صفحات 148-153آرتروز زانو یکی از مهمترین علل ناتوانی سنین بالا است. بالا رفتن سن همراه افزایش آرتروز زانو است. سن متوسط افراد جامعه رو به افزایش است. در 30 سال آینده ایران وارد جرگه ممالکی میشود که 30% جمعیت آن بیشتر از 65 سال خواهد داشت. هزینه درمان آرتروز رقم قابل توجهی از تولید ناخاص داخلی را دربرمیگیرد. آرتروز زانو درنتیجه عدم تعادل بین تخریب و ترمیم غضروف مفصلی تحت تاثیر فاکتورهای ریسک متعددی مانند تروما؛ زیادهروی «استفاده زیاد» و تمایل ژنتیکی ایجاد میشود. افزایش وزن و اختلالات مفصل بار افزونی را به نقاط بخصوصی از زانو وارد مینماید این بار اضافی عکسالعملی بافتی به بار میآورد که شرایط وقوع آرتروز را ایجاد مینماید. تاکنون برای آرتروز (1) علاجی پیدا نشده است. گرچه تعویض مفصل چارهای درجه یک بحساب میآید ولی بقا و پایندگی آن همیشگی نیست عدهای از بیماران نمایلی به عمل جراحی ندارند؛ برای عدهای عمل جراحی صلاح نیست و بالاخره اینکه هزینه عمل جراحی بالا است. درمان آرتروز زانو بدون تعویض مفصل روشی دیگر برای مقابله با این مشکل است. آکادمی ارتوپدی آمریکا با مطالعات گسترده و بررسی متون پزشکی رهبرد «گایدلاین» درمان بدون تعویض مفصل زانو را تهیه و در اختیار جامعه ارتوپدی قرار داده است. این رهبرد بشارت کیفیت زندگی همپای عمل جراحی را میدهد. آکادمی بکارگیری آن را به تمام جراحان ارتوپد توصیه مینماید و همچنین خواستار سنجش پیامد این رهبرد و پژوهشهای توام میباشد. از آنجائی که این روش درمانی ساده و کم هزینه است میتواند بهترین درمان آرتروز باشد. کافی خواهد که مراکزی را بهوجود آورد که بیماران سرگردان مطب پزشکان را درخود جا بدهند و با اتکا به توصیههای رهبرد آکادمی این روش درمانی مناسب را بدست گیرند و با پیگیری و ثبت پیامدها اعتبار آن را به ثبوت برسانند.کلیدواژگان: آرتروز زانو، درمان غیر آرتروپلاستی، گایدلاین انجمن جراحان ارتوپدی آمریکا

صفحات 154-160پیش زمینهکمردرد یکی از شایعترین بیماریهایی است که شخص را نیازمند مداخله پزشکی میکند. از آن رو که جراحی آخرین راه درمان است، پیشبینی برنامهریزی روند انجام جراحی مفید و تا حدی ضروری به نظر میرسد،که لازمه آن در قدم اول داشتن مدل بیومکانیکی صحهگذاری شده براساس اطلاعات آناتومیکی ستونفقرات بیمار است. با وجود پیشرفت بسیار در این زمینه همچنان نیاز به مدلی که هم بتواند پارامترهای آناتومیکی مهم را در بربگیرد و هم از نظرکاربرد کلینیکی قابل استفاده باشد، احساس میشود.مواد و روشهاهدف این مقاله توسعه مدل اجزا محدود شخصیسازی شده ستونفقرات کمری با تعداد 23 پارامتر آناتومیکی است. دادههای اولیه از عکس رادیولوژی میانگین افراد سالم استخراج شده و در نرمافزار کتیا طراحی شده است. سپس مدل دقیق اجزا محدود در نرمافزار آباکوس تهیه گردیده است و نتایج محدوده حرکتی سگمنتهای حرکتی در حرکات فلکشن، اکستنشن و خمشدن به چپ و راست با نتایج مطالعه تجربی موجود در ادبیات تحقیق صحهگذاری شده است.