Permanent maxillary second molars’ root canal treatment due to their root configurations complexity and high root canal systems variations, are one of the most problematic teeth. Accordingly, clinicians should consider unusual canal morphology to reduce the rate of missed canals. A thirty-year-old female was referred for root canal treatment of her maxillary right second molar tooth with obvious recurrent caries under restoration and necrotic pulp. Pre-operative radiographs showed the unusual morphology and cone-beam computed tomography was ordered for precise evaluation of the root canal anatomy. The principal aim of this paper is to report a five-root canals maxillary second molar, with two palatal root canals and a palatal root-like enamel pearl canal.

Background
This cross-sectional study evaluated the relationship between sleep disturbance levels and Trigeminal neuralgia disorder during 2019-2020 in School of Dentistry of Shiraz University of Medical Sciences.
Methods49 patients with Trigeminal neuralgia (TN) disorder under treatment, referring to department of maxillofacial medicine of School of Dentistry of Shiraz University of Medical Sciences, who were chosen according to available sampling were included in the study and 40 healthy persons also have been recruited in the control group.The Persian version of Pittsburgh sleep quality questionnaire (PSQI) was usted to evaluate sleep disturbance of the participants. The final results were compared with Chi-square test, and more than ninety-five percents reliability was reflected significant (P <0.05).
Results56.89 ± 10.93 was the TN participants’ group mean age and 37.12 ±9.55 was the mean age of the control group. The sleep disturbance of patients with TN was meaningfully higher in comparison with control group (P.value <0.0001). The sleep disturbances mean level in each group between men and women was not significant (P.value >0.05). However, the mean level of sleep disturbance of women and men in the group of TN patients was meaningfully higher than that of men and women in the group of control (P.value :0.001). The sleep disturbances mean level was higher in TN patients only in the age range of 41-50 years (P.value =0.02)
ConclusionThe patients with trigeminal neuralgia had meaningfully lower level of sleep quality than control group.

BackgroundOral lichenoid lesions (OLL) could be premalignant lesions. Hence, it is critical to investigate the epidemiologic aspects of oral lichenoid lesions.MethodsThis cross-sectional study evaluated the prevalence of oral lichenoid reaction in patients attending the Department of Oral Medicine, School of Dentistry, during 2007-2018. All clinically and histopathologically documented files of oral lichen planus (OLP)- like lesions were evaluated, and oral lichenoid reactions were identified. The site of oral involvement, potential causes of OLL, and medications accompanied with OLL were all documented. The data were analyzed using SPSS version 18.ResultsOf the 191 patients with OLL, 137 (71.7%) were females, and 54 (28.3%) were males, with a mean age of 46.242 ± 13.77. In contrast, the control group consisted of 190 patients with OLP. The control group included 112 women (58.9%) and 78 (41.5%) men. The mean age of patients with OLP was 45.4 ± 11.16 years old, which matched the OLL group. Medications (48.2%) and amalgam restorations (52%) were the two most common causes of lichenoid reaction. While levothyroxine was used more frequently (17.8%) in patients with OLP, anti-hypertensive (30.4%) medications were used more predominantly in patients with OLL. In the present research, 8.23% of lichenoid lesions and 4.2% of OLP exhibited dysplastic change. The prevalence of OLL was 15.8% in patients who were referred to the Department of Oral Medicine, School of Dentistry. The prevalence of dysplasia was statistically higher in OLL patients (P < 0.0001). The odds ratio was 4.6, 95% CI: 2.06-10.28.ConclusionThis study showed that the prevalence of dysplasia was statistically higher in OLL patients.Keywords: Lichenoid Reaction, Precancerous Conditions, Amalgam

Statement of the Problem:
Endodontic efforts are focused on eliminating intracanal pathogens. Applying intracanal medicament for infected teeth is beneficial for achieving better antibacterial effects in endodontic treatments. Different intracanal medicaments should be assessed and compared for this purpose.
Purposeto assess the antibacterial efficacy of Citrullus colocynthis seed extract comparing to Ca(OH)2 on teeth contaminated with Enterococcus faecalis.
Materials and MethodIn this in vitro study, a novel strain of Enterococcus faecalis (Enterococcus spp. ATCC 19433) yielded from the root canal treated tooth with persistent apical periodontitis. The canals of 78 human single-rooted extracted teeth were contaminated with mentioned strain and treated with Citrullus colocynthis essential oil and Ca(OH)2 for 1, 7, and 14 days. To determine the chemical composition of the oils, gas chromatography-mass spectrometry (GC–MS) was applied. The percentage reduction from baseline c.f.u./mL values was estimated.
ResultsOleic acid, benzoic acid, and gallic acid were the major contents of Citrullus colocynthis essential oil. The c.f.u./mL count decreased considerably as contact duration rose for both medicaments. After 7 days, a statistically significant difference was identified between the medicaments. Citrullus colocynthis showed higher antimicrobial efficacy. However, after 14 days, no substantial difference was found.
ConclusionCitrullus colocynthis essential oil, displayed great antimicrobial efficacy against Enterococcus faecalis higher than Ca(OH)2 over the first week contact period.
