صفحات 1-12زمینه و اهداففراوانی گزارش شده ی انواع عفونت های بیمارستانی در کشور بسیار متفاوت است. به نظر می رسد مرور سامانمند و فراتحلیل مستندات مرتبط برآورد دقیقتری از مختصات این موضوع جهت سیاستگذاری های صحیح ارائه دهد. لذا هدف از این مقاله مرور سامانمند و فراتحلیل شیوع و عوامل عفونت های بیمارستانی در ایران می باشد.مواد و روش کارتمام مقاله های منتشر شده در مجلات ایرانی و بین المللی، گزارش نهایی طرح های پژوهشی و مقالات مرتبط ارایه شده در کنگره ها و نیز پایان نامه های دانشجویی، با استفاده از کلید واژه های استاندارد و حساس مرور شدند. سپس تمام مقالات منتشر شده بین سال های 91-76 که دارای معیارهای ورود بودند، پس از کنترل کیفی، با استفاده از مدل تصادفی وارد فرآیند فراتحلیل شدند.یافته هایافته ها نشان می دهد بهترین برآورد از میزان شیوع کلی عفونت های بیمارستانی در ایران 30/43% است و شایع ترین عفونت های بیمارستانی، عفونت های تنفسی (39/4%)، عفونت های ادراری (23/88%)، باکتریمی (21/98%) و شایع ترین عوامل عفونی بیمارستانی سودوموناس آئروژینوزا (26/78%)، کلبسیلا (31/42%)، گونه های استافیلوکوک (23/6%) و اشریشیاکلی (30/93%) می باشند. هم چنین بین شیوع و عوامل یافته های پژوهش ها، یک عدم تجانس اساسی مشاهده گردید که با استفاده از روش متارگرسیونی عوامل اصلی ایجادکننده ی این عدم تجانس افراد مورد مطالعه، حجم نمونه ها، زمان مطالعه ها، جنسیت و میانگین سنی نمونه ها معرفی شدند.نتیجه گیریمرور ساده ی مستندات مطالعه شده در این بررسی نشان می دهد که میزان شیوع انواع عفونت های بیمارستانی در ایران نسبتا بالاست. از این رو تدوین برنامه های کنترلی و آموزشی مناسب و مبتنی بر شواهد جهت کاهش میزان شیوع عفونت های بیمارستانی در ایران بایستی مدنظر سیاست گذاران امر قرار گیرد.

صفحات 13-19زمینه و اهدافلیستریا منوسایتوژنز عامل مسبب لیستریوزیس و عفونت های کشنده در انسان است. هدف از این مطالعه تیپ بندی و بررسی ارتباط ژنتیکی سویه های جدا شده از نمونه های مواد غذایی به وسیله تکنیک MLVA است.مواد و روش کاربین سال های 89 تا 92 تعداد 317 نمونه مواد غذایی از سطح شهر تهران جمع آوری و پس از تشخیص نهایی، DNA آن ها به منظور انجام تکنیک MLVA استخراج شد، سپس عکس های حاصل از ژل محصولات PCR به وسیله نرم افزار Gene Tools آنالیز شد. در نهایت به کمک فرمول، تعداد تکرارهای هر لوکوس محاسبه و سویه ها تیپ بندی شدند.یافته هااز 317 نمونه غذایی تعداد 24 نمونه به وسیله تکنیک های استاندارد آزمایشگاهی برای لیستریا منوسایتوژنز مثبت بود و به کمک تکنیک MLVA این تعداد سویه در 13 تیپ مختلف قرار گرفتند و تیپ 2 با 6 سویه و تیپ 3 با 4 سویه به ترتیب فراوان ترین تیپ ها بودند.نتیجه گیرینتایج این مطالعه وجود لیستریا منوسیتوزنز را در نمونه های غذایی لبنی و گوشتی نشان می دهد، لذا توصیه می شود افراد مستعد، بخصوص زنان باردار در مصرف این مواد اصول بهداشتی را رعایت کنند. نتایج تیپ بندی نشان داد که سویه های جدا شده از منابع مختلف می تواند منشا یکسانی داشته باشند. MLVA تکنیکی آسان و با دقت بالا است و می تواند برای تیپ بندی و ارزیابی ارتباط ژنتیکی سویه های لیستریا منوسایتوژنز برای تعیین منشا آلودگی استفاده شود.

