ابتلا به HIV/AIDS یک معضل جهانی است که سه دهه از ظهور آن می گذرد (1) اما هنوز در بین دانشمندان علوم ذیربط مساله ای پیچیده و منحصربه فرد محسوب می شود. در حالی که HIV/AIDS از نظر پزشکی یک بیماری مزمن و کشنده تعریف می شود، از لحاظ اجتماعی به عنوان یک انگ اجتماعی تلقی شده که شان افراد مبتلا را مورد آماج خود قرار می دهد. به این معنی که فرد مبتلا به HIV در کنار تحمل بار درد و استرس ناشی از بیماری و ترس از مرگ ناچار است احساسات منفی دیگری نظیر طرد، پیش داوری، تحقیر و انواع تبعیض ها را از سوی جامعه تحمل نماید. به طوری که به نظر می رسد پیامدها و مشکلات ناشی از انگ اجتماعی مرتبط با HIV از خود بیماری دردناک تر باشد (2). از آنجا که بیماری HIV با رفتارهای قابل اجتنابی همچون رابطه جنسی غیرایمن و سوء مصرف مواد مخدر در ارتباط است، افراد مبتلا غالبا مسئول بیماری خود شناخته می شوند (3) و خصوصا عدم اطلاع رسانی صحیح رسانه ها موجب شده که این افراد به عنوان مجرم در جامعه شناخته شوند (4).
ترس از قضاوت یا تبعیض از سوی دیگران می تواند روش نگریستن افراد مبتلا به خود و نحوه تطابق با وضعیت بیماری را عمیقا تحت تاثیر قرار دهد. این تجربه ممکن است موجب شود افراد خود را مورد سرزنش قرار داده و سزاوار بدرفتاری بدانند (5) و حتی از حق درمان و استفاده از سایر مزایای اجتماعی دست بکشند. این امر با آثار مخربی بر پیشگیری، مراقبت و درمان HIV همراه است. انگ مرتبط با HIV با تعصبات فرهنگی و مسایل جنسیتی نیز در تعامل است. در سراسر جهان، وقتی زنی به HIV مبتلا می شود با برخوردهای تبعیض گرایانه تری نسبت به مردان مواجه می شود. از این رو بسیاری از زنانی که با HIV زندگی می کنند به دلیل ترس از طرد شدن، خشونت و حتی مرگ، وضعیت بیماری خود را از همسرانشان مخفی می کنند. این امر منجر به از دست رفتن فرصت پیشگیری از عفونت همسر می شود و همچنین امکان دسترسی به خدمات مراقبتی و درمانی مورد نیاز را از زنان سلب می کند (6).
در واقع یک ارتباط چرخشی بین انگ اجتماعی و HIV وجود دارد؛ افرادی که انگ و تبعیض را تجربه می کنند به حاشیه رانده شده و نسبت به ابتلای HIV آسیب پذیرتر می شوند. از سوی دیگر، افرادی که به HIV مبتلا هستند بیش از دیگران مستعد تجربه انگ و تبعیض می باشند (7). سازمان جهانی بهداشت، ترس از انگ و تبعیض را به عنوان مهمترین مانع برای انجام آزمایش HIV، آشکار کردن وضعیت HIV و دریافت دارو معرفی کرده است. گزارش شده است که در 35 درصد کشورها، بیش از 50 درصد مردم دارای نگرش های منفی و تبعیض آمیز نسبت به مبتلایان به HIV هستند (8) و تقریبا از هر هشت نفری که به HIV مبتلا هستند، یک نفر به دلیل ترس از انگ و تبعیض از خدمات بهداشتی استفاده نمی کند. در سال 2016، 60 درصد کشورها گزارش کردند که نگرش های تبعیض آمیز کارکنان بهداشتی به معتادین تزریقی مانع از ارایه کافی خدمات پیشگیرانه HIV به این افراد می گردد (9). انگ و تبعیض نسبت به افرادی که با HIV زندگی می کنند در میان کارکنان خدمات بهداشتی در تمام سطوح شایع بوده و به اشکال مختلفی دیده می شود. کارکنان بهداشتی ممکن است تماس خود با مبتلایان به HIV و مراقبت از این بیماران را کاهش دهند، درمان آنها را به تعویق اندازند یا افراد مبتلا به HIV را از سایر بیماران ایزوله کنند. این در حالی است که عدم تمایل افراد به انجام آزمایش HIV موجب تاخیر در تشخیص بیماری، پیشرفت بیماری به مرحله AIDS و درمان ناموثر آن شده و احتمال انتقال عفونت به دیگران و مرگ زودهنگام را افزایش می دهد (7).
