صفحات 55-63مقدمهمهم ترین عامل زمینه ساز مرگ و میر بیماران دیابتی عدم انجام خودمراقبتی است. روش های آموزشی مختلفی جهت بهبود خودمراقبتی استفاده می شود. این مطالعه با هدف مقایسه ی تاثیر آموزش گروهی و مبتنی بر موبایل بر رفتارهای خودمراقبتی در بیماران دیابتی نوع دو انجام شد.روش هادر کارآزمایی بالینی 90 بیمار مبتلا به دیابت نوع دو مراجعه کننده به مراکز درمانی دانشگاه علوم پزشکی انتخاب و با توجه به برخی ویژگی های فردی به سه گره همگن آموزش گروهی، آموزش مبتنی بر موبایل و کنترل تقسیم شدند. درگروه آموزش گروهی هشت جلسه آموزش و در آموزش مبتنی بر موبایل نرم افزار آموزشی بر روی گوشی بیماران نصب گردید. ابزار جمع آوری داده ها شامل پرسشنامه ی نیازسنجی خودمراقبتی (اصغریان)، و پرسشنامه ی دموگرافیک در بیماران دیابتی نوع دو بود. داده ها با استفاده از نرم افزار SPSS نسخه ی 22 و آزمون های آماری واریانس یک طرفه و تی زوجی در سطح معنی داری 05/0 تجزیه و تحلیل شدند.یافته هاآموزش گروهی و آموزش مبتنی بر موبایل نسبت به آموزش روتین اثربخشی معناداری داشت (001/0=P). آموزش خودمراقبتی به جز در حیطه ی مراقبت از پا که تاثیر آموزش مبتنی بر موبایل بیشتر از آموزش گروهی بوده است، در سایر موارد اختلاف معنی داری نداشتند. در مقایسه ی نمره خود مراقبتی در سه گروه اختلاف معنی دار آماری دیده شد.نتیجه گیریهرچند در یافته ها اثربخشی آموزش گروهی و مبتنی بر موبایل مشاهده شد. اما استفاده از برنامه های آموزشی مبتنی بر موبایل به دلیل دسترسی آسان، فقدان محدودیت زمانی و مکانی توصیه می شود.کلیدواژگان: دیابت نوع دو، آموزش گروهی، آموزش مبتنی بر موبایل، خودمراقبتی

صفحات 64-70مقدمهبیماران مبتلا به دیابت جهت افزایش کیفیت زندگی نیازمند آموزش موثر و کارآمد هستند. با توجه به پیشرفت های فن آوری امروزی آموزش های مبتنی بر پیام کوتاه چندرسانه ای یکی از راهبردهای موثر آموزشی محسوب می شود. هدف این مطالعه مقایسه ی تاثیر دو نوع آموزش حضوری ومبتنی بر پیام کوتاه چندرسانه ای در محیط تلگرام بر مقادیر هموگلوبین گلیکوزیله و قندخون ناشتای بیماران مبتلا به دیابت تحت درمان با انسولین بود.روش هادر این کارآزمایی بالینی، 66 نفر از بیماران مبتلا به دیابت تحت درمان با انسولین مراجعه کننده به بیمارستان سینا تبریز به صورت تصادفی در بلوک های دوتایی به دو گروه آموزش حضوری (33 نفر) و آموزش پیام کوتاه چند رسانه ای (33 نفر) تخصیص یافتند. ابزار جمع آوری اطلاعات شامل پرسشنامه ی ثبت مشخصات دموگرافیک، مقادیر هموگلوبین گلیکوزیله و قند خون ناشتای بیماران قبل و سه ماه بعد از مداخله آموزشی بود. تجزیه و تحلیل اطلاعات با آزمون های آماری تی مستقل و وابسته انجام گرفت.یافته هاقبل و بعد از آموزش بین دو گروه آموزشی از نظر میانگین مقادیر هموگلوبین گلیکوزیله و قند خون ناشتا اختلاف معنی دار آماری مشاهده نشد (P>0/05) .در مقایسه درون گروهی بعد از آموزش، در گروه پیام کوتاه چندرسانه ای کاهش معنی دار آماری در میانگین مقادیر هموگلوبین گلیکوزیله مشاهده شد (P=0/02).نتیجه گیریآموزش مبتنی بر پیام کوتاه چندرسانه ای در محیط تلگرام در مقایسه با شیوه ی آموزش حضوری موجب بهبود مراقبت از خود و کاهش مقادیر میانگین هموگلوبین کلیکوزیله در بیماران مبتلا به دیابت می شود. از این بستر آموزشی می توان برای تسهیل فرآیند آموزش مراقبت از خود در بیماران استفاده کرد.کلیدواژگان: هموگلوبین گلیکوزیله، قند خون ناشتا، آموزش به بیمار، دیابت