صفحات 20-27زمینه و اهدافهلیکوباکترپیلوری یکی از شایع ترین عفونت های مزمن باکتریال در سراسر جهان است. الگوی همه گیرشناسی آلودگی به این باکتری در کشورهای توسعه یافته و در حال توسعه متفاوت است. در مطالعه حاضر شیوع آلودگی به هلیکوباکترپیلوری و بررسی عوامل زمینه ای موثر در بروز این عفونت در بین ساکنین شهر پیشوای ورامین با روش سرولوژی بررسی شد.مواد و روش کاردر این مطالعه توصیفی، پس از تکمیل پرسشنامه، خون محیطی از314 نفر از افراد ساکنین پیشوای ورامین انجام گردید. در ادامه، تیتر IgG ضد آنتی ژن تام هلیکوباکتر پیلوری و IgG ضد CagA هلیکوباکترپیلوری با روش سرولوژی ELISA ارزیابی شد. متعاقبا اطلاعات به دست آمده با نرم افزار آماری SPSS تجزیه و تحلیل گردید.یافته هافراوانی IgG علیه آنتی ژن تام هلیکوباکترپیلوری در 164 نفر (55/2%) از 314 فرد مورد مطالعه مثبت و در 130 نفر (41/4%) منفی بود و نتایج برای آنتی بادی ضد CagA در 46 نفر (29/1%) از 158 فرد مورد مطالعه مثبت و در 105 نفر (66/5%) منفی و سایرین در محدوده مرزی بودند. ارتباط بین نتیجه مثبت سرولوژی آنتی CagA و IgG با سن، جنس، میزان تحصیلات و شغل، تعداد اتاق، تعداد افراد خانواده، مصرف کنسرو، استعمال دخانیات، بیماری قلبی، دیابت، دیالیزی و ریوی و میزان کلسترول، سابقه علائم گوارشی، سوزش معده و درد معده و ریفلاکس (p<0.05) دیده شد.نتیجه گیرینتایج این تحقیق نشان داد بیش از 55/2% افراد ساکن شهر پیشوای ورامین ازنظر سرمی مثبت و دارای هر دو نوع آنتی بادی IgG توتال و آنتی CagA می باشند. با توجه به شیوع بالا و عوارضی که به دنبال این آلودگی می تواند در افراد رخ دهد، لزوم آموزش بهداشت و کنترل دقیق عفونت توصیه می شود.

صفحات 28-35زمینه و اهدافبا توجه به اهمیت گزارش های فراوان از عفونت های ناشی از اسینتوباکتر بومانی مولد بتالاکتامازهای وسیع الطیف در سال های اخیر در کشورمان، این مطالعه باهدف تعیین مقاومت پادزیستی و تعیین فراوانی ژنهای کد کننده ی بتالاکتامازهای PERو VEB در ایزوله ی اسینتوباکتر بومانی از بیماران بیمارستان های تهران با روش PCR انجام شد.مواد و روش کار100 ایزوله ی بالینی اسینتوباکتر بومانی جمع آوری شده از بیمارستان های مختلف شهر تهران طی یک سال، با استفاده از محیط های کشت اختصاصی و تست های بیوشیمیایی شناسایی شدند. آنتی بیوگرام با 11 پادزیست مختلف با روش انتشار دیسک (Disk diffusion) بر اساس دستورالعمل CLIS انجام شد. در مرحله ی بعد MIC پادزیست های سفی پیم و سفتازیدیم انجام و جهت شناسایی سویه های مولد ESBL روش دیسک ترکیبی مورد استفاده قرار گرفت. در نهایت برای بررسی شیوع ژنهای بتالاکتاماز PERو VEB با استفاده از پرایمرهای اختصاصی،PCR انجام گردید.یافته هانتایج آنتی بیوگرام نشان داد که بیشترین مقاومت به ترتیب مربوط به پادزیست های آمیکاسین و سفی پیم و کمترین مقاومت مربوط به پادزیست پلی میکسین B بود. میزان مقاومت چند دارویی در این سویه ها حدود 70% و از مجموع ایزوله های مورد مطالعه حداقل غلظت بازدارنده نسبت به سفتازیدیم در 84% از نمونه ها MIC≥ 128 μg/mlو نسبت به سفی پیم در 91% از نمونه ها MIC≥ 128 μg/ml بود. برطبق نتایج آزمون دیسک ترکیبی نیز20% سویه ها مولد آنزیم بتالاکتاماز وسیع الطیف بودند. نتایج PCR جهت شناسایی سویه های حاوی ژن های مقاومت نشان داد که ژن های PER و VEB به ترتیب 47% و32% است.نتیجه گیریبا توجه به اینکه ژنهای PER و VEB در این باکتری شناسائی شده و امکان انتقال آنها به دیگر باکتری ها وجود دارد، لذا توصیه می گردد در الگو مصرف آنتی بیوتیک ها تجدید نظر گردد و به معیارهای کنترل عفونت های بیمارستانی توجه بیشتر شود.

