    سابقه و هدف نحوه نگهداری فرآورده های پلاکتی بر روی اثر بخشی فرآورده پلاکتی تزریقی تاثیری به سزا دارد. نحوه نگهداری فرآورده پلاکت در مراکز انتقال خون مطابق دستورالعمل های سازمان می باشد ولی اطلاع روشنی از وضعیت نگهداری آن در مراکز درمانی و میزانPSL ( platelet storage lesion) ناشی از آن در دست نیست. لذا در این تحقیق قصد داریم تا این مهم را تا روز پنجم نگهداری در شرایط پایگاه و مرکز درمانی بررسی و مقایسه نمائیم.
    مواد و روش هادر این مطالعه توصیفی، 30 فرآورده پلاکتی به صورت تصادفی طی 3 ماه از اردیبهشت تا مرداد 1394 انتخاب و در شرایط نگهداری پایگاه و مرکز درمانی قرار گرفتند. آزمایش های شمارش پلاکتی و گلبول های سفید، MPV، pH، آلودگی باکتریایی، بیان گلیکوپروتئین های CD62P و CD63 در روزهای 1، 3، 5 انجام گردید. نتایج ابتدا با روش کولوگروف اسمیرنوف و سپس با استفاده از روش های آماری paired t-test و t-test مورد ارزیابی قرار گرفتند.
    یافته هافرآورده های پلاکتی پایگاه در روز اول تولید، با استانداردهای ملی مطابق بودند. لیکن طی نگهداری کاهش معناداری در شمارش پلاکتی، لکوسیتی و pHو افزایش معنا داری درMPV و بیان مارکرهای CD62P و CD63 در هردو مرکز دیده شد(001/0p<). تفاوت بین مرکز درمانی و مرکز انتقال خون معنادار نبود.
    نتیجه گیریمطالعه حاضر نشان داد کیفیت فرآورده های پلاکتی تولیدی تا روز سوم در محدوده استاندارد قرار داشت، لیکن در روز پنجم، از لحاظ شمارش پلاکتی در محدوده پائین تر از استاندارد قرار گرفت. تفاوت معنا داری بین کیفیت فرآورده های پلاکتی پایگاه و مرکز درمانی یافت نشد.

    سابقه و هدف ترومبوسیتوپنی ناشی از هپارین، یک رخداد پیش ترومبوتیک است که پس از تجویز هپارین، کمپلکس هپارین- فاکتور 4 پلاکت تشکیل شده و تولید آنتی بادی هایی می نماید که منجر به فعال سازی پلاکت ها شده و حتی ممکن است سبب مرگ و میر بیمار گردد. در این مطالعه، ارزش استفاده از سیستم ارزیابی بالینی 4Tscore در بیماران تحت عمل جراحی قلب بررسی گردید.
    مواد و روش هامطالعه توصیفی حاضر برروی 92 بیمار تحت عمل جراحی قلب، از مهر 94 تا شهریور 95 در بیمارستان شهید مدرس تهران انجام گرفت. این بیماران از نظر 4Tscore شامل: ترومبوسیتوپنی، زمان افت پلاکت، ترومبوز، یا سایر علل ترومبوسیتوپنی بررسی شدند. برای بیماران با 4 ≥ 4Tscore ، بررسی آزمایشگاهی آنتی بادی بر علیه فاکتور 4 پلاکت/ هپارین با روش الایزا Enzyme Immune Assay انجام گردید. در بیماران با 4 ≥ 4Tscore و نتیجه الایزا قویا مثبت(1 OD>)، تشخیص احتمالی HIT در نظر گرفته شد. تحلیل یافته ها توسط آزمون من ویتنی و نرم افزار 22 SPSS انجام شد.
    یافته هااز 92 بیمار، 67 مرد (8/72%) و 25 زن (2/27%) با میانگین سنی 68/10 ±58 سال بودند. 14 بیمار (2/15%) 4 ≥ 4Tscore داشتند که از این تعداد نتیجه آزمایشگاهی آنتی بادی بر علیه فاکتور 4 پلاکت/هپارین با روش الایزا در 8 بیمار مثبت و در 3 بیمار، مثبت قوی دیده شد و فراوانی HIT در این مطالعه 2/3% به دست آمد.
    نتیجه گیری4Tscore ارزش قابل توجهی در تشخیص بیماران مشکوک به HIT دارد ولی استفاده تنها از این سیستم بدون تایید آزمایشگاهی می تواند منجر به تشخیص بیش از حد و قطع نابه جای هپارین گردد.