با وجود پیشرفت های زیادی که در زمینه شناخت HIV و روش های انتقال آن صورت گرفته اما تلاش در زمینه کاهش انگ و برچسب زدن بر این افراد کافی نبوده است (10،11). این در حالی است که برخورد موثر با انگ و تبعیض مرتبط با HIV که مانعی عمده در مسیر اقدام هماهنگ در سطوح جهانی، منطقه ای، ملی و اجتماعی است، یک الزام حقوق بشری می باشد. بنابراین به کارگیری برنامه های اختصاصی که بر حقوق بیماران مبتلا به HIV تاکید دارند راهی شناخته شده برای ریشه کنی انگ می باشد (12). از آنجا که انگ پدیده ای است که به طور اجتماعی ساخته می شود (2) ، ضروری است که رویکردی همه جانبه برای مقابله با آن اتخاذ گردد. از این رو مقابله با انگ باید در سطوح مختلف فردی، اجتماعی، سازمانی، و سیاست گذاری مورد توجه قرار گیرد. در سطح فردی، بالا بردن آگاهی افراد در خصوص انتقال، مراقبت و پیشگیری از HIV و دسترسی به خدمات و حقوق قانونی ضروری است. همچنین اهمیت حمایت های اجتماعی از مبتلایان به HIV نباید نادیده گرفته شود. در برنامه های آموزشی کارکنان خدمات بهداشتی باید محرک های فرهنگی ایجاد انگ و تبعیض از قبیل ترس های شخصی از عفونت، تعصب نسبت به گروه های مستعد و آسیب پذیر و سوء برداشت یا عدم آگاهی در مورد انتقال HIV، درمان و پیشگیری از آن مورد توجه دقیق قرار گیرند. رسانه ها باید آگاهی مردم در مورد HIV و آثار آن بر زندگی افراد را افزایش دهند و با برجسته کردن این موضوع که افراد مبتلا به HIV نیز همچون سایر انسان ها از حقوق انسانی برخوردارند، با تغییر نگرش مردم با انگ و تبعیض مرتبط با HIV مقابله نمایند. همچنین در سطح سیاست گذاری لازم است تبعیض ها در استخدام، آموزش، خدمات بهداشتی و سایر حیطه ها برای افراد مبتلا به HIV رفع گردد. در واقع، ریشه کنی انگ و تبعیض که مانع بزرگی در راه دستیابی به خدمات درمانی و پیشگیرانه است، کلید پایان دادن به همه گیری HIV است.کلیدواژگان: انگ، HIV -
صفحات 6-16زمینه و هدفسرطان پیشرفته نه تنها منجر به مشکلاتی برای بیماران می شود، بلکه برای مراقبین بیمار نیز دشواری هایی به همراه دارد. پژوهش حاضر با هدف تعیین نقش مقابله ی مذهبی در پیش بینی فشار مراقبتی در مراقبین بیماران مبتلا به سرطان مراحل انتهایی انجام گرفت.روش بررسیپژوهش حاضر از نوع مقطعی- همبستگی می باشد. تعداد 146 نفر از مراقبین بیماران مبتلا به سرطان که از فروردین تا شهریور 1396 به بخش طب تسکینی بیمارستان فیروزگر تهران مراجعه کرده بودند، به روش نمونه گیری در دسترس وارد مطالعه شدند. برای گردآوری اطلاعات از سیاهه ی فشار مراقبتی Novak و Guest و پرسشنامه ی مقابله ی مذهبی Pargament استفاده شد. داده ها با استفاده از نرم افزار SPSS نسخه 16 و روش های آماری همبستگی پیرسون و رگرسیون چندگانه به روش همزمان مورد بررسی و آزمون قرار گرفت.یافته هایافته ها نشان داد که میان سبک مقابله ی مذهبی مثبت با فشار مراقبتی رابطه ی معنی داری وجود ندارد اما سبک مقابله ی مذهبی منفی با فشار مراقبتی دارای رابطه ی مستقیم و معنی دار می باشد و میزان همبستگی آن برابر با 34 درصد است. همچنین، سبک مقابله ی مذهبی منفی (444/4t=) ، (352/0β=) به طور معنی داری، فشار مراقبتی را پیش بینی می کند ولی سبک مقابله ی مذهبی مثبت (438/0t=) ، (035/0β=) تبیین معنی داری از فشار مراقبتی ندارد.
