    مقدمه
    تشخیص زودهنگام و انجام اقدامات پیش گیرانه، باعث کاهش بارز عوارض و علایم اختلالات خلقی می شود. مطالعه ی حاضر، جهت ارزیابی علایم پیش درآمد اختلالات خلقی در جامعه ی ایرانی انجام شد.
    روش ها
    کلیه ی بیماران مبتلا به اختلالات خلقی بستری در بیمارستان نور اصفهان در ماه های شهریور تا بهمن سال 1391 به صورت گذشته نگر، از نظر علایم و نشانه های روان پزشکی طی مصاحبه ی بالینی با نگرش به سابقه ی بیماری ارزیابی شدند. یافته ها با استفاده از آزمون های Independent t و 2χ آنالیز شد.
    یافته ها
    از 175 بیمار (66 مرد و 109 زن) با میانگین سنی 1/12 ± 7/34 سال، به ترتیب 119 و 56 نفر دچار اختلالات دو قطبی و افسردگی اساسی بودند. همه ی این بیماران، به جز یک بیمار مبتلا به افسردگی اساسی، وجود حداقل یک علامت پیش درآمد را گزارش دادند. تکانشگری (Impulsivity)، زودرنجی (Irritability) و افزایش فعالیت ها و ارتباطات اجتماعی (Over-sociality) با فراوانی 8/32، 4/29 و 1/26 درصد، شایع ترین علایم پیش درآمد در اختلال دو قطبی بودند که تکانشگری و زودرنجی باعث افزایش معنی دار احتمال بروز بیماری در آینده می شد. ضعف در مهارت های تطابقی (Poor coping)، گوشه گیری (Social isolation) و اضطراب (Anxiety) نیز با فراوانی 6/78، 9/33 و 4/30 درصد، شایع ترین علایم پیش درآمد در اختلال افسردگی اساسی بودند که تنها ضعف در مهارت های تطابقی، به طور معنی داری احتمال بروز بیماری در آینده را افزایش می داد.
    نتیجه گیری
    با توجه به شیوع بالای علایم پیش درآمد در مبتلایان به اختلالات خلقی در جامعه ی ایرانی، جدی گرفتن این علایم در دوران کودکی و نوجوانی، باعث تشخیص زودرس و نیز کوتاه شدن شدت مرحله ی درمان نشده ی اختلالات خلقی می شود.

    مقدمه
    در طی ایسکمی مغزی، جریان خون و اکسیژن مغز کاهش می یابد و بعد از برقراری مجدد جریان خون و بازگشت اکسیژن در سلول ها، آسیب های شدیدتر ناشی از تولید و تهاجم رادیکال های آزاد ایجاد می گردد. نظر به این که آسیب هیپوکامپ بعد از ایسکمی- ریپرفیوژن باعث اختلالات زیادی در این عضو می شود، در مطالعه ی حاضر، تاثیر محافظتی گیرنده ی A1 (N6-cyclopentyladenosine یا CPA) و اسید اسکوربیک بر هیپوکامپ، مورد بررسی قرار گرفت.
    روش ها
    تعداد 35 سر موش سوری نر به صورت تصادفی در 5 گروه قرار گرفتند. برای القای ایسکمی، شریان های کاروتید مشترک حیوان 15 دقیقه بسته شدند. CPA و اسید اسکوربیک روزانه و داخل پریتونئوم، اولی به مدت 7 روز پس از ایسکمی و دومی از دو هفته قبل تا یک هفته بعد از ایسکمی تزریق شدند. در روز 20 پس از القای ایسکمی، مغز موش ها تثبیت، آماده و با میکروسکوپ الکترونی گذاره بررسی شدند.
    یافته ها
    بررسی فراساختار نورون های هیپوکامپ حیوانات با میکروسکوپ الکترونی گذاره (Transmission electron microscope یا TEM) و بر اساس معیارهای موجود، حفظ و بهبودی ارگانل های درون سلولی به ویژه میتوکندری ها را در گروه های درمانی پس از آسیب ایسکمی- ریپرفیوژن نشان داد. در گروه درمان ترکیبی، این حفاظت بیشتر دیده شد.
    نتیجه گیری
    تزریق داخل صفاقی CPA و اسید اسکوربیک به دنبال ایسکمی- ریپرفیوژن، موجب کاهش آسیب و تخریب نورون های CA1 هیپوکامپ می شود.

