صفحه 1زمینه و هدفعفونت های استرپتوکوک گروهGBS B از دلایل مهم مرگ و میر و بیماری در نوزدان تازه متولد شده و تب های بعد از زایمان مادران می باشد. تشخیص سریع ارگانیسم در زنان حامله، جهت شروع درمان به موقع ضروری است. در این مطالعه ما کارآیی دو روش PCR و کشت را جهت غربالگری زنان حامله ناقل GBS بررسی کردیم.روش بررسیدر این تحقیق از 125 خانم باردار که در هفته 37-35 بارداری بودند، نمونه هایی از مقعد و واژن گرفته و سپس بوسیله دو روش کشت استاندارد بر روی Todd Hewith Broth و بلادآگار و همچنین تعیین ژن cfb بوسیله آزمون PCR مورد بررسی قرار گرفت.یافته هااز 125 نمونه مورد آزمایش، کلونیزاسیون GBS بوسیله روش کشت در 10 نفر (8%) و بوسیله آزمون PCR، 12 نفر (9.6%) حامل میکروب GBS از واژن و رکتوم بودند. در مقایسه با نتایج کشت حساسیت آزمون PCR و ارزش اخباری منفی آن به ترتیب 100% و 100% بود. ویژگی و ارزش اخباری مثبت در آزمون PCR به ترتیب 98% و 83% می باشد. همچنین زمان لازم برای حصول نتیجه در آزمون PCR حدودا 2 ساعت در حالیکه در روش کشت 36 ساعت می باشد.نتیجه گیریبا توجه به نتایج مطالعه اینگونه می توان استنباط کرد که روش PCR دارای حساسیت و ویژگی بالایی بوده و کلونیزاسیون استرپتوکوک گروه B را با اطمینان و به سرعت می توان تشخیص داد.

سودوموناس آئروژینوزا یکی از باکتری های بیماریزای فرصت طلب بسیار مهم می باشد. سویه های موکوئیدی سودوموناس آئروژینوزا تولید مواد بسیار موکوئیدی می کنند که از آلژینات تشکیل شده است و مهمترین نقش را در تشکیل بیوفیلم دارد. در این مطالعه اثر اسانس بابونه بر تشکیل بیوفیلم در سودوموناس آئروژینوزا مورد بررسی قرار گرفته است.
روش بررسیاز سودوموناس آئروژینوزا 8821M به عنوان سویه استاندارد در تشکیل بیوفیلم استفاده شد. برای بررسی اثر ضد میکروبی اسانس بابونه با غلظت 50% در DMSO از روش دیسک دیفیوژن استفاده شد. اثر اسانس بر تشکیل بیوفیلم در محیط L.B حاوی غلظت های 0.2، 0.35 و 0.5 میکروگرم از اسانس در میلی لیتر و بعد از گرمخانه گذاری به مدت 24 ساعت در 37°C به روش Fonseca اندازه گیری شد. از محیط کشت تلقیح شده بدون اسانس به عنوان شاهد مثبت و از محیط کشت تلقیح نشده بدون اسانس به عنوان شاهد منفی استفاده شد.
یافته هااسانس بر باکتری فاقد خاصیت ضد میکروبی بود، و در حضور اسانس با غلظت های 0.2 mg/ml و 0.35 mg/ml تولید بیوفیلم اختلاف معناداری با شاهد مثبت نداشت، اما تولید بیوفیلم در حضور 0.5 mg/ml اسانس، کاهش معناداری نسبت به شاهد مثبت داشت.
نتیجه گیریهر چند اسانس بابونه اثر ضدمیکروبی بر روی سودوموناس آئروژینوزا ندارد، اما باعث کاهش تولید بیوفیلم می گردد. از آنجا که تشکیل بیوفیلم یکی از فاکتورهای بیماریزایی در سویه های موکوئیدی می باشد، لذا احتمالا با مطالعات بیشتر امکان کنترل عفونت های مربوطه با استفاده از این نتایج نیز وجود دارد.

