صفحات 417-424مقدمهبلع باتری های دیسکی که یکی از عوارض ورود تکنولوژی های نوین به اسباب بازی ها است که می تواند با خطرات قابل توجهی همراه باشد. هدف از این مطالعه ارزیابی علایم بالینی، روش های درمانی انجام شده و پیامدهای نهایی در این کودکان می باشد.روش کاردر یک مطالعه توصیفی بررسی بیماران کودکانی که به علت بلع باتری های دیسکی طی سالهای 1386-1391 به بیمارستان مرکز طبی کودکان تهران ارجاع شده بودند توسط رادیوگرافی و در صورت لزوم انجام آندوسکوپی بررسی شده و طی یکسال از نظر بروز عوارض مورد پیگیری قرار گرفتند. اطلاعات با نرم افزار SPSS و ازمون کای دو بررسی شد.نتایجدر این مطالعه مجموعا 89 کودک (56 پسر) با میانگین سنی 50/2±93/2 سال مورد بررسی قرار گرفتند. 4 /67% بیماران در بدو پذیرش فاقد علایم بالینی بودند. شایعترین علامت بیماران در هنگام پذیرش استفراغ (8/25 %) بود. در 22 مورد باتری در مری گیر کرده بود که در تمامی موارد تحت بررسی آندوسکوپی قرار گرفتند که در 8 /81% باتری با انجام آندوسکوپی خارج شده، و 4 بیمار نیاز به جراحی پیدا کردند. 12 مورد (19%) از بیمارانی که باتری دیسکی در معده دیده شد نیز، با انجام آندوسکوپی خارج شد و در سایر موراد تنها با پیگیری، به صورت خودبخودی از دستگاه گوارش دفع گردید. شایعترین عارضه تنگی دیررس مری بود که در 6 /14 % از بیماران دیده شد.نتیجه گیریهرچند بلع باتری های دیسکی در اغلب موارد سبب بروز عوارض جدی در بیمار نمی شود، بدلیل خطرات بالقوه بخصوص در مواردی که باتری در مری گیر کرده است مداخله اورژانس به وسیله آندوسکوپی ضروری خواهد بود.

صفحات 425-431مقدمهعفونت هلیکوباکتر پیلوری (HP) یک عفونت شایع در سراسر جهان است. از عوارض خارج معده ای مرتبط با این عفونت، نقش آن در دیابت و مقاومت به انسولین (IR) می باشد. هدف این مطالعه بررسی تاثیر درمان ریشه کنی HP بر مقاومت به انسولین در بیماران مبتلا به کبد چرب غیر الکلی (NAFLD) است.روش کاراین مطالعه کارازمایی بالینی در بیمارستان امام رضا مشهد در سال 1393 انجام شد. 51 بیمار مبتلا به NAFLD و دیس پپسی و عفونت HP در این مطالعه وارد شدند. بیماران به دو گروه (گروه مداخله و گروه کنترل) تقسیم شدند. گروه کنترل صرفا تحت تغییرات سبک زندگی و گروه مداخله علاوه بر تغییر life style تحت درمان ریشه کنی HP قرار گرفتند.
قبل و بعد از درمان ریشه کنی HP در هر دو گروه FBS، Chol، TG، LDL، HDL، insulin، HOMA-IR و BMI بررسی شد. اطلاعات به دست آمده با آزمون های تی، ویلکاکسون، من ویتنی تجزیه و تحلیل شد.نتایجدر هیچکدام از پارامترهایی که در این مطالعه مورد بررسی قرار گرفت، از جمله FBS، پروفایل لیپید، insulin، HOMA-IR و BMI تفاوت معنی داری قبل و بعد از درمان ریشه کنی HP در بین دو گروه مشاهده نگردید. همچنین هیچ یک از متغیرها در هر گروه بعد از درمان نسبت به قبل از درمان تفاوت معنی دار نداشتند.نتیجه گیریبه نظر می رسد ریشه کنی HP در بیماران NAFLD به خودی خود هیچ اثری بر FBS، پروفایل لیپید و مقاومت به انسولین ندارد. گرچه مطالعات بیشتر در این زمینه مورد نیاز است.

