صفحات 104-110مقدمهتجویز وریدی فنتانیل در طی القای بیهوشی عمومی اغلب باعث تحریک سرفه با فراوانی بین 80-18 درصد می شود. از این رو، مطالعه ی حاضر با هدف بررسی و مقایسه ی اثرات کتامین و دگزامتازون بر روی کاهش سرفه در بیماران کاندیدای بیهوشی عمومی با استفاده از فنتانیل انجام شد.روش هادر طی یک مطالعه ی کارآزمایی بالینی تصادفی دو سو کور، 99 بیمار با درجه ی American Society of Anesthesiologists (ASA) I و II در سنین 64-18 سال کاندیدای بیهوشی عمومی با استفاده از فنتانیل مورد مطالعه قرار گرفتند. بیماران به صورت تصادفی و مساوی به 3 گروه دریافت کننده ی کتامین (5/0 میلی گرم/کیلوگرم)، گروه دگزامتازون (1/0 میلی گرم/کیلوگرم) و گروه دارونما (5 میلی لیتر نرمال سالین) تقسیم شدند. کلیه ی داروها یک دقیقه قبل از تجویز 5/2 میکروگرم/کیلوگرم فنتانیل تجویز شد. علایم حیاتی، تعداد و شدت سرفه اندازه گیری و ثبت گردید. آنالیز آماری با استفاده از آزمون های One-way ANOVA و Kruskal-Wallis صورت گرفت.یافته هافراوانی سرفه به طور معنی داری در گروه کتامین (8/81 درصد) در مقایسه با گروه های دگزامتازون (9/90 درصد) و دارونما (1/93 درصد) کمتر بود (03/0 = P). شدت سرفه، به طور معنی داری در گروه کتامین (ملایم 2/18 درصد، متوسط 8/78 درصد و شدید 0/30 درصد) در مقایسه با گروه دارونما (ملایم 1/9 درصد، متوسط 7/72 درصد و شدید 2/18 درصد) کمتر بود (04/0 = P). همچنین، تفاوت های معنی داری بین سه گروه در تغییرات فشار خون سیستول و دیاستول و ضربان قلب وجود داشت (05/0 > P).نتیجه گیریتجویز دز کم کتامین، می تواند به طور موثری تعداد و شدت سرفه ی ناشی از فنتانیل را در بیماران کاندیدای بیهوشی عمومی در حین القای بیهوشی کاهش دهد.کلیدواژگان: سرفه، بیهوشی، دگزامتازون، فنتانیل، کتامین

صفحات 111-117مقدمهفوبیای خون-تزریق یک اختلال مزمن و ناتوان کننده در کودکان و نوجوانان می باشد که می تواند با اجتناب اضطرابی از تعداد زیادی از درمان های ضروری ارتباط داشته باشد. اگر چه تعدادی درمان پیشنهاد شده ی جاری برای فوبیای خون- تزریق وجود دارد، اما شواهد اندکی در مورد درمان واقعیت مجازی وجود دارد. از این رو، مطالعه ی حاضر با هدف بررسی اثر درمان به روش واقعیت مجازی بر روی فوبیای خون- تزریق کودکان و نوجوانان انجام شد.روش هادر این مطالعه ی نیمه تجربی، 31 کودک و نوجوان 18-6 ساله با تشخیص فوبیای خون-تزریق مورد بررسی قرار گرفتند. تمام کودکان 12 جلسه کار کردن با نرم افزار واقعیت مجازی را دریافت کردند. میزان اضطراب کودکان با استفاده از پرسش نامه های Spielberger و علایم اختصاصی فوبیای خون- تزریق مورد بررسی قرار گرفت. آنالیز آماری با استفاده از نرم افزار SPSS انجام شد.یافته هابعد از کار کردن با نرم افزار، میانگین امتیاز اضطراب موقعیتی، ذاتی و موقعیتی- ذاتی و نیز فوبیای خون- تزریق در کودکان به طور معنی داری پایین تر از میزان آن ها قبل از کار کردن با نرم افزار بود (050/0 > P). ارتباط مستقیم و معنی داری بین سن و میزان کاهش اضطراب موقعیتی- ذاتی وجود داشت (60/0+ = r و 001/0 > P).