Comparison of physiotherapy and Drug in Treatment of Myofacial Pain Dysfunction Syndrome

Myofacial Pain Dysfunction Syndrome (MPDS) causes series of sign and symptoms that cause pain in tempormandibular joint and masticatory muscls. MPDS has multifactor etiologies include stress, malocclusion, brouxism and clinching. Stress is main risk factor. The aim of this study was to compare the therapeutic effectiveness of ultra sound, mobilization and soft food plus drug in treatment of MPDS. The study is a Randomized Clinical Trial carried out on 63 patients with MPDS, divided in three groups by simple random sampling. The first group was treated by ultrasound at doses 0.5 W/cm2 over 5 minutes three times a week for 4 weeks. The second group was treated by using mobilization technique for 5 minute, three time a week for 4 weeks, and the third group was treated with soft diet and piroxicam (orally, 10 mg BD), and diazepam 5 mg (orally daily before bedtime) and diclofenac jell applied topically three times in day. The masticatory muscles tenderness, maximum mouth opening and lateral and forward movement of mandibles were measured before and after of treatment. All the methods employed showed improvement in pain score and mobilization in the mandibular movement. However, mobilization method was found to produce better therapeutic effectiveness.These findings suggest that mobilization method of treatment is a more favorable method in the treatment of MPDS.