صفحات 432-441مقدمهکف پای صاف از جمله تغییراتی به شمار می رود که ممکن است باعث تغییر در الگوی فعال شدن عضلات گردد. درک تغییر فعالیت عضلات در موقعیت های مختلف ایستادن افراد دارای کف پای صاف، بینش خوبی برای پیش گیری از بروز آسیب های مختلف اندام تحتانی در این افراد خواهد داد، لذا هدف از تحقیق حاضر، مقایسه الکترومایوگرافی عضلات ساق پا در موقعیت های مختلف ایستادن مردان دارای کف پای صاف و طبیعی است.روش کاراین مطالعه توصیفی در دانشگاه خوارزمی تهران در سال 91-92 انجام شده است. 40 نفر دانشجوی مرد سالم، شامل 20 نفر دارای کف پای صاف و 20 نفر دارای کف پای طبیعی در این تحقیق نیمه تجربی شرکت کردند. ناهنجاری کف پا با استفاده از تست افت ناوی اندازه گیری شد. هر آزمودنی در پنج موقعیت ایستادن قرار گرفت و فعالیت عضلات با دستگاه الکترومایوگرافی ثبت گردید. برای تجزیه تحلیل داده ها از نرم افزارMatlab و SPSS و برای مقایسه متغیرهای وابسته مورد نظر از آزمون تحلیل واریانس مختلط(آنوا) در سطح معناداری 05/0 p≤استفاده شد.نتایجتفاوت معناداری در فعالیت عضلات نازک نی بلند و درشت نی قدامی بین دو گروه در موقعیت های تک پا چشم بسته و تک پا سربالا مشاهده گردید(05/0P≤) و در فعالیت بقیه عضلات تفاوت معناداری مشاهده نشد.نتیجه گیریبه نظر می رسد وضعیت غیرطبیعی پا می تواند بر تغییر فعالیت عضلات اندام تحتانی زمان ایستادن موثر باشد که متعاقبا می تواند الگوهای آسیب را تغییر دهد که پیشنهاد می گردد جهت کاهش خطرات بر طراحی تمرینات اصلاحی و برنامه درمانی مناسب تمرکز شود.

صفحات 441-445مقدمهدانشجویان پزشکی در معرض بالای مواجهه با بیماری سل قرار دارند. این مساله لزوم بررسی دقیق تر وضعیت عفونت سلی را در این جمعیت مطرح می کند.روش کاردر این مطالعه توصیفی مقطعی تست مانتو بر 200 نفر از دانشجویان پزشکی در شروع دوره ی یک ماهه کار آموزی بخش عفونی بیمارستان امام رضا (ع) پس از اخذ رضایتشان، طی سال های 89 تا 91 انجام گرفت. پس از 72 ساعت قطر اندوراسیون اندازه گیری و تفسیر شد. سپس وضعیت تست پوستی با سن، جنس، سابقه تماس نزدیک با فرد مسلول و وجود بیماری زمینه ای در دانشجویان، مورد تجزیه تحلیل قرار گرفت. اطلاعات با نرم افزار SPSS و آزمون های تی و کای اسکوئر تجزیه و تحلیل شد.نتایجافراد مونث 58% جمعیت مورد بررسی را تشکیل می دادند. 5 /93% تست پوستی منفی و 5 /6% تست پوستی مثبت داشتند. بین دو گروه با تست پوستی مثبت و منفی، از نظر میانگین سنی، اختلاف معنی داری گزارش نشد (78 /0=p). نتیجه تست مانتو با توجه به جنسیت در دو گروه اختلاف معناداری نداشت (057 /0=p). هیچ یک از افراد سابقه ای از تماس نزدیک با فرد مسلول را نداشتند. تنها یک نفر سابقه ای از پیوند کلیه داشت که نتیجه تست وی منفی بود.نتیجه گیریدر این مطالعه 5 /6% دانشجویان تست مانتو مثبت داشتند که ارتباط معناداری با جنسیت، سابقه تماس نزدیک با فرد مسلول و بیماری زمینه ای نداشت.از آنجا که 5 /93% از دانشجویان تست مانتو منفی داشتند، ضرورت انجام دوره ای تست مانتو در این گروه پر خطر مطرح می شود تا در صورت مثبت شدن، درمان پروفیلاکسی شوند.

