صفحات 1308-1313مقدمهتمرینات ورزشی از طریق تغییر غلظت عوامل رشدی و گیرنده های آن ها می تواند بر هایپرتروفی فیزیولوژیک قلب (که برای عملکرد قلب مطلوب است) تاثیر داشته باشد. هدف از انجام این مطالعه، بررسی تاثیر هشت هفته تمرین مقاومتی بر گیرنده ی کینازی سرین/ترائونین نوع 2 بتا (ActRIIB) و سطوح پلاسمایی عامل متمایز کننده ی رشد 11 (Growth differentiation factor 11 یا GDF11) و عامل متمایز کننده ی رشد 8 (GDF8) در هایپرتروفی فیزیولوژیک قلب RATهای نر نژاد Wistar بود.روش هاتعداد 14 سر موش صحرایی Wistar نر 3 ماهه پس از طی دوره ی آشناسازی، به صورت تصادفی به 2 گروه 7تایی شاهد و مورد تقسیم شدند. تمرین مقاومتی شامل هشت هفته و هفته ای 5 جلسه صعود از یک نردبان 1 متری بود. GDF11 و GDF8 پلاسما به روش Enzyme-linked immunosorbent assay (ELISA) و ActRIIB بافت بطن چپ به روش Immunohistochemistry (IHC) اندازه گیری شد. داده ها با استفاده از آزمون های Independent t و Mann-Whitney تجزیه و تحلیل شدند. 050/0 > P به عنوان سطح معنی داری در نظر گرفته شد.یافته هامقایسه ی بین گروهی، اختلاف معنی داری بین گروه های مورد و شاهد در متغیرهای وزن قلب (004/0 = P) ، نسبت وزن قلب به وزن بدن (045/0 = P) ، سطوح پلاسمایی GDF11 (001/0 = P) و GDF11/GDF8 (015/0 = P) نشان داد، اما اختلاف معنی داری در متغیرهای ActRIIB (768/0 = P) و GDF8 (295/0 = P) مشاهده نشد.نتیجه گیریبا توجه به نتایج تحقیق حاضر، به نظر می رسد تمرین های مقاومتی با اثرات مطلوب بر عوامل رشدی یک راهبرد مهم در جهت هایپرتروفی فیزیولوژیک قلبی باشد.کلیدواژگان: تمرینات قدرتی، عامل متمایز کننده ی رشد، عامل متمایز کننده ی رشد 8، هایپرتروفی قلبی

صفحات 1314-1320مقدمهتعیین الگوی توزیع چربی کبد در بیماران کبد چرب غیر الکلی از نقطه نظر تشخیص شدت بیماری حایز اهمیت است. هدف از انجام مطالعه ی حاضر، بررسی توزیع فضایی چربی کبد با تکنیک گرادیان اکوی هم فاز- غیر هم فاز، در بیماران کبد چرب غیر الکلی بود.روش هااین مطالعه ی مقطعی، آینده نگر پس از تاییدکمیته ی اخلاق، با رضایت آگاهانه بر روی 38 داوطلب در بازه ی زمانی بیشتر از دو سال انجام شد. معیارهای ورود، شامل بیماران بزرگ سالی بود که کبد چرب غیر الکلی در گزارش بیوپسی آنان به تایید رسیده بود. معیارهای خروج، شامل حاملگی، مصرف الکل، فوبیای Magnetic resonance imaging (MRI) ، سابقه ی بیماری های کبدی و استعمال داروهای کنترل کننده ی چربی بین بیوپسی و MRI بودند. سکانس تصویربرداری گرادیان اکوی هم فاز- غیر هم فاز با دستگاه 5/1 تسلا انجام شد. Fat fraction سگمنت های مختلف کبد محاسبه و با یکدیگر مقایسه شدند. از آمار توصیفی، Regression خطی و مجموعه ای از آزمون های آماری Spearman، Kruskal-Wallis و Mann-Whitney با در نظر گرفتن سطح معنی داری 050/0 > P در نرم افزار SPSS استفاده شد.یافته ها38 بیمار (شامل 22 مرد و 16 زن) با میانگین سن (6/10 ± 4/42 سال) بررسی شدند. مقدار چربی سگمنت های 3-1 (22/10 ± 63/11 درصد) ، نواحی سطحی (67/11 ± 00/19 درصد) و عمقی سگمنت های 8-4 (59/12 ± 01/23 درصد) و بخش سطحی سگمنت 5 (19/12 ± 91/20 درصد) و کل کبد (57/11 ± 44/18 درصد) محاسبه گردید. ارتباط خطی مستقیم بین مقدار چربی ناحیه ی سطحی سگمنت 5، با یافته های بیوپسی مشاهده گردید (001/0 = P، 86/0 = 2R). مقدار میانگین چربی سگمنت های 3-1، از سایر نواحی آناتومیک کمتر (050/0 > P) و اختلاف مقدار چربی سایر گروه ها معنی دار نبود (050/0 < P).نتیجه گیریتوزیع فضایی چربی کبد در بیماران کبد چرب غیر الکلی، غیر یکنواخت است.کلیدواژگان: تصویربرداری تشدید مغناطیسی، گرادیان اکوی هم فاز- غیر هم فاز، کبد چرب غیر الکلی، توزیع چربی

