    Extracranial meningioma remains a rare entity with few cases reported in literature. This report illustrates a unique case of a 28-year-old woman presenting with a deformity at the root of the nose which was investigated and treat- ed as a primary extracranial meningioma. We discuss the presentation of this rare lesion and the challenges in its diagnosis and management. Literature has been reviewed to provide guidance for clinicians in diagnostic methods. A 28-year-old caucasian woman presented to the ear, nose and throat (ENT) clinic with a painless swelling at the root of the nose. Fine needle aspirations identified cells of an uncertain nature therefore an open biopsy was un- dertaken. This revealed features in keeping with a benign meningioma. Contrast MRI scan revealed an enhancing lesion involving the crista galli and cranial aspect of the nasal bone. No parenchymal involvement was found and the tumour was categorised as a primary extracranial meningioma. Multidisciplinary involvement from maxillofa- cial, neurosurgeons and ENT surgeons was required to arrange suitable surgical excision due to local extension of the tumour. The patient was warned of serious side effects including the risk of complete anosmia.

    Infective endocarditis is a rare but serious complication following transcatheter closure of atrial septal defect. A 76-year-old woman with a large secundum atrial septal defect, that had been repaired percutaneously, 5 years pre- viously, with an Amplatzer atrial septal defect occlude, referred to our center with fever and Staphylococcus aureus bacteremia. Transesophageal echocardiography revealed a 1.6×1.2 cm mobile mass, like a endocarditis vegetation on the left atrial side of the Amplatzer device. The endocarditis was started to treat. The Amplatzer device was removed with peripheral fragile endocarditis tissue and the defect is closed with a pericardial patch. In this paper, we present a description of late infective endocarditis in an adult with an Amplatzer device.

    Malakoplakia is an uncommon inflammatory condition usually affecting the genitourinary tract, which has been associated with infections, tumours and immunocompromised states. We report a case of malakoplakia in the colon of a 5-year-old girl with a history of bloody diarrhea. Clinically and macroscopically malakoplakia can simulate tumours or abscesses and can cause diagnostic difficulties.

    An elongated uvula can flop down and touch various structures in upper airway in the oropharyngeal junction and cause mechanical irritation.1 Here we present a case report of a patient with long uvula with respiratory tract obstruction leading to chronic cough and obstructive apnoea syndrome (OSA), in which uvulectomy was carried out for symptomatic relief.

    Infantile myofibromatosis is a rare, benign proliferative myofibroblastic tumor which occurs mostly in infants and young children. It presents as either solitary or multiple nodules arising from soft tissues, bones, or visceral organs. Accurate diagnosis and differentiation from more aggressive tumors are important because of the variations in the benign clinical course, conservative treatment, and possible spontaneous regression of infantile myofibromatosis. We present a case of multiple infantile myofibromatosis of soft tissues without involvment of visceral organs di- agnosed at the neonatal age.

    Secondary alveolar bone grafting is a well established technique in the management of patients with cleft lip and alveolus. The goals of bone grafting determine the selection of grafting material such as cortical or cancellous, membranous or endochondral. In addition, vascularity, host-bed, overall physiologic status of the patient, pro- pensity of infection and surgical expertise needs to be considered. Thus success depends on panoply of variables including the physiologic and mechanical properties of the graft material and the biology of recipient site. Here we report a case of secondary alveolar bone grafting using autogenous mandibular bone graft from symphysis.

    Giant ureteric calculi are rarely seen these days because of early diagnosis of ureteric calculi and subsequent ef- fective treatment by non-invasive and minimally invasive procedures. These giant calculi can cause a significant and irreversible compromise of renal function if not treated in time. Here we report three such cases where ureteric calculi were measured to be more than 5centimeters fulfilling the criteria to be named as giant ureteric calculi and managed successfully using the open method of ureterolithotomy.

    Blunt trauma abdomen leads to gut perforations commonly on the anti-mesenteric border due to shearing forces. We report a case of a 23-year male who presented to surgical history with a history of blunt trauma to the abdo- men. On further evaluation, an isolated perforation of the mesenteric border of jejunum was documented. The perforation was managed by a wedge resection and the patient recuperated well. This case illustrates that traumatic perforations of the gut can also occur on the mesenteric border and may remain clinically silent. We discuss this case in the context of relevant literature.

    Solar retinopathy is a photochemical retinal injury caused by direct or indirect viewing of the sun and charac- terized by visual distortion and a small yellow or red foveal lesion. Retinal damage can be identified by optical coherence tomography. This paper reports late optical coherence tomography (OCT) findings in a 10-year-old boy with solar retinopathy after watching a solar eclipse. OCT demonstrates abnormalities in the outer foveal retina in late solar eclipse retinopathy with normal Bruch membrane choriocapillaris complex.

    Acute subdural hematoma is a neurosurgical emergency. Emergency and a serious trauma with high mortality rates. Idiopathic brain calcification, also called bilateral striopallidodentate calcinosis, is rare disease characterized by symmetric cerebral calcifications. Calcifications are demonstrated on computed tomography (CT) usually taken for other reasons. CT findings in these cases may be confused with subarachnoid hemorrhaging. A 79-year-old female patient was admitted to the emergency room presenting with seizure and confusion. She had a history of atrial fibrillation for which she was using anticoagulants. On physical and neurologic examinations, she was con- fused with limited orientation and cooperation. Her Glasgow Coma Scale score was 10. Her serum hemoglobin was 9.2 g/dl and her international normalized ratio was 5.02. Her cranial CT revealed bilateral striopallidodentate calcinosis accompanied by an acute subdural hematoma on a bilateral chronic basis and this case was evaluated in light of literature.

    Allopurinol, a xanthine oxidase inhibitor, is an effective urate-lowering drug. Unlike majority of patients tolerate allopurinol well, in 2% of patients receiving allopurinol, hypersensitivity reactions have been reported. In this case a 71-year-old woman with tophaceous gout since 10 years ago, presented to our rheumatology clinic. Because of occurring hypersensitivity reactions, allopurinol was discontinued and disease worsened despite administration of colchicine. She was admitted several times due to flare up of gout. Other forms of allopurinol are not available in Iran, so we decided to conduct an allopurinol desensitization protocol with tablets form. Side effects have not been reported yet.

    Colorectal cancer may uncommonly metastasizes to central nervous system, including brain and/or meninges. This often occurs late in the course of the disease and is accompanied with other extracranial metastases, leading to poor prognosis. Rarely, it is the first manifestation of the malignancy. We herein describe a 50-year-old man with sigmoid adenocarcinoma and secondary meningeal involvement, which to the best of our knowledge, is a rare metastasis to the central nervous system. Since a wide variety of signs and symptoms are associated with menin- geal carcinomatosis, any neurologic complaint or abnormal physical finding in a patient with a known malignancy should be considered and evaluated seriously. Although we cannot do much to save the patient, in this stage.

    Stress fracture is a known entity with common occurrence in lower limbs especially in military recruits and ath- letes. Stress fracture of the upper limb is a rare occurrence, but is much rarer in clavicle. Only 16 to 20 such case re- ports have been mentioned in literature. Stress fracture or non-traumatic idiopathic fracture of the clavicle in a non athletic patient without any pre-disposing factors is very rare and till date only 2 such cases have been reported. Such patient requires a detailed work-up and high index of suspicion on the part of treating physician. Our case re- port is the 3rd such case report and highlights several points with regards to its diagnosis, treatment and prognosis.
