صفحات 666-672مقدمهاز بین اختلالات شناختی بعد از عمل جراحی به خصوص در سالمندان، دلیریوم از اهمیت بالاتری برخوردار می باشد. این اختلال می تواند باعث تغییراتی در شخصیت و حافظه ی بیمار شود و عوارض پس از عمل جراحی را افزایش دهد. در این مطالعه، تاثیر تجویز وریدی دگزامتازون و لیدوکائین بعد از القای بیهوشی عمومی در پیش گیری از بروز اختلالات شناختی بعد از عمل جراحی کاتاراکت در سالمندان مقایسه شد.روش هادر این کارآزمایی بالینی آینده نگر شاهددار دو سو کور، 333 بیمار بالای 65 سال با شاخص American Society of Anesthesiologists (ASA) درجه ی I و II که کاندیدای عمل جراحی کاتاراکت به روش بیهوشی عمومی بودند، به صورت تصادفی به سه گروه لیدوکائین، دگزامتازون و شاهد (دارونما) تقسیم شدند و در هر بیمار، مقادیر نمره ی شناختی قبل از عمل و بعد از عمل ثبت شد.یافته هااز سه گروه دریافت کننده ی لیدوکائین، دگزامتازون و شاهد، 6/4،3/6 و 5/3 درصد قبل از عمل دچار اختلال شناختی بودند، اما بین سه گروه اختلاف معنی داری وجود نداشت (62/0 = P). تعداد افراد دچار اختلال شناختی در 24 ساعت بعد از عمل در سه گروه پیش گفته به ترتیب 0/12، 9/9 و 6/9 درصد بود و تفاوت بین سه گروه معنی دار نبود (98/0 = P). در یک هفته بعد از عمل نیز در سه گروه لیدوکائین، دگزامتازون و شاهد به ترتیب 3/8، 1/8 و 8/8 درصد دچار اختلال شناختی بودند، اما تفاوت سه گروه معنی دار نبود (98/0 = P).نتیجه گیریدر این مطالعه، استفاده از لیدوکائین و دگزامتازون در کاهش بروز اختلال شناختی، ارجحیتی نسبت به یکدیگر نداشت، اما از آن جایی که دگزامتازون در بیماران تحت عمل جراحی آب مروارید به علت وجود بیماری های زمینه ای محدودیت مصرف دارد، استفاده از لیدوکائین در این بیماران نسبت به دگزامتازون ارجحیت دارد.کلیدواژگان: دگزامتازون، لیدوکائین، اختلالات شناختی، کاتاراکت

صفحات 673-679مقدمهخونریزی های مامایی، علت شایع مرگ مادران در اغلب کشورهای جهان و کشور ایران است. از این رو، بررسی عوامل بروز آن، جهت ارایه به سیاست گذاران در این زمینه، می تواند مهم ترین راهبرد بهبود سلامت مادران باشد. این مطالعه، با هدف تعیین شیوع خونریزی های بارداری و عوامل مرتبط با آن انجام شد.روش هااین پژوهش به صورت توصیفی- تحلیلی بر روی 2477 مادر باردار انجام شد. جمع آوری اطلاعات با مراجعه ی پژوهشگر به بخش های زایشگاه، مامایی و بستری مادران پر خطر در بیمارستان های سطح 2 و 3 دانشگاه علوم پزشکی اصفهان در سه ماهه ی دوم سال 1395 به دست آمد. تحلیل داده ها، با استفاده از نرم افزارSPSS و روش های آماری توصیفی و استنباطی انجام شد.یافته هاشیوع کلی خونریزی های بارداری 8/7 درصد بود و بین انجام مراقبت بارداری، فرد عامل مراقبت و بیماری های زمینه ای با انواع خونریزی ها رابطه ی معنی داری وجود داشت (05/0 > P)؛ در حالی که بین مراقبت پیش از بارداری با این عارضه، رابطه ی معنی داری وجود نداشت (05/0 < P). همچنین، Logistic regression نشان داد که مراقبت های بارداری و بیماری های زمینه ای نسبت به دیگر متغیرها، پیش بینی کننده ی معنی دارتری برای ایجاد خونریزی هستند (01/0 > P).نتیجه گیرینتایج این مطالعه مبنی بر نزدیک بودن شیوع خونریزی های بارداری به کتب رفرنس با وجود انجام پژوهش در مراکز ارجاعی، اگر چه نشان دهنده ی امکان مدیریت این عارضه در کلیه ی بیمارستان های استان اصفهان می باشد، اما نظر به قابل پیش گیری بودن بسیاری از موارد خونریزی های دوران بارداری، بر استفاده ی هر چه بیشتر از فرصت های موجود به ویژه در زمینه ی ارتقای کیفیت مراقبت ها و کنترل بیماری های زمینه ای تاکید می گردد.کلیدواژگان: شیوع، خونریزی، بارداری، مراقبت بارداری، مراقبت پیش از بارداری