    سابقه و هدف لوسمی لنفوبلاستیک حاد(ALL)، از شایع ترین بدخیمی های دوران کودکی با فراوانی 40 در هر میلیون کودک زیر 15 سال است. مطالعه حاضر باهدف تعیین فراوانی علائم و نشانه های بالینی و آزمایشگاهی کودکان مبتلابه لوسمی انجام شده است.
    مواد و روش هادر این مطالعه مقطعی، اطلاعات بیماران لوسمی مراجعه کننده به بیمارستان بعثت سنندج از سال 1391-1385، مورد بررسی قرار گرفت. داده ها توسط نرم افزار 11 Stata و آزمون های t و کای دو تجزیه و تحلیل شدند.
    یافته ها84 بیمار تحت بررسی قرار گرفتند که شامل 55 پسر(5/65%) و 29 دختر(5/34%) با میانگین سنی 48/4 ± 13/6 سال بودند. در بررسی علائم بالینی در 6/78% ضعف و بی حالی، 8/73% سابقه تب، 5/34% رنگ پریدگی، 8/29% لنفادنوپاتی، 7/60% آنمی و 8/48% هپاتواسپلنومگالی دیده شد. در ارتباط بین علائم آزمایشگاهی و نوع ایمونوفنوتیپ، بین میزان هموگلوبین و هماتوکریت پایین تر و ایمونوفنوتیپ T cell رابطه معنادار آماری وجود داشت(05/0 p<).
    نتیجه گیریعلائم بالینی و آزمایشگاهی رایج بیماران در این مطالعه با سایر مطالعه ها هم خوانی دارد هرچند، میانگین شمارش پلاکت در سنین بالای 8 سال از میانگین شمارش پلاکت در بیماران زیر8 سال کمتر است. هم چنین از مقایسه بین زیرگروه های B cell و T cell مشخص شد که T cell لوسمی به طور معنا داری با هموگلوبین پایین تری همراه است.

    سابقه و هدف نئوپلاسم های میلوپرولیفراتیو، از جمله بدخیمی های خونی هستند که علایم بالینی بسیار متغیری دارند. تقسیم بندی بیماران میلوپرولیفراتیو به دو گروه دارای کروموزوم فیلادلفیا(Ph+) و فاقد آن(Ph-) می باشد. در گروه Ph- ، بیماری های پلی سایتمی ورا، ترومبوسایتمی اساسی، میلوفیبروز اولیه و دیگر بیماری های نادر قرار می گیرد. این گروه از بیماران دارای جهش JAK2 V617F می باشند که با درصدهای مختلف دیده می شود. هدف از این مطالعه، اجرای روش اصلاح شده PCR-RFLP برای تشخیص جهشJAK2 V617F در بیماران مبتلا به نئوپلاسم های میلوپرولیفراتیو بود.
    مواد و روش هادر این مطالعه مقطعی، 47 بیمار با درخواست آزمایش JAK2 V617F ، مورد بررسی قرار گرفتند. برای تمامی نمونه ها آزمایش تشخیصی JAK2 V617F با استفاده از روش استاندارد ARMS TaqMan Probe انجام شد. جهت تایید نهایی، برخی از نمونه ها مورد توالی یابی ژنی قرار گرفتند. برای شناسایی ناحیه جهش با استفاده از روش PCR-RFLP ، آغازگر های اختصاصی و آنزیم BsaXI استفاده شد. با توجه به این که در این مطالعه نمونه های منفی و مثبت به همراه یک روش استاندارد برای تعیین جهش وجود داشت، محاسبه حساسیت و ویژگی این روش 100% تعیین گردید. تحلیل نتایج توسط آزمون t انجام شد.
    یافته هانتایج PCR-RFLP جهش JAK2 V617F برای تمامی نمونه ها کاملا مشابه و منطبق با نتایج روش های استانداردARMS TaqMan Probe و تعیین توالی بود.
    نتیجه گیریروش PCR-RFLP دارای حساسیت تشخیصی قابل مقایسه ای با روش های Real Time PCR و تعیین توالی بود. مزیت این روش اصلاح شده، توانایی آن در افتراق انواع هموزیگوت و هتروزیگوت بیماران، وجود کنترل داخلی برای عملکرد آنزیم و هم چنین سهولت و صرفه اقتصادی آن است.

    سابقه و هدف تعداد کم سلول های بنیادی خونساز در خون بند ناف، مانعی مهم برای پیوند موفقیت آمیز این سلول ها بوده و ازدیاد این سلول ها در کنار حفظ خصوصیات عملکردی آن ها، از اهمیت بالایی برخوردار است. کشت سلول ها بر روی داربست های سه بعدی، کمک بسیاری به شبیه سازی ریز محیط مکانیکی و شیمیایی بافت مغز استخوان کرده و استفاده از ترکیبات خاصی چون نیکوتین آمید به تکثیر بدون تمایز سلول ها کمک می کند. به این ترتیب در این مطالعه به منظور تکثیر با حداقل تمایز سلول های بنیادی خونساز، توسعه این سلول ها در محیطی نزدیک به مغز استخوان طبیعی در حضور نیکوتین آمید بررسی شد.
    مواد و روش هادر این مطالعه تجربی، سلول های بنیادی خونساز در 5 بار تجربه در محیط سه بعدی و در شرایط مختلف تحت تاثیر سایتوکاین ها و نیکوتین آمید به مدت 7 روز کشت داده شدند. در نهایت به منظور بررسی میزان توسعه سلولی، شمارش تام سلولی، ایمونوفنوتیپ سلولی و سنجش آپوپتوز به روش فلوسیتومتری و هم چنین بررسی توانایی کلنی زایی سلول ها انجام شد. نتایج توسط آزمون آنوا بررسی شد.
    یافته هانتایج این مطالعه حکایت از تکثیر بیشتر در تعداد کل سلول ها و سلول های CD34+، میزان کمتر آپوپتوز و توانایی کلنی زایی بالاتر در موارد استفاده توام از سایتوکاین ها و نیکوتین آمید در داربست سه بعدی نسبت به گروه های دیگر داشت.
    نتیجه گیرینیکوتین آمید با ممانعت از ایجاد تغییرات اپی ژنتیکی حاصل از کشت در محیط آزمایشگاه و داربست سه بعدی با شبیه سازی ریز محیط مغز استخوان، ترکیب بسیار مناسبی برای توسعه این سلول ها به شمار می آیند.

