    مقدمه
    عمل جراحی استرابیسم با دست کاری اجزای ساختمانی داخل چشم همراه است که می تواند منجر بروز رفلکس چشمی- قلبی (Oculocardiac reflex یا OCR) گردد. با توجه به اثرات خطرناک رفلکس قلبی- چشمی، مطالعه ی حاضر با هدف بررسی اثرات استفاده از قطره ی چشمی تتراکائین به عنوان قطع کننده ی امواج عصبی موضعی بر رفلکس چشمی- قلبی در جراحی استرابیسم انجام شد.
    روش ها
    این کارآزمایی بالینی، شامل 70 نفر از بیماران تحت جراحی استرابیسم بود که به صورت تصادفی به دو گروه دارونما (اشک مصنوعی) و گروه تتراکائین تقسیم شدند. بلافاصله پس از بیهوشی و قبل از هر گونه مداخله ی جراحی، 3 قطره از محلول مورد بررسی در هر یک از چهار جهت چشم بیماران ریخته شد. برای همه ی بیماران، تعداد ضربان قلب، فشار خون سیستول و دیاستول، میانگین فشار شریانی و تعداد تنفس در زمان های مختلف ثبت گردید. بروز (افت ضربان قلب بیش از 20 درصد) و شدت (خفیف، متوسط و شدید) OCR در مراحل آزادسازی و برش عضله، زمان لازم جهت بهبود رفلکس چشمی- قلبی و دز آتروپین مصرفی، ارزیابی و مقایسه شد.
    یافته ها
    بروز رفلکس چشمی- قلبی در مرحله ی آزادسازی میان دو گروه تتراکائین (8) و گروه دارونما (11) متفاوت نبود (42/0 = P)، اما در مرحله ی برش در گروه دارونما (9) بیشتر از گروه تتراکائین (2) بود (02/0 = P). شدت رفلکس چشمی– قلبی در مرحله ی آزادسازی در میان دو گروه تتراکائین (خفیف = 5، متوسط = 2، شدید = 1) و گروه دارونما (خفیف = 8، متوسط = 0، شدید = 3) متفاوت نبود (44/0 = P)، اما در مرحله ی برش در گروه دارونما (خفیف = 1، متوسط = 8، شدید = 0) بیشتر از گروه تتراکائین (خفیف = 1، متوسط = 1، شدید = 0) بود (02/0 = P). مدت زمان بهبود OCR (74/0 = P) و میزان آتروپین مورد استفاده (92/0 = P) در دو گروه تفاوتی نشان نداد.
    نتیجه گیری
    استفاده از قطره ی تتراکائین موضعی، تنها باعث کاهش بروز و شدت رفلکس چشمی– قلبی در مرحله ی برش جراحی استرابیسم می گردد. برای اظهار نظر قطعی در این زمینه، انجام مطالعات تکمیلی بیشتر توصیه می شود.

    مقدمه
    پژوهش حاضر با هدف بررسی تاثیر آموزش بر آگاهی، نگرش و عملکرد مادران دانش آموزان مقطع ابتدایی منطقه ی گورت اصفهان در خصوص آلودگی به شپش در سال 1396 انجام گرفت.
    روش ها
    در این مطالعه ی کارآزمایی میدانی، 154 نفر از مادران دانش آموزان مقطع ابتدایی به دو گروه 77 نفره تقسیم شدند. گروه اول تحت مداخله ی آموزشی در خصوص آلودگی به شپش قرار گرفتند و در گروه دوم هیچ مداخله ای انجام نشد. سطح آگاهی، نگرش و عملکرد مادران قبل و دو ماه پس از مداخله مورد ارزیابی و مقایسه قرار گرفت.
    یافته ها
    میانگین نمره ی آگاهی، نگرش و عملکرد مادران پس از مداخله در گروه آزمون به ترتیب 62/1 ± 25/11، 47/1 ± 77/18 و 88/0 ± 22/8 و در گروه شاهد به ترتیب 31/1 ± 07/6، 24/1 ± 01/10 و 88/0 ± 93/2 بود. بین دو گروه اختلاف معنی داری در هر سه حیطه ی آگاهی، نگرش و عملکرد وجود داشت.
    نتیجه گیری
    اجرای برنامه ی آموزش بهداشت، نقش موثری در افزایش میزان آگاهی، نگرش و عملکرد مادران در خصوص آلودگی به شپش دارد.