صفحات 680-686مقدمهسوء مصرف مواد مخدر، از مشکلات شایع اجتماعی در افراد جامعه می باشد. از این رو، توجه به وضعیت بیماران مبتلا به سوء مصرف مواد در کنار درمان های دارویی استاندارد، امری ضروری است تا بدین وسیله، اقدامات پیش گیرانه صورت گیرد. بنابراین، هدف از انجام این مطالعه، بررسی وضعیت مهارت های زندگی و سلامت خانواده در افراد مبتلا به سوء مصرف مواد و مقایسه ی آن با افراد سالم بود.روش هااین مطالعه بر روی 240 نفر (120 نفر در هر گروه) انجام گرفت. پس از اخذ رضایت از افراد و ثبت اطلاعات دموگرافیک، مهارت های زندگی توسط ابعاد ده گانه ی مهارت های زندگی سازمان بهداشت جهانی و سلامت خانواده بر اساس مقیاس سلامت خانواده ی اصلی ثبت شد. آزمون های 2χ، t و ANCOVA جهت بررسی داده ها استفاده شد.یافته هاافراد دارای سابقه ی سوء مصرف مواد، به صورت معنی داری بیشتر دارای تحصیلات پایین (001/0 > P)، بی کار (001/0 > P)، دارای همسران با تحصیلات پایین (001/0 > P) و خانه دار (001/0 > P) بودند و از طرفی، مشخص شد سلامت خانواده، مهارت های زندگی و زیر مقیاس های آن (مهارت خودآگاهی، مهارت مربوط به ارتباطات انسانی، مهارت تصمیم گیری و مهارت حل مساله) در گروه مورد به صورت معنی داری کمتر از گروه شاهد بود (001/0 > P).نتیجه گیریعملکرد خانواده، سلامت جسمانی، اجتماعی، روانی و اخلاقی اعضای خود را تعیین می کند و تحت تاثیر قرار می دهد. ناسالم بودن خانواده با پیامدهای گسترده و درازمدت بر اعضای خود همراه است. با توجه به نمره ی پایین مهارت های زندگی در افراد مبتلا به سوء مصرف مواد و نقش این مهارت ها در افزایش توانمندی های فردی و اجتماعی افراد، لازم است مداخلاتی در راستای آموزش این بیماران تدوین گردد.کلیدواژگان: مهارت های اجتماعی، سلامت خانواده، سوء مصرف مواد مخدر

صفحات 687-692مقدمهدوپامین در سیستم اعصاب مرکزی نقش مهمی در تنظیم ترشح اسید معده دارد. از طرفی، به نظر می رسد گیرنده های دوپامینی D2 در هسته ی ونترومدیال هیپوتالاموس به طور چشم گیری در تعدیل دریافت غذا درگیر می شوند. از این رو، ممکن است گیرنده های دوپامینی D2 در هسته ی ونترومدیال هیپوتالاموس، نقش بالقوه ای در تعدیل ترشح اسید معده ایفا کنند. بنابراین، هدف از انجام مطالعه ی حاضر، بررسی اثر مهار گیرنده های D2 هسته ی ونترومدیال بر ترشح پایه ی اسید معده بود.روش هاکانول گذاری در هسته ی ونترومدیال هیپوتالاموس در موش های صحرایی نژاد Wistar با وزن 250-220 گرم انجام شد. پس از 5 روز دوره ی بهبودی، سالین (5/0 میکرولیتر) و آنتاگونیست گیرنده های D2 سولپیراید (005/0، 050/0، 500/0 و 1 میکروگرم) به داخل هسته ی ونترومدیال تزریق شدند. سپس، میزان ترشح اسید معده با روش پیلوریک لیگیشن (Pylorus ligation) و تیتراسیون با سود 01/0 نرمال اندازه گیری شد.یافته هاسولپیراید به طور وابسته به دز و زمان، باعث کاهش معنی دار ترشح پایه ی اسید معده در دزهای 050/0 (010/0 > P)، 500/0 (001/0 > P) و 1 (001/0 > P) میکروگرم گردید.نتیجه گیریسولپیراید 5/0 میکروگرم بیشترین اثر مهاری را بر فعالیت ترشحی معده داشت. از این رو، مهار گیرنده های D2 دوپامینی هسته ی ونترومدیال در Rat فعالیت ترشحی اسید معده را کاهش داد.کلیدواژگان: هسته ی ونترومدیال هیپوتالاموس، گیرنده ی D2 دوپامینی، اسید معده

صفحات 693-697مقدمهتصویربرداری پرفیوژن قلبی، یکی از مهم ترین روش های تصویربرداری در ارزیابی مشکلات قلبی- عروقی است. این روش، نقش مهمی در تشخیص بیماری های قلبی- عروقی و ارزیابی درمان این بیماران دارد. با این حال، به دلیل پیچیده بودن فرایند، احتمال رخداد خطا و آرتیفکت هایی که سبب محدودیت استفاده ی بالینی آن می شوند، بسیار زیاد است. این مساله می تواند مربوط به بیمار، تجهیزات و عملکرد تکنولوژیست باشد.