    سابقه و هدف تعداد کم سلول های بنیادی خونساز خون بند ناف یک محدودیت جهت مصارف بالینی است. از این رو تکثیر آزمایشگاهی این سلول ها به منظور دستیابی به تعداد مناسب برای مقاصد درمانی ضروری است. سیستم عصبی سمپاتیک و نوروترنسمیترها در تنظیم خود نوسازی، تکثیر و تمایز سلول های بنیادی خونساز در ریز محیط مغز استخوان نقش دارند. هدف از مطالعه، ارزیابی قدرت تکثیری نوروپپتید Y در سلول های خونساز در شرایط آزمایشگاهی بود.
    مواد و روش هادر یک مطالعه تجربی، مقایسه کشت سلول های بنیادی خونساز پس از جداسازی آن ها در حضور فقط سایتوکاین (SCF، TPO، FLT3 L) به عنوان گروه کنترل و در شرایط تیمار شده با غلظت1 میکرومولار نوروپپتید Y علاوه بر سایتوکاین به عنوان آزمون انجام پذیرفت. در روز 7 تعداد تام سلول های هسته دار و سلول های CD34+ محاسبه گردید. قدرت کلنی زایی سلول های تزاید یافته بررسی شد. آزمون LTC-IC نیز جهت بررسی قدرت کلنی زایی در کشت طولانی مدت انجام شد.
    یافته هاتکثیر 7 روزه سلول های بنیادی خونساز بند ناف منجر به افزایش معنادار سلول های تام تک هسته ای و CD34+ تیمار شده با نوروپپتیدY در مقایسه با گروه کنترل شد. مشاهده انواع کلنی های مختلط، میلوئیدی و اریتروئیدی در آزمون CFU نشان دهنده حفظ قدرت تمایزی سلول های تکثیر یافته CD34+ به انواع رده های خونی بود. سلول های تکثیر شده با نوروپپتید Y قدرت کلنی زایی در کشت طولانی مدت را حفظ کرده بودند.
    نتیجه گیریمطالعه ما نشان داد نوروپپتید Y می تواند تکثیر سلول های بنیادی خونساز را با حفظ قدرت تمایز به انواع رده ها در کشت 7 روزه و در حضور مکمل سایتوکاینی حمایت نماید.

    سابقه و هدف متیلاسیون DNA در ناحیه پروموتر ژن‏های سرکوبگر تومور از جمله P15 ، به عنوان یکی از مهمترین مکانیسم‏های ایجاد بدخیمی های خونی مطرح است .هارمالین یکی از آلکالوئیدهای مشتق شده از گیاه اسپند است که دارای خاصیت آنتی‏پرولیفراتیو بر روی رده های سلولی لوسمیک می‏باشد. در این مقاله تاثیر هارمالین بر کاهش بیان ژنDnmt1 و هیپومتیلاسیون ناحیه پروموتر ژن P15 در رده سلولی NB4 مورد بررسی قرار گرفت.
    مواد و روش هادر این مطالعه تجربی با استفاده از روش MTT ، تاثیرات سیتوتوکسیک بر رده سلولی NB4 تا 72 ساعت بررسی شد. جهت بررسی چرخه سلولی پس از تیمار با هارمالین، از روش فلوسیتومتری استفاده شد. وضعیت متیلاسیون پروموتور ژن P15 با استفاده از روش Methylation specific PCR بررسی شد. با استفاده از روش Real-time PCR ، میزان بیان ژن های Dnmt1 و P15 بررسی گردید.
    یافته هانتایج نشان داد که داروی هارمالین از غلظت μg/mL 15 به بالا، پس از 48 ساعت به صورت وابسته به دوز و زمان دارای خاصیت آنتی پرولیفراتیو بر روی رده سلولی NB4 می باشد. آنالیز چرخه سلولی نشان دهنده ایست سلول ها در فاز G0G1 است. هارمالین در غلظت μg/mL15 موجب کاهش بیان ژن Dnmt1 ، هیپومتیلاسیون پروموتور ژن P15 و افزایش مجدد بیان ژنP15 در رده سلولی NB4می شود.
    نتیجه گیریبا توجه به اثر بخشی داروی هارمالین بر کاهش بیان ژن Dnmt1 و هیپومتیلاسیون پروموتور و افزایش بیان ژن P15 در این رده سلولی، می توان چنین پیشنهاد کرد که این دارو می تواند به عنوان یک راه کار درمانی مناسب به صورت منوتراپی و یا داروی مکمل همراه با دارو های متداول لوسمی پرومیلوسیتیک حاد مورد استفاده قرار گیرد.