صفحات 71-79مقدمهتشخیص زودهنگام بیماری دیابت به منظور جلوگیری از عوارض و آسیب های ناشی از این بیماری امری حیاتی است. هدف از این مقاله طراحی یک سیستم هوشمند در دسته بندی افراد مبتلا به دیابت، با روش رگرسیون مبتنی بر شبکه عصبی پرسپترون چندلایه است.روش هادر این مطالعه ی توصیفی- تحلیلی یک سیستم هوشمند برای دسته بندی افراد به دو دسته سالم و مبتلا به بیماری دیابت طراحی شده است. سیستم طراحی شده با استفاده از نرم افزار MATLAB نسخه ی 2015 (197613/0/5/8) شبیه سازی شده است. مجموعه داده مورد استفاده در این پژوهش، مجموعه داده معیار PID موجود در مخزن داده یادگیری ماشین دانشگاه ایروین کالیفرنیا است. این مجموعه داده شامل 768 رکورد از زنان هندی و 8 فاکتور تشخیصی برای بیماری دیابت است.یافته هاداده های این مجموعه پس از پیش پردازش به صورت تصادفی به 20 دسته از کل مجموعه داده تقسیم شدند که شامل داده های آموزش و آزمون متفاوت بودند. در هر دسته داده از 90 درصد داده ها برای مرحله ی آموزش و 10 درصد باقی مانده برای مرحله ی آزمون استفاده شد. نتایج حاصل در بهترین حالت براساس شاخص های حساسیت، اختصاصیت، صحت و دقت در دسته بندی افراد، به ترتیب معادل اعداد 4815/0، 9804/0، 8077/0، 9286/0 به دست آمد.نتیجه گیرینتایج به دست آمده برتری سیستم هوشمند طراحی شده در دسته بندی افراد به دو دسته سالم و بیمار را نسبت به سایر روش های پیاده سازی شده بر این مجموعه داده تائید می کند. استفاده از شبکه عصبی پرسپترون چندلایه برای تقریب تابع، موجب افزایش دقت سیستم پیشنهادی شده است.کلیدواژگان: بیماری دیابت، رگرسیون، شبکه عصبی پرسپترون چندلایه، مجموعه داده PID

صفحات 80-89مقدمهدیابت یکی از شایع ترین بیماری های مزمن بوده و کیفیت زندگی بیمار نقش مهمی در کنترل بیماری دارد. هدف از مطالعه ی حاضر، بررسی نقش واسطه ای کیفیت زندگی در رابطه ی بین افسردگی، استرس و اضطراب با شدت بیماری دیابت بود.روش هاتعداد 108 بیمار مبتلا به دیابت نوع دو (57 زن، 51 مرد) در این پژوهش شرکت کردند. شرکت کنندگان در پژوهش، پرسشنامه ی مشخصات فردی و پرسشنامه ی استاندارد 21-DASS و پرسشنامه ی کیفیت زندگی (SF-36) را تکمیل کردند.یافته هانتایج پژوهش نشان داد که میزان شدت بیماری با کیفیت زندگی رابطه ی منفی و با اضطراب، افسردگی و استرس رابطه ی مثبت معنادار دارد (01/0P<). نتایج تحلیل مسیر نیز حاکی از نقش واسطه ای کیفیت زندگی در رابطه ی بین افسردگی و اضطراب و میزان شدت بیماری دیابت نوع دو بود.نتیجه گیریبراساس نتایج حاصل، لازم است در زمینه ی کنترل بیماری دیابت، مداخلات روانشناختی را به منظور کاهش افسردگی و اضطراب و بهبود کیفیت زندگی این بیماران مورد توجه قرار داد.کلیدواژگان: افسردگی، اضطراب، دیابت، کیفیت زندگی

صفحات 90-96مقدمهدیابت یکی از بیماری های خطرناک و رایج جهان مدرن است. از آنجا که در تحقیقات پزشکی معمولا داده های کمی در دسترس است و داده های پزشکی دارای عدم قطعیت و ابهام زیادی است، برای پیدا کردن رابطه ی ورودی و خروجی در داده های پزشکی استفاده از مدل های فازی مناسب به نظر می رسد. هیچکدام از مقالات قبلی از رگرسیون فازی برای پیش بینی عوارض دیابت از جمله نفروپاتی استفاده نکرده اند. لذا در این مطالعه، مدل رگرسیون فازی برای پیش بینی نفروپاتی در بیمار دیابتی استفاده شده است.روش هادر پژوهش حاضر از نتایج GFR آزمایشات قبلی بیمار برای پیش بینی افق دورتری از GFR و در نهایت پیش بینی عارضه ی کلیوی استفاده شده است. بیماری های مزمن کلیوی براساس مقدار GFR سطح بندی شده است که فازی سازی داده ها براساس این سطوح انجام شده است. پیش بینی GFR طی مراحل زیر انجام شد: مرحله ی 1، تعریف مجموعه های فازی براساس سطوح GFR، که برای هر سطح یک مجموعه فازی در نظر گرفته شده است. مرحله ی 2، فازی سازی داده های بیمار براساس مجموعه های فازی. مرحله ی 3، پیش بینی GFR با مدل رگرسیون فازی. مرحله ی 4، غیرفازی سازی مقادیر پیش بینی با استفاده از غیرفازی ساز میانگین. مرحله ی 5، ارزیابی کارایی مدل. خطای RMSE برای مقایسه کارایی مدل استفاده شده است.یافته هانتایج پیش بینی GFR نشان داد که، مقایسه RMSE با استفاده از مدل رگرسیون خطی ساده 10.09 و با استفاده از مدل فازی 4.24 بود.نتیجه گیریمدل رگرسیون فازی قادر به پیش بینی نفروپاتی در بیماران دیابتی است.کلیدواژگان: پیش بینی، نفروپاتی دیابتی، دیابت، رگرسیون فازی، عدم قطعیت

