میزان عفونت ها استفاده کرد.

    مقدمه
    ارزیابی علائم حیاتی در تعیین وضعیت سلامت بیماران بعد از عمل جراحی ضروری است. تغییر و دگرگونی در علائم حیاتی بیمار می تواند نشانه پاسخ بدن به استرس های فیزیکی و روانی یا تغییر در عملکرد فیزیولوژیک بدن باشد. ماساژ درمانی به بهبود سیستم عصبی و قلبی عروقی کمک کرده و باعث ایجاد احساس خوب بودن، راحتی و آرامش بیمار می شود. لذا مطالعه حاضر با هدف تعیین تاثیر ماساژ بر علائم حیاتی بیماران تحت جراحی سزارین انجام شد.
    روش کار
    این مطالعه کارآزمایی بالینی در سال 1392 بر روی 80 زن مراجعه کننده به بیمارستان ام البنین (س) شهر مشهد جهت انجام سزارین انجام گرفت. افراد به طور تصادفی به دو گروه مداخله و کنترل تقسیم شدند. مداخله مورد نظر که ماساژ دست و پا بود، توسط پژوهشگر به مدت 20 دقیقه بر روی دست و پاهای زنان بعد از عمل سزارین هر کدام به مدت 5 دقیقه در بخش جراحی زنان انجام گرفت. علائم حیاتی زنان با استفاده از فشارسنج جیوه ای و شمارش نبض و تنفس، پیش و بلافاصله، 60 و 90 دقیقه بعد از مداخله اندازه گیری شد. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS (نسخه 16) و آزمون های کای اسکوئر، من ویتنی، تحلیل واریانس با اندازه گیری های مکرر و آزمون تی زوجی انجام شد. میزان p کمتر از 05/0 معنی دار در نظر گرفته شد.
    یافته ها
    ماساژ دست و پا باعث بهبود معنی دار علائم حیاتی شد؛ به طوری که میانگین تعداد تنفس (03/0=p)، نبض (003/0=p) و فشارخون (02/0=p)، قبل و بعد از ماساژ (بلافاصله، 60 و 90 دقیقه ) اختلاف آماری معنی داری را نشان داد.
    نتیجه گیری
    ماساژ دست و پا باعث بهبود علائم حیاتی زنان بعد از عمل سزارین می شود که می تواند حاکی از ایجاد آرامش و کاهش استرس در آنان باشد.

    مقدمه
    شناسایی شیوع و عوامل خطر عفونت هپاتیت B (HBV) در زنان باردار به عنوان یک منبع اصلی عفونت در برنامه های پیشگیری HBV بسیار مهم است. لذا مطالعه مروری حاضر با هدف بررسی شیوع و عوامل خطر HBV در زنان باردار ایران انجام شد.
    روش کار
    مطالعه حاضر بر اساس چک لیست PRISMA برای مطالعات مرور سیستماتیک و متاآنالیز انجام شد. جهت دستیابی به مستندات فارسی و انگلیسی مرتبط، پایگاه های الکترونیکی Scopus، PubMed، Science Direct، Cochrane، Web of Science (ISI)، Springer، Online Library Wiley،Magiran ، Iranmedex، SID، Medlib، IranDoc و هم چنین موتور جستجوی Google Scholar با استفاده از کلیدواژه های استاندارد شامل: شیوع، هپاتیت، زنان باردار، بارداری، آنتی ژن سطحی هپاتیت B، عوامل خطر، HBsAg و HBeAg بدون محدودیت زمانی تا بهمن ماه 1394 توسط دو پژوهشگران به صورت مستقل جستجو شدند. داده ها با استفاده از مدل اثرات ثابت و نرم افزار STATA (نسخه 1/11) مورد تجزیه و تحلیل آماری قرار گرفتند.
    یافته ها
    در بررسی 36 مطالعه واجد شرایط با حجم نمونه 64193 زن باردار در ایران،شیوع HBV 2/0% محاسبه شد. کمترین و بیشترین شیوع HBV مرتبط با شمال (1/0%) و شرق (3/0%) ایران بود. شیوع HBV در زنان باردار شهری و روستایی به ترتیب 6/0 و 3/0% برآورد شد. شیوع HBeAg، HBeAb و HBsAb در زنان باردار ایرانی به ترتیب 05/0، 05/0 و 3/44 محاسبه شد. از میان عوامل خطر، بی سوادی، سقط جنین، سابقه تزریق خون و اعتیاد همسر با ابتلاء به HBV از نظر آماری معنی دار بود (05/0>p)، اما این ارتباط با شهرنشینی، شغل، سابقه جراحی و خالکوبی معنی دار نبود (05/0
    نتیجه گیری
    کمترین شیوع هپاتیت B در ایران، در گروه زنان باردار می باشد و کمتر از جمعیت عمومی ایران است. سابقه تزریق خون، اعتیاد همسر، بی سوادی و سقط جنین با ابتلاء به HBV در زنان باردار ایران در ارتباط است.