    مقدمه
    هر چند مطالعات نشان داده است که در بیماران مبتلا به سرطان پستان، سطح سرمی آنتی ژن اختصاصی پروستات (Prostate-specific antigen یا PSA) بالا می رود، اما مطالعه ای جهت تعیین معیارهای ارزش تشخیصی آن در تعیین ماهیت توده های پستان انجام نشده بود. از این رو، مطالعه ی حاضر با هدف تعیین معیارهای ارزش تشخیصی PSA تام سرم در تشخیص ماهیت توده های پستان انجام شد.
    روش ها
    در یک مطالعه ی مقطعی، 100 بیمار مبتلا به توده ی پستان که کاندیدای بیوپسی بودند، انتخاب شدند و سطح آنتی ژن اختصاصی پروستات تام سرم آنان تعیین شد. سپس، بیماران تحت بیوپسی قرار گرفتند و در مقایسه با نتایج پاتولوژی، ارزش تشخیصی این نشانگر زیستی (PSA) تعیین گردید.
    یافته ها
    مناسب ترین نقطه ی برش PSA تام سرم برای افتراق توده های بدخیم از خوش خیم پستان، مقدار 21/0 نانوگرم/دسی لیتر بود و بر حسب آن، PSA در این نقطه ی برش دارای حساسیت 5/88 درصد و ویژگی 6/98 درصد بود. درصد مثبت کاذب و منفی کاذب PSA در این نقطه ی برش به ترتیب 4/1 و 5/11 درصد بود. ارزش اخباری مثبت و منفی PSA در تشخیص ماهیت توده ی پستان نیز در این نقطه ی برش به ترتیب 8/95 و 1/96 درصد بود و میزان صحت این آزمون، 96 درصد به دست آمد.
    نتیجه گیری
    PSA دارای ارزش تشخیصی بالایی برای افتراق ماهیت توده های خوش خیم و بدخیم پستان بود و با توجه به دسترسی آسان به آن، می تواند به عنوان یک نشانگر زیستی سرمی مفید در تشخیص توده های بدخیم از خوش خیم پستان، به ویژه در مراحل ابتدایی سرطان پستان به کار رود.

    مقدمه
    هدف از انجام مطالعه ی حاضر، بررسی عواملی بود که پیش از عمل جراحی، فرد را مستعد به افت درصد اشباع اکسیژن خون در Post-anesthesia care unit (PACU) می کند.
    روش ها
    در مطالعه ی مشاهده ای حاضر، 500 بیمار پیش از عمل جراحی از نظر سن، جنس، وزن، دور گردن، سابقه ی خروپف (Snoring)، اعتیاد، مصرف سیگار، بیماری زمینه ای، هموگلوبین، علایم حیاتی و درصد اشباع اکسیژن خون ارزیابی شدند. سپس، با توجه به افت یا عدم افت درصد اشباع اکسیژن خون (کمتر از 90 درصد) بیماران در ریکاوری، آنان به دو گروه دچار افت درصد اشباع اکسیژن خون و گروه بدون افت درصد اشباع اکسیژن خون تقسیم شدند. سپس، وجود یا عدم وجود عوامل مورد مطالعه در هر گروه مورد بررسی قرار گرفت.
    یافته ها
    از 500 بیمار تحت بررسی، 50 نفر (10 درصد) دچار افت درصد اشباع اکسیژن خون شدند و 445 نفر (89 درصد) مبتلابه افت درصد اشباع اکسیژن خون نشدند. در ارتباط با 5 بیمار (1 درصد) نیز وضعیت افت درصد اشباع اکسیژن خون مشخص نشد. در بیماران دچار افت درصد اشباع اکسیژن خون، میانگین سن (004/0 = P)، وزن (024/0 = P) و متوسط اندازه ی دور گردن (002/0 = P)، متوسط فشار خون سیستولیک (026/0 = P)، متوسط دمای محیطی (037/0 = P)، توزیع فراوانی مصرف سیگار (004/0/ = P) و توزیع فراوانی بیماری های دیابت (030/0 = P)، پرفشاری خون (006/0 = P) و مشکل تنفسی (020/0 = P) به طور معنی داری بیشتر از بیمارانی بود که دچار افت درصد اشباع خون شریانی نشدند. همچنین، متوسط درصد اشباع اکسیژن خون نیز در بیماران مبتلا به افت درصد اشباع اکسیژن خون، به طور معنی داری کمتر از بیماران بدون افت درصد اشباع اکسیژن خون بود (037/0 = P).
    نتیجه گیری
    با بررسی عوامل مساعد کننده، می توان احتمال افت درصد اشباع اکسیژن خون را پیش بینی کرد و با کنترل عوامل، از میزان افت درصد اشباع اکسیژن کاست.

