صفحات 1-8مقدمهپیشرفت تحصیلی دانشجویان دندان پزشکی، به رغم استعداد ذاتی و دستاوردهای تحصیلی قابل توجه، تحت الشعاع انگیزش تحصیلی قرار می گیرد. مطالعه ی حاضر، به بررسی نقش انگیزش تحصیلی در پیشرفت تحصیلی دانشجویان دندان پزشکی دانشگاه علوم پزشکی کرمان پرداخته است.مواد و روش هادر این مطالعه ی مقطعی، تعداد 172 دانشجوی دندان پزشکی سال های سوم تا آخر مورد بررسی قرار گرفتند. دانشجویان، مقیاس انگیزش تحصیلی را که دارای 37 سوال و 8 خرده مقیاس می باشد، تکمیل نمودند. معیار پیشرفت تحصیلی دانشجویان، نمره ی علوم پایه و معدل کل آنها بود. اطلاعات با نرم افزار SPSS نسخه ی 13/5 و آزمون های آماری 2χ، آنالیز واریانس و تحلیل ضریب همبستگی پیرسون تجزیه و تحلیل شدند. سطح معنی داری (p value < 0/05) بود.یافته هاانگیزش تحصیلی با نمره ی علوم پایه (p value = 0/03) و معدل کل (p value = 0/04) ارتباط مستقیم و معنی داری داشت. بین مولفه های رقابت جویی، کوشش، قدرت اجتماعی، معدل کل و نمره ی علوم پایه همبستگی مثبت دیده شد. همچنین انگیزه ی رقابت جویی در پسران بالاتر از دختران بود.نتیجه گیرینتایج حاکی از این بود که پیشرفت تحصیلی دانشجویان وابسته به انگیزش آنها می باشد.کلیدواژگان: پیشرفت تحصیلی، انگیزش تحصیلی، علوم پایه

صفحات 9-16مقدمهافزایش انتظارات بیماران نسبت به درمان های ترمیمی- زیبایی، پژوهشگران را تشویق به مطالعه ی بیشتر در زمینه ی مواد و روش هایی که ظاهری مطلوب را برای بیماران فراهم می کند، نموده است. هدف از این مطالعه، ارزیابی اثر پخت های مکرر بر رنگ گلاس سرامیک لیتیوم دی سیلیکات با ترانسلوسنسی های متفاوت بوده است.مواد و روش هادر این مطالعه ی آزمایشگاهی، بیست نمونه از گلاس سرامیک های لیتیوم دی سیلیکات (IPS e. Max CAD) با ترانسلوسنسی زیاد و کم در دو گروه تهیه شد (10 = n). نمونه ها به شکل مستطیل در ابعاد 12×14×1 میلی متر برش داده شدند. سپس نمونه ها سه مرتبه پخت و بعد از پخت اول و سوم با استفاده از اسپکتروفتومتر، مختصات تعیین کننده ی رنگ اندازه گیری شد. بین پخت اول و سوم محاسبه شد. اطلاعات با استفاده از نرم افزار SPSSنسخه ی 16 و آزمون تی مستقل تجزیه و تحلیل شدند (α = 0/05).یافته هاتغییر رنگ ناشی از سه پخت در نمونه های لیتیوم دی سیلیکات با ترانسلوسنسی کم (0/88 ± 1/342) و ترانسلوسنسی زیاد (0/40 ± 0/757) به دست آمد. میانگین تفاوت رنگ لیتیوم دی سیلیکات با ترانسلوسنسی کم نسبت به نوع با ترانسلوسنسی زیاد، تحت تاثیر پخت های مکرر قرار گرفت (p value < 0/05).نتیجه گیریسیکل های پخت متعدد، تغییر رنگ بیشتری در لیتیوم دی سیلیکات با ترانسلوسنسی کم نسبت به نوع با ترانسلوسنسی زیاد ایجاد می کند.کلیدواژگان: رنگ، سرامیک، ترمیم دندانی، CAD-CAM

صفحات 17-25مقدمهاز مهم ترین مسائل در درمان دندان پزشکی کودکان، کنترل درد در حین درمان می باشد. تزریق بلاک عصب آلوئولار تحتانی به عنوان تزریق مرسوم در بی حسی مولرهای دائمی فک پایین دارای محدودیت هایی است. همین امر سبب می شود ضرورت یک تزریق جایگزین احساس گردد. یکی از روش های پیشنهادی، تزریق انفیلتراسیون است. هدف از این مطالعه، مقایسه ی میزان موفقیت روش انفیلتراسیون توسط آرتیکایین با بلاک عصب آلوئولار تحتانی توسط لیدوکایین در بی حسی مولرهای اول دائمی مندیبل در کودکان 8-11 ساله بود.مواد و روش هااین مطالعه ی کارآزمایی بالینی از نوع تصادفی، بر روی 42 کودک 8-11 ساله ی نیازمند ترمیم مولرهای اول دائمی فک پایین به صورت دوطرفه صورت گرفت و تزریق بلاک یا انفیلتره به صورت تصادفی انجام شد. سپس رفتار بیمار در دو مرحله ی تزریق بی حسی و حین ترمیم دندان بر اساس معیار SEM (Sounds, Eyes, and Motor) ثبت و جهت مقایسه ی SEM در دو طرف از آزمون ویلکاکسون و نرم افزار آماری SPSS نسخه ی 20 استفاده شد. همچنین (p value < 0/05) معنی دار در نظر گرفته شد.یافته هایافته های مطالعه نشان داد که در حین تزریق بی حسی انفیلتراسیون آرتیکایین، هر سه معیار SEM نسبت به تزریق بلاک با لیدوکایین، کاهش معنی داری نشان داد (p value > 0/05) و همچنین بین کارایی دو روش حین ترمیم دندان، تفاوت معنی داری مشاهده نشد (p value < 0/05).نتیجه گیریبه نظر می رسد تزریق انفیلتراسیون آرتیکایین می تواند به اندازه ی تزریق بلاک عصب آلوئولار تحتانی با لیدوکایین، در بی حسی اولین مولر دائمی فک پایین موثر باشد.کلیدواژگان: آرتیکایین، انفیلتراسیون، عصب اینفراآلوئولار، لیدوکایین

