صفحات 150-154مقدمهبررسی آسیب های ستون فقرات گردنی بخشی مهمی از ارزیابی بیماران ترومایی را به خود اختصاص می دهد. بر اساس منابع موجود بیش از 98 درصد از تصویر برداری های ستون فقرات گردنی یافته مثبتی به همراه نداشته است. لذا مطالعه ممیزی بالینی حاضر با هدف ارزیابی میزان انطباق درخواست های تصویربرداری گردن در بیماران مالتیپل تروما با دو معیار نکسوس و کانادایی صورت گرفته است.روش کاردر مطالعه ممیزی بالینی حاضر، به ارزیابی میزان انطباق درخواست های تصویربرداری گردن در بیماران مالتیپل ترومای مراجعه کننده به بخش اورژانس با معیارهای تصمیم گیری بالینی بر گرفته از دو مدل تایید شده نکسوس و کانادایی پرداخته شده است. در ابتدا طی یک مطالعه میدانی به میزان انطباق مذکور ارزیابی و در ادامه ضمن آنالیز نتایج مرحله اول و با توجه به میزان پایین انطباق اقدام به طراحی یک دوره آموزشی برای تمامی پزشکان مسوول رسیدگی به بیماران گردید. در نهایت نتایج قبل و بعد از آموزش با استفاده از نرم افزار آماری SPSS 21 مورد مقایسه قرار گرفت.یافته هادر مرحله قبل و بعد از آموزش به ترتیب برای 98 (82/62 درصد) و 85 (48/54 درصد) بیمارگرافی ساده گردن درخواست شده بود. میزان دقت درخواست گرافی های گردن بر اساس معیار های استاندارد بالینی موجود از 100 (1/64 درصد) مورد قبل از آموزش به 143 (7/91 درصد) مورد در بعد از آموزش ارتقاء یافته بود (P < 0.001). سطح زیر منحنی ROC از جهت میزان انطباق مذکور از (61-43 : %95 CI) 52 به (97-87 : %95 CI) 92 ارتقاء پیدا کرده بود.نتیجه گیریآموزش معیار های تصمیم گیری بالینی نکسوس و کانادایی نقش معنی داری در بهبود میزان انطباق درخواست های گرافی های گردن بیماران ترومایی با استاندارد های موجود داشت.کلیدواژگان: معیار های تصمیم گیری بالینی، رادیوگرافی، آسیب های ستون فقرات، سرویس اورژانس، بیمارستان، آسیب های گردن

صفحات 155-160مقدمهاز مهمترین مولفه هایی که بر کیفیت خدمات تاثیر گذار است میزان رضایت پرسنل از شغل، امکانات و شرایط رفاهی می باشد. اما تاکنون مطالعه ای در زمینه بررسی رضایت پرسنل اورژانس پیش بیمارستانی از شرایط و امکانات رفاهی آمبولانس و اعزام در ایران انجام نشده است. بر این مبنا پژوهش حاضر با هدف ارزیابی سطح رضایت پرسنل از شرایط و امکانات رفاهی آمبولانس اعزام تبیین گردید.روش کاردر پژوهش مقطعی حاضر 68 نفر از پرسنل اورژانس پیش بیمارستانی بیمارستان های استان خراسان شمالی، به طور تصادفی ساده انتخاب و سطح رضایتمندی آن بر اساس یک پرسشنامه 23 سوالی مورد ارزیابی قرار گرفت. سپس سطح رضایت نمونه های مورد بررسی به دو گروه مطلوب و نامطلوب تقسیم و یافته ها به صورت فراوانی و درصد گزارش گردید.یافته هاکمترین میزان رضایت از امکانات و تجهیزات آمبولانس در زمینه عدم دسترسی به پزشک (7/14 درصد)، وجود کمکی (0/27 درصد)، راحتی صندلی پرستار (8/29 درصد) و پوزیشن گرفتن تخت بیمار (3/32 درصد) گزارش گردید. حداقل رضایتمندی نیز در زمینه شرایط رفاهی در بخش زمان وصول حق الزحمه (4/4 درصد) و محل استراحت (9/5 درصد) مشاهده شد. بر اساس طبقه بندی انجام شده تنها 22 (4/32 درصد) فرد از امکانات اعزام و 5 (4/7 درصد) نفر از شرایط رفاهی اعزام رضایت داشتند.نتیجه گیرییافته های پژوهش حاضر نشان داد میزان رضایتمندی پرسنل اورژانس پیش بیمارستانی از امکانات و شرایط رفاهی آمبولانس پایین می باشد. تمامی عوامل تاثیر گذار بر کاهش سطح رضایتمندی پرسنل قابل رفع و پیشگیری بود لذا مداخله در این موارد می تواند باعث بهبود سطح رضایت پرسنل اعزام گردد.کلیدواژگان: رضایت شغلی، پرسنل اورژانس پیش بیمارستانی، آمبولانس