صفحات 446-452مقدمهیکی از عوامل تهدید کننده سلامت سالمندان ترس از افتادن می باشد که با افزایش سن احتمال وقوعش بیشتر شده و با فعالیت فیزیکی آنان نیز در ارتباط است. هدف از انجام این مطالعه بررسی رابطه ترس از افتادن با میزان فعالیت فیزیکی سالمندان می باشد.روش کارمطالعه حاضر از نوع توصیفی تحلیلی مقطعی است که در آن 204 نفر از سالمندان شهر بیرجند با روش نمونه گیری خوشه ایدر سال 1393 وارد مطالعه شدند. ابزار جمع آوری اطلاعات علاوه بر پرسشنامه متغیرهای دموگرافیک، پرسشنامه مقیاس کارآمدی افتادن-فرم بین المللی(FES-I) و پرسشنامه فعالیت های سالم برای سالمندان(CHAMPS) است. داده ها با استفاده از نرم افزارSPSS و آزمون های آماری تی تست، آنالیز واریانس یک طرفه و ضریب همبستگی پیرسون مورد تجزیه و تحلیل قرار گرفت.نتایجمیانگین سنی سالمندان مورد مطالعه78 /6 ±78 /65 سال بود. براساس یافته ها 69 نفر (8/33 %) از سالمندان دارای سابقه سقوط می باشند. نتایج مطالعه نشان داد که 2/58 % از سالمندان دارای فعالیت جسمانی با شدت کم، 5 /34 % دارای فعالیت جسمانی با شدت متوسط و تنها 3/7 دارای فعالیت جسمانی با شدت زیاد بودند. همچنین یافته ها مبین این بود که ترس از افتادن با فعالیت جسمانی دارای رابطه منفی و معنی داری است (332 /0- =r، 001 /0>P).نتیجه گیریبا توجه به اینکه ترس از افتادن می تواند منجر به محدودیت و کاهش فعالیت جسمانی سالمندان شود، مداخلات روانشناختی و مشاوره ای در سالمندان دارای ترس از افتادن باید مورد توجه قرار گیرد.

صفحات 453-458مقدمهسل یک بیماری واگیردار و از جمله مهمترین بیماری های عفونی کشنده در جهان است. حدود یک سوم مردم دنیا آلوده به این باسیل می باشند که در عرض 5 سال، 10 % موارد به بیماری تبدیل می شود. در بیماری سل معمولا ریه ها مورد حمله قرار می گیرند ولی بر سایر قسمت ها نیز می تواند اثر بگذارد. درمان این بیماری به دوره های طولانی استفاده از آنتی بیوتیک ها نیاز داشته که این داروها عوارض زیادی را به دنبال دارد. این مطالعه به منظور بررسی فصلی بودن عفونت سل اولیه اطفال زیر 4 سال در بیماران مراکز بهداشت در مدت 5 سال(1383-1387) در مشهد انجام شده است.روش کاراین مطالعه به صورت یک بررسی توصیفی – تحلیلی مقطعی انجام شده است. حجم نمونه مورد بررسی شامل 33 نفر از کودکان کمتر از 4 سالی بود که طی سال های 1383-1387 به مراکز بهداشتی در سطح مشهد مراجعه نموده بودند و تشخیص سل با استفاده از اسمیر یا کشت مثبت از نظر مایکوباکتریوم توبرکلوزیس و یا اثبات سل توسط بیوپسی به تازگی در آنها مطرح شده بود.نتایجاز 33 کودک مورد مطالعه در گروه سنی 0-4 سال، میانگین سنی 8/2 با انحراف معیار 3/1 بود. 5/48 % از افراد مونث بودند. بیشترین میزان بروز سل در فصول زمستان (4/36 %) و پاییز (3/30 %) بود. ابتلای فصلی در افراد مورد مطالعه با سن، جنسیت، ملیت، سابقه واکسیناسیون قبلی بر علیه سل و سابقه تماس با فرد سلی ارتباط معنی داری نداشت.نتیجه گیریتوزیع فصلی سل در پاییز و زمستان با این تئوری که روند انتقال عفونت سل با افزایش زمان حضور در ازدحام، تهویه ضعیف و افزایش شیوع سرفه ناشی از سایر عفونت های تنفسی در فصول سرد مرتبط می باشد، قابل توجیه است.