صفحات 26-36مقدمهپروتزهای متکی بر ایمپلنت، می توانند پیچ شونده یا سمان شونده باشند. استفاده از سمان های دائمی که در روکش ها به صورت متداول استفاده می شود، در پروتزهای متکی بر ایمپلنت توصیه نمی شود. این سمان ها بسیار قوی هستند و امکان جداسازی پروتز، به راحتی امکان پذیر نمی باشد. با استفاده از سمان های موقت، نه تنها شکست های سمان دائم، حذف می شوند، بلکه پروتز در موارد نیاز، قابل جداسازی می باشد. هدف از این مطالعه، بررسی تاثیر مواد پرکننده ی فضای پیچ و سمان در گیر روکش های ایمپلنت بود.مواد و روش هادر این مطالعه ی آزمایشگاهی، 5 اباتمنت زاویه دار (15 درجه) بر روی آنالوگ، پیچانده شده و 10 روکش برای هر یک از آنها ساخته شد. چهار ماده ی پرکننده، Coltosol، کامپوزیت رزین، نوار تفلون و سیلیکون، برای پر کردن حفره ی دسترسی استفاده گردید و یکی از اباتمنت ها خالی گذاشته شد (تنها پیچ آن پوشانده شد). دو سمان موقت (Temb Bond و Temp Bond NE) برای سمان روکش ها استفاده گردید. نیروی مورد نیاز برای جداسازی روکش ها با Universal testing machine با سرعت 5 میلی متر در دقیقه اندازه گیری شد. داده ها با استفاده از آزمون های آماری ANOVA و توکی مورد تجزیه و تحلیل قرار گرفتند (α = 0/05).یافته هادر تمام نمونه ها، نیروی مورد نیاز برای جدا کردن روکش های ریختگی از پایه ی ایمپلنت در گروه Temp Bond NE بالاتر و این تفاوت از نظر آماری معنی دار بود (p value = 0/007). علاوه بر این، تفاوت میان مواد پرکننده ی حفره ی دسترسی از لحاظ آماری معنی دار بود (p value < 0/001). از این رو در هر گروه شامل Clotosol، گروه فاقد ماده ی پر کننده، کامپوزیت رزین، تفلون و سیلیکون تراکمی، با روند صعودی به ترتیب بالاترین میزان نیروی جداسازی را نشان می دهند.نتیجه گیریگیر روکش های متکی بر ایمپلنت، تحت تاثیر انواع مواد پرکننده، حفره های دسترسی و سمان های مورد استفاده می باشد.کلیدواژگان: روکش، پایه ی ایمپلنت، گیر، سمان موقت

صفحات 37-45مقدمهکنترل درد در دندان پزشکی به خصوص در هنگام انجام درمان ریشه، اهمیت بسزایی دارد. هدف از این مطالعه، بررسی اثر تزریق انفیلتراسیون دگزامتازون، همراه با بلاک عصب آلوئولار تحتانی (لیدوکایین با اپی نفرین 80000/1) بر میزان درد بعد از کار، در دندان های خلفی با پالپیت غیرقابل برگشت می باشد.مواد و روش هادر این مطالعه ی کارآزمایی بالینی، 60 بیمار با پالپیت غیر قابل برگشت و درد حاد در دندان های مولر فک پایین، به دو گروه تقسیم شدند. پس از تزریق بلاک، برای هر بیمار، تزریق انفیلتراسیون باکالی دگزامتازون یا نرمال سالین انجام شد. قبل از شروع درمان و در فواصل زمانی 4، 6، 12 و 24 ساعت پس از درمان، شدت درد هر بیمار توسط Visual analogue scale ثبت گردید. تجزیه و تحلیل داده ها با آزمون های آماری تی مستقل، کای اسکوئر، من ویتنی و فریدمن انجام شد (α = 0/05).یافته هابین دو گروه از نظر سن، جنس و درد قبل از درمان، تفاوت معنی داری وجود نداشت. در گروه شاهد، درد 12 و 24 ساعت پس از درمان به طور معنی داری کمتر از درد 4 ساعت پس از درمان بود، اما در گروه دگزامتازون، این تفاوت معنی دار نبود. با کنترل اثر فاکتورهای مداخله گر، درد قبل از درمان به طور معنی داری بر درد پس از درمان (در 4 و 6 ساعت) اثر داشت و جنسیت بر درد بعد از درمان در 12 ساعت موثر بود.نتیجه گیریتزریق موضعی دگزامتازون، بر درد 4 و 6 ساعت بعد از درمان اثر قابل توجهی داشت. درد قبل از درمان بر درد پس از درمان ریشه ی دندان های مولر مندیبل با پالپیت برگشت ناپذیر در 4 و 6 ساعت اثر معنی داری داشت.کلیدواژگان: درمان ریشه، درد، دگزامتازون

صفحات 46-53مقدمهیکی از نکات مهم در آکریل های جدید، تعیین میزان استحکام خمشی و ثبات رنگ دنچرهای تهیه شده با این مواد است. هدف کلی از این مطالعه، بررسی و مقایسه ی استحکام خمشی و ثبات رنگ دو نوع آکریل تزریقی ایووبیس و ورتکس کاستاواریا با آکریل گرماسخت ملیودنت بود.مواد و روش هاددر این مطالعه ی تجربی- آزمایشگاهی، در مجموع 30 نمونه ی مکعب مستطیل شکل از هر سه نوع آکریل، با ضخامت 3 میلی متر و با ابعاد 15×67 میلی متر تهیه شد. استحکام خمشی نمونه های هر گروه، توسط آزمون خمش سه نقطه ای و تغییرات رنگ نمونه ها در اثر 30 روز غوطه وری در چای، با دستگاه اسپکتروفتومتر مشخص شد. داده های به دست آمده، به نرم افزار SPSS نسخه ی 22 منتقل و توسط آنالیزهای آماری One way ANOVA و آزمون توکی با سطح معنی داری 0/05 تجزیه و تحلیل شد.یافته هامیانگین استحکام خمشی نمونه های آیووبیس، برابر با 4/2 ± 58/16، نمونه های ورتکس برابر با 7/2 ± 66/01 و نمونه های ملیودنت برابر با 9/7 ± 77/38 مگاپاسکال بود. در بررسی ثبات رنگ، پس از غوطه وری E∆ نمونه های آیووبیس برابر با 1/8 ± 6/56، نمونه های ورتکس برابر با 1/3 ± 3/59 و نمونه های ملیودنت برابر با 2/1 ± 6/23 بود.نتیجه گیریآکریل ملیودنت گرماسخت، با استفاده از روش کانونشنال استحکام خمشی بهتر و آکریل ورتکس کاستاواریا نسبت به سایر مواد، رنگ پذیری کمتری از خود نشان دادند.کلیدواژگان: استحکام خمشی، غوطه وری، رزین های آکریلی، پایه ی دست دندان، رنگ