صفحات 36-43زمینه و اهدافسودوموناس آئروژینوزا یک باکتری منحصر به فرد است که در محیط های مختلف، طی یک پروسه سازگاری پیچیده، جهت ادامه حیات، تغییرات مختلفی در بیان ژن های ویرولانس و مقاومت دارویی خود ایجاد می نماید. هدف این تحقیق بررسی وجود ارتباطی منطقی بین اگزوتوکسین A و مقاومت پادزیستی در سویه های واجد این ژن است.مواد و روش کارتست آنتی بیوگرام به روش انتشار دیسک (Kirby Bauer) براساس پروتوکل CLSI انجام شد. در این تحقیق، وجود ژن toxA به کمک واکنش زنجیره ای پلیمراز (PCR) در 102 سویه بالینی از نمونه های خون، زخم، ادرار و تراشه مورد بررسی قرار گرفت. برای بررسی ارتباط بین اگزوتوکسین A و مقاومت پادزیستی از آزمون کای 2 استفاده گردید.یافته ها81 سویه (79/4%) +toxA بودند. فراوانی ژن toxA در سویه های جدا شده از عفونت های مختلف عبارت بود از زخم (91/4%)، خون (85/7%)، تراشه (72/7%) و ادرار (42/1%). شاخص مقاومت چندگانه در سویه های واجد ژن toxA، 75% محاسبه گردید. نتیجه آزمون کای 2 برای تعیین ارتباط بین مقاومت دارویی و وجود ژن toxA معنی دار بود (P<0.05).نتیجه گیریمقدار معنی داری آزمون کای 2 و افزایش مقاومت چندگانه در سویه های +toxA جدا شده از بیماران بستری، می تواند بیانگر سوئیچ ژنتیکی قابل توجه بین فعالیت اگزوتوکسین A و مقاومت پادزیستی، به ترتیب در عفونت های خون، ادرار، تراشه و زخم باشد که منجر به روشن شدن ژن های تنظیم کننده مقاومت دارویی در باکتری می گردد. این نتایج با روش های ساترن بلات، آنالیز بیان ژن toxA و تعیین مکانیسم مقاومت در سویه های مقاوم قابل بررسی خواهد بود.