صفحات 459-465مقدمهبیماری دیابت می تواند بر کارکردهای شناختی در مبتلایان تاثیر بگذارد، بررسی این اثرگذاری و راه های پیشگیری از آن در بهبود سلامت سالمندان جامعه مفید خواهد بود.هدف از این پژوهش بررسی کارکردهای عصب روانشناختی (حافظه، تمرکز) در بیماران مبتلا به دیابت نوع 2 است.روش کاراین پژوهش از نوع توصیفی- تحلیلی و در زمره ی تحقیقات علی– مقایسه ای (پس رویدادی) است که جامعه ی آماری شامل کلیه ی مبتلایان به دیابت دو مراجه کننده به مرکز تحقیقات غدد و متابولیسم صدیقه طاهره و کارمندان آزمایشگاه های تشخیص طبیافراد شهرستان اصفهان در تابستان سال 1392 بودند. به شیوه نمونه گیری دردسترس تصادفی گروه بیماران مبتلا به دیابت نوع دو (30n=) انتخاب و با یک گروه از افراد سالم (31n=) از لحاظ سن و جنس وسایر متغیرهای جمعیت شناختی همتا شدند. ازآزمون حافظه ی وکسلر و تست استروپ استفاده شد. داده ها با روش مقایسه ی میانگین های دو گروه (آزمون تی) و ضریب همبستگی پیرسون باکمک نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفتند.نتایجنشان داد بیماران مبتلا به دیابت نوع دو از لحاظ شاخص های توجه و تمرکز (زمان صرف شده در آزمون استروپ و خطای درجاماندگی) نسبت به گروه گواه ضعیف تر عمل می کنند.(0sig=) همچنین بین این دو گروه از لحاظ حافظه ی منطقی تفاوت معنادار وجود دارد(0sig=). سایر یافته ها نشان داد بین حافظه با قند خون رابطه ی معناداری وجود ندارد.نتیجه گیریمی توان گفت بیماری های جسمی نظیر دیابت می تواند در ایجاد اختلال در عملکردهای عصب روانشناختی این بیماران تاثیر داشته باشد.بیماری دیابت در سیستم حافظه و تمرکز بیماران اختلال ایجاد کرده، عملکرد این دسته از افراد در بلندمدت کاهش می یابد.

صفحات 466-472مقدمهآپاندیسیت حاد شایعترین علت مراجعه بیماران با درد حاد شکمی به بخش اورژانس می باشد. با وجود روش های تشخیصی پیشرفته، هنوز تشخیص آپاندیسیت حاد از مشکلات جراحی بوده و میزان آپاندکتومی منفی در بین مردان وزنان از 8% تا 45% گزارش شده است. مطالعه حاضر جهت مقایسه دو روش سونوگرافی و سی تی اسکن در تشخیص آپاندیسیت حاد انجام شده است.روش کاراین مطالعه توصیفی مقطعی بر بیماران مشکوک به آپاندیسیت مراجعه کننده به اورژانس بیمارستانهای قائم (عج) و امام رضا (ع) از سال 1392- 1393، انجام گردید. نتایح سونوگرافی و سی تی اسکن این بیماران با نتایج آسیب شناسی مقایسه و دقت تشخیصی این دو روش رادیولوژیک مقایسه گردید. سپس داده ها در SPSS وارد و با استفاده از آزمون تی دانشجویی و کا اسکوئر تجزیه و تحلیل شد.نتایجدر مجموع 46 بیمار مذکر و 35 بیمار مونث با میانگین سنی9/8 ± 6/24سال در این مطالعه وارد شدند. حساسیت، اختصاصیت، ارزش اخباری مثبت و منفی سونوگرافی با توجه به نتایج آسیب شناسی به ترتیب 9 /73 %، 5 /62 %، 4 /94 % و 2 /78 % بود. حساسیت، اختصاصیت، ارزش اخباری مثبت و منفی سی تی اسکن به ترتیب 8 /88 %، 9 /80 %، 1 /94 % و 6 /66 % بود.نتیجه گیریبا توجه به نتایج مطالعه حاضر سی تی اسکن نسبت به سونوگرافی در تشخیص آپاندیسیت حاد دقت بیشتری دارد و این برتری بخصوص در زنان و افراد با علائم بالینی غیراختصاصی بارزتر است.

