    مقدمه
    در سال های اخیر، ارتباط الگوهای دریافت مواد مغذی با بیماری های مزمن و چاقی مورد توجه قرار گرفته است. مطالعه ی حاضر با هدف ارزیابی ارتباط الگوی دریافت مواد مغذی با تغییرات وزن و دورکمر انجام شد.
    مواد و روش ها
    پژوهش حاضر روی 1637 نفر از بزرگسالان 75-30 ساله ی شرکت کننده در مطالعه ی قند و لیپید تهران (سال های 1387-1384) که دارای داده های تغذیه ای و تن سنجی و فاقد بیماری های قلبی عروقی و سرطان بودند و تا مرحله ی بعدی (سال های 1390-1387) این مطالعه پی گیری شده بودند، انجام شد. دریافت های 33 ماده ی مغذی، با استفاده از پرسش نامه ی روا و پایای بسامد خوراک محاسبه شد و تغییرات سه ساله ی وزن و دور کمر اندازه گیری شد. الگوهای مواد مغذی، با استفاده از تحلیل عاملی به دست آمد.
    یافته ها
    4 الگوی مواد مغذی شناسایی شد. میانه و دامنه ی میان چارکی تغییرات وزن و دور کمر افراد مورد مطالعه، به ترتیب (0/3 – 0/1-)0/1 کیلوگرم و (0/8 – 0/0)0/3 سانتی متر بود. الگوی اول (الگوی گوشت و لبنیات پرچرب) که غنی از پروتئین حیوانی، اسید چرب اشباع، کلسترول، ویتامین دی، ریبوفلاوین، پانتوتنیک اسید، ویتامین B12، کلسیم، فسفر و روی بود، پس از تعدیل اثر مخدوش گرهای احتمالی با افزایش وزن ارتباط مستقیم نشان داد (009/0P for trend= و (449/0- 066/0) 258/0=β). برای سایر الگوها (میوه سبزی و لبنیات، حبوبات و مغزها، غلات و میوه ها) یافته ی معنی داری در ارتباط با تغییرات وزن و دور کمر دیده نشد.
    نتیجه گیری
    دریافت الگوی رژیمی غنی از مواد مغذی با منبع حیوانی، با افزایش وزن رابطه مستقیم دارد.

    مقدمه
    دیابت نوع یک، یکی از شایع ترین اختلالات متابولیک مزمن در دوران نوجوانی است. این بیماری اغلب به عنوان بیماری خانواده در نظر گرفته می شود، زیرا اداره امور فرد بیمار به میزان زیادی نیاز به مداخله و حمایت خانواده دارد. با توجه به ماهیت بیماری دیابت و لزوم خودمراقبتی در این بیماران، الگوی توانمندسازی خانواده محور با تاکید بر موثر بودن نقش فرد و اعضای خانواده در سه بعد انگیزشی، روان شناختی و ویژگی های بیماری می تواند چهارچوب مناسبی برای مداخله ی آموزشی در زمینه ی فوق باشد. بنابراین، مطالعه حاضر با هدف تعیین تاثیر الگوی توانمندسازی خانواده محور بر کیفیت زندگی و خودکارآمدی نوجوانان مبتلا به دیابت انجام شد.
    مواد و روش ها
    در این مطالعه نیمه تجربی، 100 نوجوان مبتلا به دیابت نوع یک به طور تصادفی در دو گروه مداخله و شاهد (هر کدام 50 نفر) تقسیم شدند. الگوی توانمندسازی خانواده محور، منطبق با چهار بعد تهدید درک شده، خودکارآمدی، مشارکت آموزشی و ارزشیابی، در 5 جلسه در گروه مداخله اجرا شد. داده ها با استفاده از فرم مشخصات دموگرافی بیمار، پرسش نامه ی استاندارد مقیاس خودکارآمدی در مدیریت دیابت و پرسش نامه ی کیفیت زندگی نوجوانان دیابتی، پیش، بلافاصله، و 5/1 ماه و 3 ماه پس از مداخله جمع آوری شدند.
    یافته ها
    پس از اجرای الگوی توانمندسازی خانواده محور، میانگین نمرات خودکارآمدی و کیفیت زندگی در 5/1 ماه و 3 ماه پس از پایان مداخله، در گروه مداخله افزایش داشت و این اختلاف از لحاظ آماری معنی دار بود (001/0>p).
    نتیجه گیری
    نتایج مطالعه نشان داد، اجرای روش های آموزشی مبتنی بر توانمندسازی بیمار که با مشارکت و محوریت بیمار و خانواده انجام شود، می تواند خودکارآمدی بیماران را بهبود بخشد و منجر به ارتقای کیفیت زندگی نوجوانان مبتلا به دیابت شود. بنابراین به کارگیری این الگو توسط پرستاران، جهت مشارکت خانواده و بیمار در مراقبت های درمانی کاربردی است.