صفحه 15زمینه و اهدافویبریو کلرا عامل بیماری اسهالی وبا می باشد، که بعنوان عامل بیماری و مرگ و میر بسیاری از مناطق جهان محسوب می گردد. سویه های O1 و O139 ویبریو کلرا عامل ایجاد کننده وبای کلاسیک بوده و دیگر سویه های ویبریو کلرا از جمله ویبریوهای غیر آگلوتینه شونده (NAG) می توانند عامل بیماری اسهالی در انسان بصورت تک گیر باشند. به دلیل مشابهت خصوصیات بیوشیمیایی، تعیین هویت این ارگانیسم را مشکل ساز می نماید. در تحقیق حاضر انجام تکنیک PCR جهت تعیین هویت ویبریو کلرا مورد استفاده قرار گرفت و نیز سویه های NAG جداشده از نظر سروتیپ O139 مورد بررسی قرار گرفتند.روش بررسیتعداد 156 مورد ویبریو کلرای جدا شده از نمونه های محیطی و بالینی توسط کشت و تست های بیوشیمیایی و سروتایپینگ، از نظر مولکولی مورد بررسی قرار گرفتند. تست PCR با استفاده از پرایمرهای اختصاص کد کننده پروتئین های غشا خارجی (OmpW) برای تایید موارد ویبریو کلرا و پرایمرهای اختصاصی O139-rfb برای جستجوی سروتایپ O139 روی سویه های NAG انجام گرفت.یافته هادر روش متداول آزمایشگاهی، 6 مورد به عنوان ویبریو کلرای التور O1 سروتایپ اوگاوا (5 عدد) و هیکوجیما (یک عدد) و 150 مورد بعنوان سوش های غیر آگلوتینه شونده ویبریو کلرا (NAG) جدا سازی شدند. توسط تکنیک PCR، از 156 نمونه، 136 نمونه (87.1%) مثبت شد که نتایج کشت را تایید می نمود. هیچیک از سویه های NAG توسط پرایمرهای اختصاصی O139 مثبت نگردیدند.نتیجه گیریدر تحقیق حاضر تکنیک PCR مزیت قابل توجهی نسبت به روش های تشخیصی غیر مولکولی نشان نداد. گونه غالب ویبریوکلرا در منطقه مورد بررسی NAG تشخیص داده شد. تعداد موارد معدود بیماریزای جدا شده سروتیپ O1 اوگاوا بود و هیچ موردی از O139 در منطقه شناسایی نگردید.

صفحه 21زمینه و اهدافهلیکوباکترپیلوری پاتوژنی با پتانسیل بالای تغییرات ژنتیکی است. این باکتری می تواند میلیون ها انسان را بصورت مزمن در سراسر جهان آلوده کند. هلیکوباکترپیلوری از عوامل مهم زخم های پپتیک و سرطان معده در بیماران مبتلا است. هدف از این مطالعه تعیین ژنویپ هر یک از سویه های هلیکوباکترپیلوری جداشده از بیماران مبتلا به زخم های پپتیک، سرطان معده و سوء هاضمه های بدون زخم با استفاده از روش RAPD-PCR می باشد.روش بررسی80 بیوپسی از بیماران با مشکلات معدی که توسط پزشک متخصص تشخیص داده شده بود از بخش آندوسکوپی بیمارستان ها بدست آمد. نمونه های بیوپسی بر روی محیط اختصاصی تحت شرایط میکروآئروفیلیک طی 3 الی 5 روز کشت داده شد. سپس ژنوم باکتری استخراج گردید و RAPD-PCR برای تعیین ژنوتیپ مورد استفاده قرار گرفت.یافته ها6 الگوی مختلف (A-F) با استفاده از این روش بدست آمد. این الگوها عبارتند از: الگوی A: 10 ایزوله (16.98%)، B: 6 ایزوله (11.33%)، C: 5 ایزوله (9.43%)، D: 3 ایزوله (5.66%)، E: 2 ایزوله (3.77%) و F: 2 ایزوله (3.77%). ضمنا 26 ایزوله (49.06%) باقیمانده از 54 ایزوله دارای الگوی واحد و مشابه ای با یکدیگر و با شش الگوی دیگر نبودند. ارتباط معنی داری بین هیچ کدام از الگوهای RAPD و بیماری خاص معدی در این مطالعه دیده نشد (P>0.05).نتیجه گیرینتایج این تحقیق نشان می دهد که تغییرات ژنومی زیادی در سویه های هلیکوباکترپیلوری مورد بررسی در این مطالعه وجود دارد و بهتر است تعیین ترادف ژنومی بعنوان روش مکملی همراه با روش RAPD-PCR بکار رود.

