    مقدمه
    کیست هیداتیک، مرحله ی لاروی کرم Echinococcus granulosus است. اثر این انگل روی رشد سرطان در محیط کشت و در مطالعات حیوانی مختلف بررسی شده است. در جهت مطالعه ی مکانیسم های ضد سرطانی این انگل، در مطالعه ی حاضر، واکنش آنتی سرم های ضد آنتی ژن های کیست هیداتیک با آنتی ژن های سطح سلول های سرطانی ملانوما و پستان موش و سلول های طحال موش سالم به روش فلوسیتومتری مورد بررسی قرار گرفت.
    روش ها
    آنتی سرم های ضد آنتی ژن های کیست هیداتیک در خرگوش تهیه شد. این آنتی سرم ها، با رده های سلولی ملانوما، پستان و سلول های طحال موش طبیعی مجاورت داده شد و واکنش آنتی بادی ها با آنتی ژن های سطحی این سلول ها به روش فلوسیتومتری بررسی گردید.
    یافته ها
    میزان واکنش آنتی سرم ضد کیست هیداتیک (Protoscolex، دیواره و مایع کیست) با رده ی سلول های سرطانی ملانوما تفاوت معنی داری نداشت، اما میزان واکنش آنتی سرم های ضد دیواره و مایع کیست با رده ی سلول های سرطانی پستان اختلاف معنی داری داشت. علاوه بر این، میزان واکنش آنتی سرم های ضد Protoscolex، دیواره و مایع کیست با سلول های طحال موش کمتر از میزان واکنش سرم خرگوش طبیعی بود.
    نتیجه گیری
    آنتی سرم ضد کیست هیداتیک با رده ی سلول های سرطانی ملانوما واکنش نمی دهد، اما با رده ی سلول های سرطانی پستان واکنش می دهد. این نتایج، تایید کننده ی مکانیسم های ضد سرطان پستان این انگل می باشد. بنابراین، ممکن است بتوان از آن برای دارورسانی انتخابی استفاده کرد.

    مقدمه: پاکیزه سازی دقیق، به عنوان اولین گام فرایند استفاده ی مجدد، می تواند موجب استریلیزاسیون موثرتر و حفاظت تیم درمانی و بیماران از عفونت های قابل انتقال گردد. این مطالعه، با هدف مقایسه ی دو روش پاکیزه سازی بیمارستان و روش پاکیزه سازی انجمن Association for the Advancement of Medical Instrumentation (AAMI) بر روی میزان کاهش کلونیزاسیون میکروارگانیسم های ابزار لاپاراسکوپی انجام گردید.
    روش ها
    در این پژوهش نیمه تجربی، 128 ابزار لاپاراسکوپی به طور تصادفی در دو گروه پاکیزه سازی بیمارستان و پاکیزه سازی AAMI قرار گرفتند و طی دو مرحله ی بلافاصله بعد از جراحی و بعد از فرایند پاکیزه سازی با هر یک از روش های پیش گفته، از ابزار مورد نظر نمونه برداری شد. تعداد و نوع میکروارگانیسم های جدا شده در محیط های کشت Sabouraud Dextrose Agar، Blood Agar و MacConkey Agar تعیین گردید
    یافته ها
    میانگین تعداد کل میکروارگانیسم های جدا شده از ابزار بلافاصله بعد از جراحی، 105 × 24 Colony forming unit در 100 میلی لیتر (CFU در 100 میلی لیتر) به دست آمد. بعد از انجام فرایند پاکیزه سازی در گروه پاکیزه سازی بیمارستان به 105 × 2/7 CFU در 100 میلی لیتر و در گروه AAMI به 105 × 34/0 CFU در 100 میلی لیتر کاهش پیدا نمود که این کاهش در گروه AAMI به طور معنی داری بیشتر از گروه بیمارستان بود (050/0 > P). بیشترین فراوانی نوع میکروارگانیسم های جدا شده بعد از جراحی به ترتیب مربوط به Escherichia coli (2/81 درصد) ، Pseudomonas aeruginosa (8/68 درصد) و Klebsiella (8/57 درصد) بود.
    نتیجه گیری
    کاهش چشم گیر میانگین تعداد کل و فراوانی نوع میکروارگانیسم ها در گروه AAMI نشان داد که مرطوب سازی ابزار لاپاراسکوپی در حین جراحی و غوطه وری در محلول آنزیمی که در روش AAMI مورد تاکید است، موجب پاکیزه سازی بهتر و دقیق تر ابزار می گردد و روش موثرتری نسبت به شیوه نامه ی بیمارستان می باشد.