    مقدمه
    دیابت نوع 2 یکی از شایع ترین بیماری های متابولیک است. نیترات به عنوان درمان جدیدی برای دیابت مطرح است. با توجه به اینکه هم دیابت و هم نیترات سدیم بر شمارش سلول های خونی اثر دارند و حدود 30 درصد افراد دیابتی کم خونی دارند، این مطالعه با هدف بررسی اثر نیترات سدیم بر تعداد سلول های خونی در موش های صحرایی نر چاق دیابتی نوع 2 انجام شد.
    مواد و روش ها
    48 سر موش صحرایی نر نژاد ویستار به چهار گروه کنترل، کنترل + نیترات، دیابت و دیابت+نیترات تقسیم شدند. هر یک از گروه هایی که نیترات دریافت می کردند به دو زیر گروه تقسیم شدند: کنترل + نیترات 100 (CN 100) و کنترل+نیترات 250 (CN250)؛ دیابت+نیترات 100 (DN100) و دیابت نیترات 250 (DN250). نیترات سدیم به مدت دو ماه در آب آشامیدنی در گروه های CN100، CN250، DN100، DN250 تجویز شد. دیابت با تجویز رژیم پرچرب به مدت 14 روز و تزریق استرپتوزوتوسین القاء شد. شمارش سلول های خونی در پایان مطالعه انجام شد.
    یافته ها
    تجویز نیترات به مدت دو ماه در موش های دیابتی موجب کاهش وزن، قند خون، هماتوکریت و نوتروفیل شد (05/0>p)، اما تعداد کل گلبول های سفید و لنفوسیت را افزایش داد (05/0>p). تجویز نیترات اثری بر تعداد گلبول های قرمز، هموگلوبین، MCV، MCH، MCHC و تعداد پلاکت نداشت.
    نتیجه گیری
    از این مطالعه نتیجه گیری می شود که تجویز نیترات به عنوان یک عامل درمانی در موش های دیابتی مبتلا به دیابت نوع 2 منجر به کاهش قند خون می شود و اثرات زیان بار عمده ای بر پارامترهای خونی ندارد و حتی با کاهش تعداد نوتروفیل ها ممکن است اثرات ضد التهابی داشته باشد.

    مقدمه
    در مطالعات مختلف، در مورد عوارض استفاده از انسولین و متفورمین در هایپرگلایسمی های پس از عمل جراحی بای پس عروق کرونر، نتایج متناقضی را گزارش کرده اند. هدف از مطالعه ی حاضر، مقایسه ی تاثیر انسولین و متفورمین بر اسیدیته، بیکربنات، پتاسیم و منیزیم خون بیماران فاقد دیابت تحت عمل جراحی بای پس عروق کرونر بود.
    مواد و روش ها
    در یک کارآزمایی بالینی، 56 بیمار فاقد دیابت که در بیمارستان کوثر سمنان تحت عمل جراحی بای پس عروق کرونر قرار گرفته و دچار هیپرگلایسمی پس از عمل شده بودند، به طور تصادفی در دو گروه دریافت کننده ی انسولین و متفورمین قرار گرفتند. میزان قند خون، اسیدیته، بیکربنات، پتاسیم و منیزیوم خون بیماران هر دو گروه، در پیش و پس از عمل، اندازه گیری و ثبت شد.
    یافته ها
    در زمان پذیرش بیماران در بخش ICU، در گروه دریافت کننده انسولین میانگین اسیدیته ی خون شریانی، بیکربنات، پتاسیم و منیزیم به ترتیب 07/0±37/7، 5/2±8/21 میلی اکی والان بر لیتر، 5/0±3/4 میلی مول بر لیتر و 3/0±1/2 میلی گرم بر دسی لیتر و در گروه دریافت کننده متفورمین این مقادیر به ترتیب 05/0±40/7، 8/2±8/22 میلی اکی والان بر لیتر، 5/0±1/4 میلی مول بر لیتر و 4/0±1/2 میلی گرم بر دسی لیتر بود. بین میانگین پتاسیم، منیزیم و بیکربنات بیماران دو گروه مورد مطالعه، در زمان های مختلف، تفاوت معنی دار آماری دیده نشد (05/0P>). اما بین میانگین اسیدیته ی خون شریانی در 72 ساعت پس از پذیرش در ICU، در دو گروه انسولین (03/0±41/7) و متفورمین (04/0±39/7) تفاوت معنی داری آمار وجود داشت (047/0P=). میانگین pH خون از حد نرمال خارج نشد.
    نتیجه گیری
    در بیماران بدون دیابت که به دنبال عمل جراحی بای پس عروق کرونر دچار هیپرگلایسمی می شوند، استفاده از متفورمین و انسولین تاثیری بر بیکربنات، پتاسیم و منیزیم خون ندارد، اما استفاده از متفورمین ممکن است در طول زمان به تدریج PH خون را اسیدی کند. بنابراین پیشنهاد می شود در این بیماران، در صورت استفاده از متفورمین، pH خون پایش شود.