صفحه 27زمینه و اهدافظهور مقاومت باکتری ها نسبت به آنتی بیوتیک های بتالاکتام مربوط به پلاسمید کد کننده پنی سیلیناز است که به سرعت در بین نمونه های کلینیکی مربوط انتشار یافته است. پلاسمیدهای ESBL اجداد SHV-1، TEM-2، TEM-1 هستند که هم اکنون با جهش نقطه ای در آن باعث بروز بیش از 90 تیپ TEM و 25 تیپ SHV شده است. در همین راستا مطالعه ای جهت بررسی فراوانی انواع پلاسمیدهای ESBL در سویه های اشریشیاکلی و کلبسیلا پنومونیه انجام گرفته است.روش بررسیدر این پژوهش تعداد 253 نمونه (ادرار، خون، آسیت، زخم و تراشه) ارسالی به بخش باکتری شناسی بیمارستان الزهرا (ص) اصفهان کشت و مورد بررسی قرار گرفتند که در این میان 148 سویه اشریشیاکلی و 70 سویه کلبسیلا پنومونیه بوسیله تست های بیوشیمیایی شناسایی شدند. سپس با استفاده از روش دیسک ترکیبی و سینرژیسم دوبل سویه های تولید کننده آنزیم های ESBL شناسایی گردیدند. همچنین تعداد 13 سویه اشریشیاکلی و 10 سویه کلبسیلا پنومونیه با استفاده از کیت استخراج پلاسمید و بکار بردن پرایمرهای اختصاصی TEM-1 و SHV-1 توسط روش PCR، پلاسمیدهای، TEM و SHV شناسایی گردیدند.یافته هانتایج حاصل از این پژوهش نشان داد که در اشریشیاکلی 10.1% مقاومت کروموزومی و 50% پلاسمیدی و در کلبسیلا پنومونیه 12.8% مقاومت کروموزومی و 63% پلاسمیدی می باشد، در حالیکه بقیه نمونه ها حساس به سفالوسپورینهای وسیع الطیف بودند. نتایج حاصل از PCR نشان داد که از کل سویه های اشریشیاکلی مورد مطالعه 84.6% پلاسمید TEM و 69.2% دارای پلاسمید SHV و 53.8% از آنها دارای دو پلاسمید TEM و SHV بودند. در نمونه های کلبسیلا پنومونیه نیز 80% پلاسمید SHV و 80% دارای پلاسمید TEM و 63% سویه ها دارای هر دو پلاسمید TEM و SHV بودند.نتیجه گیریبا توجه به مقاومت بالای سوی ها به آنتی بیوتیک های بتالاکتام، ضرورت شناسایی کامل ESBLها توسط آزمایشگاه، شناخت مکانیسم های مقاومت ESBLها توسط پزشکان و استفاده از آنتی بیوتیک های دارای قدرت ممانعت کننده بتالاکتام در کاهش ظهور مقاومت بسیار موثرند.

صفحه 35زمینه و اهدافکلامیدیا تراکوماتیس یکی از علل شایع عفونت های منتقله از راه جنسی است که می تواند عوارض شدیدی در پی داشته باشد. بدیهی است که برای طراحی سیاست مناسب در جهت کنترل و پیشگیری این عفونت ابتدا بایستی اپیدمیولوژی این عفونت در جمعیت های مختلف مشخص گردد. بنابراین در این مطالعه بر آن شدیم تا شیوع سطح سرمی آنتی بادی ضد کلامیدیا تراکوماتیس با توجه به متغیرهای مخلتف در شهر مشهد بررس گردد.روش بررسیدر این مطالعه سرم 76 بیمار دارای علایم عفونت های سیستم تناسلی پس از تکمیل پرسشنامه از نظر آنتی بادی های ضد کلامیدیا تراکوماتیس توسط آزمایش الیزا و ایمنوفلورسانت مورد بررسی قرار گرفت و نتایج به کمک نرم افزار SPSS تحلیل گردید.یافته هادر روش الیزا (ELISA) 11 نفر دارای تیتر IgG مثبت و 3 نفر دارای تیتر IgM مثبت بودند و در اندازه گیری تیتر آنتی بادی به روش ایمنوفلورسانس مستقیم 1 نفر مرد دارای تیتر 1.32، 6 نفر دارای تیتر 1.64، 3 نفر دارای تیتر 1.128 و 1 نفر زن دارای تیتر 1.256 آنتی بادی ضدکلامیدیا تراکوماتیس بود.نتیجه گیریاین مطالعه به خوبی نشان داد که شیوع کلامیدیا در منطقه ما به اندازه قابل توجهی بالا است که این مساله غربالگری و درمان آنرا ضروری می نماید. بنابراین باید آزمایشاتی جهت جستجوی کلامیدیا در عفونت های ادراری تناسلی نیز بعنوان یکی از آزمایشات روتین در بررسی های عفونت های تناسلی جای بگیرد.

