صفحه 7زمینهارتباط بین تک نوکلئوتیدهای چند شکلی(SNPها) با برخی از بیماری ها دغدغه بسیاری از محققین است.هدفمطالعه به منظورتعیین ارتباط بین SNPهای موجود در ژن DNMT1] دی ان ای (سیتوزین-5) متیل1 [از کروموزوم 19 انسان با سرطان روده بزرگ انجام شد.مواد و روش هادر این مطالعه مورد-شاهدی، 100 بیمار مبتلا به سرطان روده بزرگ، مراجعه کننده به مرکز تحقیقات بیماری های گوارش و کبد دانشگاه علوم پزشکی شهید بهشتی در سال 1387 به عنوان گروه مورد و 100 نفر از سایر بیماران مراجعه کننده به این مرکز، به عنوان گروه شاهد انتخاب شدند. آزمایش ژنتیک، جهت تعیین ژنوتیپ 6 مورد SNP از ژن DNMT1 از کروموزوم 19 انسان برای این بیماران در آزمایشگاه این مرکز انجام شد. داده ها با آزمونهای آماری مجذورکای و رگرسیون لجستیک تحلیل شدند.یافته هاتمام SNPهای مورد بررسی با سرطان روده بزرگ، رابطه معنی داری نشان دادند. در تمام آن ها شانس ابتلا به سرطان روده بزرگ در افراد با ژنوتیپ 1 یا 2، چندین برابر افراد با ژنوتیپ صفر بود.نتیجه گیریمی توان با شناسایی SNPهای افراد، میزان خطر ابتلا به بیماری سرطان روده بزرگ را پیش بینی نمود و جهت پیشگیری از ابتلای آنان، اقدام لازم را انجام داد.

سرب از طریق آب، غذا و هوا وارد بدن انسان می شود و در استخوانها به جای کلسیم تجمع می یابد. این عنصر می تواند به سیستم عصبی، کلیه ها و سیستم تناسلی به خصوص در کودکان آسیب برساند و در ساخت هموگلوبین نیز اثر گذارد.
هدفمطالعه به منظور بهینه سازی شرایط جذب سرب توسط ساکارومایسس سرویزیه در شرایط آزمایشگاهی انجام شد.
مواد و روش هااین مطالعه مقطعی در سالهای 1387 و 1388 در مرکز آزمایشگاهی دانشگاه آزاد واحد علوم و تحقیقات ایران انجام شد. در این تحقیق از ساکارومایسس سرویزیه تجاری کارخانه خمیرمایه لرستان استفاده شد. عوامل موثر در جذب زیستی مانند غلظت اولیه فلز، زمان تماس و PH بررسی شدند. برای تثبیت سلولهای مخمر از روش به دام انداختن استفاده شد.
یافته هابیشترین میزان جذب مخمر (80 میلی گرم بر گرم وزن خشک) در زمان تماس 2 ساعت، غلظت فلز سرب 500 میلیگرم در لیتر و درPH 5/4 مشاهده شد. میزان جذب فلز توسط سویه مخمر پیش تیمار شده با روش اتوکلاو، 3/35 میلیگرم برگرم وزن خشک گزارش شد. میزان جذب توسط سلولهای تثبیت شده در مقایسه با سلولهای آزاد حدود دو برابر افزایش یافت.
نتیجه گیریبا توجه به یافته ها، ساکارومایسس سرویزیه پیش تیمار نشده و تثبیت شده یک جذب کننده خوب برای حذف یون های سرب از محیط است.

مقایسه رابطه پروتئین در نمونه ادرار 2 ساعته با نمونه ادرار 24 ساعته در بیماران مشکوک به پره اکلامپسیصفحه 22زمینه
استاندارد طلایی تشخیص اختلال های پرفشاری خون در حاملگی (پره اکلامپسی) جمع آوری ادرار 24 ساعته است.
هدفمطالعه به منظور مقایسه رابطه پروتئین در نمونه ادرار 2 ساعته با نمونه ادرار 24 ساعته در بیماران مشکوک به پره اکلامپسی انجام شد.
مواد و روش هااین مطالعه ارزیابی آزمون های تشخیصی یک گروهی بر روی 60 بیمار مشکوک به پره اکلامپسی بستری در مرکز آموزشی- درمانی بیمارستان کوثر قزوین طی پاییز و زمستان 1387 انجام شد. تمام بیماران سن حاملگی حداقل 20 هفته، فشار خون حداقل 90/140 و پروتئین حداقل +1 در نمونه تصادفی ادرار داشتند. نمونه های ادراری بیماران یکی در 2 ساعت اول و دیگری در 22 ساعت بعدی جمع آوری شد. پروتئین ادرار نمونه های 2 و 24 ساعته محاسبه شد. نقطه برش میزان پروتئین ادرار 2 ساعته برای پروتئینوری خفیف با استفاده از نمودار ROC به دست آمد. داده ها با آزمون آماری ضریب همبستگی پیرسون تحلیل شدند.
یافته هااز60 بیمار مورد مطالعه، 42 نفر%70 پروتئینوری کم تر از 3/0 گرم و 18 نفر %30 پروتئینوری بیش تر از 3/0گرم در 24 ساعت داشتند. هیچ کدام از بیماران پروتئینوری شدید (بیش تر از 2 گرم در 24 ساعت) نداشتند. ارتباط معنی داری بین پروتئین ادرار 2 ساعته و 24 ساعته وجود داشت (778/0= r و 000/0= P). نقطه برش برای پروتئینوری خفیف 035/0 گرم در دو ساعت با حساسیت %3/63، ویژگی %5/89، ارزش اخباری مثبت %8/77 و ارزش اخباری منفی %95/80 تعیین شد.
نتیجه گیریدر زنان حامله مبتلا به پره اکلامپسی، مقدار پروتئین ادرار 2 ساعته از ارتباط مثبتی با میزان پروتئین ادرار 24 ساعته برخوردار است و به نظر می رسد می تواند جای گزین مناسبی باشد که ساده تر، سریع تر و ارزان تر است.