    مقدمه
    مطالعات گسترده روی انسان و مدل های حیوانی، تاثیر مواجهه ی جنین در بازه های زما نی ویژه و تعیین کننده ی پیش از تولد با عوامل محیطی (از جمله هورمون ها، مواد شیمیایی، تغذیه و عوامل اجتماعی)، بر رشد و تکامل جنین را نشان داده اند. هورمون های جنسی از جمله ی این عوامل محیطی هستند که بیشترین اثرات را بر رشد و تکامل بخش های مختلف جنین به جا می گذارند و در این بین، آندروژن ها، به لحاظ دارا بودن منابع مختلف تولید و ترشح، بیشتر از سایر هورمو ن ها مورد توجه هستند. یافته های حاصل از این تحقیقات نشان داده اند که آندروژن مازاد می تواند در دوران جنینی به عنوان یک تراتوژن عمل کند و باعث ایجاد نقایصی در زمینه های مختلفی، از قبیل اختلال در عملکرد غدد درون ریز، دستگاه عصبی و هم چنین بروز اختلالات روان شناختی و رفتاری در بزرگ سالی شود. به لحاظ اهمیت دوران بحرانی پیش از تولد در بروز ناهنجاری های بزرگسالی، مطالعات بنیادی بسیاری بر چگونگی تاثیر عوامل محیطی بر این دوران حساس متمرکز شده اند. این مقاله به ارائه ی شواهدی می پردازد که تاثیر آندروژن های مازاد در این دوران را بر رشد و تکامل جنین، که می تواند زمینه ساز بروز برخی فنوتیپ/بیماری های ویژه در بزرگسالی باشد، نشان می دهد.

    مقدمه
    آدیپو کین ها هورمون هایی هستند که از بافت چربی، در پاسخ به محرک های خارج سلولی و تغییر سوخت و ساز بدن ترشح می شوند. این هورمون ها در تنظیم تعادل انرژی، متابولیسم لیپید و حساسیت به انسولین نقش دارند. یکی از انواع آدیپوکین ها، آدیپونکتین نام دارد که دارای اثرات ضد دیابت، ضدالتهاب و ضد آتروژنیک است. هدف از این مطالعه مروری سیستماتیک، بررسی نقش آدیپونکتین در دیابت بارداری، پره اکلامپسی و چاقی دوران بارداری بود.
    مواد و روش ها
    برای انجام پژوهش حاضر، مرور سیستماتیک بر روی مطالعات توصیفی مشاهده ای در بانک های اطلاعاتی PubMed ، Iran Medex، SID و Google Scholar از ژانویه سال 2000 تا فوریه 2017 با واردکردن کلید واژه های مورد نظر انجام گرفت. کیفیت مقالات استخراج شده بر اساس فهرست مندرجات STORBE ارزیابی شد و در نهایت 20 مقاله تحلیل شدند.
    یافته ها
    سطح آدیپونکتین در اوایل بارداری افزایش می یابد، سپس در طول بارداری هم زمان با افزایش مقاومت به انسولین، سطح این هورمون در گردش خون مادر و بافت چربی به تدریج کاهش پیدا می کند. کاهش غلظت آدیپونکتین در چاقی و دیابت بارداری نیز دیده می شود و احتمالا سبب تشدید مقاومت به انسولین می شود، در حالی که به نظر می رسد در پره اکلامپسی سطح این هورمون افزایش پیدا می کند.
    نتیجه گیری
    این یافته ها نشان می دهند که کاهش سطح آدیپونکتین ممکن است در مقاومت به انسولین در طول بارداری، چاقی و دیابت بارداری نقش داشته باشد و افزایش سطح این هورمون در پره اکلامپسی احتمالا پاسخی فیزیولوژیک برای بهبود عملکرد عروقی و حساسیت به انسولین است.