صفحات 1321-1328مقدمهتغییرات هورمونی و جسمی در دوره ی بعد از زایمان بر عملکرد جنسی زنان تاثیرگذار است. عملکرد جنسی، به طور مستقیم با رضایت جنسی افراد در ارتباط است. گفتگوی زوج پیرامون مسایل جنسی، خواسته ها و ترجیح های جنسی در برقراری رابطه ی زناشویی رضایت بخش تاثیر به سزایی دارد. مطالعه ی حاضر با هدف بررسی رابطه ی بین عملکرد جنسی و خود ابرازی جنسی زنان بعد از زایمان انجام شد.روش هااین مطالعه ی همبستگی، بر روی 160 زن مراجعه کننده به مراکز بهداشتی- درمانی مشهد که در بازه ی زمانی 4 هفته تا 6 ماه پس از زایمان بودند، در سال 1396 انجام شد. ابزار پژوهش، پرسش نامه های مشخصات دموگرافیک و زناشویی و شاخص عملکرد جنسی زنان و پرسش نامه ی خود ابرازی جنسی Hurlbert بود. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS و آزمون های Kruskall-Wallis و همبستگی Spearman با سطح معنی داری 050/0 > P انجام شد.یافته هاضریب همبستگی Spearman بین خود ابرازی جنسی و عملکرد جنسی با مقدار 600/0 معنی دار بود (001/0 > P).نتیجه گیریبا توجه به همبستگی مثبت بین عملکرد جنسی و خود ابرازی جنسی در دوره ی بعد از زایمان، پیشنهاد می شود جهت بهبود عملکرد جنسی زنان، ماماها در ضمن مراقبت های بعد از زایمان با استفاده از روش های مشاوره ای مختلف، اقدام به افزایش خود ابرازی جنسی زنان نمایند.کلیدواژگان: عملکرد جنسی، خود ابرازی، پس از زایمان