صفحات 97-109هدفدیابت نوع یک، یکی از شایع ترین اختلالات متابولیکی در دوران کودکی است، به طوری که از هر 400 تا 600 کودک یک نفر به این بیماری مبتلا می شود. هدف از انجام این پژوهش اثربخشی درمان شناختی رفتاری بر تنظیم هیجان کودکان مبتلا به دیابت نوع یک بود.روش هاطرح پژوهش نیمه آزمایشی از نوع پیش آزمون، پس آزمون و پیگیری با گروه گواه بود. نمونه پژوهش 25 نفر از کودکان 8 تا 13 سال مبتلا به دیابت نوع یک با تشخیص پزشک متخصص غدد درون ریز بودند که به صورت هدفمند انتخاب شدند. و در دو گروه گواه (15 نفر) و آزمایش (10 نفر) قرار گرفتند. آزمودنی ها پرسشنامه ی نظم جویی شناختی هیجان CERQ-K-P (گارنفسکی و همکاران، 2007) را در پیش آزمون، پس آزمون و پیگیری تکمیل کردند. داده های حاصل از پژوهش با روش تحلیل واریانس با اندازه گیری مکرر مورد تجزیه و تحلیل قرار گرفت.یافته هانتایج پژوهش نشان داد که بین ابعاد تنظیم هیجان در پیش آزمون، پس آزمون و پیگیری تفاوت معنی داری وجود داشت (01/0>P). همچنین بین ابعاد تنظیم هیجان دو گروه آزمایش و گواه تفاوت معنی داری وجود داشت (05/0>P).نتیجه گیریدرمان شناختی رفتاری می تواند به عنوان یک مداخله ی اثربخش جهت تنظیم هیجان کودکان مبتلا به دیابت محسوب شود.کلیدواژگان: درمان شناختی رفتاری، تنظیم هیجان، دیابت نوع یک

The aim of this study was to Comparison of the Effect of Group Based and Mobile Based Education on Self-Care Behaviors in Type II Diabetic PatientsMethodsThis randomized clinical trial was conducted on 90 patients' diabetic type 2 who referred to diabetic clinic of Ahvaz University of Medical Sciences. Initially, patients were divided into three groups of homogeneous mobile-based education, group training and control group based on individual characteristics. . In group training, eight sessions of training were conducted, mobile education was installed on the patient's phone and the control group through had given routine education. The data collection tool was self-care questionnaire, demographic, and demographic questionnaire for type II diabetic patients. Data were analyzed using SPSS 22 software and one-way and one-way ANOVA tests at a significant level was (0.05).ResultsGroup training and mobile-based education had a significant effect on routine education (P = 0.001). Self-care education, except in the field of foot care that the effect of mobile-based education was more than group training. In comparison, self-care score in the three groups was statistically significant. Although there was no statistically significant difference between the two educational groups, the effectiveness of education in the mobile group was more effective.ConclusionHowever, the findings found the effectiveness of group-based and mobile-based education. Pages 64-70BackgroundPatients with diabetes need to be educated efficiently and effectively in order to increase their quality of life. According to modern technological developments, multimedia message-based education is considered as one of the effective educational strategies. The purpose of this study was to investigate the effect of multimedia-based education in the Telegram application and in-person