    Introduction
    The association of nutrient patterns with chronic disease and obesity has been much focused on recently. This study investigated the relation between nutrient patterns with changes in weight and waist circumference.
    Materials And Methods
    The study population included 1637 subjects, aged 30-75 years, free of cardiovascular diseases, and cancer, recruited from Tehran Lipid and Glucose Study (2005-2008). Participants had complete anthropometric and dietary information and were followed up to the next phase (2008-2011). Dietary intake of 33 nutrients was collected by a valid and reliable food frequency questionnaire and three year changes of weight and waist circumference were documented. Nutrient patterns were obtained using principal component analysis.
    Results
    Four nutrient patterns were identified. Median and interquartile range (IQR) of weight and waist circumference changes of participants were 1.0 (-1.0–3.0) kilogram and 3.0 (0.0–8.0) centimetres, respectively. The first pattern (high fat dairy and meat), rich in animal protein, saturated fatty acid, cholesterol, vitamin D, riboflavin, pantothenic acid, vitamin B12, calcium, phosphorous and zinc was associated with higher weight gain 0.258 (0.066–0.449) P for trend=0.009 after adjusting for confounder variables. There were no significant associations for other patterns (fruit and vegetables with dairy, nuts and legumes, and grains and fruits) with changes in weight and waist circumference.
    Conclusion
    Dietary intakes of nutrient patterns rich in animal source foods are directly associated with higher weight gain.

    Introduction
    Type 1 diabetes is one of the most common of chronic metabolic disturbances in the adolescent period. This disease is often considered as a family disease because its management needs and support of family members, considering the nature of diabetes and the need for self-care in patients, the family-centered empowerment model of with emphasis on the effective role of the individual and family members in three-dimensions of prevocational, psychological and properties of the problem can be a suitable frame for instructional intervention. Therefore, this study aimed to determine the effects of the empowerment model of family-centered intervention on the quality of life and self-efficacy of adolescents with diabetes.
    Materials And Methods
    This is quasi-experimental study of 100 adolescents with type 1 diabetes, have divided randomly in two groups (n=50 each) of intervention and controls. Family-centered empoverment model intervention with four dimensions of perceived threat, self-efficacy, instructional partnership and evaluation was done in 5 sessions in the intervention group. Data of the patient’s demographic profile, questionnaire of standard scale self-efficacy in the management of diabetes and the quality of life of questionnaire for adolescents with diabetes were collected before, after immediately, and 1.5 months and 3 months after of intervention. For analysis of data, we used SPSS software version 21 and statistical test, K-square, paired and independent T and analysis of variance of duplicate sizes.

    Results showed that after implementation of empowerment pattern of family-centered increased mean of the self-efficacy scores and quality of life at 1.5 months and 3 months after intervention in the intervention group a difference that was meaningful in terms of statistical significance (p
    Conclusion
    Study results showed that diabetes education with the patient empowerment approach which performed at the family level can improve patients self-efficacy and increase quality of life adolescences with diabetes, and can be implemented by nurses to assist the family and patient in medical care.

    Introduction
    Type 2 diabetes is one of the most common metabolic diseases. Nitrate has been introduced as a new therapeutic agent for type 2 diabetes. Considering that both diabetes and nitrate have some effects on blood cell count and 30% of diabetic patients have anemia, the aim of this study was to determine the effect of sodium nitrate on blood cell count in obese type 2 diabetic rats.
    Materials And Methods
    Forty-eight male Wistar rats were divided into four groups: Control, Control nitrate, Diabetes and Diabetes nitrate. The groups that received nitrate (Control nitrate, Diabetes nitrate) again were divided into two subgroups, which received sodium nitrate (100 and 250 mg/L in drinking water) for two months: control鸫歪ힵ (CN100), control鸫歪 250 (CN250), diabetes鸫歪ힵ (DN100), and diabetes鸫歪ힶ (DN250). Diabetes was induced using a high-fat diet for 14 days and injection of streptozotocin. Blood cell count was performed at the end of the study.

