    مقدمه
    بیماران تحت جراحی قلب به طور معمول تحت درمان با داروهای ضد ترومبوز (استیل سالیسیلیک اسید و آنتی کواگولان ها) می باشند که منجر به بروز برخی مشکلات گوارشی مثل زخم پپتیک بدون علامت در آن ها می شود و بدین جهت، درمان پروفیلاکتیک جهت مشکلات دستگاه گوارش فوقانی ناشی از عمل جراحی قلب ضروری به نظر می رسد. مطالعه ی حاضر، با هدف مقایسه ی تاثیر پنتوپرازول و رانیتیدین در کاهش بروز عوارض گوارشی بعد از عمل در بیماران تحت جراحی پیوند عروق کرونر به انجام رسید.
    روش ها
    طی یک مطالعه ی کارآزمایی بالینی، 90 بیمار کاندیدای عمل جراحی پیوند عروق کرونر به روش تصادفی به دو گروه 45 نفره تقسیم شدند. جهت پیش گیری از عوارض گوارشی بعد از عمل، در گروه اول پنتوپرازول و در گروه دوم رانیتیدین، قبل و بعد از عمل جراحی تا زمان ترخیص از بخش مراقبت های ویژه تجویز و از نظر بروز عوارض دستگاه گوارش فوقانی تا زمان ترخیص از بخش مراقبت های ویژه بررسی و نتایج به دست آمده با استفاده از نرم افزار SPSS تجزیه و تحلیل و مقایسه شد.
    یافته ها
    در طی مدت بستری در بخش مراقبت های ویژه، 13 نفر (9/28 درصد) از گروه پنتوپرازول و 15 نفر (3/33 درصد) از گروه رانیتیدین، دچار علایم گوارشی شدند، اما تفاوت بین دو گروه معنی دار نبود (65/0 = P). علایم گوارشی پیش گفته، شامل تهوع و Distention شکم بود و موردی از Hematemesis و Melena در دو گروه مشاهده نشد. میانگین مدت زمان بستری در بخش مراقبت های ویژه در دو گروه پنتوپرازول و رانیتیدین به ترتیب 9/12 ± 1/44 و 1/28 ± 0/51 ساعت بود، اما اختلاف دو گروه معنی دار نبود (140/0 = P).
    نتیجه گیری
    برابر نتایج این مطالعه، مزیتی در استفاده از پنتوپرازول در پیش گیری از عوارض گوارشی بعد از عمل جراحی پیوند عروق کرونر نسبت به رانیتیدین مشاهده نمی شود، اما با توجه به ارزان تر بودن رانیتیدین نسبت به پنتوپرازول، استفاده از رانیتیدین به لحاظ اقتصادی ارجحیت دارد.

  • بررسی سمیت عصاره ی هیدروالکلی هندوانه ی کوهی Momordica Charantia بر کبد موش های سوری نر

    مقدمه
    هندوانه ی کوهی، به علت خاصیت هیپوگلایسمیک شهرت دارد و به صورت گسترده در جامعه جهت درمان دیابت استفاده می شود. در این تحقیق، علاوه بر بررسی ظرفیت آنتی اکسیدانی گیاه و اثر آن بر ظرفیت آنتی اکسیدانی خون، احتمال هپاتوتوکسیسیتی این گیاه با بررسی شیمیایی و هیستولوژیک مورد ارزیابی قرار گرفت.
    روش ها
    در این مطالعه، 70 موش سوری نر با وزن 30-25 گرم وارد مطالعه شدند و به طور تصادفی در 7 گروه قرار گرفتند. موش ها به مدت 7 روز در شرایط آزمایشگاهی نگهداری و سپس در روز هشتم دوزهای مختلف عصاره ی هندوانه ی کوهی از طریق تزریق داخل صفاقی دریافت کردند. گروه های درمان تک دوز شامل 100، 500، 1000، 2000 و 4000 میلی گرم بر کیلوگرم و گروهی که یک هفته تحت درمان با دوز 500 میلی گرم بر کیلوگرم به صورت روزانه قرار گرفت. سپس به مدت 72 ساعت موش ها تحت نظر بودند و بعد از آن کشته شدند. کبد موش ها جهت بررسی هیستولوژی برداشته و تغییرات هیستوپاتولوژیک کبد بررسی شد. سرم موش ها نیز از نظر آنزیم های کبدی (ALP یا Alkaline phosphatase، SGPT یا Serum glutamic-pyruvic transaminase و SGOT یا Serum glutamic oxaloacetic transaminase) بررسی شد. ظرفیت آنتی اکسیدانی خون و گیاه نیز محاسبه شد.
    یافته ها
    در کلیه ی گروه هایی که دارو را به صورت تک دوز دریافت کرده بودند، تغییرات آماری معنی داری در میزان آنزیم های کبدی SGOT، SGPT و ALP و همچنین در مطالعات بافت شناسی مشاهده نشد (05/0 < P). مقادیر آنزیم های SGOT، SGPT و ALP به ترتیب 0/76 ± 8/180، 0/54 ± 6/80 و 0/88 ± 7/204 بود. ظرفیت آنتی اکسیدانی هندوانه ی کوهی 68 درصد و ظرفیت آنتی اکسیدانی خون 564 میکرومول بر لیتر به دست آمد.
    نتیجه گیری
    هندوانه ی کوهی تا دوز 4000 میلی گرم بر کیلوگرم به صورت تک دوز برای کبد آسیب و سمیتی ایجاد نمی کند، اما انجام مطالعات بیشتر با دوز های مختلف دارو پیشنهاد می شود.

