    سرطان یک بیماری چند عاملی است که در اثر عوامل محیطی و ژنتیکی متعدد ایجاد می شود. ژن های دخیل در بروز سرطان را به چند گروه شامل پروتوآنکوژن ها، ژن های سرکوب گر تومور، ژن های دخیل در پایداری ژنوم و مهاجرت سلول طبقه بندی می کنند. در اثر تجمع تغییرات ژنتیکی، توده توموری ایجادشده، منبع خونی برای تغذیه و تداوم رشد خود را کسب می کند. توده توموری مجموعه ای از سلول های منفرد نیست و برهم کنش های دوطرفه با محیط در برگیرنده خود دارد. محیط دربرگیرنده تومور (TME) عملکرد مشابهی با کنام سلول های بنیادی داشته که پیشرفت تومور و ایجاد متاستاز را متاثر می نماید. مطالعه ماهیت این محیط در تشخیص و درمان مولکولی سرطان موثر است و اطلاعات ارزشمند و جدیدی برای کنترل بدخیمی تومور و ارزیابی خطر فراهم می کند (1). در این مقاله به بررسی عناصر تشکیل دهنده TME و اهداف مولکولی درمان سرطان پرداخته می شود.
    شناسایی TME توسط پروفایل های سلولی و مولکولی نشان می دهد که در این محیط انواع متفاوت سلول وجود دارند که ضمن تحریک تغییر نئوپلاستیک و متاستاز، تومور را از سیستم ایمنی میزبان محافظت نموده و منجر به مقاومت به درمان می شوند (2). از میان انواع متفاوت سلول های حاضر در TME، شامل سلول های پارانشیمال تومور، فیبروبلاست، سلول های اپی تلیال و التهابی، ماتریکس خارج سلولی و مولکول های پیام رسان، خون و رگ های لنفی، فیبروبلاست ها بیشترین تعداد سلول را به خود اختصاص می دهند. در مراحل اولیه سرطان زایی، فیبروبلاست های نرمال از رشد تومور جلوگیری می کنند. به مرور با ایجاد تغییرات ژنتیکی، این سلول ها با کمک عوامل التهابی، فاکتورهای رشدی را آزاد می کنند که یا مستقیما سلول های تحریک کننده تکثیر تومور را متاثر می نمایند و یا به صورت غیرمستقیم آپپتوز را با تحریک رشد و القای آنژیوژنز مهار می کنند. در مجموع، با درگیرشدن تعداد متنوعی از عوامل سلولی و پیام های مولکولی، سیستم پیچیده ای از برهم کنش ها ایجاد می شود
    (3، 4).
    در یک TME، میان کنش های دوطرفه سلول های توموری با ماتریکس خارج سلولی (ECM) ، ماکروفاژهای مرتبط با تومور (TAM) ، فیبروبلاست های تمایزیافته (CAF) ، سلول های بنیادی مزانشیمی (MSC) و سلول های اندوتلیال (EC) به اثبات رسیده است. این ارتباطات با کمک کموکاین ها، فاکتورهای رشد، ماتریکس متالوپروتئازها (MMP) و پروتئین های ECM برقرار شده و نهایتا منجر به مهاجرت، حمله به اندام های دوردست و ایجاد متاستاز می شود (5). TME با ایجاد تغییرات در فراخوانی سلول های استرومال و ایمنی، بافت را بازسازی کرده و در تومور تغییرات متابولیکی ایجاد می کند. این بازسازی در یک محیط القایی پیش برنده تومور، مشابه ناحیه اطراف یک زخم است که توسط مجموعه ای از سلول ها احاطه می شود (6). بسته به نوع سرطان، بیش از 40 درصد CAFها می توانند مشتق از سلول های پیش ساز مغز استخوان باشند که به مکان تومور در حال رشد فراخوانده می شوند. اگرچه، CAFها هم چنین ممکن است ناشی از سلول های سرطانی اپی تلیالی یا فیبروبلاست های ساکنی باشند که به میوفیبروبلاست تمایز یافته اند. در تومورهای اپی تلیالی، فیبروبلاست ها عمدتا از طریق ترشح فاکتورهای رشد و کموکاین ها منجر به ایجاد یک ماتریکس خارج سلولی تغییریافته شده و سیگنال های تکثیر و متاستاز را افزایش داده و نهایتا منجر به تحریک پیشرفت تومور می شوند (7). ماتریکس خارج سلولی نیز داربستی از سلول های التهابی، رگ های لنفی و عصب را تشکیل می دهد. به طورکلی، در پدیده متاستاز، تومورهای مهاجم بایستی توانایی حرکت، ظرفیت تجزیه ماتریکس خارج سلولی بافت، تشکیل رگ های خونی جدید، بقا در خون و تثبیت در یک محیط بافتی جدید را کسب کنند. در مطالعاتی که برای شناخت چگونگی کسب این قابلیت ها در سلول های سرطانی انجام شده، مشخص شده است TME اهمیت حیاتی در ایجاد این پدیده دارد. TME با ارسال سیگنال هایی به سلول های استرومایی یا غیر بدخیم و فعال کردن رونویسی برخی از ژن ها، حمله تومور به بافت های دوردست و محیط جدید را تثبیت می کند (8، 9). هم چنین، سلول های پیش ساز آنژیوژنز که تحت شرایط هیپوکسی به TME فراخونده شده اند با متاستاز در ارتباط هستند. برخی از مطالعات نشان می دهند که مولکول هایی تحت عنوان miRNA تنظیم کننده اصلی این فعالیت بوده و منجر به تغییر فیبروبلاست ها در TME می شوند. MiR-21، miR-31، miR-214 و miR-155 نقش مهمی در تمایز فیبروبلاست های نرمال به CAF دارند (10). هرچند عملکرد miRNAها در TME هنوز هم به طور کامل شناسایی نشده است، اما مطالعات حاکی از این است که miRNAهای تولیدشده توسط سلول هایTME ، به خصوص CAFها، روی رشد تومور اثر می گذارند (11). موسومسی و همکاران نقش miRNAها در TME را در سرطان پروستات نشان دادند. در مطالعه آن ها مشخص شد در این سرطان بیان miR-15aوmiR-16 در فیبروبلاست های حاضر در TME کاهش می یابد و آنکوژن هایی مانند Bcl-2 و اجزای مسیر WNT توسط این miRNAها هدف گیری می شوند (12).
    استراتژی های مختلفی برای هدف قرار دادن اجزای TME در درمان سرطان پیشنهاد شده است (2). مسدود نمودن فراخوانی و فعال سازی سلول های استرومال در TME یکی از این راهکارهاست و داروی Avastin در درمان سرطان کلون و گلیوبلاستوما بر همین اساس طراحی شده است. برخی داروها نیز میان کنش بین سلول های TME با تومور و یا آنژیوژنز، ECM و ترکیبات التهابی در TME را مورد هدف قرار می دهند. siltuximab یک آنتی بادی ضد IL-6 انسانی است که مسیر فعالیت IL-6/STAT3 را در سلول های سرطانی مهار می کند و اثرات درمانی آن در مدل های زنوگرافت گزارش شده است. اثر این دارو نیز در فاز II کلینیکال تریال در سرطان تخمدان مقاوم به platinum در دست مطالعه است. شناسایی دقیق تر شبکه های ژنی و مسیرهای سلولی به بهبود درک ما از پاتوژنز سرطان و پیشرفت روش های درمانی کمک می کند. بنابراین علاوه بر کنترل مسیرهای پیام رسان در داخل تومور، شناسایی TME مرتبط با آن نیز ضرورت دارد. اگرچه علی رغم شناسایی اهمیت TME در سرطان زایی، با توجه به تعدد سلول های درگیر در آن، منشا ایجاد جهش های مولکولی در اجزای آن هنوز هم به طور کامل مشخص نشده است و نیازمند اجرای مطالعات گسترده در این زمینه می باشد.