    مقدمه: آرتروپلاستی زانو، یکی از درمان های موثر در استئوآرتریت زانو می باشد. خونریزی بعد از عمل جراحی و به دنبال آن تزریق خون به بیمار، همچنان یکی از مشکلات این عمل جراحی است. این مطالعه، با هدف بررسی و مقایسه ی تاثیر روش استفاده ی موضعی حین عمل جراحی و روش تزریق ترانگزامیک اسید در درن کلامپ شده انجام گردید.

    روش ها

    در این کارآزمایی بالینی، بیماران استئوآرتریت اولیه به سه گروه جهت تزریق دارونما داخل درن، تزریق ترانگزامیک اسید به داخل درن و دریافت ترانگزامیک اسید به صورت موضعی تقسیم شدند. اطلاعات دموگرافیک و سپس هموگلوبین قبل از عمل، 12 و 48 ساعت بعد از عمل ثبت شد. از نرم افزار SPSS جهت واکاوی آماری استفاده شد. برای بررسی ارتباط میان داده ها، از آزمون های 2χ، ANOVA و t استفاده شد.

    یافته ها

    75 بیمار (67 زن و 8 مرد) با میانگین سنی 15/5 ± 26/63 سال به مطالعه وارد شدند. میزان هموگلوبین بیماران قبل از عمل جراحی 23/1 ± 29/13 گرم/دسی لیتر بود و اختلاف معنی داری در هموگلوبین بیماران قبل از عمل جراحی وجود نداشت (891/0 = P). با بررسی هموگلوبین های 48 ساعت بعد از عمل در گروه های مورد مطالعه، می توان نتیجه گرفت که میزان هموگلوبین در بیماران عضو گروه تزریق دارونما به داخل درن به مقدار بیشتری کاهش پیدا کرد و این کاهش نسبت به دو گروه دیگر معنی دار بود (001/0 = P).

    نتیجه گیری

    تزریق موضعی و همچنین تزریق داخل درن داروی ترانگزامیک اسید می تواند تاثیر بیشتری در کاهش نیاز به انتقال خون و میزان آنمی بیماران بعد از عمل جراحی آرتروپلاستی زانو نسبت به دارونما داشته باشد، ولی تفاوت این دو روش در بررسی حاضر معنی دار نبود.

    مقدمه
    Escherichia coli یکی از عوامل اصلی عفونت ادراری در انسان است. مقاوم شدن این باکتری به انواع آنتی بیوتیک ها درمان آن را با مشکل مواجه کرده است. مقاومت وابسته به پلاسمید نسبت به کینولون ها به طور روزافزون در خانواده ی Enterobacteriaceae در جهان در حال گسترش می باشد. هدف از انجام این تحقیق، بررسی فراوانی ژن های پلاسمیدی qnr مقاوم به کینولون ها در جدایه های Escherichia coli عامل عفونت ادراری بود.
    روش ها
    در این مطالعه ی توصیفی- مقطعی، 96 جدایه ی ادراری Escherichia coli شناسایی شدند. حساسیت آنتی بیوتیکی جدایه ها به روش انتشار دیسک بررسی شد. حضور ژن های پلاسمیدی qnrA، qnrB و qnrS به روش مولکولی با پرایمرهای اختصاصی بررسی گردید.
    یافته ها
    از 96 جدایه ی مورد بررسی، 53 جدایه (16/40 درصد) نسبت به آنتی بیوتیک های گروه فلوروکینولون ها مقاوم بودند. همچنین، حضور ژن qnrA در 18 جدایه (96/33 درصد) ، ژن qnrB در 8 جدایه (10/15 درصد) و ژن qnrS در 5 جدایه (43/9 درصد) از 53 جدایه ی مقاوم نسبت به فلوروکینولون توسط آزمون Polymerase chain reaction (PCR) تایید گردید.
    نتیجه گیری
    نتایج حاصل از این تحقیق نشان می دهد که ژن های گروه qnr وابسته به پلاسمید در اصفهان گسترش یافته است. فراوانی ژن qnrA نسبت به qnrB و qnrS در میان جدایه های Escherichia coli مقاوم به کینولون در اصفهان بیشتر است.