صفحه 28زمینهشناسایی عوامل پیشگیری کننده از ابتلا به پره اکلامپسی در زنان باردار پرخطر از اهمیت ویژه ای برخوردار است. مکمل امگا3 ممکن است به کاهش خطر ابتلا به پره اکلامپسی کمک کند.هدفمطالعه به منظور تعیین اثر مکمل امگا 3 بر پره اکلامپسی در زنان باردار پرخطر انجام شد.مواد و روش هااین کارآزمایی بالینی یک سوکور در سال 1387 در قزوین اجرا شد. تعداد 100 زن باردار پرخطر برای پره اکلامپسی در هفته های 14 تا 18 بارداری وارد مطالعه شدند و به طور تصادفی درگروه امگا 3 و یا در گروه دارونما قرار گرفتند. (هر گروه 50 نفر) زنان تا پایان بارداری روزانه 1000میلیگرم امگا 3 یا دارونما استفاده کردند. سپس میزان ابتلا به پره اکلامپسی و نتیجه بارداری دردوگروه بررسی شد. داده ها با آزمون های آماری تی، کای دو، من ویتنی و فیشر تحلیل شدند.یافته هااختلاف آماری معنی داری بین گروه امگا 3 با گروه دارونما در میزان ابتلا (015/0=p) و شدت پره اکلامپسی(01/0=p) به دست آمد. ارتباط معنی داری بین مصرف امگا 3 با بهبود نتیجه بارداری شامل نمره آپگار و وزن زمان تولد نوزاد به دست آمد (002/0=p و000/0=p).نتیجه گیریبه نظر می رسد مصرف مکمل امگا 3 در طول بارداری میزان خطر ابتلا به پره اکلامپسی را در زنان باردار پرخطر کاهش می دهد. لذا تدوین و ارایه برنامه های آموزشی تغذیه برای زنان باردارضروری است.

صفحه 34زمینهکاهش درد زایمان توسط ماماهای آموزش دیده تمایل مادران باردار را برای انجام زایمان طبیعی افزایش خواهد داد.هدفمطالعه به منظور تعیین اثر آموزش بر آگاهی، نگرش و عملکرد ماماها در مورد روش های غیردارویی کاهش درد زایمان انجام شد.مواد و روش هااین مطالعه مداخله ای و نیمه تجربی در سال 1385 در گیلان انجام شد. تعداد 59 نفر از ماماهای بخشهای زایمان بیمارستانهای غیرآموزشی با نمونه گیری سهمیه ای انتخاب شدند. ابزار گردآوری داده ها پرسش نامه بود که اعتبار علمی آن با روش اعتبار محتوا و پایایی آن با روش آزمون مجدد سنجیده شد. داده ها در دو مرحله قبل و بعد از آموزش جمع آوری شدند. روش های مختلف کاهش درد با تاکید بر به کارگیری روش های غیردارویی نظیر ماساژ ملایم، برقراری تماس چشمی با مادر، تشویق مادر به ذکر دعا و روش های حمایتی، فیزیولوژیک و جای گزینی (استفاده از کمپرس گرم، تغییر وضعیت و استفاده از روغنهای معطر) درکارگاه یک روزه آموزش داده شد. داده ها با آزمونهای آماری تی زوج و ویلکاکسون تحلیل شدند.یافته هاآگاهی، نگرش و به کارگیری روش های کاهش درد زایمان توسط ماماها بعد از آموزش به طور معنی داری افزایش یافت (05/0P<).نتیجه گیریآموزش کارگاهی درارتقای میزان آگاهی، نگرش و به کارگیری روش های کاهش درد زایمان توسط ماماها، به ویژه روش های دارای عوارض کمتر، اثربخشی بیشتر وکاربرد ساده تر مؤثر بود.