نتیجه گیری کلی: با توجه به نتایج مطالعه، سبک مقابله ی مذهبی مثبت بر خلاف سبک مقابله ی منفی، قادر به پیش بینی تحمل فشار در مراقبین نمی باشد. بنابراین، آموزش مهارتها ی مقابلهای با تمرکز بر کاهش و اصلاح استفاده از راهبردهای مقابله ی مذهبی منفی در مراقبین بیماران مبتلا به سرطان، می تواند در کاهش فشار مراقبتی آن ها مناسب باشد.کلیدواژگان: فشار مراقبتی، مقابله مذهبی، مراقبین، سرطان -
صفحات 17-27زمینه و هدفمهارت های ارتباطی سنگ بنای قابلیت های بالینی برای ارائه دهندگان خدمات مراقبی هستند. یکی از ابعاد ارتباط در بالین فرآیند انتقال خبر ناگوار است که از جزئیات روانشناختی ظریفی برخوردار بوده و همواره یک فرآیند چالش برانگیز برای تیم درمان است. این پژوهش با هدف سنجش میزان آگاهی دانشجویان پرستاری از نحوه انتقال خبر ناگوار به بیماران و همراهان آن ها در بیمارستان های دانشگاه علوم پزشکی بوشهر انجام شد.روش بررسیاین پژوهش توصیفی- تحلیلی که به صورت مقطعی در سال 1396 انجام شده است، آگاهی دانشجویان پرستاری دانشگاه علوم پزشکی بوشهر را از نحوه انتقال خبر ناگوار به بیماران و همراهان آنها می سنجد. داده ها از طریق پرسشنامه محقق ساخته جمع آوری و در نرم افزار. SPSS نسخه 18 با استفاده از آمار توصیفی و تحلیلی مورد تجزیه و تحلیل قرار گرفت.یافته هاآنالیز داده ها نشان داد که دانشجویان پرستاری از سطح آگاهی متوسط به بالایی برخوردار بودند. تفاوت آماری معنی داری نیز در میزان آگاهی بر حسب گذراندن واحد درسی اخلاق پرستاری و ارتباط حرفه ای (011/0P=) و سال تحصیل (037/0P=) دیده شد. به این صورت که دانشجویانی که این واحد را گذرانده و در سال پایین تر تحصیلی قرار داشتند نمره بالاتری کسب کرده بودند. با این وجود، در میزان آگاهی بر حسب جنسیت (550/0P=) و علاقه به رشته (546/0P=) تفاوت معنی داری مشاهده نشد.
نتیجه گیری کلی: داشتن آگاهی در زمینه اخلاق حرفه ای و توانایی ارتباط موثر برای کادر درمان یک برتری محسوب می شود. با توجه به نتایج به دست آمده می توان امیدوار بود با ارائه دروس مرتبط با اخلاق و ارتباط در آموزش پرستاری و همچنین تدوین برنامه های آموزشی بیشتر در زمینه مهارتهای اجتماعی و ارتباطی توسط برنامه ریزان و مسئولان آموزش علوم پزشکی بتوان به نتایج مطلوب تری در این زمینه دست یافت.کلیدواژگان: خبر ناگوار، ارتباطات بهداشتی، اخلاق حرفه ای، پرستاری، آگاهی، بیمار -
صفحات 28-37زمینه و هدفبا توجه به اهمیت مدیریت درد در بیماران مبتلا به سوختگی، این مطالعه با هدف تعیین تاثیر رایحه درمانی ماساژی با روغن معطر ترکیبی اسطوخودوس و بابونه بر شدت درد زمینه ای بیماران مبتلا به سوختگی انجام شد.روش بررسیدر یک مطالعه نیمه تجربی، در بازه زمانی دیماه 1396 تا اردیبهشت 1397، 105 بیمار مبتلا به سوختگی، بستری در مرکز آموزشی درمانی شهید مطهری که دارای شرایط ورود به مطالعه بودند به روش نمونه گیری در دسترس وارد مطالعه شده و به روش قرعه کشی و بلوک چهارتایی با کارت در سه گروه کنترل، ماساژ با دارونما و ماساژ با روغن معطر ترکیبی قرار گرفتند. مداخلات در طول یک هفته در سه جلسه، قبل از خواب و هر بار به مدت 20 دقیقه انجام شد و گروه کنترل فقط تحت مراقبت های روزانه قرار گرفتند. داده ها با استفاده از مقیاس دیداری درد (VAS) جمع آوری شد و در نرم افزار آماریSPSS نسخه20 مورد تجزیه و تحلیل قرار گرفت. برای تحلیل داده ها از آمار توصیفی (فراوانی، درصد فراوانی، میانگین و انحراف معیار) و برای تعیین معنی داری از آمار استنباطی (کای دو، دقیق فیشر، آنالیز واریانس و تی زوجی) استفاده شد.یافته هاقبل از مداخله بین سه گروه از لحاظ شدت درد تفاوت معنی دار آماری وجود نداشت (746/0P =). در حالی که بعد از مداخله بین سه گروه از لحاظ شدت درد تفاوت معنی دار آماری مشاهده شد (001/0P<). به طوری که میانگین شدت درد در گروه روغن ترکیبی نسبت به دو گروه دیگر و دارونما نسبت به کنترل کمتر بود.
