صفحات 328-334مقدمهبارداری بر عملکرد جنسی زنان اثرگذار است. اگر چه مطالعات قبلی عوامل مختلفی را به عنوان عوامل موثر بر عملکرد جنسی معرفی کرده اند، اما مطالعات اندکی در زمینه ی عوامل پیش گویی کننده ی اختلال جنسی در دوران بارداری انجام شده است. این عوامل، تحت تاثیر مسایل فرهنگی هر جامعه قرار دارند. از این رو، مطالعه ی حاضر با هدف تعیین عوامل پیش گویی کننده ی اختلال عملکرد جنسی در نمونه ای از زنان باردار شهر تهران انجام شد.روش هااین پژوهش توصیفی- تحلیلی بر روی 250 زن باردار تهرانی با روش نمونه گیری در دسترس انجام شد. ابزار گردآوری اطلاعات، شامل پرسش نامه ی اطلاعات فردی و مامایی، شاخص عملکرد جنسی زنان، پرسش نامه ی رضایت زناشویی Enrich (نسخه ی کوتاه)، مقیاس دیسترس جنسی زنان و پرسش نامه ی نگرانی های دوره ی بارداری بود. داده ها با استفاده از نرم افزار آماری SPSS و آزمون های آماری Kolmogorov-Smirnov، One-way ANOVA، همبستگی Pearson و Linear regression به روش Enter تجزیه و تحلیل شد. 05/0 > P به عنوان سطح معنی داری در نظر گرفته شد.یافته هامیانگین ± انحراف معیار نمره ی عملکرد جنسی 73/9 ± 7/22 بود و بیشتر زنان باردار (72 درصد) اختلال عملکرد جنسی داشتند. در میان حوزه های عملکرد جنسی، زنان در حوزه ی «درد» بیشترین میزان اختلال جنسی را داشتند. مدت ازدواج، نگرانی دوران بارداری و دیسترس جنسی به طور معنی داری پیش گویی کننده ی عملکرد جنسی زنان باردار بودند (050/0 ≥ P).نتیجه گیریمدت ازدواج، نگرانی دوران بارداری و دیسترس جنسی، پیش گویی کننده ی عملکرد جنسی زنان باردار می باشند. شناخت این عوامل گامی رو به جلو در راستای طراحی مطالعات مداخلاتی به منظور کمک به زوجین برای بهبود سلامت جنسی در دوران بارداری می باشد.کلیدواژگان: اختلال عملکرد جنسی، بارداری، زنان

صفحات 335-341مقدمهاسترس اکسیداتیو، به عنوان اختلال در تعادل بین تولید گونه های اکسیژن واکنشی (رادیکال های آزاد) و توانایی بدن در دفاع و سم زدایی آن از طریق آنتی اکسیدانی تعریف می شود و می تواند منجر به آسیب بافتی در بیماران مبتلا به دیابت گردد. با توجه به شیوع افزایش بروز دیابت بارداری در طی دهه های اخیر و نتایج برخی مطالعات مبنی بر تاثیر عوامل تغذیه ای و استرس اکسیداتیو در بروز آن و همچنین، عدم انجام مطالعات کافی در این زمینه، مطالعه ی حاضر با هدف مقایسه ی شاخص های استرس اکسیداتیو در زنان مبتلا و غیر مبتلا به دیابت بارداری انجام شد.روش هامطالعه ی مورد- شاهدی حاضر در سال های 96-1395 در مرکز بهداشت شماره ی 2 اصفهان انجام شد. 37 زن مبتلا به دیابت بارداری و 53 زن غیر مبتلا به دیابت بارداری به روش نمونه گیری آسان انتخاب و وارد مطالعه شدند و شاخص های استرس اکسیداتیو شامل غلظت Glutathione (GSH)، ظرفیت تام اکسیدانی، Non-esterified fatty acids (NEFA)، سوپراکسید دسموتاز، کاتالاز و مالون دی آلدئید در دو گروه تعیین و مقایسه شد.یافته هامیانگین ظرفیت تام آنتی اکسیدانی در دو گروه مبتلا و غیر مبتلا به دیابت به ترتیب 0/63 ± 6/441 و 9/44 ± 5/562 میکرومول/لیتر بود (011/0 = P). میانگین سطح کاتالاز نیز در دو گروه پیش گفته به ترتیب 32/0 ± 12/1 و 59/0 ± 79/1 واحد در لیتر به دست آمد و تفاوت دو گروه معنی دار بود (024/0 = P)، اما سایر عوامل در دو گروه مبتلا و غیر مبتلا به دیابت اختلاف معنی داری نداشت.نتیجه گیریسطح برخی نشانگرهای استرس اکسیداتیو از جمله سطح ظرفیت تام آنتی اکسیدانی و کاتالاز در زنان غیر مبتلا به دیابت به طور معنی داری بالاتر بود. از این رو، به نظر می رسد استرس اکسیداتیو در پاتوژنز دیابت بارداری نقش داشته باشد.کلیدواژگان: استرس اکسیداتیو، دیابت، بارداری