method on fasting blood glucose and glycosylated hemoglobin levels in patients with insulin-dependent diabetes.MethodsIn this clinical trial study, a sample of 66 patients with insulin-dependent diabetes who referred to the emergency department and the clinic of Sina hospital in Tabriz, were randomly assigned in double blocks into two groups: in-person education and multimedia-based education. Data gathering tools included a demographic form, glycosylated hemoglobin and fasting blood glucose were measured before and three months after the educational intervention. Data were analyzed with independent and paired samples t-tests.ResultsThe results indicated that there were no significant differences in the mean values of glycosylated hemoglobin and fasting blood glucose between two groups before and after education(P >0.05). In within-group comparison, there was a statistically significant difference in the multimedia message group on the reduction of mean values of glycosylated hemoglobin (p= 0.02) but these values differences were not significant in in-person group (p= 0.33).ConclusionMultimedia-based education in the Telegram application compared to in-person education improves self-care and reduces the mean values of glycosylated hemoglobin in diabetic patients. Pages 71-79BackgroundsEarly detection of diabetes is critical to avoid complications and damage caused by this disease. The purpose of this paper is designing an intelligent system for Diabetes prediction (healthy or patient) by using regression method based on Multilayer Perceptron Neural Network.MethodsIn this descriptive-analytic study, an intelligent system is designed to classification diabetes patients. The system is simulated by MATLAB software 2015 (8.5.0.197613). In this study, used PID dataset in UCI Machine Learning Repository. The dataset is contained 768 records from Indian women and 8 diagnostic factors for Diabetes.ResultsThe data were then divided randomly in 20 groups for training and testing, after preprocessing. 90% of the data is used for training phase and 10% for the test phase. The results obtained based on sensitivity, specificity, accuracy and precision were 0.4815, 0.9804, 0.8077 and 0.9286, respectively.ConclusionThe obtained results, showed superiority of designed intelligent system to classify individuals (healthy and patient) in comparison with other methods implemented on this dataset. Pages 80-89BackgroundDiabetes mellitus is one of the most common chronic diseases and the patient's quality of life plays an important role in controlling the disease. The purpose of this study was to investigate the mediating role of quality of life in the relationship between depression, stress and anxiety, with severity of diabetes.Methods108 patients with type 2 diabetes (57 women, 51 males) participated in this study. The participants completed the 21st-DASS Questionnaire, a quality of life questionnaire (SF-36), and a demographic questionnaire.ResultsThe results of the study showed that the severity of the disease was negatively correlated with quality of life and positively correlated with anxiety, depression and stress (P <0.01). The results of path analysis also indicated the mediating role of quality of life in the relationship between depression and anxiety and the severity of type 2 diabetes.ConclusionBased on the results, it is necessary to consider psychological interventions in order to reduce depression and anxiety and improve the quality of life of patients in the field of diabetes management.Keywords: Depression, Anxiety, Diabetes, Quality of Life