    زمینه و هدف
    میگرن یک بیماری شایع است که معمولا به صورت ضربان دار و یک طرفه با علائم اورا مثل تهوع، استفراغ و فتوفوبیا تظاهر می یابد. در میگرن به دلیل التهاب ناشی از کاهش سروتونین، میزان پروکلسی تونین سرم افزایش می یابد. این مطالعه در نظر دارد با استفاده از داروهای آگونیست سروتونین، میزان پروکلسی تونین سرم را در افراد مبتلا به میگرن بررسی کند.
    مواد و روش ها
    در این مطالعه مورد-شاهدی، 40 نمونه مبتلا به میگرن در مرکز بیماران مبتلا به میگرن بیمارستان الزهرای شهر اصفهان جهت بررسی جمع آوری شدند که 26 نفر تحت درمان با سروتونین و 14 نفر تحت درمان با داروهای دیگر به غیر از آگونیست های سوماتریپتان بودند. سپس میزان پروکلسی تونین در سرم خون این بیماران اندازه گیری و آنالیز شد.
    یافته ها
    طی نتایج به دست آمده، اختلافی بین میزان پروکلسی تونین در سرم نمونه و شاهد دیده شد که به نظر می رسد این سطح در افراد نمونه نسبت به شاهد کمتر باشد.
    نتیجه گیری
    در نهایت، این مطالعه بیان کننده ی تاثیر سروتونین در کاهش میزان پروکلسی تونین سرم است.

    زمینه و هدف
    تحقیقات اخیر در مورد سلول درمانی جنبه های جدیدی از درمان بیماری های تحلیل نورونی را فراهم آورده است. بر اساس تحقیقات اخیر، کاربرد داخل بینی سلول های بنیادی در درمان موش های آزمایشگاهی مدل پارکینسون توانسته باعث بهبودی طولانی مدت این بیماری شود. سلول های بالغ بنیادی آندومتر انسانی نوعی از سلول های آماده و در دسترس هستند که برای تولید نورون های دوپامینرژیک و اهداف سلول درمانی استفاده می شوند. هدف از این تحقیق، کاربرد داخل بینی سلول های بنیادی آندومتر انسانی در موش های آزمایشگاهی مدل پارکینسون است.
    مواد و روش ها
    در این تحقیق تجربی، از 35 سر موش نر با محدوده ی وزنی 25 تا 30 گرم در 5 گروه استفاده شده است. صد و بیست روز پس از سلول درمانی، رفتار چرخشی موش ها مورد ارزیابی قرار گرفت. برای آشکارسازی سلول های بنیادی در مغز از رنگ آمیزی ایمونوهیستوشیمی استفاده گردید.
    یافته ها
    سلول های بنیادی اندومتر انسانی به کاربرده شده در داخل بینی توانستند وارد ناحیه ی جسم سیاه مغز شوند و چرخش موش های پارکینسونی را بهبود بخشند.
    نتیجه گیری
    سلول های بنیادی اندومتر انسانی به عنوان منبعی بالقوه از سلول های بنیادی آلوژنیک می توانند بیماری پارکینسون را بهبود ببخشد.