یافتههابه منظور مشاهده کاربرد مدل پارامتری شخصیسازی شده از ستونفقرات کمری، مدلی از یک بیمار پس از جراحی فیوژن ناحیه کمری ارائه شد و نتایج محدوده حرکتی سگمنتهای حرکتی و فشار می اندیسکی با مدل سالم مقایسه گردید.نتیجهگیریباتوجه به اینکه نتایج قابل قبولی در هر مرحله مشاهده گردید، میتوان با داشتن مدل پارامتری شخصیسازی شده وگسترش مدل برای هر بیمار، به پیشبینی نتیجه عمل جراحی فیوژن و مقایسه نتایج بیومکانیکی در صورت کمتر یا بیشتر کردن سطوح فیوژن پرداخت که میتواند گام موثری در کلینیک باشد.کلیدواژگان: ستون فقرات، مدل اجزا محدود، مکانیک، عمل فیوژن، مدل شخصیسازی شده

صفحات 161-166پیش زمینهشکستگیهای ناپایدار مچ پا یکی از متداولترین علل مراجعه به بخش عمومی بیمارستان میباشد و جراحی برای بیحرکتی ناحیه شکسته شده بهعنوان راهکار مناسبی برای بهبود وضعیت بیماران پس از مرخص شدن به اثبات رسیده است. هرکدام از مراحل مدیریت بیمار میتواند بر طول مدت دوران نقاهت و وضعیت بیمار نقش موثری ایفا کند. هدف از این مطالعه صورت گرفته، تعیین نحوه مدیریت بیمار از لحظه پذیرش تا زمان مرخص شدن میباشد و همچنین اثرات محسوس و قابلتوجه تنوع در روشهای مدیریت بیمار نیز مورد بررسی قرار گرفته است.مواد و روشهادادههای مورد آنالیز قرار گرفته مربوط به 49 بیمار است که بین آوریل تا دسامبر 2017 به مرکز درمانی نیمه خصوصی HEFT انگلستان مراجعه کردهاند و مشکل شکستگی ناپایدار مچ پا داشتهاند. گفتنی است که تمام مراحل از زمان پذیرش در بخش اورژانس مرکز تا زمان ترخیص مورد بررسی قرار گرفته است. همچنین یادداشتهای بعد از جراحی نیز مرور شدهاند. گرداوری دادهها نیز از طریق منبع TARN انجام شده است.یافتههاهمانطور که گفته شد، 49 بیمار در مدت ذکر شده به مرکز مراجعه کردند که مشکل شکستگی ناپایدار مچپا داشتهاند. در مدتی که بیماران در بخش اورژانس قرار داشتند برای کمتر از 50% آنها در مدتی کمتر از 30 دقیقه پس از رسیدن به مرکز تشخیص رادیوگرافی انجام شده بود. 25 بیمار نیز پس از حدود 4 ساعت به بخش تخصصی پذیرش شده بودند، در حالی که برخی تا بیش از 9 ساعت مجبور به انتظار شدند. 27 بیمار (بیش از 50%) در حدود 2 روز پس از پذیرش تحت عمل جراحی قرار گرفته بودند، و 6 نفر هم بیش از 6 روز برای جراحی خود معطل مانده بودند که گروه اول دوره بستری ماندن پس از جراحی کمتری نیز داشتند. برای 47 نفر از بین 49 نفر داروهای ترومبوپروفیلاکسی (ضدلختگی) تجویز شد، به تمامی بیماران توصیه شد که حداقل تا 6 هفته بار سنگینی را روی پای خود وارد نکنند و خوشبختانه هیچگونه عوارض جانبی و مرگ و میر پس از جراحی گزارش نشده است.نتیجهگیریدرمان سریعتر شکستگی ناپایدار مچپا، دوره بستری در بیمارستان را کوتاهتر میکند.کلیدواژگان: شکستگی مچ پا، آسیب های مچ پا، دوره بستری، ترخیص بیمار

صفحات 167-170پیش زمینهاستئوتومی میچل بهعنوان یکی از روش های تصحیح تغییر شکل والگوس شست پا در قسمت دیستال متاتارسال اول میباشد. هدف از این مطالعه، بررسی نتایج عمل استئوتومی میچل در بیماران دارای هالوکس والگوس بود.