تاثیر برنامه آرام سازی پیشرونده عضلانی بر خودکارآمدی و کیفیت زندگی مراقبین بیماران مولتیپل اسکلروزیسصفحه 41زمینهمولتیپل اسکلروزیس (MS) یک بیماری مزمن سیستم عصبی مرکزی است که بر تمام جنبه های اقتصادی، مالی، اجتماعی و عاطفی فرد، خانواده و جامعه اثر می گذارد. انجام مراقبت مداوم، تنش مزمنی را در مراقبین بیماران به وجود می آورد؛ به طوری که خودکارآمدی و کیفیت زندگی آنان را به مخاطره می اندازد.هدفمطالعه به منظور تعیین تاثیر برنامه آرام سازی پیشرونده عضلانی بر کیفیت زندگی و خودکارآمدی مراقبین خانوادگی بیماران MS انجام شد.مواد و روش هااین کارآزمایی بالینی با حضور 70 نفر از مراقبین بیماران MS در قالب دو گروه همگن شاهد و آزمون در سال 1388 در بیمارستان کاشانی شهرکرد انجام شد. روش آرام سازی پیشرونده عضلانی در طول سه ماه (90 جلسه) برای گروه آزمون به کار رفت و با استفاده از چک لیستهای خودگزارش دهی، میزان به کارگیری آن پی گیری شد. پس از 3 ماه، پرسشنامه های سنجش خودکارآمدی و کیفیت زندگی مجددا در اختیار گروه آزمون و شاهد قرار گرفت. داده ها با آزمونهای آماری تی مستقل، تی زوج، کای دو و من ویتنی تحلیل شدند.یافته هاافزایش معنی داری در میزان خودکارامدی وکیفیت زندگی مراقبین خانوادگی بیماران گروه آزمون 3 ماه بعد از مداخله مشاهده شد (001/0P=)، در حالی که این تفاوت در گروه شاهد معنی دار نبود.نتیجه گیریبا توجه به یافته ها، مداخله های مانند برنامه آرام سازی پیشرونده عضلانی می تواند مراقبین بیماران به ویژه بیماران MS را یاری دهد و برخودکارآمدی وکیفیت مراقبت و کیفیت زندگی آنان بیفزاید.

صفحه 48زمینهمیزان ابتلا به اضطراب و افسردگی در دو دهه اخیر گسترش یافته است. محققان در جستجوی راه های غیرتهاجمی و دارویی جهت درمان این اختلالها هستندکه از آن میان هاتایوگا به عنوان پزشکی مکمل مورد توجه قرار گرفته است.هدفمطالعه به منظور مقایسه اثر تمرین های هاتایوگا و آمادگی جسمانی بر اضطراب و افسردگی زنان انجام شد.مواد و روش هااین پژوهش نیمه تجربی در سال 1388 بر روی زنان واجد شرایط مراجعه کننده به باشگاه های برگزار کننده یوگا و آمادگی جسمانی شهر شیراز (42 نفر) انجام شد. زنانی که به علت اضطراب و افسردگی به پزشک مراجعه کرده بودند و دارو استفاده نمی کردند، در صورت تمایل وارد مطالعه شدند. این افراد در دو گروه مساوی تحت 24 جلسه تمرین هاتایوگا و آمادگی جسمانی قرار گرفتند. تمرینها به صورت سه جلسه در هفته و هر جلسه90 دقیقه و به مدت دو ماه انجام شدند. به منظورسنجش اضطراب و افسردگی از پرسش نامه چند عاملی سنجش افسردگی، اضطراب و فشار روانی استفاده شد. داده ها با آزمونهای آماری کلموگروف اسمیرنوف، t زوج، یومن ویتنی و ویلکاکسون تحلیل شدند.یافته هامیانگین سن زنان شرکت کننده در پژوهش3/4± 4/30 سال بود. اختلاف میزان اضطراب و افسردگی قبل و بعد از تمرینها در گروه هاتایوگا معنی دار بود (05/0P<). اما این اختلاف در گروه آمادگی جسمانی معنی دار نبود؛ هر چند کاهش نشان می داد. همچنین نتایج پس آزمون اضطراب و افسردگی در گروه هاتایوگا به طور معنی داری بهتر از گروه آمادگی جسمانی بود (001/0P=).نتیجه گیریبا توجه به یافته ها، تمرینهای دو ماهه هاتایوگا و آمادگی جسمانی می توانند به عنوان درمانهای مکمل برای کاهش اضطراب و افسردگی مد نظر قرار گیرند.

صفحه 55زمینهتوسعه سرمایه های انسانی در سازمان های بهداشتی که با جان و سلامت انسانها سر و کار دارند، بسیار مهم است.هدفمطالعه به منظور شناسایی عوامل مؤثر بر توسعه سرمایه های انسانی بیمارستانهای سازمان تامین اجتماعی ایران انجام شد.مواد و روش هااین پژوهش مقطعی در سال 1389 در بیمارستانهای تامین اجتماعی ایران اجرا شد. با مطالعه الگوهای توسعه سرمایه های انسانی، الگوی مفهومی اولیه طراحی و با نظر سنجی از خبرگان تایید شد. سپس پرسشنامه طراحی شده بر اساس الگوی اولیه توسط تمام مسئوولین توسعه سرمایه های انسانی مدیریت درمانهای سازمان تامین اجتماعی (68 نفر) تکمیل شد. تحلیل عامل اکتشافی با روش بیشینه احتمال و تفسیر متغیرها با روش واریماکس انجام شد. برای تایید عوامل به دست آمده از تحلیل عامل تاییدی و برای تعیین روابط مستقیم و غیر مستقیم مؤلفه ها از روش الگوی معادله های ساختاری استفاده شد.یافته هاعوامل مربوط به مؤلفه تعهد (توسعه سازمان و تعهد مدیریت) با عامل تعیین مسیر پیشرفت شغلی ارتباط داشت. عامل تعیین مسیر پیشرفت شغلی با عامل برنامه ریزی آموزش رابطه مستقیم داشت. عامل برنامه ریزی آموزش رابطه ای مستقیم با عوامل اقدام های توسعه سرمایه های انسانی (آموزش و توسعه فردی) برقرار می کرد و عامل ارزشیابی نیز به عنوان یک عامل مستقل تاثیرگذار بر تمام عوامل شناخته شد.نتیجه گیریبا توجه به یافته ها، عوامل شناسایی شده در این پژوهش برای توسعه سرمایه های انسانی بیمارستانهای سازمان تامین اجتماعی مناسب به نظر می رسد.