صفحات 44-49زمینه و اهدافمهم ترین اصل در داروسازی حصول اطمینان از سلامت دارو است. داروها متناسب با عوامل ترکیبی خود می توانند محیط مناسب برای رشد و تکثیر میکروارگانیسم های گوناگون باشند. هدف از این تحقیق شناسایی میکروارگانیسم های مواد اولیه، آب و محیط اطراف و از طرف دیگر بررسی تاثیر ضد میکروبی مواد ضدعفونی کننده در حذف آن ها می باشد.مواد و روش کاردر این مطالعه توصیفی از 5 مرحله 52 سری تولید کارخانه تولید شربت معده قبل و بعد از ضدعفونی کردن نمونه برداری انجام گردید.از روش شمارش در پلیت برای اندازه گیری میکروارگانیسم ها استفاده شد. در صورت وجود آلودگی از محیط کشت های اختصاصی و تست های بیوشیمیایی برای تشخیص میکروارگانیسم هایی مثل استافیلوکوکوس اورئوس، اشربشیا کلی، سودوموناس آئروژینوزا، آسپرژیلوس نایجر و گونه های سالمونلا استفاده گردیدیافته هانتیجه آزمایش ها نشان داد که میکروکووس در 3 نمونه، سراشیا در 2 نمونه، باسیلوس سوبتیلیس در 2 نمونه، سالمونلا در 1 نمونه، استافیلوکوکوس اورئوس در 1 نمونه و آسپرژیلوس نایجر در 1 نمونه حضور داشتند.نتیجه گیریآزمایش ها نشان داد نمونه های آلوده 19/2% می باشند. 13/5% از آن ها شامل میکروارگانیسم های غیر بیماری زا، با بیش از CFU 100 در هر میلی لیتر می باشند. 5/7% از نمونه های باقی مانده شامل میکروارگانیسم های بیماری زایی مانند استافیلوکوکوس اورئوس 1/1%، سالمونلا 1/1% و آسپرژیلوس نایجر 1/1% بوده اند.

صفحات 50-58زمینه و اهدافپروتئازهای قلیایی در صنایع مختلفی از جمله داروسازی، عکاسی، ابریشم، غذایی، چرم سازی، شوینده و... کاربرد دارند. اکتینومیست ها باکتری هایی گرم مثبت هستند که آنزیم های مختلفی از جمله پروتئازها را تولید می کنند. هدف از این پژوهش غربال گری اکتینومیست های بومی مولد پروتئاز قلیایی از نمونه های خاک و بهینه سازی شرایط تولید آنزیم پروتئاز قلیایی آن ها می باشد.مواد و روش کاردر این تحقیق اکتینومیست های مختلف از نمونه خاک مناطق استان های خوزستان، چهارمحال و بختیاری و اصفهان غربال گری شدند و پس از تعیین بهترین سویه های مولد آنزیم پروتئاز قلیایی با سنجش به روش لوری، بهینه سازی شرایط تولید این آنزیم انجام گرفت.یافته هااز میان سویه های اکتینومیست مولد پروتئاز قلیایی غربال گری شده از خاک بیشترین فعالیت آنزیمی متعلق به سویه استرپتومایسس دی استاتیکوس بود. حداکثر فعالیت آنزیم در غلظت 10 گرم بر لیتر ساکارز بود. pHو دمای بهینه برای فعالیت آنزیم پروتئاز قلیایی به ترتیب pH برابر 10 و دمای 30 درجه سلسیوس بود. حداکثر فعالیت آنزیم پروتئاز قلیایی در سرعت هوادهی 200 دور بر دقیقه اتفاق افتاد.نتیجه گیریاین تحقیق اولین گزارش از تولید پروتئاز قلیایی توسط استرپتومایسس دی استاتیکوس در ایران است. آزمون های انجام شده در این تحقیق فرضیات بین المللی قبلی در تولید آنزیم پروتئاز قلیایی توسط اکتینومیست ها را به طور نسبی تایید نمود. با توجه به کاربردهای فراوان آنزیم پروتئاز قلیایی در صنایع مختلف و با توجه به جداشدن یک سویه بومی از اکتینومیست های مولد آنزیم مذکور می توان به تولید این آنزیم در کشور و فراهم شدن زمینه خودکفایی و جلوگیری از واردات روزافزون آن امیدوار بود.