نتیجه گیری کلی: از آنجایی که رایحه درمانی ماساژی به عنوان روشی غیردارویی، ساده و بدون عارضه توانسته است منجر به کاهش درد زمینه ای در بیماران مبتلا به سوختگی شود، پیشنهاد می گردد که پرستاران و تیم مراقبت بهداشتی سوختگی از این روش در برنامه مراقبتی روزانه این بیماران استفاده کنند.کلیدواژگان: رایحه درمانی، ماساژ، بابونه، اسطوخودوس، درد زمینه ای، سوختگی -
صفحات 38-48زمینه و هدفسقوط بیمار در سیستم مراقبتی یکی از عوامل تهدید کننده ایمنی بیمار است. پرستاران جایگاه مهمی در حفظ و ارتقاء شاخص های ایمنی بیماران دارند. "شرایط محیط کار" پرستار از عوامل مهم تاثیر گذار در این زمینه است. مطالعه حاضر با هدف تعیین ارتباط شرایط محیط کار پرستار با میزان وقوع سقوط بیماران انجام شده است.روش بررسیدر این مطالعه همبستگی، تعداد 300 نفر از پرستاران شاغل در بیمارستان های وابسته به دانشگاه علوم پزشکی تهران به روش نمونه گیری طبقه ای متناسب در سال 1395 انتخاب شدند. جهت جمع آوری اطلاعات از یک پرسشنامه سه قسمتی شامل اطلاعات جمعیت شناختی، پرسشنامه شرایط محیط کار پرستار و پرسشنامه سقوط بیمار استفاده شد. داده ها با استفاده از نرم افزاز SPSS نسخه 16 و آزمون پیرسون و کروسکال والیس و تی مستقل و جداول فراوانی تحلیل شدند.یافته هامیانگین سقوط بیماران در طی سه ماه (61/1 ± 6) و میانگین نمره حاصل در این پژوهش برای محیط کار پرستار 34/67 (مطلوب) به دست آمد اگر چه یک ارتباط ضعیف اما معنی دار بین شرایط محیط کار پرستاری و وقوع سقوط بیماران دیده شد )340/0 (P< (15/. -=r).
نتیجه گیری کلی: با توجه به ارتباط معکوس و معنی دار بین میزان وقوع سقوط بیمار و شرایط محیط کار پرستار، به نظر می رسد ضروری است مدیران پرستاری با استراتژی های مژثر جهت ارتقاء شرایط کاری نظیر مشارکت پرستاران در تصمیم گیری ها، حمایت و کفایت نیروها و منابع، زمینه را برای مراقبت ایمن از بیماران و پیشگیری از سقوط، فراهم آورند.کلیدواژگان: ایمنی بیمار، سقوط بیمار، محیط کار، پرستار -
صفحات 49-64زمینه و هدفپرستاران از مهم ترین اعضای تیم مراقبت بهداشتی و درمانی هستند که مسئولیت مراقبت مستقیم از سالمندان را به عهده دارند. یکی از ارکان اصلی در ارائه هر مراقبت، داشتن نگرش مثبت در مورد دریافت کنندگان مراقبت است. کیفیت ارائه خدمت به سالمندان به خصوص سالمندان مبتلا به دلیریوم تحت تاثیر عوامل مختلفی از جمله نگرش پرستاران نسبت به این گروه است. این پژوهش نگرش پرستاران در چهار حیطه دانش، توانایی و اعتماد به نفس، قابلیت شناسایی و درک پیامدهای آن، تمایل به دانستن بیشتر و بار مراقبتی را نسبت به سالمندان مبتلا به دلیریوم بررسی نمود.روش بررسیاین مطالعه مقطعی از نوع توصیفی- تحلیلی بود. واحدهای مورد مطالعه با استفاده از روش نمونه گیری طبقه ای با تخصیص متناسب به صورت در دسترس 200 نفر از پرستارانی بودند که معیارهای ورود به مطالعه را داشتند و در سه مراکز آموزشی و درمانی دانشگاه علوم پزشکی ایران مشغول به کار بودند. ابزار گردآوری داده ها پرسشنامه مشخصات فردی و ابزار نگرش پرستاران نسبت به دلیریوم با 38 سوال بود. داده ها با نرم افزار SPSS نسخه 16 تجزیه و تحلیل شد و مقادیر 05/0 P≤معنی دار در نظر گرفته شد.یافته هانتایج مطالعه نگرش پرستاران نسبت به مراقبت از سالمندان مبتلا به دلیریوم نشان داد نگرش سرپرستاران به طور معنی داری (005/0P=) بهتر از پرستاران است. نتایج آنالیز واریانس اختلاف معنی دار آماری (003/0P=) را با متغیر سابقه کار با سالمند نشان داد. همچنین سمت شغلی (سرپرستار) تنها متغیری بود که در مدل رگرسیون خطی معنی دار شد.
