صفحات 517-521مقدمهبیماری مزمن انسدادی ریه (Chronic obstructive pulmonary disease یا COPD) نوعی بیماری انسدادی ریوی است که با انسداد مسیر هوایی به صورت مزمن مشخص می شود. هدف از انجام این مطالعه، تشخیص مولکولی Haemophilus influenzae، در بیماران مبتلا به بیماری انسداد ریوی مزمن بود.روش هاتعداد 100 نمونه از ترشحات تنفسی بیماران مبتلا به COPD جمع آوری شد. شناسایی مولکولی Haemophilus influenzae با استفاده از پرایمرهای اختصاصی در مقابل کیت استاندارد به روش واکنش زنجیره ای پلیمراز (Polymerase chain reaction یا PCR) انجام شد.یافته ها10 نفر از 100 نفر با روش کیت و 8 نفر از 100 نفر در PCR با استفاده از پرایمرهای اختصاصی از نظر باکتری Haemophilus influenzae مثبت بودند.نتیجه گیریکلیدواژگان: Haemophilus influenzae، بیماری انسداد ریوی مزمن، مدل های مولکولی، پرایمرهای DNA

صفحات 522-528مقدمهدرحین عمل جراحی، تیم جراحی در تماس با پاتوژن های عفونی و بیماری زا می باشند. دستکش های جراحی، به عنوان یکی از وسایل حفاظت شخصی، از انتقال آلودگی بین اعضای تیم جراحی و بیمار جلوگیری می کند که اهمیت این مساله در اعمال جراحی ارتوپدی به دلیل وجود وسایل تیز و برنده، بیشتر است. هدف از انجام این مطالعه، بررسی فراوانی آسیب دیدگی دستکش های جراحی و شناسایی علل این آسیب دیدگی ها، در اعمال جراحی ارتوپدی بود.روش هادر این مطالعه ی توصیفی- تحلیلی، از مجموع 32 عمل جراحی (16 مورد دست و 16 مورد پا)، 384 عدد دستکش جراحی با روش نمونه گیری آسان انتخاب و بررسی شد. نمونه ها، از اعمال جراحی پلاک گذاری ارتوپدی دست و پا در اتاق عمل ارتوپدی و اعصاب بیمارستان آیت اله کاشانی اصفهان در تابستان 1395 گرفته شدند. به منظور بررسی میزان آسیب دیدگی دستکش های جراحی، از روش نشت آب استفاده شد. ابزار گرداوری داده ها، چک لیست محقق ساخته بود. برای تجزیه و تحلیل داده ها، از آزمون های آمار توصیفی و تحلیلی استفاده گردید و داده ها با استفاده از نرم افزار SPSS واکاوی گردید.یافته هااز 384 دستکش مورد مطالعه، 103 مورد (8/26 درصد) دچار آسیب دیدگی شده بود. میزان آسیب دیدگی در اعمال جراحی پلاک گذاری دست (192 مورد) برابر 36 مورد (8/18 درصد) و در اعمال پلاک گذاری پا (192 مورد) برابر 67 مورد (9/34 درصد) بود. 3/33 درصد از دستکش ها در اعمال جراحی دست و 3/34 درصد در اعمال جراحی پا دارای آسیب دیدگی مخفی بودند. بیشترین علت آسیب آشکار دستکش های جراحی نیز تماس با بافت های ضخیم و استخوانی بیماران بود.نتیجه گیریتوجه به این که میزان آسیب دیدگی دستکش ها در اعمال جراحی پلاک گذاری ارتوپدی بالا می باشد، تعویض دستکش ها در حین عمل، می تواند باعث کاهش احتمال انتقال آلودگی بین اعضای تیم جراحی و بیماران گردد. با توجه به این که بیشترین عامل آسیب زننده به دستکش های جراحی، نسوج ضخیم و استخوانی بدن بیمار بود، پیشنهاد می شود افراد تیم جراحی در هنگام تماس با این بافت ها، بیشتر احتیاط کنند.کلیدواژگان: دستکش های جراحی، اتاق عمل، جراحی ارتوپدی