صفحات 59-68زمینه و اهدافمطالعه آب های معدنی بطری با هدف تعیین آلودگی به باکتری های هتروتروفیک، که سلامت آن ها برای افراد سالخورده، نوزادان، مادران باردار، بیماران مبتلا به ضعف سیستم ایمنی ضروری است، صورت پذیرفت.مواد و روش کاربرندهای مختلف آب معدنی بطری به صورت تصادفی انتخاب و در مدت 24تا 48 ساعت پس از خریداری با استفاده از محیط های کشت اختصاصی و تست های بیوشیمیایی مورد ارزیابی قرار گرفتند. نمونه ها با فیلترهای 0/45 میکرون فیلتر شده و در محیط های کشت اختصاصی تلقیح، در دمای ̊C 37به مدت48-24ساعت انکوبه شدند.یافته هامطالعه مورفولوژی و تست های بیوشیمیایی، تعدادی از باکتری ها را در برخی از نمونه های آب های معدنی بطری مورد مطالعه نشان داد. باکتری های هتروتروفیک(کوکسی گرم مثبت، باسیل گرم مثبت اسپور دار و بدون اسپور، باسیل و کوکوباسیل های گرم منفی شامل سودوموناس و استنوتروفوموناس) در70% نمونه های آب دیده شدند. موردی از آلودگی مدفوعی یا اشریشیاکلی یافت نشد.نتیجه گیریآب های بطری استریل نبوده و به طور طبیعی یا طی عمل آوری حاوی تعداد جزئی باکتری می شوند. آزمایش ها میکروبی آب شرب پیچیده، زمان بر و گران می باشد. هرگاه وجود کلی فرم های تام در آب محرز شود، احتمال آلودگی محیطی وجود دارد. نظر به اینکه نقش باکتری های هتروتروفیک در ارتباط با سلامت و بیماری زایی مشخص نشده است، نیاز ضروری به تدوین یک معیار معین برای شمارش این باکتری ها در آب های معدنی بطری دارد. شرایط محیط کشت برای رشد، نقش مهمی در تعیین باکتری های هتروتروفیک آب های معدنی ایفا می کند.

صفحات 69-73زمینه و اهدافعفونت بیمارستانی به عفونتی که طی بستری در بیمارستان (حداقل 48 تا 72 ساعت بعد از پذیرش) اتفاق بیفتد اطلاق می شود. عفونت های بیمارستانی یکی از مشکلات مهم سلامت می باشد. این مطالعه به هدف تعیین شیوع عفونت بیمارستانی و عوامل مرتبط با آن در بیمارستان های بالای 100 تختخوابی استان خراسان جنوبی انجام شدمواد و روش کاردر این مطالعه مقطعی برای کلیه بیماران با تشخیص عفونت بیمارستانی در بیمارستان های بالای 100 تختخوابی استان خراسان جنوبی پرسشنامه پژوهشگر ساخته شامل مشخصات دموگرافیک، بخش بستری، مدت بستری، نوع پاتوژن و عوامل خطر در سال 1391 تکمیل شد. این پرسشنامه بر اساس پرسشنامه استاندارد نظام مراقبت عفونت های بیمارستانی مرکز کنترل بیماری های واگیر وزارت بهداشت تدوین شد. داده ها پس از جمع آوری توسط نرم افزار spss(16) مورد تجزیه و تحلیل قرار گرفت.
یافته ها و بحث: تعداد بیماران با تشخیص عفونت بیمارستانی 358 نفر بود. میزان بروز عفونت بیمارستانی در بیمارستان های استان 0/9% محاسبه شد. بیشترین میزان بروز در ICU بود (17/3%). شایع ترین عفونت بیمارستانی پنومونی (43%) و بعد از آن عفونت ادراری (15/1%) بود. در 33/5% موارد نتیجه کشت منفی بود و شایع ترین پاتوژن عامل عفونت گونه کلبسیلا (12/8%) و پسودموناس ائروژینوزا (9/8%) بود. شایع ترین عوامل خطر همراه با عفونت بیمارستانی داشتن کاتتر ادراری (70/4%)، ساکشن (66/8%) و لوله تراشه (54/2%) بود. 24% از بیماران فوت کردند. نتایج مطالعه میزان پایین تری از عفونت بیمارستانی را نشان داد که علت اصلی آن عدم تشخیص و گزارش موارد واقعی عفونت بیمارستانی می باشد. جهت کنترل و پیشگیری عفونت های بیمارستانی ارتقای نظام شناسایی و گزارش دهی موارد عفونت های بیمارستانی توصیه می شود.

