صفحات 460-465مقدمهپس از وقوع تروما، معاینات بالینی و روش های آزمایشگاهی در جهت ارزیابی تروما می توانند یک نمای کلی از پیش آگهی بیماران را نشان دهند که در جهت مدیریت بهینه ی بیماران اهمیت زیادی دارد. این طرح پژوهشی، با هدف بررسی و مقایسه ی دو سیستم نمره دهی Sequential organ failure assessment (SOFA) و Acute physiology and chronic health evaluation II (APACHE II) از نظر توان پیش بینی مرگ و میر بیماران دچار ترومای بستری در Intensive care unit (ICU) انجام شد.روش هادر این مطالعه ی مقطعی- توصیفی از نوع گذشته نگر، 100 بیمار دچار تروما که در ICU بستری شده بودند، به دو گروه فوت شده و گروه زنده تقسیم بندی شدند. اطلاعات دموگرافیک جهت محاسبه ی امتیازهای APACHE و SOFA از پرونده ی بیماران استخراج شد و اطلاعات این تحقیق در دو گروه بررسی و مقایسه گردید.یافته هامیانگین سیستم نمره دهی SOFA در گروه زنده و فوت شده به ترتیب 19/6 و 56/15 و میانگین APACHE II در گروه زنده و فوت شده به ترتیب 26/21 و 87/46 بود و همچنین، میانگین نمره ی SOFA و APACHE II در گروه فوت شده به صورت معنی داری بالاتر از گروه زنده بود (001/0 > P). میزان حساسیت (Sensitivity) APACHE II و SOFA به ترتیب 4/97 و 4/97 درصد و میزان ویژگی (Specificity) آن ها به ترتیب 1/36 و 4/16 درصد بود.نتیجه گیریاستفاده از دو سیستم SOFA و APACHE II در جهت پیش بینی مرگ و میر در بیماران دچار ترومای بستری در ICU بسیار خوب و موثر است، اما میزان حساسیت APACHE II و SOFA شبیه به هم و میزان ویژگی APACHE II بهتر از SOFA است.کلیدواژگان: تروما، بخش مراقبت های ویژه، APACHE II، SOFA

صفحات 466-471مقدمههدف از انجام این مطالعه، پیش بینی و مقایسه ی توزیع دز در اعضای حیاتی سالم در پرتودرمانی سرطان بیضه از نوع سمینومای مرحله ی 1 برای دز یکسان به حجم هدف در دو روش دو بعدی (2 Dimensional) مرسوم و سه بعدی (3 Dimensional) تطبیقی بود.روش هادرمان دو بعدی بر اساس آناتومی استخوانی و درمان سه بعدی بر اساس آناتومی عروقی و غددی برای 10 بیمار با پرتو فوتون 6 مگاالکترون ولت و دز 25 گری طراحی شد. توزیع دز در اعضای داخل و خارج میدان به ترتیب محاسبه (با سیستم طراحی درمان) و اندازه گیری (با سیستم پروفایلر) شد.یافته هابرای اعضای داخل میدان تابشی، میانگین دز کلیه ها در درمان دو بعدی و سه بعدی به ترتیب 58/5 و 16/7 گری و به همین ترتیب برای پانکراس 67/20 و 29/19، کبد 20/5 و 19/3، معده 73/9 و 43/6 و کولون 96/5 و 6/7 گری بود. برای اعضای خارج میدان تابشی، میانگین دز دریافتی کلیه ی راست 55/1 گری و کلیه ی چپ 48/1 گری بود. همچنین، میانگین دز بیضه برای درمان دو بعدی 7 سانتی گری و برای درمان سه بعدی 6 سانتی گری به دست آمد.نتیجه گیریدر درمان سه بعدی، به علت طراحی دقیق تر حجم هدف، احتمال عود بیماری کمتر و احتمال آسیب به کلیه و کولون بیشتر پیش بینی شد.کلیدواژگان: عضو حیاتی، دز، آسیب پرتوی، پرتودرمانی تطبیقی، سمینوما

صفحات 472-477مقدمهیکی از حیطه های مهندسی بافت، ساخت جایگزین ها و زخم پوش های پوستی است. از آن جایی که یون مس یکی از مهم ترین عوامل مورد نیاز در تولید ماتریکس خارج سلولی پوست است، در این تحقیق اثر زخم پوش کلاژن- ژلاتین- شیشه ی زیستی حاوی مس در تسریع روند بهبود زخم تمام ضخامت در موش بررسی شد.روش هااین مطالعه بر روی 12 سر موش کوچک آزمایشگاهی در دو گروه انجام شد که بر پشت هر یک 2 زخم یکسان (با قطر 5 میلی متر و تمام ضخامت) ایجاد گردید. یک زخم به عنوان شاهد بدون هیچ درمانی و زخم دیگر در یک گروه با زخم پوش کلاژن- ژلاتین- شیشه ی زیستی و در گروه دیگر با زخم پوش کلاژن- ژلاتین- شیشه ی زیستی حاوی مس پانسمان شد. 14 روز پس از ایجاد مدل زخم، میزان بهبودی ماکروسکوپی و میکروسکوپی (پس از بیوپسی با رنگ آمیزی هماتوکسیلین و ائوزین) بررسی شد.یافته هازخم پوش ها ساختاری متخلخل داشتند و زیست سازگار بودند و باعث بهبود ترمیم زخم تمام ضخامت موش گردیدند. میزان ترمیم در زخم پوش حاوی مس با گروه دیگر معنی دار بود (05/0 > P).نتیجه گیریاز آن جایی که زخم پوش زیست سازگار کلاژن- ژلاتین- شیشه ی زیستی حاوی مس به بهبود فرایند ترمیم زخم در مدل حیوانی (موش) منجر شده است، می تواند به عنوان راهبرد جدیدی در طراحی و ساخت زخم پوش با کاربرد انسانی پیشنهاد شود.کلیدواژگان: زخم پوش، مهندسی بافت، شیشه ی زیستی، مس