صفحات 1329-1335مقدمهمطالعه ی حاضر، با هدف تعیین تاثیر اسپیرنولاکتون در میزان کنترل پرفشاری خون ریوی در بیماران مبتلا به بیماری انسدادی مزمن ریوی مراجعه کننده به بیمارستان های الزهرا (س) و شهید چمران اصفهان در سال های 96-1395 به انجام رسید.روش هاطی یک مطالعه ی کارآزمایی بالینی، 80 بیمار مبتلا به انسداد مزمن ریوی در دو گروه 40 نفره توزیع شدند. گروه مورد، اسپیرنولاکتون با دز 25 میلی گرم دریافت کردند و گروه شاهد، درمان معمول انجام شد و اسپیرونولاکتون تجویز نشد. تمام بیماران، در بدو شروع و شش ماه بعد از شروع درمان، تحت اکوکاردیوگرافی و اسپیرومتری قرار گرفتند و تغییرات پارامترهای اسپیرومتری و اکوکاردیوگرافی در دو گروه مقایسه شد.یافته هامیانگین فشار خون ریوی در قبل از درمان در دو گروه شاهد و مورد به ترتیب 90/6 ± 83/38 و 00/8 ± 65/39 میلی متر جیوه بود (560/0 = P). این میانگین در 6 ماه بعد از درمان در دو گروه مورد و شاهد به ترتیب به 12/6 ± 73/21 و 42/7 ± 50/24 میلی متر جیوه رسید (080/0 = P). اختلاف میانگین فشار خون ریوی قبل و بعد از مداخله در گروه شاهد 20/5 ± 40/25 و در گروه مورد 20/5 ± 60/27 میلی متر جیوه بود و اختلاف معنی داری بین دو گروه مشاهده نشد. همچنین، بهبودی شاخص های اسپیرومتری و اکوکاردیوگرافی در این دو گروه اختلاف معنی داری نشان نداد.نتیجه گیریبر اساس یافته های این مطالعه، مصرف اسپیرونولاکتون در بیماران مبتلا به بیماری انسدادی مزمن ریه تاثیر معنی داری در کاهش فشار خون ریوی، شاخص های اسپیرومتری و اکوکاردیوگرافی ندارد و لازم است مطالعات بیشتری در این زمینه انجام گیرد.کلیدواژگان: بیماری انسدادی مزمن ریه، اسپیرونولاکتون، فشار خون ریوی

مقدمه: با توجه به وضعیت بحرانی بیماران بستری در بخش مراقبت های ویژه، این بیماران به شدت در معرض تهدید حیات می باشند و احیای قلبی- ریوی (Cardiopulmonary resuscitation یا CPR) یکی از اقدامات رایج در این بخش است. لازم است پزشکان و پرسنل در این بخش، به اصول اولیه و پیشرفته ی حمایت از حیات آشنا باشند. از این رو، مطالعه ی حاضر با هدف بررسی پیامد بیماران احیا شده ی قلبی- ریوی داخلی و جراحی بستری در بخش مراقبت های ویژه ی بیمارستان امام خمینی (ره) ارومیه انجام شد.روش هادر این مطالعه ی مقطعی، پرونده ی بیماران مورد احیای قلبی- ریوی بستری در بخش مراقبت های ویژه در مدت 7 سال (95-1389) مورد بررسی قرار گرفت. مشخصات دموگرافیک، داروهای به کار رفته در جریان CPR، زمان ایست قلبی- ریوی و زمان انجام و پیامد CPR، استخراج و مورد تجزیه و تحلیل قرار گرفت.یافته هادر بازه ی زمانی مورد مطالعه، تعداد 6365 بیمار در بخش مراقبت های ویژه ی بیمارستان امام خمینی (ره) ارومیه بستری شده بودند که از این تعداد، 728 مورد (43/11 درصد) دچار ایست قلبی- ریوی شده و مورد CPR قرار گرفته بودند. از 728 CPR انجام شده، 117 مورد (1/16 درصد) موفق و 611 مورد (9/83 درصد) ناموفق بود. از 433 نفر بیمار داخلی، 52 نفر (0/12 درصد) و از 295 نفر بیمار جراحی، 65 نفر (0/22 درصد) عملیات احیای قلبی- ریوی موفق داشتند (001/0 = P). دیس ریتمی قابل مشاهده در بین بیماران داخلی و جراحی، تفاوت آماری معنی داری داشت (001/0 = P). میانگین طول مدت احیای قلبی- ریوی در بیماران داخلی 90/9 ± 72/32 و در بیماران جراحی، 90/8 ± 02/33 دقیقه بود (600/0 = P).نتیجه گیریدر مطالعه ی حاضر، زمان CPR، استفاده از داروها با تعداد و تکرار دز، در بیماران با CPR ناموفق، بیشتر بود. فراوانی CPR موفق قابل قبول بود.کلیدواژگان: احیای قلبی- ریوی، بررسی پیامد، بخش مراقبت های ویژه

