بیماری های قلبی مادرزادی، از شایع ترین ناهنجاری های مادرزادی می باشد. هدف این مطالعه، بررسی اجرای فرایند راهنمای بالینی جهت شناسایی نقاط قوت و ضعف آن می باشد.
روش هادر این مطالعه ی توصیفی- مقطعی، نوزادان سالم و ترم متولد بهمن ماه 1401 تا تیرماه 1402 در بیمارستان شهید بهشتی اصفهان مورد بررسی قرار گرفتند. غربالگری پالس اکسیمتری برای کلیه ی نوزادان سالم و بدون علامت در 24 ساعت بدو تولد و قبل از ترخیص از بیمارستان انجام گرفت. ابزار جمع آوری داده ها در این مطالعه چک لیست بود. SpO2 89 درصد یا کمتر در دست راست و یا پا به عنوان غربالگری مثبت در نظر گرفته شد. در SpO2 بین 90 تا 94 درصد غربالگری یک ساعت بعد تکرار و در صورت نتایج یکسان، نوزاد جهت بررسی بیشتر به متخصص ارجاع داده می شد. نوزادان سالم با نتایج مثبت 2 ماه بعد مجدد پیگیری شدند.
یافته هااز 500 نوزاد مورد بررسی، 472 نفر (94/4 درصد) SpO2 بالای 95 درصد داشتند. از بین 21 نوزاد با نتیجه ی غربالگری مثبت، یک نفر بیماری قلبی (نقص کامل دیواره دهلیزی- بطنی همراه با فشار ریوی) داشت. برای 2 نوزاد سالم با غربالگری مثبت بعد از 2 ماه پیگیری، سوراخ بیضی باز (PFO) تشخیص داده شد. بررسی اجرای فرایند غربالگری نوزادان بر اساس چک لیست توصیه های راهنمای بالینی در مدت زمان 6 ماه از 58/3 درصد به 91/6 درصد افزایش یافت.
نتیجه گیریاجرای توصیه های راهنمای بالینی در این مطالعه مطلوب و قابل قبول ارزیابی شد و پیاده سازی آن در سایر بیمارستان ها توصیه می شود.

ابدومینوپلاستی به علت دو مانور پلیکاسیون فاشیا و رزکسیون فلاپ با احتمال افزایش فشار شکمی و عوارض ناشی از آن همراه است. این مطالعه با هدف ارزیابی تغییرات فشار شکمی طی ابدومینوپلاستی و متغیرها و عوارض احتمالی آن انجام شد.
روش هااین پژوهش به صورت مطالعه ی همگروهی در 20 بیمار خانم که در سال 1402-1401 در بیمارستان های قائم و طالقانی مشهد تحت لیپوابدومینوپلاستی قرار گرفتند انجام شد. اطلاعات بیماران در دو بخش دموگرافیکی و بالینی جمع آوری گردید. فشار شکمی بعد بیهوشی، پلیکاسیون، انتهای عمل و روز بعد عمل و عوارض یک ماهه ثبت شد.
یافته هاتفاوت آماری معنی داری در میانگین تغییرات فشار شکمی بعد بیهوشی، پلیکاسیون، انتهای عمل و روز بعد عمل وجود داشت. افزایش فشار شکمی بعد بیهوشی و بعد پلیکاسیون تنها با افزایش وزن نمونه جراحی ارتباط معنی دار داشت، اما با عرض پلیکاسیون مرتبط نبود.
نتیجه گیریعرض پلیکاسیون با افزایش فشار داخل شکمی در مطالعه ی ما ارتباطی نداشت. با وجود افزایش معنی دار فشار شکمی، عوارض سیستمیک قابل توجهی دیده نشد. تکنیک صحیح علاوه بر مانیتورینگ بیمار، ایمنی و رضایت بیمار را بدنبال خواهد داشت.

با توجه به بار بالای بیماری کووید-19 و فشاری که به سیستم بهداشت و درمان بخصوص در بخش مراقبت های ویژه وارد شد، بررسی متغیرهای پیش بینی کننده ی پیامد این بیماری می تواند در شناخت بهتر بیماران در معرض خطر و تخصیص مفیدتر منابع محدود، کمک کننده باشد.
روش هامبتلایان به کووید-19 بستری شده در بخش مراقبت ویژه مرکز پزشکی الزهرا(س) اصفهان در پاییز سال 1399، در یک مطالعه ی مشاهده ای بررسی شدند. یافته های دموگرافیک، علائم بالینی و آزمایشگاهی جهت یافتن عوامل موثر بر مرگ و میر بین دو گروه افراد فوت شده و بهبود یافته مقایسه شد.
یافته ها1144 بیمار مبتلا به کووید-19 بستری در بخش مراقبت ویژه در مطالعه بررسی شدند که 37/8 درصد از آنان فوت کردند. متغیرهای گلبول سفید (0/001 > P)، نوتروفیل (0/001 > P)، نسبت نوتروفیل به لنفوسیت (0/004 = P)، نیتروژن اوره خون (0/001 > P)، کراتینین (0/037 = P)، پروکلسی تونین (0/001 > P)، گلوکز (0/001 > P)، D-dimer (0/001 > P)، طول مدت بستری (0/001 > P) و اکسیژن درمانی (0/001 > P) ارتباط مستقیم با مرگ و میر داشتند و بیشترین نسبت شانس مربوط به نوتروفیل (3/59 = OR) بود. متغیرهای هموگلوبین (0/001 > P)، لنفوسیت (0/028 = P)، پلاکت (0/007 = P)، ائوزینوفیل (0/001 > P)، سدیم (0/001 > P)، پتاسیم (0/001 > P)، منیزیم (0/001 > P)، زمان پروترومبین (0/001 > P)، تعداد تنفس (0/022 > P) و علائم تنفسی (0/032 > P) ارتباط معکوس با فوت داشتند و کمترین نسبت شانس مربوط به منیزیم (0/01 = OR) بود.
نتیجه گیریمتغیرهای بالینی و آزمایشگاهی می توانند پیش بینی کننده ی پیامد بیماری در بیماران مبتلا به کووید-19 باشند و نقش به سزایی در مدیریت بهتر بیماران داشته باشند.