صفحات 54-65مقدمهایمپلنت های دندانی، یک روش موفق در جایگزینی دندان های از دست رفته می باشد. دندان پزشکان عمومی علاقه مند به درمان ایمپلنت، باید از دانش لازم در این زمینه برخوردار باشند تا بتوانند در راستای ارتقای سلامت جامعه، خدمات دندان پزشکی پیشرفته ای را ارایه دهند. مطالعه ی حاضر با هدف بررسی آگاهی دندان پزشکان عمومی شهر اصفهان در مواجهه با دشواری های حین جراحی و عوارض پس از جراحی ایمپلنت دندانی در سال 1395 در شهر اصفهان انجام شد.مواد و روش هاپژوهش حاضر یک مطالعه ی توصیفی بود، تعداد 100 نفر از دندان پزشکان عمومی شهر اصفهان انتخاب شدند و پرسش نامه ها توسط آن ها تکمیل گردید. نتایج این پرسش نامه ها با آزمون های تی مستقل و ضریب همبستگی پیرسون در نرم افزار SPSS نسخه ی 22 در سطح معنی داری کمتر از 5 درصد محاسبه گردید.یافته هاسطح آگاهی اکثر دندان پزشکان مورد بررسی (78 درصد) کم یا متوسط بود. نمره ی آگاهی دندان پزشکان با سن (66/0 = p value) و سابقه ی طبابت (45/0 = p value) آن ها رابطه ی معنی دار نداشت. میانگین نمره ی آگاهی دندان پزشکانی که سابقه ی انجام درمان ایمپلنت داشتند، بطور معنی داری بیشتر از سایرین بود (p value < 0/001).نتیجه گیریمیزان آگاهی دندان پزشکان عمومی در مواجهه با دشواری ها و عوارض حین و پس از جراحی ایمپلنت دندانی کافی نیست. همچنین میزان آموزش تئوری و مخصوصا عملی در این زمینه، محدود بوده و دندان پزشکان عمومی به خوبی مطالب تئوری و عملی این مبحث را در طی دوران تحصیل خود آموزش ندیده اند.کلیدواژگان: آگاهی، عوارض، ایمپلنت دندانی

صفحات 66-74مقدمهکنترل عفونت در دندان پزشکی، همواره از مباحث کلیدی بوده است. دندان پزشکان با میکروارگانیسم های گوناگونی در تماس می باشند که منابع آنها، خون و بزاق هستند. در پروتز قالب های آلوده ، با کارکنان لابراتور و مطب در تماس می باشند. بنابراین ضدعفونی کردن مناسب قالب ها، امری بسیار مهم است. این مطالعه به منظور بررسی تاثیر ضدعفونی کنندگی سه ماده با ترکیب مختلف بر روی دیسک های آلژیناتی به روش غوطه وری انجام شد.مواد و روشدر این مطالعه، قدرت ضدعفونی کنندگی سه ماده ی هیپوکلریت سدیم 0/5 درصد، دکونکس (Deconex) و سورفوسپت (Surfosept) به روش غوطه وری بر روی 240 دیسک آلژیناتی که به 3 میکروارگانیسم آلوده شده بودند، مورد بررسی قرار گرفت. برای هر نوع میکروارگانیسم، تعداد 80 دیسک آلژیناتی تهیه گردید. دیسک ها داخل ماده ی ضدعفونی کننده قرار داده شدند. سپس از آنها کشت تهیه شد و پس از 24 ساعت، کلونی های مورد نظر شمارش گردید. داده ها با آزمون های آماری کای اسکوئر و رگرسیون توسط نرم افزار SPSS نسخه ی 20 تجزیه و تحلیل شدند و سطح معنی داری p value < 0/05 در نظر گرفته شد.یافته هانتایج نشان داد که قدرت ضدعفونی کنندگی سه ماده ی دکونکس، سورفوسپت و هیپوکلریت 5/0 درصد، از لحاظ آماری معنی دار بود (p value < 0/0001). سورفوسپت، بیش ترین تاثیر را بر استافیلوکوکوس آرئوس (میانگین کلونی 3/28 ± 20/05) و دکونکس، بیش ترین تاثیر را بر سودوموناس آئروژینوزا (میانگین کلونی 4/27 ± 31/15) و کاندیدا آلبیکانس (میانگین کلونی ها 2/37 ± 7/55) نشان داد.نتیجه گیریبا توجه به یافته های پژوهش، هیپوکلریت سدیم 0/5 درصد، در هر سه مورد، کم ترین تاثیر را داشت. در مورد کاندیدا آلبیکانس و سودوموناس آئروژینوزا، بیش ترین تاثیر در مجاورت دکونکس و در مورد استافیلوکوکوس آرئوس در مجاورت با سورفوسپت مشاهده شد.کلیدواژگان: آلژینات، ضدعفونی کننده، غوطه وری