    زمینه و هدف
    سانسور فاصله ای در مطالعات پزشکی در داده هایی که به صورت دوره ای تکرار می شوند رخ می دهند. روش های مرسومی به منظور بررسی این نوع داده وجود دارد. هدف از انجام این مطالعه، بررسی تکنیک جانهی تصادفی در تجزیه و تحلیل داده های سانسورشده فاصله ای است.
    مواد و روش ها
    با استفاده از تکنیک شبیه سازی مونت کارلو به ارزیابی توان این روش می پردازیم و هم چنین در نهایت با استفاده از مجموعه داده واقعی عملکرد آن را ارزیابی می کنیم. این مجموعه داده مربوط به اطلاعات دندان پزشکی شهر ارومیه است که شامل 207 کودک بوده است. تمام محاسبات با استفاده از نرم افزار R 3. 2. 3 انجام گرفته است.
    یافته ها
    نتایج حاصل از شبیه سازی نشان می دهد که توان آزمون محاسبه شده در تکنیک جانهی تصادفی خوب و قابل قبول است. مقدار p حاصل در داده های واقعی نشان می دهد که با استفاده از تکنیک جانهی تصادفی تفاوتی وجود ندارد.
    نتیجه گیری
    روش جانهی تصادفی در مقایسه با سایر روش های مرسوم به عنوان یک روش جایگزین می تواند مورد استفاده قرار بگیرد.

    زمینه و هدف
    الایژیک اسید (EA) یک ترکیب آنتی اکسیدان طبیعی با ساختار فنلی است. در این مطالعه تاثیرات حفاظتی آن بر جمعیت و فعالیت سلول‫های گلیال در مدل حیوانی ام اس در شرایط استرس اکسیداتیو مورد بررسی قرار گرفت. مواد و روش ‫ها: موش‫های C57BL/6 بالغ نر 8 تا 9 هفته ای تهیه و در شرایط استاندارد نگهداری شدند. جهت ایجاد مدل، حیوانات به مدت شش هفته غذای حاوی 2/0 درصد کوپریزون (Cup) را مصرف نمودند. حیوانات به هشت گروه شامل کنترل، کنترل دریافت کننده سه دوز EA (20، 40 و 80 میلی گرم برکیلوگرم) ، Cup و Cup دریافت کننده سه دوز EA تقسیم شدند. در انتها بافت مغز حیوانات توسط روش های مولکولی شامل ایمونوهیستوشیمی (IHC) ، وسترن بلات (WB) و ریل تایم پی سی ار (q-PCR) جهت تجزیه و تحلیل مارکرهای اختصاصی سلول های گلیال مورد استفاده قرار گرفت.
    یافته ها
    آنالیز IHC و WB نشان داد که تنها غلظت بالای EA قادر به کاهش بیان پروتئین های GFAP (شاخص آستروسیت‫های فعال) ، Mac-3 (شاخص میکروگلیال های فعال) ، افزایش بیان پروتئین‫ Olig-2 (شاخص الیگودندروسیت های پیش ساز) و در نهایت کاهش معنادار نسبت پروتئین APC (شاخص الیگودندروسیت های بالغ) به Olig-2 نسبت به گروه Cup می باشد. تجریه و تحلیل q-PCR نیز نشان داد تغییرات بیان mRNA این شاخص ها نیز با تغییرات پروتئین های آن ها همسان بوده و از این رو نتایج قبلی تایید گردید.
    نتیجه گیری
    مصرف (80 میلی گرم بر کیلوگرم) EA به طور موثر فعالیت آستروسیت ها و میکروگلیال ها را کاهش داده و محیط را جهت مهاجرت سلول های پیش ساز الیگودندروسیتی به ناحیه عارضه و تبدیل فرآیندهای تخریبی به مکانیسم های ترمیمی مساعد می نماید.

    زمینه و هدف
    در کشور ایران یک چهارم زوج ها تجربه ناباروری داشته اند که این مشکل می تواند بر ابعاد مختلف زندگی زناشویی تاثیرگذار باشد. این مطالعه با هدف مقایسه ویژگی های روانشناختی در زنان بارور و نابارور مراجعه کننده به مراکز بهداشتی و مرکز ناباروری در شهر قم انجام شده است.
    مواد و روش ها
    این مطالعه از نوع توصیفی-تحلیلی مقطعی روی 160 نفر از زنان انجام گرفت. 80 نفر از زنان در سنین باروری بوده و برای دریافت مراقبت های روتین کودکانشان به مراکز بهداشتی مراجعه می نمودند و 80 نفر دیگر از میان زنان نابارور مراجعه کننده به مرکز ناباروری بودند که به صورت تصادفی ساده انتخاب گردیدند. داده ها با استفاده از پرسشنامه های استاندارد اضطراب، افسردگی و سازگاری زناشویی جمع آوری گردیده و با استفاده از نرم افزار آماری SPSS نسخه 20 از طریق آزمون های دقیق فیشر، تی مستقل و کای اسکوئر تجزیه و تحلیل شد.
    یافته ها
    از نظر میانگین نمرات افسردگی (005/0= p) ، اضطراب (02/0= p) ، و سازگاری زناشویی (01/0= p) ، تفاوت معنی داری در بین دو گروه مشاهده شد. به طوری که میانگین نمره افسردگی و اضطراب در زنان بارور بیشتر از زنان نابارور بود در حالی که میانگین نمره سازگاری زناشویی در زنان نابارور بیشتر از زنان بارور بود.
    نتیجه گیری
    از آن جایی که باروری می تواند با عوامل روانشناختی و سایر عوامل زمینه ای فرد مرتبط باشد، بنابراین توجه به نیازهای مختلف آموزشی زنان بارور و نابارور باید به طور جدی مدنظر قرار گیرد.