Congenital heart disease is one of the most common birth defects. This study aims to examine the process of clinical guideline implementation to identify strengths and weaknesses.
MethodsIn this cross-sectional study, healthy term and term neonates born from January 2023 to July 2023 were evaluated at Shahid Beheshti Hospital in Isfahan. The data collection tool was a checklist. The pulse oximetry (POX) was performed for healthy newborns within the first 24 hours after birth and before discharge from the hospital. Oxygen saturation (SpO2) < 89% in the baby's hand and foot was considered positive screening. If the SpO2 level was between 90-94%, the screening was repeated one hour later and if in the case of the same results, the newborn was referred to a specialist for further assessment. Healthy neonates with positive test results were followed up two months after primary POX screening.
FindingsOut of 500 neonates, 472 (94.4%) had a SpO2 level ³ 95%. Among the 21 cases who had a positive screening, one case had cardiac diseases (i.e., complete atrioventricular septal defect/pulmonary hypertension). Two healthy neonates with a positive screening were diagnosed with PFO after two months of follow-up. The assessment of the implementation process of newborn screening based on the checklist recommendations of clinical guidelines increased from 58.3 % to 91.6 % within six months.
ConclusionThe implementation of the critical congenital heart disease clinical guideline was found to be favorable and acceptable in this study and recommended for other hospitals to implement.

Abdominoplasty due to the two maneuvers of fascia plication and flap resection is associated with the possibility of increased intra-abdominal pressure and its related complications. This study was conducted to evaluate the changes in intra-abdominal pressure during abdominoplasty, as well as its variables and possible complications.
MethodsThis research was conducted as a cohort study on 20 female patients who underwent lipoabdominoplasty in 2022-2023 at Taleghani and Ghaem hospitals, Mashhad City, Iran. Patient information was collected in both demographic and clinical sections. Intra-abdominal pressure was recorded after anesthesia, plication, at the end of the operation, and the day after the operation, along with monitoring complications for one month.
FindingsThere was a statistically significant difference in the mean changes in intra-abdominal pressure after anesthesia, plication, at the end of the operation, and the day after the operation. The increase in intra-abdominal pressure after anesthesia and after plication was only significantly related to the increase in the weight of the surgical specimen, but it wasn’t related to plication width.
ConclusionPlication width was not related to increased intra-abdominal pressure in our study. Despite the significant increase in intra-abdominal pressure, no significant systemic complications were observed. In addition to monitoring the patient, the correct technique will ensure the safety and satisfaction of the patient.

Considering the high burden of the COVID-19 disease and the pressure that was put on the health care system, especially in the intensive care unit (ICU), the examination of variables predicting the outcome of this disease can help in better understanding of patients at risk and more useful allocation of limited resources.
MethodsPatients with COVID-19 hospitalized in the ICU of Al-Zahra Medical Center were investigated in an observational (descriptive-analytical) study. Demographic, clinical, and laboratory findings were compared to find factors affecting mortality between two groups of deceased and survived patients. A comparison of demographic, clinical, and laboratory findings was conducted to identify factors influencing mortality rates in deceased and survived patients.
Findings1144 patients with COVID-19 were examined in the study, of which 674 were men (58.9%) and 470 were women (41.1%). The patients were split into two groups: 432 patients (37.8%) died, and 712 patients (62.2%) survived. The variables of white blood cells (P < 0.001), neutrophil (P < 0.001), neutrophil to lymphocyte ratio (P = 0.004), blood urea nitrogen (P < 0.001), creatinine (P = 0.037), procalcitonin (P < 0.001), D-dimer (P < 0.001), length of hospitalization (P < 0.001) and oxygen therapy (P < 0.001) were directly related with mortality and the highest odd ratio was related to neutrophil count (OR = 3.59). Variables of lymphocyte (P = 0.028), eosinophil (P < 0.001), hemoglobin (P < 0.001), platelet (P = 0.007), sodium (P < 0.001), potassium (P < 0.001), magnesium (P < 0.001), prothrombin time (P < 0.001), respiratory rate group (P < 0.022) and respiratory symptom (P < 0.032) were inversely related to mortality and the lowest odd ratio was related to magnesium (OR = 0.01).
ConclusionExamining clinical and laboratory characteristics helps us in better evaluation of patients, recognition of risk factors involved in the progression of the disease, and better management of patients.