نتیجه گیریدرمان واقعیت مجازی، می تواند به طور موثری اضطراب موقعیتی، ذاتی و موقعیتی- ذاتی و نیز فوبیای خون- تزریق را در کودکان و نوجوانان کاهش دهد. با این وجود، مطالعات بیشتری برای تعیین بهترین نسخه ی نرم افزار واقعیت مجازی برای فوبیای خون- تزریق لازم است.کلیدواژگان: نوجوان، خون، کودک، تزریق، فوبیا، واقعیت مجازی

صفحات 118-123مقدمهاستفاده از القاگرهای سمی، مانند Isopropyl β-D-1-thiogalactopyranoside (IPTG)، در تولید زیست داروها بر اساس رهنمودهای سازمان های نظارتی ممنوع می باشد. از این رو، مطالعه ی حاضر با هدف ارزیابی و بررسی امکان استفاده از سیستم های بیانی خود القایی برای تولید پپتید YY(3-36) به عنوان یک ماده ی اولیه ی دارویی (Active pharmaceutical ingredient یا API) در سیستم بیانی Escherichia coli انجام شد.روش هاتوالی پروموتر خود القایی با استفاده بانک های اطلاعاتی به دست آمد. سپس، توالی مورد نظر در وکتور pUC18 سنتز شد. ژن کد کننده ی YY(3-36) نیز با سنتز پرایمر در انتهای’3 پروموتر قرار داده شد. البته، جهت بیان خارج سلولی، یک توالی 22 اسید آمینه ای سیگنال ترشحی آنزیم آسپاراژیناز در قسمت بالادستی ژن پپتید مورد نظر قرار داده شد. پس از آماده سازی باکتری Escherichia coli از وکتور pUC18 برای انتقال ژن به باکتری استفاده شد. سپس، با مقایسه ی بیان ژن پپتید در سیستم بیانی pET22 به عنوان شاهد مثبت، بیان پپتید در دو حالت خودالقایی و استفاده از IPTG با استفاده از ژل الکتروفورز بررسی گردید.یافته هامیزان بیان با پروموتر خود القاگر در مقایسه با شاهد مثبت (IPTG با غلظت 1 میلی مولار) مناسب بود.نتیجه گیرییافته های مطالعه ی حاضر حاکی از سهولت و اقتصادی بودن کاربرد پروموتر انتخابی در تولید زیست داروهای با ارزش افزوده ی بالا می باشد.کلیدواژگان: باکتری Escherichia coli، پپتید YY، پروموتر

صفحات 124-130مقدمهمطالعات نشان داده است در تعداد قابل توجهی از بیماران مبتلا به صرع مقاوم به درمان، منشا بیماری روان شناختی است و با پایش طولانی مدت بیماری، می توان موارد صرع کاذب را از صرع واقعی تفکیک کرد. از این رو، مطالعه ی حاضر با هدف تعیین نقش پایش طولانی مدت در تصحیح الگوهای دارویی و درمان بیماران مبتلا به صرع مقاوم به درمان انجام شد.روش هااین مطالعه، یک مطالعه ی مقطعی بود که بر روی بیماران متبلا به صرع مقاوم به درمان مراجعه کننده به بیمارستان آیت الله کاشانی اصفهان در سال های 95-1393 انجام شد. با استفاده از برنامه ی مراقبت طولانی مدت و با توجه به معیارهای این برنامه، موارد صرع کاذب از صرع واقعی جدا شد و الگوی دارویی در بیماران تصحیح گردید.یافته هادر این مطالعه، 249 بیمار مبتلا به صرع مقاوم به درمان مورد بررسی قرار گرفتند که طبق برنامه ی مراقبت طولانی مدت، 65 نفر (1/26 درصد) تحت درمان طبی قرار گرفتند. 