    معرفی تکنولوژی DNA نو ترکیب در سال 1970 عصر جدیدی در حوزه زیست شناسی گشود. با ارائه این تکنیک برای اولین بار محققان زیستی توانستند مولکول های DNA را به منظور ابداع راهکارهای درمانی جدید دستکاری نمایند. امروزه روش های هدفمند تغییر ژنوم سلول های زنده ابزار قدرتمندی در درمان بیماری های ژنتیکی فراهم نموده است. تکنیک CRISPR/Cas9 یکی از جدیدترین ابزارهای تغییر ژنوم است که بر پایه روش های ویرایش ژن سیستم ایمنی باکتری طراحی شده است. این تکنیک بسیار کارآمد، سریع و ساده می باشد که امید می رود تا بتوانیم هر نوع ژنی را از سلول حذف یا به اضافه نموده و یا تغییر دهیم. در این نوشتار اساس این تکنیک و کاربرد آن در درمان بیماری های ژنتیکی مورد بررسی قرار گرفته است.

  • بیماری های ناخن-بخش سوم / بدشکلی های عادتی، لکه های سفید و خطوط طولی ناخن

    در این بخش چند حالت از ناهنجاری های ناخن شامل: تیک عادتی، لکونیشیا و خطوط طولی روی ناخن مورد بحث قرار می گیرد. تیک عادتی بر روی ناخن ها، یک عادت نه چندان شایع است و اغلب موجب یک بدشکلی مشخص می گردد. آسیب معمولا بر یکی یا هر دو ناخن شست دست به واسطه انگشتان همان دست وارد می شود. انگشت در چین ناخنی دورسال قرار می گیرد و سپس به جلو در روی صفحه ناخن کشیده می شود و این عمل مکررا تکرار می گردد.
    لکونیشیا، به صورت لکه ها و یا خطوط سفید رنگ در صفحه ناخن دیده می شوند. این عارضه که نسبتا شایع است، در اثر آسیب های خفیف به ماتریکس ناخن پدید می آید. این لکه ها اهمیت بالینی ندارند و با رشد ناخن به بیرون رانده شده و نیازمند درمان نمی باشند.
    خطوط طولی روی ناخن، یک پدیده طبیعی می باشد و به طور خفیف در اکثر ناخن ها دیده می شود. با توجه به تفاوت جنس ناخن در افراد مختلف، میزان و شدت این خطوط در افراد متغیر است. اما بعضی عوامل مانند افزایش سن و بیماری های مزمن موجب تشدید این خطوط می شوند. این خطوط به خودی خود یک بیماری نیستند بلکه صرفا از نظر زیبایی ظاهر مناسبی به ناخن نمی دهند. اونیکورکسی به حالتی گفته می شود که خطوط طولی به همراه شکنندگی در صفحه ناخن مشاهده شده باشد.

    بیوفیلم میکروبی ساختاری از جامعه میکروب ها می باشد که به صورت متصل به سطوح زنده یا غیر زنده یافت می شوند. این ساختارهای سه بعدی همواره در داخل یک ماتریکس پیچیده محصور شده اند. تشکیل بیوفیلم در هر دو رده باکتری های گرم مثبت و گرم منفی مشاهده می شود. در حقیقت یکی از اصلی ترین مکانیسم های بقاء باکتری ها در محیط های مختلف استفاده از همین توان تولید بیوفیلم و زندگی در آن می باشد. شکل گیری آن ها طی چندین مرحله مختلف انجام می شود که طی آن باکتری از فرم پلانکتونی و آزاد در محیط به فرم متصل به سطوح و ثابت تبدیل می شود. در مسیر انجام این رخداد باکتری ها اقدام به تولید و ترشح ترکیباتی با محوریت پروتئین ها می کنند که عملا جامعه آن ها را در برابر شرایط محیطی ایزوله می کند. این ماتریکس قابلیت جذب سایر میکروب های موجود در محیط و در مجاورت خود را نیز دارد. عوارض گسترده ای ناشی از تولید بیوفیلم در مسیر سلامت و زیست انسان ها ایجاد می شود. درصد بالایی از بیماری های عفونی مزمن ناشی از توان بیوفیلم زایی باکتری ها است. همچنین به دلیل شرایط اختصاصی حاکم در محیط بیوفیلم درمان و استفاده از آنتی بیوتیک های علیه آن هم یکی از اصلی ترین چالش های پیش روی علم پزشکی محسوب می شود.