    زمینه و هدف
    نوروپاتی، شایع ترین عارضه عصبی دیابت شیرین است که با آسیب های مغزی به ویژه در هیپوکامپ مشخص می گردد. مطالعه حاضر اثر عصاره دارچین (CZE) بر حافظه، آسیب نورونی و سطح آنزیم های آنتی اکسیدانی در مدل حیوانی دیابت را ارزیابی می کند.
    مواد و روش ها
    50 سر موش صحرایی بالغ نژاد اسپراگ داولی به صورت تصادفی به 5 گروه تقسیم شدند: گروه کنترل، گروهSTZ (50 میلی گرم بر کیلوگرم استرپتوزتوسین، درون صفاقی) و گروه های STZ+CZE100،STZ+CZE200 و STZ+CZE400 که با عصاره دارچین به ترتیب با دوزهای 100، 200 و 400 میلی گرم تیمار شدند. عصاره طی 14 روز و به صورت خوراکی تجویز شد. متعاقب سنجش حافظه کاری و فضایی، فعالیت آنزیم های کاتالاز (CAT) و گلوتاتیون پراکسیداز (GPx) هیپوکامپ به روش الایزا ارزیابی شد. سپس، ارزیابی هیستوپاتولوژیک هیپوکامپ صورت گرفت.
    یافته ها
    در مقایسه با گروه کنترل، گروه STZ افزایش مدت زمان و مسافت پیمایش تا رسیدن به سکوی مخفی طی ماز آبی موریس، کاهش رفتارهای تناوبی، کاهش دانسیته نورونی و فعالیت CAT و GPx هیپوکامپ را نشان داد (05/0> p). از طرفی، تیمار با عصاره دارچین سبب کاهش مدت زمان و مسافت پیمایش مسیر در ماز آبی و افزایش رفتارهای تناوبی، دانسیته نورونی و فعالیت آنزیم های آنتی اکسیدانی هیپوکامپ در مقایسه با گروه STZ گردید (05/0> p).
    نتیجه گیری
    دیابت با کاهش دانسیته نورونی و فعالیت آنزیم های آنتی اکسیدانی هیپوکامپ، باعث نقص در حافظه فضایی و کاری موش های صحرایی می شود، درحالی که تیمار با عصاره دارچین سبب بهبود این اختلالات نوروپاتولوژیکی می گردد.

    زمینه و هدف
    بیماری مولتیپل اسکلروزیس (ام اس) یک بیماری رایج دمیلینه کننده سیستم عصبی مرکزی است که التهاب و فرآیندهای استرس اکسیداتیو نقش مهمی در پیشرفت و علایم بالینی آن دارند. هدف اصلی مطالعه حاضر تخمین اثرات ضدالتهابی ملاتونین بر روی خستگی در بیماری مالتیپل اسکلروزیس است.
    مواد و روش ها
    این مطالعه کارآزمایی بالینی دوسوکور بر روی 62 بیمار مبتلا به ام اس عود کننده-بهبود یابنده انجام شد که به طور تصادفی جهت دریافت داروی ملاتونین به صورت 3 میلی گرم در روز یا به صورت پلاسبو برای 8 هفته انتخاب شدند. شدت خستگی در ام اس توسط پرسشنامه FSS (Fatigue Severity Scale) در هفته 1 و 8 بررسی شد.
    یافته ها
    47 بیمار در گروه مداخله و 15 بیمار در گروه پلاسبو مطالعه را تکمیل نمودند. در گروه مداخله درمان با ملاتونین کاهش چشمگیری در امتیاز FSS نسبت به گروه پلاسبو ایجاد کرد. میانگین نتایج پرسشنامه FSS قبل از مداخله در بیماران گروه مداخله و کنترل به ترتیب برابر 97/13±04/32 و 44/15±86/35 و هم چنین بعد از مداخله در بیماران گروه مداخله و کنترل به ترتیب برابر 40/12±42/30 و 66/12±41 بود (001/0 = p). امتیاز گروه کنترل 866/35 بود که بعد از مداخله به 41 رسید. از آن جا که امتیاز گروه کنترل بیشتر یا مساوی 36 بود، این مطالعه نشان داد ملاتونین اثر چشمگیری در بهبود و کاهش خستگی دارد.
    نتیجه گیری
    استفاده از ملاتونین در کنار داروهای خط اول مانند میتوکسانترون و اینترفرون بتا و گلاتیمر استات می تواند خستگی و ناتوانی در بیماران مبتلا به ام اس را بهبود بخشد.

    زمینه و هدف
    سندرم نقص ایمنی اکتسابی از آلودگی بدن با ویروس نقص ایمنی انسانی ایجاد می شود. عوامل متعددی بر بقای افراد مبتلا به AIDS موثر هستند که اغلب با استفاده از مدل های رگرسیونی کاکس مورد بررسی قرار گرفته اند. پیش فرض استفاده از مدل کاکس، متناسب بودن مخاطرات است که در صورت تخطی، این مدل از کارایی لازم برخوردار نیست. هدف از این مطالعه، تعیین عوامل موثر بر بقای مبتلایان به AIDS با استفاده از مدل کاکس توسعه‎یافته می‎باشد. مواد و روش‎ ها: در این مطالعه کوهورت گذشته نگر اطلاعات 1050 بیمار مبتلا به AIDS مراجعه کننده به مراکز مشاوره بیماری های رفتاری امام خمینی و زمزم تهران طی سال های 2004 تا 2014 بررسی شد و اثر عوامل بالینی و دموگرافیکی بر بقای بیماران با استفاده از مدل های کاکس تعمیم یافته مورد مطالعه قرار گرفت.
    یافته ها
    79 درصد از بیماران را مردان تشکیل می دادند. میانگین سن مبتلایان 21/84±10/34 سال و میانگین زمان تشخیص بیماری HIV تا ابتلا به ایدز 53/64±15/23 ماه بود. 76 درصد از افراد سانسور شدند. متغیرهای سن بیمار در زمان تشخیص، راه های انتقال بیماری، استفاده از ART و تعداد CD4 و زمان تشخیص بیماری تا ابتلا به ایدز در تحلیل تک متغیره و چندمتغیره معنی دار شدند (05/0>p).
    نتیجه گیری
    عوامل بالینی متعددی بر بقای بیماران مبتلا به ایدز تاثیرگذار است. مهم ترین این عوامل، استفاده از داروی ART و تشخیص زودهنگام بیماری است که شانس زنده ماندن مبتلایان را به طور چشمگیری افزایش می دهد. بنابراین به نظر می رسد ارائه خدمات غربال گری و درمانی برای افراد پرخطر کاملا ضرورت دارد.