نتیجه گیری کلی: در این مطالعه، سمت شغلی و سابقه کار با سالمند بیشترین ارتباط معنی دار را با نگرش پرستاران نشان داد. برنامه ریزی کاربردی آموزش مداوم، مبتنی بر شواهد و عملکرد جهت ارتقاء نگرش و عملکرد پرستاران شاغل انجام شود. همچنین مراقبت از بیماران سالمند به کادری آموزش دیده، مجرب، مشتاق و علاقه مند واگذار شود.کلیدواژگان: دلیریوم، نگرش، مراقبت سالمندی -
صفحات 65-75زمینه و هدفسرطان، یکی از مهم ترین علل مرگ در دنیا است. بسیاری از بیماران مبتلا به سرطان در انتهای زندگی در بیمارستان بستری می شوند و نیازمند مراقبت های پایان حیات هستند. درک پرستاران از مراقبت های پایان حیات منجر به اصلاح رفتارهای نامناسب مراقبتی و ارتقای کیفیت مراقبت از این بیماران می شود. این مطالعه با هدف درک پرستاران از مراقبت های پایان حیات در بیماران مبتلا به سرطان انجام شد.روش بررسیاین مطالعه با رویکرد کیفی و به شیوه تحلیل محتوای قراردادی در سال 1396 انجام شد. مشارکت کنندگان 16 پرستار شاغل در بخش های سرطان بودند که با نمونه گیری هدفمند وارد مطالعه شدند. روش گرد آوری داده ها مصاحبه های نیمه ساختارمند بود. تمام مصاحبه ها پیاده سازی و بازخوانی شدند و به شیوه تحلیل محتوای قراردادی مورد تحلیل قرار گرفتند.یافته هادرک پرستاران از مراقبت های پایان حیات تحت عنوان طبقه اصلی مراقبت جامع نگر و سه زیر طبقه "مراقبت ارزش محور"، "مراقبت حمایت محور"و "عوامل موثر بر نحوه مراقبت" مورد شناسایی قرار گرفت.
نتیجه گیری کلی: دید کل نگر مراقبتی پرستاران در مراقبت های پایان حیات که نشات گرفته از درک آنان از مراقبت این بیماران است می تواند به آنها در ارائه مراقبت با کیفیت که مبتنی بر ارزش، حمایت و توام با مراقبت پرستاری است، کمک نماید. به نظر می رسد این شیوه مراقبت می تواند احساس خوب مراقبت را برای پرستاران و مرگ آرام را برای بیماران بدنبال داشته باشد.کلیدواژگان: مراقبت پایان حیات، سرطان، پرستار -
صفحات 76-85زمینه و هدفکارکنان پرستاری در محیط کار با از عوامل استرس زای فیزیکی و روانی فراوانی روبرو هستند. شناسایی میزان استرس پرستاران و کنترل تنش در محیط کار باعث بهبود کیفیت مراقبت از بیمار می گردد. هدف از پژوهش حاضر شناسایی ابعاد استرس شغلی پرستاران شاغل در بخش های مختلف مراکز درمانی دانشگاه علوم پزشکی گیلان بود.روش بررسیدر این مطالعه تحلیلی- مقطعی که در سال 1396 انجام شد، تعداد 324 پرستار با استفاده از روش نمونه گیری خوشه ایچند مرحله ای انتخاب شدند. در ابتدا 12 مرکز درمانی منتخب از سه خوشه مرکز، غرب و شرق استان گیلان مد نظر قرار گرفت و سپس با توجه به سهم هر مرکز درمانی از حجم نمونه مورد نیاز، نمونه ها به صورت تصادفی از بخش های مختلف (اورژانس، داخلی، جراحی، کودکان، دفتر پرستاری، CCU، ICU، همودیالیز و اتاق عمل) انتخاب شدند. ابزار پژوهش پرسشنامه استرس شغلی بیمارستانی(HSS-35) مشتمل بر 35 سوال و 11 زیرمقیاس بود. تجزیه و تحلیل داده ها از طریق آمار توصیفی و آنالیز واریانس با استفاده از نرم افزار SPSS نسخه 19 انجام شد.یافته هابیشتر نمونه های مورد پژوهش (8/39%) در گروه سنی 40-31 قرار داشته و از نظر جنسیت بیشتر آن ها (96%) زن، متاهل (2/77%) و دارای مدرک کارشناسی (2/93%) بودند. از نظر میانگین نمره استرس کلی، کارکنان بخش اورژانس (با میانگین و انحراف معیار 13/16 ± 17/120) از بیشترین میزان استرس و کارکنان بخش ICU (با میانگین و انحراف معیار 01/19 ± 14/114) از کمترین استرس برخوردار بودند. این تفاوت از نظر آماری معنی دار نبود (05/0P>). از نظر شدت ابعاد استرس، بعد "ابهام نقش" (با میانگین و انحراف معیار 63/0 ± 58/3) و بعد"استرس شیمیایی" (با میانگین و انحراف معیار 26/1 ± 12/3) به ترتیب بیشترین و کمترین شدت استرس زایی را دارا بودند.