    مقدمه
    هدف از درمان و بازسازی ضایعات وسیع بافت نرم اندام تحتانی، پوشش و ترمیم زخم، حفظ عملکرد اندام های آسیب دیده، جلوگیری از قطع عضو و حفظ کیفیت زندگی بیمار است.
    روش ها
    این مطالعه یک مطالعه ی توصیفی- تحلیلی بود. این مطالعه، در مرکز آموزشی- درمانی الزهرای (س) اصفهان بر روی 27 بیمار انجام شد که در سال های 94-1390 تحت بازسازی ضایعات وسیع اندام تحتانی با فلپ آزاد (Free flap) قرار گرفته بودند. در این مطالعه، علت و محل ضایعه، نوع فلپ آزاد، نوع آناستوموز، میزان موفقیت کلی فلپ آزاد در پوشش ضایعات (Survival rate)، عوارض و علل آن مورد بررسی و مطالعه قرار گرفت.
    یافته ها
    27 بیمار، 26 مرد و 1 زن با میانگین سنی 31 سال با ضایعات وسیع اندام تحتانی طی سال های 90-1394 با میانگین مدت پی گیری 2 ماه، تحت مطالعه و بررسی قرار گرفتند. محل ضایعات، یک سوم میانی و تحتانی ساق پا (21 مورد)، پاشنه و کف پا (4 مورد) و زانو (2 مورد) و علت ضایعات، تروما (25 مورد) و زخم مزمن (2 مورد) بود. نوع Musculocutaneous free flap استفاده شده عبارت از Latissimus dorsi (15 مورد)، Rectus abdominis (8 مورد) و Gracilis (4 مورد) با آناستوموز عروقی End to end (43 مورد) و End to side (11 مورد) بود. موفقیت کلی فلپ آزاد در پوشش کامل ضایعات 9/88 درصد (24 از 27) به دست آمد. سه مورد رد کامل فلپ به علت ترومبوز به دلیل استفاده از گرافت وریدی، وجود وریدهای واریسی و شریان آترواسکلروتیک مشاهده شد.
    نتیجه گیری
    فلپ آزاد در صورت وجود پایه ی عروقی مناسب، روش قابل اعتمادی برای بازسازی ضایعات وسیع اندام تحتانی می باشد.

    Background
    By early detection and performing preventive interventions, it would be possible to significantly prevent side effects and burden of mood disorders. This study aimed to investigate the premorbid signs and symptoms of major mood disorders in a sample of Iranian patients.
    Methods
    All the patients hospitalized due to mood disorders in Noor hospital in Isfahan City, Iran, were interviewed to investigate past history of any psychiatric sign and symptom retrospectively using convenience sampling method. Independent sample t and chi-square tests were used to analyze the data.
    Findings: From 175 patients (66 men and 109 women) with the mean age of 34.7 ± 12.1 years, 119 participants were diagnosed as bipolar mood disorder (BMD) and 56 as major depressive disorder (MDD). All the patients (except 1 with major depressive disorder) had at least 1 premorbid sign or symptom. Impulsivity (32.8%), irritability (29.4%), and over-sociality (26.1%) were the most frequent premorbid sign/symptom in the past history of the patients with bipolar mood disorder; impulsivity and irritability significantly increased the risk of the future bipolar mood disorder. Poor coping (78.6%), social isolation (33.9%), and anxiety (30.4%) were the most frequent premorbid sign/symptom among the patients with major depressive disorder; only poor coping significantly increased the risk of the future major depressive disorder.
    Conclusion
    Considering the high prevalence of premorbid signs and symptoms among Iranian patients with major mood disorders, more attention should be paid to them during childhood and adolescence periods to make early diagnosis and shorten the untreated phase of these disorders.