گزارش مورد: در این مقاله، گزارش چند مورد از آرتیفکت های ایجاد شده در تصویربرداری پرفیوژن قلبی به روش Single-photon emission computed tomography (SPECT) ارایه شده است که ناشی از موارد مختلفی مربوط به تکنولوژیست و یا بیمار است.نتیجه گیریآگاهی از عوامل ایجاد کننده ی آرتیفکت، به منظور محدود ساختن و شناسایی آن ها در پیش گیری از تفسیر نادرست نتایج تصاویر پرفیوژن قلبی ضروری استکلیدواژگان: تصویربرداری پرفیوژن قلبی، آرتیفکت، روش تصویربرداری قلب، مقطع نگاری کامپیوتری نشر تک فوتونی

Pages 666-672BackgroundPostoperative cognitive disorder is one of the most common complications after surgery especially in the elderly population, of which delirium is the most important. This complication may cause changes in personality and memory of patients, and increase postoperative complications. In this study, we decided to compare the preventative effects of intravenous administration of dexamethasone and lidocaine on postoperative cognitive disorder after cataract surgery in the elderly.MethodsIn this double-blind clinical trial study with control group, 333 patients above 65 years of age were divided randomly into three groups receiving lidocaine, dexamethasone, and placebo. The cognitive scores before and after the surgery were compared between the groups.
Findings: In the three groups receiving lidocaine, dexamethasone, and placebo, 6.4%, 3.6%, and 5.3% had cognitive disorder prior to surgery, respectively, which had no significant difference (P = 0.62). The prevalence of the postoperative cognitive disorders in the three groups of lidocaine, dexamethasone, and placebo was 12.0%, 9.9%, and 6.9%, respectively, which was not significantly different either (P = 0.98). The prevalence of cognitive disorders after one week in the three groups of lidocaine, dexamethasone, and placebo was 3.8%, 1.8%, and 8.8%, respectively, which was not significantly different (P = 0.98).ConclusionThe administration of lidocaine or dexamethasone had no significant effect in reducing postoperative cognitive disorder, but since the use of dexamethasone is limited in patients with cataract undergoing surgery due to various underlying medical conditions, using lidocaine is preferable in such patients.Keywords: Dexamethasone, Lidocaine, Cognitive dysfunction, Cataract -
Pages 673-679BackgroundObstetrics hemorrhage is a common reason of mortality among all of countries and also the country of Iran. Therefore, assessment of its incidence could be one of the most important strategies for the policy makers to improve mothers health. This study aimed to determine the prevalence of obstetric hemorrhage and its related factors in the second- and third-level hospitals affiliated to Isfahan University of Medical Sciences, Iran, during the third quarter of year 2016.MethodsThis descriptive-analytic study was conducted on 2477 pregnant mothers. Data were collected by daily visiting of the labor, and gynecology and high-risk pregnant mothers wards. Data were analyzed using descriptive and inferential statistics via SPSS software.