    مقدمه
    سقط جنین مکرر خود به خودی با علت ناشناخته (Unexplained recurrent spontaneous abortion یا URSA) یکی از مشکلات ایمونولوژیکی درطی بارداری است و نقش پاتولوژیک ایمنی سلولی در این بیماران گزارش شده است. مطالعات نشان داده اندکه سلول های اپی تلیال پرده ی آمنیون جفت انسان (Human aortic endothelial cells یا hAECs) دارای توانایی تعدیل کنندگی پاسخ های ایمنی می باشند. هدف از انجام این مطالعه، بررسی اثرات تعدیل کنندگی ایمنی این سلول ها بر روی بیان نشانگر فعالیت CD107a بر سطح سلول های NKT خون محیطی بیماران مبتلا به URSA بود.
    روش ها
    سلول های تک هسته ای خون محیطی (Peripheral blood mononuclear cell یا PBMCs) از خون 14 بیمار مبتلا به URSA با استفاده از شیب غلظت فایکول جدا شدند. بعد از کشت هم زمان PBMCs با hAECs در 3 نسبت مختلف، سلول های NKT (Natural killer T cells یا NKT cells) با استفاده از آنتی بادی های مونوکلونال Anti-CD56 و Anti-CD3 شناسایی شدند و میانگین درصد بیان نشانگر CD107a بر سطح سلول های NKT با استفاده از آنتی بادی های مونوکلونال مربوط و روش فلوسیتومتری ارزیابی گردید.
    یافته ها
    میانگین درصد بیان نشانگر CD107a در سطح سلول های NKT در نسبت های 1 به 1 (031/0 = P) و 5 به 1 (043/0 = P) کشت هم زمان PBMCs با hAECs، به طور معنی داری در مقایسه با گروه شاهد افزایش داشت.
    نتیجه گیری
    بر طبق نتایج به دست آمده، می توان گفت که شاید یکی از مکانیسم های سلول های hAEC جهت اعمال اثرات تعدیل کنندگی ایمنی خود بر روی سایر سلول های ایمنی، اثر بر فعالیت سیتوتوکسیک سلول های NKT است که خود نقش تنظیم کنندگی سیستم ایمنی را دارند. از طرفی، مطالعات گسترده تری در مورد نقش تعدیل کنندگی این سلول ها در بیماران مبتلا به RSA بر روی سلول های NKT و سایر سلول های ایمنی موثر در RSA لازم است تا بتوان این سلول ها را به عنوان یک روش درمانی پیشنهاد کرد.

    مقدمه
    مطالعه ی حاضر با هدف ارزیابی سطح مقطع و ارتشاح چربی عضلات مولتی فیدوس، ارکتور اسپاین و سواس در مهره ی L5 در بیماران مبتلا به کمردرد مزمن در مقایسه با بیماران مبتلا به کمردرد حاد انجام شد.
    روش ها
    این مطالعه ی آزمایشی بر روی 30 بیمار با کمردرد مزمن و 30 بیمار با کمردرد حاد انجام گردید. شاخص توده ی بدنی، نمره ی درد (بر اساس شاخص دیداری)، میزان ناتوانی بیمار (بر اساس شاخص تاتوانی Oswestry)، طول مدت درد، اندازه ی سطح مقطع عضلات مولتی فیدوس، ارکتور اسپاین و سواس و میزان ارتشاح چربی این عضلات در بیماران تحت مطالعه از طریق Magnetic resonance imaging (MRI) اندازه گیری و مقایسه گردید.
    یافته ها
    شاخص توده ی بدنی و نمره ی درد بیماران در دو گروه تحت مطالعه تفاوت معنی دار آماری نداشت (050/0 < P). نمره ی شاخص ناتوانی بیماران در گروه کمردرد حاد، به طور معنی داری از گروه کمردرد مزمن بیشتر بود (007/0 = P). میانگین سطح مقطع عضلات مولتی فیدوس، ارکتور اسپاین و سواس بین دو گروه تفاوت معنی دار آماری نداشت. میانگین میزان ارتشاح چربی عضلات در بیماران با کمردرد مزمن به طور معنی داری از بیماران با کمردرد حاد بیشتر بود (050/0 > P). شاخص توده ی بدنی با سطح مقطع عضله ی مولتی فیدوس (030/0 = P، 280/0 = r) و سطح مقطع عضله ی سواس (036/0 = P، 271/0 = r) ارتباط مستقیم و معنی داری داشت.
    نتیجه گیری
    بیشتر بودن میزان ارتشاح چربی عضلات پارا اسپاینال در بیماران با کمردرد مزمن نسبت به مبتلایان به کمردرد حاد، می تواند نشان دهنده ی اهمیت و نقش آن در مزمن شدن انواع کمردرد باشد.

