    Background
Survivors of childhood malignancy are at risk of long-term late effects. One of the most commonly involved systems in this effect is endocrine system. Appropriate timely recognition and treatment of late complications including endocrine complication is essential for the continuing health and improvement in quality of life in cancer survivors.
    Objectives
Therefore, this study has been conducted to investigate the aforementioned complications resulting from the treatment of the common malignancies in children in Iran.
    Patients and
    Methods
We performed a cross sectional study for evaluation of endocrine complication in cancer survivors who had finished their cancer treatment and come for followed up in oncology clinic of children medical center.
    Results
Demographic data, treatment modality, and endocrine late effect recorded and analyzed. There were 96 survivors with median age of 13.8 ± 5.8 years (range 3 - 31 years). The median follow up time was 2.7 years (range 1 - 10 years). Hyperinsulinemia was the most common late effect. Other complication was dyslipidemia, overweight, obesity, osteopenia, hypothyroidism, diabetes mellitus and osteoporosis respectively.
    Conclusions
Our study has demonstrated a significant prevalence of endocrine complication after childhood cancer therapy and a long term follow-up program for survivors of childhood cancer is therefore needed.

    Background
Parkinson’s disease is a progressive neurological disease. Measuring quality of life in these patients could be a useful tool for evaluation and prioritization of care services. The 39-item Parkinson’s Disease questionnaire (PDQ-39) is an acceptable measure to assess quality of life in these patients.
    Objectives
This study aims to assess the validity and reliability of the Persian version of PDQ-39 questionnaire.
    Patients and
    Methods
In this cross-sectional study, Persian version of questionnaire was prepared with translation, back translation, cognation debriefing and culture adaptation. One hundred four Parkinson’s disease patients referred to Shahid Motahari clinic affiliated to Shiraz University of Medical Sciences were included in the study. Then PDQ-39 and general quality of life questionnaire (36-Item short form health survey: SF-36) were completed by trained interviewers. The reliability of the questionnaire was measured through Cronbach’s alpha coefficient. Construct validity assessed by item-scale correlation method and criterion validity was determined by discriminant validity and convergenent validity methods.
    Results
Range of Cronbach’s alpha coefficient for different dimensions varied from 0.64 to 0.92. Cronbach’s alpha coefficient was higher than 0.70. Item-convergent validity ranged from 0.38 to 0.83. Similar dimensions of PDQ-39 and SF-36 questionnaire were highly correlated. The quality of life among higher stage patients was lower (P
    Conclusions
Findings showed reliability and validity of Persian version of PDQ-39 is acceptable. It can be used to measure health related quality of life in Persian-speaking Parkinson’s disease patients.

    Background
Kawasaki disease (KD) is a systemic syndrome of inflammatory with unknown cause and usually occurs in children. It is the most common cause of acquired heart disease in industrialized countries.
    Objectives
The aim of this study was to examine the course of cardiovascular involvement in patients with KD after treatment with intravenous immunoglobulin and aspirin.
    Patients and
    Methods
This retrospective study was performed on all children who were admitted with diagnosis of KD and had received intravenous immunoglobulin therapies and aspirin in Ali Ibne Abi Talib hospital during 2004 to 2013 years. Analysis was done by SPSS-20 with 0.05 as significant level.
    Results
Out of 53 patients, 60% were male. Early involvement of the right and left coronary heart disease were 20.75% and 26% respectively. The frequency of aneurysm for left and right coronary artery was 3 (5%) cases. The mean of left coronary artery and right coronary artery diameters in before and after treatment were 2.39 ± 0.84, 1.95 ± 0.72 and 2.12 ± 0.54, 1.81 ± 0.31 mm respectively and they had significant differences (P = 0.001). The means of duration of fever for with and without coronary aneurysm (13 ± 4.2 vs. 8.8 ± 2.8 days), sedimentation rate (121.5 ± 21 vs. 73.9 ± 29.8 mm) and alkaline phosphatase (808 ± 101 vs. 476 ± 148 Iu/L) in patients with KD had significant difference of P = 0.04, P = 0.03 and P = 0.003 respectively.
    Conclusions
We found coronary artery disease and aortic aneurysm is almost similar to other studies and showed age, duration of fever onset, intravenous immunoglobulin, sedimentation and alkaline phosphatase have a role in coronary involvement and aneurysm.