صفحات 161-168مقدمهدر حال حاضر اطلاع دقیقی از میزان تطابق دوره های بازآموزی رشته طب اورژانس با سرفصل های ضروری آموزشی این رشته وجود ندارد. لذا مطالعه حاضر در نظر دارد تا میزان تطابق بازآموزی های برگزار شده طی سال های 1389 تا 1393 با سرفصل های آموزشی رشته تخصصی طب اورژانس را مورد بررسی قرار داد.روش کاردر مطالعه مقطعی حاضر اطلاعات ثبت شده از بازآموزی های مرتبط با رشته تخصصی طب اورژانس از سامانه آموزش مداوم وزارت بهداشت جمع آوری گردید. سپس 30 آیتم از مهمترین مباحث آموزشی رشته طب اورژانس با نظر 3 متخصص طب اورژانس انتخاب شد و اقدام به ارزیابی تطابق بازآموزی ها از لحاظ مدت زمان آموزش، روند تغییرات سالیانه و کفایت با سرفصل های آموزشی رشته طب اورژانس گردید.یافته هاطی سال های 1389 تا 1393 به طور مجموع، 10147 ساعت بازآموزی مرتبط با سرفصل های آموزشی رشته طب اورژانس ارائه شده بود. بیشترین فراوانی (1015 ساعت) بازآموزی ها مرتبط با اورژانس های جراحی بود. در رتبه بعدی بازآموزی در زمینه اورژانس مسمومیت با میزان 810 ساعت (8/0درصد) اورژانس های نورولوژیک با میزان 700 ساعت (6/9 درصد)، اورژانس های اطفال با میزان 695 ساعت (6/8 درصد)، اورژانس های داخلی با 675 ساعت (6/7 درصد) و احیا قلبی ریوی با 555 ساعت (5/5 درصد) قرار داشتند. علاوه بر این مشخص گردید که هیچ یک از باز آموزی ها تطابق کامل با استانداردهای مورد انتظار ندارند. آزمون کاپای کوهن نشان داد میزان توافق بین وضعیت موجود بازآموزی ها با استاندارد تنها 46/7 درصد (با فاصله اطمینان 95 درصدی برابر 52/55-40/9) بود که در طبقه موافقت ضعیف قرار می گیرد (0/19=p).نتیجه گیرییافته های پژوهش حاضر حاکی از توافق ضیف باز آموزی های انجام شده در حوزه طب اورژانس با استانداردهای مورد انتظار بود.کلیدواژگان: بازآموزی، سرفصل های آموزشی، میزان تطابق

صفحات 169-173مقدمهتاکنون مطالعه ای با حجم نمونه قابل قبول به منظور کارایی استفاده از چک لیست های از پیش طراحی شده در جمع آوری داده های پزشکی بر بالین بیماران ترومایی در کشور انجام نشده است. بر این مبنا مطالعه حاضر با هدف بررسی تاثیر استفاده از چک لیست از پیش طراحی شده بر کیفیت ثبت شرح حال بیماران ترومایی مراجعه کننده به بخش اورژانس طراحی شد.روش کاردر مطالعه مقطعی آینده نگر حاضر ثبت داده های بیماران ترومایی مراجعه کننده به بخش اورژانس به صورت تصادفی به دو روش استفاده از چک لیست و بدون استفاده از آن انجام پذیرفت. چک لیست طراحی شده حاوی قسمت های اطلاعات فردی، مکانیسم تروما، عمق تشریحی و شدت تروما، اندازه ضایعات، محل ضایعات و برنامه تشخیصی و درمانی بیمار بود. وضعیت تکمیل داده ها در هر یک از آیتم های مورد بررسی به سه طبقه ثبت کامل، ثبت ناقص و عدم ثبت تقسیم گردید. مقایسه وضعیت تکمیل داده ها بین دو گروه، با استفاده از آزمون کای دو یا تست دقیق فیشر انجام شد. در تمامی آنالیزها 05/0>p به عنوان سطح معنی داری تعریف شد.یافته هادر پژوهش حاضر داده های 795 بیمار بدون استفاده از فرم و 384 بیمار با استفاده از آن جمع آوری شد. ثبت کامل و دقیق داده ها در تمامی آیتم های مورد مطالعه در گروه استفاده کننده از چک لیست بیشتر بود (001/0>p). ثبت محل ضایعه بدون استفاده از فرم تنها در 8/20 درصد موارد به طور صحیح و کامل ثبت شد. این میزان با استفاده از فرم به 6/95 درصد افزایش یافت. همچنین ثبت کامل و دقیق برنامه تشخیصی از 9 درصد به 8/72 درصد و ثبت کامل و دقیق برنامه درمانی از 14 درصد به 7/68 درصد رسید. این بهبود در وضعیت ثبت داده بیماران در تمامی موارد معنی دار بود.نتیجه گیریبه نظر می رسد استفاده از چک لیست های از پیش طراحی شده در جمع آوری اطلاعات بالینی بیماران ترومایی، باعث بهبود چشمگیر ثبت داده ها و کیفیت شرح حال گیری می گردد.کلیدواژگان: تروما، پرونده پزشکی، خطای پزشکی، چک لیست

