    کلستریدیوم دیفیسیل عامل مهم گسترش بیماری های گوارشی به دلیل مصرف آنتی بیوتیک و پیدایش مقاومت میکروبی می باشد. در گذشته، این باکتری تنها به عنوان عامل عفونت در بیمارستان معرفی شد. اما در اپیدمیولوژی عفونت تغییراتی مشاهده گردید و سویه های هایپرویرولانت کلستریدیوم دیفیسیل در خارج از محیط های درمانی مانند حیوانات و فراورده های غذایی شناسایی شد. با این وجود، این باکتری به عنوان پاتوژن مشترک انسان و دام یا غذازاد معرفی نشده است. اما افراد حساس مانند زنان باردار، سالمندان و کودکان ممکن است با مصرف فراورده های غذایی آلوده، کلستریدیوم دیفیسیل وارد بدنشان شود و شرایط ابتلا به عفونت در آن ها مهیا گردد. بنابراین تدوین گایدلاین های پیشگیری از انتقال باکتری به انسان در جامعه مانند محیط های بالینی اهمیت پیدا می کند. در این مقاله مروری، تاریخچه، عوامل خطر و پژوهش های صورت گرفته بر روی شیوع کلستریدیوم دیفیسیل در محیط های بالینی و خارج از آن در ایران بررسی می شود. در پایان فرضیاتی مطرح می شود که به دلیل عدم تشابه در پروفایل ریبوتایپ و ساختار ژنتیکی کلستریدیوم دیفیسیل در ایران با سویه های بین المللی، سویه های بومی در کشورمان وجود دارد که می توانند در بروز عفونت این باکتری در بالین و جامعه نقش داشته باشند. همچنین با توجه به گزارشات موجود از مصرف غیرمنطقی آنتی بیوتیک ها در کشورمان ایران، به نظر می رسد عفونت ناشی از کلستریدیوم دیفیسیل رو به افزایش باشد. اثبات این فرضیات نیازمند ارزیابی مداوم و دقیق بیماری و بررسی جامع مولکولی اپیدمیولوژی در محیط های درمانی و خارج از آن توسط سازمان های متولی سلامت می باشد.

    زمینه و هدف
    ذرات نانوهیدروکسی آپاتیت با داشتن سطح تماس بیشتر وحلالیت بالاتر نسبت به هیدروکسی آپاتیت معمولی، مورد توجه پژوهشگران به عنوان یک پیوند موثر و جدید استخوانی قرار گرفته اند. مطالعات نشان داده اند که تناقض هایی در زمینه سازگاری زیستی این ذرات وجود دارد. هدف از مطالعه بررسی فعالیت حیاتی سلول اپی تلیوم دهان انسان در مجاورت با ذرات نانوهیدروکسی آپاتیت بود.
    روش بررسی
    مطالعه به صورت تجربی در آبان 1392 انجام شد. غلظت های مختلف نانوهیدروکسی آپاتیت از 01/0 تا mg/ml 5 در 24، 48 و 72 ساعت بر سلول های اپی تلیال دهان انسان رده KB در محیط کشت تاثیر داده شد. میزان سمیت سلولی این ماده با روش متیل تیازول تترازولیوم بروماید و به وسیله دستگاه الیزا ریدر (Falcon، Becton-Dickinson، USA) بررسی شد.
    یافته ها
    سمیت سلولی غلظت های مختلف نانوهیدروکسی آپاتیت در 24 ساعت (001/0P<)، 48 ساعت (001/0P<) و 72 ساعت (001/0P<) بر سلول های اپی تلیال دهان انسان، اختلاف معنادار داشت. ذرات نانوهیدروکسی آپاتیت در غلظت 5/0 تا mg/ml 1 در زمان های 24، 48 و 72 ساعت بیشترین سمیت سلولی را بر سلول های اپی تلیوم دهان انسان داشتند. در تمامی زمان ها، غلظت های کمتر از mg/ml 05/0 بهترین سازگاری زیستی را داشتند.
    نتیجه گیری
    نانوهیدروکسی آپاتیت سازگاری زیستی خوبی دارد و به دوز و زمان وابسته است. غلظت های کمتر از mg/ml 05/0 از نانوهیدروکسی آپاتیت بهترین سازگاری زیستی را با گذشت زمان دارند.

