    ویروس ها از عوامل مهم بیماری های حاد و مزمن در انسان ها هستند. صرف نظر از کثرت ایجاد عفونت در جوامع، بیشتر انواع ویروس ها پاتوژن های مهم بیمارستانی هستند. عفونت های وابسته به اقدامات بهداشتی یک منبع مهم در ابتلا و مرگ و میر بیماران می باشند. ضدعفونی یک راه اصلی در کاهش یا قطع انتقال ویروس ها از راه سطوح محیطی، ابزار و دست ها می باشد که به ترتیب با ضدعفونی کننده های شیمیایی و آنتی سپتیک ها انجام می پذیرد. در این مقاله مروری درباره عوامل شیمیایی با خواص ویروسیدی، مکانیسم های عمل و کاربردشان در کنترل عفونت های ویروسی وابسته به اقدامات بهداشتی بحث شده است. علاوه بر این نظر اجمالی در سلسله مراتب ویروس ها در چالش با ضدعفونی کننده ها، عوامل موثر در غیرفعال سازی ویروسی به عبارتی ویروس های هدف، پایداری و مدت زمان بقا ویروس ها در سطوح محیطی و دست ها بحث شد. همچنین ضدعفونی سطوح، چالش ها در غیرفعال سازی پاتوژن های ویروسی نوپدید، مقاومت های ویروسی به ضدعفونی کننده های شیمیایی توضیح داده شد. سپس، خواص آنتی سپتیک ها شرح داده شد و آنتی سپتیک های منتخب با خواص ویروسیدی گسترده معرفی شدند، زیرا دست ها یک نقش مهمی در گسترش بسیاری از بیماری های ویروسی بازی می کنند و بهداشت مناسب و منظم دست ها برای آلودگی های اساسی است و می تواند زنجیره گسترش ویروسی را قطع نماید. در این مقاله روش های اخیر آزمایشگاهی، روش های استاندارد از چندین کشور و نوع ویروس ها در این روش ها برای ارزیابی فعالیت ویروسیدی را مقایسه کردیم. در پایان، بهتر است بدانیم: هر ضدعفونی کننده، ویروسید نیست مگر اینکه با روش های آزمایشگاهی به تایید برسد.

    زمینه و هدف
    پیش دیابت حالتی پرخطر برای ابتلا به بیماری قند و امراض قلبی- عروقی می باشد. هدف مطالعه تعیین بروز پیش دیابت وفنوتیپ های آن و عوامل خطر مربوطه در پیگیری 9 ساله بود.
    روش بررسی
    مطالعه قند و لیپید تهران مطالعه آینده نگری است که در ساکنین منطقه 13 تهران با هدف تعیین شیوع و بروز و عوامل خطر بیماری های غیرقابل انتقال انجام شده است. چهار فاز از این مطالعه اجرا شده که شامل دوره های سه ساله از سال 1377 تا 1389 می باشد. از بین 12808 فرد 20 سال و بالاتر در جمعیت پایه، پس از خارج کردن افرادی که شرایط لازم را نداشتند، آنالیزها در 2597 مرد و 3282 زن اجرا شد.
    یافته ها
    در مردان، سن (سال)، شاخص توده بدنی (kg/m2)، سابقه خانوادگی دیابت، سطح تحصیلات بالای 12 سال، قندخون ناشتا (mmol/l)، قندخون دو ساعت پس از مصرف گلوکز (mmol/l)، با افزایش بروز پیش دیابت همراه بودند، اما مداخلات جامعه محور اثر محافظتی معنا دار داشتند. درزنان، سابقه خانوادگی دیابت، نسبت دور کمر به قد، قندخون دو ساعت پس از مصرف گلوکز، نسبت کلسترول تام به کلسترول لیپوپروتیین با چگالی بالا، قندخون ناشتا، خطر پیش دیابت را افزودند در حالی که وضعیت مطلقه/بیوه با خطر کمتر همراه بود. قندخون ناشتا، قندخون دو ساعت پس از مصرف گلوکز، شاخص توده بدنی و نسبت کلسترول تام به کلسترول لیپوپروتیین با چگالی بالا عوامل خطر بروز ترکیب گلوکز ناشتای مختل و اختلال تحمل گلوکز، اما دور باسن اثر محافظتی داشت. بروز پیش دیابت 1/46 در هر1000 شخص در سال در مردان و 8/36 در هر 1000شخص در سال در زنان محاسبه شد.
    نتیجه گیری
    مطالعه نشان داد که بروز پیش دیابت و فنوتیپ های مختلف آن در جمعیت ایرانی بالا است.