Pages 90-96BackgroundDiabetes is one of the most dangerous and common diseases of the modern world. Since medical research usually has limited data available and medical data is very ambiguous, it seems appropriate to use the fuzzy model to find out the relationship between input and output in medical data. None of the previous articles of fuzzy regression have been used to predict complications of diabetes, including nephropathy. Therefore, in this study, a fuzzy regression model was used to predict nephropathy in a diabetic patient.MethodsIn the present study, GFR results of previous patient experiments were used to predict a deeper horizons of GFR and ultimately to predict renal disease. Chronic kidney disease has been stratified based on the amount of GFR, that fuzzy data has been constructed based on these levels. The GFR prediction was performed in the following steps. Step 1: Define fuzzy sets based on the GFR level, which is considered for each level of a fuzzy set. Step 2: Fuzzify patient data Based on fuzzy sets. Step 3: GFR prediction with fuzzy regression model. Step 4: Defuzzifying the predictions. Step 5: Evaluating the model efficiency. The RMSE error is used to compare the performance of the model.ResultsThe results of GFR prediction showed that comparison RMSE was 10.09 with using simple linear regression model and 4.24 in fuzzy model.Conclusionsfuzzy regression model can predict nephropathy in diabetic patients.Keywords: Prediction, Diabetes, Diabetic Nephropathy, Fuzzy Regression

Pages 97-109BackgroundType 1 diabetes is one of the most common metabolic abnormalities in childhood, with one in every 400 to 600 children affected by the disease. The aim of study was to evaluate the effectiveness of cognitive-behavioral therapy on emotional regulation of children with type 1 diabetes.MethodsThe research design was a quasi-experimental design with pre-test, post-test and follow-up and control group. The sample of 25 children aged 8 to 13 years with type 1 diabetes was diagnosed by endocrinologist. They were randomly assigned control (n = 15) and experimental (n = 10) groups. Subjects completed an cognitive emotion regulation questionnaire (Garnefski et al., 2007) in a pre-test, post-test, and one month and a half follow-up. Data were analyzed using repeated measure analysis of variances.ResultsThe results of this study showed that there was a significant difference between the mean scores of emotional regulation in pre-test, post-test and follow-up (P <0.01). Also, there was a significant difference between emotional regulation in the experimental and control groups (P <0.05).ConclusionCognitive-behavioral therapy can be considered as an effective intervention to regulate the excitement of children with diabetes.Keywords: Cognitive behavioral therapy_Emotional regulation_Type 1 diabetes