صفحات 478-484مقدمهاین مطالعه با هدف مقایسه ی چهار روش هم صفحه (Coplanar) و غیر هم صفحه (Noncoplanar) با روش پرتودرمانی تعدیل یافته (Intensity modulated radiation therapy یا IMRT) در تومورهای Glioblastoma multiforme جهت دستیابی به یک روش بهینه ی درمان، انجام شد.روش هاکانتورینگ و طراحی درمان بر روی Computed tomography scan (CT scan) 20 بیمار در دو مرحله ی جداگانه انجام شد. چهار روش هم صفحه و غیر هم صفحه ی IMRT در دو انرژی 6 و 15 مگاولت با هم مقایسه شدند. سرانجام، با مقایسه ی پارامترهای دزیمتری همانند شاخص انطباق و شاخص یکنواختی و دز بیشینه و میانگین، طراحی درمان بهینه ارایه گردید.یافته هانتایج حاصل از چهار روش نشان داد که دز دریافتی اعضای بحرانی در روش های غیر هم صفحه نسبت به روش های هم صفحه کاهش معنی دار و قابل توجهی می یابد. افزایش انرژی در روش های غیر هم صفحه تفاوت قابل توجهی ایجاد نکرد.نتیجه گیریدز دریافتی تمام اعضای بحرانی در روش غیر هم صفحه ی IMRT با انرژی 6 مگاولت کاهش قابل توجه و معنی داری یافت و همچنین، در این روش به دلیل استفاده از انرژی 6 مگاولت، آلودگی نوترونی وجود نداشت.کلیدواژگان: برنامه ی درمان، Glioblastoma multiforme، پرتودرمانی

صفحات 485-493مقدمههدف از انجام پژوهش حاضر، بررسی اثربخشی کیفیت زندگی درمانی بر بهبود روابط با همکاران و کاهش تعارضات کار- خانواده کارکنان مرکز آموزشی- درمانی الزهرای (س) شهر اصفهان بود.روش هااین مطالعه به روش نیمه آزمایشی با طرح پیش آزمون، پس آزمون و پیگیری و همراه با گروه شاهد انجام شد. جامعه ی آماری پژوهش را کلیه کارکنان مرکز آموزشی- درمانی الزهرای (س) شهر اصفهان تشکیل داد. نمونه ها شامل 30 نفر از کارکنان مرکز آموزشی- درمانی الزهرا (س) بود که به صورت تصادفی در دو گروه آزمایش (15 نفر) و شاهد (15 نفر) قرار گرفتند. گروه آزمایش طی 9 جلسه 90 دقیقه ای، آموزش کیفیت زندگی درمانی را دریافت کردند؛ در حالی که گروه شاهد در لیست انتظار قرار داشت. داده ها با کمک پرسش نامه های روابط با همکاران و تعارض کار- خانواده جمع آوری گردید و سپس با استفاده از آمار توصیفی (میانگین و انحراف معیار) و استنباطی (آزمون Repeated measures ANCOVA) در نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفت.یافته هاآموزش کیفیت زندگی درمانی در بهبود نمرات روابط همکاران (198/33 = F، 001/0 > P) و تعارضات کار- خانواده (480/10 = F و 001/0 > P) گروه آزمایش در مراحل پس آزمون و پیگیری نسبت به گروه شاهد در طول زمان موثر بود.نتیجه گیریکیفیت زندگی درمانی تاثیر معنی داری بر بهبود روابط با همکاران و کاهش تعارضات کار- خانواده کارکنان دارد. بدین ترتیب می توان نتیجه گرفت که کیفیت زندگی درمانی جهت بهبود روابط با همکاران و کاهش تعارضات کار- خانواده موثر است.کلیدواژگان: روابط با همکاران، کار، خانواده، تعارضات، کیفیت زندگی درمانی