    زمینه و هدف
    مطالعات انجام شده در زمینه عوامل موثر ژنتیکی در بروز چاقی نشان از نقش مهم ژن FABP2 در بروز چاقی می باشد. تاکنون تعداد 52 لوکوس ژنی مرتبط با بروز چاقی را شناسایی شده اند که این امر می تواند گامی مهم برای پیش بینی، پیشگیری و درمان چاقی ایفا نماید. هدف از مطالعه حاضر بررسی ارتباط میان پلی مورفیسم rs1799883 واقع بر ژن FABP2 و چاقی در جعمعیت قند و لیپید تهران بود.
    روش بررسی
    مطالعه مورد-شاهدی حاضر متشکل 217 فرد چاق با شاخص توده بدنی kg/m2 30 یا بالاتر و 159 فرد سالم با شاخص توده بدنی kg/m2 20-18 می باشد که از میان شرکت کنندگان در فاز چهار مطالعه قند و لیپید تهران انتخاب شدند (سال های 1390-1386). DNA ژنومی از لنفوسیت های خون محیطی استخراج و ژنوتیپ هر یک از افراد با روش Tetra-primer amplification refractory mutation system-polymerase chain reaction (T-ARMS-PCR) تعیین شد.
    یافته ها
    بررسی توزیع فراوانی آللی در دو گروه بیمار و سالم نشان داد که آلل T (ریسک) در هر دو گروه دارای کمترین فراوانی می باشد (به ترتیب 2/6% و 4/5%). از سوی دیگر، فراوانی ژنوتیپ های GT و GG در گروه بیمار به ترتیب 4/12%، 6/87% و در گروه سالم به ترتیب 7/10% و 3/89% بود. نتایج به دست آمده حاکی از عدم وجود اختلاف معنادار میان فراوانی آللی در دو گروه مورد و شاهد بود (61/0P=). بر این اساس بودن آلل T (آلل ریسک)، شانس ابتلا به چاقی را در افراد حامل این آلل در مقایسه با افراد فاقد آن به صورت معنادار افزایش نمی داد (19/2-62/0 :95%CI، 17/1OR=).
    نتیجه گیری
    یافته های به دست آمده نشان دهنده عدم وجود ارتباط میان پلی مورفیسم rs1799883 و شانس ابتلا به چاقی در جمعیت قند و لیپید تهران می باشد.

    زمینه و هدف
    سزارین جزء شایعترین اعمال جراحی در زنان است و عوارض زخم آن بالغ بر 16-5/2% می شود. از این رو مطالعه حاضر با هدف انتخاب روش مناسب جهت بستن پوست انجام گردید.
    روش بررسی
    مطالعه به صورت کارآزمایی بالینی در بیمارستان مطهری وابسته به دانشگاه علوم پزشکی ارومیه از اردیبهشت تا آذر سال 1393 در زنان حامله با زایمان سزارین، انجام شد. 266 نفر واجد معیارهای ورود رضایت آگاهانه، سن حداقل 18 سال و سن حاملگی حداقل24 هفته، وارد مطالعه گردیده و در دو گروه که به طور پنهانی با رنگ های مختلف که توسط خود بیماران انتخاب می گردید، قرار گرفتند. همه بیماران از نظر عوارض جداشدگی زخم، درد، اسکار، هماتوم، سروما و مدت زمان عمل مقایسه گردیدند.
    یافته ها
    بیشترین اندیکاسیون سزارین در هر دو گروه، سابقه سزارین با جنین ترم بود که تفاوت معنادارنبود (381/0P=). شدت درد (Visual Analogue Scale، VAS) در روز اول و هفته ششم در گروه سوچور به طور معنادار بیشتر بود (001/0P=). زمان بستن پوست در گروه استپل 07/0±03/1 و در سوچور 67/0±68/4 دقیقه بود که تفاوت معنادار بود (001/0P=). فراوانی باز شدن زخم در گروه استپل (3/11%)15 و در سوچور پنج مورد (8/3%) بود که تفاوت معناداری مشاهده شد (017/0P=). نمره معیار ونکوور از نظر اسکار گروه استپل 9/0±5/7 و سوچور 8/0±6/6 بود که تفاوت معنادار بود (001/0P=). بیماران با عارضه بازشدن زخم، دارای تفاوت معناداری از نظر سن (022/0P=) و شاخص توده بدنی (001/0P=) با افراد بدون این عارضه داشتند.
    نتیجه گیری
    استفاده از استپل می تواند باعث کاهش درد و همچنین مدت زمان عمل بیماران گردد اما عارضه بازشدن زخم و اسکار در آنها بیشتر می باشد.