    سابقه و هدف آترواسکلروزیس، از مهم ترین وقایع ترومبوتیک در دیابت ملیتوس نوعII است. هیپرگلیسمی مزمن یک فاکتور اصلی برای فعال شدن پلاکت ها در این بیماران می باشد. پدیده حساسیت زدایی در پلاکت ها، پروسه ای فیزیولوژیک است که نقش مهمی در خاموش کردن مسیرهای انتقال سیگنال در پاسخ به آگونیست دارد. در این پژوهش به بررسی این پدیده در پلاکت ها در دیابت پرداخته شد.
    مواد و روش هادر این مطالعه بنیادی کاربردی،40 بیمار دیابت ملیتوس نوع II و 35 نفر از افراد داوطلب سالم به عنوان کنترل، انتخاب شدند. پس از جمع آوری نمونه های سیتراته و جدا کردن PRP ، حساسیت زدایی با دوز پایین ADP انجام شد. میزان بیان مارکر CD62P و میکروپارتیکل های CD41+-Annexin-V+ با استفاده از فلوسیتومتری ارزیابی شد. نتایج داده ها با استفاده از آزمون های آنوا و t-test تجزیه و تحلیل شدند.
    یافته هانتایج نشان داد پلاکت های دیابتیک در حالت Baseline نسبت به افراد سالم، CD62P بالاتری بیان می کنند. هم چنین در پلاکت هایی که حساسیت زدایی شدند، میزان بیان CD62P کمتر و میزان تشکیل میکروپارتیکل بیشتری داشتند(003/0 p=). میزان شمارش میکروپارتیکل های پلاکتی در مورد پلاکت های دیابتی بالاتر بود(52/74 ± SD: ، 004/0 p=).
    نتیجه گیریپلاکت ها در شرایط تیمار خفیف آگونیستی، با وجود بیان CD62P بالا، تحریک پذیری پایینی به ADP دارند که ممکن است ناشی از پدیده حساسیت زدایی باشد که به پلاکت ها در سازگاری با محیط متابولیک خود کمک می کند. هم چنین پلاکت ها تحت تاثیر این پدیده مستعد تولید میکروپارتیکل های پلاکتی بیشتری می شوند که می تواند در عوارض ترومبوتیک دیابت نقش داشته باشد.

    سابقه و هدف یکی از مباحث عمده در سیستم های بهداشت و درمان جهانی، مسئله بهبود عملکرد زنجیره تامین و عدم قطعیت موجود در تقاضا می باشد. هدف مطالعه حاضر، پیش بینی میزان تقاضای پلاکت خون با مدل های شبکه عصبی مصنوعی و آریما، در زنجیره تامین سازمان انتقال خون استان سیستان و بلوچستان بود.
    مواد و روش هادر این مطالعه کاربردی، داده های مربوط به تقاضا برای 8 نوع پلاکت خون در بازه زمانی سال های 1390 لغایت 1394 از مرکز انتقال خون زاهدان جمع آوری گردید. سپس با بهره گیری از مدل های شبکه عصبی مصنوعی و آریما، پیش بینی تقاضای روزانه انجام پذیرفت. در انتها با توجه به معیار ارزیابی کارآیی MSE ، نتایج به دست آمده از روش های مذکور با هم مقایسه شدند. تحلیل داده ها با نرم افزارهای MatlabR2016b وEviews 6 انجام شد.
    یافته هانتایج این مطالعه حاکی از دقت بالای مدل های شبکه عصبی و سپس آریما نسبت به روند فعلی سازمان بود. دقت میانگین با توجه به MSE دو مدل مذکور برای انواع پلاکت عبارتند از: (0048/0 ± 0132/0)O+ ، (0041/0 ± 0115/0) O- ، (0043/0 ± 0205/0) A+ ، (0033/0 ± 01085/0) A- ،(0086/0 ± 0221/0)B+ ، (0009/0 ± 0045/0) B- ، (0031/0 ± 0136/0) AB+ و (0005/0 ± 0034/0) AB- که به ترتیب بیانگر میانگین و انحراف معیار خطا می باشد.
    نتیجه گیرینتایج این مطالعه نشانگر دقت بالای مدل های شبکه عصبی مصنوعی و سپس آریما در پیش بینی تقاضای پلاکت خون است. لذا استفاده از مدل های شبکه عصبی مصنوعی جهت پیش بینی تقاضا به جای روش های پیش بینی آماری رایج در مراکز انتقال خون قابل توصیه می باشد.