75 نفر (1/30 درصد) به علت مشخص بودن کانون تشنج جهت جراحی آماده شدند. 59 نفر (7/23 درصد) جهت اقدمات تشخیصی تکمیلی به مراکز Magnetic resonance imaging (MRI)، Positron-emission tomography scan (PET scan) و Single-photon emission computed tomography (SPECT) معرفی شدند و 50 نفر (1/20 درصد) نیز به علت ابتلا به صرع کاذب، به روان پزشک ارجاع داده شدند.نتیجه گیریپایش طولانی مدت در بیماران با تشخیص صرع مقاوم به درمان، در افتراق بیماران مبتلا به صرع کاذب بسیار سودمند است و کارایی این روش در تشخیص واقعی بیماری و بهبود بیماران غیر صرعی و حذف هزینه های اضافی درمان و پیش گیری از عوارض دارویی بسیار قابل توجه می باشد.کلیدواژگان: صرع مقاوم به درمان، پایش طولانی مدت، تشنج

صفحات 131-141پروبیوتیک ها یکی از روش های نوین، موثر و پذیرفته شده به منظور تخفیف حدت و کمک به درمان برخی از بیماری ها در عصر حاضر می باشند. با این وجود، محدودیت هایی پیش روی تولید، عرضه و مصرف باکتری های زنده قرار دارد. از جمله روش های جدید که تا حد بسیار زیادی می تواند این محدودیت ها را برطرف کند، استفاده از پاراپروبیوتیک ها است. پاراپروبیوتیک ها، فرم های غیر زنده ی باکتری های پروبیوتیک هستند که به صورت دست نخورده یا سلول شکسته شده و یا عصاره ی سلول قابل استفاده هستند. بهبود برخی از بیماری ها در انسان و حیوانات در صورت مصرف خوراکی یا موضعی پاراپروبیوتیک ها اثبات شده است. در این مقاله، پس از معرفی پاراپروبیوتیک ها، سعی شده است روش های تولید و نیز تاثیر آن ها بر سلامتی انسان بر اساس پژوهش های انجام شده در دو دهه ی گذشته (با تاکید بر مطالعات اخیر) ارایه شود.کلیدواژگان: پاراپروبیوتیک، پروبیوتیک، سیستم ایمنی

Pages 104-110BackgroundIntravenous (IV) administration of fentanyl during induction of general anesthesia often elicits cough, with frequency of 18% to 80%. Therefore, in this study, we aimed to compare the effects of ketamine and dexamethasone on decreasing cough in patients undergoing general anesthesia with fentanyl.MethodsDuring a double-blind randomized clinical trial study, 99 patients aged 18-64 years with class I and II based on the scale of American Society of Anesthesiologists (ASA) who were undergoing general anesthesia with fentanyl were investigated. Patients were randomly divided equally into 3 groups receiving ketamine (0.5 mg/kg), dexamethasone (0.1 mg/kg), and placebo (5 ml normal saline). All drugs were administered 1 minute before administration of 2.5 µg/kg fentanyl. Vital signs, and frequency and intensity of cough were measured and recorded. Statistical analysis was performed using one-way ANOVA and Kruskal-Wallis tests.