صفحات 75-82مقدمهزندگی تحصیلی، از مهم ترین ابعاد زندگی افراد است که تاثیر بسزایی بر ابعاد دیگر زندگی دارد. هوش هیجانی، از موثرترین عوامل کسب موفقیت در عملکرد تحصیلی افراد است. هدف از این مطالعه، تعیین ارتباط هوش هیجانی با عملکرد تحصیلی دانشجویان دکترای عمومی دندان پزشکی اصفهان در سال تحصیلی 1393-1394 بود.مواد و روش هادر این مطالعه ی توصیفی، از پرسش نامه ی استاندارد هوش هیجانی بار-آن (Bar On Emotional Intelligence) استفاده شد که شامل مشخصات دموگرافیک و سوالات هوش هیجانی با 5 مولفه (درون فردی، بین فردی، سازگاری، مدیریت استرس و خلق عمومی) بود. پرسش نامه بصورت تصادفی در اختیار 156 نفر دانشجوی دانشکده ی دندان پزشکی اصفهان قرارداده شد. معدل دانشجویان به عنوان شاخص وضعیت تحصیلی در نظر گرفته و نمره گذاری بر اساس طیف لیکرت از 1 تا 5 تعیین شد. داده ها توسط نرم افزار SPSS نسخه ی 18 و آنالیزهای آماری توصیفی، ضریب همبستگی پیرسون و تحلیل رگرسیون مورد تجزیه و تحلیل قرار گرفت (0/05 = α).یافته هانمره ی کل هوش هیجانی زنان (71) 223 و مردان (43) 218/95 بود. نمره ی کل هوش هیجانی افراد مجرد (91) 223/31 بیشتر از متاهلین (23) 218/13 بود. از نظر نوع سکونت، بیش ترین میانگین مربوط به افرادی بود که خانه ی شخصی داشتند (63) 224/12 و کم ترین مربوط به خانه ی استیجاری بود (15) 209/73. عزت نفس، تحمل فشار روانی، شادمانی، همدلی و انعطاف پذیری با عملکرد تحصیلی، ارتباط معنی داری داشت
(0/001< p value). نتایج رگرسیون نشان داد، خرده مقیاس خودآگاهی هیجانی (0/014 = p value)، عزت نفس (0/014 = p value) و شادمانی (0/024 = p value) در حضور سایر شاخص ها و ویژگی های دموگرافیک با عملکرد تحصیلی، ارتباط معنی داری داشتند (0/770 = r).نتیجه گیرینمره ی کل هوش هیجانی دختران، بیشتر از پسران بود. بین عملکرد تحصیلی دانشجویان و نوع سکونت شخصی و زندگی با خانواده، تفاوت معنی داری وجود داشت. بین خرده مقیاس های خودآگاهی، عزت نفس، شادمانی و عملکرد تحصیلی، رابطه ی معنی داری وجود داشت.کلیدواژگان: هوش هیجانی، عملکرد تحصیلی، دانشجویان -
صفحات 83-91مقدمهآمالگام، از رایج ترین مواد پر کننده ی دندان به شمار می رود که همچنان استفاده ی کلینیکی از آن مورد بحث محققین می باشد. این آلیاژ به واسطه ی محتوی جیوه و خطرات ناشی از آن و مقاومت به خوردگی محدود در محیط بدن، مورد انتقاد پژوهشگران قرار دارد. لذا در مطالعه ی حاضر، کامپوزیت های آمالگام/نانوهیدروکسی آپاتیت و آمالگام/نانوبایوگلاس برای کاهش محدودیت های فعلی آمالگام، طراحی و ساخته شد و تاثیر نوع تقویت کننده بر خواص مکانیکی و خوردگی آمالگام دندانی بررسی گردید.مواد و روش هادر این مطالعه ی آزمایشگاهی و تحلیلی، کامپوزیت های آمالگام/نانو هیدروکسی آپاتیت و آمالگام/نانوبایوگلاس تهیه شد؛ به این روش که 2 درصد وزنی نانوبیوسرامیک، قبل از اضافه کردن جیوه، به مخلوط پودر فلزات تشکیل دهنده ی آمالگام افزوده شد و سپس در آمالگاماتور با جیوه مخلوط شدند و سپس در قالب فولادی توسط سنبه فشرده شدند. سپس نمونه های سخت شده، توسط میکروسکوپ الکترونی روبشی (SEM) مورد ارزیابی ریزساختاری قرار گرفتند. همچنین توسط دستگاه آزمون فشار یونیورسال، خواص مکانیکی کامپوزیت ها مورد ارزیابی قرار گرفتند. مقاومت به خوردگی کامپوزیت ها به روش پولاریزاسیون پتانسیودینامیک، مورد مطالعه قرار گرفت. برای مقایسه ی دقیق داده های کمی، از آنالیز واریانس یک طرفه با استفاده از نرم افزار GraphPad Prism 6 استفاده شد. اختلاف آماری، معنی دار اعلام گردید (p value < 0/05).یافته هادر حالی که با افزودن هر دو نانوبیوسرامیک هیدروکسی آپاتیت و بایوگلاس، مقاومت به خوردگی کامپوزیت افزایش می یابد، تاثیر هیدروکسی آپاتیت در افزایش مقاومت به خوردگی آمالگام در محلول نمکی، بیشتر بوده است. همچنین افزایش دو درصدی ذرات تقویت کننده ی هیدروکسی آپاتیت و بایوگلاس، سبب بهبود خواص مکانیکی کامپوزیت به ترتیب به مقدار 15 و 12 درصد شده است.نتیجه گیریبه نظر می رسد، کامپوزیت های پایه ی آمالگام توسعه داده شده، گزینه ی مناسبی برای اهداف پر کردن دندان های خلفی با اثرات سوء کمتر (خوردگی کمتر و استحکام بالاتر) باشد و نانو ذرات هیدروکسی آپاتیت، موثرتر از بایوگلاس عمل کرده اند.کلیدواژگان: هیدروکسی آپاتیت، بایوگلاس، آمالگام، خواص مکانیکی