    زمینه و هدف
    ژنCPEB پروتئین مهمی را رمز می کند که در تنظیم فرآیند ترجمه در تخمک زایی و اسپرم زایی نقش مهمی را بر عهده دارد. هدف از این مطالعه، ارزیابی همراهی بین چندشکلی rs12643066 ژن CPEB2 با خطر ابتلای مردان به آزواسپرمی/الیگواسپرمی شدید ایدیوپاتیک بود.
    مواد و روش ها
    مطالعه حاضر در قالب یک بررسی مورد-شاهدی بر روی 100 نمونه خون مردان مبتلا به آزواسپرمی/الیگواسپرمی شدید ایدیوپاتیک و 100 نمونه خون مردان بارور انجام شد. برای ارزیابی چندشکلی ژن CPEB2 از روش PCR-RFLP استفاده شد. تحلیل داده ها با آزمون آماری مجذور کای انجام شد.
    یافته ها
    در ارزیابی حاضر، اختلاف معناداری در فراوانی ژنوتیپی چندشکلی ژن CPEB2 بین دو گروه نشان داده نشد (129/2-701/0CI=، 222/1OR=، 479/0p=).
    نتیجه گیری
    مطالعه نشان داد که چندشکلی ژن CEBP2 با خطر ابتلای مردان به الیگواسپرمی/آزواسپرمی ایدیوپاتیک ارتباط معناداری ندارد. بنابراین، ممکن است ارزیابی این چندشکلی به عنوان یک بیوماکر تشخیصی در بررسی مردان مبتلا به آزواسپرمی/الیگواسپرمی شدید ایدیوپاتیک نقشی نداشته باشد.

    زمینه و هدف
    پرتودرمانی، نابودی سلول های سرطانی است و در تمام بیماران مبتلا به سرطان پستان باعث کاهش عود مجدد در آینده، تسکین درد در تومورهای موضعی و علائم متاستازها می شود. روش های درمانی مختلفی مانند فیلدهای الکترونی یا فوتونی یا ترکیبی از هر دو نوع فیلد وجود دارد.
    مواد و روش ها
    در این مطالعه برای درمان غدد لنفاوی ناحیه ای سوپراکلاویکل از دو روش درمانی 1) درمان فوتون و 2) ترکیب فوتون–الکترون در پرتودرمانی استفاده شد. 30 بیمار مبتلا به سرطان پستان (راست) همراه با درگیری غدد لنفاوی ناحیه ای انتخاب شدند. با استفاده از نرم افزار ایزوگری اطلاعات طراحی درمان بیماران با دو روش مذکور به دست آمد.
    یافته ها
    ماکزیمم و میانگین دوز رسیده به سوپراکلاویکل در روش اول 64/1±08/52 و 51/59±0/42 گری و برای روش دوم 64/1±24/54 و 43/0±67/43 گری گزارش شد. V90 (درصدی از حجم سوپراکلاوکل که 90 درصد دوز تجویزی را دریافت کرده است) در روش اول 94/1±74/59 درصد و در روش دوم 94/0±26/70 درصد محاسبه و مقدار p، 004/0 گزارش شد. ماکزیمم دوز رسیده به نخاع در دو روش اول 9/1±66/14 و 92/22±0/10 و برای تیروئید به ترتیب 1/3±62/42 و 02/5±67/37 گری بود.
    نتیجه گیری
    طبق نتایج ماکزیمم دوز رسیده به نخاع و تیروئید در روش جدید با سطح معنی داری کاهش داشت. ماکزیمم و میانگین دوز رسیده به سوپراکلاویکل در درمان جدید افزایش داشت. پوشش دهی حجم تومور در روش جدید بهینه تر گزارش شد.

    زمینه و هدف
    دیابت نوع دو اثرات ضد آنژیوژنزی در بافت قلب اعمال می کند. هدف از پژوهش حاضر، بررسی اثر تمرین استقامتی بر بیان ژن VEGF و VEGFR2 در بافت قلب رت های نر ویستار دیابتی نوع دوم بود.
    مواد و روش ها
    36 سر رت نر نژاد ویستار به طور تصادفی به سه گروه دیابتی با تمرین استقامتی (DET) ، گروه کنترل دیابتی (DC) و گروه کنترل سالم (HC) تقسیم شدند. القای دیابت نوع دوم از طریق تزریق درون صفاقی STZ صورت گرفت. پروتکل تمرین شامل 10 هفته تمرین استقامتی، 5 جلسه در هفته به مدت 20 تا 30 دقیقه و با سرعت 27 متر بر دقیقه و شدت 75 درصد VO2max در هفته اول بود که به تدریج به 60 دقیقه با سرعت 27 متر بر دقیقه و شدت 75 درصد VO2max در هفته دهم رسید. بیان ژن و پروتئین VEGF و VEGFR2 با روش Real-Time PCR و وسترن بلات بررسی شد.
    یافته ها
    بر طبق نتایج Real-Time PCR، تمرین استقامتی باعث افزایش بیان ژن VEGF (05/0>p) و VEGFR2 (05/0>p) در مقایسه با گروه DC شد. نتایج وسترن بلات نیز نشان داد که تمرین استقامتی موجب افزایش بیان پروتئین VEGF (05/0>p) در مقایسه با گروه DC می شود و تغییری در بیان پروتئین VEGFR2 وجود ندارد (05/0<p).
    نتیجه گیری
    دیابت نوع دو فرآیند آنژیوژنز در قلب دیابتی را مختل می کند. به نظر می رسد تمرین استقامتی دارای اثر مثبت بر آنژیوژنز است و می تواند باعث بهبود قلب دیابتی شود.