    زمینه و هدف
    اسپرم زایی فرآیندی پیچیده و سازمان یافته از تکثیر و تمایز سلول های بنیادی اسپرماتوگونی است. سلول های بنیادی اسپرماتوگونی (SSCs) به عنوان سلول های بنیادی منحصر به فرد با توان خود نوزایی، تمایز و انتقال داده های ژنتیکی به نسل بعد در حفظ باروری نقش اساسی دارند. همچنین سلول های سرتولی در تعادل بین تکثیر و تمایز سلول های بنیادی اسپرماتوگونی موثر هستند. با توجه به اهمیت و کاربرد گسترده SSCs به ویژه در درمان ناباروری، هدف از انجام این پژوهش نقش هم کشتی با سلول های سرتولی در تعیین سرنوشت سلول های بنیادی اسپرماتوگونی بود.
    روش بررسی
    این مطالعه تجربی از آبان 1392 تا آذر 1393 در گروه آناتومی دانشکده پزشکی دانشگاه علوم پزشکی تهران، بر روی موش های NMRIنابالغ (6-3 روزه) انجام گرفت. ابتدا سلول های سرتولی و سلول های بنیادی اسپرماتوگونی از موش های نوزاد طی دو مرحله هضم آنزیمی جدا شدند. ماهیت سلول های سرتولی با نشانگر ویمنتین و SSCs با نشانگر پروتئین انگشت روی لوسمی پرومیلوسیتیک (Promyelocytic leukemia zinc-finger، PLZF) تایید شد. سلول های بنیادی اسپرماتوگونی در دو گروه هم کشتی با سلول های سرتولی و بدون هم کشتی (کنترل) قرار گرفتند. میزان بیان ژن تمایزنیافته مهارکننده اتصال DNA چهار (ID4) و تمایزیافته c-Kit توسط تکنیک واکنش زنجیره پلی مراز ارزیابی شدند.
    یافته ها
    درصد خلوص SSCs با روش فلوسایتومتری 66/91% گزارش شد. بیان ژن ID4 در گروه هم کشتی نسبت به کنترل افزایش معنا داری را نشان داد (045/0P=)، در حالی که بیان ژن c-Kit در گروه هم کشتی در پایان هر هفته در مقایسه با کنترل کاهش معنا داری داشت (046/0P=).
    نتیجه گیری
    با توجه به نتایج حاصل از این مطالعه، هم کشتی با سلول های سرتولی برای مدت بیشتری سلول های بنیادی اسپرماتوگونی را در مرحله تکثیر نگه می دارد، بنابراین می تواند جهت بهینه سازی محیط کشت در کلینیک استفاده شود.

    زمینه و هدف
    استافیلوکوک های کواگوالاز منفی پیش تر به عنوان آلوده کننده های محیط کشت در نظر گرفته می شدند. امروزه یکی از فراوانترین باکتری های پاتوژن بیمارستانی محسوب می شوند. محدوده مقاومت به بتالاکتام ها در این گونه های استافیلوکوک باعث ایجاد نگرانی هایی در بیمارستان ها شده است. شناسایی سریع مکانیسم های مقاومتی و تایید مقاومتشان به متی سیلین یک اصل اساسی برای درمان های آنتی بیوتیکی است. این مطالعه با هدف تعیین الگوی مقاومت آنتی بیوتیکی و فراوانی ژن mecA و تعیین تیپ های SCCmec در استافیلوکوک های کواگوالاز منفی جدا شده از بیمارستان های آموزشی شهر همدان انجام شد.
    روش بررسی
    این مطالعه مقطعی از شهریور تا بهمن 1394 بر روی صد نمونه استافیلوکوک کواگوالاز منفی (CNS) جدا شده از بیماران با محدوده سنی 69-7 سال از چهار بیمارستان آموزشی مختلف دانشگاه علوم پزشکی شهر همدان انجام شد. بررسی حساسیت آنتی بیوتیکی ایزوله ها به روش دیسک دیفیوژن آگار انجام شد و در مرحله بعد شناسایی ژن mecA و تعیین تیپ های SCCmec با روش واکنش زنجیره پلیمراز انجام شد.
    یافته ها
    فراوانی سویه ها به ترتیب استافیلوکوکوس اپیدرمیدیس (55%)55، استافیلوکوکوس همولتیکوس (40%)40 و استافیلوکوکوس ساپروفتیکوس (5%)5 گزارش گردید. در تست آنتی بیوگرام بالاترین حساسیت به آنتی بیوتیک ریفامپین (96%)96 و بیشترین مقاومت به تری متوپریم سولفامتوکسازول (47%)47 مشاهده شد. شیوع مقاومت به متی سیلین نیز در (50%)50 از نمونه ها مشاهده گردید. فراوانی تیپ های SCCmec در 50 ایزوله مقاوم به متی سیلین به ترتیب تیپ 3 (13%)13 عدد، تیپ 5 (11%)11، تیپ 2 (6%)6، تیپ 4 (4%)4، تیپ 1 (3%)3 عدد مشاهده گردید و (13%)13 تا از ایزوله ها با روش PCR تیپ بندی نشدند.
    نتیجه گیری
    نتایج مطالعه حاضر نشان داد که تیپ غالب SCCmec مربوط به تیپ 3 می باشد و ژن های مقاومت زیادی توسط این تیپ کد می شود.