نتیجه گیری کلی: با توجه به نتایج مطالعه مبنی بر وجود بیشترین میزان استرس بیمارستانی در پرسنل بخش های اورژانس و کودکان لزوم توجه بیشتر مسئولین ذیربط به کارکنان این بخش ها در جهت برنامه ریزی برای کاهش استرس بیمارستانی و آموزش بهترین روش های مقابله ای احساس می گردد.کلیدواژگان: استرس شغلی، پرستار، بیمارستان

HIV/AIDS is a global health concern that spans three decades of its emergence(1). However, it remains a complex, unique issue among experts. While HIV/AIDS is medically recognized as a chronic, fatal disease, there is a social stigma attached to the disease, which targets the dignity of the patients. In addition to the burden of the disease, stress, and fear of death, an HIV-infected patient is faced with negative emotions, such as social rejection, prejudice, humiliation, and discrimination, so that HIV-related stigma seems to be more burdensome for the patients than the disease(2). Since HIV infection is associated with avoidable behaviors (e.g., unsafe sexual relations and substance abuse), the patients are often responsible for their own illness(3). On the same note, the failure of the media to properly inform people has led the populace to regard AIDS immoral(4).
The fear of judgment or discrimination from others deeply affects the self-image of the patients with HIV infection, as well as their coping abilities. This experience may cause self-blame in the patients, who may, as a result, presume themselves to be deserving of maltreatment(5). They may even give up their rights to treat and receive other social benefits, which is associated with devastating effects on HIV prevention, care, and treatment. HIV-related stigma also interacts with cultural prejudices and gender issues. Globally, when a woman is infected with HIV, her status subjects her to more discriminatory treatment compared to a man. Therefore, many women living with HIV tend to hide their status from their spouses due to the fear of rejection, violence, and even death. This leads to the loss of the opportunity to prevent the infection of the spouse, and also deprives women of access to the necessary care and treatment(6).
There is a cyclical relationship between stigma and HIV; the individuals who experience stigma and discrimination due to HIV infection are marginalized, thereby becoming more vulnerable to HIV. On the other hand, patients with HIV are more prone to discrimination and stigma than others(7). The World Health Organization has identified the fear of stigma and discrimination as the main barriers to HIV testing, HIV status disclosure, and access to HIV treatment. It is reported that in 35% of countries, more than 50% of the population have discriminatory attitudes toward HIV patients(8), and about one out of eight peopleinfected with HIV does not use health services due to the fear of discrimination and social stigma. Text Box: 4In 2016, 60% of the countries across the world reported the discriminatory attitude of healthcare providers toward intravenous drug users, which in turn discourages them to provide adequate services for HIV prevention to these patients(9). HIV-related stigma and discrimination have been frequently observed in healthcare professionals at all levels, and the attitudes are manifested in a variety of manners. Healthcare providers may minimize their contact with HIV, neglect their proper care, delay their treatment, and isolate these individuals from other patients. However, the unwillingness of people to receive HIV testing delays the diagnosis of the disease, leading to its progression to the stage of AIDS and ineffective treatment, thereby increasing the risk of infection transmission and untimely death(7).
Despite the advancement in the detection of HIV and its transmission, there have not been sufficient effort to reduce the social stigma and labeling attached to this disease(10,11). Management of HIV-related stigma and discrimination is a major impediment to the coordinated action at global, regional, national, and social levels, as well as a human rights obligation. Implementing specific programs that emphasize on the rights of HIV patients is a well-known way of eradicating HIV-related stigma(12).