صفحات 529-534مقدمهبه تازگی، توجه زیادی به سمت تاثیرات تماس پره ناتال با سرب معطوف گردیده است. با توجه به شیوع بالای زایمان زودرس و عوارضی که به دنبال آن برای نوزادان ایجاد می شود، مطالعه ی حاضر با هدف بررسی سطح خونی سرب در مادران باردار و ارتباط آن با زایمان زودرس انجام شد.روش هاطی یک مطالعه ی توصیفی- تحلیلی مقطعی، از 63 خانم با زایمان زودرس و 63 خانم با بارداری Term، حین زایمان توسط پرستار نمونه ی خون گرفته شد. آزمایش های مورد نظر انجام و نتایج بین دو گروه مقایسه شد. سایر داده های جمع آوری شده، شامل سن، جنس، دور سر، قد و وزن بود.یافته هاوزن نوزادان در گروه های Term و زودرس به ترتیب 10/70 ± 80/2000 و 80/35 ± 44/3239 گرم، قد نوزادان به ترتیب 10/0 ± 46/50 و 40/0 ± 00/44 سانتی متر و دور سر نوزادان به ترتیب 10/0 ± 47/34 و 30/0 ± 49/31 سانتی متر بود. سطح خونی سرب (Blood lead level یا BLL) مادران در گروه های Term و زودرس به ترتیب 10/0 ± 05/2 و 10/0 01/3 میکروگرم/دسی لیتر بود (040/0 = P).نتیجه گیریسطح خونی سرب در مادران با زایمان زودرس، به طور معنی داری بالاتر از مادران با بارداری Term بود.کلیدواژگان: زایمان زودرس، سرب، وزن پایین تولد

صفحات 535-541مقدمهواریانت های ژنتیکی می توانند با تاثیر بر بیان MicroRNA (MIR)ها، موجب تغییر بیان ژن های هدف آن ها شوند و به دنبال آن، شانس ابتلا به سرطان را افزایش دهند. هدف از انجام مطالعه ی حاضر، بررسی ارتباط پلی مورفیسم تک نوکلئوتیدی شایع ژن MIR196A2 با احتمال ابتلا به سرطان پروستات (Prostate cancer یا PCa) در استان چهارمحال و بختیاری بود.روش هادر این مطالعه ی مورد- شاهدی، 99 بیمار در گروه مورد و 100 فرد سالم در گروه شاهد وارد شدند. از تمام شرکت کنندگان، 5 سی سی خون وریدی تهیه و DNA آن ها به روش فنل کلروفرم استخراج گردید. پرایمر اختصاصی برای پلی مورفیسم rs11614913 طراحی و کلیه ی افراد با روش High resolution melt (HRM) تعیین ژنوتیپ شدند. تعادل Hardy-Weinberg برای ژنوتیپ ها با استفاده از آزمون آماری 2χ بررسی شد و ارتباط بین ژنوتیپ های مختلف و خطر ابتلا به سرطان پروستات نیز با استفاده از آزمون Logistic regression بررسی گردید.یافته هااز میان 99 بیمار، 68 بیمار به PCa و 31 نفر به هایپرپلازی خوش خیم پروستات (Benign prostatic hyperplasia یا BPH) مبتلا بودند. آزمون Logistic regression نشان داد افراد با ژنوتیپ CC در مقایسه با افراد ناقل آلل T (TT + CT)، شانس بالاتری برای ابتلا به سرطان پروستات دارند (08/1 = Odd ratio یا OR). تجزیه و تحلیل فراوانی آللی نیز نشان داد که آلل C نسبت به آلل T، 30/1 برابر احتمال ابتلا به سرطان پروستات و هایپرپلازی خوش خیم پروستات افزایش می دهد.نتیجه گیریآلل C در پلی مورفیسم rs11614913 باعث افزایش خطر ابتلا به سرطان پروستات در مردان استان چهارمحال و بختیاری می گردد. از آن جایی که این پلی مورفیسم درون ژن MIR196A2 قرار دارد، سنجش بیان این ژن و ژن های هدف آن در مبتلایان به سرطان پروستات، می تواند ارزشمند باشد.کلیدواژگان: ژن ها، سرطان پروستات، MicroRNAs، پلی مورفیسم ژنتیک