    مقدمه
    شانه ی منجمد، یک وضعیت دردناک شانه است که در صورت عدم پاسخ به درمان کنزرواتیو، جراحی کمک کننده است. در این مطالعه ی آینده نگر، سعی شد مقایسه ای بین دو روش شایع درمان جراحی انجام گیرد.
    روش ها
    در سال های 91-1390، 54 بیمار مبتلا به شانه ی منجمد به یکی از دو روش آزادسازی آرتروسکوپی و Manipoulation زیر بیهوشی تحت درمان قرار گرفتند. متغیرهای مورد مطالعه عبارت از درد پس از عمل، نمرات Simple shoulder test (SST) و American Shoulder and Elbow Surgeons (ASES)، دامنه ی حرکات در جهات مختلف و درد در پی گیری نهایی بودند.
    یافته ها
    تمامی بیماران در هر دو گروه، بهبود واضحی نسبت به قبل از عمل جراحی در پی گیری حداقل یک ساله داشتند. تفاوت متغیرها در دو گروه از نظر چرخش به داخل، فلکسیون قدامی و درد، در پی گیری نهایی معنی دار نبود، اما در سایر متغیرها، گروه آرتروسکوپی بهبود بیشتری نشان دادند. ارتباط معنی دار آماری بین متغیرها با دیابت، سن، درگیری سمت غالب و جنس پیدا نشد.
    نتیجه گیری
    بر اساس نتایج مطالعه ی فعلی، به نظر می رسد که هر دو روش Manipoulation زیر بیهوشی و آرتروسکوپی، درمان های موثری برای شانه ی منجمد می باشند.

    مقدمه
    این مطالعه، با هدف بررسی اثر ضد دردی استامینوفن وریدی در مقایسه با مورفین وریدی در بیماران دچار شکستگی ساق پا و ساعد دست انجام شد.
    روش ها
    این مطالعه، یک مطالعه کارآزمایی دو سو کور بود که بر روی بیماران 65-15 ساله ی مبتلا به شکستگی ساق پا و ساعد دست با درد متوسط تا شدید (نمره ی VAS یا Visual analog scale بیشتر از 4) انجام شد. بیماران به دو گروه تقسیم شدند. در گروه اول، استامینوفن با دوز 1 گرم و در گروه دوم مورفین با دوز 1/0 میلی گرم در هر کیلوگرم در 100 میلی لیتر نرمال سالین تزریق شد. بعد از 30 دقیقه، درد بیماران با استفاده از مقیاس آنالوگ اندازه گیری شد. عواض جانبی دارو نیز ثبت گردید.
    یافته ها
    55 بیمار وارد مطالعه شدند. نمره ی درد در مقیاس آنالوگ بین گروهی که استامینوفن دریافت کرده بودند، با گروهی که مورفین دریافت کرده بودند، تفاوت معنی داری نداشت (140/0 = P)، اما در مورد عوارض جانبی، بین دو گروه تفاوت معنی داری وجود داشت (014/0 = P).
    نتیجه گیری
    به نظر می رسد، استامینوفن وریدی در مقایسه با مورفین وریدی، سطح درمانی مشابه، اما عوارض جانبی کمتری دارد. مطالعات بزرگ تری توصیه می شود.