دقت تشخیصی قطر ورید اجوف تحتانی، شریان آئورت و ورید ژوگولار داخلی در تخمین میزان دهیدراتاسیون کودکانصفحات 174-181مقدمهارزیابی حجم داخل عروقی موضوعی مهم اما پیچیده در مدیریت بیماران بدحال به ویژه در کودکان می باشد. تکنیک های تهاجمی هرچند قادرند تخمین بسیار دقیقی از حجم داخل عروقی ارائه نمایند اما عوارض ناشی از آن بسیار خطرناک می باشد. لذا مطالعه حاضر با هدف مقایسه دقت تشخیصی قطر سونوگرافیک ورید اجوف تحتانی، شریان آئورت، قطر ورید ژوگولار داخلی و نسبت قطر ورید اجوف به شریان آئورت در تعیین وضعیت دهیدراتاسیون کودکان مراجعه کننده به بخش اورژانس طراحی شده است.روش کارمطالعه مقطعی حاضر با هدف تعیین ارزش تشخیصی قطر سونوگرافیک وریدهای اجوف تحتانی و ژوگولار و شریان آئورت در تخمین میزان دهیدراتاسیون بر روی کودکان مراجعه کننده به بخش اورژانس مبتلا به دهیدراتاسیون خفیف و متوسط انجام شد. از محاسبه شاخص های غربالگری شامل سطح زیر منحنی، حساسیت و ویژگی برای نیل به هدف مذکور بهره گرفته شد. داده ها توسط نرم افزار آماری STATA 11.0 تجزیه تحلیل گردید و سطح معنی داری معادل 05/0 در نظر گرفته شد.یافته هادر نهایت 54 بیمار وارد مطالعه شدند (4/57 درصد پسر، میانگین سنی برابر 7/2±9/4 سال). سطح زیر منحنی قطر ورید اجوف تحتانی در تشخیص دهیدارتاسیون متوسط در نمای ساژیتال و عرضی به ترتیب برابر 775/0 (91/0-65/0:CI 95%) و 8086/0 (93/0-69/0:CI 95%) بود. همچنین قطر شریان آئورت نیز در تشخیص دهیدراتاسیون متوسط در نمای ساژیتال و عرضی دارای سطح زیر منحنی برابر 658/0 (81/0-51/0: CI 95%) و 7126/0 (86/0-57/0:CI 95%) بود. قطر ورید ژوگولار نیز دارای سطح زیر منحنی برابر 7332/0 (88/0-59/0: CI 95%) بود. مقایسه سطح زیر منحنی پارامترهای مورد مطالعه نشان داد قطر ورید اجوف تحتانی در نمای ساژیتال (004/0=p) و عرضی (001/0>p) شاخص بهتری برای تشخیص دهیدراتاسیون متوسط می باشد.نتیجه گیریبر اساس یافته های پژوهش حاضر به نظر می رسد که قطر ورید ژوگلار، ورید اجوف تحتانی و شریان آئورت معیار چندان دقیقی برای غربالگری وضعیت دهیدراتاسیون کودکان نباشد چرا که حتی در مورد قطر سونوگرافیک ورید اجوف تحتانی که نسبت به سایر پارامترها به طور معنی داری دقت بیشتری داشت حساسیت 48/81 درصدی و ویژگی 15/48 در تمایز دهیدراتاسیون متوسط از خفیف بدست آمد.کلیدواژگان: حجم داخل عروقی، دهیدارتاسیون، تشخیص، اولترا سونوگرافی