Pages 1308-1313BackgroundExercise training can be effective on cardiac physiological hypertrophy (that is favorable for cardiac function) by changing the concentration of growth factors and their receptors. The aim of this study was to determine the effects of eight weeks of resistance training on activin receptor type II-B (ActRIIβ), as well as plasma levels of growth differentiation factor 11 (GDF11) and growth differentiation factor 8 (GDF8) in cardiac physiological hypertrophy in male Wistar rats.MethodsAfter the familiarization period, fourteen 3-month-old male Wistar rats were randomly divided into control and training groups (n= 7). Resistance training included 8 weeks and 5 sessions per week climbing from a 1-meter ladder. GDF11 and GDF8 of plasma were measured using enzyme-linked immunosorbent assay (ELISA) method, and ActRIIB of left ventricular myocardium was measured using immunohistochemistry (IHC) method. Data were analyzed using independent t test and Mann-Whitney U at the significance level of P < 0.050. Findings: There was a significant difference between the training and control groups in heart weight (P = 0.004), heart weight/body weight (P =0.045), GDF11 (P = 0.001), and GDF8/GDF11 (P = 0.015). There was not any significant differences between the training and control groups in ActRIIβ (P = 0.768) and GDF8 (P = 0.295) levels.ConclusionAccording to the results of the present study, it seems that resistance training with favorable effects on growth factors is an important strategy in cardiac physiological hypertrophy.Keywords: Resistance training, Activin receptor type II-B, Growth-differentiation factor 11, Growth differentiation factor 8, Cardiac hypertrophy

Pages 1314-1320BackgroundThe aim of this study was to evaluate the spatial distribution of the liver fat with in-phase and out-of-phase gradient echo imaging technique in patients with nonalcoholic fatty liver disease (NAFLD).MethodsThis cross-sectional prospective study was conducted on 38 volunteers after approval by the Ethics Committee and obtaining informed consent, in the period of more than two years. Inclusion criteria were as being adult with confirmed NAFLD in biopsy findings, and exclusion criteria included pregnancy, alcohol consumption, claustrophobia, history of liver disease, and using lipid control drugs between biopsy and magnetic resonance imaging (MRI). The in-phase and out-of-phase sequences was performed with a 1.5 Tesla scanner. Fat fractions were calculated for different parts of the liver, and compared with each other. Descriptive statistics, linear regression, and a set of Spearman, Kruskul-Wallis, and Mann-Whitney tests were used with a significance level of P < 0.050 in SPSS software. Findings: 38 patients (22 men and 16 women) with mean age of 42.4 ± 10.6 years were investigated. The fat content for segments 1 to 3 (11.63 ± 10.22 percent), superficial (19.00 ± 11.67 percent) and deep (23.01 ± 12.59 percent) regions of segments 4 to 8, superficial region of segment 5 (19.00 ± 11.67 percent), and the whole liver (18.44 ± 11.57 percent) was measured. A direct linear relationship was found between the fat content of the surface region of segment 5 and biopsy findings (R2 = 0.86, P = 0.001). The average fat content of segments 1 to 3 was lower than other anatomical regions (P < 0.050), and the difference in fat content of other groups was not significant (P > 0.050).ConclusionThe spatial distribution of liver fat is heterogeneous in patients with NAFLD.Keywords: Magnetic resonance imaging, Nonalcoholic fatty liver disease, Body fat distribution