    Cancer is a multifactorial Disorder caused by variations in multiple genes coupled with environmental risk factors. The genes involved in the carcinogenesis can be classified into several groups, including proto-oncogenes, tumor suppressor genes, genes involved in genome stability and cell migration. The accumulations of genetic changes lead to tumor mass and formation of new blood vessels to grow. The tumor is not a collection of single cells and has bilateral interactions with its environments. The tumor microenvironment (TME) has a similar function to stem cells niches that affect tumor progression and metastasis. The study of this environment is effective in diagnosis and treatment of cancer and provides valuable and new information for controlling tumor malignancy and risk assessment (1). This paper focuses on TME components and the molecular targets for cancer treatment. Investigating of TME by cellular and molecular profiles indicated that there are different types of cells in this environment that promote neoplastic changes and metastasis and protect the tumor from the immune system and lead to resistance to treatment (2). Among the different types of cells present in the TME, including parenchymal tumor, fibroblasts, epithelial and inflammatory cells, extracellular matrix and signaling molecules, blood and lymph vessels, the highest number of cells are fibroblasts. In the early stages of carcinogenesis, normal fibroblasts prevent tumor growth. The genetic changes of these cells, with the help of inflammatory agents, release the growth factors that directly inhibit tumor-stimulating cells or indirectly inhibit apoptosis by stimulating growth and inducing angiogenesis. Therefore, a complex system of interactions is created by the involvement of a variety of cellular factors and molecular signals (3,4). Within the TME infrastructure, there are interactions of tumor cells with extracellular matrix (ECM), tumor-associated macrophages (TAMs), cancer-associated fibroblasts (CAFs), mesenchymal stem cells (MSCs) and endothelial cells (EC). These communications have been established with the help of chemokines, growth factors, matrix metalloprotezes (MMPs) and ECM proteins, that lead to migration, invasion to distant organs and metastasis (5). TME restores tissue and induces metabolic changes in the tumor by making changes in the stromal and immune cells. This remodeling in a TME is similar to around of scar surrounded by different cells (6). Based on tissue type’s cancers, more than 40% of the CAFs can be derived from bone marrow progenitors that are recruited to the growing TME. Although CAFs may also be derived of epithelial cancer cells or stained fibroblasts that differentiate into myofibroblasts. In epithelial tumors, fibroblasts, mainly through the secretion of growth factors and chemokines, led to an altered ECM, and increase signals of proliferation and metastasis, and ultimately lead to tumor progression (7). The ECM also accumulated a scaffold of inflammatory and immune cells, lymph and nerve arteries. In general, in the metastatic phenomenon, the invasive tumors should be able to move, to break up the extracellular matrix of the tissue, to form new blood vessels, to survive in the blood and to stabilize in a new tissue environment. In studies that have been conducted to understand how these capabilities are achieved in cancer cells, TME has been identified as critical to the development of this phenomenon. TME stabilizes invasion of tumor to distant organs via signals to stromal or non-malignant cells and activation of transcription of genes (8,9). Also, angiogenesis precursor cells that are recruited to TME under hypoxic conditions are associated with metastasis. Some studies have shown that miRNA molecules are the main regulator of this activity, leading to changes in fibroblasts in the TME. MiR-21, miR-31, miR-214 and miR-155 play an important role in differentiation of normal fibroblasts to CAF (10). Although miRNAs in TME have not yet been fully identified, some studies indicated that miRNAs produced by TME cells and specially CAFs affect on tumor growth (11). Musumeci and colleagues showed the role of miRNAs in TME in prostate cancer. Their study found that expression of miR-15a and miR-16 down-regulated in fibroblasts of TME in prostate cancer. MiRNAs target oncogenes such as Bcl-2 and WNT pathway components (12). Several strategies have been proposed to remodel TME components in cancer treatment (2). Blocking the recruitment and activation of stromal cells in TME is one of these molecular approaches. Based on this strategy, Avastin has been designed to treat clone and glioblastoma cancer. Some drugs also block the interaction between the TME cells with the tumor and angiogenesis, ECM and inflammatory compounds in TME. Siltuximab is a human anti-IL-6 antibody that inhibits the pathway of IL-6 / STAT3 in cancer cells and its therapeutic effects have been reported in xenografet models. The effect of this drug in the Phase II clinical trials in platinuim-resistant ovarian cancer is under survey. More accurate identification of gene networks and cell pathways will help us improve our understanding of the pathogenesis of cancer and the advancement of therapeutic approaches. Therefore, in addition to controlling the signaling pathway inside the tumor, it is also necessary to identify the TME. Although, despite the recognition of the importance of TME in carcinogenesis, due to the multiplicity of involved cells, the origin of molecular mutations in its components is still not fully detected and requires extensive research in this area.

    Background and Aim
    Migraine is a common disease with throbbing and unilateral quality, which is accompanied by aura symptoms such as nausea, vomiting and photophobia. The level of procalcitonin is increased in migraine, which is due to the inflammation caused by a low level of serum serotonin. This study aims to investigate the level of serum procalcitonin in patient with migraine by prescribing serotonin agonist drugs.
    Materials and Methods
    In this case-control study, 40 migraine patients in the center of migraine patients of al-zahra university hospital in Isfahan were selected, among which 26 patients received serotonin as a treatment while the other 14 ones were treated by drugs other than sumatriptan agonist. Then, the level of procalcitonin in these patients was calculated and analyzed. Findings: The results of the study showed that there was a difference between of the case and control groups in the level of procalcitonin in the serum of their sample blood tests. It seems that the case group patients have a lower level of procalcitonin compared to the control group.
    Conclusion
    Totally, the current study indicates the effect of serotonin in reducing the level of serum procalcitonin.