    سابقه و هدف فعالیت های بدنی می توانند تغییرات هماتولوژیک مختلفی ایجاد نمایند. هدف این مطالعه، بررسی پاسخ شاخص های هماتولوژیکی و تغییرات آهن و فریتین متعاقب شش ماه تمرین هوازی بود.
    مواد و روش هادر یک کارآزمایی بالینی، 19 زن میانسال و غیرفعال با میانگین سن 3/3 ± 2/42 سال و میانگین وزن 8/6 ± 64 کیلوگرم در دو گروه تجربی و کنترل قرار گرفتند. گروه تجربی تمرینات هوازی را به مدت شش ماه انجام دادند.
    یافته هادر مقایسه درون گروهی در گروه تجربی، میزان پلاکت به طور معنا داری افزایش(3/65944 ± 3/267636) و میزان آهن و فریتین به طور معنا داری کاهش یافت. هم چنین تغییرات بین گروهی در شاخص پلاکت افزایش معنا دار و در متغیرهای آهن و فریتین کاهش معنا دار نشان داد(به ترتیب(033/0 p=) ، (015/0 p=) و (026/0 p=)).
    نتیجه گیریافزایش تعداد پلاکت ها می تواند نشان دهنده افزایش فعال شدن و تجمع پلاکتی باشد. افراد مستعد/مبتلا به بیماری های قلبی- عروقی برای به حداقل رساندن خطر بهتر است فعالیت های ورزشی منظم را با شدت پایین تا متوسط انجام دهند.

    سابقه و هدف پس از انتقال بیماری ایدز و هپاتیت به برخی بیماران هموفیلی در اثر فاکتورهای خونی آلوده، موضوع مسئولیت مدنی دولت، ناشی از توزیع خون آلوده در نظام حقوقی ما به صورت جدی مطرح گردید. مبانی متعددی برای مسئول تلقی نمودن دولت(سازمان انتقال خون) از سوی حقوقدانان مطرح گردیده که از آن جمله می توان به تقصیر، مسئولیت نوعی(نقض تعهد ایمنی)، نقص تجهیزات و مبنای نفی ضرر اشاره نمود. هدف از نگارش این مقاله آن است که نظریه قابلیت استناد، به عنوان مبنای مسئولیت دولت در این زمینه معرفی شود.
    مواد و روش هاروش تحقیق در این مقاله، روش توصیفی - تحلیلی و مبتنی بر تحلیل مقررات قانونی و دیدگاه های حقوقدانان و فقها بود.
    یافته هایافته های این مقاله حکایت از این داشت که در فقه امامیه قواعد اتلاف، تسبیب و غرور در جهت اثبات مسئولیت دولت در عرضه خون آلوده، مورد استفاده قرار گرفته که قاعده تسبیب، هماهنگی بیشتری با این موضوع دارد. در حقوق ایران نیز بسیاری از حقوقدانان، تلاش نموده اند که این مسئولیت را بر اساس مسئولیت محض تحلیل نمایند.
    نتیجه گیرینظریه قابلیت استناد به عنوان یکی از نظریات مطرح شده در مبانی مسئولیت مدنی است که متکی به فقه امامیه می باشد. این نظریه بدون نیاز به اثبات تقصیر و یا تمسک به مسئولیت محض که در نظام حقوقی ما قابل پذیرش نمی باشد، به خوبی می تواند مسئولیت دولت ناشی از توزیع خون های آلوده را توجیه نماید.

    سابقه و هدف همودیالیز، برای بیماران کلیوی که در مراحل پایانی روند بیماری هستند، روش درمانی مهمی به شمار می رود، اما آن ها را به برخی از عفونت ها به خصوص ویروس های منتقله از راه خون مستعد می کند. هدف از این تحقیق، بررسی فراوانی مارکرهای سرولوژیکی HBsAg ، anti-HBs ، anti-HCV و HIV Abدر بیماران همودیالیزی شهر زنجان بود.
    مواد و روش هادر یک مطالعه توصیفی، نمونه های سرم پس از جمع آوری با استفاده از دستگاه ECL از لحاظ مارکرهای سرولوژیکی HBsAg ، anti-HBs ، anti-HCV و anti-HIVمورد سنجش قرار گرفتند و یافته ها توسط آزمون های کای دو، t و فیشر تجزیه و تحلیل شدند.
    یافته هافراوانی مارکرهای سرولوژیکی 3/2% HBsAg ، 3/65% HBs Ab ، 1/1% HCV Ab و0% HIV Ab به دست آمد.
    نتیجه گیرینتایج حاصل از این مطالعه نشان داد که فراوانی HBsAg در بیماران همودیالیزی نسبت به جمعیت کلی کمتر است. در این مطالعه فراوانی HBs Ab نسبت به دیگر مطالعه های انجام شده در ایران و سایر کشورها تفاوت چندانی نداشت. هم چنین فراوانی HCV Ab با جمعیت کلی کشور یکسان و فراوانی HIV Ab در مقایسه با سایر مطالعه های انجام شده در وضعیت مشابهی قرار داشت.