تضاد و تعارض پدیده ای اجتناب ناپذیر درزندگی بشر و مقتضای حیات اجتماعی است. نظام های بهداشتی- درمانی به خصوص بخشهای پرستاری مستعد بروز انواع تعارض هستند که باید توسط سرپرستاران هدایت و کنترل شوند. مذاکره به عنوان روشی با صرفه، موثر و واقعی در حل تعارض شناخته شده است.
هدفمطالعه به منظور تعیین سطح تعارض پرستاران بیمارستانهای عمومی استان بوشهر و ارتباط آن با میزان استفاده ازسبک های مذاکره توسط سرپرستاران انجام شد.
مواد و روش هااین مطالعه همبستگی در سال 1386 بر روی 362 نفر از پرستاران و 53 نفر از سرپرستاران بخشهای مختلف بیمارستانهای عمومی استان بوشهر انجام شد. دو پرسشنامه تعارض دوبرین و سبک مذاکره پیره برای جمع آوری داده ها استفاده شدند. داده ها با آزمون های آماری آنوای یک طرفه و ضریب همبستگی اسپیرمن تحلیل شدند.
یافته هاسطح تعارض پرستاران به جز در بیمارستان فاطمه زهرا(س) که زیاد بود، در سایر بیمارستان ها در سطح متوسطی قرارداشت. سرپرستاران به میزان بالایی از سبک های مذاکره واقع گرایی، تحلیلی و هنجاری استفاده می کردند و تنها از سبک مذاکره شهودی به میزان مناسبی بهره می بردند. بین سطح تعارض پرستاران و میزان استفاده سرپرستاران از سبک های مذاکره همبستگی معکوسی وجود داشت، هرچند رابطه آماری معنی داری بین این متغیرها مشاهده نشد.
نتیجه گیریمدیران بیمارستانهای مورد مطالعه باید ریشه های تعارض را شناسایی و آن را اداره کنند. آنها می توانند از سطح تعارض متوسطی به عنوان مزیت رقابتی در بازارهای سلامت بهره گیرند. سرپرستاران باید درباره مهارتهای حل تعارض مانند مذاکره، آموزش ببینند وسبک های مذاکره خود را بر پایه شرایط خاص تعدیل کنند.

در طبقه بندی مال اکلوژن توسط انگل، مولر بالا بهترین و علمیترین شاخص تشریحی برای تعیین نوع مال اکلوژن و عامل این اختلال، فک پایین عنوان شده است.
هدفمطالعه به منظور تعیین ارتباط شاخصهای مختلف سفالومتریک با موقعیت قدامی- خلفی مولر دایمی اول فک بالا انجام شد.
مواد و روش هااین مطالعه مقطعی در سال 1386 بر روی 250 سفالومتری جانبی که به صورت تصادفی قبل از درمان ارتودنسی انتخاب شدند، انجام شد. نمونه ها به دو گروه دندانهای شیری- دایمی و دندانهای دایمی تقسیم شدند. سه اندازه گیری زاویه ای و هفت اندازه گیری طولی برای هر سفالومتری محاسبه شد. برای مقایسه موقعیت مولر اول فک بالا در دو گروه از آزمونهای آماری تی و ضریب همبستگی خطی استفاده شد.
یافته هادر هیچ یک از دو گروه، رابطه ای بین سن و رابطه دو فک (زاویه ANB) با موقعیت دندان مولر اول فک بالا مشاهده نشد. در گروه دندانهای دایمی، همبستگی مثبت معنی داری بین موقعیت دندان مولر اول فک بالا و شاخصهای جارابک (337/0=r)، Ba-PTM (727/0=r) و طول موثر کام(262/0=r) و همبستگی منفی معنی داری با FMA (388/0- =r) و زاویه بازال (363/0- =r) مشاهده شد. در گروه دندانهای شیری- دایمی، این همبستگی قوی با Ba -PTM (678/0=r) و Ba-A (818/0=r) مشاهده شد.
نتیجه گیریبا توجه به یافته ها، موقعیت قدامی- خلفی مولر اول دایمی فک بالا در گروه دندانهای شیری- دایمی و دایمی متفاوت بود. به نظر می رسد موقعیت دندان مولر اول فک بالا در گروه دندانهای دایمی به الگوی رشدی صورت، عمق صورت و طول فک بالا بستگی دارد.

