    آنمی فقر آهن و آنمی ناشی از بیماری های مزمن، از شایع ترین کم خونی ها می باشند. اختلال در متابولیسم آهن با تصویرهای مختلف بالینی از قبیل کم خونی فقر آهن، گرانباری آهن، گرانباری آهن با آب مروارید و اختلالات نورولوژیک به ویژه کاهش دقت و تمرکز همراه است. تنظیم آهن بدن یک پروسه بسیار پیچیده است که در آن فاکتورهای مختلفی در تنظیم جذب و رها شدن آن از ماکروفاژها برای مصرف بافت خون ساز نقش دارند. جهش در فاکتورهای تنظیم کننده سطح آهن بدن یکی از عوامل مهم آنمی فقر آهن مقاوم به درمان (IRIDA) می باشد. در این مقاله سعی شده است که نگاه مولکولی به پروسه تنظیم آهن و آنمی فقر آهن مقاوم به درمان گردد.

    هدف از ارائه این مطالب که به صورت پی در پی در مجله آزمایشگاه و تشخیص درج می گردند آشنایی با بیماری های قارچی ناخن و روش های تشخیص صحیح آزمایشگاهی آن است. در شماره پیشین آناتومی و فیزیولوژی ناخن مرور گردید و سپس به مبحث آسیب شناسی ناخن پرداخته شد و ابتدا از بین ضایعات ناشی از تروما مسئله خونمردگی و خونریزی زیر بستر ناخن مورد بررسی قرار گرفت. در ادامه مبحث ضایعات ناشی از تروما، در این قسمت اونیکولیز و اونیکوگریفوزیس مورد بحث قرار می گیرد.

    سندرم تخمدان پلی کیستیک (PCOS)شایع ترین اختلال غدد درون ریز میان زنان در سنین باروری است و شایع ترین علت ناباروری ناشی از عدم تخمک گذاری است. اختلال تخمک گذاری، هایپرآندروژنیسم و ظاهر مرفولوژیک تخمدان های پلی کیستیک از ویژگی های کلیدی آن است. تظاهرات بالینی PCOS به شکل گسترده ای متفاوت است. زنان مبتلا به این سندرم اغلب برای اختلالات سیکل قاعدگی، تظاهرات بالینی هایپرآندروژنیسم و ناباروری به دنبال درمان اند. اختلالات قاعدگی که معمولا در PCOS مشاهده می شود شامل الیگومنوره، آمنوره و خون ریزی قاعدگی نا منظم و طولانی مدت است. نشانه های عمده هایپرآندروژنمی شامل هیرسوتیسم، آکنه و آلوپسی آندروژنیک است. این سندرم یک بیماری منحصر به دوره باروری و نوجوانی نیست، بلکه می تواند با تاثیرات مختلف در زندگی فرد مرتبط باشد. بیماران مبتلا به این سندرم در معرض خطر بیماری های متابولیک مانند مقاومت به انسولین، اختلال تحمل گلوکز، دیابت، اختلالات چربی، فشار خون بالا، بیماری قلبی و عروقی، افزایش خطر ابتلا به سرطان آندومتر و احتمالا سرطان های رحم و پستان هستند. با توجه به شیوع بالای PCOS، چالش ها و عوارض مرتبط با آن می تواند بار بهداشتی و اقتصادی فراوانی در کشورها ایجاد کند، بنابراین پزشکانی که مراقبت از این بیماران را بر عهده دارند در کنار درمان مسائل باروری و آرایشی بهداشتی بیماران PCOS، باید جهت جلوگیری از پیامد های بلند مدت آن نیز اقدام کنند. لذا در مقاله حاضر، ابتدا مروری اجمالی بر ویژگی های بالینی این سندرم خواهیم داشت سپس به پیامدهای بلند مدت آن می پردازیم.
    روش کار
    جست و جو با استفاده از پایگاه های اطلاعاتی pubmed، google scholar، Elsevier و SID با کلید واژه های سندرم تخمدان پلی کیستیک (PCOS)، اختلالات قاعدگی، هیرسوتیسم، آکنه، ناباروری، دیابت، بیماری قلبی عروقی، سرطان انجام شد و از میان مقالات منتشر شده از سال 2006 تا 2016، 37 مقاله بررسی شد.