    آزمایشات مولکولی بیش از دو دهه می باشد که وارد آزمایشگاه های تشخیص طبی شده است و توانسته اند پاسخگوی بسیاری از معضلات تشخیصی بیماری های انسانی باشند. به همین لحاظ شاهد بوده ایم که حجم گسترده ای از تحقیقات در این حیطه صورت گرفته و شرکت های تجاری سرمایه گذاری عظیمی را به واسطه درخواست ها در این حوزه انجام داده اند. همچنان که حساسیت و ویژگی تست از اهمیت به سزایی برخوردار است، استاندار سازی و تکرار پذیر نمودن نتایج هر یک از تست های مولکولی نیز هیچگاه تا به این اندازه مکتوب نشده بود. معهذا مشاهده می شود که تعداد قابل توجهی از نتایج منفی گزارش شده مورد رضایت پزشکان نمی باشد. در این مقاله سعی شده علل این مهم و نقش کنترل ها به ویژه کنترل داخلی بررسی شود.

    کم خونی ناشی از فقر آهن (IDA) شایع ترین شکل کم خونی در سرتاسر جهان است. در مردان و زنان یائسه معمول ترین علت IDA خونریزی از ضایعات موجود در مجرای معدی، روده ای است که از علل شایع مراجعه به متخصص دستگاه گوارش می باشد. علل IDA مربوط به خونریزی و یا سوء جذب آهن می باشد. پس از تایید با آزمون های آزمایشگاهی، بررسی معدی، روده ای اغلب برای تشخیص افتراقی آن از بدخیمی معدی، روده ای به کار می رود. تیمار درمانی باید شامل جایگزینی فوری آهن به اضافه مراحل تشخیصی براساس علت اساسی IDA باشد. مکمل آهن خوراکی ارزان و موثر است اما درمان تزریقی ممکن است به علت عدم تحمل، عدم پذیرش یا شکست تیمار با درمان خوراکی، مورد نیاز باشد.

    افزایش کلی سن جمعیت و افزایش TB مقاوم به دارو نیاز به بهبود تشخیص سریع و روش های جدید جهت تشخیص بیماری سل و بیماری سل مقاوم به درمان را بیشتر می کند. تشخیص زودهنگام و دقیق بیماری سل ریوی باید با استفاده از اشعه X، بررسی میکروسکوپی خلط، کشت در هر دو محیط مایع و جامد و تکثیر اسید نوکلئیک انجام شود. سی تی اسکن سینه، آزمون هیستوپاتولوژی بیوپسی نمونه ها و آزمون های تشخیص مولکولی جدید می تواند جهت تشخیص زود هنگام بیماران به خصوص در بیماران با بیماری سل ریوی اسمیر منفی و بیماری سل مقاوم به دارو استفاده شود.

    The development of recombinant DNA technology in the 1970s marked the beginning of a new era for biology. For the first time, molecular biologists gained the ability to manipulate DNA molecules, making it possible to develop novel medicine. Nowadays, the introduction of targeted genomic sequence changes into living cells and organisms has become a powerful tool for biological research and provided a potential avenue for therapy of genetic diseases. Of these new technologies, the most exciting is CRISPR/Cas9, a gene editing system adopted from bacterial immune system that is efficient, rapid, and easy-to-use. CRISPR/Cas9 technology allows targeted knock-in and knock-out and/or changes of any gene within the genome. In this study, this technology and its applications in treatment of genetic disease was discussed.