Pages 460-465BackgroundAfter trauma, clinical examinations and laboratory tests to assess trauma can provide an overview of patient's prognosis, which is important for the optimal management of patients. In this research project, we decided to compare two scoring systems of Sequential Organ Failure Assessment (SOFA) and Acute Physiology and Chronic Health Evaluation II (APACHE II) for predicting the mortality rate of patients with trauma hospitalized in intensive care unit (ICU).MethodsIn this retrospective cross-sectional descriptive study, 100 patients with trauma admitted to the intensive care unit were divided into two groups of death and living patients. Demographic data were extracted from patient's records for calculating APACHE and SOFA scores. The data of this study were compared between the two groups.
Findings: The mean SOFA score was 6.19 and 15.56, and mean APACHE II score was 21.26 and 46.87 in living and death groups, respectively; the mean scores of SOFA and APACHE II in the death group was significantly higher than the living group (PConclusionThe SOFA and APACHE II systems are good and effective to predict mortality in patients with trauma hospitalized in intensive care unit. Pages 466-471BackgroundThe aim of this study was to predict and compare the dose received by normal critical organs in radiotherapy of stage one seminoma in conventional (2-dimensional or 2D) and conformal (3-dimensional or 3D) methods.MethodsConventional treatment was designed based on bony anatomy, and conformal treatment based on vessels and nodal anatomy, for 10 patients with 6-MV photon and total dose of 25 Gy. Dose distribution of inside and outside of field was measured (with the profiler system) and calculated (with a treatment planning system).
Findings: Mean received dose was 5.58 and 7.16, 20.67 and 19.29, 5.20 and 3.19, 9.73 and 6.43, 5.96 and 7.60 Gy for kidneys, pancreas, liver, stomach, and colon in conventional and conformal methods, respectively. For organs out of the field, mean received dose was 1.55 and 1.48 Gy for right and left kidneys in conventional and conformal methods, respectively. Moreover, the mean received dose by testis was 7 and 6 cGy in conventional and conformal methods, respectively.ConclusionIn conformal treatment method, due to more accurate design of target volume, probability of recurrence was less, and the damage to kidneys and colon was predicted more.Keywords: Organs at risk, Radiotherapy dosage, Radiation injuries, Radiotherapy, Seminoma -
Pages 472-477BackgroundProduction of skin dressings or substitutes is one of the most important tissue engineering fields. Since copper is an important agent in skin extracellular matrix synthesis, we investigated the effect of collagen-gelatin-bioglass scaffolds containing copper in accelerating the healing process of full-thickness skin wound in mouse model.MethodsWe used 12 mice with two identical skin wounds on their back (with 5 mm diameter, circular, and full-thickness), one wound as control, second dressed with collagen-gelatin-bioglass scaffold, and third dressed with collagen-gelatin-bioglass scaffold containing copper. After 14 days, the wound healing process was analyzed using both macroscopic and microscopic (after hematoxylin and eosin staining) methods.
Findings: The wound dressings had porous structure, and were biocompatible. They improved the healing process of full-thickness wound in mouse model. This process was statistically much better in the dressings comprising copper (PConclusionSince wounds dressed with collagen-gelatin-bioglass scaffold containing copper improved wound healing in animal models, it can be suggested as a new approach in design and synthesis of wound dressings for human.Keywords: Wound dressing, Tissue engineering, Bioglass, Copper -
Pages 478-484BackgroundThis study aimed to make a comparison among 4 coplanar and noncoplanar intensity-modulated radiation therapy (IMRT) techniques in glioblastoma multiforme (GBM) tumors, to achieve an optimal treatment technique considering the absorbed doses of organs at risk.MethodsTreatment planning and contouring were performed on 20 patients with glioblastoma multiforme tumors in 2 phases based on Radiation Therapy Oncology Group (RTOG) protocol. Four coplanar and noncoplanar intensity-modulated radiation therapy techniques at 6 and 15 MV energy levels were compared. Finally, comparing the conformity and homogeneity indices, and maximum and mean doses (using dose volume histogram), the optimal treatment technique was proposed.
Findings: The absorbed doses of critical organs in the cases of noncoplanar techniques reduced drastically compared with the cases of coplanar techniques. Increase in energy levels in noncoplanar techniques did not impose any significant changes.ConclusionThe absorbed doses of all critical organs in noncoplanar technique with 6 MV energy level reduced significantly. Pages 485-493BackgroundThe main objective of this study was to evaluate the effectiveness of quality of life therapy on improving relationships with coworkers, and reducing work-family conflicts among the personnel in Alzahra Medical Education Center, Isfahan Iran.MethodsThis was a quasi-experimental study with pretest, posttest, and follow-up method and control group. The statistical population of this research included all employees of Alzahra in Isfahan. The sample consisted of 30 employees who were selected non-randomly, and randomly assigned to two equal groups of experimental and control. The experimental group received 9 treatment sessions of one and a half hours, under the training of quality of life therapy, while the control group did not receive any treatment. In the pretest, posttest, and follow-up period, both groups completed the relationships with coworkers scale, and the 18-item questionnaire on work-family conflict. The collected data were analyzed using descriptive (means and standard deviations) and inferential statistics (repeated measures analysis of variance).
Findings: The quality of life therapy significantly improved the relationships with coworkers (F = 33.198; PConclusionThe results showed that quality of life therapy has an effect on improving relationships with coworkers, and reducing work-family conflicts.Keywords: Personal communication, Work, Family, Conflict, Quality of life