    گزارش های توصیف خطرات مهم سلامتی به علت عدم کفایت وضیعت ویتامین D همچنان علاقه قابل توجهی در میان جوامع پزشکی و جوامع غیر حرفه ای ایجاد کرده است. تحقیقات اخیر درباره فعالیت های مختلف مولکولی سیستم ویتامین D شامل گیرنده هسته ای ویتامین D و گیرنده های دیگر برای 1،25-دی هیدروکسی ویتامین D و متابولیسم ویتامین D شواهدی ارائه کرده است که سیستم ویتامین D در میان طیف وسیعی از بافت ها مشابه دیگر گیرنده های هسته ای هورمون ها، فعالیت های بیولوژیکی انجام می دهد. این دانش، خواص درمانی مختلف و معقول از نظر فیزیولوژی ارائه می کند که ادعا می شود توسط ویتامین Dفراهم می گردد و از طرح های پیشنهادی برای انجام آزمایش های بالینی حمایت می کند. سیستم ویتامین D نقش حیاتی در حفظ کلسیم و فسفات پلاسما و هومئوستاز (ثبات و توازن) معدنی استخوان بازی می کند. شواهد اخیر تایید می کند که هومئوستازی کلسیم پلاسما فاکتور مهمی در تنظیم فعالیت ویتامین D دارد. فعالیت های ویتامین D در اسکلت شامل تحریک یا مهار باز جذب استخوان و مهار یا تحریک تشکیل استخوان است. سه نوع سلول عمده استخوان که اوستئوبلاست ها، استئوسیت ها و اوستئوکلاست ها هستند می توانند از طریق گیرنده هسته ای ویتامین D به ویتامین D پاسخ دهند و جهت فعال کردن گیرنده ویتامین D و تنظیم بیان ژن، 25- هیدروکسی ویتامین D را به 1،25- دی هیدروکسی متابولیزه می کنند. جذب کلسیم مواد غذایی با متابولیسم ویتامین D در سطح هردو بافت کلیه و استخوان میانکنش دارد این میانکنش به طور مستقیم یا فعالیت کاتابولیکی بر روی استخوان از طریق سیستم آندوکرینی زمانی که جذب کلسیم ناکافی است یا فعالیت آنابولیکی از طریق اتوکرین استخوان یا سیستم پاراکرین زمانی که جذب کلسیم کافی است، صورت می گیرد.

    ویبریو ولنیفیکوس باکتری گرم منفی میله ای شکل و غیر اسپور‏زای متعلق به خانواده Vibrionaceae می ‏باشد که تهاجمی ‏ترین گونه‏ ویبریو برای انسان در نظر گرفته شده است. این باکتری به عنوان پاتوژن منتقله از مواد غذایی محسوب می‏ شود که مصرف غذاهای دریایی از جمله صدف باعث ایجاد بیماری در انسان می ‏گردد. ویبریو ولنیفیکوس باعث ایجاد سه بیماری مهم در انسان می ‏شود که گاستروآنتریت (Gastroenteritis)، عفونت‏ های زخم (Wound infections) و سپتی سمی (septicemia) را شامل می ‏شود. این باکتری فاکتورهای ویرولانس متعددی تولید می‏کند که در ارتباط با بیماریزایی این باکتری است. کپسول، سیدروفورها، توکسین‏هایی مانند همولیزین، کلاژناز، پروتئاز، الاستاز، DNase، موسیناز، هیالورونیداز، فیبرینولیزین، لیپاز و فسفولیپاز از فاکتورهای ویرولانس این باکتری است. از آنجا که مصرف غذاهای دریایی روز به روز محبوبیت بیشتری پیدا می‏کند لذا بسته‏ بندی بهداشتی غذاهای دریایی و حمل و نقل مناسب برای جلوگیری از رشد باکتری ویبریو ولنیفیکوس ضروری به نظر می ‏رسد. از طرف دیگر افراد مستعد نیز نباید این محصولات را به صورت خام یا نیمه پخته بخورند، زیرا خطرات عفونت‏ های ناشی از ویبریو ولنیفیکوس برای آن ها تهدیدی بزرگ محسوب می ‏شود. از این رو جهت کنترل و پیشگیری از عفونت ‏های این باکتری در مواد غذایی دریایی که به عنوان مخزن این باکتری نیز محسوب می ‏شوند، وضع استانداردهای لازم امری ضروری به نظر می رسد.