صفحات 342-349مقدمهاندازه گیری کارایی سیستم های امتیازبندی جهت تعیین پیش آگهی بیماری ها در یک مطالعه ی جامع در کشور، ضروری به نظر می رسد. هدف از انجام پژوهش حاضر، مقایسه ی فراوانی مرگ و میر با میزان پیش بینی شده ی سیستم های امتیازدهی شناخته شده بود.روش هادر این تحقیق مقطعی- تحلیلی که در سال 1397 در بخش مراقبت های ویژه (Intensive care unit یا ICU) بیمارستان امین اصفهان انجام گردید، پرونده های سال های 1394 تا 1396 بیماران غیر ترومایی با سن بیشتر از 16 سال مورد بررسی قرار گرفت. اطلاعات بیماران جهت محاسبه ی نمره در سیستم های Acute Physiology and Chronic Health Evaluation II (APACHE II)، Sequential Organ Failure Assessment (SOFA) و چک لیست جدید (M score) استخراج شد. سپس عاقبت بستری آن ها با امتیاز هر یک از این سیستم ها مقایسه گردید.یافته هانقطه ی برش مناسب برای پیش بینی مرگ و میر بیماران بخش ICU در سیستم های APACHE II، M score و SOFA به ترتیب برابر با 5/13، 5/15 و 5/4 گزارش شد. بر اساس یافته ها، با افزایش هر کدام از متغیرها برای یک بیمار، احتمال از دست رفتن وی افزایش می یافت.نتیجه گیریچک لیست M score که با هدف اصلی ارزیابی دقیق کلینیکی و پاراکلینیکی روزانه ی بیمار توسط پزشک و پرستار طراحی شده، توانسته است در ارزیابی دقیق پیش آگهی بیماران بستری در ICU با سیستم های SOFA و APACHE II رقابت نماید.کلیدواژگان: بخش مراقبت های ویژه، مرگ و میر، ارزش پیش بینی تست ها

صفحات 350-356مقدمهبخش مراقبت های ویژه، از جمله مهم ترین بخش های بیمارستانی به شمار می رود و در همین راستا، سیستم های امتیازبندی متنوعی به ارزیابی وضعیت بیمار و پیش بینی نتیجه ی بستری در Intensive care unit (ICU) پرداخته اند که هر یک نقاط قوت و ضعفی داشته اند و منجر به معرفی سیستم های جدید شده اند. مطالعه ی حاضر، با هدف مقایسه ی دو سیستم امتیازبندی Mortality probability model-III (III-MPM) و Simplified acute physiology score-III (SAPS-III) انجام شد.روش هامطالعه ی حاضر، از نوع مقطعی بود و بر روی 200 بیمار بستری در ICU به علت تروما در سال های 95-1394 انجام شد. داده ها شامل مشخصات دموگرافیک، میانگین فشار خون سیستول، دیاستول و شریانی، نبض، تعداد تنفس، تب، Glasgow coma scale (GCS)، تحلیل گاز شریانی، شمارش لکوسیتی، هماتوکریت، سطح بیلی روبین و کراتینین، نوع بستری و وجود بیماری زمینه ای از پرونده ی بیماران استخراج شد و امتیاز MPM-III و SAPS-III محاسبه و مقایسه گردید.یافته هاسیستم MPM-III در نقطه ی برش 13/0، قدرت تشخیصی (Discrimination) 935/0 (89/0-97/0 :Confidence interval یا CI 95 درصد، 001/0 > P) و به ترتیب حساسیت و ویژگی 87 و 84 درصد داشت. همچنین، سیستم SAPS-III در نقطه ی برش 13/0، قدرت تشخیصی (Discrimination) 77/0 (001/0 > P، 85/0-69/0 :CI 95 درصد)، حساسیت 80 درصد و ویژگی 68 درصد داشت. بر اساس سیستم امتیازدهی MPM-III و SAPS-III، مدت زمان بستری در ICU (001/0 = P برای هر دو سیستم) و مدت زمان اینتوباسیون (001/0 > P برای هر دو سیستم) ارتباط معنی داری با مرگ و میر داشتند. زمان کل بستری (001/0 > P) فقط با مرگ و میر بر اساس سیستم SAPS-III ارتباط داشت.نتیجه گیریسیستم امتیازبندی MPM-III نسبت به SAPS-III قدرت تشخیصی بالاتری دارد. همچنین، نرخ مرگ و میر بر اساس هر دو سیستم، ارتباط مستقیمی با تعداد روز بستری در ICU و مدت اینتوباسیون داشت.کلیدواژگان: بخش مراقبت های ویژه، Simplified acute physiology score، مرگ و میر، احتمال