    Background and Objectives Platelet concentrates are the most vulnerable blood products with the shortest shelf life. The aim was to assess the quality of produced platelet concentrates in the East Azerbaijan. In addition, the quality of products stored in the blood center was compared by products stored in one hospital during the period of five days.
    Materials and Methods In this descriptive study, thirty random donor platelet concentrates during 3 months from April to August 2015 were enrolled in the study. They were randomly classified into two groups, half were sent to the hospital and half remained in the blood center. The changes of their quality (platelet count, WBC count, MPV, pH, bacterial contamination, and CD62P and CD63 expression levels) were examined in the first, third and fifth days of storage in each group and comparison was made by accomplishing the paired sample t-test and t-test as deemed appropriate.
    Results All PCs on the first day met the national standard criteria; however, the quality parameters decreased significantly during storage. In addition there were no significant differences in the storage related changes in the quality between the blood center and hospital.
    Conclusions The quality of PCs produced in the blood center decreased during the storage time. By improving the storage conditions in the blood centers, it might be possible to extend the storage time of PCs by an acceptable quality. The significant difference was not observed between the platelets stored in the blood center compared to that stored in the hospital.

    Background and Objectives Heparin induced thrombocytopenia (HIT) is a prothrombotic disorder. The antibody against PF4-heparin complex may lead to platelet activation, thrombotic events and death. We decided to evaluate diagnostic value of 4T score as a clinical diagnostic system among cardiac surgery patients for diagnosis of HIT.
    Materials and Methods In this descriptive study, 92 patients that underwent cardiac surgery from September 2015 till September 2016 in Tehran Modarres Hospital have been included. We evaluated 4Tscore criteria (4T stands for Thrombocytopenia, Time of thrombocytopenia, Thrombotic events and Other reasons for Thrombocytopenia) in all cases. Blood samples were obtained from patients with 4Tscore ≥ 4. Anti-platelet factor 4/heparin antibody (enzyme-immune assay, ELISA) was done for patients with 4Tscore ≥ 4 and the OD ≥ 0.2 was considered as positive and ≥1 as strong positive. Patients with 4Tscore ≥ 4 and strong positivity of ELISA test were considered as HIT.
    Results From 92 patients, 67 (72.8%) were male and 25 (27.2%) female. Mean age was 58±10.68 with 14 patients (15.2%) having 4Tscore ≥ 4. ELISA was positive (OD > 0.2) in 8 cases in 3 of whom ELISA being strongly positive (OD > 1). According to HIT definition in this study, the prevalence of HIT was 3.2%.
    Conclusions 4T score should be evaluated for preliminary diagnosis of HIT in suspected patients treated with heparin but the use of this system alone without laboratory confirmation can lead to over diagnosis of HIT and inappropriate termination of heparin treatment.

    Background and Objectives One of the most common childhood malignancies is leukemia with prevalance of 40 per million among children aged under 15. This study aimed to determine the prevalence of clinical and paraclinical features of leukemia among children aged under 15 with acute lymphoblastic leukemia.
    Materials and Methods In this cross-sectional analysis of hospital records, clinical and paraclinical features of patients admitted to Besat Hospital during 2006-2012 were reviewed. Data were analyzed by Stata (version 11) software. c2 test and t-test were used.
    Results Of the total study population (84 cases), 65.5% were male and 34.5% were female. The mean age was 6.13 ± 4/48 years. The most common clinical feauture was lethargy (78.6%) followed by history of fever (73.8%), pallor (34.5%), lymphadenopathy (29.8%), anemia (60.7%) and hepatosplenomegaly (48.8%). There was a significant correlation between T cell leukemia and lower hemoglobin level, and likewise with lower platelet count in children more than 8 years of age (p Conclusions This study revealed lethargy as the most common clinical feature in our study population. As well it showed a significant correlation between lower platelet count and age (above 8 years old); the same true between lower hemoglobin level and T cell leukemia.

    Background and Objectives Chronic myeloproliferative neoplasms (MPNs) are clonal hematopoietic stem cell disorders. They are heterogeneous in symptoms and mainly consist of Philadelphia chromosome positive (Ph) and negative (Ph-). The Ph- group includes polycythemia Vera (PV), essential thrombocythemia (ET), primary myelofibrosis (PMF) and other rare disorders. In the latter group, substitution of phenylalanine for valine at codon 617 (JAK2V617F), could be relevant to the disease. In this study we used modified PCR-RFLP to detect JAK2 V617 F mutation in chronic myeloproliferative neoplasms.
    Materials and Methods In this cross sectional study, for detection of JAK2 V617 F mutation, 47 subjects were enrolled and peripheral blood samples were collected between 2015 and 2016. The samples were collected from the clinic center of Arak city and diagnosed to be JAK2 V617F positive by TaqMan probe and sequencing methods. All samples were investigated for the mutation by modified PCR-RFLP using specific primers and BsaXI restriction enzyme. The data were analyzed by t-test.
    Results Our modified PCR-RFLP for detection of JAK2 V617F mutation showed the same results as TaqMan probe and sequencing standard methods. In this study we have positive and negative samples and a standard method whose sensitivity and specificity was calculated to be 100% .
    Conclusions Our modified PCR-RFLP is a comparable method with gold standard methods, TaqMan probe and sequencing, for detection of JAK2 V617F mutation. The advantages of this modified method include its ability to detect JAK2 V617F in homozygote and heterozygote conditions, and the presence of the internal digestion control.