صفحه 43زمینه و اهدافپریتونیت خود به خودی عارضه مکرر و اغلب کشنده عفونت مایع آسیت بدون علل داخل شکمی می باشد. در بالغین، باکتری های عامل پریتونیت خود به خودی معمولا باکتری های گرم منفی می باشند، اما در کودکان این عوامل متفاوت است. این باکتری ها اغلب مقاوم به آنتی بیوتیک ها می باشند، لذا شناسایی عوامل سببی و تعیین مقاومت آنتی بیوتیکی آنها ضروری است. هدف از این مطالعه، تعیین کردن عوامل سببی پریتونیت خودبه خودی باکتریایی و تعیین الگوی مقاومت آنتی بیوتیکی آنها در کودکان مبتلا به اختلالات کبدی و آسیت می باشد.روش بررسیدر این مطالعه از 85 بیمار مبتلا به بیماری های کبدی، نمونه مایع آسیت گرفته شد و با استفاده از آزمایش مستقیم، کشت و تست های بیوشیمیایی عوامل میکروبی شناسایی شدند. سپس برای نمونه های مثبت، تست آنتی بیوگرم (به روش دیسک دیفیوژن) انجام شد و در نهایت نتایج بدست آمده آنالیز و مورد بررسی قرار گرفت.یافته هااز بین 85 نمونه مورد بررسی، 32 مورد باکتری و 2 مورد مخمر کاندیدا جدا گردید. از 32 مورد باکتریایی، اشریشیاکلی (31.25 درصد) و استافیلوکوک کوآگولاز منفی (18.75 درصد) بیشترین باکتری های جدا شده بودند و بقیه شامل استرپتوکوک ها و انتروباکتریاسه بودند. همچنین از آنتی بیوگرام های انجام شده مشخص شد که بیشتر استافیلوکک های کوآگولاز منفی جداشده، به کوتریموکسازول، آموکسی سیلین، پنی سیلین و سفالوسپورین های نسل اول مقاوم هستند و اغلب باکتری های گرم منفی جدا شده نسبت به آمیکاسین، کربوکسی سیلین، ونکومایسن و جنتامایسین مقاومت نشان دادند.نتیجه گیریمطالعه حاضر نشان داد که اشریشیاکلی و استافیلوکوک های کوآگولاز منفی مهم ترین عوامل سببی پریتونیت خودبه خودی در کودکان می باشند و سفالوسپورین های نسل سوم (مانند سفتریاکسون) و سفوکسیتین می توانند آنتی بیوتیک مناسب جهت درمان تجربی این کودکان باشند.

گیاهان دارویی در طب سنتی برای درمان انواع بیماری ها از جمله بیماری های عفونی بکار می روند. با توجه به اینکه درمان عفونت های ویروسی با داروهای شیمیایی موجود به علت پیدایش مقاومت دارویی در ویروس ها معمولا با مشکلاتی روبرو می شوند، نیاز مبرم به داروهای ضد ویروسی نوین همیشه احساس می شود. در این پژوهش بیست و پنج گونه گیاهان دارویی ایران که در طب سنتی بر علیه بیماری های عفونی بکار می روند از نظر داشتن اثرات ضد ویروسی در برابر آدنو ویروس، ویروس سرخک، روتا ویروس، اکو ویروس 11 و HSV-1 مورد بررسی قرار گرفتند.
روش بررسیعصاره آبی گیاهان مورد نظر تهیه گردید و آستانه توکسیسیته آنها روی دودمان های سلولی RD، BSC-1، Hep-II، Vero با روش جذب رنگ قرمز خنثی و مشاهده میکروسکوپی CPE تعیین گردید. اثرات ضد ویروسی داروها با روش های سنجش مهار اثرات CPE و سنجش کاهش پلاک مورد بررسی قرار گرفتند.
یافته هااز بین گیاهان مورد بررسی عصاره برگ نیلوفر سفید، سماق، مامیران و هلیله زرد روی HSV-1 و آدنو ویروس موثر بودند. غلظت 1000 mg/ml مامیران بطور کامل تکثیر HSV-1 و آدنو ویروس را مهار کرد؛ ولی اثر آن روی HSV-1 چشمگیرتر بود؛ بطوریکه غلظت 500 mg/ml آن نیز از تکثیر ویروس کاملا جلوگیری می کرد. هلیله زرد اثر کمتری در ممانعت از تکثیر هر دو ویروس از خود نشان داد.