هادر طی سالهای 90 تا 94، در مجموع 18 بیمار تحت عمل جراحی قرار گرفتند. نتایج بالینی با استفاده از معیار امتیازدهی انجمن جراحان ارتوپد پا و مچ پا آمریکا (هالوکس متاتارسوفالانژیال و مفصل بین انگشت شست پا) ، میزان درد بر اساس مقیاس آنالوگ بصری و تغییرات زوایای هالوکس والگوس و اینترمتاتارسال با حداقل فاصله زمانی یک سال بعد از عمل جراحی و Paired sample t-testیافتهها6/5 و 2/5±8/5 قبل از عمل به 2/1± نتیجه گیری شود.کلیدواژگان: هالوکس والگوس، شست پا، استوتومی میشل -
صفحات 171-174پیش زمینهناهنجاریهای مادرزادی پا یکی از شایعترین ناهنجاریهای مادرزادی است که موجب اختلال در راهرفتن طبیعی میشود و بر کیفیت زندگی بیماران تاثیر میگذارد. هدف از این مطالعه بررسی شیوع چنین ناهنجاریهایی در نوزادان متولد شده در مرکز مراجعه در شمال ایران است.مواد و روشهااین مطالعه، یک مطالعه توصیفی مقطعی بر روی تمام نوزادان متولد شده در یک دوره دو ساله در بیمارستان امام خمینی ساری بود. متغیرهایی که در مطالعه ما مورد بررسی قرار گرفت، سن حاملگی در زمان زایمان، شرایط بالینی که بارداری را پیچیده میکند مانند دیابت، فشار خون مزمن، عوارض ناشی از بارداری نظیر اوگلوهیدرمانیا، پلی هیدرامنیوس و مالپرس، سن مادر، دیابت مزمن، فشار خون بالا، اعتیاد مادران، جنسیت و وزن تولد نوزاد، تاریخچه داشتن یک کودک با ناهنجاری اندام مادرزادی و نوع زایمان است. عکسبرداری از پاهای نوزادان در اولین روز، تولد توسط جراح ارتوپدی ارزیابی شد. علاوه بر این، تمام نوزادان یک روز پس از زایمان توسط یکی از دو متخصص اطفال مورد بررسی قرار گرفتند و موارد مشکوک به جراح ارتوپد ارجاع شدند. تجزیه و تحلیل آماری با استفاده از نرم افزار SPSS انجام شد.یافتههااز 2025 نوزاد مورد مطالعه، 210 مورد (4/10%) کالکالئووالگوس، 7 (0. 3%) پاچنبری، 156 مورد (7%) موارد "Pes Varus"، 22 مورد (1. 1%) متارسوس آدوکتوس و 1 مورد پلی آداکتیل مشخص گردید. در میان همه متغیرها، تنها سن مادر و سیگار کشیدن در دوران بارداری با ناهنجاری های مادرزادی پا ارتباط داشت.نتیجهگیریدر مطالعه ما، شایعترین ناهنجاری مادرزادی پا کالکالئووالگوس و "Pes Varus" موضعی خفیف پا بود. میزان کم پاچنبری ، پلی داکتیلی و متارسوس آدوکتوس قابل توجه است.کلیدواژگان: پاچنبری، ناهنجاری، مادرزادی، شیوع، نوزادان

صفحات 175-177آکرومتاستاز، یعنی متاستاز یک تومور بدخیم به استخوان، ولی دیستال به آرنج و/یا زانو یک اتفاق بسیار نادر می باشد. یک زن سی و یک ساله، مورد شناخته شده کانسر پستان از 3 سال قبل با تورم انگشت جهارم دست راست مراجعه کرد. رادیوگرافی و در مرحله بعد بیوپسی متاستازهای مجزا به بندهای وسط و آخر انگشت را نشان دادند، که اتفاق بینهایت نادری است. درمان با آمپوتاسیون ری چهارم انجام شد. چهار ماه بعد متاستاز دوم اتفاق افتاد، این بار به تیبیای راست، که این هم محل نادری برای اتفاق افتادن متاستاز است. او از متاستاز ریوی 9 ماه بعد از متاستاز دست فوت کرد. آکرومتاستاز یک یافته نادر و پیشبینی کننده زندهماندن کوتاه مدت است.کلیدواژگان: متاستاز نئوپلاسم، دست، تیبیا

Pages 148-153Knee osteoarthritis is one of the most important causes of disability in elderly. Increasing age is associated with increased knee osteoarthritis. The average population age is increasing. In the next 30 years, Iran will enter the club of countries with the 30% of the population aged over 65. The cost of osteoarthritis treatment accounts for a significant amount of each country’s GDP. Knee osteoarthritis is caused by an imbalance between the destruction and repair of articular cartilage under the influence of several risk factors such as trauma, excessive use, and genetic predisposition. Overweight and joint disorders apply an increased load to the specific points of the knee. This overload creates a tissue response, which provides the conditions for the occurrence of osteoarthritis. No cure has been found for knee osteoarthritis so far. Although joint replacement (knee arthroplasty) at this point is the best solution, its survival and durability is not permanent. Some patients are not suitable for surgery; some are reluctant to have surgery, and finally, the cost of surgery is high. Treating knee osteoarthritis without joint replacement is another method to deal with this problem. Through extensive