Stigma is a social construction(2), and it is imperative to adopt a comprehensive approach to deal with it. Therefore, tackling with stigma should be taken into consideration at the individual, social, organizational, and policy-making levels. At the individual level, raising public awareness regarding HIV transmission, care, and prevention, while ensuring access to proper services and legal rights is absolutely essential. Another influential factor in this regard is providing social support to HIV patients. In the educational programs for healthcare providers, the cultural stimuli for the establishment of HIV-related stigma and discrimination (e.g., fear of infection, prejudice toward vulnerable groups, and misconceptions/lack of knowledge regarding HIV transmission, treatment, and prevention) should be considered. Furthermore, the media should raise public awareness regarding HIV and its effects on the lives of people, thereby tackling HIV-related stigma through changing the false attitudes and emphasizing on the fact that patients with HIV have the same human rights as others. At the policy-making level, factors such as discrimination in employment, education level, health service provision, and other needs of HIV patients must be addressed. Eradicating the discrimination and stigma attached to HIV infection, which is a major obstacle to access to health and preventive services, is the key to stopping the HIV epidemic.Keywords: Stigma, HIV -
Religious Coping as a Predictor of the Burden of Care in the Caregivers of End-stage Cancer PatientsPages 6-16Background & AimsAdvanced cancer causes problems not only for the patients, but also for their caregivers. The present study aimed to determine the role of religious coping in the prediction of the burden of care in the caregivers of end-stage cancer patients.Materials & MethodsThis cross-sectional, correlational study was conducted on 146 cancer caregivers referring to the palliative care center of Firoozgar Hospital in Tehran, Iran during April-September 2017. The participants were selected via availability sampling. Data were collected using Novak and Guest’s caregiver burden inventory (CBI) and Pargament’s religious coping questionnaire (RCOPE). Data analysis was performed in SPSS-16 using Pearson’s correlation-coefficient and multiple regression analysis.ResultsNo significant correlation was observed between the positive religious coping style and burden of the caregivers. However, the negative religious coping style had a direct, significant correlation with the burden of the caregivers (34%). Moreover, the negative religious coping style could significantly predict the burden of the caregivers (t=4.444; β=0.352), while the positive religious coping style could not significantly predict the burden of the caregivers (t=0.438; β=0.035).ConclusionAccording to the results, contrary to the negative religious coping style, the positive religious coping style could not predict the burden of care in the caregivers of end-stage cancer patients. The process of delivering bad news is one of the most challenging aspects of clinical communication in health care, which has subtle psychological details. The present study aimed to assess the knowledge of nursing students regarding the conveying of bad news to patients and their companions in the hospitals affiliated to Bushehr University of Medical Sciences, Iran.Materials & MethodsThis cross-sectional, descriptive-analytical study was conducted in 2017 in order to assess the knowledge of nursing students at Bushehr University of Medical Sciences regarding the delivery of bad news to patients and their companions. Data were collected using a valid and reliable researcher-made questionnaire. Data analysis was performed in SPSS version 18 using descriptive and analytical statistics.ResultsThe nursing students had moderate to high knowledge of delivering bad news to patients. In addition, significant associations were observed between the knowledge level, passing the courses of nursing ethics and professional communication (P=0.011), and academic year of education (P=0.037), so that the students who had passed the courses and were in inferior academic years had higher knowledge levels in this regard. However, no significant correlation was observed between the knowledge level, gender (P=0.550), and interest in the academic discipline (P=0.546).ConclusionAdequate knowledge of professional ethics and effective communication is an advantage for healthcare providers. Pages 28-37Background & AimsGiven the importance of pain management in burn patients, the present study aimed to assess the effect of aromatherapy massage with aromatic oils of lavender and chamomile on the intensity of the background pain in burn patients.Materials & MethodsThis quasi-experimental study was conducted on 105 patients with burn injuries admitted to Shaheed Motahari Hospital in Tehran, Iran during January-May 2018. The patients were selected via convenience sampling and allocated to three groups of control, placebo massage, and combined aromatic oil massage by drawing lots and blocks of four with cards. The interventions were performed 20 minutes before bedtime in three sessions for one week. The control group only received routine care. Data were collected using the visual analogue scale (VAS). Data analysis was performed in SPSS version 20 using descriptive statistics (frequency, frequency percentage, mean, and standard deviation), and inferential statistics (Chi-square, Fisher’s exact test, analysis of variance, and paired t-test) were applied to determine the significance.ResultsBefore the interventions, the three groups had no significant difference in terms of the pain score (P=0.746), while after the intervention, the results indicated a significant difference between the three groups in terms of the pain score, (P<0.001), so that the mean pain intensity was lower in the combination oil massage group compared to the other groups, as well as the placebo group compared to the control group.ConclusionConsidering that aromatherapy massage could effectively improve the background pain in burn patients as a non-pharmacological, simple method, it is recommended that nurses and burn injuries medical teams apply this technique in the routine care program of these patients.Keywords: Aromatherapy Massage, Chamomile, Lavender, Pain, Burn