    زمینه و هدف
    تلومراز با افزودن توالی تکراری به انتهای 3 کروموزوم نقش بسیار مهمی در حفظ طول تلومر دارد. از طرف دیگر، سلول های بنیادی مزانشیمی بند ناف انسانی (hUC-MSCs) قابلیت تمایز به بیشتر رده های سلولی مانند سلول های عصبی و قلبی را داشته و دچار تغییرات بیولوژیکی در کشت طولانی مدت می شوند. از این رو انتخاب پاساژ سلولی مناسب می تواند در بررسی های تکثیر و تمایز سلولی بسیار مهم باشد. هدف از پژوهش کنونی بررسی ارتباط بین فعالیت آنزیم تلومراز در پاساژهای مختلف کشت hUC-MSCs بود.
    روش بررسی
    این مطالعه تجربی از فروردین 1393 تا آذر 1393 در دانشگاه علوم پزشکی اردبیل بر روی نمونه های بندناف نوزادان تازه متولد شده در بیمارستان علوی اردبیل انجام شد. قطعات کوچکی از بندناف در محیط کشت DMEM حاوی 20% سرم جنین گاو کشت داده شدند. سپس، hUC-MSC حاصل از پاساژهای یک تا سه برای بررسی زمان دو برابر شدن جمعیت سلولی (PDT) و استخراج پروتئین به روش CHAPS انتخاب شدند. در نهایت، فعالیت آنزیم تلومرازی آن ها در پاساژهای مختلف با استفاده از روش های Telomeric repeat amplification protocol (TRAP) و qRT-TRAP assay بررسی شد.
    یافته ها
    PDT در پاساژهای یک تا سه به ترتیب 92/1±68/54، 71/1±03/55 و 54/2±41/69 بود. میانگین فعالیت تلومرازی نیز به ترتیب 51/0±58/30، 74/0±24/27 و 75/0±13/32 برآورد شد که افزایش معناداری را در پاساژ دوم نشان داد (021/0P=).
    نتیجه گیری
    با رسم منحنی رشد، تعیین PDT و سنجش فعالیت تلومرازی سلول های بنیادی مزانشیمی بند ناف انسانی مشخص شد که کشت طولانی مدت می تواند بر فرایند تکثیر سلولی و فعالیت تلومرازی تاثیر بگذارد.

    زمینه و هدف
    استفاده از آنزیم های مرحله استرس اکسیداتیو روشی غیرتهاجمی در پیش بینی پاتولوژی کبد در مبتلایان به کبد چرب غیرالکلی است. پژوهش کنونی با هدف بررسی ارتباط سطح سرمی روی در بیماران مبتلا به کبد چرب غیرالکلی (NAFLD) با یافته های پاتولوژی کبد انجام شد.
    روش بررسی
    مطالعه به صورت کوهورت همراه با گروه کنترل در بیماران مبتلا به کبد چرب غیرالکلی مراجعه کننده به درمانگاه گوارش بیمارستان سینا، تهران در مدت یک سال (فروردین 1393 تا فروردین 1394) انجام شد. افرادی که فاقد بیماری در بالین و سونوگرافی کبدی و آزمایشات طبیعی بودند به عنوان گروه کنترل وارد مطالعه شدند. در افراد شرکت کننده سطح سرمی روی با روش اسپکترومتری جذب اتمی مورد بررسی قرار گرفت. سطح طبیعی روی μmol/L 9/22- 7/10 (μg/dL 70-150) بوده و سطوح کمتر از (μg/dL 46) μg/dl 7 به عنوان کمبود روی در نظر گرفته شد.
    یافته ها
    دو گروه 60 نفری در این مطالعه شرکت کردند. در گروه مورد، شش نفر و در گروه شاهد 26 نفر به دلیل عدم رضایت به انجام آزمایشات حذف شدند. در نهایت 54 نفر در گروه مورد (35 مرد، 19 زن) و 34 نفر در گروه شاهد (22 مرد، 12 زن) در بررسی نهایی شرکت داده شدند. میانگین سنی بیماران در گروه مورد 82/9±02/37 سال و در گروه شاهد 01/12±24/33 سال بود (111/0P=). اختلاف آماری معناداری بین دو گروه از نظر سطح روی وجود نداشت (50/0P=).
    نتیجه گیری
    یافته های به دست آمده نشان داد که ارتباط آماری معناداری میان سطح سرمی روی و استئاتوز، التهاب لوبولار و فیبروز کبد در مبتلایان به کبد چرب غیرالکلی وجود ندارد.