Findings: The frequency of cough was significantly lower in the ketamine group (81.8%) compared with dexamethasone (90.9%) and placebo (93.1%) groups (P = 0.03). The intensity of cough was significantly lower in the ketamine group (mild: 18.2%; moderate: 78.8%; severe: 3.0%) compared with placebo group (mild: 9.1%, moderate: 72.7%; severe: 18.2%) (P = 0.04). Furthermore, there were significant differences between the three groups in variations of systolic blood pressure, diastolic blood pressure, and heart rate (PConclusionAdministration of low-dose ketamine can effectively reduce frequency and intensity of fentanyl-induced cough during anesthesia in patients undergoing general anesthesia.Keywords: Cough, Anesthesia, Dexamethasone, Fentanyl, Ketamine -
Pages 111-117BackgroundBlood-injection phobia is a chronic and debilitating disorder in children and adolescents which can be associated with anxiety avoidance of many essential treatments. Although there are some current recommended treatments for blood-injection phobia, little evidence is available on the virtual reality therapy. Therefore, in this study we aimed to investigate the effect of virtual reality therapy on blood-injection phobia among children and adolescents.MethodsIn this quasi-experimental study, 31 children and adolescents aged 6 to 18 years with the diagnosis of blood-injection phobia were investigated. All children received 12 sessions of working with virtual reality software. The children's anxiety level was assessed using Spielberger and specific symptoms of blood-injection phobia questionnaires. Statistical analysis was performed using SPSS software.
Findings: After working with the software, the mean scores of state, trait, and state-trait anxiety and also blood-injection phobia in children were significantly lower than their levels before working with the software (PConclusionVirtual reality therapy can effectively reduce state, trait, and state-trait anxiety and also blood-injection phobia in children and adolescents. Pages 118-123BackgroundApplication of toxic inducers, i.e. isopropyl β-D-1-thiogalactopyranoside (IPTG), for production of biopharmaceuticals has been prohibited by regulatory agencies. Here, we sought to evaluate the feasibility of a self-induced expression system for production of YY(3-36) peptide as an active pharmaceutical ingredient (API) in the Escherichia coli expression system.MethodsThe sequence of self-induced promoter was obtained from Data banks, and synthetized in pUC18 vector. The nucleotide sequence of YY(3-36) was inserted downstream of the promoter, after the 22 residue of asparaginase II signal sequence. The expression of the peptide was compared with pET21 harboring the same construct under isopropyl β-D-1-thiogalactopyranoside induction in sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE).
Findings: The expression of target peptide was comparable with isopropyl β-D-1-thiogalactopyranoside (1 mM) expressed peptide.ConclusionOur results demonstrate the ease and economical application of used self-induced promoter as an alternative system in production of high value added biopharmaceuticals.Keywords: Escherichia coli, Peptide YY, Promoter -
Pages 124-130BackgroundStudies show that a considerable part of intractable epilepsy have psychological cause, and long-term monitoring can separate pseudoseizure from epilepsy. So, this study aimed to detect the effect of long-term monitoring on correction of medication among patients with intractable epilepsy.MethodsIn a cross-sectional study, patients treated as intractable epilepsy in Ayatollah Kashani hospital, Isfahan, Iran, during the years 2016-2016 were enrolled. Using long-term monitoring, and according to criteria of this program, pseudoseizure cases were separated from true epilepsy, and medication pattern was corrected in these patients.
Findings: 249 patients with intractable epilepsy were studied. According to long-term monitoring, in 65 patients (26.1%), the medication was changed, 75 (30.1%) referred for surgery, 59 (23.7%) referred for further assessment with magnetic resonance imaging (MRL), positron-emission tomography (PET) scan, and single-photon emission computed tomography (SPECT), and 50 (20.1) referred to psychologist due to pseudoseizure.ConclusionLong-term monitoring in patients with intractable epilepsy can help to separate of pseudoseizure from true epilepsy. The efficacy of this method was proved in better diagnosis and improvement of patients with non-epileptic seizure, decreasing treatment costs, and prevention of drug side effects.Keywords: Intractable epilepsy, Patient monitoring, Seizure -
Pages 131-141Probiotics are one of the innovative, effective and accepted ways to mitigate acuteness, and to help cure some diseases at the present era. However, there are some limitations in manufacturing and using live bacteria. Paraprobiotics are of the new methods that can handle these problems at the large scale. They are defined as non-viable form of probiotics that can be utilized as intact or broken cells, and also as cell extract. Treatment of some diseases in human and animals has been demonstrated in case of using edible of topical application of paraprobiotics.