Pages 38-48Background & AimsPatient falls are among the most significant threats to patient safety. Nurses play a pivotal role in maintaining and improving the standards of patient safety. The work environment of nurses is among the major influential factors in this regard. The present study aimed to investigate the association between the incidence of patient falls and work environment of nurses.Materials & MethodsThis correlational study was conducted on 300 nurses employed in the hospitals affiliated to Tehran University of Medical Sciences, Iran, who were selected via proportionate stratification sampling. Data were collected using a questionnaire with three sections of demographic data, work environment of nurses, and patient falls. Data analysis was performed in SPSS version 16 using Pearson’s correlation-coefficient, Kruskal-Wallis test, t-test, and frequency tables.ResultsMean age of the participants was 32±6.17 years, and the majority were female (86.7%) and married (64%). Mean incidence of patient falls was 6±1.61 within the past three months, and the mean score of the work environment of nurses was estimated at 67.34 (favorable). A significant, weak correlation was observed between the work environment of nurses and incidence of patient falls (P<0.034; r=-0.15).ConclusionGiven the significant association between the incidence of patient falls and work environment of nurses, it seems essential that nursing managers adopt effective strategies to improve the working conditions of nurses through the participation of nurses in decision-making, support, and adequate staff and resources, thereby facilitating safe patient care.Keywords: Patient Safety, Patient Falls, Work Environment, Nurse

Positive attitude toward the recipients of care plays a fundamental role in the provision of care. The quality of care provision to the elderly, especially those with delirium, is influenced by several factors, including the attitude of nurses toward this patient group. The present study aimed to assess the attitude of nurses toward the care of elderly patients with delirium in the dimensions of knowledge, ability and self-confidence, ability to identify and understand the consequences, willingness to enhance knowledge, and burden of care.Materials & MethodsThis cross-sectional, descriptive-analytical study was conducted on 200 nurses employed in three teaching hospitals affiliated to Iran University of Medical Sciences, Iran, who were selected via convenience, proportionate stratification sampling in case they met the inclusion criteria. Data were collected using questionnaires of demographic data and attitude of nurses toward delirium consisting of 38 items. Data analysis was performed in SPSS version 16 at the significance level of P≤0.05.ResultsThe attitude of head nurses toward the elderly patients with delirium was significantly better compared to nurses (P=0.005). The results of the analysis of variance indicated a significant difference with the variable of work experience with the elderly (P=0.003). In addition, occupational position (head nurse) was the only significant variable in the linear regression model.ConclusionAccording to the results, occupational position and work experience with the elderly had the most significant associations with the attitude of the nurses toward the elderly patients with delirium. It is suggested that continuous training, evidence-based, and performance-based education be implemented to improve the attitude and practice of nurses. Pages 65-75Background & AimsCancer is an important cause of mortality worldwide. Many cancer patients are hospitalized at the end of their life, requiring end-of-life care. The perception of nurses toward end-of-life care could correct inappropriate care behaviors and enhance the quality of care. The present study aimed to assess the understanding of nurses regarding the end-of-life care of cancer patients.Materials & MethodsThis qualitative study was conducted using the conventional content analysis method in 2017. Sample size included 16 nurses employed in oncology wards, who were selected via purposive sampling. Data were collected via semi-structured interviews. All the interviews were transcribed and reviewed, and data analysis was performed using content analysis.ResultsThe nurses perceived end-of-life care as the main category of ‘comprehensive care’ and three subcategories of ‘value-based care’, ‘support-based care’, and factors in care’.ConclusionAccording to the results, the comprehensive perception of the nurses toward end-of-life care, which arises from their perception toward the care of these patients, could help them provide high-quality care that is value-based, nursing-based, and support-based. Pages 76-85Background & AimsNurses are faced with numerous physical and psychological stressors in their workplace. Identification of stressors and their control in nurses could improve the quality of patient care. The present study aimed to identify the dimensions of occupational stress in the nurses employed in various wards of the hospitals affiliated to Guilan University of Medical Sciences, Iran.Materials & MethodsThis analytical, cross-sectional study was conducted on 324 nurses, who were selected via multistage cluster sampling. Initially, 12 healthcare centers were selected from three clusters of the center, west, and east of Guilan province, Iran. Based on the share of each healthcare center in the required sample size, the samples were randomly selected from various wards, including the emergency, internal medicine, surgery, pediatrics, nursing office, critical care unit (CCU), intensive care unit (ICU), hemodialysis ward, and operating room. Data were collected using the hospital stress scale (HSS-35) consisting of 35 items and 11 subscales. Data analysis was performed in SPSS version 19 using descriptive statistics and analysis of variance (ANOVA).ResultsThe majority of the subjects (39.8%) were aged 31-40 years, female (96%), and married (77.2%) and had a BSc degree (93.2%). Regarding the mean score of general stress, the personnel of the emergency ward had the highest stress level (120.17 ± 16.13), while the ICU personnel had the lowest stress level (114.14 ± 19.01). However, the difference was not statistically significant (P>0.05). In terms of the severity of stress, the dimensions of ‘role ambiguity’ (3.58 ± 0.63) and ‘chemical stress’ (3.12 ± 1.26) represented the most and least significant stressors, respectively.ConclusionAccording to the results, the highest hospital stress levels were observed in the staff of the emergency and pediatrics wards. Therefore, it is recommended that hospital managers pay special attention to the personnel of these wards through planning for the reduction of hospital stress and training the staff on the most effective coping strategies.Keywords: Occupational Stress, Nurse, Hospital