نتیجه گیریگیاهان دارویی یکی از زمینه هایی هستند که با بررسی آنها با احتمال قریب به یقین می توان به عوامل دارویی موثری دست پیدا کرد. در این بررسی نیز اثر ضد ویروسی چهار نوع از آنها مشخص گردید ولی برای مشخص شدن مکانیسم اثر داروها و بدست آوردن داروهای موثری از آنها، پژوهش های بیشتری لازم است.

صفحه 61زمینه و اهدافعفونت با ویروس BK معمولا در کودکی رخ می دهد و موجب نهفتگی ویروس می شود. فعال شدن این ویروس در افرادی که پیوند سلول های بنیادی هماتوپوئتیک انجام داده اند بنظر می رسد با عفونت تاخیری خونریزی دهنده مثانه مرتبط باشد. این خونریزی ممکن است به صورت میکروسکوپی یا ماکروسکوپی همراه با لخته و انسداد مجرای ادرار و ایجاد مشکلات قابل توجه برای بیمار شود.روش بررسیدر این مطالعه گذشته نگر اطلاعات مربوط به 31 دریافت کننده سلول های بنیادی هماتوپوئتیک شامل 18 کودک و 13 بالغ که در سال های 2002 و 2003 در بیمارستان دانشگاه هودینگ (Hudding University) مورد پیوند قرار گرفته بودند و داوطلب شرکت در این مطالعه بودند بررسی گردید و در مجموع 127 نمونه ادرار از یک هفته قبل از دریافت سلول های بنیادی ه هماتوپوئتیک تا 12 ماه پس از آن گردآوری شد و با روش Nested PCR از نظر وجود DNA ویروس BK مورد آزمایش قرار گرفت. تمام نمونه های BK مثبت با روش Real Time PCR کمی از نظر تعداد کپی های ویروس BK در یک میکرولیتر ادرار مورد بررسی قرار گرفت و نتایج با استفاده از تست کای اسکور آنالیز آماری شدند.یافته هاDNA ویروس BK به کمک روش های Nested PCR و Q-RT-PCR در نمونه ادرار 16 نفر از 31 نفر (52%) افراد دریافت کننده سلول های بنیادی هماتوپوئتیک نشان داده شد. 5 نفر از 6 نفری که از نظر ویروس BK مثبت بودند و عفونت خونریزی دهنده مثانه داشتند قبل از شروع عفونت دارای ویروس BK در ادرار بودند. در صورتی ویروس BK فقط در ادرار 10 نفر از 25 نفری که مبتلا به عفونت نشدند دیده نشد که حاکی از تفاوت معنی دار وجود ویروس BK در ادرار مبتلایان با غیر مبتلایان به عفونت خونریزی مثانه است (P=0.04).نتیجه گیریوجود ویروس BK در ادرار افرادی که سیستم ایمنی شان تضعیف شده است نشان فعال شدن این ویروس است که در هر دو گروه بیماران عفونت خونریزی دهنده و غیر بیماران مشاهده می شود و به تنهایی برای پیش بینی وقوع این عفونت کافی نیست و بدین منظور می توان از مشاهده بیش از 106 ویروس در یک میکرولیتر ادرار با روش Q-RT-PCR استفاده کرد. این مطالعه حاکی از این است که میزان DNA ویروس BK و بویژه وجود بیش از 106 ویروس در یک میکرولیتر ادرار فرد دریافت کننده سلول های بنیادی هماتوپوئتیک ممکن است توانایی پیش بینی عفونت خونریزی دهنده مثانه را داشته باشد این ارتباط وقتی بیشتر می شود که وجود 106 ویروس در یک میکرولیتر ادرار همراه با بیماری (GVH) Graft- Versus- Host حاد باشد.