    بروسلوز با بسیاری از تظاهرات بالینی همراه است که تشخیص آن را کاری دشوار ساخته است. حتی پس از گزارش اولین آزمون سرولوژیکی برای بروسلوز، تکنیک تشخیص قطعی بطور فعال جستجو می شود. رایج ترین روش های تشخیصی مورد استفاده بر پایه سرولوژی است که توانایی سرم (آنتی بادی) را برای آگلوتیناسیون مقدار استانداردی از Brucella abortus کشته شده (آنتی ژن) دارای زنجیر جانبی-O اندازه گیری می کنند. این آزمون ها به دلیل ایمن بودن به طور وسیعی استفاده می شوند. با این حال در این نوع آزمون ها به علت وجود باکتری های واکنش دهنده متقاطع، نتایج مثبت کاذب زیاد دیده می شود علاوه بر آن در شناسایی Brucella canis و Brucella ovis که فاقد زنجیر جانبی- O هستند کارایی ندارند. دیگر آزمون های مفید مانند بررسی مستقیم اسمیر که یک روش احتمالی است، شامل ایجاد اسمیر از سواپ واژنی، جفت یا جنین های سقط شده و رنگ آمیزی آن ها با روش زیل نلسون اصلاح یافته است. Coxiella burnetti می تواند موجب اشتباه در تشخیص شود بنابراین تایید آن باید از طریق کشت اختصاصی در محیط انتخابی انجام گیرد. کشت و ایزولاسیون ارگانیسم از نمونه های خونی یا بافتی تنها روش صحیح اما بدون حساسیت، باقی مانده است و نتایج آن به عمل اختصاصی آزمایشگاه و نحوه پیگیری فعال نتیجه کشت ها بستگی دارد. تلقیح حیوانات آزمایشگاهی ابزار مفیدی است اما در معرض تداخل با اسیدهای معده قرار دارد. به تازگی نشان داده شده است که واکنش زنجیره ای پلیمراز (PCR) آزمون مفید و بسیار حساسی است اما برای استفاده استاندارد آزمایشگاهی معتبر نیست. این مقاله نمونه های مفید و به طور خاصی تفاوت روش مرسوم و تکنیک های مولکولی بسیار پیشرفته برای تشخیص بروسلوز را مورد تاکید قرار می دهد.

    Iron deficiency anemia and anemia of chronic disorders are among the most common anemia. Disorders of iron metabolism lead to different clinical pictures such as iron deficiency, iron overloads, iron overloads with cataract and neurocognitive disorders. Body iron regulation is a highly complex process and different regulatory proteins are involved for iron absorption and iron release from macrophages toward hematopoietic tissues. Mutation with loss or gain of function of these regulatory proteins leads to iron overload or iron refractory iron deficiency anemia. This article briefly review molecular glance to iron metabolism and iron refractory iron deficiency anemia.

    In this section three kinds of traumatic nail disorders (onycholysis, onychauxis, and onychogryphosis) will be explained.
    Onycholysis is a common nail disorder. It is the loosening or separation of a fingernail or toenail from its nail bed. It usually starts at the tip of the nail and progresses back. Onycholysis can occur in many conditions, including psoriasis. In thyrotoxicosis it is thought to be due to sympathetic overactivity. It may also be seen in infections or trauma. Other causes includes: Idiopathic, Trauma, excessive manicuring, Infection: especially fungal, Skin disease: for example psoriasis, and dermatitis, Impaired peripheral circulation e.g. Raynauds, Systemic disease: hyper- and hypothyroidism, reactive arthritis, porphyria cutanea tarda, Sometimes a reaction to detergents (e.g. washing dishes with bare hands, using detergent-based shampoos or soaps). Patients with hepatocellular dysfunction may develop hair-thinning or hair loss and nail changes such as clubbing, leukonychia (whitening), or onycholysis, affecting the nails of the hands and feet.
    Onychauxis presents with thickened nails without deformity, and this simple thickening may be the result of trauma, acromegaly, Darier's disease, psoriasis, or pityriasis rubra pilaris, or, in some cases, hereditary.
    Onychogryphosis or onychogryposis is a hypertrophy that may produce nails resembling claws or a ram's horn. Onychogryphosis may be caused by trauma or peripheral vascular disease,Diabetes, and failure to cut the nails for extended periods of time. This condition is most commonly seen in the elderly.