    Habit-tic deformity is a form of nail dystrophy caused by habitual external trauma to the nail matrix. Although any nail can be affected, the thumbnails are often primary targets. Most patients report manipulating the proximal nail fold or periungual area with an adjacent fingernail, often in an unconscious manner. Characteristic changes to the nail include central depression and transverse, parallel ridging, both of which run from the nail fold to the distal edge of the nail. In severe cases, the cuticles may disappear, and the lunulae may hypertrophy. Habit-tic deformity lacks the nail-thickening and onycholysis of fungal infections of the nail. The principal differential diagnosis is median canaliform dystrophy, a more rare condition that manifests as a midline splitting of the nail with oblique ridging, giving a fir-tree–like appearance to the surface of the nail. In median canaliform dystrophy, the cuticle may retain a normal appearance.
    Leukonychia is a common nail condition which partial white spots are seen or in some cases, the entire nail discolours. There are several types of leukonychia including: Leukonychia totalis, Where the entire nail is affected; Leukonychia partialis, which parts of the nail are affected, this is further divided into: Leukonychia striata, also known as Muehrcke's lines, leukonychia striata is a whitening of the nail in bands or stripes. Leukonychia punctate, also known as "true leukonchia", this is the most common type of leukonchia. The most common cause of leukonchia is trauma to the matrix (base), this could be from a knock, pushing the cuticles too hard, picking & biting the nails, frequent application of false nails etc. New cells, growing at the root of the nail are white in appearance, as they grow out, they harden & flatten, becoming translucent. If trauma occurs, white spots may appear as the nail grows out. By this time, you have probably forgotten about the trauma. These spots resolve or fade as the nail grows out. Zinc deficiency is another common cause of leukonchia.
    Having vertical (longitudinal) ridges on the fingernails refers to the presence of tiny raised lines or ridges that run up and down the length of the nail. Ridges are among the different kinds of nail abnormalities that can develop due to aging, nail injury, or trauma, or underlying medical conditions. Many people notice an increase in vertical ridges of the fingernails as they age, and this can be a normal development. Aging has been described as the predominant cause of vertical ridges on the fingernails. Onychorrhexis is the medical term used to describe longitudinal ridges on the nails that often accompany brittle nails.

    Microbial biofilm are a structure of the microbial community that are found attached to living or non-living surfaces. These three-dimensional structures are always enclosed within a complex matrix. Biofilm formation is observed in both gram-positive and gram-negative bacteria. In fact, one of the main mechanisms of survival of bacteria in different environments is the use of biofilm production and living in it. Their formation takes place in several different stages, during which the bacteria form a planktonic and free form in the environment, converted to form on the surface and fixed. In the course of this event, bacteria produce and secrete protein-centered compounds that virtually isolate their community against environmental conditions. This matrix also has the ability to absorb other microbes in the environment and in its vicinity. The widespread effects of biofilm production on the health and life of humans are caused. A high percentage of chronic infectious diseases is due to the biofilm potential of bacteria. It is also considered as one of the main challenges facing medical science due to the specific conditions prevailing in the biofilm environment and the use of antibiotics against it.

    Molecular methods have been applied in diagnostic laboratories more than two decades. These tests are enabled to respond lots of unanswered questions and replying to many difficulties. Hence several researches have been carried on this field and many companies have specified huge investments for requests. Despite of this efforts sensitivity and specificity still have an important situation in designing in diagnostic protocols. Standardization and reproducibility of each test have not been documented such this up to now. But we are still facing with numerous negative results that are not satisfactory for the clinicians. The aim of this survey is to look at the role of controls and causes of subject at the final results of the diagnostic tests.

    Iron deficiency anaemia (IDA) is the most common form of anaemia worldwide. In men and postmenopausal women the commonest cause of IDA is blood loss from lesions in the gastrointestinal tract, making it a common cause of referral to gastroenterologists. Causes of IDA relate either to blood loss or iron alabsorption. After confirmation with laboratory tests, gastrointestinal evaluation is almost always indicated to exclude gastrointestinal malignancy.Treatment should involve prompt iron replacement plus diagnostic steps directed towards correcting the underlying cause of IDA. Oral iron replacement is cheap and effective, but intravenou) therapy may be required due to intolerance, oncompliance or treatment failure with oral therapy.

    Increases the rise of drug-resistant Mycobacterium tuberculosis have reinforced the need for rapid diagnostic improvements to detect TB and drug-resistant TB, as well as to improve TB control. An early and accurate diagnosis of pulmonary TB should be established using chest X-ray, sputum microscopy, culture in both liquid and solid media, and nucleic acid amplification. Chest computed tomography, histopathological examination of biopsy samples, and new molecular diagnostic tests can be used for earlier and improved diagnoses.