    Background and Objectives The low number of umbilical cord blood cells is an important barrier to successful bone marrow transplantation. Therefore, the growth of these cells while maintaining their functional characteristics is of great importance. Culturing cells on 3D scaffolds like DBM has greatly contributed to the simulation of the mechanical and chemical micro-environment of the bone marrow tissue. There are also specific compounds for proliferation without differentiation of cells one of which is nicotinamide.
    Materials and Methods In this experimental study, hematopoietic stem cells (HSCs) were cultured 7 days in DBM under the following conditions of (1) negative control, (2) cytokine, (3) nicotinamide, (4) nicotinamide and cytokine. After 7 days, cell count, purity using flow cytometry, colony forming unit assay and apoptosis were evaluated and performed. The data were analyzed using Anova.
    Results Our results indicated that in all cases, the combination of nicotinamide and cytokine was accompanied with more proliferation, more CD34 cells, less apoptosis, and higher colony-forming ability than other groups.
    Conclusions Nicotinamide is a very suitable compound for the development of umbilical cord blood cells given the role of the former in preventing epigenetic changes that are the outcome of laboratorial cell culture and the simulation of bone marrow niche by DMB.

    Background and Objectives The low number of hematopoietic stem cells (HSCs) in each unit of UCB is a limitation for clinical application of this source of HSCs. Ex vivo expansion of HSCs to obtain a sufficient number for therapeutic purposes is necessary. The sympathetic nervous system (SNS) and neurotransmitters regulate HSCs self-renewality, proliferation and differentiation in the bone marrow. The aim of this study was to evaluate the proliferative effect of NPY on the CB-HSCs in ex vivo condition.
    Materials and Methods After isolation of HSCs, the comparison of HSCs Ex vivo expansion using 1 µM NPY and cytokine as test group with cytokine as control group was performed . TNC number and CD34 cell number were calculated on day 7. Colony formation assay was performed in methylcelloluse medium. LTC-IC assay was performed to investigate colonization potential of expanded cells in long time culture.
    ResultsEx vivo expansion of CB-HSCs after 7 days resulted in significant increase in the number of total nucleated cells and CD34 cells in NPY-treated groups in comparison to control group. The formation of different cell colonies identified as erythrocytic, granulocytic and mix colonies in CFU assay reflect the differentiation potential of expanded CD34 cells. Treated group with NPY also retained their ability to formate colonies in long term culture.
    Conclusions Our study demonstrated that Neuropeptide Y can successfully support expansion of CD34 hematopoietic stem cells while retaining their potential of being differentiated into various cell lineages after 7-day culture period with cytokine supplementation.

    Background and Objectives One of the most important mechanisms of hematologic malignancies is DNA methylation in the promoter region of tumor suppressor genes such as P15. Harmalin is one of the alkaloids derived from plants that show anti-proliferative activity on leukemic cell lines. The effect of harmalin on Dnmt1 gene expression and hypomethylation of P15 promoter in NB4 cell line was evaluated.
    Materials and Methods In this experimental study, cytotoxic effects of harmaline were investigated until 72 hours on the NB4 cell line using MTT. Flow cytometry was performed in order to evaluate the cell cycle of NB4 cell line after treatment with harmalin. Gene promoter methylation status of P15 was evaluated by Methylation Specific PCR. Dnmt1 and P15 gene expression was analyzed by Real-time PCR technique.
    Results 15μg/ml harmalin after 48 hours showed a dose and time-dependent anti-proliferative properties on NB4 cell line, cell cycle analysis would stop NB4 cells in G0G1 phase of the cell cycle. Real-time PCR results showed that 15μg/ml harmalin reduced gene expression of Dnmt1, induced hypomethylation of P15 gene promoter and increased P15 gene expression in NB4 cell line.
    Conclusions According to our data, the effect of harmalin on reduction of Dnmt1 expression, P15 promoter hypomethylation and P15 reactivation in this cell line showed that it could be suggested as a therapeutic strategy, suitable for monotherapy or supplement for medication which is commonly used for APL (Acute Promyeloid Leukemia).

    Background and Objectives Patients with diabetes mellitus type 2 have accelerated atherosclerosis. Hyperactive platelets and platelets micro particles play an important role in this prothrombotic state. Desensitization is a physiological process which plays an important role in turning off receptor mediated signal transduction pathways. In this study, we described and compared this process in diabetes mellitus type 2 patients and healthy individuals.
    Materials and Methods In this fundamental-applied study, 40 diabetes mellitus type 2 patients and 35 healthy individuals participated in the study. After centrifuging at 200 g, PRP was prepared. Desensitization was induced with mild agonist stimulation and low dose of ADP (1 μM). Expression of platelets CD62P and CD41+ Annexin-v micro particles were evaluated by Flow cytometry. Data were analyzed by Anova and t-test.
    Results In comparison with healthy individuals, the diabetic group showed a significant increase in expression of CD62P in baseline. Desensitized platelets had less expression of CD62Pb but these platelets produced more micro particles in this state. Diabetic group showed a significant increase in production of platelet micro particles in all of states with ADP treatment. (p ≤ 0.004)
    Conclusions The flow cytometric data show that platelets in patients with diabetes mellitus type 2 patients are hyperactive that it associated to metabolic conditions. Induction of desensitization state helps platelets to reduce platelets activation and platelets sensitivity to ADP in diabetic environment. Also the production of platelets microparticles is high in these patients and desensitized platelets are more susceptible to microparticle shedding.