    Polycystic ovary syndrome (PCOS) is the most common endocrine disorder among women of reproductive age and is the most common cause of infertility due to anovulation. Ovulatory dysfunction, hyperandrogenism and morphological appearance of polycystic ovaries are key features of this syndrome.Women with PCOS often seekmedical care for menstrual cycle disorders, clinical manifestations of hyperandrogenism and infertility. Menstrual disorders that are commonly seen in PCOS include oligomenorrhea, amenorrhea and prolonged irregular menstrual bleeding. The main clinical evidence of hyperandrogenism includes hirsutism, acneand androgenic alopecia. This syndrome is not a disease exclusive to fertility and adolescence period; even it can be associated with different effects on a patient's life. PCOS patients are at risk of type 2 diabetes, fat disorders, high blood pressure, cardiovascular disease and even endometrial cancer. There is also limited and contradictory evidence on the risk of ovarian and breast cancer in women with PCOS which requires further studies to prove it. Due to the high prevalence of PCOS, the complications and challenges associated with it can create a major health and economic burden in countries. Therefore, the physicians caring for these patients, along with treating fertility and cosmetic issues of patients with PCOS, should act to prevent it's long-term consequences. Therefore, in the present article, we will first give a brief overview of the common clinical features of this syndrome and then will discuss it's long-term consequences.
    Method
    The search was done by using pubmed, google scholar, Elsevier and SID databases with polycystic ovary syndrome (PCOS), menstrual disorders, hirsutism, acne, infertility, diabetes, cardiovascular disease and cancer keywords. Among articles that published from 2006 to 2016, 37 papers were reviewed.

    Reports describing significant health risks due to inadequate vitamin D status continue to generate considerable interest amongst the medical and lay communities alike. Recent research on the various molecular activities of the vitamin D system, including the nuclear vitamin D receptor and other receptors for 1,25-dihydroxyvitamin D and vitamin D metabolism, provides evidence that the vitamin D system carries out biological activities across a wide range of tissues similar to other nuclear receptor hormones. This knowledge provides physiological plausibility of the various health benefits claimed to be provided by vitamin D and supports the proposals for conducting clinical trials. The vitamin D system plays critical roles in the maintenance of plasma calcium and phosphate and bone mineral homeostasis. Recent evidence confirms that plasma calcium homeostasis is the critical factor modulating vitamin D activity. Vitamin D activities in the skeleton include stimulation or inhibition of bone resorption and inhibition or stimulation of bone formation. The three major bone cell types, which are osteoblasts, osteocytes and osteoclasts, can all respond to vitamin D via the classical nuclear vitamin D receptor and metabolize 25-hydroxyvitamin D to 1,25-dihydroxyvitamin D to activate the vitamin D receptor and modulate gene expression. Dietary calcium intake interacts with vitamin D metabolism at both the renal and bone tissue levels to direct either a catabolic action on the bone through the endocrine system when calcium intake is inadequate or an anabolic action through a bone autocrine or paracrine system when calcium intake is sufficient.

    Vibrio vulnificus is a gram-negative bacterium, rod-shaped, halophiles, non-sporulated and belongs to the Vibrionaceae family. Vibrio vulnificus is considered the most infectious and invasive of all the human pathogenic Vibrio’s. This bacteria is a food-borne pathogens and Consumption of raw oyster and other seafoods are implicated in outbreaks. Vibrio vulnificus can cause the three important diseases in human; gastroenteritis, septicemia and wound infection. V. vulnificus produces several virulence factors, that is associated with virulence: capsules, siderophores, and toxins such as hemolysins, collagenase, protease, elastase, DNase, mucinase, hyaluronidase, fibrinolysin, lipase, and phospholipase. Seafood consumption gained huge popularity day by day, so health packaging of seafood is essential and convenient transportation is required to prevent the growth of Vibrio vulnificus. On the other hand, susceptible people should not eat these products raw or lightly cooked, because infections caused by Vibrio vulnificus is a major threat for them. Thus, standard guidelines is essential for the control and prevention of infections by Vibrio vulnificus in seafoods.

    Brucellosis presents with many clinical manifestation that make its diagnosis a difficult task. The most widely used methods of diagnosis are based on serology. These tests are most commonly used because they are safe. Other useful tests include the direct smear examination which is a presumptive method that involves making smears from vaginal swabs, placentas or aborted foetuses, stained with the stamp modification of the Ziehl-Neelsen method Culture and isolation of the organism from blood or tissue samples has remained the only “unequivocal” method but lacks sensitivity, and its outcome depends on individual laboratory practices, and how actively the obtaining of cultures is pursued. More recently, the polymerase chain reaction (PCR) has been found to be a useful and more sensitive test, but has not been validated for standard laboratory use. This paper highlights useful samples and, especially the different conventional to more sophisticated molecular techniques for the diagnosis of brucellosis.