Findings: The prevalence of obstetric hemorrhage was 7.8%, and significant relation was between performing and the number of prenatal care (P 0.05). Logistic regression showed that prenatal care and underlying disease were more predictable for hemorrhage than other variables (PConclusionThe prevalence of obstetric hemorrhages was close to the reference textbooks in the referral hospitals affiliated to Isfahan University of Medical Sciences, which indicated the possibility of managing this incidence in all of the hospitals of Isfahan Province. Pages 680-686BackgroundSubstance abuse is one of the most common social problems in societies. In order to perform preventing actions, it is essential to consider the condition of substance abusers along with standard drug therapy. Therefore, this study aimed to investigate the status of life skills and family health among substance abusers, and compare it with healthy cases.MethodsThis study was performed on 240 cases (120 individuals in each group). After obtaining consent from individuals and recording demographic data, life skills were measured based on the ten dimensions of life skills according to World Health Organization, and family health based on Family-of-Origin Scale. Chi-square, Student t, and ANCOVA tests were used to compare the data.
Findings: Subjects with a history of substance abuse were significantly less educated (PConclusionFamily function determines and affects its member's physical, social, mental, and moral health, and an unhealthy family has long-term and widespread outcomes on its members. Considering the low score of life skills in people with a history of substance abuse, and the role of these skills in increasing individual and social abilities, it is necessary to codify interventions to educate these patients.Keywords: Social skills, Family health, Substance abuse -
Pages 687-692BackgroundDopamine plays an important role in regulating gastric acid secretion in the central nervous system. Dopamine D2 receptors of hypothalamic ventromedial nucleus (VMH) are involved in modulation of food intake, suggesting a potential role of the VMH D2 receptor in modulating gastric acid secretion. Hence in this study, we investigated the effect of the VMH D2 receptor blockade on basal gastric acid secretion.MethodsA guide cannula was stereotaxically implanted in VMH of male Wistar rats weighing 220-250 g. The D2 receptors antagonist sulpiride (0.005, 0.050, 0.500, and 1.000 μg/0.5 μl) or saline (0.5 μl) were injected into VMH after a 5-day recovery period. Gastric acid secretion was measured using the pylorus-ligation method and titration with NaOH 0.01 N.
Findings: Sulpiride at the doses of 0.050 (PConclusionSulpiride at the dose of 0.500 μg/0.5 μl had maximum effect on the secretion activity of stomach. Pages 693-697BackgroundMyocardial perfusion imaging (MPI) is an important imaging technique in the monitoring of cardiovascular complications. This modality plays an important role in diagnosis and treatment assessment of patients with cardiovascular diseases. Although myocardial perfusion imaging is a valuable diagnostic tool, complexity of this modality can result in a variety of artifacts and pitfalls which may limit its clinical applications. These issues may originate from the patients, the nuclear medicine equipment or the technologist.
Case Report: This report provides three illustrative examples of artifacts in myocardial perfusion single-photon emission computed tomography (SPECT) due to both technologists and patient's mistakes.ConclusionIt is crucial to be aware of factors resulting in artifacts, to limit and recognize them in order to prevent misinterpretation of the results of myocardial perfusion imaging.Keywords: Myocardial perfusion imaging, Artifacts, Cardiac imaging techniques, Single, photon emission computerized tomography