    مقدمه

    هدف ازانجام این مطالعه، بررسی مقایسه ای تاثیر لوله گذاری داخل تراشه در سه روش دهانی، بینی با کمک پنس مگیل و بینی بدون کمک پنس مگیل بر ضربان قلب و فشار خون در بیماران تحت اعمال جراحی فک و صورت بود.

    روش ها

    در این مطالعه ی کارآزمایی بالینی یک سو کور تصادفی، 96 بیمار که تحت اعمال جراحی فک و صورت بودند، به صورت تصادفی به سه گروه لوله گذاری تراشه از طریق دهان و از بینی با و یا بدون استفاده از پنس مگیل تقسیم شدند. ضربان قلب و فشار خون در بیماران قبل از لوله گذاری و دقایق 0، 1، 2، 5 و 10 بعد از لوله گذاری و عوارض بعد از لوله گذاری مورد بررسی قرار گرفت.

    یافته ها

    بین سه گروه اختلاف معنی داری بر اساس ضربان قلب، فشار خون سیستول و دیاستول وجود نداشت (05/0< P برای همه ی موارد). زخم گلو در گروه لوله گذاری از طریق دهان و اپیستاکسی در گروه لوله گذاری از طریق بینی بدون استفاده از مگیل به صورت معنی داری بیشتر از سایر گروه ها بود (05/0 > P). همچنین، طول مدت زمان لوله گذاری در روش دهانی به طور معنی داری کمتر از سایر روش ها بود (05/0 > P).

    نتیجه گیری

    هر سه روش دهانی، بینی با کمک پنس مگیل و بینی بدون کمک پنس مگیل، به یک میزان باعث تغییرات ضربان قلب و فشار خون می شوند و تنها تفاوت این سه روش در ایجاد عوارض جانبی بعد از لوله گذاری است.

    Background
    Hydatid cyst is the larval stage of the Echinococcus granulosus worm which is located in the humans and animals viscera. In order to study the anticancer mechanisms of this parasite, in this study the response of antisera against hydatid cyst antigens with tumor cells antigens was investigated using flow cytometry technique.
    Methods
    Antisera against hydatid cyst antigens were prepared in rabbits, and added to the melanoma (B16F10) and breast cancer cell (4T1) lines, and also normal mouse spleen cells. The reaction was then monitored via flow cytometry. Findings: There was no significant difference in the reaction of hydatid cyst antisera (protoscolex, cell wall, and cystic fluid) with the melanoma cell line. However, the reaction of antisera against cyst fluid and cyst wall with breast cancer cells was significantly different. Additionally, the reaction of the antisera against protoscolex, wall, and cyst fluid with mouse spleen cells was less than that of normal rabbit serum (as a negative control).
    Conclusion
    The results show that antisera against cyst wall and fluid react with the breast cancer cell line, but not with melanoma cells. These results confirm the anticancer mechanisms of this parasite. So, it may be possible to use it for selective drug delivery

    Background
    An accurate cleaning, as the first step of reprocessing, would make the sterilization more effective, and protects healthcare providers and patients against infections. The present study aimed to compare the effect of two cleaning methods of conventional and the Association for the Advancement of Medical Instrumentation (AAMI) on the reduction of microorganism colonization on laparoscopic instruments.
    Methods
    In this quasi-experimental study, 128 laparoscopic instruments was randomly divided into two groups of conventional and AAMI cleaning protocols. Sampling was done immediately after surgery and the cleaning process. The number and types of microorganisms which isolated from the Sabouraud Dextrose Agar, Blood Agar and MacConkey Agar media were determined. Findings: The total mean number of microorganisms isolated from instruments was 24 × 105 colony forming unit (CFU)/100 ml immediately after surgery. After cleaning process, it was reduced to 7.2 × 105 CFU/100 ml and 0.34 × 105 CFU/100 ml in conventional and AAMI groups, respectively. This reduction was significantly higher in the AAMI group than in the conventional cleaning group (P < 0.050). The most frequent type of the microorganisms isolated after surgery were as Escherichia coli (81.2%), Pseudomonas aeroginosa (68.8%), and Klebsiella spp. (57.8%), respectively.
    Conclusion
    Reduction of total mean number and frequency of the microorganisms after the cleaning process were higher in the AAMI group than in conventional group. Therefore, the AAMI method may prevent hospital-acquired infection, and is recommended as an effective cleaning method for laparoscopic instruments after surgery

