    استئوآرتریت یک اختلال مفصلی مزمن و سیستمیک است که می تواند هر مفصلی را در بدن درگیر کند ولی بیشتر، مفاصل تحمل کننده وزن اندام پایینی و دست را درگیر می کند. شیوع این بیماری در انسان با بالا رفتن سن و چاقی افزایش می یابد. تغییرات بیومکانیکی و بیولوژیکی که به دنبال آسیب های زانو رخ می دهند، در توسعه استئوآرتریت پس ضربه ای دخیل هستند و به عنوان یک بیماری تحلیل برنده مفاصل، سبب ایجاد تخریب غیرقابل بازگشت غضروف هیالین می شود. که با ایجاد درد دائم و ازبین بردن عملکرد مفصل، کیفیت زندگی بیمار را پایین می آورد.
    برای درک بهتر چگونگی پیشرفت استئوآرتریت، به ویژه در مراحل اولیه و نیز ارزیابی اثر داروها و روش های درمانی جدید، نیاز به مدل سازی آن در حیوانات می باشد. این مدل ها می توانند خود به خودی و یا القائی باشند. استئوآرتریت خود به خودی می تواند به صورت طبیعی در موش های نژاد Str-Ort که به صورت ژنتیکی دست کاری شده اند، رخ دهد. تزریق داخل مفصلی مواد شیمیایی نظیر منوآیودی استات نیز می تواند سبب القاء استئوآرتریت گردد.
    همچنین امروزه به صورت گسترده از مدل سازی های جراحی، برای القاء استئوآرتریت استفاده می شود که همگی ناپایداری مفصل را برای ایجاد این بیماری نشانه می روند. خوکچه ها معمولا برای تحقیق درباره ی غضروف مفصلی مورد استفاده قرار می گیرند و یک مدل معمول برای مطالعه روند بازسازی نقیصه غضروفی کانونی می باشند. پیشرفت شرایط تجربی القاء استئوآرتریت مشابه روند طبیعی این بیماری ناشی از بی ثباتی مفصل است و پاتولوژی گراس نهایی نیز دقیقا مشابه می باشد.
    روش هایی که امروزه برای ایجاد استئوآرتریت استفاده می شود شامل برداشت منیسک جانبی و قطع رباط صلیبی قدامی است؛ که به نظر می رسد این روش ها باعث التهابات کمتری شده و تنها یک بی ثباتی مفصلی خفیف ایجاد می کنند که نه تنها ضایعات تکرارپذیری را در مدل ایجاد می کند بلکه ضایعات ایجاد شده مشابه ضایعات استئوآرتریت در انسان است. این روش امکان ارزیابی دژنراسیون غضروفی را به وسیله رادیوگرافی، بافت شناسی و آنالیز ملکولی فراهم می کند.

    نمرات نهایی هریس هیپ خوب یا عالی بودند (78%). یک مورد جابه جایی مشاهده شد.
    در مواقع ضروری، میتوان از ترکیب کاپ سیمان در پوسته بدون سیمان بدون مشکلات و عوارض مهم استفاده شود و نتایج قابل قبولی را در کوتاه مدت با پیگیری متوسط انتظار داشت.

    روش ترجیحی در تثبیت برخی شکستگی ها در استخوان های بلند استفاده از میله های داخل مغز استخوانی به صورت بسته می باشد. مزیت آن در قدرت بیومکانیکال بیشتر و حفظ کردن محل شکستگی از ترومای اضافی ناشی از جراحی است. با وجود این مزایا (به دلیل وقت گیر، در معرض اشعه بودند و خسته کننده بودن آن) تعدادکمی از جراحان این روش را انتخاب می نمایند. هدف این مقاله معرفی یک وسیله ابداعی برای کارگذاشتن سریع میله داخل استخوانی و با کاهش استفاده از اشعه ایکس است. هفت بیمار مرد دارای شکستگی فمور با میانگین سنی 32-28 سال نیازمند فیکساسیون با میله های اینترلاک با استفاده از ست ابداعی عمل جراحی شدند. متغیرهای مدت زمان عمل، نوع شکستگی و محل شکستگی مورد ارزیابی قرار گرفتند. عمل جراحی تمامی بیماران با شکستگی بسته دیافیز فمور بدون هیچ گونه شکستگی همراه، در مدت زمان کل عمل جراحی با میانگین 116. 57 دقیقه انجام شد. میانگین زمان مصرفی ست ابداعی از شروع اتصال قطعات ست ابداعی به استخوان و جااندازی و عبور گاید از محل شکستگی و رسیدن گاید به ته کانال دیستال 21. 85 دقیقه بود. طول و راستای صحیح در تمام موارد به دست آمد.