    زمینه و هدف
    از آنجایی که میانکنش HLA-E بیان شده در سطح سلول های هدف با رسپتور مهاری CD94/NKG2 سلول های NK نقش مهمی را در تنظیم ایمنی ذاتی علیه عوامل پاتوژن ایفا می کند، بیان متفاوت مولکول HLA-E در سطح سلول در نحوه مقاومت میزبان به ویروس ها و همچنین پاسخ به درمان واجد اهمیت می باشد. بنابراین پژوهش کنونی با هدف بررسی فراوانی ژنوتیپ های مختلف مولکول HLA-E در بیماران تالاسمی ماژور که اغلب در معرض خطر عفونت های منتقله از راه انتقال خون می باشند، انجام گردید.
    روش بررسی
    پژوهش انجام شده از نوع مقطعی بوده که نمونه گیری به صورت تصادفی بر روی 104 بیمار تالاسمی ماژور مراجعه کننده به درمانگاه تالاسمی بزرگ سالان انتقال خون تهران بین سال های 1392 تا 1394 انجام شد. DNA استخراج شده از نمونه های خون بیماران با روش واکنش زنجیره پلیمراز- پرایمر مختص سکوانس (Sequence-Specific Primer Polymerase Chain Reaction، SSP-PCR) مورد آنالیز قرار گرفت. همچنین جهت تایید نتایج، محصولات از نظر تعیین توالی DNA بررسی گردیدند.
    یافته ها
    از مجموع بیماران، 49 مرد با فراوانی نسبی 1/47% و 55 زن با فراوانی نسبی 9/52% بودند. بیماران در محدوده سنی 16 تا 43 سال با میانگین سنی 03/31 و انحراف معیار 7/4 قرار داشتند. فراوانی نسبی ژنوتیپ HLA-E*01010103 (4/%64) به طور معناداری (001/0P=) بیشتر از ژنوتیپ های HLA-E*01010101 (4/%15) و HLA-E*01030103 (2/%20) بوده در حالی که تفاوتی بین فراوانی نسبی آلل های HLA-E*0103 (4/%52) و HLA-E*0101 (6/%47) وجود نداشت.
    نتیجه گیری
    فراوانی ژنوتیپ HLA-E*01010103 به صورت معناداری بیشتر از سایر ژنوتیپ های HLA-E بوده در حالی که بین فراوانی آلل های HLA-E*0103 و HLA-E*0101 تفاوت معناداری وجود نداشت.

    زمینه و هدف
    سوختگی و عوارض آن در حال حاضر به عنوان یکی از مشکلات مهم اجتماعی و فرهنگی در جامعه مطرح می باشد. این پژوهش به منظور تعیین اپیدمیولوژی و پیامد سوختگی در بیماران دچار سوختگی در بیمارستان مطهری تهران انجام شد.
    روش بررسی
    این مطالعه از نوع توصیفی است که طی مدت یک سال بر روی 200 بیمار سوختگی از بخش اورژانس بیمارستان سوانح سوختگی مطهری انجام شد. در هفته اول از زخم سوختگی پس از تمیز کردن محل با نرمال سالین با سواب استریل جمع آوری شد. نمونه ها در بلاد آگار و مک کانکی آگار کشت داده شدند. پس از انکوباسیون در °C 37 به مدت 48 ساعت، شناسایی با توجه به تست های استاندارد معمولی بیوشیمیایی انجام شد. درمان بیماران سوختگی نیز بر اساس ترمیم و اپی تلیالیزه شدن سطح زخم سوختگی و نتایج حاصل از کشت میکروبی در پرسش نامه بیمار ثبت شد.
    یافته ها
    مایعات داغ با 57% و مایعات مشتعل شونده با 21% بیشترین موارد سوختگی را شامل می شدند. همچنین سن 21-30 سال در جنس مرد با 5/17% و در جنس زن با 7% بیشترین موارد سوختگی را در بر می گرفتند. باکتری های گرم منفی با 7/85% مهمترین علل عامل میکروبی و در میان آن ها، سودوموناس با 5/37% مهمترین عامل عفونت در سوختگی بود.
    نتیجه گیری
    بیشترین علت سوختگی در هر دو جنس مایعات داغ بود. مردان در معرض خطر بیشتری بودند و این ممکن است به دلیل این واقعیت باشد که مردان بیشتر در معرض کارهای پرخطر از زن ها هستند.

    زمینه و هدف
    دیسپلازی تکاملی مفصل هیپ (Developmental dysplasia of the hip، DDH) طیفی از اختلالات وابسته به تکامل غیرطبیعی هیپ را در برمی گیرد که ممکن است در هر زمانی (دوره جنینی، شیرخوارگی و یا کودکی) نمایان گردد. هدف از این مطالعه بررسی نتایج بالینی و رادیوگرافی بیماران مبتلا به دیسپلازی تکاملی هیپ پس از درمان با جااندازی باز، کپسولورافی و استئوتومی به روش پمبرتون می باشد.
    روش بررسی
    مطالعه به صورت آینده نگر بر روی 13 کودک مبتلا به دیسپلازی تکاملی هیپ در بیمارستان رازی اهواز در سال های 93-1391 انجام شد. معیارهای ورود شامل محدوده سنی 18 ماه تا هشت سال و شاخص استابولار بالاتر یا مساوی 40 درجه بودند. معیارهای خروج شامل بیماری های بافت همبند، دررفتگی ثانویه به علت عفونت پیشین و دیسپلازی استابولوم در زمینه سندرم خاص بودند. بیماران پیش از جراحی و حداقل یک سال پس از عمل به لحاظ بالینی، میزان Dislocation یا Subluxation مفصل هیپ، Congruity مفصل هیپ و یافته های رادیوگرافی به ترتیب بر اساس معیارهای McKay، Tonnis grading، Severin و شاخص استابولار مورد ارزیابی قرار گرفتند.
    یافته ها
    شاخص استابولار پس از عمل کاهش معناداری در مقایسه با پیش از عمل داشت (002/0P=). بر اساس تقسیم بندی Tonnis grading، پس از عمل 6/91% بیماران در کلاس I-II قرار داشتند. بر اساس معیار ارزیابی McKay، 6/%66 بیماران نتایج عالی و خوب را پس از عمل نشان دادند. در بررسی یافته های رادیوگرافی پس از عمل بر اساس معیار Severin، 2/%83 بیماران در کلاس I-III قرار گرفتند. معیارهای ارزیابی Tonnis grading، McKay و Severin بهبود چشمگیری از نظر آماری را در بیماران نسبت به پیش از عمل نشان دادند.
    نتیجه گیری
    استئوتومی پمبرتون می تواند یک روش موثر در درمان بیماران مبتلا به دیسپلازی تکاملی هیپ باشد که در سنین بالا و با تاخیر مراجعه می کنند و می تواند همراه با سایر روش های جراحی انجام شود.

