صفحات 214-224مقدمهتعیین جایگاه و نقش هر کشور در تولیدات علمی در حوزههای مختلف موضوعی، نشان دهنده ی میزان توانمندی و عملکرد آن کشور در جهت ارتقا و کمک به بهبود آن موضوع میباشد. هدف از مطالعه ی حاضر، بررسی کمی و کیفی تولیدات علمی حوزه ی دیابت جمهوری اسلامی ایران و تعیین جایگاه آن در منطقه و جهان میباشد.روش هامطالعه ی حاضر به روش پیمایشی توصیفی (مقطعی) و با رویکرد علمسنجی صورت گرفته است. جامعه ی مطالعه ی حاضر را 2291 مقاله و مدرک علمی جمهوری اسلامی ایران در حوزه ی دیابت تشکیل میدهند که طی دو دهه ی اخیر و در پایگاه اطلاعاتی اسکوپوس نمایه شدهاند. برای تجزیه و تحلیل اطلاعات و ترسیم گرافها، از نرم افزارهای SPSS، NodeXL و VOSviewer استفاده شده است.یافته هاروند تولیدات علمی ایران در حوزه ی دیابت در طی سالهای مورد بررسی رشد داشته و از 5 مورد در سال 1996 به 299 مورد در سال 2015 رسیده است. یافتهها نشان داد که دانشگاه علوم پزشکی تهران، ایالات متحده آمریکا و دکتر فریدون عزیزی به ترتیب به عنوان مهمترین مرکز تولید کننده، کشور همکار و نویسنده ی پرتولید شناسایی شدهاند. همچنین جایگاه جمهوری اسلامی ایران طی سالهای مورد بررسی رشد مطلوبی داشته و ایران به ترتیب درسطح منطقهای و سطح بینالمللی به رتبه ی دوم و بیست و یکم ارتقاء یافته است.نتیجه گیریهرچند به لحاظ کمی تولیدات علمی کشورمان در حوزه ی دیابت طی سالهای مورد بررسی رشد مثبتی داشته است، ولی سهم جمهوری اسلامی ایران از کل تولیدات علمی حوزه ی دیابت بسیار ناچیز بوده است. همچنین بخش قابل توجهی از مدارک علمی منتشر شده در مجلات نه چندان معتبر و باکیفیت داخلی منتشر شدهاند. سیاستگذاری و برنامهریزی دانشگاههای علوم پزشکی و در راس آن وزارت بهداشت، درمان و آموزش پزشکی برای افزایش مطالعات و پژوهشهای مرتبط با حوزه دیابت و ارتقای سطح کمی و کیفی تولیدات علمی لازم و ضروری به نظر میرسد.کلیدواژگان: تولید علم، مقالات علمی، دیابت، علم سنجی، ایران، پایگاه اطلاعاتی اسکوپوس

دیابت یکی از شایع ترین بیماری های غدد است که در جوامع مختلف شیوع متفاوتی دارد. 90 % تا 95 % انواع دیابت را دیابت نوع دو شامل می شود. زندگی صنعتی و شهرنشینی عامل اصلی افزایش تعداد مبتلایان به این بیماری است. برنامه ی کشوری پیشگیری و کنترل دیابت در راستای پیشگیری و کنترل دیابت و عوارض ناشی از آن، در کشور درحال انجام است و هنوز تاثیردقیق برنامه بررسی نشده است. این مطالعه با هدف بررسی تاثیر برنامه بر میزان قند خون ناشتا و هموگلوبین گلیکوزیله ی بیماران در زمینه ی کنترل دیابت انجام شد.
روش هاپژوهش حاضر یک مطالعه ی مقطعی از نوع قبل و بعد بود که بر روی 100سالمند دیابتی که قصد ورود به برنامه را داشتند، به صورت در دسترس در شهر اصفهان انجام شد. برای جمع آوری داده ها از پرسشنامه دو قسمتی شامل: اطلاعات جمعیت شناختی، سلامت و سنجش بیوشیمیایی قندخون ناشتا و هموگلوبین گلیکوزیله استفاده شد. اطلاعات به صورت قبل از ورود به برنامه و سپس سه ماه پس از ورود به برنامه جمع آوری شد.