صفحه 78زمینهحمل دستی بار از مهم ترین علل ایجاد کمردردهای شغلی محسوب می شود. لذا، تعیین حدود مجاز وزن بار جهت جلوگیری از آسیب های ناشی از جابه جا کردن و هل دادن اشیا در کارگران ضروری به نظر می رسد.هدفمطالعه به منظور تعیین حدود مجاز وزن بار در حمل دستی در خطوط بسته بندی یکی از کارخانه های شهر صنعتی قزوین انجام شد.مواد و روش هااین مطالعه مقطعی در سال 1387 در یکی از کارخانه های شهرصنعتی قزوین انجام شد. در این مطالعه سه نفر از افراد پالت گذار به صورت تصادفی انتخاب شدند. بر روی هر پالت 4 طبقه 16 تایی از کارتن ها چیده شد. از 16 نقطه در هر طبقه، 8 نقطه جهت ارزیابی انتخاب شدند. سپس با تعیین ضرایب بلند کردن بار و با استفاده از معادله نایوش، حد مجاز بار RWL)) به دست آمد. مقدار اندیس بلند کردن بار(LI) برای هر کارگر نیز مشخص شد. محاسبه ها توسط برنامه Excel انجام شد.یافته هاوزن کارتن های در حال جا به جایی (15 کیلوگرم) نسبت به حداکثر RWL در مبدا (55/9 کیلوگرم) و مقصد (33/7 کیلوگرم) بالاتر بود و در تمام نقاط مورد نظر، LI بیش تر از یک به دست آمد.نتیجه گیریبا تصحیح ضرایب مربوط به ارتفاع بلند کردن بار، میزان جابه جایی کالا و زاویه چرخش تنه، می توان مقدار LI را تا حدود بسیار زیادی کاهش داد.

صفحه 86: دیابت نوع یک شایعترین بیماری متابولیک در جهان است. بیماری های خودایمنی تیروئید شایعترین اختلال غدد درون ریز همراه با دیابت نوع یک است و در صورت بروز کم کاری تیروئید بر سیر درمان و کیفیت زندگی این بیماران تاثیر می گذارد.
مطالعه به منظور مقایسه آنتی بادی های ضد تیروئید در افراد (کودک و نوجوان) مبتلا به دیابت نوع یک با افراد سالم انجام شد.مواد و روش هادر این مطالعه تحلیلی 65 فرد مبتلا به دیابت نوع یک و 65 فرد سالم مراجعه کننده به مرکز آموزشی- درمانی قدس قزوین در سال 1389 با استفاده از روش نمونه گیری آسان انتخاب شدند. میزان آنتی بادی ضدتیروگلوبولین (Anti-TG) و تیروپراکسیداز (Anti-Tpo) و هورمون های TSH و T4 اندازه گیری شدند. داده ها با آزمونهای آماری مجذور کای و تی مستقل تحلیل شدند.
دو گروه از لحاظ سن و جنس تفاوت معنی دار آماری نداشتند، ولی تفاوت صدک شاخص توده بدنی بین دو گروه معنی دار بود (049/0p=). Anti-TG 7 نفر از افراد دیابتی (%8/10) و یک نفر از افراد سالم (%5/1) مثبت و تفاوت بین دو گروه معنی دار بود (029/0P<). Anti-Tpo در 11 فرد دیابتی (%9/16) و 2 نفر از افراد سالم (%1/3) مثبت و تفاوت بین دو گروه معنی دار بود (024/0 =p) ، 7 فرد دیابتی (%8/10) افزایش TSHو کاهشT4 (کم کاری دیابت آشکار) و 3 فرد سالم (%6/4) افزایش TSH و T4طبیعی (هیپوتیروئیدیسم تحت بالینی) داشتند. میزان T4 در دو گروه تفاوت معنی داری داشت (007/0p=) ، ولی تفاوت میزان TSH از لحاظ آماری معنی دار نبود.

صفحه 94زمینهعوامل زمینه ساز زیادی شانس بروز عفونت ادراری را افزایش می دهند. در برخی مطالعه ها به نقش هیپرکلسیوری در ایجادعفونت ادراری اشاره شده است.هدفمطالعه به منظور ارزیابی وضعیت هیپرکلسیوری در کودکان مبتلا به عفونت ادراری انجام شد.مواد و روش هادر این مطالعه مقطعی که در سال 1386 در مرکز آموزشی- درمانی کودکان قدس قزوین انجام شد، 50 کودک مبتلا به عفونت ادراری از نظر هیپرکلسیوری بررسی شدند. برای تعیین وجود هیپرکلسیوری از نسبت کلسیم بر کراتینین تصادفی ادرار بر حسب میلیگرم استفاده شد. متغیرهای سن، جنس، سابقه عفونت ادراری و هماچوری در گروه هیپرکلسیوریک و گروه با دفع طبیعی کلسیم در ادرار (نرموکلسیوریک) مقایسه شدند. داده ها با آزمونهای آماری، تی و کای دو تحلیل شدند.یافته هااز 50 کودک مبتلا به عفونت ادرای، 27 کودک (%54) به هیپرکلسیوری مبتلا بودند. اختلاف معنی داری بین دو گروه هیپرکلسیوریک و نرموکلسیوریک از نظر وجود یافته های سونوگرافی مشاهده شد(05/0P<).نتیجه گیریبا توجه به یافته ها، ضروری است هیپرکلسیوری در کودکان مبتلا به عفونت ادراری بررسی شود.

Page 7BackgroundThe relation between Single Nucleotide Polymorphisms (SNPs) and a number of diseases has triggered many researches.ObjectiveInvestigating the possible relations between SNPs present in DNMT1 gene from man’s chromosome 19 and colorectal cancer.MethodsThis was a case/control study in which 100 patients with colorectal cancer, referred to Research Center of Gastroenterology and Liver Diseases of Shaheed Beheshti Medical University during 2008, were chosen as case group. The control group included an equal number of patients who visited the medical center for a variety of reasons. Genetic test was carried out on all patients to determine the type of 6 SNPs of DNMT1 gene from man’s chromosome 19. Data were statistically analyzed by SPSS software using chi-square test and logistic regression.FindingsAll SNPs investigated showed significant relations with colorectal cancer indicating that in all cases the chance of getting colorectal cancer in people with genotype 1 and 2 was much higher than those with genotype 0.ConclusionBy exploring people’s SNPs, it is feasible to predict the risk of catching colorectal cancer and thus establishing proper preventive measures.