    زمینه و هدف
    استئومیلیت حاد یکی از اورژانس های طب اطفال می باشد که تاخیر در تشخیص و درمان مناسب می تواند منجر به عواقب ناگواری برای بیمار شود. از آنجایی که توجه به نکات تشخیصی و درمانی پرونده بیماران بستری شده می تواند سبب جلوگیری از اشتباهات تشخیصی و درمان های نادرست شود، بنابراین پژوهش کنونی با هدف بررسی مشخصات بالینی و آزمایشگاهی و پاسخ به درمان در کودکان مبتلا به استئومیلیت حاد انجام گردید.
    روش بررسی
    در این پژوهش مقطعی گذشته نگر، 54 بیمار 15 روزه تا 15 ساله که طی سال های 1375 تا 1388 با تشخیص استئومیلیت حاد در بخش کودکان بیمارستان امام خمینی (ره) بستری شده بودند، مورد بررسی قرار گرفتند. بیماران با کمبود داده های پرونده از مطالعه خارج شده و در مجموع 54 کودک وارد این مطالعه شدند. پرسش نامه ای شامل نشانه های بالینی، یافته های پاراکلینیک و پاسخ به درمان توسط مجری طرح از پرونده بیماران تکمیل گردید.
    یافته ها
    به طور کلی 35 نفر از کودکان پسر و 19 نفر دختر بودند. در این پژوهش شایع ترین علامت درد و شایع ترین نشانه تورم بود که به ترتیب در 25 و 48 نفر دیده شد. مقایسه فراوانی تب در میان دو گروه سنی کمتر از دو سال و 15-2 سال نشان دهنده اختلاف آماری معنادار بین آن ها بود (3/%14 در مقابل 8/84%) (001/0P=). شایع ترین ارگانیسم جدا شده استافیلوکوک اورئوس (5/%73) بود. متوسط طول مدت درمان با آنتی بیوتیک داخل وریدی 1/28 روز بود.
    نتیجه گیری
    بیش از نیمی از کودکان مبتلا به استئومیلیت حاد به درمان با آنتی بیوتیک پاسخ نداده و نیاز به عمل جراحی پیدا کردند.

    زمینه و هدف
    بررسی علل لغو اعمال جراحی و تلاش برای حذف و کاهش آن ها موجب استفاده بهینه از منابع در ارایه خدمات جراحی می شود.
    روش بررسی
    پژوهش کنونی یک مطالعه اقدام پژوهی مشارکتی بوده که از فروردین تا اسفند سال 1392 در بخش اتاق عمل بیمارستان شهید رجایی تهران انجام شد. با استفاده از یک مدل مدیریت کیفیت، فرایندهای کاری بخش به منظور کاهش لغو اعمال جراحی استاندارد و ارتقا یافتند.
    یافته ها
    دلایل مربوط به جراح، ویزیت بیهوشی، عدم اتمام به موقع جراحی پیشین و نبود تخت در بخش مراقبت های ویژه از دلایل مهم لغو اعمال جراحی بودند. لغو اعمال جراحی با اجرای مدیریت کیفیت به میزان 4/32% کاهش یافت. استانداردسازی فرایندهای کاری، شناسایی مشکلات، برنامه ریزی صحیح، کار تیمی و فعال نمودن کلینیک های بیهوشی پیش از عمل جراحی از جمله اقدامات انجام شده برای کاهش لغو اعمال جراحی بوده است.
    نتیجه گیری
    به کارگیری یک مدل مناسب مدیریت کیفیت و اجرای درست آن منجر به افزایش بهره وری بخش اعمال جراحی بیمارستان می شود.