یافته هامیانگین و انحراف معیار قند خون ناشتای بیماران قبل از ورود به برنامه به ترتیب 73/172و 68/71 و سه ماه بعد از برنامه 02/ 143، 19/51 بود (30/5 = t، 001/0>P). همچنین میانگین و انحراف معیار قند هموگلوبین گلیکوزیله بیماران قبل از ورود به برنامه به ترتیب 24/8 و 90/1 و بعد از برنامه به ترتیب 44/7 و 66/1 بود (79/4 = t، 001/0>P).
نتیجه گیرینتایج این مطالعه، تاثیر برنامه ی کشوری پیشگیری و کنترل دیابت را در کنترل میزان قند خون ناشتا و هموگلوبین گلیکوزیله سالمندان دیابتی نشان داد. پیشنهاد می گردد این برنامه در همه ی مراکز بهداشتی-درمانی کشور انجام شود.

صفحات 233-241مقدمهاز آنجا که شناخت مولفه های تعیین کننده ی تبعیت بیماران دیابتی از رژیم های دارویی در جهت طراحی مداخلات مناسب ضرورت دارد، هدف مطالعه ی حاضر روشن ساختن تجارب بیماران مبتلا به دیابت نوع دو در استفاده از انسولین می باشد.روش هااین مطالعه ی کیفی روی 22 بیمار مبتلا به دیابت نوع دو مراجعه کننده به کلینیک غدد بیمارستان فیروزگر در سال 1396 با استفاده از نمونه گیری مبتنی بر هدف انجام شد. داده ها از طریق مصاحبه های بدون ساختار جمع آوری، پیاده سازی و به روش تجزیه و تحلیل محتوایی (از نوع قراردادی) تحلیل شدند.یافته هامیانگین سن بیماران مشارکت کننده 72/56 سال با انحراف معیار16/9 بود. پس از تحلیل و کدبندی اطلاعات، 2 مضمون اصلی شامل عوامل فردی (با 6 زیر طبقه شامل نگرش منفی به انسولین، عوامل هیجانی، دانش، ترس، اعتقاد در مورد اثربخشی بیشتر درمان های مکمل نسبت به تزریق انسولین و نداشتن مهارت کافی برای نگه داری و تزریق انسولین) و عوامل محیطی (با 3 زیر طبقه شامل حمایت اجتماعی، قیمت و تاثیرگذارنده های موقعیتی) به عنوان تجارب بیماران در زمینه ی استفاده از انسولین ظهور یافتند.نتیجه گیریتاسیس مراکز مشاوره و آموزش بیماران دیابتی و خانواده های آنها در کلینیک های غدد، افزایش حمایت های بیمه از بیماران دیابتی و لزوم صرف وقت بیشتر برای آموزش بیماران توسط پزشکان ضرورت دارد.کلیدواژگان: دیابت نوع دو، استفاده از انسولین، تجارب، مطالعه کیفی

صفحات 242-256مقدمهدیابت بیماری مزمنی است که بر بسیاری از ابعاد و جنبه های زندگی تاثیر می گذارد. تجربه ی بیماران جنبه های مختلف زندگی او را در بر می گیرد زیرا بیمار به واسطه ی بیکاری اش زندگی می کند. هدف این مطالعه توضیح نقش سبک زندگی در زندگی بیمار است. بررسی تجربه ی زیسته ی بیماران در این مورد می تواند راهکار اساسی در اصلاح سبک زندگی بیماران دیابتیک باشد.روش هااین مطالعه یک بررسی کیفی از نوع پدیدارشناسی است که در آن نمونه گیری از میان مراجعین به مرکز خیریه ی دیابت اصفهان به طور هدفمند، با حداکثر تنوع انتخاب شد. 33 بیمار زن مبتلا به دیابت در سنین بین 35 تا 69 سال با سابقه ی حداقل یک سال ابتلا به دیابت و نداشتن بیماری روانی حاد، انتخاب شدند. نمونه گیری به صورت تدریجی از بهمن 95 تا بهار 96 انجام گرفت. بیماران در این مطالعه با انجام مصاحبه های نیمه ساختاریافته طی 45 تا 50 دقیقه مورد بررسی قرار گرفتند. متن مصاحبه های ضبط شده، پس از پیاده سازی به روش تحلیل کلایزی مورد تجزیه و تحلیل قرار گرفتند.یافته هاطی فرآیند تجزیه و تحلیل داده های حاصل از تجربه ی زیسته ی بیماران مبتلا به دیابت، چهار درون مایه شامل بعد سلامت جسمانی، بعد سلامت روان شناختی، بعد سلامت معنوی و سلامت اجتماعی استخراج گردید.