Page 1Background And ObjectivesGroup B Streptococcus (GBS) (Streptococcus agalactiae) is the leading cause of morbidity and mortality of newborn infants and accounted as a leading factor causing septicemia after birth in mothers. Infections in infants are usually acquired by contact with the genital tract of the mothers during labor and delivery. In two last decades, significant progress toward detection, prevention and treatment of pregnant women carrying GBS has been achieved. A rapid screening test for GBS that could accurately identify pregnant women carrying the bacteria at the time of delivery would obviate the need for prenatal screening. The standard method for the diagnosis of GBS colonization consists of culturing vaginal and anal secretions in a selective broth medium which inhibits the growth of other microorganisms. Today, it is accepted that PCR has a high sensitivity and specifically in diagnosis. The goal of this study was to screen pregnant woman carrying GBS by PCR.Material And MethodsSamples were taken from anal and vaginal mucus of 125 pregnant women who were at 28-38 weeks of ingestion by swab. Samples were tested by standard culture using Todd Hewitt Broth and Blood Agar and also by PCR using primers specific for cfbgene.ResultsCulture identified 10 (8%) women as carriage of GBSout of 125 women tested. On the other hand, the PCR assay could identify 12 (9/6%) women positive for GBS. In comparison to culture results, sensitivity, NPV, specificity, and PPV of PCR were 100%, 100%, 98%, and 83%, respectively. The time required for PCR assay and culture were 2h and 36h,respectively.ConclusionWe found that GBS can be detected rapidly and reliably by a PCR assay using combined vaginal and anal secretions from pregnant women at the time of delivery. Also this study shows that the rate of incidence of GBS is high in Iranian pregnant women. Pseudomonas aeruginosais an important opportunistic pathogen. The mucoid strains of P. aeruginosa produce hyper viscous substances consisting mainly of alginate which have important roles in formation of biofilm.We investigated the effect of essential oil of Matricaria chamomilla L. on biofilm production in P. aeruginosa.
Material And MethodsP. aeruginosa 8821M was used as standard strain for biofilm production. Antibacterial effects of essential oil of M. chamomilla L. (50% in DMSO) was tested by disk diffusion method. The effect of essential oil on biofilm formation of P. aeruginosa 8821M was evaluated following inoculation of bacteria in LB broth medium containing 0.5, 0.35 and 0.2 µg/ml of oil which were incubated for 24h at 37°C. The biofilm formation was measured by Fonseca method. Bacteria inoculated and uninoculated media without oil were used as positive and negative controls, respectively.
ResultsThe results showed that the essential oil did not have any antibacterial effect or reduction in biofilm formation in the presence of 0.35 and 0.2 µg/ml of oil. On the other hand, bacteria biofilm formationwas significantly reduced in the presence of 0.5µg/ml of oil in comparison with positive control.
ConclusionThis research showed that the essential oil of Matricaria chamomilla L. had no antibacterial effect, but caused reduced biofilm formation in P. aeruginosa. Biofilm formation is an important virulence factor in mucoid strains and our results may suggest the possible use of essential oil in control of infections caused by P. aeruginosa or other related infections

Page 15Background And ObjectivesVibrio cholerae, the etiologic agent for the diarrheal disease of cholera, continues to be an important cause of mortality and morbidity in many parts of the world. V. cholera serotypes O1 and O139 are associated with classic cholera, however, other V. cholera strains, including nonagglutinable vibrios (NAG) are occasionally isolated from the cases of diarrhea. Identification of V. cholera is usually achieved through a series of culture and biochemical tests, but close relatedness among V. cholera and other member of Vibrio spp. or Aeromonas spp. has often made identification of the organism quite difficult. The objective of this study was evaluation of PCR targeting outer membrane proteins (ompW) for detection of V. cholera in comparison with conventional method of culture and biochemical tests.Material And MethodsA total of 156 V. cholera isolates from both clinical and environmental sources identified on the basis of conventional culture, biochemical tests and serotyping. Polymerase chain reaction (PCR) assay was carried out using primers targeting the gene of outer membrane proteins. Second PCR assay was also performed using primers based on O139-rfbregion within the V. cholerae chromosome.ResultsBased on the results from biochemical tests and serotyping, 6 isolates were identified as V. cholera O1, serotypes Ogawa (five cases) and Hikojima (one case) and 150 non-agglutinable vibrios (NAG). PCR showed 136 isolates (87.9%) were positive for V. choleraand 20 others (12.1%) were negative. PCR results on NAG isolates revealed none of the isolates were belong to O139 serotype.ConclusionIn the present study, PCR assay showed no priority over the conventional methods. The prevalent V. cholerae isolates in the region of study were NAG and the least dominant isolates were O1 Ogawa-serotype. Page 21Background And ObjectivesHelicobacter pylori is