Pages 150-154IntroductionEvaluation of cervical spine injuries makes up a major part of trauma patient assessments. Based on the existing sources, more than 98% of the cervical spine X-rays show no positive findings. Therefore, the present clinical audit aimed to evaluate the correlation of ordered cervical spine X-rays in multiple trauma patients with NEXUS and Canadian c-spine clinical decision rules.MethodsThe present clinical audit, evaluated the correlation of cervical spine imaging orders in multiple trauma patients presented to the emergency department, with NEXUS and Canadian c-spine rules. Initially, in a pilot study, the mentioned correlation was evaluated, and afterwards the results of this phase was analyzed. Since the correlation was low, an educational training was planned for all the physicians in charge. Finally, the calculated correlations for before and after training were compared using SPSS version 21.ResultsBefore and after training, cervical spine X-ray was ordered for 98 (62.82%) and 85 (54.48%) patients, respectively. Accuracy of cervical spine X-ray orders, based on the standard clinical decision rules, increased from 100 (64.1%) cases before training, to 143 (91.7%) cases after training (pConclusionTeaching NEXUS and Canadian c-spine clinical decision rules plays a significant role in improving the correlation of cervical spine X-ray orders in multiple trauma patients with the existing standards.Keywords: Decision support techniques, radiography, spinal injuries, emergency service, hospital, neck injuries

Pages 155-160IntroductionPersonnel job satisfaction, facilities and welfare are among the most important factors affecting quality of service. To date, no study has been done in the field of Emergency medical service (EMS) personnel satisfaction regarding facilities and welfare in Iran. Therefore, the present study was designed to evaluate the level of satisfaction among EMS personnel regarding facilities and welfare.MethodsIn the present cross-sectional study, 68 of the EMS personnel of Northern Khorasan, Iran, were selected by simple randomization and their level of satisfaction was evaluated using a 23-question questionnaire. Afterwards, their satisfaction level was divided into 2 groups of desirable and non-desirable and the findings were reported as frequency and percentage.ResultsThe lowest satisfaction level regarding facilities were reported to be linked to the physicians being out of reach (14.7%), presence of help (27.0%), comfortableness of nurses chair (29.8%), and ability of patients bed to be positioned (32.3%). Minimum satisfaction regarding welfare was also seen concerning the time of receiving their wage (4.4%) and place for rest (5.9%). Based on the classifications done, only 22 (32.4%) of the personnel were satisfied with the facilities and 5 (7.4%) were satisfied with the welfare conditions.ConclusionThe findings of the present study showed that satisfaction rate of the EMS personnel regarding facilities and welfare is low. All the factors leading to this decrease in satisfaction level of the personnel could be prevented and relieved, therefore intervention in these regards can improve satisfaction rate among the personnel.Keywords: Job satisfaction, emergency medical services, ambulances, health planning

Pages 161-168IntroductionCurrently, there is a lack of data on the agreement between the continuous medical education (CME) programs and the necessary training topics in the field of emergency medicine in Iran. The present study aimed to evaluate the mentioned agreement in CME programs held during 2010-2014, all over the country.MethodsIn the present cross-sectional study, the data recorded regarding emergency medicine-related CME programs were extracted from continuous training database of the Ministry of Health. Then, 30 items from the most important topics taught in emergency medicine training were selected by 3 emergency medicine specialists. Subsequently, the agreement of the selected training topics and CME programs was evaluated regarding duration of training, annual changes, and efficiency.ResultsDuring 2010-2014, a total of 10147 hours of CME programs were held in relation to emergency medicine topics in Iran, which were most frequently (1015 hours) related to surgical emergencies. The next most frequent emergency-related CME programs were in the fields of toxicology with 810 hours (8.0%), neurology with 700 hours (6.9%), pediatric emergencies with 695 hours (6.8%), internal medicine with 675 hours (6.7%), and cardiopulmonary resuscitation with 555 hours (5.5%). In addition, it was determined that none of the CME programs were in complete agreement with the expected standards. Cohens kappa test showed that the agreement between the current conditions of CME programs with the standard was only 46.7% (95% confidence interval: 40.9 52.55) which shows a weak agreement (p = 0.19).ConclusionThe findings of the present study show the weak agreement of the CME programs held in the field of emergency medicine with the expected standards.Keywords: Education, medical, continuing, emergency medicine