صفحات 92-103زمینه و هدفهدف از این مطالعه، تعیین فراوانی انواع خطاهای درمانی در بیماران دارای شکایات بدون منشا فیزیکی (بدون توضیح فیزیکی قانع کننده) مراجعه کننده به کلینیک دردهای دهان، فک و صورت دانشکده ی دندان پزشکی مشهد از سال 1392 تا 1396 بود.مواد و روش هااین مطالعه، توصیفی- تحلیلی از نوع مقطعی بود. پرونده های بیماران مراجعه کننده به کلینیک درد دهان، فک و صورت از سال 1392 تا 1396 بررسی شدند. برای بیماران، چک لیستی شامل انواع شکایات بیماران، درمان های انجام شده، روش های تشخیصی و الگوی ارجاع تکمیل شد. این بیماران توسط متخصصین بیماری های دهان معاینه شده بودند و شکایات آنها که توضیح فیزیکی قانع کننده ای نداشت بر مبنای گرفتن شرح حال، معاینه ی بالینی و آزمایشات پاراکلینیکی بررسی و بر این اساس، نوع اختلال حسی تعیین گردیده بود. سپس اختلال روان پزشکی آنها توسط روان پزشک مورد بررسی قرار گرفته بود. فراوانی انواع خطاهای درمانی، نوع اختلال روان پزشکی و اختلال حسی، تعیین و ارتباط آنها با یکدیگر از لحاظ آماری بررسی گردید. داده ها با استفاده از نرم افزار SPSS نسخه ی 11/5 با سطح معنی داری 0/05 تجزیه و تحلیل شدند. تحلیل داده ها به کمک جداول فراوانی، نمودار، شاخص های میانگین و انحراف معیار با استفاده از آمار توصیفی انجام شد. همچنین جهت مقایسه ی بین زیر گروه ها از آزمون های 2χ، آنوا، تی تست و در صورت عدم وجود شرایط لازم از آزمون های ناپارامتری من ویتنی و کروسکال والیس استفاده شد.یافته هابیبیش ترین فراوانی شکایات در زمان مراجعه، مربوط به درد صورت، فک و یا گردن (74/8 درصد) و کم ترین فراوانی شکایات، احساس درد گنگ در کل دهان بود. دارودرمانی (49 درصد) و پس از آن کشیدن دندان (14 درصد)، شایع ترین خطاهای درمانی انجام شده بودند. بیش ترین خطا در روش های تشخیصی تجویز شده، رادیوگرافی (65/2 درصد) و پس از آن سی تی اسکن (24/1 درصد) و کم ترین خطا مربوط به سونوگرافی (2/3 درصد) گزارش شد.نتیجه گیریبر اساس این مطالعه، چنانچه بیماری با شکایت بدون توضیح فیزیکی به کلینیک دندان پزشکی مراجعه کند، احتمال بروز خطای تشخیصی و درمانی زیاد است. بنابراین توصیه می شود برنامه های آموزشی اختصاصی برای دندان پزشکان جهت شناسایی شکایات با منشا ذهنی و تشخیص صحیح آنها و یا ارجاع صحیح این بیماران وجود داشته باشد.کلیدواژگان: خطاهای پزشکی، اختلالات روانی، اختلالات حسی

صفحات 104-112مقدمهدستگاه فانکشنال فرانکل، یک وسیله ی با ارزش برای درمان مال اکلوژن کلاسII می باشد که بیش ترین اثرات اسکلتال را به دنبال دارد. هدف از انجام این مطالعه، بررسی تغییرات اسکلتی و دندانی در بیماران با مال اکلوژن کلاسII به دنبال درمان با دستگاه فرانکل بود.مواد و روش هاااین مطالعه ی کارآزمایی بالینی، بر روی 8 بیمار با محدوده ی سنی 12-9 سال، دارای مال اکلوژن کلاس II دندانی و اسکلتی، صورت گرفت. رادیوگرافی های سفالومتری لترال در ابتدا و انتهای درمان گرفته شدند. سفالوگرام ها ترسیم شدند و اندازه گیری های لازم انجام شد. تجزیه و تحلیل داده ها با استفاده از آزمون t زوجی صورت گرفت (α < 0/05).یافته هااز بین متغیرهای ایجاد شده در قاعده ی جمجمه، بین دو گروه شاهد و درمان شده با فرانکل، طول S-N افزایش معنی دار داشت (p value = 0/006). در مجموعه ی ماگزیلا بین دو گروه، کاهش زاویه ی SNA (p value = 0/04) و افزایش طول ماگزیلا (p value = 0/01) معنی دار بود. از بین متغیرهای ایجاد شده در فک پایین، تغییرات زاویه ی گونیال (p value = 0/03)، زاویه ی SNB (p value = 0/001) و SNBpog (p value = 0/0005)، طول راموس (p value = 0/01)، قاعده ی مندیبل (p value = 0/007) و طول مندیبل (p value = 0/01) تفاوت معنی دار داشتند. از بین متغیرهای ایجاد شده ی فک بالا و پایین نسبت به هم، زاویه ی ANB (0005/0 = p value)، از بین متغیرهای ایجاد شده ارتفاع و روابط صورتی، S-GO (p value = 0/01) و N-Me (p value = 0/001) و از بین متغیرهای ایجاد شده ی روابط دندانی، میانگین تغییرات بین دو گروه تفاوت آماری معنی دار وجود داشت (p value = 0/01).نتیجه گیریااثرات اسکلتی فرانکل شامل ممانعت از رشد ماگزیلا به سمت جلو و جابه جایی مندیبل به سمت قدام و در نتیجه رفع نقص مندیبل و اصلاح ناهماهنگی فکین بود و اثرات دندانی فرانکل حداقل و در جهت کاهش اورجت و اصلاح دندانی بیماران دارای مال اکلوژن کلاس II بوده است.کلیدواژگان: مال اکلوژن کلاس II، دستگاه فرانکل، درمان اصلاح رشد

صفحات 113-119مقدمهآملوبلاستیک کارسینوما، یکی از ضایعات بدخیم نادر با ویژگیهای هیستوپاتولوژیک کاراکتریستیک می باشد، در حالی که گاهی برخی از این ویژگی ها مبهم است. سلول های تومورال، تظاهری مشابه آملوبلاستوما دارند، اما آتیپی سیتولوژی را نشان می دهند. در نمای رادیوگرافی مشابه آمیلوبلاستومای تیپیک (متداول) رفتار می کند. ممکن است به صورت یک ضایعه ی کیستیک با ویژگی های کلینیکی خوش خیم تظاهر یابد یا به صورت یک افزایش حجم و زخم و تخریب استخوان مشخص و از دست دادن دندان ها مشخص شود. ضایعه اغلب به دنبال یک بیوپسی انسیژنال تشخیص داده می شود. در حالی که به خاطر رفتار مهاجم ضایعه (گسترش به بافت نرم اطراف، تخریب استخوانی شدید و درگیری لنفنودها و متاستاز) درمان انتخابی اکسیژن وسیع می باشد. برداشت لنفنودهای ناحیه، رادیوتراپی و شیمی درمانی محدود به ارزش استفاده ی از آن ها در درمان است و ارزیابی دوره ای بیمار الزامی است.شرح مورددر این مقاله، یک مورد آملوبلاستیک کارسینوما در فک بالا در یک مرد 46 ساله همراه با ویژگی های رادیوگرافیک، کلینیکی و پاتولوژیک گزارش شده است. نمای کلینیکی و رادیوگرافیک ضایعه از آملوبلاستوما تقلید می کرد، در حالی که نمای هیستولوژیک آتیپی سیتولوژیک مرتبط با آملوبلاستیک کارسینوما را آشکار ساخت. انجام رادیوتراپی قبل از جراحی رادیکال در طرح درمان بیمار قرار گرفت.کلیدواژگان: آملوبلاستیک کارسینوما، آملوبلاستوما، ماگزیلا، گزارش مورد