صفحات 357-371مقدمهتوانایی نوسازی، چند توانی و ویژگی تنظیم ایمنی، سلول های بنیادی مزانشیمی را به کاندیدای مناسبی برای سلول درمانی مبدل ساخته است. به تازگی، Mesenchymal stem cells (MSCs) و ترشحات آن ها در کنترل بیماری های عفونی مورد توجه قرار گرفته اند. MSCs، می تواند پاتوژن را بشناسد، به محل عفونت حرکت کند و با جهت دهی پاسخ های ایمنی و تولید پپتیدهای ضد میکروبی، با آن ها مبارزه کند. مطالعه ی مرورری حاضر با هدف بررسی نقش درمانی MSCs در بیماری های عفونی انجام شد.روش هااین مطالعه به صورت مروری و با جستجوی واژگان کلیدی MSCs، میان کنش MSCs و سلول های سیستم ایمنی، نقش MSCs در عفونت و سلول درمانی بیماری های عفونی با MSCs در پایگاه های اطلاعاتی PubMed، Science Direct، Scopus و موتور جستجوگر Google scholar انجام شد. در نهایت، 100 مقاله انتخاب و بررسی شدند.یافته هابر اساس مطالعات انجام شده، MSCs درمانی یک روش امیدوار کننده در کنترل بیماری های عفونی می باشد. MSCs می تواند با هر دو سیستم ایمنی میزبان و پاتوژن میان کنش دهد. حاصل این میان کنش، ایجاد پاسخ های التهابی و ضد التهابی، ترشح پپتیدهای ضد میکروبی و تاثیر بر تمایز و عملکرد سلول های ایمنی است.نتیجه گیریاثرات مثبت و منفی MSCs در هدایت پاسخ های ایمنی به دفعات تزریق، مرحله ی عفونت و تحریک گیرنده های MSCs بستگی دارد. بنابراین، انجام مطالعات فراگیر جهت ارایه ی راه کار درمانی مناسب در هر بیماری عفونی ضروری استکلیدواژگان: سلول های بنیادی مزانشیمی، عفونت، التهاب، سلول درمانی

Pages 328-334BackgroundPregnancy affects women’s sexual function. Although previous studies have introduced various factors affecting sexual function, but few studies have examined the prognostic factors on sexual dysfunction during pregnancy. These factors are influenced by cultural issues of each society. This study aimed to determine the prognostic factors of sexual dysfunction in selected pregnant women in Tehran City, Iran.MethodsThis descriptive analytical study was conducted on 250 pregnant women in Tehran selected using convenience sampling method. The data collection instrument consisted of a questionnaire for individual and midwifery information, female sexual function index, Enrich marital satisfaction questionnaire (short version), female sexual distress scale, and prenatal distress questionnaire. The data were analyzed via SPSS software using Kolmogorov-Smirnov, one-way ANOVA, Pearson correlation, and linear regression (Enter method) statistical tests. The significance level was considered to be less than 0.05.FindingsThe mean (standard deviation) of sexual function score was 22.27 (9.73), and the majority of pregnant women (72%) had sexual dysfunction. Among the areas of sexual function, women had the highest rates of sexual dysfunction in the area of “pain”. The duration of marriage, prenatal distress, and sexual distress were significantly predictive factors of sexual dysfunction in pregnant women (P < 0.05).ConclusionThe duration of marriage, prenatal distress, and sexual distress significantly predicted sexual dysfunction in pregnant women. Knowing these factors is an important achievement for better prenatal care and designing interventional studies to improve sexual health during pregnancy.Keywords: Sexual dysfunctions, Pregnancy, Women

Pages 335-341BackgroundOxidative stress, defined as a disturbance in the balance between the production of reactive oxygen species (free radicals) and antioxidant defenses, is discussed in relation to its possible role in the production of tissue damage in diabetes mellitus. Regarding the prevalence of gestational diabetes mellitus in recent decades and the results of some studies about the effect of nutritional anomalies and oxidative stress, as well as insufficiency of studies on its occurrence, this study aimed to compare of oxidative stress indices in women with and without gestational diabetes mellitus.MethodsIn a case-control study conducted in Isfahan Health Center No. 2, Isfahan, Iran, in 2017-2018, 37 women with gestational diabetes mellitus and 53 women without it mellitus were selected using convenient sampling method. Oxidative stress indices including glutathione (GSH), total antioxidant capacity, superoxide dismutase, non-esterified fatty acids (NEFA), catalase, and malondialdehyde were measured and compared between the two groups.FindingsThe mean total antioxidant capacity was 441.6 ± 63 and 562.5 ± 44.9 μmol/l in women with and without gestational diabetes mellitus, respectively (P = 0.011). The mean catalase level in the two groups was 0.32 ± 1.12 and 1.79 ± 0.59 U/l, respectively, and the difference between the two groups was statistically significant (P = 0.024), but the other factors were not significantly different between the women with and without gestational diabetes mellitus.ConclusionThe levels of some oxidative stress markers such as total antioxidant capacity and catalase were significantly higher in women with gestational diabetes mellitus. Therefore, oxidative stress seems to play a role in the pathogenesis of gestational diabetes.Keywords: Oxidative stress, Diabetes mellitus, Pregnan