    Background and Objectives One of the major issues in global healthcare systems is the issue of improving supply chain performance and uncertainties in demand. The aim of this study is to forecast blood platelet demand with artificial neural network and Arima Models in the blood transfusion supply chain in Sistan and Baluchistan province.
    Materials and Methods In this applied study, the data on demand for 8 types of blood platelets were collected from the Zahedan Blood Center between 2011 and 2015. Then, using artificial neural network models and ARIMA models, daily demand forecasts were made. Then, according to MSE performance evaluation criteria, the results of the above-mentioned methods were compared. The data were analyzed by MetlabR2016b and Eviews 6 softwares.
    Results The results of this study indicate the high accuracy of neural network models followed by Arima compared to that calculated in the current profile of IBTO. The average accuracy according to MSE of the two models for platelet types are: O (0.0132±0.0048), O- (0.0115 ± 0.0041), A (0.0205 ± 0.0043), A- (0.0108 ± 0.0033), B (0.0221 ± 0.0086), B- (0.0045 ± 0.0009), AB (0.0136 ± 0.0031), AB- (0.0034 ± 0.0005) which represent the mean and standard deviation of the error, respectively.
    Conclusions The results of this study indicate the high accuracy of artificial neural network models followd by Arima in predicting blood platelet demand. Therefore, using artificial neural network models for prediction of demand is recommended instead of common statistical prediction methods in blood centers.

    Background and Objectives Physical activities can make different hematological changes. The aim of this research was to evaluate the response of hematological indices and the changes of iron and ferritin subsequent to 6 months of aerobic exercise.
    Materials and Methods In this semi-experimental study, 19 sedentary middle-aged women with the mean age of 42.2 ± 3.3 years and the mean weight of 64.0 ± 8.6 kg, were randomly divided into two groups: experimental and control. Experimental group underwent aerobic exercise for six months.
    Results Plt (p = 0.045) showed a significant increase (267636.3 ± 65966.3) and iron (p = 0.008) and Ferritin (p = 0.017) showed a significant decrease in the experimental group. The changes between groups in Plt (p = 0.033), iron (p = 0.015) and Ferritin (p = 0.026) were significant.
    Conclusions Increase in the number of platelets may indicate an increase in platelet aggregation and activation. Persons apt/suffering from cardiovascular diseases should perform exercises with low to moderate intensity in order to minimize the risk.

    Background and Objectives After the transmission of AIDS and hepatitis to some hemophilic patient’s due to infected blood transfusions, the issue of civil liability of the state caused by the supply of blood contaminated in our legal system was seriously raised. The lawyers have mentioned several bases for the civil liability of the state (blood transfusion organization), including the basis of fault, strict liability (breach of safety obligation), equipment defect, and the basis for the not-harm. The purpose of this article is to describe the opposability as the basis for the state's liability in this field.
    Materials and Methods The research method in this paper is an analytical and descriptive method based on the analysis of legal provisions and the views of lawyers and jurists.
    Results The findings of this paper indicate that in the Shiiet jurisprudence, the Etlaf, Tasbib and Ghoror rules have been used to prove the state's responsibility for the supply of contaminated blood, which the Tasbib rule is more in harmony with this issue. In Iranian law, many lawyers have also tried to analyze this responsibility on the basis of strict liability.
    Conclusions The theory of opposability as one of the theories expressed in the basis of civil responsibility, which relies on Shiite jurisprudence, can well justify the responsibility of the state for supply of contaminated blood, without the need to prove the fault or to be liable to the strict liability that is not acceptable in our legal system.

    Background and Objectives Hemodialysis is a major method of treatment for the patients of end-stage renal disease (ESRD). It so disposes them to some infections, specially blood born viruses. The aim of this study was to investigate the prevalence of serological markers for HBsAg, anti-HBs, anti-HCV and HIV Ab in hemodialysis patients in Zanjan city.
    Materials and Methods After 10 ml blood samples were used for detection of markers for hepatitis B surface antigen (HBsAg), hepatitis B surface antibody (HBs Ab), antibody to HCV (anti-HCV), HIV antibody by ECL. Data were analyzed by c2, t test and fisher exact test.
    Results Positive HBsAg was found in .2.3%, HBs Ab 65.3% and HCV Ab 1.1% of cases. HIV Ab cases were negative in these patients.
    Conclusions The prevalence of HBsAg in hemodialysis patients has been re­ported to be lower than in the general population. In this study, the prevalence of HBs Ab compared to other studies in Iran and other countries is not much different. The results show that the prevalence of HCV Ab in hemodialysis patients is the same with the general population and the prevalence of HIV Ab in this study is similar to the result recent studies.