    زمینه و هدف
    افزایش فشارخون یکی از شایع ترین بیماری های صده ی اخیر با عوارض فراوان می باشد. هدف از مطالعه کنونی بررسی اثر عصاره آبی- الکلی برگ گردو (Juglans regia L.) بر روی فشارخون و تداخل اثر آن با سیستم آدرنرژیک در موش صحرایی بود.
    روش بررسی
    در این مطالعه تجربی که در آزمایشگاه تحقیقاتی فیزیولوژی دانشکده علوم، دانشگاه شیراز از مهر 1392 تا شهریور 1393 انجام شد، 10 سر موش صحرایی نر در محدوده ی وزنی 250-180 گرم مورد استفاده قرار گرفتند و به طور تصادفی به دو گروه تقسیم شدند: گروه دریافت کننده عصاره برگ گردو و دریافت کننده عصاره برگ گردو با آدرنالین. پارامترهای فشارخون (میانگین سرخرگی، فشار سیستولی و فشار دیاستولی) پیش و پس از تزریق درون سیاهرگی عصاره، حلال عصاره، آدرنالین و عصاره توام با آدرنالین از طریق کانول سیاهرگی، ثبت گردید.
    یافته ها
    پارامترهای فشارخون در پاسخ به عصاره برگ گردو نسبت به حالت کنترل و شاهد کاهش یافت (05/0P<). تزریق توام عصاره و آدرنالین نیز باعث کاهش پارامترهای فشارخون به نسبت حالت شاهد آن شد (05/0P<).
    نتیجه گیری
    عصاره آبی- الکلی برگ گردو دارای اثر کاهندگی فشارخون است که شاید این اثر از طریق برهمکنش با سیستم آدرنرژیک باشد.

    زمینه و هدف
    وارفارین داروی ضدانعقاد می باشد که نقش آن پیشگیری از ایجاد لختگی است. هدف از این مقاله، ارایه روشی مناسب برای انتخاب ویژگی های مهم کلینیکی و ژنتیکی و پیش بینی میزان دوز وارفارین بود.
    روش بررسی
    این مطالعه تجربی، از اردیبهشت تا خرداد 1394 بر روی 552 نفر از بیمارانی که در بیمارستان مرکز قلب تهران کاندید استفاده از وارفارین بودند، انجام گرفت. عوامل تاثیرگذار در میزان دوز استخراج شده، و روش های انتخاب ویژگی و شبکه های عصبی در نرم افزار MATLAB (MathWorks، MA، USA) پیاده سازی گردید.
    یافته ها
    از بین دو الگوریتم به کار گرفته شده، الگوریتم بهینه سازی ازدحام ذرات دارای دقت مناسب تری بوده و برای میانگین مربعات خطا، جذر میانگین مربعات خطا و میانگین قدر مطلق خطا به ترتیب 0262/0، 1621/0 و 1164/0 به دست آمد.
    نتیجه گیری
    بر اساس نتایج به دست آمده، با تعداد ویژگی های کمتر، می توان به دقت مناسبی از پیش بینی دوز وارفارین دست یافت. همچنین از این مدل می توان به عنوان یک سیستم پشتیبان تصمیم گیری بهره برد.