    Background

    Knee arthroplasty is one of the most effective treatments in osteoarthritis. However, postsurgical bleeding, followed by blood transfusion, is one of the important complications. This study aimed to compare the effect of topical versus injection of tranexamic acid into the clamped drain on postsurgical bleeding in knee arthroplasty surgery.

    Methods

    In this double-blinded clinical trial study, patients with primary osteoarthritis were randomly divided into three groups to receive placebo injection into the drain clamp, tranexamic acid injection into the drain, and topical tranexamic acid, respectively. Demographic data, as well as hemoglobin level before surgery, and 12 and 48 hours after it were recorded. Data analysis was done using chi-square, ANOVA, and t tests via SPSS software. Findings: 75 patients (67 women and 8 men) with mean age of 63.26 ± 5.15 years were enrolled. Mean hemoglobin level was 13.29 ± 1.23 g/dl before the surgery and did not have any difference between the three groups (P = 0.891). Considering the level of hemoglobin after 48 hours, it decreased significantly more in group of tranexamic acid injection into the clamped drain compared to other groups (P = 0.001).

    Conclusion

    Topical and injection into the clamped drain of tranexamic acid could be effective in reducing the need for blood transfusion and the degree of anemia in patients undergoing knee arthroplasty surgery, but the difference was not significant in this study

    Background
    Escherichia coli is one of the main causes of urinary tract infection in humans. The bacteria have become resistant to antibiotics and its treatment is difficult. The plasmid-dependent resistance to quinolones is increasing in the family of Enterobacteriaceae. This study aimed to assess the frequency of plasmid qnr genes in quinolone-resistant isolates of Escherichia coli causing urinary tract infection
    Methods
    In this descriptive study, 96 isolates of Escherichia coli were identified. Antibiotic susceptibility of isolates was investigated using disc diffusion method. The presence of qnrA, qnrB, and qnrS plasmids was assessed using molecular methods with specific primers. Findings: Of the 96 isolates examined, 53 isolates (40.16%) were resistant to fluoroquinolone antibiotics. Moreover, the presence of qnrA gene in 18 isolates (33.96%), qnrB gene in 8 isolates (15.1%), and qnrS gene in 5 isolates (9.43%) of 53 isolates resistant to fluoroquinolones was confirmed via polymerase chain reaction (PCR) assay.
    Conclusion
    The results of this study show that genes of plasmid-dependent qnr group have expanded in Isfahan City, Iran. The frequency of the qnrA gene relative to qnrB and qnrS is higher among quinolone-resistant Escherichia coli isolates in Isfahan.

    Background

    The purpose of this study was to compare the effect of endotracheal intubation in three methods of oral, and nasal with and without the help of Magill forceps, on heart rate and blood pressure in patients undergoing maxillofacial surgery.

    Methods

    In this single-blind randomized clinical trial study, 96 patients who underwent facial and maxillofacial surgery, were randomly divided into three groups of oral, and nasal (with or without the use of Magill forceps) endotracheal intubation. Heart rate and blood pressure were studied in patients before the intubation, and 0, 1, 2, 5, and 10 minutes after it, and post-tubal complications were assessed as well. Findings: There was no significant difference between the three groups based on heart rate, and systolic and diastolic blood pressure (P > 0.05 for all). Throat wound in the oral intubation group, and epistaxis in nasal intubation group without help of Magill forceps was significantly higher than in other groups (P < 0.05). Moreover, the duration of intubation in the oral procedure was significantly lower than other methods (P < 0.05).

    Conclusion

    All three methods of oral, and nasal, with and nose without the help of Magill forceps, cause similar changes in heart rate and blood pressure. The only difference between these three methods is the side effects caused by intubation.