    کافئین، یک متیل گزانتین مشتق شده از پورین می باشد که در گیاهان سراسر جهان (قهوه، چای سیاه و کولا) یافت می شود. امروزه، کافئین کاربرد وسیعی در تولید لوازم آرایشی دارد و در این زمینه، مورد استقبال واقع شده است. در این مقاله، مکانیسم آثار متنوع و گسترده ی آن بر پوست و مو مورد بررسی قرار می گیرد. اضافه کردن کافئین به فرمولاسیون لوازم آرایشی (مانند فراورده های ضد آفتاب) در کاهش تاثیر اشعه ی Ultraviolet (UV) موثر است. کافئین، تخریب کننده ی رادیکال های آزاد ناشی از اشعه ی UV می باشد. این ماده، با مهار آنزیم فسفو دی استراز و افزایش غلظت Cyclic adenosine monophosphate (cAMP)، آپوپتوز را در سلول های کراتینوسیت آسیب دیده ی پوست انسان افزایش می دهد. همچنین، کافئین با مهار عملکرد Ataxia-telangiectasia and Rad3-related (ATR)، چرخه ی ATR-chk، باعث توقف چرخه ی سلولی و القای آپوپتوز می گردد. از سوی دیگر، اثرات مختلف کافئین در درمان ریزش مو به صورت محلول موضعی یا افزودن در فرمولاسیون شامپوها استفاده می شود. cAMP ناشی از این ماده، کشش عضله ی صاف را در نزدیکی فولیکول مو کاهش می دهد و تحویل مواد مغذی از طریق رگ های خونی را آسان تر می کند. کافئین همچنین، تاثیر تستوسترون در فولیکول های مو در مردان را متوقف می کند. امروزه، بیشتر محصولات مراقبتی مو حاوی کافئین هستند. سایر مستندات در خصوص مکانیسم کافئین در این زمینه به صورت دقیق تری در متن مقاله مورد بحث قرار می گیرد.

    Background
    Patients undergoing coronary surgery are usually treated by anti-thrombus [acetyl salicylic acid (ASA) and anti-coagulant] drugs that lead to emergence of some gastrological problems such as peptic ulcer without any symptom. The present study aims at comparing the effects of pantoprazole and ranitidine in reducing the incidence of gastrointestinal complications in patients undergoing coronary artery bypass graft surgery (CABG).
    Methods
    In a clinical trial, 90 candidates for CABG surgery were randomly divided into two groups of 45 patients. In the first group, pantoprazole and in the second group, ranitidine was prescribed before and after surgery until releasing from intensive care unit (ICU). They were studied in terms of incidence of upper gastrological problems until releasing from ICU and the obtained data were analyzed by SPSS software.
    Findings: During hospitalization in ICU, 13 patients from pantoprazole and 15 patients from ranitidine group showed gastrological symptoms (28.9% on contrary to 33.3%); however, the difference between two groups was not significant (P = 0.65). The mentioned gastrological symptoms include abdominal distention and vomiting and no case of hematomas and melena was observed in either of groups. The mean duration of hospitalization in ICU in pantoprazole and ranitidine groups were 44.1 ± 12.9 and 51.0 ± 28.1 hours, respectively; however, the difference between two groups was not significant (P = 0.14).
    Conclusion
    There is no advantage in using pantoprazole rather than ranitidine for patients hospitalized in ICU and ranitidine could be preferred due to economical matters.