Pages 169-173IntroductionTo date, no study with a proper sample size has been done to evaluate the efficiency of using pre-designed checklists in history taking of trauma patients in Iran. Therefore, the present study was designed to evaluate the pre-designed checklists effects on the quality of history taking in trauma patients referred to the emergency department (ED).MethodsIn the present prospective cross-sectional study, data of the trauma patients presented to the ED were recorded either with or without using a checklist, randomly. The designed checklist consisted of personal data, trauma mechanism, wound characteristics and trauma severity, severity and location of injuries, and the patients diagnostic and therapeutic plans. The quality of data gathering in each evaluated item was divided into 3 groups of complete recording, incomplete recording, and not recorded. Data recording quality was compared between the 2 groups using chi square or exact Fishers test. In all analyses, pResultsIn the preset study, patient data were gathered without using a checklist in 795 and using a checklist in 384 cases. Pages 174-181IntroductionEvaluating intravascular volume is an important but complicated matter in management of critically ill patients, especially in children. Although invasive techniques have the ability to accurately estimate the intravascular volume, but they have dangerous side effects. Therefore, the present study was designed with the aim of comparing the diagnostic accuracy of sonographic diameters of inferior vena cava (IVC), aorta, internal jugular vein (IJV), and IVC/aorta ratio in identifying pediatric dehydration in children presented to the emergency department (ED).MethodsThe present prospective cross-sectional study was carried out with the aim of determining the diagnostic accuracy of sonographic diameters of IVC, IJV, and aorta, in estimation of dehydration rate for children presented to the ED with mild to moderate dehydration. Their screening performance characteristics, such as area under the ROC curve, sensitivity and specificity, were calculated and used for this purpose. The data were analyzed using STATA 11.0 and 0.05 was considered as significance level.ResultsIn the end, 54 patients were enrolled in the study (57.4% male, mean age of 4.9 ± 2.7 years). Area under the ROC curve for IVC in diagnosis of moderate dehydration in sagittal and transverse planes were 0.775 (95% CI: 0.65 0.91) and 0.8086 (95%CI: 0.96 0.93), respectively. In addition, the diameter of aorta in this regard were 0.658 (95%CI: 0.51 0.81) for the sagittal and 0.7126 (95% CI: 0.57 0.86) for the transverse plane. IJV diameter had an area under the curve of 0.7332 (95% CI: 0.59 0.88). Comparing the area under the ROC curves for the studied parameters showed that IVC diameter in the sagittal (p = 0.004) and transverse (pConclusionBased on the findings of the present study, it seems that IJV, IVC, and aorta diameters are not very accurate for determining the condition of pediatric dehydration, since even sonographic IVC diameter, which was more accurate than the other parameters, had a sensitivity of 81.48% and specificity of 48.15% in differentiating mild and moderate dehydration.Keywords: Dehydration, diagnosis, ultrasonography, aorta, jugular veins, vena cava, inferior

Pages 185-186In the previous two parts of educational manuscript series in Emergency, we explained some screening characteristics of diagnostic tests including accuracy, sensitivity, specificity, and positive and negative predictive values. In the 3rd part we aimed to explain positive and negative likelihood ratio (LR) as one of the most reliable performance measures of a diagnostic test. To better understand this characteristic of a test, it is first necessary to fully understand the concept of sensitivity and specificity. So we strongly advise you to review the 1st part of this series again. In short, the likelihood ratios are about the percentage of people with and without a disease but having the same test result. The prevalence of a disease can directly influence screening characteristics of a diagnostic test, especially its sensitivity and specificity. Trying to eliminate this effect, LR was developed. Pre-test probability of a disease multiplied by positive or negative LR can estimate post-test probability. Therefore, LR is the most important characteristic of a test to rule out or rule in a diagnosis. A positive likelihood ratio > 1 means higher probability of the disease to be present in a patient with a positive test. The further from 1, either higher or lower, the stronger the evidence to rule in or rule out the disease, respectively. It is obvious that tests with LR close to one are less practical. On the other hand, LR further from one will have more value for application in medicine. Usually tests with 0.1 10 are considered suitable for implication in routine practice.Keywords: Positive Likelihood Ratio, Negative Likelihood Ratio