studies and review of medical literature, The American Academy of Orthopaedic Surgeons has prepared the Guideline for Non-Arthroplasty Treatment of Knee Osteoarthritis. This Guideline brings the promise of quality of life equal to the surgical approach. The Academy recommends its use to all orthopaedic surgeons and also calls for an assessment of the outcome studies. Nonarthroplasty treatment of knee osteoarthritis is a simple and inexpensive treatment method. The majority of patients will benefit from this treatment. Pages 154-160BackgroundLow back pain is one of the most common problems that force individuals to seek medical care. Since surgery is the last treatment strategy, predicting the process of conducting surgical procedures seems beneficial and somehow crucial. In this regard, the first step is having a validated biomechanical model based on the anatomical parameters of patients’ lumbar spine. Despite the impressive progress in this field, there is still a need to designing a model that could include important anatomical parameters and be applicable in terms of clinical applications.MethodsThis study aimed to develop the personalized spinal finite element model with 23 anatomical parameters. The initial data was extracted from the radiology picture of the average healthy volunteers and was designed in Catia software. Afterwards, the finite element model was analyzed in Abaqus, and results of the range of motion of motionsegments in movements of flexion, extension and left and right lateral bending were verified based on the results of experimental studies present in the literature.ResultsIn order to observe the application of the patient-specific spinal parametric model, a model of a patient after spinal fusion was presented. Moreover, results of the range of motion of the motion segments and intradiscal pressure were compared to the healthy model.ConclusionSince acceptable results were obtained at each step, it is possible to predict the result of spinal fusion and compare the biomechanical results in case of decreased or increased fusion level by developing a parametric parient-specific model for each patient, which can be an effective achievement for clinical fields.Keywords: Spine, Finite ElementAnalysis, Mechanics, Spinal Fusion, Patient-Specific Modeling

Pages 161-166IntroductionUnstable ankle fractures are a common presentation and operative intervention has proven to improve patient outcomes. Each stage of patient management can have an impact on patient length of stay and clinical outcome. The aim of the present study is to assess how we manage the patient at each stage from their presentation right through to discharge, and if/how variation in practice differed significantly.MethodsFrom April 2017 to December 2017, in the Heart of England NHS Foundation Trust (HEFT), we retrospectively analysed the data of 49 patients with a diagnosis of an unstable ankle fracture. The patient journey was assessed from presentation to the Emergency Department (ED) right through to discharge. Post-operative notes were also reviewed. Data was obtained through our Trauma Audit and Research Network (TARN).Results49 patients had surgical fixation for their ankle fracture. Whilst in the ED, less than 50% of the patients had a radiographic diagnosis of an ankle fracture within 30 minutes of arrival.25 patients were admitted to the ward within 4 hours, with some patients having to wait more than 9 hours.27 patients (more than 50%) had their surgery within 2 days after being admitted, in comparison to 6 patients who had to wait more than 6 days for their operation. The former were found to have shorter hospital length of stay post-operatively.47 out of the 49 patients were given chemical thromboprophylaxis, all patients were advised to not weight bear for a minimum of 6 weeks, and no post-operative morbidity or mortality was encountered.ConclusionWe found an association between early ankle fracture fixation, and reduced post-operative hospital Length of Stay (LOS).Keywords: Ankle fracture, Ankle injuries, Length of stay, Patient Discharge