    زمینه و هدف
    تشخیص دقیق و زودهنگام بیماری ها به ویژه سرطان، به خصوص زمانی که بیماری هنوز ایجاد علامت نکرده و فرد احساس ناخوشی ندارد، مهم تر و آسان تر از درمان بیماری در مراحل پیشرفته و توام با ظهور عوارض است.
    روش بررسی
    گرماسنجی روبشی تفاضلی، روشی انتخابی جهت بررسی خواص ترمودینامیکی واسرشته شدن پروتئین ها و استفاده از آن در شناسایی و تفکیک پروتئین های پلاسمایی می باشد. مایعات زیستی مانند خون، مایع مغزی- نخاعی، ادرار، مغز استخوان و بزاق حاوی داده های ارزشمندی از نوع و میزان پیشرفت بیماری می باشد.
    یافته ها
    نتایج حاکی از آن است که نمودار حرارتی حاصل از بیماری ها و سرطان ها در مراحل و شدت های مختلف، شکل های متفاوت و منحصر به فردی از خود بروز می نمایند که می تواند در تشخیص اولیه و زودهنگام بیماری ها از آن بهره برد.
    نتیجه گیری
    در این مقاله گرماسنجی روبشی دمایی، به عنوان یک روش جدید و حساس برای شناسایی نشانگرهای زیستی پروتئینی و در نهایت تشخیص بعضی بیماری ها معرفی می گردد.

    Clostridium difficile (C. difficile) is an important factor in the development of the gastrointestinal diseases because of irrational antibiotic prescription and antimicrobial resistance. In the past, this bacterium was introduced as an agent of the infection in the hospitals called "hospital acquired Clostridium difficile infection". This infection is a main cause of morbidity and mortality internationally. But changing in the epidemiology of the infection was observed in recent years. People not taking antibiotics as well as any contact with the clinical system were hospitalized due to the infection named "Community-Associated Clostridium difficile infection". Furthermore, the hypervirulent strains of C. difficile were identified outside of the health care facilities in different sources such as environment, animals and food products. Today the role of C. difficile has not been confirmed as a zoonotic agent or foodborne pathogen. Taking into account, it should be taken attention to the sensitive individuals such as pregnant women, elderly and children for the consumption of the contaminated food products with C. difficile spores and probable cause of the infection in these individuals. For this purpose, presentation of the guidelines or the prevention strategies for the transmission of bacteria in the society as well as the healthcare facilities is important. In this review study, the history, the risk factors of disease and the reports of infection in the healthcare facilities and outside of this environment in Iran were discussed. Finally, we supposed that based on the isolation of C. difficile with different genetic profile in Iran in comparison with international ribotypes, the existence of native strains leading to the infection in the community and the healthcare facilities is possible. This hypothesis shows the significance of regional differences in the epidemiology and microbiology of disease. In addition, according to the present reports on the irrational prescription of the antibiotics in our country, it seems that C. difficile infection is increasing but any continuous monitoring is not being occurred for the supervision in Iran. Approving these hypotheses need to the careful and continuous assessment besides comprehensive examination of molecular epidemiology of disease in the organizations related to the health in Iran.

    Background
    Hydroxyapatite nanoparticles have a more surface contact and solubility than conventional hydroxyapatite. Hydroxynanoparticles enhances the biological and mechanical properties of new regenerated tissues. The hydroxyapatite nanoparticles have received attention as a new and effective osseous graft for using as scaffolds in bone regeneration. The reports on hydroxyapatite nanoparticles biocompatibility are controversial. It has been shown that hydroxyapatite nanoparticles induces inflammatory reaction and apoptosis. The aim of the present study was to evaluate the cytotoxicity of nano-hydroxyapatite on the human epithelial cells.
    Methods
    The study was experimental and completed in vitro. The study was carried out in department of Immonulogy, Faculty of Medicine, Shahid Beheshti University of Medical Sciences in November 2014. The human-derived oral epithelium cell line (KB) obtained from Pasteur Institute, Tehran, Iran were exposed to hydroxyapatite nanoparticles at 0.01, 0.05, 0.1, 0.5, 0.75, 1, 2.5 and 5 mg/ml concentrations in 24, 48 and 72 hours. Rod-shaped hydroxyapatite nanoparticles with 99% purity and maximum 100 nm sized particles were used. Methylthiazol tetrazolium bromide (MTT) method was employed for cell vitality evaluation. Enzyme-linked immunosorbent assay (ELISA) was used for assessing the viability of cells. Distilled water and fetal bovine serum (FBS) were positive and negative controls. ANOVA and Duncan tests were used for statistical analysis.

    Conclusion
    Hydroxyapatite nanoparticles had a good biocompatibility. The biocompatibility of hydroxyapatite nanoparticles were dose and time dependent. The lower concentrations than 0.05 mg/ml of nano-hydroxyapatite had the best biocompatibility over time.