    شکستگی مچ پا حدود 10% کل شکستگی های اندام تحتانی را شامل می شود که با توجه به اهمیت این مفصل در راه رفتن و تحمل وزن نتایج درمانی شکستگی های این مفصل از اهمیت بالایی برخوردار است. هدف پژوهش حاضر بررسی‏ نتایج درمان شکستگی های مچ پا با روش جااندازی باز و فیکساسیون داخلی است. ‏
    پژوهش حاضر از نوع عرضی بود. 169 بیمار با شکستگی های مچ پا که تحت درمان جراحی با روش جااندازی باز و فیکساسیون داخلی بودند نمونه آماری پژوهش را تشکیل دادند. بیماران توسط سیستم امتیاز دهی کلینیکی مچ پا و هیندفوت مورد ارزیابی و امتیازدهی قرار گرفتند. و براساس تقسیم بندی وبر به سه گروه A وB وC تقسیم شدند.
    از 169 نمونه 53 نفر زن و 116 نفر مرد بود. شایع ترین نوع شکستگی بای مالئول بود و اکثر بیماران از اختلال در عملکرد روزانه و درد رنج می بردند. همچنین الگوی راه رفتن آنان دچار تغییر شده بود. و براساس تقسیم بندی دنیس وبر بیشترین میزان شکستگی در تیپ B بود (45%). میانگین امتیاز کلینیکی مچ پا و هیندفوت 51 (49-53 ‏CI‏ 95%) بود و میانگین نمره گروه B 47 بود که نسبت به گروه A و C کمتر است.

    تاکنون مطالعات مختلفی در مورد اثرات موضعی و سیستمیک سخت افزارهای ارتوپدی انجام شده است. هدف از این مطالعه بررسی واکنش های پاتولوژیک نسبت به کارگذاری سخت افزارهای ارتوپدی (نیل و پلاک) در بیماران بود.
    در یک مطالعه توصیفی، واکنش های پاتولوژیک 15 بیمار (12 مرد و 3 زن) با میانگین سنی 41/37 که تحت جراحی درآوردن سخت افزارهای کارگزاری شده ارتوپدی در دو بیمارستان در شهر ساری قرار گرفته بودند از طریق چک لیست، مشاهده و آزمایش پاتولوژی مورد بررسی قرار گرفت.
    واکنش پوستی بعد از گذاشتن سخت افزار در هیچ کدام از بیماران مشاهده نشد. تنها در 66/6 % از بیماران عفونت بعد از گذاشتن سخت افزار مشاهده شد و جوش نخوردن نیز در یک مورد در بیماران دیده شد. عارضه سیستماتیک نیز در هیچ کدام از بیماران مورد مطالعه دیده نشد. لیکن اکثریت بیماران التهاب بافتی متوسط در محل کارگذاری سخت افزار داشتند. شواهدی دال بر بدخیمی نیز در هیچ کدام از نمونه های پاتولوژی مربوط به بیماران مشاهده نشد.

    شکستگی های داخل مفصلی دیستال فمور (DFIAF) می توانند بسیار چالش برانگیز باشند. اگرچه نتایج خوبی از درمان این آسیب ها گزارش شده است اما همچنان مشکلات متعددی برای این بیماران باقی می ماند. ما در این مطالعه گذشته نگر به بررسی نتایج درمان DFIAF با روش جراحی باز و فیکساسیون داخلی (ORIF) با استفاد از پلاک قفل شونده (LCP) پرداختیم.
    در این مطالعه که در سال 1396 انجام شد، 51 بیمار که طی سال های 1393 و 1394 بدلیل DFIAF تحت ORIF قرار گرفته بودند، بررسی شدند. در ویزیت نهایی پرسشنامه «لی شلم» برای بیماران تکمیل شد. شدت درد و میزان رضایت بیماران با استفاده از معیار خطی بصری (VAS) اندازه گیری شد. همچنین بروز تغییرات دژنراتیو در مفصل ارزیابی گردید. دامنه حرکتی هر دو زانو اندازه گیری و با هم مقایسه شد. بیماران به طور میانگین 2/7±4/17 ماه (دامنه 27-13) پیگیری شدند.