    Viruses are important causes of acute and chronic diseases in humans. Newer viruses are still being discovered. Apart from frequently causing infections in the general community, many types of viruses are significant nosocomial pathogens that with emerging viruses has become a real issue in medical field. There are specific treatments, vaccine and physical barrier to fight some of these infections. Health care-associated viral infections are an important source of patient’s morbidity and mortality. The method of sterilization or disinfection depends on the intended use of the medical devices (comprising critical, semicritical and noncritical items) and failure to perform proper sterilization or disinfection of these items may leads to introduction of viruses, resulting in infection. Disinfection is an essential way in reducing or disruption of transmission of viruses by environmental surfaces, instruments and hands which achieves by chemical disinfectants and antiseptics, respectively. This review discusses about chemical agents with virocids properties (e.g. alcohols, chlorine compounds, formaldehyde, phenolic compounds, glutaraldehyde, ortho-phthaldehyde, hydrogen peroxide, peracetic acid, iodophor, ammonium compounds quaternary, bigunides and so on.), mechanisms of action and their applications in health care-associated viral infection control. As well as, we described an overview for hierarchy of viruses in challenge with disinfantans, effective agents on viral inactivation, i.e.targect viruses, viral stability or survival duration time in enviromental surfaces and hands. We explained disinfection of surfaces, challenges in emerging viral pathogens inactivation, viral resistance to chemical disinfectants and antiseptics. Because, there are laboratory studies and clinical evidences for some viruses which viral resistance to biocide or failure to perform proper disinfection can lead to infection outbreaks. Also, we described virucidal properties of antiseptics and introduced selected antiseptics with extensive virucidal action, because hands play an important role in the spread of many viral diseases, and regular proper hands hygiene is essential to decontaminate hands and can interrupt the spread of viruses. Here, we compared the currently available laboratory methods, standard methods from many countries and kinds of viruses in these methods for evaluation of virocide activity. Finally, it’s good to know: any disinfectant is not virocide unless it confirms by laboratory methods.

    Background
    Pre-diabetes is a high risk condition for developing type 2 diabetes mellitus and cardiovascular disease. The target of this study is evaluation of incidence rate of pre-diabetes and its risk factors in a long- term follow up in the Tehran Lipid and Glucose Study.
    Methods
    The Tehran Lipid and Glucose Study is a prospective study being accomplished on a characteristic sample of the Tehranian residents at District 13, targeted determining the prevalence and incidence of non-communicable diseases and their risk factors. Four phases of the study have been implemented, including three-year periods from 1999 to 2011. From an overall of 12808, twenty years old and older people at baseline, after excluding participants who did not have the necessary conditions, analyses were accompanied on a 2597 male and 3282 female to estimate pre-diabetes incidence and detect their risk factors.
    Results
    In men with incident pre-diabetes, age, fasting plasma glucose, 2-hour post-challenge plasma glucose and body mass index in addition to a positive family history of diabetes and
    Conclusion
    The study showed that the incidence of diabetes and its variants in the Iranian people is high.

    Background
    The major issue to address in obesity etiology is to identify the genetic changes in the disease and their occurrence in different populations. Uncovering these genetic changes may be important in developing potential biomarkers for early diagnosis and prognosis of obesity. Among all obesity susceptibility genes studied before, convincing association has been found with variants in the FABP2 gene and this disease; however, the contributions of these genetic variants in different populations and ethnic groups are not similar. Accordingly, this study was carried out to replicate the previous findings to assess whether a missense variation (rs1799883) in this gene is associated with obesity in the Tehran Lipid and Glucose Study (TLGS) population.
    Methods
    A case–control study was designed to determine the possible association between rs1799883 and occurrence of obesity “in phase IV of the study between the years of 2008 to 2011”. The study group consisted of 217 subjects with body mass index (BMI, kg/m2) greater than 30 as cases and 159 healthy individual as control group (1820). All subjects were recruited among the Tehran Lipid and Glucose Study (TLGS) participants in phase IV of the study between the years of 2008 to 2011. The genomic DNA was extracted from peripheral blood leucocytes using the salting out method and subsequently subjects were genotyped for this marker using The tetra-primer amplification refractory mutation system-polymerase chain reaction (ARMS-PCR). Association of risk allele with obesity was assessed using the SPSS software, version 20 (Chicago, IL, USA).
    Results
    The results showed no significant differences between case and control groups in terms of allele frequency (P=0.61). According to the findings, the presence of T allele as the risk allele was not associated with increased risk of obesity in carriers of this allele compared to individuals carrying the normal allele (OR=1.17; CI%95= 0.62-2.19, P=0.61).
    Conclusion
    The results did not support the previous findings of an association between genetic polymorphism in the FABP2 gene and risk of obesity. However, a number of replicated studies with other ethnicity are suggested to make a conclusion about the role of this genetic polymorphisms and susceptibility to obesity in Iranian population.