    Administration of sodium nitrate, which is considered as a therapeutic agent in type 2 diabetes, decreased blood glucose in the type 2 diabetic rats but had no major harmful effects on blood parameters; in addition, it may also have anti-inflammatory effects by decreasing the number of neutrophils.

    Introduction
    There is controversy regarding the effects of insulin and metformin usage in postoperative hyperglycemia after coronary artery bypass graft surgery. The purpose of this study is to compare the effect of insulin and metformin on blood acidity, bicarbonate, potassium and magnesium in nondiabetic patients undergoing coronary artery bypass graft surgery.
    Materials And Methods
    In a clinical trial, 56 nondiabetic patients who underwent coronary artery bypass graft surgery in the Kowsar Hospital in Semnan and suffered post-operative hyperglycemia, randomly received insulin and metformin. Levels of blood glucose, acidity, bicarbonate, potassium and magnesium were measured and recorded for both groups before and after surgery. Data was analyzed (significant level 0.05) using Chi-square, Mann-Whitney and Friedman tests.
    Results
    At the time of admission of patients in the ICU, means of arterial blood acidity, bicarbonate, potassium and magnesium were 7.37±0.07, 21.8±2.5 mEq/l, 4.3±0.5 mmol/l, and ± 2.1±0.3 mg/dl respectively in the insulin group and in the metformin group these values were 7.40±0.05, 22.2±2.8 mEq/l, 4.1±0.5 mmol/l, and 2.1±0.4 mg/dl respectively. There was no significant difference between the means of potassium, magnesium and bicarbonate in the two groups at different times (P>0.05). However, there was a significant difference between the mean values of arterial blood acidity 72 hours after admission in ICU in the two groups of insulin (7.41±0.03) and metformin (7.39±0.04) (P=0.047). However mean PH of the blood remained normal.
    Conclusion
    In nondiabetic patients who have hyperglycemia following coronary artery bypass graft surgery, the use of metformin and insulin does not affect bicarbonate, potassium and magnesium of blood. However, overtime the use of metformin may gradually increases the pH of the blood. Therefore, it is recommended that metformin should be used in patients with blood pH control.

    Introduction
    The effects of environmental exposure during critical periods of gestational life on fetal growth and development have been confirmed by a large number of studies on human and animal models. Sex hormons are among the most influential environmental factor which affect on growth and development of different organs of fetus. Among them, androgens are the most important ones because they have various sources of production and secretion. Result previous studies that exposure to androgens during pregnancy may act as a teratogenic agent and cause defects, deministrated in offspring's endocrine and neural system developments. Considering the importance of this critical period for the development of some abnormal features in adulthood, basic research and clinical prevention efforts need to be run at this stage. This review article presents evidence on the effect of excessive androgens during fetal life on embryo development and evolution, which can lead to the development of certain phenotypes / diseases in adulthood.

    Introduction
    Adipokines are hormones secreted from adipose tissue in response to extracellular stimuli and changes in metabolism. These hormones are involved in the regulation of energy homeostasis, lipid metabolism and insulin sensitivity. Adiponectin is one of these adipokines that has anti-diabetic, anti-inflammatory and anti-atherogenic properties. The aim of this systematic review was to evaluate the role of adiponectin in gestational diabetes mellitus, preeclampsia and obesity during pregnancy.
    Materials And Methods
    To conduct this research, a systematic of descriptive _observational studies by review was conducted searching databases, i.e PubMed, Iran Medex, SID, Google Scholar, between January 2000-February 2017, using the related keywords.The quality of the extracted articles was evaluated based on the STORBE checklist of contents and finally 20 articles were analyzed.
    Results
    Adiponectin levels early in pregnancy increased, and with increasing insulin resistance during pregnancy, these levels gradually declined in maternal circulation and adipose tissue. Decreased adiponectin concentrations were also observed in obesity and gestational diabetes mellitus and can exacerbate insulin resistance, although levels of this hormone appears to increase in preeclampsia.
    Conclusion
    These results indicate that reducing adiponectin levels is possibly involved in insulin resistance during pregnancy, obesity and gestational diabetes mellitus and the increase in levels of this hormone in preeclampsia is probably a physiological response to improving vascular function and insulin sensitivity.