Pages 167-170BackgroundMitchell osteotomy is one of first metatarsal distal corrective osteotomies for hallux valgus deformity. The purpose of this study was to evaluate the outcome of Mitchellosteotomy.MethodsEighteen patients underwent Mitchell corrective osteotomy using screw fixation for hallux valgus deformities from 2011 to 2015 were included. Clinical outcome was assessed using American Orthopedic Foot and Ankle Score, Hallux metatarsophalangeal-interphalangeal scale, (AOFAS Hallux), visual analogue score, and changes in hallux valgus and intermetatarsal angles based on standard weight-bearing radiographs, at least one year after surgery.ResultsAbout 95 % of patients were completely satisfied. Mean of AOFAS score was 86.7 at final follow-up. The mean of changes in intermetatarsal and hallux valgus angles were 5.6 ± 3.1 and 17.0 ± 5.2 respectively. Pain of the cases based on visual analogue score decreased from 5.8 ± 0.8 preoperatively to 1.3 ± 1.2 postoperatively. There were no deep infections, non::union:: or osteonecrosis of first metatarsal head.ConclusionMitchell corrective osteotomy with screw fixation could bea simple and effective procedure to correct hallux valgus deformity with high levels of patient satisfaction.Keywords: Hallux Valgus, Big Toe, Mitchell Osteotomy

Pages 171-174Backgroundcongenital foot deformities are among the most common congenital abnormalities causing morbidities, disturbing the normal gait and affecting patients’ quality of life. The aim of our study was to find out the prevalence of such abnormalities in neonates born at a referral center in northern Iran.MethodsThis study was a descriptive cross-sectional study on the neonates born in Imam Khomeini hospital of Sari in a two-year period. The variables considered in our study were gestational age at the time of delivery, medical conditions complicating the pregnancy including diabetes, chronic hypertension, obstetrical complications like olygohydramnios, polyhydramnios, and malpresentations, mothers' age, chronic diabetes, hypertension, mothers' addiction, sex and birth weight of the newborn, history of having a child with congenital limb anomaly and the type of delivery. Photography of newborns' feet was taken on first day of birth to be assessed by an orthopaedic surgeon. In addition, the babies were examined the day after birth by one of the two neonatologists and the suspicious cases were referred to the orthopedic surgeon. Statistical analysis was performed by SPSS software.ResultsFrom 2025 neonates studied, 210(10.4%) calcaneovalgus, 7 (0.3%) clubfoot, 156(7%) cases of ''PesVarus'', 22 cases of (1.1%) metatarsus adductus, and 1 case of polydactyl were recognized. Among all variables, only mothers' age and cigarette smoking during pregnancy had association with congenital foot abnormalities.ConclusionThe most prevalent congenital abnormalities in our study was Pes Calcaneovalgus and a positional mild ''Pes Varus''. Pages 175-177Acrometastasis or metastasis distal to elbow and/or knee is a very rare event. A 31 y/o female, a known case of breast cancer for 3 years presented with a swollen 4th right finger. Radiographs and then biopsy showed separate metastases to middle and distal phalanges, an extremely rare event. She was managed with ray amputation. Four months later she developed a second metastasis, this time to her left tibia, again a rare site for metastasis, that was managed by closed intramedullary nailing. Sadly, she died of pulmonary metastasis, 9 months after her hand event. Acrometastasis is a rare finding and predicts a short survival.Keywords: Neoplasm Metastasis, Hand, Tibia, finger