صفحات 473-480در سالهای اخیر، اپیدمی جهانی افزایش وزن منجر به افزایش توجه به شناخت بیولوژی بافت چربی سفید شده است. آدیپوسیت ها و ماکروفاژهای موجود در نسج چربی تولیدکننده چندین پروتئین واسطه ای تحت عنوان آدیپوکین ها است. آدیپوکین ها شامل انواع مختلفی از پپتیدها با خواص پیش التهابی از جمله لپتین، رزیستین، لیپوکالین 2، اینترلوکین 18 واثرات ضد التهابی مثل آدیپونکتین می باشند. رزیستین یکی از این پپتیدهای جدید مشتق از آدیپوسیتها است که به طور قوی باعث ایجاد مقاومت به انسولین در نسج کبد می شود و در فرایندهای فیزیولوژیک و روندهای التهابی و خود ایمنی شرکت می کند. غلظت سرمی آن دربرخی بیماری های التهابی مثل کرون و بیماری های بافت همبند از جمله آرتریت روماتوئید و لوپوس بالا بوده است.
نقش آدیپوکین ها در تنظیم پروسه های التهابی این سوال مهم را مطرح می سازد که آیا مهار تولید غیر طبیعی آدیپوکین ها در بیماری های اتوایمیون می تواند هدف بیولوژیک برای درمان این اختلالات باشد.
این مقاله مروری به بحث در مورد نقش آدیپوکین ها بویوه رزیستین در بیماری های التهابی و خود ایمنی پرداخته است.

Pages 417-424ObjectiveDisc battery ingestion as a consequence of performing the novel technology in toys, can produce significant danger in children. The aim of this study was to evaluate clinical manifestations, therapeutic protocols, and complications of disc battery ingestion in children.MethodsIn a case series study during march 2007 to march 2012, all children who referred to Children’s medical center hospital in Tehran because of disc battery ingestion were evaluated using plain radiography and upper endoscopy. A one year follow up was performed to assess complications.ResultsA total of 89 children (56 male) with mean age of2.50±2.93 years were enroled. 67.4 % were asymptomatic at the time of admision. Vomiting (25.8%) was the most common complaint at presentation. Esophagoscopy was performed in 22 cases who the disc battery was impacted in the esophagus. But successful removal ensued in 81.8 percent, and 4 cases needed surgical intervention. The disc battery was removed in 12 cases (19%) from the stomach and it was passed through the gastrointestinal tract in the others. The most common complication was delayed esophageal stricture in 14.6%.ConclusionAlthough usually the disc battery ingestion is not a cause of significant complication, due to its potential dangers, emergent endoscopic intervention is mandatory in cases with impaction of the disc battery in the esophagus.Keywords: Disc Battery, Ingestion, Complication, Children