    Background
    Cesarean delivery is the most common surgical procedure and this prevalence is on the rise. Given these trends, cesarean wound complications, such as disruption or infection, remain an important cause of post-cesarean morbidity.
    Methods
    We conducted a single-center randomized controlled trial that included women with viable pregnancies (≥24 weeks) undergoing cesarean delivery at Motahary University Hospital, Urmia, Iran from April to November 2014. All cesarean types were included: scheduled or unscheduled and primary or repeat cesareans. Women were excluded for the following reasons: inability to obtain informed consent, immune compromising disease (e.g. AIDS), chronic steroid use, diabetic mellitus and BMI≥30. Of 266 women, 133 were randomized to staples and 133 women to suture group.
    Results
    The mean±SD age of the staples group was 27.6±5.4 years and mean±SD age of suture was 28.7±5.9 years. Multiparity is the most frequent in both groups that by using Chi-square test, no significant differences were observed between the two groups (P=0.393). The most frequent indication for cesarean section in both groups was history of cesarean section in staple 40 cases (30.1%) and suture 32 cases (24.1%). The survey was conducted using the Chi-square test was not significant (P=0.381). Pain at 6 weeks postoperatively was significantly less in the staple group (P=0.001).     Conclusion
    The staple group had low pain and operation time but had a significant wound disruption and scar. The patients who have suffered a significant wound disruption were affected by age (P=0.022) and BMI (P=0.001) at compared those who were not affected by factors such as age or high BMI as risk factors for open surgical wound.

    Background
    Spermatogenesis is a complex and highly organized process of proliferation and differentiation of spermatogonial stem cells. Spermatogonial stem cells (SSCs) as a unique stem cell have the potential to self-renewal, differentiation and transmit genetic information to the next generation and play a vital role in maintaining fertility. Sertoli cells as the only somatic cells within the seminiferous epithelium play central roles in the formation of niche and balance between self-renewal and differentiation by secrete many growth factors. Given the importance and widespread use of SSCs, particularly in the treatment of infertility, the aim of this study was to create an optimal environment for the proliferation of SSCs. So we decided to study of undifferentiated (ID4) and differentiated (c-Kit) gene expression in SSCs followed by co-culture with Sertoli cells for a one-month.
    Methods
    This experimental study was conducted from November 2013 to December 2014 in Department of Anatomy, School of Medicine, Tehran University of Medical Sciences, on immature NMRI mouse (6-3 days old). Initially, Sertoli cells and SSCs were isolated from neonates mouse testes during the two-step enzymatic digestion characteristics Sertoli cells with vimentin marker and SSCs with promyelocytic leukemia zinc-finger (PLZF) marker were confirmed. Then SSCs were cultured in two groups: co-culture with Sertoli and without co-culture (control). Undifferentiated (ID4) and differentiation (c-Kit) gene expression were evaluated by Real-time PCR technique.
    Results
    Spermatogonial stem cells purity was obtained 66.91% by flow cytometry. The relative expression levels of gene ID4 in co-culture group at the end of each week, compared to the control group showed a significant increase (P
    Conclusion
    According to the results of this study, co-culture with Sertoli cells maintains SSCs in the prolifration stage for long-term, so can be used to optimize the culture medium at the clinic.