    Background
Constitutional delay of growth and puberty (CDGP) is the most common cause of short stature. This variant of normal growth can be associated with psychological stress in some affected individuals.
    Objectives
We aimed to assess the effects of clonidine on short-term growth rate in children and adolescents with constitutional growth delay.
    Patients and
    Methods
In this prospective, randomized clinical trial study, 48 prepubertal constitutionally short boys (6 -12 years old) with height standard deviation scores (HSDS) less than -2, bone age delay 2.1 ± 0.2 years, GH concentration > 10 μg/L after provocation and growth rates
    Results
Mean age and height in two groups were similar at baseline. Mean growth velocity in group A was 2.9 ± 0.91 and 3.1 ± 0.98 cm/6 month at baseline and 6th months, respectively and in group B was 2.9 ± 0.94 and 2.4 ± 0.47 cm/6 month at baseline and 6th months, respectively (P = 0.01). Mean HSDS in group A was -2.79 ± 0.65 and 2.39 ± 0.39 at baseline and 6th months, respectively and in group B was 2.69±0.73 and 2.39 ± 0.48 at baseline and 6th months, respectively.
    Conclusions
Although a slightly increase in HSDS was observed in treated group, but difference was not significant compared with control subjects. So, clonidine therapy could not improve height standard deviation score and growth velocity of children and adolescents with CDGP.

    Background
Anxiety is an unpleasant state of feeling agitation and pressure induced by the patient’s fear of hospitalization, anesthesia or surgery.
    Objectives
The present study is aimed to compare the effects of dried extract of saffron and diazepam on soothing the pre-herniorrhaphy surgery anxiety.
    Patients and
    Methods
In this double blind clinical study, during 8 months, 102 patients were studied in 2012. all the patients in intervention group (n = 50) received 25 mg dried extract of saffron and participants in control group (n = 52) received 5 mg oral diazepam. Level of anxiety of the patients was measured using speillberger state-trait anxiety inventory (STAI) before intervention and 3 hours after administration (immediately after entering the surgery room) filled out by the patients. The patients in the two groups were compared concerning level of anxiety before/after intervention. The two groups of participants had almost equal demographic features.
    Results
According to the results, level of trait anxiety in the intervention groups decreased from 37.24 ± 7.4 to 34.72 ± 7.45 and the trend among the control group was vice versa and increased from 34.51 ± 10.74 to 38.8 ± 9.97. The data showed significant relation between level of trait anxiety before and after intervention in control and experiment groups (P = 0.019).
    Conclusions
Considering the findings, in comparison to diazepam, dried extract of saffron at 25 mg dose is more effective on soothing anxiety.

    Background
Rumination is one of the extreme negative cognitions in patients with obsessive compulsive disorder (OCD). It is assumed that some cognitive problems such as attentional control can facilitate rumination and some internal capabilities such as resiliency can reduce engaging in these ruminations.
    Objectives
The present study investigated the relationship between rumination with attentional control and resiliency in patients with OCD.
    Patients and
    Methods
In this correlational study, 50 patients with OCD were selected through accessible purposive sampling. The data was obtained through ruminative response, Connor and Davidson resiliency, and scalability questionnaires and were analyzed using the Pearson correlation and multiple regression tests.
    Results
The results indicated a significant negative correlation (P = 0.001) between rumination and attentional control (r = 0.573) and resiliency (r = 0.579). Results of regression analysis indicated increased attentional control and resiliency can predict reduction of rumination (P = 0.001).
    Conclusions
Regarding that attentional control and resiliency are associated with rumination and can predict rumination in patients with obsession, nurturing resiliency traits and teaching techniques to improve attentional control can be effective in the control and prevention of rumination.