    Background
    In brain ischemia, blood and oxygen supply decrease and after reperfusion, free radicals and reactive oxygen species (ROS) cause severe damage. As hippocampal injury after ischemia-reperfusion causes some complications, in this study we analyzed the effect of adenosine receptor agonist (N6-cyclopentyladenosine or CPA) and ascorbic acid on ultrastructure of hippocampal CA1 neurons after ischemia-reperfusion.
    Methods
    35 male rats in 5 groups were used. Ischemia-reperfusion performed by occlusion of common carotids for 15 minutes. CPA and ascorbic acids were intraperitoneally injected for 7 days after ischemia, and 2 weeks before and for 7 days after ischemia, respectively. After 20 days, brain samples were isolated, prepared, and assayed using transmission electron microscopy (TEM).
    Findings: Ultrastructure assay of hippocampal CA1 neurons after ischemia-reperfusion with transmission electron microscopy showed recovery of intracellular organelles particularly mitochondria of treated groups. In combination therapy, these improvements were better.
    Conclusion
    Intraperitoneal injection of CPA and ascorbic acid after ischemia-reperfusion can reduce neural damage in CA1 region of hippocampus.

    Background
    Previous studies had shown that patients with breast cancer have a high level of prostate-specific antigen (PSA). But there was not any study about the role of prostate-specific antigen in detection of breast cancer. So, this study was done to determine the diagnostic values of prostate-specific antigen in detection of breast masses nature.
    Methods
    In a cross sectional study, 100 patients with breast mass candidate for biopsy were selected and serum level of total serum prostate-specific antigen was measured. Then, patients underwent biopsy and diagnostic values were calculated for it in comparison with pathology results.
    Findings: The cut point of prostate-specific antigen for detection of breast mass nature was 0.21 ng/dl and according to it, sensitivity and specificity of prostate-specific antigen was 88.5 and 98.6 percent, respectively. False negative and positive was 1.4 and 11.5 percent, respectively. In addition, positive and negative predictive values were 95.8 and 96.1 percent, respectively, and accuracy rate of prostate-specific antigen was 96 percent.
    Conclusion
    According to our findings, prostate-specific antigen have a high diagnostic value for detection of breast mass nature and it can be used as a serum biomarker to detect the nature of breast mass, especially in early stages.

    Background
    We aimed to investigate preoperative factors tend to decrease in oxygen saturation in post-anesthesia care unit (PACU).
    Methods
    This observational study evaluated 500 patients before operation for age, gender, weight, neck circumference, smoking, addiction, underlying diseases, hemoglobin level, vital signs, and oxygen saturation. After operation, patients were divided into two groups of those experienced oxygen desaturation and those did not experience this situation. All above-mentioned variables were compared between the two groups.
    Findings: The prevalence of patients with oxygen desaturation was 10%; while 89% did not experience this condition and 1% was unkown for oxygen saturation. There were significant differences between the two groups in age (P = 0.004), weight (P = 0.024), neck circumference (P = 0.002), systolic blood pressure (P = 0.026), peripheral and central temperature (P = 0.037), smoking (P = 0.004), diabetes (P = 0.030), hypertension (P = 0.006), respiratory disease (P = 0.020), and preoperative oxygen saturation (P = 0.037).
    Conclusion
    By evaluating predictive factors, oxygen desaturation can be predictable. We can improve respiratory complications in post-anesthesia care unit by controlling and monitoring these factors.

    Background
    The aims of soft-tissue reconstruction of lower leg defects are covering and repairing wounds, maintaining limb function, preventing amputation, and improving quality of life.
    Methods
    In this cross-sectional study, 27 patients underwent soft-tissue reconstruction of lower extremities with free flaps were enrolled. The cause and the site of defects, free flap types, anastomosis types, survival rate, and complications were assessed.
    Findings: 27 patients with mean age of 31 years with large lower extremity defects with at least 2 months follow up were assessed. The sites of defects were middle and distal one-third of the legs (21 cases), foot and heel (4 cases), and knees (2 cases), and causes of defects were trauma (21 cases), and chronic wounds (2 cases). Used musculocutaneous free flaps were for latissimus dorsi (15 cases), rctus abdominis (8 cases), and gracilis (4 cases). 43 and 11 cases had end-to-end and end-to-site anastomosis, respectively. Survival rate was 88.9 percent and 3 flaps were failed due to thrombosis.
    Conclusion
    Using free flaps with appropriate vascular pedicles is a reliable and effective method for reconstruction of lower leg large defects.