    مقدمه
    NADPH اکسیداز (Nicotinamide adenine dinucleotide phosphate oxidase یا Nox) یکی از منابع اصلی تولید رادیکال های آزاد اکسیژن در دیواره ی عروق است. اندوتلین-1، یک عامل منقبض کننده ی قوی است که فعالیت میتوژنیک در سلول های عضلات صاف عروق دارد. فعال شدن گیرنده ی Transforming growth factor beta (TGF-β) توسط اندوتلین-1 نقش مهمی در بیماری های قلبی- عروقی ایفا می کند. هدف از انجام این مطالعه، بررسی تغییرات بیان ژن آنزیم Nox در سلول های عضلات صاف تیمار شده با اندوتلین-1 در حضور مهار کننده ی گیرنده ی TGF-β بود.
    روش ها
    سلول های عضلات صاف آئورت انسانی (Human aortic smooth muscle cells یا HA-SMCs) با اندوتلین-1 (100 نانومولار) و TGF-β (2 نانوگرم/میلی لیتر) در حضور و عدم حضور مهار کننده ی گیرنده ی TGF-β تیمار شدند. بیان Messenger RNA (mRNA) مربوط بهNox1 و Nox4 با استفاده از روش Real-time polymerase chain reaction (Real-time PCR) مورد ارزیابی قرار گرفت.
    یافته ها
    بیان ژن Nox1 در حضور اندوتلین-1 در مقایسه با گروه شاهد افزایش یافت، اما در بیان ژن Nox4 تغییری مشاهده نشد. افزایش بیان Nox1 در حضور مهار کننده ی گیرنده ی TGF-β (10 میکرومولار) کاهش یافت.
    نتیجه گیری
    نتایج حاصل از این مطالعه نشان داد که یکی از مکانیسم های افزایش بیان mRNA مربوط به Nox1 توسط اندوتلین-1 از طریق فعال سازی مسیر TGF-β می باشد.

    Background
    Strabismus surgery is accompanied with manipulation of intraocular structures that may lead to oculocardiac reflex (OCR). Due to possible perilous effects of oculocardiac reflex, this study aimed to assess the effects of tetracaine eyedrop as a local nerve impulse cutter on oculocardiac reflex in strabismus surgery.
    Methods
    This clinical-trial study included 70 patients undergone strabismus surgery. They were randomly divided in two groups of placebo (artificial tear) and tetracaine. Immediately after anesthesia and prior to any surgical procedure, three drops of each solution were dropped in four direction of patient's eye. For all patients, heart rate (HR), systolic blood pressure (SBP),diastiloc blood pressure (DBP), mean arterial pressure (MAP), and pulse rate (RR) were assessed in various times. Incidence (20% decrease in heart rate) and severity (mild, moderate, and severe) of oculocardiac reflex in muscle release and cutting, required time for oculocardiac reflex recovery, and atropine dose were compared.
    Findings: Incidence of oculocardiac reflex was not different in two groups in release phase (8 and 11 in tetracaine and placebo groups, respectively; P = 0.42); but it was significantly more in cutting phase (2 and 9 in tetracaine and placebo groups, respectively; P = 0.02). Oculocardiac reflex severity was not different in release phase between the two groups (mild: 5, moderate: 2, and severe: 1 in tetracaine, and mild: 8, moderate: 0, and severe: 3 in placebo group; P = 0.44), but was more in placebo group (mild: 1, moderate: 1, and severe: 0 in tetracaine, and mild: 1, moderate: 8, and severe: 0 in placebo group; P = 0.02). Duration of oculocardiac reflex recovery (P = 0.74) and used atropine dose (P = 0.92) were not different between the two groups.
    Conclusion
    Using local tetracaine eyedrop could merely decrease incidence and severity of oculocardiac reflex in cutting phase of strabismus surgery. For definitive comments, further studies are recommended.