صفحات 542-547مقدمهخواب یکی از مهم ترین نیازهای فیزیولوژیک در چرخه ی شبانه روزی و زیست محیطی و بخش مهمی از کیفیت زندگی به شمار می رود. به منظور ارزیابی وضع موجود برای مداخلات ارتقای سلامت، پژوهش حاضر با هدف بررسی ارتباط کیفیت خواب و توانایی شغلی در کارمندان انجام شد.روش هادر این مطالعه ی مقطعی، کارمندان دانشگاه علوم پزشکی اصفهان به روش نمونه گیری تصادفی آسان انتخاب شدند و پرسش نامه های ترجمه و معتبر شده ی استاندارد کیفیت خواب Pittsburgh (Pittsburgh Sleep Quality Index یا PSQI) (حاوی سوالات تاخیر، طول مدت، اثربخشی خواب، اختلالات خواب، مصرف داروی خواب آور و اختلال عملکرد روزانه) و شاخص توانایی شغلی (Work Ability Index یا WAI) (در ابعاد توانایی جسمی، فکری، بیماری، غیبت از کار، پیش بینی توانایی دو سال آینده و...) را تکمیل کردند. پاسخ ها جمع زده شد که جمع نمره ی PSQI کمتر از 5 نشان دهنده ی کیفیت خواب مطلوب و بیشتر از 5 بیانگر کیفیت خواب نامطلوب بود (نمره ی بالاتر کیفیت پایین تر خواب را نشان داد).یافته ها120 کارمند با میانگین سنی 27/7 ± 28/42 سال مورد بررسی قرار گرفتند. شاخص توانمندی شغلی عالی، خوب و متوسط در مردان به ترتیب 8/27، 0/41 و 2/31 درصد و در زنان 5/28، 5/49 و 0/22 درصد به دست آمد. میانگین نمره ی کیفیت خواب 38/2 ± 36/4 بود. ارتباط معنی داری بین کیفیت خواب و WAI کارمندان مشاهده شد. سن کارمندان با کیفیت خواب رابطه ی معنی داری را نشان داد، اما میان کیفیت خواب و میزان WAI با جنسیت، محل کار و وضعیت تاهل کارمندان ارتباط معنی داری وجود نداشت.نتیجه گیریکیفیت خواب گروه مورد مطالعه تا حدودی پایین بود. در افرادی که کیفیت خواب خوبی ندارند، توانایی و عملکرد شغلی پایین می آید که می تواند کیفیت خواب علت آن باشد یا تحت تاثیر عامل دیگری هر دو متغیر تغییر کرده باشد. تحقیقات بیشتر درباره ی علل کاهش کیفیت خواب و مداخلات اصلاح آن در زمینه ی زمان، مدت، آرامش در خواب و بیدار شدن پیشنهاد می شود.

Pages 517-521BackgroundChronic obstructive pulmonary disease (COPD) is a type of obstructive pulmonary disease characterized by chronic airway blockage. The aim of this study was to investigate the presence of Haemophilus influenzae in patients with COPD using molecular method.MethodsA total of 100 samples of respiratory secretion were collected from patients with COPD. The molecular identification of Haemophilus influenza was performed using specific primers against standard kits by polymerase chain reaction (PCR) method.FindingsHaemophilus influenzae bacteria were positive in 10 out of 100 patients, and 8 of 100 were positive in PCR using specific primers.ConclusionNucleic acid-like PCR tests, due to their sensitivity, high accuracy, and time saving, are the appropriate method for diagnosis of Haemophilus influenzae in patients with chronic pulmonary obstruction.Keywords: Haemophilus influenzae, Chronic obstructive pulmonary disease, Molecular models, DNA Primers

Pages 522-528BackgroundDuring surgery, the surgery team is in contact with infectious and pathogenic microorganisms. Surgical gloves prevent the transmission of contamination between surgical team members and the patient, which is more important in orthopedic surgeries due to the presence of sharp and winning devices. The purpose of this study was to determine the frequency of damage of surgical gloves, and identify the factors involved in these damages in orthopedic surgeries.MethodsIn this descriptive-analytical study, 384 surgical gloves (192 pairs) used in surgical procedures for palpation of the hands and feet were selected via simple sampling method. To determine the degree of damage of surgical gloves, a water leak test was used. The data-gathering tool was a researcher-made checklist. Descriptive and analytical statistics were used to analyze the data via SPSS software.FindingsThe frequency of damage to surgical gloves was 26.8% (103/384). The frequency of damages was 18.8% (36/192) in hand surgeries, and 34.9% (67/192) in foot plating. 33.3% of gloves in hand surgeries and 34.3% in foot surgical procedures had hidden damage. The most common cause of obvious damage to surgical gloves was contact with thick and bone tissues during the procedure.ConclusionConsidering the high rate of gloves damages in orthopedic plating surgery, replacement of gloves during surgery can reduce the transmission of contamination among surgical team members and patients. Considering that the most common cause of damage in surgical gloves was thick and bony tissues, more cautious is suggested during surgical treatment of these tissues.Keywords: Surgical gloves, Operating rooms, Orthopedic surgery