a genetically diverse gastric pathogen that chronically infects billions of people worldwide, typically beginning in infancy and lasting for decades. It is a major cause of peptic ulcers and it is an early risk factor for gastric cancer which is the most frequently lethal malignancy globally. This project was designed to genotype H. pyloriisolates isolated from patients with NUD, DU, GU and GC by the polymerase chain reaction(PCR)-based on Randomly Amplified Polymorphic DNA (RAPD) fingerprinting technique.Material And MethodsEighty patients admitted to the gastroenterology unit at Sharyati hospital in Iran were included in this study. Gastric biopsy specimens were inoculated onto selective medium then were cultured for 3 to 5 days at 37 ° C under microaerobic conditions. Genomic DNA was extracted using a commercially available Qiagen kit. RAPD-PCR was used to genotype isolates.ResultsSix different RAPD patterns (A-F) were seen in morethan one isolate which were as follow; pattern A: 9 (16.98%), B: 6 (11.33%), C: 5 (9.43%), D: 3 (5.66%), E: 2 (3.77%) and F: 2 (3.77%). Twenty six (49.06%) of 53 isolates showed a unique RAPD pattern that were not similar to each other. A significant relationship was not seen between a single RAPD pattern and a gastric disorder (P>0.05).ConclusionThe results of this study suggest a high level of DNA sequence diversity among H. pylori isolates and it is better to use sequencing method for surveying of Helicobacter pylorigenome rather than RAPD-PCR. Page 27Background And ObjectivesThe Study of E.coli and Klebsiella resistance to wide spectrum antibiotics and molecular evaluation of resistance in these bacteria were examined. Treatment with wide spectrum antibiotics against bacteria can lead to resistance. The antimicrobial resistance can be seen in two types: 1) chromosomal alterations which could result in changes in structure of receptors of specific drugs. Most of this mutation can cause the absence of a penicillin binding proteins (pbps) and 2) is plasmid resistance. Plasmid genes can usually produce enzymes which result in destruction of antibiotics. An example is extended spectrum B-lactamase (ESBLs) which causes résistance to the third generation of cephalosporin, monobactams and new penicillin. ESBL plasmids are derived from TEM-1, TEM-2 and SHV-2. It has been recognized that because of point mutations in these plasmids, there are about 90 types of TEM and 25 types of SHV. The reason for the formation of point mutations within the ESBL plasmids is due to high consumption of broad spectrum antibiotics.Material And MethodsIn a cross-sectional study, 218 specimens from patients were collected and after identification of bacteria, the percentage of ESBLs among the isolates was calculated. For this purpose combined disk method and double disk method are used. Then TEM and SHV plasmids from 23 specimens of E.coli and Klebsiella were examined using plasmid extraction kit and PCR.ResultsThe result of this study showed; the antibiotic resistance in 10% of E.coli was chromosomal and 50% were plasmids. The remaining isolates were sensitive. In Klebsiella, 12.8% of resistance was chromosomal and 62% were due to plasmids and remaining isolates were sensitive. The results from PCR showed; in E. coli 52.8% of the isolates were TEM positive and 84.6% were SHV positive and 69.2% were positive for both TEM and SHV. For Klebsiella 80% were TEM positive and 80% were SHV and 60% were positive for both TEM and SHV.ConclusionThe rate of ESBLs in Iran is higher than the results in similar studies in other countries. This could be because of the overuse of third generation of cephalosporin. For the purpose of having proper treatment using antibiotics, medical education and laboratory detection of ESBLs could be effective to choose choice antibiotics.Keywords: E ESBLs, E.coli, Klebsiella, PCR

Appropriate preventive requires knowledge of epidemiology of infection in different population in order to target interventions in a cost-effective manner. In this study prevalence of C. trachomatis infections were determined according to some parameter in Mashhad.Materials and MethodsIn this study serum from 76 patients with STD were examined by ELISA and IFA for C. trachomatis. Statistical evaluation was done using SPSS program.ResultsELISA showed that 11 and 3 patients with IgG and IgM against C. trachomatis,respectively. IFA analysis showed that 1 patient had titer of 1/32, 6patients with 1/64 and 3 patients with 1/128. Onefemale patient showed titer of 1/256.ConclusionThis study provides strong evidence that Chlamydia prevalence in our region is significantly high which necessitate screening and treatment. Page 43Background And ObjectivesSpontaneous bacterial peritonitis (SBP) is a frequent and often fatal complication of ascites without a demonstrable intra abdominal cause. In adults, the organisms of SBP are usually gram-negative bacteria, but they may differin children. Since these organisms are resistant to most antibiotics, identification of active antimicrobial agents and determination of resistance pattern areessential. The aim of the present study was also to determine the causative agents of SBP in children with liver disease and ascites, referred to pediatrics ward of Imam Khomeini hospital during 1384-85.Material And MethodsIn this study, ascite samples were taken from 85 