  • Evaluating the Liver Toxicity of Hydroalcoholic Extract of Momordica Charantia in Male Balb/C Mice

    Background
    Momordica charantia (bitter melon) is known for its hypoglycemic effect and widely used for the treatment of diabetes. This study other than evaluating plant antioxidant and its effect on blood antioxidant capacity, examined the effects and safety of bitter melon fruit in laboratory mice.
    Methods
    70 male mice (2-3 weeks old, body weight 25-30 g) were randomly divided into 7 groups. The mice were acclimatized to laboratory conditions for 7 days and at day 8, they were dosed intraperitoneally (single dose groups: 0, 100, 500, 1000, 2000, 4000 mg/kg and the group which was treated for 7 days: 500 mg/kg/day). Mice were then observed for 72 hours before they were scarified, immediately livers were taken for histology. Serum samples were assayed for liver functions [alkaline phosphatase (ALT), serum glutamic-pyruvic transaminase (SGPT) and serum glutamic oxaloacetic transaminase (SGOT)]. Blood and bitter melon antioxidant activity was measured.
    Findings: All single dose groups showed normal behavior after the dosing and no statistical changes were observed in all liver parameters including for SGOT, SGPT or ALP (P > 0.05). Lab data were shown as follow: ALP = 204.7 ± 88.0, SGOT = 180.8 ± 76.0, SGPT = 80.6 ± 54.0. Histological examinations revealed normal organ structures. Antioxidant activity of bitter melon was 68% and blood antioxidant activity was 564 µmol/ml.
    Conclusion
    Doses up to 4000 mg/kg did not have any effects on the mice liver functions nor its histology. We suggest more studies with different doses.

    Background
    Frozen shoulder is a painful condition in which surgery may be helpful in case of failure of conservative treatment. In this prospective study we tried to compare two common methods of surgical treatment.
    Methods
    Between 2011 and 2012, 54 patients with frozen shoulder were treated by one of the two methods of arthroscopic release and manipulation under anesthesia. The variables studied were postoperative pain, American Shoulder and Elbow Surgeons (ASES) and Simple shoulder test (SST) scores, range of motion in different directions and pain in the last follow up.
    Findings: All the patients in the two groups had significant improvement in comparison to preoperative scores at the least follow up of one year. The difference between the two groups was not significant in internal rotation, forward flexion and pain in the last follow up, but among the other variables, the arthroscopy group showed more improvement. No statistically significant association was found among the variables and diabetes, age, dominant limb involvement and sex.
    Conclusion
    Based upon the findings of the present study it seems that both manipulation under anesthesia and arthroscopic release are effective treatments for frozen shoulder.

    Background
    This study evaluated the analgesic and safety effects of intravenous acetaminophen versus intravenous morphine in management of pain in forearm and leg fractures.
    Methods
    This study was a randomized double-blind clinical trial conducted on patients (15-60 years-old) with forearm and leg fracture and moderate to severe pain (pain score of 4 or more). Patients were randomized in to two groups, then, 1 g of intravenous acetaminophen or 0.1 mg/kg morphine was infused in 100 ml normal saline to either acetaminophen or morphine group. The pain severity was measured by Numeric Rating scale on arrival and 30 minutes after drug administration. The adverse reactions were also recorded.
    Findings: 55 patients were entered. There was no significant difference in analgesic effect between the Intravenous Acetaminophen and morphine groups (P = 0.140), but, there was a significant difference in side effects between the two groups. (P = 0.014).
    Conclusion
    Intravenous acetaminophen appears to provide a similar level of analgesic comparable to intravenous morphine in limb fracture, however, further larger studies are required.

    Caffeine -a methylxanthine derived from purine- is found in the various plants (coffee, tea, cola) all over the world and is widely used in cosmetics due to its ability to penetrate in skin barriers. In this paper, the mechanism of broad effects of caffeine on skin and hair is discussed. Commercial use of caffeine in the formulation of cosmetics (e.g. sunscreen products) can effectively reduce Ultraviolet (UV) irradiation side effects and devastating UV induced free radicals. It inhibits phosphodiesterase activity, increases the concentration of cyclic adenosine monophosphate (cAMP) and augments apoptosis in damaged keratinocytes of skin. Caffeine inhibits cell cycle and induces apoptosis by inhibition of ataxia-telangiectasia mutated and Rad3-related (ATR), cycle ATR-chk function. On the other hand, Caffeine is topically used in shampoo as an adjuvant for hair loss treatment. Increasing the cAMP concentration caused by caffeine reduces the tension in smooth muscle near the hair follicle and leads to easier delivery of nutrients through blood vessels. It also prevents negative effects of testosterone on hair follicles in men. Most of currently used hair products contain caffeine. Detailed mechanisms of other effects of caffeine in this field are also discussed.