    Background
    Telomerase as an enzyme with reverse transcriptase activity has an essential role in telomere maintenance by adding a telomere repeat sequence to the 3' end of chromosome and is important for regulating of many processes in embryonic development including cell proliferation and differentiation. Human umbilical cord-derived mesenchymal stem cells (hUC-MSCs) with a self-renewal capacity are cells that can differentiate into various germ layer derivatives including neural cells and cardiomyocytes, and undergo biological changes during long-term cultivation. Hence, the passage number in which the cells expanded seems to be very important for proliferating and differentiating. This study was aimed at investigating the relationship between the telomerase activity and the growth rate of (hUC-MSCs) at different passages.
    Methods
    This experimental study was performed in Ardabil University of Medical Sciences, Iran, from March 2014 to December 2014. The umbilical cord samples were obtained from full-term neonate hospitalized in Alavi’s Hospital in Ardabil under sterile conditions. The umbilical vessels were clear off and the small pieces of the umbilical cord were cultured in Dulbecco's modified eagle's medium (DMEM) supplemented with 20% fetal bovine serum (FBS). Then, the hUC-MSCs were harvested from passage one to three to calculate the population doubling time (PDT) and extract proteins by using CHAPS lysis buffer. Finally, the telomerase activity of the cells at different passages was measured by telomeric repeat amplification protocol (TRAP) and qRT-TRAP assays.
    Results
    The hUC-MSCs population doubling time at passage from 1 to 3 were calculated as the average of 54.68±1.92, 55.03±1.71 and 69.41±2.54 hours, respectively, suggesting the higher cell passage number, the more extended PDT. The threshold cycles (CTs) for the telomerase activity also showed 30.58±0.51, 27.24±0.74 and 32.13±0.75 for the cell passage from one to three, respectively, representing the significant increasing in telomerase activity at passage two compared with the other passages (P= 0.021).
    Conclusion
    Analysis of the growth curve, PDT determination and measurement of telomerase activity of the human umbilical cord-derived mesenchymal stem cells showed that the long-term cell culture can affect on the cell proliferation and the telomerase activity.

    Background
    Nonalcoholic fatty liver disease (NAFLD) includes steatosis, nonalcoholic steatohepatitis, fibrosis and liver cirrhosis. The oxidative stress enzymes are the diagnostic markers to prediction of histologic status of liver in nonalcoholic steatohepatitis disease. The aim of the study was to assessment of relationship between serum Zinc (Zn) levels with pathologic manifestation in patients with nonalcoholic steatohepatitis.
    Methods
    This cohort study was done in patients with nonalcoholic steatohepatitis that had been visited in gastrointestinal clinic of Sina Hospital, Tehran, Iran from April, 2014 to April, 2015. Control group included the patients with no clinical manifestation of nonalcoholic steatohepatitis and normal liver ultrasonography, lab test and liver biopsy. Serum Zn level was measured with atomic absorption spectroscopy. Normal Serum level of Zn was considered 10.7-22.9 µmol/L (70-150 µg/dL) and less than 7 µg/dL was considered as Zn deficiency. Pathological findings were grading according to NAFLD activity score.
    Results
    One hundred twenty patients were selected for the study in two equal groups. Six and 26 patients were excluded in case and control groups, respectively due to no consent to lab test. Finally, 54 patients (35 male/19 female) and 34 patients (22 male/12 female) in control group were participated in data analysis. The mean age on case and control group was 37.02±9.82 year and 33.24±12.01 year, respectively (P= 0.111). Zn level in case and control groups were 90.82±13.69 and 88.82±13.10, respectively. There were no statistically significant differences between two group in serum Zn level (P= 0.50). Also, there were no statistically significant differences between pathological grading in case group participants (steatosis: P= 0.640; Lobular inflammation: P= 0.882; fibrosis: P= 0.531).
    Conclusion
    The finding of the study showed no significant association between serum zinc level and hepatic steatosis, lobular inflammation and fibrosis of the liver in nonalcoholic steatohepatitis.