It seems review systematic and Meta analysis of related documents gives precise estimate of this subject for correct politisize. So tha aim of this study the review systematic and meta analysis of prevalence and causes of nosocomial infection in iran.Materials And MethodsFor this study all articles published in Iranian journals and international journals، Final Report of Research Projects، related papers presented at congresses and thesis were reviewed with using standard and sensitive keywords. Then، all articles published between 1997-2010 years that had eligibility Inclusion criteria after quality control، using random model، intered to process of meta-analysis.ResultsThe finding show that the best estimate of total prevalence of nosocomial infection in Iran is 30. 43% and the most common infections of nosocomial infection are respiratory infection 39. 4%%، urinary infection 23. 88%، bacteremia 21. 98% and the most common factors of nosocomial infection are Pseudomonas aeroginosa 26. 78%، klebsiella 31. 42%، Staphylococcus 23. 6% and E. coli 30. 93%. The research also found a substantial heterogeneity that using meta regression method the main cause of produce of this heterogeneity، participants people، sample size، average age of the samples، time of study and gender were introduced.ConclusionsThe simple review of studied documents in this survey show that prevalence rate of different nosocomial infection in Iran is high relatively. Hence make appropriate and evidence-based educational and control programs to reduce nosocomial infections prevalence rate in Iran should be considered by policy makers.Keywords: Nosocomial Infection, Meta, Analysis, Iran

Pages 13-19Background And AimListeria monocytogenes cause listeriosis and fatal infections in humans. The aim of this study was typing and evaluation of the genetic relatedness of L. monocytogenes strains from food samples using MLVA technique.Materials And Methods317 food samples were collected from 2009 to 2013 in Tehran,Iran. After final diagnosis of L. monocytogenes DNA was extracted to perform of MLVA technique, and also PCR products were analyzed by Gene Tools software. The number of tandem repeats was determined by using special equation for each selected locus. Also typing of strains was done.Results24 samples of 317 food samples were positive for L. monocytogenes using standard laboratory techniques. A total 13 different types were determined by MLVA technique that type 2 and type 3 were the most abundant types by 6 and 4 strains, respectively.ConclusionsThe results of this study showed the presence of L. monocytogenes in dairy products and meat samples, therefore all people, especially pregnant women should observe health tips when using these products. The results of typing showed that L. monocytogenes strains from different sources can have the same origin. MLVA technique is easy with high accuracy and this method can be used in typing and evaluation of the genetic relatedness of L. monocytogenes for determination the source of contamination.Keywords: Listeria monocytogenes, Dairy, meat samples, MLVA

Pages 20-27Background And AimThe epidemiologic pattern of Helicobacter pylori infection is differed between developed countries and developing countries, and also it is depend on the total standard of living in each region. At the present study, the seroprevalence of H.pylori infection and effectiveness of underlying factors in prevalence of this infection among residents of Pishva city of Varamin was evaluated.Materials And MethodsThis descriptive study was conducted after completion the questionnaire. The peripheral blood of 314 people without any confirmed gastric problem were collected. Then, the titer of total IgG and IgG anti CagA of H. pylori was evaluated by ELISA method. Consequently, the correlation between serologic data and different factors were analyzed by SPSS statistical software.ResultsThe existence of total IgG was detected in 164 (55.2%) of 314 patients and was negative in 130 people (41.4%).Also, the IgG anti CagA were positive in 46 (29.1%), it was negative in 105 people (66.5%) and the rested were in borderline. There was statistical meaningful correlation between positive result of serology test of IgG anti CagA to some risk factors such as age, the number of the member of family, the educational status and occupation, the consumption of can, heart diseases, the rate of cholesterol, the history of gastrointestinal symptoms, heartburn and reflux (P <0.05).ConclusionsWith regard to high prevalence of H. pylori in this area (55.2%) and its presumptive effect in infected people, the necessary of hygiene education and precise control of infection is suggested.Keywords: Helicobacter pylori, ELISA, Serology, CagA