    Background and Aim
    Recent findings in cell therapy have presented new perspectives and opportunities for the treatment of neurodegenerative disease. The experimental research with intranasal (IN) administration of Stem Cells in Parkinson’s disease (PD) mouse model can work and in some cases induce major, long-lasting improvement. Adult Human endometrial derived stem cells (HEDSCs), a readily obtainable type of mesenchymal stem-like cell were used to generate dopaminergic cells and for cell therapy. In this study, we investigated the therapeutic effects of IN-delivered HEDSCs in mice model of Parkinson.
    Materials and Methods
    In this experimental research, 35 male mouse weighting 25-30 g were divided into 5 groups. On day 120 post cell administration, the rotational behavior was measured. Immunohistochemistry staining was used to detect HEDSCs in mice brain. Findings: IN application of HEDSCs resulted in the appearance of cells in the substantia nigra (SN) and decrease in the rotational behavior of case group.
    Conclusion
    HEDSCs are a highly inducible source of allogenic stem cells that improve Parkinson’s disease.

    Background and Aim
    Interval censored data occur in repeated data in medical studies. There are common methods to analysis this type of data. The purpose of this study is to examine the random imputation technique in the analysis of interval censored data.
    Materials and Methods
    Using the Monte Carlo simulation technique, we evaluate the power of Random Imputation method, and finally we assess its performance using the actual data set. Actual dataset is related to dental information in Urmia, which contains 207 children. All calculations are done using R 3.2.3 software. Findings: The simulation results show that the power of random imputation technique is good and acceptable. The p-value in real data shows that there is no difference using the random imputation technique.
    Conclusion
    Random imputation technique can be used as an alternative method in comparison with other conventional methods.

    Background and Aim
    Ellagic acid (EA) is a natural antioxidant with phenolic structure. In this study, we evaluate the effects of EA consumption on population and activation of neuroglia cells in the animal model of MS under oxidative stress.
    Materials and Methods
    Mature male mice with age of between 8 to 9 weeks were kept in the standard conditions. For model induction, animals received powder normal diet containing 0.2% Cuprizone (Cup) for six weeks. Mice were divided into eight groups containing control, control receiving three doses of EA (20, 40 and 80 mg/kg), Cup and Cup receiving three doses of EA. Finally, specific glial cell markers in the animal brain tissues were analyzed by molecular methods such as immunohistochemistry (IHC), western blotting (WB) and Real Time-PCR (RT-PCR). Findings: IHC and WB analysis have shown that only high concentration of EA is able to reduce protein expression of GFAP (activated astrocytes marker), Mac-3 (activated microglial marker), increase protein expression of Olig-2 (oligodendrocytes precursor marker) and ultimately significant reduction on APC (mature oligodendrocytes marker)/Olig-2 ratio in comparison with Cup group. In addition, RT-PCR evaluation indicated that changes in the mRNA expression of target markers were consistent with observed changes in their protein expression and therefore, IHC and WB results were confirmed.
    Conclusion
    Consumption of 80 mg/kg of EA effectively decreased activation of astrocytes and microglial and so appropriates environment for migration of oligodendrocyte precursor cells to the lesion area and shifting from damage course into the repair progressions.

    Background and Aim
    In Iran, one quarter of couples experience infertility, which can affect various aspects of their marital life. The aim of this study was to compare psychological characteristics in fertile and infertile women referring to health centers and an infertility center in Qom.
    Materials and Methods
    This descriptive-analytical cross-sectional study was carried out on 160 women. of all, 80 women were selected via simple random sampling method and were among those in reproductive age who referred to health centers to receive routine childcare services. The other 80 women were selected via simple random sampling method among infertile women who referred to the infertility center. The required data were collected using standard anxiety, depression, and marital adjustment questionnaires. Data were entered into SPSS V.20 software and were analyzed using Fisher’s exact test, independent t-test and Chi-square. Findings: There were a significant difference between the two groups in terms of the mean scores of depression (p=0.005), anxiety (p=0.02), and marital adjustment (p=0.01), as the mean score of depression and anxiety was higher in fertile women than in infertile women. However, the mean score of marital adjustment was higher in infertile women than in fertile women.
    Conclusion
    Since fertility can be dependent on psychological factors and other underlying factors of an individual, therefore, it is necessary to consider the educational needs of fertile and infertile women.

    Background and Aim
    Neuropathy is the most common abnormality in diabetes mellitus which characterized with cerebral damages especially in hippocampus. This study evaluates the effect of Cinnamomum Zeylanicum extract (CZE) on memory, hippocampal neuron damage and antioxidant enzymes levels in animal model of diabetes.
    Materials and Methods
    50 adult Sprague dawley rats were randomly divided into 5 groups: Control, STZ (Streptozotocin, 50 mg/kg; i.p.), and STZ + CZE100, STZ + CZE200 and STZ + CZE400 which were treated with CZE in 100, 200 and 400 mg/kg, respectively. CZE was administered in 14 days, orally. After evaluation of working and spatial memory, activity of catalase (CAT) and glutathione peroxidase (GPx) enzymes was assessed by ELISA. Then, histopathological assessment of hippocampus was done. Findings: In comparison with the controls, STZ group showed an increase in latency time and distance to the hidden platform in MWM, a decrease in alteration behaviors, cell density and activity of CAT and GPx enzymes in hippocampus (p˂0.05). In addition, treatment with CZE decreased latency time and distance in MWM and increased alteration behavior, hippocampal cell density and activity of antioxidant enzymes in comparison with the STZ group (p˂0.05).
    Conclusion
    Diabetes with reduction of neuronal density and activity of antioxidant enzymes in the hippocampus causes deficits in spatial and working memory. However, Administration of CZE ameliorates these neuropathologic disorders.