Lead enters the human body through water, food, air and accumulates in bones instead of calcium. It can damage nervous system, kidneys and genital system especially in children and also affects hemoglobin synthesis.
ObjectiveTo optimize the environment conditions to increase the biosorption with Saccharomyces cerevisiae as well as cell immobilization and determination of its efficiency associated with degree of biosorption.
MethodsThis was a cross-sectional study carried out at the Laboratory Center, Islamic Azad University, Sciences & Researches Division, Tehran (Iran) during 2009-2010. The strain of Saccharomyces cerevisiae used in this study was obtained from Lorestan yeast manufacturer, Lorestan, Iran. All factors affecting biosorption including the initial lead concentration, contact time, and pH were investigated. Entrapment method was used to immobilize cells.
FindingsThe maximum biosorption capacity of 80 mg g-1dw was observed at the following conditions: contact time 2 h, initial lead concentration 500 ppm, and pH 4.5. The metal biosorption capacity of pretreated yeast strain by autoclave was 35.3 mg g-1dw. Also, the biosorption capacity of immobilized cells was almost doubled compared to that of free cells.
ConclusionRegarding the data found in our study, non-pretreated and stabilized Saccharomyces cerevisiae is a suitable biosorbent for removal of lead ions from environment.

Determining the protein content of a 24-hour urine sample is the gold standard for diagnosis of hypertensive disorders in pregnancy.
ObjectiveTo determine whether the 2-hour urine protein value can be used as a substitute for the 24-hour urine protein value in patients with preeclampsia.
MethodsThis was a cross sectional study performed on 60 inpatient women with suspected preeclampsia due to positive urinary test strip with minimum protein content of 1+ and BP≥140/90 at Kosar Teaching Hospital in Qazvin (Iran) during autumn and winter 2008. Urine samples were collected within 24 hours in successive periods: The first 2-hour and the next 22-hours urine, in separate containers. The protein contents of 2-hour and 24-hour urine samples were calculated and the correlation between both groups was determined using Pearson's correlation. The cut off point for 2-hour urine protein was obtained with ROC curve.
FindingsOf 60 patients studied, 42 had proteinuria less then 0.3gr/24h and 18 with proteinuria higher than 0.3gr/24h. No patient with severe proteinuria (>2gr/24h) was detected. There was a significant correlation between the 2-hour and 24-hour urine protein content in patients’ samples. Pearson's correlation coefficient (r) was 0.788 (p=0.000). The cut off point for mild proteinuria was determined at 0.035gr/2h with sensitivity, specificity, positive predictive value, and negative predictive value of 63.3%, 89.5%, 77.8%, and 80.95%, respectively.
ConclusionTotal protein values of 2-hour samples positively correlated with values of 24-hour samples of preeclamptic women and therefore can be substituted for assessing the protein content of 24-hour urine samples as a more convenient, faster, and cheaper method for diagnosis of preeclampsia.

Page 28BackgroundIdentification of factors preventing preeclampsia in high risk pregnant women is of prime importance. The omega-3 supplementation may reduce the risk of preeclampsia.ObjectiveThe aim of the present study was to assess the effects of omega-3 supplementation on high risk pregnancy for preeclampsia in the city of Qazvin.MethodsThis was a single-blind randomized clinical trial conducted on 100 high risk pregnant women for preeclampsia over a period between 14 to 18 weeks of gestation in the city of Qazvin, Iran. Fifty patients were assigned to omega-3 group and another 50 to placebo group. Women were given a daily supplementation of 1000mg omega-3 or placebo until delivery. Later, preeclampsia and pregnancy outcome were assessed in both groups. Data were analyzed statistically using t-test, chi square test, Mannwhitney, and Fisher exact tests.FindingsThere were significant differences between the omega-3 and placebo groups regarding the rate (p=0.015) and intensity (p=0.01) of preeclampsia. Statistically, a significant correlation between the omega-3 group and outcome of pregnancy including the birth weight (p=0.000) and apgar score (p=0.002) was found.ConclusionOur results suggest that supplementation with omega-3 during pregnancy reduces the risk of preeclampsia in high risk pregnant women. Thus it seems necessary to promote educational programs on nutrition for pregnant women.

Non-pharmacologic approaches toward this goal are consistent with midwifery care and the method of choice for many women. Midwives have a pivotal role in using different pain relief approaches.ObjectiveThis study was designed to evaluate the role of education on midwive's knowledge, attitude, and practice towards pain reducing approaches during labor.MethodsThis was a quasi-experimental study carried out at labor wards within the hospitals affiliated to Gilan University of Medical Sciences, Iran. Data were collected using a questionnaire confirmed by a panel of experts and test-retest method for its validity and reliability, respectively. A total of 59 midwives were chosen by quota sampling and participated in a one-day workshop. Pretest and post-test questionnaire were completed before and four months after education. Data were analyzed by descriptive and inference statistical methods using SPSS.FindingsResults showed that the midwive's knowledge, attitude and practice about pain relieving methods changed significantly following workshop (p<0/005). No significant difference between the mean knowledge, attitude and practice scores and demographic characteristics was found.ConclusionAlthough education caused a significant effect on knowledge, attitude, and practice of midwives in using different labor pain relieving approaches as well as a positive attitude towards these approaches nevertheless the health manager's support plays an important role in regular practice of such approaches in labor wards.