نتیجه گیریبرای اصلاح سبک زندگی بیماران نیازبه نگرشی همه جانبه نسبت به موضوع سبک زندگی است. درک تجربه ی زیسته ی و نتایج حاصل از تجارب مربوط به سبک زندگی سلامت در بیماران مبتلا به دیابت می تواند اعضای تیم درمانی و مراقبتی را در ارائه ی خدمت مطلوب تر کمک نماید و از بازگشت های مکرر به سبک زندگی نادرست جلوگیری به عمل آید.کلیدواژگان: تجربه ی زیسته، سبک زندگی سلامت، دیابت نوع دو

صفحات 257-263مقدمهناامنی غذایی با کنترل ضعیف قند خون همراه است. از این رو، مطالعه ی حاضر با هدف تعیین امنیت غذایی در بیماران دیابتی نوع دو در اردبیل انجام شد.روش هادر این مطالعه توصیفی- تحلیلی، 153 بیمار دیابتی نوع دو (98 زن و 55 مرد) در اولین ویزیت خود در کلینیک تغذیه در سال 1395 انتخاب شدند. مشارکت کنندگان بعد از اندازه گیری وزن و قد، پرسشنامه های ثبت غذایی سه روز (2 روز عادی و یک روز تعطیل) را تکمیل کردند. با استفاده از نرم افزار آنالیز غذایی N4 مقدار دریافت انرژی، درشت مغذی ها و ریزمغذی ها محاسبه شد. ناامنی غذایی با مقایسه ی مقادیر دریافت مواد مغذی با مقادیر مجاز توصیه شده اندازه گیری شد. از آزمون های تی تک نمونه ای و تی مستقل برای انجام مقایسه استفاده شد.یافته هامیانگین سن، مدت بیماری و نمایه ی توده ی بدنی مشارکت کنندگان به ترتیب 36/7±56/49 (سال) ، 46/5±34/6 (سال) و 25/4±55/28 (کیلوگرم بر مترمربع) بود. فقط 6/19 درصد مشارکت کنندگان در محدوده ی طبیعی وزن قرار داشتند. میانگین مصرف انرژی 59/515± 20/1922 کیلوکالری در روز بود. کربوهیدرات، پروتئین و چربی به ترتیب 26/63، 86/14 و 88/21 درصد انرژی دریافتی را تشکیل دادند. مصرف ریز مغذی ها (کلسیم، منیزیم، ویتامین های A، E و C) کمتر از مقادیر مجاز توصیه شده بود.نتیجه گیریناامنی غذایی به ویژه در مصرف کلسیم، منیزیم، ویتامین های A، E و C در بیماران دیابتی نوع دو شایع بود. توصیه می شود این بیماران برای مقادیر مورد نیاز رژیمی شان آموزش داده شوند.کلیدواژگان: بیماران دیابتی نوع دو، دریافت مواد مغذی، ناامنی غذایی

شیوع بالای چاقی و اضافه وزن در زنان ابتلا به سندرم متابولیک و دیابت را بالا می برد. با توجه به تاثیر متفاوت فعالیت های ورزشی مختلف و تفاوت حجم عضلات بالا تنه و پایین تنه درگیر در فعالیت، احتمالا اثر فعالیت تناوبی شدید با کارسنج دستی و پایی بر متابولیسم و مقاومت به انسولین متفاوت می باشد. لذا هدف از این تحقیق بررسی تاثیر فعالیت تناوبی اندام فوقانی و اندام تحتانی بر متابولیسم کربوهیدرات و مقاومت به انسولن در زنان چاق بود.
روش هابدین منظور 12 زن چاق (نمایه ی توده ی بدن 5/5±1/31 کیلوگرم بر مترمربع) به طور داوطلبانه در این تحقیق شرکت کردند. آزمودنی ها یک جلسه فعالیت تناوبی را روی دوچرخه کارسنج و در جلسه ای دیگر روی کارسنج دستی با فاصله ی یک هفته، 2 دقیقه فعالیت با شدت 85% حداکثر اکسیژن مصرفی همراه با 4 دقیقه استراحت فعال با شدت 45% حداکثر اکسیژن مصرفی به مدت 30 دقیقه انجام دادند. قبل و بلافاصله پس از 30 دقیقه فعالیت دو نمونه ی خونی برای اندازه گیری انسولین و گلوکز گرفته شد. میزان اکسیژن مصرفی و دی اکسیدکربن تولیدی حین فعالیت برای محاسبه اکسیداسیون کربوهیدرات اندازه گیری شد.