Pages 1-8IntroductionDespite their ascribed intellectual ability and significant academic achievements, dental students’ academic performance is influenced by their academic motivation. This study examined the role of academic motivation in academic achievements of dental students in Kerman University of Medical Sciences.Materials and MethodsIn this cross-sectional study, 172 dental students, from the 3rd to the final year, at School of Dentistry, Kerman University of Medical Sciences, completed the Inventory of School Motivation (ISM), comprising of 37 items and measuring 8 aspects of motivation. The gold standard for academic achievement was their overall average grades and their scores on basic sciences examination. Data were analyzed with SPSS 13.5, using ANOVA, Pearson’s correlation coefficient and chi-squared test at a significance level of p value < 0.05.ResultsAcademic motivation had a significant and direct relationship with scores on basic sciences examination (p value = 0.03) and the overall mean grades (p value = 0.04). Areas of competition, effort and social responsibility had a positive correlation with higher average grades and scores on basic sciences examination. Competition motivation in male students was higher than that in female student.ConclusionThe results showed that students' academic achievement depends on their motivation.Keywords: Academic achievement, Academic motivation, Basic sciences

The purpose of this study was to evaluate theeffect of repeated firing on color changes of lithium disilicate (with high and low translucency).Materials & MethodsIn this in-vitro study, 20 samples of high and low lithium disilicate were prepared forboth groups (n = 10). Samples were sectioned and polished to provide a thickness of 1mm. The specimens were subjected to firing for three times. The color coordinationafter the first and second firing were measured using spectrophotometer. Delta E was calculated between first and second firing.the data were analyzed by SPSS (version 16) stastical software (α = 0.05).ResultsThe mean and standard deviation of low translucency was 1.342 ± 0.88 and high transcendental was 0.757 ± 0.49.Pair- wise comparison of color change prepared by three times firing in lithium dicliate samples caused higher ΔE of LDS with low translucent significantly (p value = 0.019).ConclusionMultiple firing casued significantly higher color changes in lithium disilicate with low translucency than that of with high translucent.Keywords: Color, Ceramics, Dental restoration, CAD-CAM

Pages 17-25IntroductionOne of the most important issues in pediatric dentistry is pain control during treatment. The inferior alveolar nerve block has some limitations as a conventional technique for the anesthesia of mandibular permanent molars, necessitating the use of alternative techniques, one of which is the infiltration technique. The aim of this study was to compare the efficacy of inferior alveolar dental nerve block using lidocaine with infiltration technique with articaine in mandibular first permanent molar anesthesia in children 8−11 year of age.Materials & MethodsIn this randomized cross-over clinical trial on 42 children 8−11 year of age both mandibular first permanent molars were restored. Block or infiltration injection was selected randomly. Patient's behavior was registered in two steps of injection and restoration using the SEM scores. For comparison of the two sides, Wilcoxon signed rank test and SPSS 20 were used (α = 0.05).ResultsBased on the results, the infiltration technique resulted in a decrease in all the three SEM scores compared to block injection (p value < 0.05). There was no significant difference in the efficacy of these two anesthetic techniques (p value > 0.05).ConclusionIt appears that infiltration technique with articaine is a better alternative for block technique with lidocaine in the restoration of the mandibular first permanent molars.Keywords: Articaine, Inferior alveolar nerve, Infiltration, Lidocaine

Pages 26-36IntroductionImplant-supported prostheses can be screw-type or can be cemented onto the abutment. Use of permanent cements, which are used in conventional prostheses, is not recommended for implant-supported prostheses. These cements are very strong and do not allow easy removal of the prosthesis. By using temporary cements not only permanent cement faults will be eliminated but also prosthesis can be retrieved as needed. The aim of this study was to evaluate the effect of screw access filling material and cement on the retention of implant cemented crowns.Materials & MethodsIn this in vitro study, 5 angulated abutments (15°) were screwed onto the analogue and ten crowns were fabricated for each of them. Four filling materials, Coltosol, composite resin, Teflon tape and silicone, were used to fill the access cavity and one abutment was left empty (only the screw was covered). Two temporary cements (Temp Bond and Temp Bond NE) were used to cement the crowns. The force required to remove the crowns was measured with a universal testing machine at a crosshead speed of 5 mm/min. Data were analyzed with ANOVA and Tukey test (α = 0.05).ResultsIn all the samples, the force required to separate the casting crowns from the abutment were higher in Temp Bond NE group and this difference was statistically significant (p value = 0.007). In addition, the difference between the access filling material was statistically significant (p value 0.001). Therefore in each group, Coltosol, empty, composite resin, Teflon and the condensational silicone exhibited the highest means of separating force in ascending order.ConclusionRetention of implant-supported crowns was influenced by types of filling material, access cavity and cement used.Keywords: Crown, Implant abutment, Retention, Temporary Cement

Pages 37-45IntroductionPain control in dental treatment, especially in root canal treatment, is an important issue. The aim of this study was to investigate the effect of dexamethasone infiltration along with the inferior alveolar nerve block with lidocaine with 1:80000 epinephrine on postoperative pain in posterior teeth with irreversible pulpitis.Materials & MethodsIn this clinical trial, 60 patients with irreversible pulpitis and acute pain in lower molar teeth were selected and randomly divided into two groups. After standard block injection for each patient, buccal infiltration injections of dexamethasone or normal saline solution was performed. Preoperative pain was recorded using visual analogue scale (VAS) from 0 to 10 before treatment, and at 4-, 6-, 12- and 24-hour postoperative intervals. Data were analyzed with independent t-test and chi-squared, Mann-Whitney and Friedman tests (α = 0.05).ResultsThere were no significant differences between the two groups in age, sex, tooth and pain severity before treatment. Pain in the control group 12 and 24 hours after treatment was significantly less severe than that at 4-hour interval; however, in the dexamethasone group this difference was not significant. By controlling confounding factors, preoperative pain significantly affected the pain after treatment at 4 and 6 hours. Gender affected pain at 12-hour postoperative interval.ConclusionBuccal infiltration of dexamethasone significantly affected pain at 4- and 6-hour postoperative intervals. Preoperative pain significantly affected pain at 4- and 6-hour postoperative intervals in mandibular molars with irreversible pulpitis.Keywords: Dexamethasone, Pain, Root canal therapy