Performing continuous care leads to chronic stress in caregivers of patients and this in turn threatens their self-efficacy and quality of life.ObjectiveThe aim of the present study was to determine the effect of progressive muscle relaxation on self-efficacy and quality of life in caregivers of patients with multiple sclerosis.MethodsThis was a controlled clinical trial in which 70 family caregivers of MS patients were investigated. The study population was divided into two age- and sex-matched case and control groups. Progressive muscle relaxation techniques were employed for the case group for three months (90 sessions) and its application was followed using a self-report checklist. After three months, self-efficacy and quality of life questionnaires were re-filled by members of both groups. Data were analyzed using descriptive and presumption statistical tests while a p≤0.05 was considered as significant.FindingsPaired t-test showed a significant difference in self efficacy and quality of life for family caregivers of case group three months after intervention (p=0.001). On the contrary, no significant difference regarding the efficacy and quality of life in family caregivers of MS patients in control group was found (P≥0.05).ConclusionSeveral interventions such as progressive muscle relaxation program with low cost, good safety and effect can help reduce the stress of family caregivers of patients in particular those with chronic diseases such as multiple sclerosis and thus leading to an increase in their self-efficacy, care quality, and quality of life.

Page 48BackgroundAnxiety and depression have increasingly affected many people within the last two decades and therefore researchers are trying to find non-invasive and non-drug methods for these disorders. In this respect, Hatta yoga practice, as an alternative medical system, seems to have attracted considerable attention.ObjectiveThe purpose of this study was to compare the effects of Hatha yoga and physical fitness training on anxiety and depression in women.MethodsThis was a quasi-experimental study with pre-test and post-test performed on 42 women who visited the sport clubs of yoga and physical fitness in the city of Shiraz (Iran) in 2009. Women diagnosed as having anxiety and depression but declined to take medications were invited to fill out the questionnaire designed by the researchers if they were willing to do so. The study population was randomly divided in two groups as Hatha yoga and physical fitness of 21 members each. Exercise protocol included 24 sessions of Hatha yoga and physical fitness over eight weeks and performed as three training sessions of 90 min duration a week. Depression, anxiety, and stress scales were used to evaluate the degree of anxiety and depression.FindingsThere was a significant difference in degree of anxiety and depression between the pre-test and post-test findings in Hatha yoga group (p=0.017) but not in the physical fitness group (p=0.664). Also, a significant difference in post-test results for anxiety and depression between the Hatha yoga and physical fitness groups was demonstrated (p=0.001).ConclusionConsistent with other similar studies, our study showed that the Hatha yoga and physical fitness practices have beneficial effects on management of anxiety and depression symptoms and could lead to appearance of a balance in physical, emotional, mental, and spiritual capacities of an individual. Hence the Hatha yoga and physical fitness practices can be used as an alternative medical system for these disorders.

Initially, a conceptual model was designed based on comparative study of Human Capital Development models which was later confirmed by a group of expert. In addition, a questionnaire was designed based on conceptual models of components and variants and filled out by the organization’s Education authorities (n=68) in two stages with a 20-day interval to assess the current state of the Human Capital Development. After determining the adequacy of sample volume by Kaiser Meyer Olkin method and meaningfulness of coefficient matrix with Bartlott test, the exploration factors analysis using maximum probable method as well as the interpretation of variants by Varimax method were performed. Later, to confirm the acquired factors verifying factor analysis and to determine the direct and indirect relationships of components, the structural equations model method was used.FindingsFactors-related commitment component (organization development and management commitment) showed a direct relationship with planning component (career development path and plan for education). The factors of career development path and master plan for education demonstrated a bilateral relationship in which a direct correlation between these factors and the factors of Human Capital Development (education and personal development) was established. Also, it was found that the Evaluation factor is an effective independent factor influencing all other factors.ConclusionThe factors recognized in this research are suitable for Human Capital Development in hospitals affiliated to Iran social security organization.

Conflict and dispute are inevitable phenomena in man’s life and considered as necessity of social activities. Healthcare system, especially nursing ward is susceptible to develop kinds of conflict that must be directed and controlled by head nurses. Negotiation as an advantageous, effective and factual method is known to resolve the conflict.
ObjectiveThis research intended to investigate the correlation between nurses’ conflict level and head nurses negotiation styles at general hospitals in province of Boushehr.
MethodsThis was a kind of correlation study carried out in a given time course in 2007. The study population consisted of 362 nurses and 53 head nurses from general hospitals in Boushehr province south of Iran. Two questionnaires including the Dubrin conflict and Pierre negotiation styles questionnaires were used to collect data based on literature survey. Descriptive and analytic statistics were used for data analysis.
FindingsResults showed that the nurse's conflict level was located at average level except for Fatemeh Zahra Hospital in which a high level of conflict was observed. Head nurses had overuse of factual, analytic and normative negotiation style with only proper use of initiative negotiation style. There was a reverse correlation between the nurse's conflict level and the use of negotiation styles yet no significant difference between these variables was demonstrated.
ConclusionHospital managers must recognize the causes of conflict to properly handle it. There are some important chances in hospitals with average level of conflict to improve the performance and produce creativity. These opportunities could be used as competitive advantage in health markets. Head nurses must learn conflict resolving skills such as negotiation but they must also use these skills correctly and adjust their negotiation styles on the basis of special situations.