Pages 342-349BackgroundIt seem necessary to assess the precision of scoring systems for estimating disease severity and prognosis of the patients in intensive care units (ICUs). In this study, we aimed to compare the predictive accuracy of a novel checklist (M score) with well-known scoring systems in non-trauma patients in ICU.MethodsThroughout a cross-sectional study in 2018, 835 non-trauma patients admitted in ICU with the age of more than 16 years were included. Mortality prediction was assessed using Acute Physiology and Chronic Health Evaluation II (APACHE II), Sequential Organ Failure Assessment (SOFA), and M score at the first and last day of their ICU stay. Using logistic regression test, receiver operation curve, Pearson’s regression, chi-square, and independent samples t tests, the data from these three scoring systems were analyzed, and the sensitivity and specificity of systems were calculated.FindingsThe cut-off point for predicting mortality was 13.5 for APACHE II, 15.5 for M score, and 6.5 for SOFA. The probability of death increased by increasing in any of studied scores.ConclusionM score, which has been prepared for accurate easement of clinical and paraclinical status of patients by nurses and physicians, seems to be a good corrival for well-known scoring systems.Keywords: Intensive care unit, Mortality, Predictive value of tests

Pages 350-356BackgroundIntensive care unit (ICU) is among the most important hospital wards. Variety of scoring systems for evaluation of patients' status and prediction of hospitalization outcomes in ICU has been raised that each has strong and weak points; assessment of these characteristics tends to promote new scoring systems. The current study compared scoring systems of Mortality Probability Model-III (MPM-III) and Simplified Acute Physiology Score-III (SAPS-III) in trauma patients in ICU.MethodsThis randomized cross-sectional study was conducted on 200 patients admitted in ICU because of trauma in years 2016-17. Patients' information including demographics, mean of systolic, diastolic, and arterial blood pressure, pulse, respiratory rate, temperature, Glasgow coma scale (GCS), arterial gas analysis, white blood cell (WBC) counts, hematocrit, bilirubin, creatinine, type of admission, and presence of underlying diseases were extracted from records; MPM-III and SAPS-III were measured for these patients and compared.FindingsMPM-III scoring system had discrimination of 0.935 [95% confidence interval (95%CI): 0.89-0.97; P < 0.001) in cut-off point of 0.13, and its sensitivity and specificity was 87% and 84%, respectively. For SAPS-III system, in cut-off point of 0.13, the discrimination was 0.77 (95%CI: 0.69-0.85; P < 0.001), with the sensitivity of 80% and specificity of 68%. Based on both MPM-III and SAPS-III systems, mortality was in correlation with duration of ICU admission (P = 0.001 for both systems) and duration of intubation (P < 0.001 for both systems), while only for SAPS-III, total duration of hospitalization was in correlation with mortality (P < 0.001).ConclusionMPM-III scoring system was superior to SAPS-III regarding discrimination power in trauma patients. In addition, based on both systems, mortality rate was in direct association with days of ICU admission and intubation duration.Keywords: Intensive care unit, Simplified Acute Physiology Score, Mortality, Probability

Pages 357-371BackgroundSelf-renewal, multipotent, and immunomodulatory are properties of mesenchymal stem cells (MSCs) that make them a good candidate for cell therapy. Recently, MSCs and their secretions are considered in control of infectious disease. MSCs can recognize pathogens, migrate to infection site, and fight against them by redirecting immune responses and anti-microbial peptide secretion. In this review, the therapeutic role of MSCs in infectious disease is discussed.MethodsIn this review article, we searched MSCs, interaction of MSCs and immune cells, MSCs in infection, and MSCs therapy in infectious disease as key words in valid databases including PubMed, Science Direct, Scopus, and Google scholar. Finally, 100 articles were selected and reviewed completely.FindingsAccording to the studies, MSCs therapy is a promising method for control of infectious disease. MSCs interact with both host immune systems and pathogen. The result of this interaction is inflammatory and anti-inflammatory responses, secretion of antimicrobial peptides, and influence on the differentiation and function of immune cells.ConclusionPositive and negative effects of MSCs in the direction of immune response depend on the number of injections, infection phase, and stimulation of MSCs receptors. Therefore, comprehensive studies are needed to represent effective therapeutic protocols for any infectious disease.Keywords: Mesenchymal stem cells, Infection, Inflammation, Cell therapy