    Background
    Coagulase-negative staphylococci (CoNS) were considered as contaminats previously, but, during the past decade considered as one of the most common photogenic bacteria in hospital. Resistance to beta-lactams especially methicillin in staphylococcus species is being worrying in hospitals. Rapid identification of mechanisms of resistance and confirmation of their resistance to methicillin is a basic principle for antibiotic treatment. The aim of this study was to determine antibiotic resistance, frequency of mecA gene, and determination of SCCmec types in CoNS isolates from teaching hospitals in Iran.
    Methods
    The descriptive cross-sectional study was carried out one hundred clinical samples isolated from patients with an average age of 7-69 years at teaching hospitals in Hamadan City, Iran, from September 2014 to February 2015. After confirmation of isolates by microbiological standard biochemical tests, antimicrobial susceptibility testing was performed by disk agar diffusion (DAD) method. After extraction of isolated genomicm, mecA gene was detected. Then, the types of SCCmec were performed by PCR.
    Results
    In this study, 387 clinical samples were collected which among 100 CoNS isolated, Staphylococcus epidermidis was the most prevalent species with frequency 55 (55%), followed by S. haemolyticus 40(40%) and S. saprophyticus 5(5%). The highest antibiotic susceptibility was to rifampin 96(96%) and the lowest resistance was detected for trimethoprim/sulfamethoxazole (TMP/SMX) 47(47%). None of the strains were resistant to vancomycin. Resistance to methicillin was detected in 50% of CoNS isolates. Typing of SCCmec was performed by The polymerase chain reaction (PCR). Frequency types of SCCmec was type III with frequency 13(13%), type V 11(11%), type II 6(6%), type IV 4 (4%), type I 3(3%) respectively. Thirteen isolated was not typable in this study.
    Conclusion
    The result of this study showed that a large percentage of coagulase-negative staphylococci are resistance to methicillin, and the prevalence of SCCmec type was type III, which encodes the largest number of resistance genes. This information could be use in epidemiological study for preventing of infectious control in hospital and health centers.

    Background
    Hypertension is one of the most common diseases in recent century with several complications. The purpose of this study was to evaluate the effect of hydroalcoholic extract of Juglans regia L. leaves (Walnut tree) on blood pressure and its interaction with the adrenergic system in male rats.
    Methods
    In this experimental study that established in the physiology lab, School of scinse in Shiraz University from September to October 2013, in order to determine some of hydroalcoholic extract of Juglans regia L. leaves effect on blood pressure, the present study was performed by following procedure: 10 adult male wistar rats weighing between 180-250g were used. They were divided into two groups (Each group contained 5 rats) randomly: Juglans regia L. leaf extract group and Juglans regia L. leaf extract and adrenaline group. Then each rat was anesthetized by IP injection of 1.2 g/kg urethane. After tracheostomy the femoral vine and artery were cannulated for drug injection and blood pressure recording respectively. Arterial cannula for recording arterial blood pressure connected to a pressure transducer (PowerLab, ADInstruments, Sydney, Australia). Blood pressure parameters were recorded before and after IV administration of hydroalcoholic extract of Juglans regia L. leaf, solvent, adrenalin and extract with adrenaline.
    Results
    The result showed a significant decrease of mean arterial pressure, systolic and diastolic pressure in response to extract with compare to control and sham group (P
    Conclusion
    It can be concluded that hydroalcoholic extract of Juglans regia L. leaf suggested as a hypotensive agent. It seems that this effect is probably due to inhibitory effect on adrenergic system.

    Background
    Warfarin is one of the most common oral anticoagulant, which role is to prevent the clots. The dose of this medicine is very important because changes can be dangerous for patients. Diagnosis is difficult for physicians because increase and decrease in use of warfarin is so dangerous for patients. Identifying the clinical and genetic features involved in determining dose could be useful to predict using data mining techniques. The aim of this paper is to provide a convenient way to select the clinical and genetic features to determine the dose of warfarin using artificial neural networks (ANN) and evaluate it in order to predict the dose patients.
    Methods
    This experimental study, was investigate from April to May 2014 on 552 patients in Tehran Heart Center Hospital (THC) candidates for warfarin anticoagulant therapy within the international normalized ratio (INR) therapeutic target. Factors affecting the dose include clinical characteristics and genetic extracted, and different methods of feature selection based on genetic algorithm and particle swarm optimization (PSO) and evaluation function neural networks in MATLAB (MathWorks, MA, USA), were performed.
    Results
    Between algorithms used, particle swarm optimization algorithm accuracy was more appropriate, for the mean square error (MSE), root mean square error (RMSE) and mean absolute error (MAE) were 0.0262, 0.1621 and 0.1164, respectively.
    Conclusion
    In this article, the most important characteristics were identified using methods of feature selection and the stable dose had been predicted based on artificial neural networks. The output is acceptable and with less features, it is possible to achieve the prediction warfarin dose accurately. Since the prescribed dose for the patients is important, the output of the obtained model can be used as a decision support system.