Pages 1321-1328BackgroundHormonal and physical changes during postpartum have significant effects on women's sexual function. Sexual function is directly related to sexual satisfaction. Couple conversation about sexual issues, desires, and preferences has a significant impact on marital relationships. This study aimed to assess the relationship between sexual function and sexual self-disclosure in women after childbirth.MethodsThis correlation study was performed on 160 women referring to Mashhad City health centers, Iran, in the period from 4 weeks to 6 months postpartum. The research instruments consisted of demographic and sexual characteristics questionnaire, Female Sexual Function Index (FSFI), and Hurlbert Sexual Self-Disclosure Questionnaire. Data analysis was performed using SPSS software, and Kruskal-Wallis and Spearman correlation tests with significance level of P < 0.050. Findings: Spearman's correlation coefficient between sexual self-disclosure and sexual function with an amount of 0.600 was significant (P < 0.001).ConclusionConsidering the positive correlation between sexual function and sexual self-disclosure after childbirth, it is suggested that, in the context of postpartum care, midwives use different counseling methods to increase women's sexual self-disclosure.Keywords: Sexuality, Self disclosure, Childbirth

Pages 1329-1335BackgroundThis study aimed to determine the effect of spironolactone in control of pulmonary hypertension in patients with obstructive pulmonary disease (COPD) who admitted to Alzahra and Chamran hospitals in Isfahan, Iran, during the years 2017-2018.MethodsIn a clinical trial study, 80 patients with COPD were selected and randomly divided in to two groups of 40. The first group received spironolactone at a dose of 25 mg, and the second group received routine treatment (without spironolactone). All patients underwent echocardiography and spirometry, and spirometry and electrocardiography parameters were compared between the two groups at the beginning and six months after the start of treatment. Findings: The mean pulmonary arterial pressure was 38.83 ± 6.9 and 39.65 ± 8.8 mmHg (P = 0.560), and 21.73 ± 6.12 and 24.5 ± 7.42 mmHg (P = 0.080), in control and intervention groups, before and 6 months after the treatment, respectively. The mean difference of pulmonary arterial pressure before and after the treatment was 25.4 ± 5.2 and 27.6 ± 5.2 mmHg in control and intervention groups, respectively, and no significant difference were seen between the two groups. Moreover, the improvements of spirometry and echocardiography indices were not statistically different between the two groups.ConclusionUsing spironolactone in patients with COPD has no significant effect on pulmonary hypertension, and spirometry and echocardiography indices, and more studies are needed in this regard.Keywords: Chronic obstructive pulmonary disease, Spironolactone, Pulmonary hypertension

Pages 1336-1342BackgroundPatients admitted to the intensive care unit (ICU) due to their critical conditions are at life-threatening situations, and cardiopulmonary resuscitation (CPR) is one of the most commonly used treatments in this area. Physicians and staff working in this section should be familiar with the basic and advanced life support. This study aimed to assess the outcomes of cardiopulmonary resuscitation in patients with surgical and non-surgical diseases in ICU of Imam Khomeini hospital in Urmia City, Iran.MethodsIn this cross-sectional study, the medical record of patients admitted to the ICU and underwent CPR during a 7-year period (2010-2016) were analyzed. Demographic characteristics, medicines used in the course of CPR, time of cardiopulmonary arrest, duration of CPR, and its outcome were extracted and analyzed. Findings: Of 6365 patients admitted to the ICU of Imam hospital in Urmia City, 728 (11.43%) had cardiac arrest and received CPR. Of 728 CPRs performed in the ICU, 117 cases (16.1%) were successful and 611 (83.9%) were unsuccessful. Out of 433 patients with non-surgical diseases, 52 (12%) had successful CPR, and out of 295 patients with surgical diseases, 65 (22%) had successful CPR (P = 0.001). There was a statistically significant difference between patients with non-surgical and surgical diseases in visible dysrhythmia (P = 0.001). The mean duration of CPR was 32.72 ± 9.90 and 33.02 ± 8.90 minutes in patients with non-surgical and surgical diseases, respectively (P = 0.600).Keywords: Cardiopulmonary resuscitation, Outcome studies, Intensive care unit