    کنترل کافی و مناسب درد بعد از اعمال جراحی آرتروپلاستی مفصل ران و زانو می تواند یک چالش بزرگ باشد (1-2). مطالعات نشان داده که بیشتر از 50 درصد از بیماران که تحت این نوع جراحی قرار می گیرند از درد شدید و قابل توجه بعد از عمل شکایت دارند ((3،4و توتال نی درد بیشتری نسبت به توتال هیپ دارد. درد بعد از عمل درصورتی که کنترل نشود ممکن است منجربه پیدایش عواقب و عوارض بسیاری شود: از قبیل ایسکمی میوکارد، اختلال درعمل ریه، ایلئوس پارالیتیک، رتانسیون ادراری، عوارض ترومبو آمبولی، عفونت در اثر نقص و اختلال سیستم ایمنی، تشویش و اضطراب و بالاخره مزمن شدن درد چنانچه درد بیشتر از سه ماه طول بکشد. (Chou R، Shekellp Jama،2010) بعلاوه کنترل ناقص و ناکافی درد می تواند عدم رضایت بیمار از جراحی، اشکال در توان بخشی بیمار و بستری شدن طولانی را به همراه داشته باشد (3). تاثیر منفی درد در توانبخشی سبب تاخیر و اختلال در بازگشت فانکشن و قدرت عضلات و برگشت به فعالیت معمول بیمار می گردد. مطالعات زیادی نشان داده که در توتال نی در مقایسه با توتال هیپ به دلیل درد بیشتر زمان بهبودی و برگشت به فعالیت، طولانی تر بوده و حدود 50روز طول می کشد (5) لذا کنترل درد در آرتروپلاستی زانو بواسطه بهبود دامنه حرکتی و قدرت عضلات اهمیت بیشتری نسبت به آرتروپلاستی هپ دارد(

    Osteoarthritis is a chronic systemic disease that can involve any joint in the body, but more often lower limb bearing joints and hand joints. The prevalence of this disease in humans increases with age and obesity. Biomechanical and biological changes that occur after knee injuries are involved in the development of post-traumatic osteoarthritis and, as a degenerative joint disease, cause irreversible hyaline cartilage destruction; which reduces the quality of life by causing permanent pain and joint dysfunction.
    In order to better understand how osteoarthritis progresses, it needs to be modeled in animals; especially in the early stages of the disease, as well as the evaluation of the effects of new drugs and therapies. These models can be spontaneous or inductive. Spontaneous osteoarthritis can occur naturally in genetically engineered strains of rats. Intra-articular injection of chemicals such as mono-iodiacetate can also induce osteoarthritis. Today, many surgical modelings are also used to induce osteoarthritis, all of which indicate joint instability to cause the disease. Guinea pigs are commonly used for research on articular cartilage and are common models for studying the reconstruction process of focal cartilage defects. The development of experimental conditions inducing osteoarthritis is similar to the natural course of the disease in human due to joint instability and the final gross pathology is exactly the same.
    The recent used methods to modeling osteoarthritis include the removal of lateral meniscus and anterior cruciate ligament transection, which appear to reduce inflammation and only create a mild joint instability; that way not only creates repeatable lesions in the model, but also the same kind as the lesions of osteoarthritis in humans. This method allows for the evaluation of cartilage degeneration by radiography, histology and molecular analysis.

    Introduction
    In some special situations during the revision total hip arthroplasty with well fixed, well oriented acetabular cementless cup, orthopedic surgeons may prefer to retain the well-fixed cementless cup. The aim of this study was to report the follow-up of some cases treated with such a technique during a 10-year period.
    Methods
    During 2004-2014, all cases of revision total hip surgery performed in Sina Hospital, Tehran, Iran were evaluated. 14 of 82 cases (16.5%) had cemented polyethylene liner in cementless metal shell. The demographic data, reasons for use of such revision technique result was evaluated by Harris Hip Score.
    Results
    The most common reason was osteolysis (64%), and the most common cause was impaired locking mechanism (65%). There were few complications (one osteolysis and one infection). In five cases (36%) constrained liner was added and in 10 cases (72%) allograft was needed. The final Harris Hip Scores was good or excellent in 78%. Only one dislocation was seen.
    Conclusion
    Cemented cup in cementless shell can be used in revision hip arthroplasty in well-fixed Acetabular shell situations with acceptable outcom.

    Introduction

    The Preferential method in Fixation of some long bone fractures consists of using intramedullary nailing. The benefit of this method lies in its better biomechanical stability, as well as preventing additional surgical trauma to the fracture site. Despite these benefits, few surgeons conduct this method as it is time-consuming, risky (Performing x- ray fluoroscopy) and also tedious.

    Method

    Seven male patients with diaphyseal femoral fractures, with an average age of 32.28, underwent closed femoral nailing surgery making use of the innovative set. The duration of operation, fracture type and fracture site were evaluated.
    The innovative solution: To align the bone fragments and make the guidance stick cross the fracture site closely, the present close reduction set was designed to make crossing the guidance stick through the fracture site possible on the normal bed of operation room in a short period of time without fluoroscopy, with the least possible extra trauma and the least degree of annoyance in the reduction stage.