    Background
    Human leukocyte antigen E is a member of non-classical HLA class I. Interaction between HLA-E molecule on the target cells and inhibitory CD94/NKG2A receptor on the cell surface of natural killer (NK) cells has an important role in the regulation of immune system against pathogens; therefore different cell surface expression of HLA-E molecule plays an important role in host resistance against viral infections as well as host response to treatment. Considering this fact, we analyzed the frequency of different HLA-E genotypes (HLA-E*01010101, HLA-E*01030103, HLA-E*01010103) in major thalassemic patients who underwent frequent transfusion therapy and are thus more susceptible to infectious diseases.
    Methods
    This study was a cross-sectional study of 104 major thalassemic patients who referred to Tehran Thalassemia Clinic between the years 2015 to 2016. Blood DNA was extracted and proliferated by sequence-specific primer polymerase chain reaction (SSP PCR). The PCR product was subjected to electrophoresis on 1.5 percent agarose gel then DNA fragment bands on the gel were detected by exposing to UV light. Furthermore, PCR products were also subjected to sequencing analysis for further confirmation.
    Results
    From 104 patients in this study, 49 (47.1%) were man and 55 (52.9%) were women. These patients were in the age range of 16 to 43 years (mean; 31.03±4.7 year). The frequency of HLA-E*01010103 genotype (64.4 percent) was significantly (P= 0.001) higher than the genotypes of HLA-E*01010101 (15.4%) and HLA-E*01030103 (20.2%) whereas there was no difference between the frequency of HLA-E*0103 allele (52.4%) and HLA-E*0101 (47.6%).
    Conclusion
    This is the first study that examined the HLA-E polymorphisms in Iranian thalassemic patients referred to Tehran Thalassemia Clinic. This study has shown that the frequency of HLA-E*01010103 genotype was significantly higher than other genotypes of HLA-E whereas there was no difference between the frequency of HLA-E*0103 allele and HLA-E*0101 allele. Whether different frequencies of HLA-E genotype may affect thalassemic patients’ susceptibility to blood-borne infections will be of interest for future studies.

    Background
    Burns and its complications are regarded as a major problem in the society. Skin injuries resulted from ultraviolet radiation, radioactivity, electricity or chemicals as well as respiratory damage from smoke inhalation are considered burns. This study aimed to determine the epidemiology and outcome of burn patients admitted to Motahari Hospital, Tehran, Iran.
    Methods
    Two hundred patients with second-degree burns admitted to Motahari Referral Center of Burn in Tehran, Iran. They were studied during a period of 12 months from May 2012 to May 2013. During the first week of treatment swabs were collected from the burn wounds after cleaning the site with sterile normal saline. Samples were inoculated in blood agar and McConkey agar, then incubation at 37 C for 48 hours. Identification was carried out according to standard conventional biochemical tests. Treatment continued up to epithelial formation and wound healing. Results of microbial culture for each patient was recorded. Healing time of the burn wounds in patients was recorded in log books. Chi-square test and SPSS Software v.19 (IBM, NY, USA) were used for data analysis.
    Results
    Our findings indicate that the most causes of burns are hot liquids in 57% of cases and flammable liquid in 21% of cases. The most cases of burns were found to be in the range of 21 to 30 percent with 17.5% and 7% in male and female respectively. Gram-negative bacteria were dominated in 85.7% and among them pseudomonas spp. with 37.5% were the most common cause of infected burns, followed by Enterobacter, Escherichia coli, Staphylococcus aureus, Acinetobacter and Klebsiella spp.
    Conclusion
    The results of this study showed that the most cause of burns in both sex is hot liquid. Men were more expose to burn than women and this might be due to the fact that men are involved in more dangerous jobs than female. Pseudomonas aeruginosa was the most common organism encountered in burn infection.

    Background
    Acetabular dysplasia is a well-known cause of early osteoarthritis of hip which may appear at any time (perinatal, breast-feeding and childhood). The aim of this study was to evaluate the clinical and radiographic outcomes of children with de-velopmental dysplasia of hip (DDH) after undergoing open reduction, capsulorrhaphy and Pemberton osteotomy procedures.
    Methods
    This study prospectively conducted on 13 patients with DDH who attended to Razi Hospital at Ahvaz Jundishapur University of Medical Sciences at Ahvaz, Iran, from April 2012 to March 2015. Inclusion criteria were children with age range of 18 months to 8 years and acetabular index≥ 40 degree. Exclusion criteria were the age less than 18 months or above 8 years, connective tissue diseases, secondary dislocation due to previous infection, and acetabular dysplasia with specific syndrome. All patients were evaluated before surgery and at least one year after surgery in terms of clinical evaluations, dislocation or subluxation of hip, congruity of hip and radiological out-comes according to grading systems of McKay, Tonnis grading system, Severin classi-fication and acetabular index, respectively. All patients underwent open reduction, capsulorrhaphy, and Pemberton’s osteotomy in single-stage surgery and if necessary femoral shortening was performed.
    Results
    Ten patients (12 hips) were evaluated. Of those, 4 patients (40%) had right hip involvement. The mean age score was 38.92±12.37 months (range: 24-65 months). Acetabular index showed significant reduction after surgery in compare to before sur-gery (P= 0.002). According to Tonnis grading, 91.6% of cases were in I-II classes. Moreover, 66.6% of cases at clinical examinations of McKay criteria had excellent and good results after surgery. According to Severin radiographic findings criteria, 83.2% of cases were in I-III classes after surgery. There was statistically significant improvement in patients according to different grading systems.
    Conclusion
    In conclusion, Pemberton osteotomy could be an effective procedure and also in conjunction with other surgical procedures for the treatment of patients with DDH who presented in higher age and late.