Pages 425-431Introductionhelicobacter pylori infection is a common infection worldwide. One of the side effects that are suggested for this infection is Diabetes and insulin resistance (IR). The aim of this study is evaluation of HP eradication on insulin resistance is NAFLD.Methods51 patients with NAFLD and HP infection and dyspepsia were selected for this study. Patients were divided into two groups (control group and intervention group). Control group received only the life style changes and intervention group, on the other hand, received HP eradication therapy in addition to the life style changes. FBS, Chol, TG, LDL, HDL, insulin, BMI, HOMA-IR were checked before and after HP eradication in both groups.ResultsIn both groups, no significance difference were observed between the parameters which were evaluated.Conclusionit seems that HP eradication per se might not effect on FBS, lipid profile and insulin resistance. Pages 432-441Introductionpesplanus is considered that makes muscle activation changes. Being aware of muscles activity changes in various standing positions among pesplanus patients, provide insight to prevent lower extremity injuries in this population. This study is undertaken to compare the electromyography of the leg muscles in various standing positions between young healthy and pesplanus men male individuals.Materials And Methods40 healthy male university students, 20 normal and 20 subjects with pesplanus deformity participated in this semi-experimental study. Deformity of pesplanus was assessed with navicular drop test. Each subject stood in five different positions and muscles activity was recorded with emg device. For data analysis, Matlab and Spss software applied and Mixed variance analysis test(Mixed ANOVA) was used to compare the dependent variables at a significance level of P ≤ 0/05.ResultsSignificant differences were observed between the two groups for muscles activity of the longus peroneus and anterior tibialis(at single leg with closed eye and upward head postions)(p≤0/05); while no significant differences was observed in other muscles.ConclusionThe changes in the normal structure of the foot might affect muscle activities during standing that can cause changes in the injury patterns. Pages 441-445IntroductionMedical students are highly exposed to Tuberculosis. Thus it is of great importance investigation of TB infection among such population.Material And MethodsManteux was done on 200 medical students, who passed the one month infectious disease course at Imam Reza hospital, at the beginning of extern session between 2010 and 2013 with their permission. Induration diameter was measured and studied after 72 hours. Then skin test status with age, sex, history of close contact to a person with tuberculosis and underlying diseases of the students analyzed by T- test and.Results58% of examined population were females. 93.5% and 6.5% had negative and positive skin test results, respectively. Meaningful difference of average age was not reported between the two positive and negative groups (p=0.78). Meaningful difference of gender was not reported in PPD skin test results between the two groups (p=0.057). None of the participants had history of close contact to a person with tuberculosis. Only one person had history of kidney transplantation with negative test result.ConclusionThis study showed that manteux test results of the students were positive in 6.5%, which had no meaningful correlation with the gender, close contact with person with tuberculosis, and underlying disease. Since 93.5% of the students had negative manteux test results and they will have more exposure to tuberculosis through the next of the medical course, this indicates the importance of conducting the manteux test periodically in this group to receive prophylactic care in seroconvert cases.Keywords: PPD Skin Test, Tuberculosis, Medical Students

Pages 446-452IntroductionFear of falling (FOF) is a major health problem among the elderly. One of the major consequences of fear of falling is activity restriction. The aims of this study were to investigate the prevalence of fear of falling among a population of older adults and its correlation with physical activity.Material And MethodsThis was a cross-sectional study in which 204 elderly people were questioned in Birjand City, Iran. Data collection was performed by structural questionnaires. FOF was measured using the FES-I questionnaire, and physical activity was measured using the CHAMPS questionnaire. To analyze the data we used SPSS20 software, and statistical tests such as T test, and one way ANOVA and PearsonCorrelationCoefficient were applied.ResultsMean age was 65.78±6.78 years. 33.8% of elderly had history of fall, 58.2% had Low-intensity physical activity, 34.5% had moderate-intensity physical activity and only 7.3% had high-intensity physical activity. There was significant relationship between FOF with physical activity (r = - 0.332, P<0.001).ConclusionFOF can lead to restriction and decrease physical activity. So psychology and counseling interventions should be considered in elderly with history fall.Keywords: Aged, Fear of Falling, Physical Activity

Pages 453-458IntroductionTuberculosis is a contagious disease and one of the most deadly infectious diseases in the world. About a third of the world's people are infected with the tuberculosis. During 5 years of tuberculosis infection, 10% of cases involved disease. Tuberculosis can affect virtually every organ, most importantly the lungs. Tuberculosis treatment needs a long-period use of antibiotics with numerous side effects. The purpose of this study is survey of Seasonality primary TB infection in children under 4 years In Mashhad health centers from 2004-2008.MethodsIt is a descriptive analysis cross-sectional study among 33 children fewer than 4 years with primary tuberculosis attending to health centers of Khorasan Razavi from 2003 to 2008. Tuberculosis diagnoses newly were confirmed with sputum smear, culture and biopsy.ResultsIn 33 cases primary tuberculosis between ages 0-4 years, the average age were 2/8 ± 1/3 years. 48.5% were female. The most common incidence of tuberculosis was in winter (%36/4) and fall (30/3). There is not a significant relationship between of Seasonality and age, sex, nationality, tuberculosis vaccination, tuberculosis contact history.ConclusionThe distribution of tuberculosis in the fall and winter season with the theory that the presence of TB infection with an increased in overcrowding, poor ventilation and the increased prevalence of cough caused by other respiratory infections in cold weather related, is justified.Keywords: Pulmonary Tuberculosis, Primary Tuberculosis, Children, Seasons