    Results

    All the patients with diaphyseal femoral fracture, without any peripheral fractures, underwent nailing surgery, while total time of the surgery was 116.57 minutes on average; and pure time of the innovative set application and passing guidance stick to the bottom of the distal canal was 21.85 minutes.

    Conclusion

    The results indicated that closed intramedullary nailing of femoral fractures without performing Fluoroscope, using the aforementioned innovative set is possible for the patients with utter convenience in an appropriate time period.

    Background
    The outcome of ankle fractures treatment is great importance. The purpose of the present study was to investigate the outcome of ankle fractures treatment by open reduction method and internal fixation.
    Material and
    Methods
    The method of study was cross-sectional. A total of 169 patients with ankle fracture who underwent open surgical and internal fixation surgery were the statistical sample. Patients were evaluated by a Clinical rating system for the ankle and hindfoot and eventually gave them a point from 100.
    Results
    Of the 169 samples, 53 were female and 116 were male. The average a Clinical rating system for the ankle and hindfoot was 51 (95% CI 49-53). The most common fractures type was bimalleoalr fracture. The majority of patients suffering from daily dysfunction and pain, their gait pattern was also changed.
    Conclusions
    According to the results and available studies, the results of surgical treatment of ankle fractures were not optimal and further investigation and finding more appropriate intervention methods were needed in order to obtain the desired results. To achieve optimal surgical outcome, it is recommended that the time interval between injury and surgery should not exceed 4 days.

    Backgrounds
    Various studies have been conducted on the local and systemic effects of orthopedic hardware. This study aimed to evaluate the pathologic reactions to orthopedic hardware (nail and plate) in patients.
    Methods
    In this descriptive study, the pathologic reactions of 15 patients (12 males and 3 females) with a mean age of 37.41 years, who underwent orthopaedic hardware removal surgery in two Hospitals in Sari, Iran, were assessed through observation, laboratory tissue evaluation, and a questionnaire.
    Results
    In this research, no allergic reaction was observed in the patients after the insertion of hardware. In this regard, 6.66% of patients reported infection after insertion of hardware, and there was one case of non::union::. Furthermore, no systematic complication was observed in any of the patients. The majority of patients had moderate tissue inflammation at the hardware site, and there was no evidence of malignancy in any of the pathology samples.
    Conclusion
    According to the results of the research, a moderate level of tissue inflammation might be observed at the site of orthopedic hardware placement.

    Background
    Distal femoral intra-articular fractures (DFIAF) can be very challenging. Although, ýsatisfactory outcomes have been reported, however, there are several ýcomplications interfering the patients after treatment. In current retrospective ýstudy, the outcomes of treating DFIAF with open reduction internal fixation ýý(ORIF) using locking plates (LCP) were investigatedý.
    Material and
    Methods
    There were 51 patients enrolled. In last visit, Lysholm score was completed for all ýof the patients. Pain intensity and satisfaction were measured using visual ýanalogue scale (VAS). Degenerative changes were assessed on plain x-rays. ýBilateral knee range of motion was measured and compared. The patients were ýfollowed for 17.4±7.2 monthsý.
    Results
    Fractures were united in all of the patients. Degenerative changes were found in ýý15 patients (29.4%). The pain intensity was 2.7±1.3. Nineteen patients required to ýuse analgesics (37.2%). VAS for satisfaction averaged 7.7±1.1. The Lysholm score ýaveraged 81.8±8.3. The range of knee motion was significantly limited inn ýoperated side compared to the healthy one (116.6±12.4 Vs 125.3±7.3; p
    Conclusions
    Treatment of DFIAF using ORIF can be associated with complete fracture healing. ýHowever, the incidence of degenerative changes and consequent complications ýsuch as pain and loss of ROM are considerably concerning.ý

    Adequate control of postoperative Pain following Hip and knee arthroplasty can be challenging Task. Previous studies have shown that over 50% of Patients undergoing surgery, report Postoperative pain as a major concern. TKA patients complain of more pain than THA patients. Consequences of uncontrolled pain can lead to myocardial Ischemia and Infarction, pulmonary infections, paralytic Ileus, Urinary retention, thromboembolism, impaired immune function as well as anxiety and chronic postsurgical pain. Inadequate control of pain may result in patient dissatisfaction, impaired patient rehabilitation, and prolonged hospitalization. Functional recovery and return of muscle strength dependant on the ability of these patients to comply with rehabilitation, and pain control can provide the above mentioned requirements especially for knee Arthroplasty patients(4).