    Introduction
    Cesarean section is a surgery which has an important role in decreasing maternal and fetal morbidity in some special deliveries. But, it should be noted that infection due to caesarean section is one of the major cause of maternal morbidity and antibiotics are effective in prophylaxis of infection. This study was performed with aim to determine the necessary duration of antibiotic therapy to reduce infections in cesarean section.
    Methods
    This randomized clinical trial, case-control study was performed on 204 women who were candidate for non-emergency cesarean section in Shohadaye Khalije Fars, Bushehr from 2014 to 2015. They were randomly divided to intervention (received single dose of prophylactic antibiotic after clamping umbilical cord) and control (received single dose of prophylactic antibiotic plus seven days of antibiotic after surgery) groups. They were assessed during 48 h after hospitalization in terms of fever, chills, cellulitis, discharge from incision site, uterine tenderness and they were also followed one week after discharge by referring to the clinic or hospital. Their infants were also evaluated during mother's hospitalization and also when referring by mother in terms of neonatal outcomes such as diarrhea, rash, and oral candidiasis. Data analysis was performed by SPSS software (version 18) and independent t-test, Chi-square and Fisher's exact test.
    Results
    There was no significant difference between the intervention and control groups in term of maternal complications including urinary tract infection by urine analysis (P=0.118), urine culture (P=0.180), metritis (P=0.622) and surgical site infection (P=0.076). Neonatal complications including diarrhea (P=0.621), rash (P=1).and oral candidiasis (P=1) has no significant difference between intervention and control groups.
    Conclusion
    Single dose antibiotic can be used instead of seven day regimen of antibiotics to reduce the infection in non-emergency cesarean sections.

    Introduction
    Assessment of vital signs is essential in determining the patient's health status. Change in vital signs can be the symptoms of the body's response to physical and psychological stress or changes in physiological functioning of the body. The massage-therapy helps to the improvement of nervous and car­diovascular system and cause a feeling of well­being, relaxation and comfort in the patient. Therefore, this study was performed with aim to determine the effect of massage on vital signs of women after cesarean section.
    Methods
    This clinical trial was performed on 80 women referred to Mashhad Omolbanin hospital in 2013. The participants were randomly divided into two intervention and control groups. The intervention was hand and foot massage which was conducted by researcher for 20 minutes on the hands and feet of women after cesarean section each for 5 minutes in the women's surgical ward. Then their vital signs were measured using a mercury sphygmomanometer and counting pulse and respiration were measured before and immediately, 60 and 90 minutes after the intervention. Data were analyzed using SPSS software (version 16) and Mann-Whitney, Chi-square, analysis of variance with repeated measures, and Paired t-test.
    Results
    Hand and foot massage caused significant decrease in vital signs, so that mean number of breathing (P=0.03), pulse (P =0.003) and blood pressure (P =0.02) showed significant difference before and after massage (immediately, 60 and 90 minutes).
    Conclusion
    Hand and foot massage improves the womenʹ vital signs after caesarean section that can indicate relaxation and reducing stress on them.

    Introduction
    Identification of prevalence and risk factors of hepatitis B (HBV) in pregnant women is very important as a major source of infection in HBV prevention program. Therefore, this review study was performed with aim to determine the prevalence and risk factors of hepatitis B (HBV) infection in pregnant women of Iran.
    Methods
    Current study was conducted based on PRISMA checklist for systematic review and meta-analysis studies. To access to the English and Persian documents, two independent authors searched Scopus, PubMed, ScienceDirect, Cochrane, Web of Science (ISI), Springer, Online Library Wiley, Magiran, Iranmedex, SID, Medlib, IranDoc and also Google Scholar search engine without any time limitation to January 2016 by using mesh keywords including: Prevalence, Hepatitis, Pregnant women, Pregnancy, Risk Factors, Hepatitis B Surface Antigens, HBsAg, and HBeAg. Data was analyzed using the fixed-effects model via Stata software (Version 11.1).
    Results
    A total of 36 eligible studies with sample size of 64,193 pregnant women in Iran, HBV prevalence was estimated 0.5%. Minimum and maximum of this range were related to the North (0.4%) and East (1.6%) of the Iran. HBV prevalence in urban and rural pregnant women was estimated 1.1 and 1.2%, respectively. The prevalence of HBeAg, HBeAb, HBsAb in pregnant women of Iran were calculated 0.05, 0.05 and 40%, respectively. Among the risk factors, illiteracy, abortion, history of blood transfusion and husband addiction were significant related with prevalence of HBV (P0.05).
    Conclusion
    The lowest prevalence of HBV in Iran was related to pregnant women and less than the general population. History of blood transfusions, husband addiction, illiteracy and abortion are associated with HBV in Iranian pregnant women.