    Background and Aim
    Multiple Sclerosis is (MS) is a common demyelinating disease of CNS that inflammation and stress oxidative processes have an important role in clinical courses and progression of it. The aim of the present study was to determine the anti-inflammatory effects of Melatonin on fatigue in MS.
    Materials and Methods
    In a double-blind clinical trials study, 62 patients with relapsing remitting MS were randomized to oral Melatonin 3 mg or placebo daily for 8 weeks. Scale of Fatigue in MS was studied by assessing FSS (Fatigue Severity Scale) on weeks 1 and 8. Findings: 47 patients in intervention group and 15 patients in placebo group completed the study. In intervention group, treatment with Melatonin produced a significant decrease in FSS vs placebo group (before intervention: the average of FSS in patient group was 32.04±13.97 and in control group was 35.86±15.44; after intervention: the average of FSS in patient group was 30.42±12.40 and in control group was 41±12.66). Although in control group, the average of FSS was 35.866 and it was 41 after intervention. Because the score of placebo patient is near or upper than 36, this study suggested that Melatonin has a significant effect on improving and decreasing fatigue in MS.
    Conclusion
    The use of melatonin alongside of First-line drugs such as mitoxantrone, IFN – B or glatimer acetate can improve fatigue and disability in patient with MS.

    Background and Aim
    Acquired immunodeficiency syndrome is caused by body contamination with human immunodeficiency virus. Several factors affect the survival of people with AIDS, which are often examined using Cox regression models. The default is to use the Cox model as a proportionate hazard, which in case of a violation, this model does not have the required performance. Therefore, the aim of this study was to determine the effective factors on survival of AIDS patients using the developed Cox model.
    Materials and Methods
    In this cohort study, 1050 patients with AIDS who referred to Imam Khomeini and Zamzam Behavioral Counseling Centers in Tehran Province during the years 2004 to 2014 were studied. The effect of clinical and demographic factors on survival of patients has been investigated using extended Cox model. Findings: 79% of patients were men. The mean age of the patients was 34.84 ± 10. 21 years old. The mean of the diagnosis time to AIDS was 23.64 ± 15.53 months. 76% of patients were cencored. Age at diagnosis, transmission modes, ART, CD4 count, and time to diagnosis of AIDS in univariate and multivariate analysis were significant (p <0.05).
    Conclusion
    Several clinical factors have implications for the survival of AIDS patients. The most important of these factors is the use of ART and early detection of the disease, which greatly increases the chance of survival of the affected population. Therefore, it seems necessary to provide screening and treatment services for high-risk individuals.

    Background and Aim
    The CPEB gene encodes an important protein, which play critical roles in translational regulation of oogenesis and spermatogenesis procedures. The aim of this study was to evaluate the association between CPEB2 rs12643066 gene polymorphism with the risk of idiopathic azoospermia/severe oligozoospermia of men.
    Materials and Methods
    This study was designed as a case-control investigation on 100 blood samples of men with idiopathic azoospermia/severe oligozoospermia and 100 blood samples of fertile men. To evaluate CPEB2 gene polymorphism, PCR-RFLP method was used. Data analysis was performed by chi-squat test. Findings: In the present study, the genotype frequencies did not show a statically significant difference between groups (p=0.479, OR=1.222; CI=0.701-2.129).
    Conclusion
    The study showed that the CPEB2 gene polymorphism was not associated with the risk of idiopathic azoospermia/severe oligozoospermia of men. However, it is conceivable that evaluation of this gene polymorphism can not be used as a biomarker in diagnosis of men with idiopathic azoospermia/severe oligozoospermia.

    Background and Aim
    Radiation therapy is the destruction of cancer cells that in all patients with breast cancer reduces tumor recurrence, relieves pain in local tumors and metastases. There are different treatment methods around the world such as electron, photon alone or a combination of both types of fields.
    Materials and Methods
    In this study, photon therapy (PT) and mixed photon-electron therapy (MPET) were used to treat malignancies of the supraclavicular lymph nodes. 30 patients with right-sided breast cancer with local lymph node metastasis were recruited. The ISOgray software was utilized to collect data about treatment planning methods with PT and MPET. Findings: The maximum and mean delivered doses of radiation to the supraclavicular region were 52.08±1.64, 42.59±0.51 Gy and 54.24±1.64, 43.67±0.43 Gy in the PT and MPET methods, respectively. The mean irradiated volumes of supraclavicular fossa that received 90% of the radiation dose were 59.74±1.94% and 70.26±0.94% in the PT and MPET methods, respectively (p=0.004). The maximum doses delivered to the spine were 14.66±1.9 Gy and 10.22±0.92 Gy and the thyroid were 42.62±3.1 Gy and 37.67±5.02 Gy in the PT and MPET methods, respectively.
    Conclusion
    The maximum doses delivered to the spine and thyroid significantly diminished by the novel method. Additionally, supraclavicular region received higher maximum and mean doses in the new treatment modality compared to the conventional methods. The new method improved dose coverage for the tumor.

    Background and Aim
    Type 2 diabetes exerts an anti-angiogenesis effects on the cardiac tissue. The aim of present study was to investigate the effects of endurance training on gene expression of VEGF and VEGFR2 of cardiac tissue in type 2 diabetic male wistar rats.
    Materials and Methods
    36 male wistar rats were randomly divided into three groups, Diabetic Endurance Training (DET), Diabetic Control (DC) and Healthy Control (HC). Type 2 diabetes was induced by intraperitoneal injection of STZ. The endurance training included 10 weeks, 5 sessions per week running at speed of 27 m/min and intensity of 75% VO2max for 20-30min in 1st week and reached to 27 m/min and intensity of 75% VO2max for 60 min/day in 10th weeks. The gene expression of VEGF and VEGFR2 were examined by Real-Time PCR AND Western Blotting. Findings: The results of Real-Time PCR showed that the endurance training caused increase in VEGF mRNA (p<0.05) and VEGFR2 mRNA (p<0.05) compared to DC group. The results of Western Blotting also indicated that the endurance training induced increase in VEGF protein expression (p<0.05) compared to DC group and there was no significant change in expression of VEGFR2 protein (p>0.05).
    Conclusion
    Type 2 diabetes impairs the angiogenesis process in diabetic cardiac. It appears that endurance training has positive impact on angiogenesis and it can cause the improvement of diabetic cardiac.