In Angle classification, the maxillary first molar furnishes more nearly than any other tooth or point in the anatomy an exact scientific basis from which to reason on malocclusion. Angle believed that malocclusions are of the mandibular origin.
Objectivethe purpose of this study was to evaluate the association between some cephalometric parameters and sagittal position of the maxillary first permanent molar.
MethodsThis was a descriptive analytical study in which 250 lateral cephalometric samples that were randomly selected before orthodontic treatment were examined. The samples were divided into two groups marked as mixed dentition and permanent dentition. Statistical analyses including the linear correlation coefficients and student t-test were used to compare the upper molar position in two groups.
FindingsThere was no significant correlation between the age, maxillary length, and ANB in two groups. The position of maxillary first molar in the permanent dentition demonstrated a significant positive relationship with ANS-PNS, Jaraback index, and Ba-PTM whereas a significant negative correlation was found with FMA and Basal angle. Moreover, the maxillary first molar position in the mixed dentition showed a significant positive relationship with Ba-A and Ba-PTM.
ConclusionBased on results found in the present study, the sagittal position of maxillary molar between mixed and permanent dentition was different. Maxillary first molar position in the permanent dentition showed a significant relationship with facial growth pattern, facial depth, and maxillary length. However, more longitudinal studies to confirm the findings of this study are needed.

The reasons for application of recommended weight limit is due to injuries frequently occur in workers while pushing or conveying objects.ObjectiveThe aim of this research was to assess the recommended weight limit in manual carrying of loads using NIOSH standard in Packaging lines of a factory within the industrial city of Qazvin.MethodsThis was a descriptive cross-sectional study carried out in one of the factories of industrial city of Qazvin in 2008. Three palette workers who were selected randomly included in our study. On each palette, cartons were arranged in 4 rows each containing 16 cartons. From 16 points on each row, 8 points were selected for assessment. Later, through determination of the multipliers of load lifting, the RWL (resistible weight limit), which is the final product of this equation, was obtained and the LI (lifting index) of each worker calculated. Calculations (definition of variables and coefficients as well as calculation of RWL and LI) and drawing of diagrams were performed using Excel program.FindingsConsidering the recommended RWL, the box weight used in this study (15 kg) was heavier than the recommended weight limit (9.55 kg for origin and 7.33 kg for destination) and the LI was excited in all cases.ConclusionBy correcting the coefficients associated with load lifting height, distance of load displacement, and turning angle of the body, the LI can decrease to a great extent.

Page 86BackgroundType 1 diabetes is the most common metabolic disease worldwide. Thyroid autoimmune diseases is the most common endocrinopathy in diabetes type 1. Therefore, assessment of thyroid function tests and anti thyroid antibodies in diabetic patients (type 1) is beneficial in early.ObjectiveThe purpose of this study was to compare the anti-thyroid antibodies in diabetic children (type 1) with healthy children.MethodsIn this descriptive-analytic study, 65 children with Type 1 diabetes and 65 healthy children who had referred to Children's Hospital in Qazvin were selected by simple sampling. Anti-TG, Anti-TPO, TSH and T4 were measured in both groups. Chi-square, t-independent and Kruskal Valis was used for statistical analysis.FindingsThe difference between sex and age in both groups (case and control) was not significant but BMI percentile was significantly different. Positive Anti-TG was 10.8% in patients and was 1.5% in controls. The difference was statistically significant (p=0.029). 16.9% of patients and 3.1% of controls had positive Anti-TPO (p=0.024).10.8% of patients had increased TSH and decreased T4 level (Overt Hypothyroidism). 4.6% of controls had increased TSH and normal T4 level (Subclinical Hypothyroidism). The difference was statistically significant for T4.ConclusionIt seems that the prevalence of Hashimoto thyroiditis in patients with type 1 diabetes is more than healthy people. Hence, evaluation of thyroid function tests (TFT) and antithyroid antibodies (Anti-TPO) is necessary to diagnosis and treatment of autoimmune thyroid disorders.

Page 94BackgroundThere are many underlying factors that increase the chance for occurrence of UTI and some reports indicate a role for hypercalciuria in UTI.ObjectiveThe objective of this study was to assess the Ca/Cr status in urinary tract infection of children.MethodsThis was a cross- sectional antegrade study conducted in Qods teaching hospital of Qazvin from 2006-2007, in which 50 children with urinary tract infection were studied for hypercalciuria. To determine hypercalciuria, the Ca/Cr ratio of a random urine sample was measured. Different variables such as age, gender, history of UTI and etc, were compared between hypercalciuric and normocalciuric groups. Results were analyzed with SPSS using t-test and chi square test.FindingsOf 50 patients, 27 cases (54%) were found to have hypercalciuria. There was a significant difference between hypercalciuric and normocalciuric groups regarding the abnormal sonographic findings (p<0.05).ConclusionBased on results of this study evaluation of hypercalciuria in UTI is recommended.