    Background
    The purpose of this study was to investigate the effect of education on knowledge, attitude, and practice in the field of pediculosis among the mothers of elementary school students in Gavart district, Isfahan, Iran, in year 2016.
    Methods
    In this field trial study, 154 mothers of primary school students in Gavart district were selected and randomly divided into two groups of 77. The first group went under education about pediculosis, and in the second group, no intervention was applied. Awareness, attitude, and practice of the mothers were evaluated before and 2 months after the intervention, and were compared between the two groups.
    Findings: The mean scores of knowledge, attitude, and practice after the intervention was 11.25 ± 1.62, 18.77 ± 1.47, and 8.22 ± 0.88 in intervention group, and was 6.07 ± 1.31, 10.01 ± 1.24, and 2.93±0.88, in control group, respectively; and the differences between the two groups were statistically significant.
    Conclusion
    Implementation of the health education program probably has an effective role in increasing awareness, attitude, and practice among mothers regarding infection with lice.

    Background
    Unexplained recurrent spontaneous abortion (URSA) is the most common immunological complication during pregnancy. The evidence of in-vitro assays has shown that immune cells are responsible for embryotoxicity. In recent years, it has been found that cells such as human amnion epithelial cells (hAECs) have the potency to modulate immune responses in vitro and in vivo. This study aimed to investigate the immunomodulatory effects of human amnion epithelial cells on expression of CD107a marker on natural killer T (NKT) cells in the peripheral blood of the women with unexplained recurrent spontaneous abortion.
    Methods
    Peripheral blood mononuclear cells (PBMC) were obtained from 14 patients. After co-culture of human amnion epithelial cells with peripheral blood mononuclear cells, natural killer T cells were identified using anti-CD56 and anti-CD3 monoclonal antibodies (mAb); and the expression of CD107a marker on natural killer T cells was investigated using specific mAb analyzed by flow cytometry.
    Findings: The expression of CD107a marker was significantly upregulated on natural killer T cells following incubation with human amnion epithelial cells (P = 0.031 in 1:1 ratio; P = 0.043 in 5:1 ratio).
    Conclusion
    Our results suggest human amnion epithelial cells have immune regulatory effects on activation and cytotoxicity of natural killer T cells. Potential therapeutic application of human amnion epithelial cells for dysregulated immunity of natural killer T cells in patients with recurrent spontaneous abortion should be investigated in the future.

    Background
    This study aimed to evaluate the cross-sectional area (CSA) and fat infiltration of paraspinal muscles in the level of L5 in patients with chronic and acute low back pain (LBP).
    Methods
    This pilot study was done on 30 patients with acute and 30 with chronic low back pain. Body mass index (BMI), pain score (using visual analog scale), patients’ disability (using Oswestry Disability Index), duration of pain, cross-sectional area of paraspinal muscles, and fat infiltration in multifidus, erector spinae, and psoas muscles (using magnetic resonance imaging) were measured and compared in the two groups of patients.
    Findings: Patients body mass index and pain score were not significantly different between studied groups (P > 0.050 for both). Patients’ disability index score was significantly higher in acute low back pain group than chronic group (P = 0.007). The mean cross-sectional area in multifidus, erector spinae, and psoas muscles was not significantly different between the groups (P > 0.050 for both). The mean muscles fat infiltration was significantly higher in chronic low back pain than the acute grroup (P
    Conclusion
    The higher rate of fat infiltration of paraspinal muscles in patients with chronic low back pain compared to those with acute pain may show its potential role in chronic low back pain.

    Background
    Nicotinamide adenine dinucleotide phosphate (NADPH) oxidase (Nox) is one of the predominant sources of the production of free oxygen radicals in the vascular wall. Endothelin-1 is a potent vasoconstrictor factor that also has mitogenic activity in vascular smooth muscle cells. The activation of transforming growth factor beta (TGF–β) receptor by endothelin-1 plays an important role in cardiovascular diseases. The aim of this study was to investigate the changes of Nox gene expression in smooth muscle cells treated with endothelin-1 in the presence of the transforming growth factor beta-receptor antagonist.
    Methods
    Human aortic smooth muscle cells (HA-SMCs) were treated with endothelin-1 (100 nM) and transforming growth factor beta (2 ng/ml) in the presence and absence of transforming growth factor beta-receptor antagonist. The mRNA expression of Nox1 and Nox4 were assessed using real-time polymerase chain reaction (PCR) technique.
    Findings: The gene expression of Nox1 increased in the presence of endothelin-1 compared to control group, but there was no change in gene expression of Nox4. Increasing the expression of Nox1 decreased in the presence of transforming growth factor beta-receptor antagonist (10 μM).
    Conclusion
    The results of this study showed that activation of transforming growth factor beta pathway is one of the mechanisms for increasing the mRNA expression of Nox1 by endothelin-1.