یافته هااگرچه فعالیت تناوبی میزان انسولین را کاهش داد، اما پاسخ انسولین به فعالیت تناوبی اندام فوقانی و تحتانی تفاوت معنی داری نشان نداد (05/0P>). میزان گلوکز با فعالیت تناوبی کاهش یافت و این کاهش متعاقب فعالیت تناوبی اندام تحتانی بیشتر از فعالیت اندام فوقانی بود (05/0P<). همچنین میزان HOMA-IR در جلسه ی پائین تنه بیشتر از بالاتنه کاهش یافت (05/0P<). فعالیت تناوبی منجر به افزایش معنی دار اکسیداسیون کربوهیدرات شد (05/0P<) ، اما مقایسه ی دو نوع فعالیت تناوبی اندام فوقانی و تحتانی، تفاوت معنی داری نشان نداد (05/0P>).
نتیجه گیرینتایج مطالعه پیش رو نشان داد فعالیت تناوبی در زنان چاق، متابولیسم کربوهیدرات و مقاومت به انسولین را تغییر می دهد و تغییرات مقاومت به انسولین وابسته به نوع فعالیت (اندام فوقانی و اندام تحتانی) می باشد، اما به اکسیداسیون کربوهیدرات وابسته نیست.

Pages 214-224BackgroundDetermine of position and role of each country on scientific production in different subject areas shows level of capacity and performance of the country to help and improve this subject area. The aim of this study is to quantitative and qualitative examine of scientific production of Islamic Republic of Iran in the field of Diabetes and determine its regional and international position.MethodsThis is Survey-Descriptive Study and use Scientometrics approach. Statistical populations include 2291 scientific articles and papers which have been published by Iranian authors in the field of Diabetes in Scopus Database during 2 last decades. We used SPSS, NodeXL and VOSviewer to draw graphs and analysis data.ResultsFindings show that Iranian scientific outputs in the field of Diabetes increased from 5 in 1996 to 299 in 2015. Tehran University of Medical Science, United States and Dr Freedom Azizi Respectively founded as the most important producer Center, Partner country and high producer Author. Also place of Islamic Republic of Iran upgraded in the period study and stay on second position in region and twenty first position in the word.ConclusionAlthough Iranian scientific production in the field of Diabetes have been increased in a quantitative factors, but share of the Islamic Republic of Iran among all world production in the field of diabetes was so low. Also most of scientific papers and articles published in internal journals which haven’t good and high quality. Diabetes is a chronic disease that is increasing in the world that has a different outbreak in different societies. 90% to 95% of all types of diabetes include type 2 diabetes. This disease is spreading due to the Industrial life and urbanization .The Nationwide Program for Prevention and Control of Diabetes is already under way for controlling the disease and its side-effects. This study is intended to probe into the effects of the program in the country.
MethodsThis study used a cross-sectional pre- and post-test design involving 100 elderly patients newly introduced to be suffering from diabetes who were selected by convenience sampling in Isfahan. The data collection methods were a questionnaire on demography and health, and another one on patients’ levels of fasting blood sugar (FBS) and hemoglobin glycosides (HbA1c(The data was gathered before and three months after the patients’ entrance into the program.
ResultsThe mean and standard deviation of the fasting blood sugar before the program were 172.73 and 71.68 and after the program 143.02 and 51.19, respectively (P<.001; t=5.30). And finally, glycated hemoglobin mean and standard deviation before the program were 8.24 and 1.9 and after the program 7.44 and 1.66, respectively (P<.001; t=4.79).
ConclusionThe results indicate the importance of the program for the prevention and control of the elderly patients’ diabetes by controlling their fasting blood sugar and glycated hemoglobin.It is recommended that this program be implemented in all health centers of the country

Pages 233-241BackgroundGiven the recognizing the determinants of medication adherence in diabetic patients is necessary to design appropriate interventions, the aim of this study was to clarification experiences of parsons with type II diabetes in terms of insulin use.MethodsThis qualitative study was conducted on 22 patients with type II diabetes who were referred to Glands Clinic of Firozgar Hospital, Iran University of Medical Sciences, Tehran, Iran in 2017 which were purposefully selected and interviewed. All interviews were recorded, transcribed and then analyzed.ResultsThe mean age of participants was 56.72 (SD=9.16) yr old. After analyzing and coding of data, individual factors with 6 categories (included negative attitude regarding Insulin, emotional factors, knowledge, fear, belief about the more effectiveness of complementary therapies than insulin injection and inadequate skill for storage and inject insulin) and environmental factors with 3 categories (included cost, situational influences and social support) was identified as experiences participants, in terms of insulin use.ConclusionEstablishment of counseling and training centers for patients with diabetes and their families in endocrinology clinics, increase the support of insurance from diabetic patients, and the need for more time to training patients by doctors is essentialKeywords: Type II diabetes, Insulin use, Experiences, Qualitative study

Pages 242-256BackgroundDiabetes is a chronic disease with consequences affecting many aspects and dimensions of life. Patients experience relates to many aspects of their lives because the disease involves their whole life.