Pages 46-53IntroductionAn important consideration in newly introduced acrylic resins is to determine the flexural strength and color stability of dentures fabricated using these materials. The general aim of this study was to evaluate and compare the flexural strength and color stability of two injectable acrylic resins (IvoBase High-Impact and Vertex Castavaria) and a heat-cured acrylic resin (Meliodent).Materials & MethodsIn this study, 30 cuboid samples were fabricated (10 specimens from each group), measuring 3 mm in thickness and 67×15 mm in dimension. The flexural strengths of the samples were determined using 3-point flexural strength test and their color changes were evaluated by immersion in tea using spectrophotometry technique. Data were analyzed with SPSS 22, using one-way ANOVA and HSD Tukey tests (α = 0.05).ResultsThe mean flexural strength values of the IvoBase, Vertex Castavaria and Meliodent samples were 58.16, 66.01 and 77.38 MPa, respectively. In relation to color changes, the ΔE values of IvoBase, Vertex Castavaria and Meliodent samples were 6.56, 3.59 and 6.23, respectively.ConclusionMeliodent heat-cured acrylic resin which uses the conventional system showed the best flexural strength, and Vertex Castavaria showed the best color stability.Keywords: Acrylic resins, Color, Denture bases, Flexural strength, Immersion

General dental practitioners interested in implants should have the necessary knowledge in this field to improve public health in order to provide their advanced dental services. This study aimed to assess the knowledge of dentists in Isfahan about problems and complications during and after implant surgery in 2017 in Isfahan.Materials & MethodsA total of 100 general dentists of Isfahan were selected for the purpose of this cross-sectional study. Questionnaires were completed by the dentists. Data collected from the questionnaires were analyzed with independent t-test and Pearson’s correlation coefficient, using SPSS 22 (α = 0.05).ResultsMost dentists (78%) had low or average level of awareness. Age (p value = 0.66) and experience of practice (p value = 0.45) had no significant relationship with knowledge score. Mean knowledge scores of dentists who had implant therapy experience were significantly higher than the others (p value < 0.001).ConclusionThe level of knowledge of general dentist was not adequate about the problems and complications encountered during and after implant surgery. In addition, the level of theoretical and practical education in particular in this area was limited and general dentists were not properly instructed in implant dentistry during their years of training.Keywords: Complications, Dental Implants, Knowledge

Pages 66-74IntroductionInfection control has always been one of the most important issues in dentistry. Dentists are exposed to harmful microorganisms originating from the blood and saliva. In prosthodontics contaminated impression materials are in contact with the laboratory personnel. Therefore it is absolutely necessary to establish a method for disinfecting these materials. The aim of this study was to survey the effect of three different disinfectants on alginate disks using the immersion method.Materials & MethodsThe disinfecting efficacy of 0.5% sodium hypochlorite, Deconex and Surfosept was evaluated in vitro by immersion method using 240 alginate disks contaminated with 3 microorganisms. Eighty alginate disks were prepared for each microorganism. The disks were immersed in the disinfecting solution. Cultures were prepared and colony counts were determined after 24 hours. Data were analyzed with chi-squared test and logistic regression using SPSS 20 (α = 0.05).ResultsThe antimicrobial activities of 0.5% sodium hypochlorite solution, Deconex and Surfosept were significant (p value < 0.001). Surfosept exhibited the greatest effect on Staphylococcus aureus (mean colony count of 20.05 ± 3.28) while Deconex exhibited the greatest effect on Pseudomonas aeruginosa (mean colony count of 31.15 ± 4.27) and Candida albicans (mean colony count of 7.55 ± 2.37).ConclusionThe results of this study showed that 0.5% sodium hypochlorite solution in all the three cases was the least effective. In the case of Candida albicans and Pseudomonas aeruginosa, the greatest effects were recorded with Deconex and for Staphylococcus aureus with Surfosept.Keywords: Alginate, Disinfection, Immersion

Emotional intelligence is one of the most effective factors in achieving success in different aspects of life. The present study was designed to investigate the relationship between educational status and emotional intelligence in dental students of Isfahan University of Medical Sciences.Materials & MethodsBar-On questionnaire was used to evaluate emotional intelligence. This questionnaire has 2 parts: Demographic characteristics and emotional intelligence in 5 components (inter-personal, intra-personal, compatibility, stress management, general mood). The questionnaire was completed by 156 undergraduate students in Isfahan School of Dentistry. Student’s overall average grades were used as an index for their educational status. Likert’s 1–5 scale was used for scoring. Data were analyzed with SPSS 18, using descriptive statistics, Pearson correlation coefficient and regression analysis (α = 0.05).ResultsThe total scores of emotional intelligende were 223 and 218.95 in female (71) and male (43) subjects, respectively. The total scores of single (91) and married (23) subjects were 223.31 and 218.13, respectively. In relation to accomodation, the highest mean, 224.12, belonged to the subjects who lived in their own private homes (63) and the lowest mean, 209.73, belonged to those living in rented houses. Self-esteem, mental pressure tolerance, happiness, empathy, and adaplabity were significantly associated with academic performance (p value < 0.000). Regression analysis indicated that emotional self-awareness subscale (p value = 0.014), self-esteem (p value = 0.04), and happiness (p value = 0.024) had a significant association with academic performance in the presence of other criteria and demographic features (r = 0.770).ConclusionThe total score of emotional intelligence was highr in females compared to males. There was a significant relationship between students’ academic performance and type of living (living alone or with family). In addition, there was a significant relationship between academic performance and self-awareness, self-steem and happiness subscales.Keywords: Academic performance, Emotional intelligence, Students