patients with liver disease and ascites of Emam Khomeini Hospital, pediatrics ward, and they were examined by direct test, culture on different media and biochemistry tests. Antibiogram tests by disk diffusion were done on each positive sample.ResultsOf 85 examined samples, 32 bacterial and 2 yeast agents were isolated. Of bacterial cases, Escherichia coli (31.25%) and coagulase negative Staphylococci (18.75%) were the most isolated agents and the rest, included Streptococci and Enterobacteriaceae. Moreover, antibiogram tests identify that most of coagulase negative Staphylococci isolates as resistant to cotrimoxazol, amoxicillin, penicillin and cephalosporin (first generation). The most of gram negative isolated bacteria were resistant to amikacin, vancomycin and gentamicin.ConclusionSince the spontaneous bacterial peritonitis is not detectable by clinical signs, ascite samples should be examined in order to determine the etiologic agents. In general spontaneous bacterial peritonitis agents are mostly composed of normal flora bacteria, in our study most isolated bacteria were Escherichia coli and coagulase negative staphylococci, two major normal flora of gastrointestinal tract and skin. The isolated bacteria showed a high antibiotic resistance against common drugs in our study. In general, this study showed that the major agents of spontaneous bacterial peritonitis should be identified by ascite examination and antibiogram test to establish a perfect treatment pattern in order to treat the patients rapidly.Keywords: spontaneous bacterial peritonitis, antibiotic resistance, Disk diffusion

Medicinal plants have been traditionally used for different kinds of ailments including infectious diseases. There is an increasing need for substances with antiviral activity since the treatment of viral infections with the available antiviral drugs often lead to the problem of viral resistance. There is a need to search for new and more effective antiviral agents, therefore in the present study 25 plants with ethno-medical background from different families were screened for antiviral activity against HSV-1, Adenovirus type 5, Echovirus type 11, Measles virus and Rotavirus.
Material And MethodsDifferent parts of the plants collected from Iran were extracted with aqueous solvents to obtain crude extracts. These extracts were screened for their cytotoxicity against Vero, BSC-1, Hep-II and RD cell lines by micro-culture neutral red dye absorption and microscopically follow up forCPE. Antiviral properties of the plant extracts were determined by cytopathic effect inhibition assay and plaque reduction assay.
ResultsFour plants extract; Nymphea alba, Rhus coriaria L., Chelidonium majusand Terminalia chebula Retzexhibited significant antiviral activity against HSV-1 and adenovirus type 5 at non-toxic concentration. The extracts of Chelidonium majus showed great anti viral activity against HSV-1 andpartial activity against adenovirus at higher concentrations.
ConclusionSome of the medicinal plants have shown antiviral activity. Further research is needed to elucidate the active constituents of these plants which may be useful in the development of new and effective antiviral agents.

Page 61Background And ObjectivesA possible temporal correlation between high BK virus (BKV) load in urine alone or in combination with acute graft versus host disease (GVHD) and the development of hemorrhagic cystitis (HC) was examined in this study.Material And Methods31 allogeneic hematopoietic stem cell transplanted (SCT) patients were include in this study. BKV DNA was detected by nested and quantitative Real-Time PCR in the urine of 16 out of 31 patients. HC occurred in 6/16 patients with BKV DNAin their urine samples. BKV load was evaluated in the urine samples from 5 of 6 HC patients.ResultsPresence of BKV or BKV load >106 copies alone in urine samples showed some predictive ability for HC, while acute GVHD alone or conditioning regiments did not. However, during the period after SCT to HC onset a combination of BKV load >106 copies and acute GVHD, discriminated the best between HC (4/5) and non-HC (2/25) patients (p=0.003).ConclusionThis study indicates that BKV DNA and particularly>106 BKV copies/µl of urine from SCT patients may have some predictive ability for HC. Page 67Background And ObjectivesCholerae disease caused by toxigenic V. cholerae, is a major public health problem in developing countries including Iran. Epidemiological surveillance and comparative molecular analysis of isolates have demonstrated clonal diversity among epidemic strains and a continual emergence of new clones of toxigenic V.cholerae.Material And MethodsA total of 20 V.cholerae strains were sent to Pasteur Institute of Iran in September of 2007 from Kordestan province which includes strains of different sub-serotypes. After biochemical identification, isolates subjected to molecular analysis including Pulsed-Field Gel Electrophoresis (PFGE) of NotI digested genomic DNA according to the standardized protocol by Centre of Disease Control (CDC).ResultsPFGE results showed a single pattern for all isolates.ConclusionThe results were interpreted in comparison with patterns obtained by isolates of previous years and showed clonal dissemination of a new clone in Kordestan province in this year.Keywords: Vibrio cholerae, Kurdastan, PFGE, Inaba, 2007