The present study aims at defining the role of life style in patient's life. Reviewing their lived-experience can provide basic solutions to reform the life style of patients with diabetes.Methodin this phenomenology qualitative research, sampling was done among patients referred to Isfahan Diabetes Charity Center in a purposeful manner with the most possible variety. A number of 33 women with diabetes in the age range of 35 to 69 years with an experience of at least one year of living with diabetes and without a history of acute mental disease were chosen. Gradual sampling was conducted from January 2017 to March 2017. Patients took part in 45 to 50 minute semi-structured interviews. The interviews were recorded and then the Colaizzi method format was analyzed.Resultsanalysis process of data gathered from these patients' lived-experiences, resulted in extracting four themes including physical health, psychologic health, spiritual health and social health.ConclusionIn order to reform the life style of patient's' with diabetes, a general comprehensive attitude in way of living is needed. Pages 257-263BackgroundFood insecurity is associated with the poor glycemic control. Therefore, this study was amid to determine food insecurity among type 2 diabetic patients in Ardabil.MethodsIn this analytical cross-sectional study, 153 type 2 diabetic patients (98 females and 55 males) were recruited from the nutrition clinic in their first visit in 2016. After measurement of weight and height, the participants were completed 3-day food records (2 weekdays and 1 weekend day). The energy, macronutrients and micronutrients intakes were calculated using N4 food analysis software. Food insecurity was measured by comparisons between the nutrients intakes and recommended dietary allowances. One sample T-Test and Independent sample T-Test were used for comparison.ResultsThe mean of age, disease duration and body mass index were 49.56± 7.36 years, 6.34± 5.46 years and 28.55 ± 4.25 kg/m2, respectively. Only one-fifth (19.6%) of participants fell within the normal weight range. The mean energy intake was 1922.20 ± 515.59 kcal/day. The carbohydrate, protein, and fat were contributed to 63.26%, 14.86% and 21.88% of the energy intake, respectively. The micronutrient intakes (calcium, magnesium, vitamins A, E, and C) were less than recommended dietary allowances.ConclusionFood insecurity, especially in calcium, magnesium, vitamins A, E, and C intakes was prevalence among type 2 diabetic patients. Prevalence of obesity and overweight in women increases the possibility of metabolic syndrome and diabetes. Based on the variety of the effects of different type of exercise and the differences in the upper and lower body muscle volume involved in the exercise, the effects of high intensity interval exercise with arm and leg ergometer on metabolism and insulin resistance might be different. The aim of this study was to compare the effect of upper and lower body interval exercise on carbohydrate metabolism and insulin resistance in obese women.
MethodsTwelve obese and overweight women (body mass index, 31.1±5.5 kg/m2) performed two interval exercise trials of 30-min on arm and leg ergometer with one week intervening. Interval exercise included 5 sets of 6 min in which 2- min activity at 85% VO2max and 4 min active rest at 45% VO2max was performed. Two blood samples were taken before and immediately after exercise for measuring glucose and insulin. VO2 and VCO2m were collected during exercise and used to calculate the amount of carbohydrate oxidation.
ResultAlthough interval exercise reduced the insulin concentration, responses of insulin to upper and lower body exercises were not significantly different (P>0.05). Glucose level reduced following interval exercise, and this reduction was significantly (P<0.05) higher following lower body interval exercise than upper body exercise. Similarly the reductions in HOMA-IR were significantly (P<0.05) higher following lower body interval exercise than upper body exercise. Interval exercise resulted in increases in oxidation of carbohydrate, though; responses to upper and lower exercises were not significantly different.
ConclusionIt could be concluded that interval exercise in obese women leads to changes in carbohydrate metabolism and insulin resistance, and that the changes in insulin resistance are related to type of exercise (upper and lower body), though carbohydrate oxidation is not