Pages 28-35Background And AimAccording to numerous reports of infections caused by spectrum beta lactamases (ESBLs) producing Acinetobacter baumannii in our country in recent years, this study was performed to define the antibiotic susceptibility patterns and detect the prevalence PER and VEB beta-lactamase genes among A.baumannii isolated from patients in Tehran by PCR.Materials And Methods100 A.baumannii clinical isolates collected from various hospitals in Tehran during a year, using special culture media and biochemical tests were identified. The antibiograms of the isolates against 11 antibiotics by disk diffusion method (Disk diffusion) according to Clinical and Laboratory Standards Institute (CLSI) guidelines was performed. Then minimum inhibitory concentrations (MIC) was determined for cefepime and ceftazidime and for the identification of ESBL-producing strains was used in combination disk method, and finally to assess the prevalence of PER and VEB beta-lactamase genes using specific primers PCR was performed.ResultsAntibiograms results showed that the greatest resistance to the antibiotics amikacin, cefepime, and less resistance to polymyxin B were obtained. Rates of multi-drug resistant strains of about 70% was achieved. Of the isolates studied, the MIC of ceftazidime in 84% and for cefepime in 91% were ≥ 128 μg/ml. The results of the combined-disk test demonstrated that 20% of samples were ESBL positive. The PCR results showed that 47% and 32% of our isolates had PER and VEB genes respectively.ConclusionsRegarding to existence of PER and VEB genes in this bacterium and possibility of transformation of these genes to the other bacteria, reconsideration in antibiotics consumption patterns and more attention to nosocomial infections control criteria are inevitable.Keywords: Acinetobacter baumannii, Polymerase Chain Reaction, VEB beta, lactamase, PER beta, lactamase

Pages 36-43Background And AimPseudomonas aeruginosa is a unique bacteria that in order to survive in different environments by complex adaptation process can make changes in his virulence genes expression and drug resistance. The aim of this research is the investigation of existence of a logical association between toxA gene and antibiotic resistance in strains possess the gene.Materials And MethodsAntibiogram test by disk diffusion method (Kirby Bauer) was performed according to CLSI protocols. In this study, the existence of toxA gene with the help of polymerase chain reaction (PCR) in 102 clinical isolates from blood samples, wound, urine and trachea was examined. Chi-square test was used to investigate the relationship between exotoxin A and antibiotic resistance.ResultsThe 81 strains (79.4%) had toxA gene. Frequency of toxA genes in isolated strains from different infections were wound (91.4%), blood (85.7%), trachea (72.7%), and urine (42.1%). Multiple resistance index in strains possess the toxA gene was calculated 75%. Chi 2 test to determine the relationship between drug resistance and gene toxA was significant (P<0.05).ConclusionsThe significant chi-square test and an increase in multi-resistant strains possessing the toxA gene, can represent a considerable genetic switch between exotoxin A activity and resistance to antibiotics in the blood, urine, tracheal, wound infections Respectively, which lead to turn genes on of drug resistance regulating in bacteria. Pages 44-49Background And AimThe main principle of ensuring the health and pharmaceutical medicine.In accordance with its combination of drugs can be the perfect environment for growth of microorganisms.The aim this research was to identify microorganisms in raw materials, water and environment, and other antimicrobial disinfectant effect of in eliminating them.Materials And MethodsIn this descriptive study, 5 step 52 series production of gastric juice manufacturing factory sampling was performed before and after sterilization.Plate count method was used to measure microorganisms. If there is contamination of the culture media and biochemical tests for the detection of specific microorganisms such as Staphylococcus aureus, E.coli, Pseudomonas aeruginosa, Aspergillus niger and Salmonella spp. were used.ResultsThe result showed that Micrococcus in 3 cases, 2 cases of Serratia spp., Bacillus subtilis, in two instances, in one instance, Salmonella spp., S. aureus and A.niger in one sample were present in one sample.ConclusionsThe experiments showed that the infected samples 19.2% are. 13.5%, which includes microorganisms, non-pathogenic, with over 100 CFU/ml are 5/7% of the sample remained, including microorganisms, virulence such as S. aureus, 1.1%, Salmonella 1.1% the A.niger and 1.1% respectively.Keywords: Micrococcus, Cultured, Colony Counting, Pharmaceuticals