    Background
    Acute osteomyelitis is one of pediatric emergency which can cause unpleasant complications among them. This is especially accurate if the diagnosis had been delayed or the treatment was inappropriate. There is some misunderstanding in the detection of patients’ hospital files and it’s difficult to detect the diagnostic and treatment malpractice. We performed an investigation of the clinical and paraclinical data in children with acute osteomyelitis.
    Methods
    We performed a retrospective cross-sectional study in pediatric department of Imam Khomeini University Hospital in Tehran, Iran, from April 1997 to March 2010. The hospital records of all neonates and children from 15 days old to 15 years old were investigated. Patients with any defect in records were excluded from the study. A total of 54 children were included in this study. A questionnaire including clinical features, paraclinical findings and treatment response was completed for all subjects by design's executer. Data were analyzed using the software package SPSS version 16 (IBM, Armonk, NY, USA).
    Results
    In general, 35 children were male and 19 children were female. The average of age was 5.89 years. Children were divided into 4 groups based on age (less than 2, 2-7, 7-12 and 12-15 years) that majority of them were less than 2 years old (38.9%). The most common symptom and sign were pain (46.3%) and swelling (88.8%) respectively in this study. The most primary presenting symptoms were pain (46.3%) and swelling (24.1%). The comparison of frequency ranges of fever between children younger than 2 years and children 2-15 years old demonstrated a significant different (14.3% vs 84.8%, respectively) (P= 0.001). Staphylococcus aureus was the most commonly isolated organism (73.5%). Thirty-two patients (59.2%) needed surgical procedures in addition to antibiotic therapy that had successful outcome in all cases. The average of treatment duration with intravenous antibiotic was 28.14±12.84 days.
    Conclusion
    More than half of the children with acute osteomyelitis didnt response to antibiotic therapy and they needed different types of surgeries.

    Background
    Operation theatre in a hospital requires considerable human and physical resources to deliver surgery services on an agreed schedule. However, operation theatres are sometimes underutilized due to avoidable last minute cancellations of operations. Cancellation of operations on the day of intended surgery results in operation theatre planning difficulties, hospital inefficiency and resource wastage. In addition, it causes stress for patients and their relatives and results in unnecessary hospital staying. Cancellation of planned operations could be avoided by applying appropriate management strategies and techniques. Quality management as an organizational strategy helps enhance hospital departments’ productivity.
    Methods
    This study aimed to reduce cancelled surgeries in Shahid Rajaei Hospital in Tehran using a quality management model. A participatory action research was used for the intervention between April 2013 and March 2014. Information on operations cancelled on the day of surgery obtained each day from the operating theatre list. Using a checklist, the reasons for operations cancellation were identified, investigated and an action plan was developed for its reduction. The plan was implemented using the action research cycle.
    Results
    The number of surgeries increased by 4.06 percent and operations cancellation was reduced by 32.4 percent using the quality management strategy. Surgeon and anesthetist related factors, over-running of previous surgery, changes in patient clinical status and lack of intensive care unit beds were the main reasons for cancelling surgeries. Standardization of processes, proper planning and using anesthetics clinic helped reduce the operations cancellation.
    Conclusion
    Last minute surgeries cancellation is potentially avoidable. Implementing an appropriate quality management model helps enhance hospital departments’ productivity and reduce surgical cancellation.

    Background
    Cancer is the most common cause of death in the world, and it incidence has been increasing for many years in economically developed countries. Early detection of cancers greatly increases the chances for successful treatment. So finding cancers before they start to cause symptoms is a most effective treatment. Recent studies have proposed that blood plasma contains a rich source of disease biomarkers for detecting, diagnosing and monitoring diseases. While some researchers have dismissed the low molecular weight serum peptidome as biological trash, recent work using differential scanning calorimetry has indicated that the peptidome may reflect biological event and contain diagnostic biomarkers.
    Methods
    Differential scanning calorimetry (DSC), a highly sensitive tool for analysis of blood plasma and other biofluids has recently been reported. Louisville Bioscience, Inc. (LBIdx™), The Plasma Thermogram™ (pT™) company has made a significant breakthrough in the analysis of blood plasma using differential scanning calorimetry for clinical monitoring and diagnostic applications.
    Results
    DSC analysis of plasma from diseased individuals revealed significant changes in the thermogram which are suggested to result not from changes in the concentration of the major plasma proteins but from interactions of small molecules or peptides with these proteins. The difference in plasma thermograms between healthy and disease individuals caused this method was recognized as a novel technique for disease diagnosis and monitoring.
    Conclusion
    Measurement of plasma proteins is a powerful clinical is standard medical practice which hope revolutionizes strategies for early cancer detection.