Pages 83-91IntroductionAmalgam is one of the most commonly used dental restorative materials, whose clinical use is still a matter of controversy among researchers. The alloy is criticized due to its mercury content and its dangers and limited resistance to corrosion in the body. In the present study amalgam/nano-hydroxyapatite and amalgam/nano-bioglass composites were produced to eliminate the currently existing limitations and evaluate the effect of the type of reinforcing agent on the mechanical properties and corrosion resistance of amalgam.Materials & MethodsIn this analytical in vitro study, amalgam/nano-hydroxyapatite and amalgam/nano-bioglass composites with 2 wt% of nano-bioceramic were produced. Nano-bioceramic was added to the amalgam powder before adding mercury, followed by mixing with mercury in an amalgamator and compression in a steel mold. The microstructure of hardened samples was evaluated by scanning electron microscopy (SEM). In addition, the mechanical properties of the composites were evaluated using a universal compression test device. Resistance to corrosion of composites was studied by potentiodynamic polarization method. To compare the quantitative data, one-way ANOVA was performed using Graph Pad Prism 6 software (α = 0.05).ResultsThe results showed that by increasing both bioceramics of hydroxyapatite and bioglass, corrosion resistance of composites increased, with hydroxyapatite exerting a greater impact on corrosion resistance of amalgam in the saline solution. In addition, incorporation of 2 wt% of bioglass and hydroxyapatite resulted in 12 and 15 percentages of improvement in the mechanical properties of the composites.ConclusionThe developed amalgam basic composites seemed to be good choices for restoration of posterior teeth, with less harmful effects (lower corrosion and higher strength), and hydroxyapatite nanoparticles were more effective than bioglass.Keywords: Amalgam, Bioglass, Hydroxyapatite, Mechanical properties

Pages 92-103IntroductionThe aim of this study was to evaluate the frequency of different types of medical errors in patients having complaints with no physical explanation, referred to Mashhad Dental Faculty Orofacial Pain Clinic.Materials & MethodsIn this cross-sectional/descriptive-analytical study, records of patients referred to the Orofacial Pain Clinic of Mashhad Dental School from 2013 to 2017 were evaluated. A checklist consisting of the following was completed: type of complaint at the visit, method of treatment, diagnostic methods and referral pattern. All the patients had been examined by oral medicine specialists. Their complaints that could not be physically explained based on the history, clinical examinations and paraclinical tests had been evaluated, and the type of sensory disorder had been determined. Furthermore, all the patients had been interviewed by a psychiatrist to determine any psychiatric disorder. The frequency of different types of medical errors, diagnostic methods, psychiatric disorder and sensory impairment were determined and the possible connection was statistically analyzed. Data were analyzed with SPSS (α = 0.05), using frequency tables, graphs and means and standard deviations. ANOVA, t-test and chi-squared test were used to compare the subgroups. In addition, if necessary, non-parametric tests of Mann-Whitney and Kruskal-Wallis were used.ResultsThe highest frequency of complaints at the time of visit was facial, mandibular or neck pain (74.8%), and the lowest frequency was the complaint about a dull pain in the whole mouth. The most common treatment error was medication errors (49%), followed by tooth extraction. The most common error in diagnostic methods was radiography (65.2%), followed by CT scan (24.1%), and the lowest error occurred in sonography (2.3%).ConclusionBased on the findings of this study, when a patient with a complaint without any physical explanation is referred to the dental clinic, there is a high probability of diagnostic and therapeutic medical errors. Therefore, it is recommended that specific training programs be organized for dentists to identify complaints with a mental origin and their proper diagnosis or to consider referral of these patients.Keywords: Medical errors, Mental disorders, Sensation disorders

Pages 104-112IntroductionFrankel functional appliance is considered to be a very valuable device in the treatment of Cl II malocclusion, with the greatest skeletal effects. The aim of this study was to evaluate the dental and skeletal changes after treatment with Frankel appliance in patients with Class II malocclusion.Materials & MethodsThis clinical trial was carried out on 8 patients, 9–12 years of age, with Class II skeletal and dental malocclusion. Lateral cephalometric radiographs were prepared before and after treatment. Tracings were carried out on cephalograms and the necessary measurements were made. Data were analyzed with t-test (α = 0.05).ResultsOf all the variables on the base of the skull, comparison between the group treated with Frankel appliance and control group showed that SN height increased significantly (p value = 0.006). Comparison between the two groups showed that in the maxillary complex, the decrease in SNA angle (p value = 0.04) and the increase in maxillary height (p value = 0.01) were significantly different between the two groups. In the mandible, gonial angle (p value = 0.03), SNB angle (p value = 0.001), SNBpog (p value = 0.0005), height of the ramus (p value = 0.01), base of the mandible (p value = 0.007) and length of the mandible (p value = 0.01) were significantly different between the two groups. Of all the variables created in the upper and lower jaws, the ANB angle (p value = 0.0005) was significantly different between the two groups. Of all the variables, height and relationships of facial variables, S-GO (p value = 0.01) and N-Me (p value = 0.001) were significantly different; and of all the dental relationship variables the mean changes were significantly different between the two groups (p value = 0.01).ConclusionFrankel skeletal effects included prevention of maxillary forward growth and mandibular anterior movement, eliminating the mandibular deficiency and correction of the discrepancy between the two jaws. Pages 113-119IntroductionAmeloblastic carcinoma is a rare malignant lesion with characteristic histopathologic features; however, sometimes these features are vague. Tumoral cells have an appearance similar to ameloblastoma but they exhibit cytologic atypia. On radiographic view, they behave similar to typical ameloblastoma. It might present as a cystic lesion with benign clinical features or as a large ulceration, with significant bone destruction and tooth loss. The lesion is often found after an incisional biopsy unexpectedly. Because of its aggressive behavior (extension to surrounding soft tissues, extensive bone destruction, lymph node involvement and metastasis), the treatment of choice for the lesion is wide excision. Regional lymph node dissection, radiotherapy and chemotherapy have restricted value for treatment. Close periodic evaluation of the patient is obligatory.Case PresentationWe report a case of ameloblastic carcinoma of the maxilla in a 46-years-old male patient, including radiographic, clinical and pathologic features. The clinical and radiographic views mimicked ameloblastoma, while histologic sections revealed cytologic atypia associated by ameloblastic carcinoma.