Pages 50-58Background And AimAlkaline proteases are used in pharmaceutical, film and photography, silk production and food, leather and detergent industries. Actinomycetes are gram positive bacteria that produce different enzymes such as proteases. The aims of this research were isolation of native alkaline protease-producing Actinomycete spp. from different soil samples as well as optimizing the conditions for enzyme production.Materials And MethodsThe different soil samples were collected from different locations of the provinces of Khouzestan, Chahar Mahalo Bakhtiari and Isfahan, Iran. After determining of the best alkaline protease producing species using Lowry method, the optimization of alkaline protease was performed.ResultsThe alkaline protease producing Actinomycete spp. was isolated from soil. The most enzyme activity was measured in S.diastaticus. The best concentration of sucrose as the carbon source for the highest production of alkaline protease was 10 g/l. The optimum pH and temperature for the alkaline protease production by S. diastaticus were 10 and 30°C respectively. The maximum activity of alkaline protease was measured at 200 rpm as the best aeration speed.ConclusionsThis is the first report of alkaline protease production by Streptomyces diastaticus in Iran. The accomplished examinations in this research confirmed the previous theories of alkaline protease production by Actinomycetes relatively. Pages 59-68Background And AimThis project focuses on the level of heterotrophic baceria in bottled mineral water which could be a health concern for the elderly, infants, pregnant women and immuno-compromised patients.Materials And MethodsDifferent brands of bottled water samples were selected randomly and evaluated for their bacteriological quality, using different specific culture media and biochemical tests. Water samples were analyzed within 24 hours of their purchase/collection. Samples were filtered with 0.45 micron and filters were plated in different media. Then media were incubated at 37˚C for 24-48 hours.ResultsMorphological study and biochemical tests revealed a number of bacteria in different brands of bottled water. Heterotrophic bacteria(Gram positive cocci, Spore forming gram positive bacilli, non spore forming gram positive bacilli, gram negative bacilli, and gram negative coccobacilli; Pseudomonas and Stenotrophomonas) counted in 70% of bottled water samples. There were no cases of fecal contamination or the presence of E.coli.ConclusionsBottled water is not sterile and contains trace amounts of bacteria naturally present or introduced during processing. Testing drinking water for all possible pathogens is complex, time-consuming, and expensive. If only total coliform bacteria are detected in drinking water, the source is probably environmental. Since the significance of non-pathogenic heterotrophic bacteria in relation to health and diseases is not understood, there is an urgent need to establish a maximum limit for the heterotrophic count in the bottled mineral water. Pages 69-73Background And AimNosocomial infections are defined as infections occurring during a patient's stay at hospital (48-72h after admission).Nosocomial infections are one of the important problems of health. This study aimed was determine the prevalence of nosocomial infections, and related factors in hospitals with more than 100 beds in south Khorasan Province.Materials And MethodsIn this crass-sectional study, an investigator-administered questionnaire was completed for each patient with nosocomial infections diagnosis in hospitals with more than 100 beds in South Khorasan. This questionnaire conation demographic characteristic of patients, department, duration of admission, kind of pathogen and risk factors that was designed according to standard questionnaire of Iranian Nasocomial infections surveillance system (INIS) of Center for communicable Disease Control, Ministry of Health. Data were analyzed with SPSS 16 software.Results And DiscussionNumber of patients with nosocomial infection was 358. The incidence of nosocomial infection was 0.9%. ICU had the highest incidence rate (17.3%). The most common nosocomial infection was pneumonia (43%), and urinary tract infection (UTI) (15.1%). In 33.5% culture result were negative. In other cases, culture results showed klebsiella spp. (12.8%) and Pseudomonas aeruginosa (9.8%) were the most prevalent bacteria. Most factors associated with nosocomial infection in patients were urinary catheters (70.4%), suction (66.8%) and tracheal tube (54.2%). 24% of patients expired. The results showed lower ratio of nosocomial infection, that the main reason is failure to detection and reporting of actual cases of nosocomial infection.