Pages 459-465Introductionthis study was aimed at investigating of the neuropsychological performance (memory and concentration) in patients with diabetes type 2. Diabetes Disease has an influence on the Cognitive performance of these patients. The investigation of this affect and the methods of prevention can be valueable to improve the health of elderly people of society.ObjectiveThis study was in a series of Discriptive-Analytic and in the group of causal-Comparitive (Ex post Facto) reasearchs. The Statistical society involved all of the patients with Diabtes type 2 that going to center of researchs of glands and metabolism Sedighe Tahere and and all of the personals of the laboratories in Isfahan province in summer 1392. In the form of simple randomly sampling vicinity, a group involved patients with Diabetes type 2 (n=30) had been selected and became similar with the healthy cases(n=31) with regard to age, sex and education. Have been used The Wechsler memory scale and Stroop test. T-TEST and Pierson Corrolation by the help of software spss-19 were used to analyze the data.Findingsfindings shows that the patients of Diabetes type 2 are different in terms of attention and Concentration (the spent time in Stroop test and executed more omission errors) with healthy cases, also these two groups have significant difference in logical Memory. Other findings shown that there is not any singnificant difference between Blood Sugar and Memory.ConclusionIt can be said that physical diseases can affect the development of disorder in neuropsychological performances of the patients. Pages 466-472IntroductionAcute appendicitis is one of the main cause of acute abdomen referral to emergency departments worldwide. And despite recent investigations in diagnostic methods, appendicitis accurate diagnosis is still one of the controversial issues in surgery. It is estimated that incidence of negative appendectomy in men and women varies from 8% to 45%. In this study we compared the accuracy of ultrasound and Computer tomography scan (CT scan) in patients suspected to acute appendicitis.Material And MethodsIn this cross-sectional study, patients were enrolled the study who were referred to emergency department of Imam Reza hospital with clinical suspicion ofacute appendicitis. Radiologic result of patients underwent surgery for acute appendicitis was compared with pathology reports. Data were analyzed by chi-square and T-student tests in SPSS16.ResultsOverall 46 males and 35 females (mean age: 24.6± 8.9 years) were studied. Sensitivity, specificity, positive predictive value and negative predictive value of abdominal ultrasound for acute appendicitis were 73/9%, 62/5%, 94/4%, and 78/2%, respectively. These values for CT scan were 88/8%, 80/9%, 94/1% and 66/6%, respectively.Conclusionour results showed that CT scan is more accurate than sonography for diagnosis of acute appendicitis, particularly in female patients and patient with nonspecific symptoms.Keywords: Acute Appendicitis, Computer Tomography Scan, Ultrasound

Pages 473-480In recent years, the universal epidemic of obesity, cause more interest in performing further researches about the biology of white adipose tissue. Adipocytes and macrophages within the adipose tissue secreted protein mediators are named as adipokines. Some adipokines such as leptin, resistin, lipokalin and interlukine-18 play an important pro-inflammatory role while adiponectin has mainly anti-inflammatory properties. Resistin as the novel adipocyte–derived peptid induced a significant insulin resistance in the liver tissue as well as contribute in physiologic besides inflammatory and autoimmune process. Serum concentration of this marker is correlated with some inflammatory processes such as Crohn disease and collagen vascular disorders like rheumatoid arthritis and systemic lupus erythematosus. The roles of adipokines in modifing inflammatory processes raise an important question to answer whether inhibition of abnormal adipocytokines production in autoimmune diseases could be a potential biologic target for treatment of this disorder.