Pages 529-534BackgroundAttentions have recently been paid to the effects of contact with lead (Pb) and blood lead levels (BLL) on prenatal complications. Given the high prevalence of preterm labor and its following neonatal complications, this study aimed to evaluate the BLL in pregnant mothers and its relation with preterm labor.MethodsThroughout a cross-sectional analytical descriptive study, 63 women with preterm labor and 63 cases with term labor participated. A blood sample was obtained from each mother by the same nurse and BLLs were compared between the two groups. Other collected data included mother’s age and sex, and neonates’ head circumference, height, and weight.FindingsNewborns in term and preterm groups weighed 2009.80 ± 70.1 and 3239.44 ± 35.80 grams respectively; their mean height was 50.46 ± 0.10 and 44.00 ± 0.40 cm, and their mean head circumference was 34.47 ± 0.1 and 31.49 ± 0.30 cm, respectively. The mean BLL of mothers in term and preterm newborns was 2.05 ± 0.10 and 3.01 ± 0.1 μg/dl, respectively (P = 0.04).ConclusionBLL in mothers with preterm labor was significantly higher than in mothers with preterm labor.Keywords: Preterm labor, Lead, Low birth weight

Pages 535-541BackgroundGenetic variants affect the expression of microRNAs (miRNA) resulted in changes in the expression levels of the miRNA target genes that may lead to the increased risk of cancer. The aim of the current study was to investigate the association between the common single-nucleotide gene polymorphism T < C (rs11614913) in MIR196A2 and the risk of developing prostate cancer (PCa) in Chahar Mahal and Bakhtiari Province, Iran.MethodsIn this case-control study, 99 patients were enrolled in the case group and 100 healthy controls in the control group. From all participants, 5 ml venous blood sample was collected and DNA was extracted by phenol-chloroform extraction. Specific primers were designed for rs11614913 polymorphism, and this polymorphism was genotyped using High Resolution Melt (HRM) analysis. Hardy-Weinberg equilibrium for the genotypes was tested by chi-squared test. The association between different genotypes and the risk of prostate cancer was investigated by logistic regression analysis.FindingsAmong 99 patients, 68 had prostate cancer and 31 had benign prostatic hyperplasia (BPH). Logistic regression analysis showed that those cases with CC genotype had a higher risk of developing prostate cancer [Odds ratio (OR) = 1.08] than T (TT+CT) carriers. Allele frequency analysis revealed that the C allele increased the risk of developing prostate cancer and BPH 1.3 more than T allele.ConclusionThe C allele in rs11614913 polymorphism increases the risk of prostate cancer. Because this polymorphism is present in MIR196A2 gene, evaluation of expression of the miRNA gene and its target genes in patients with prostate cancer could be valuable.Keywords: Genes, Prostate cancer, MicroRNAs, Genetic polymorphism

Pages 542-547BackgroundSleep is one of the important physiologic needs in daily life cycle, and a part of quality of life. To design health promotion interventions, we assessed the relationship of sleep quality with job ability.MethodsIn this cross-sectional study, the staffs of Isfahan University of Medical Sciences, Isfahan, Iran, selected by simple random sampling method, filled the validated questionnaires of Pittsburgh sleep quality (PSQI) and work ability index (WAI). The first included questions about sleep latency, duration, and efficiency, use of sleeping medication, and daytime dysfunction. The second included physical and mental abilities, diseases, job losses, prognosis of work ability for 2 years, etc. The answers were summated; the scores less than 5 in PSQI showed optimal quality of sleep, and more than 5 showed undesirable quality of sleep.Findings120 employees with mean age of 42.28 ± 7.27 years were enrolled. The WAI was high, good, and moderate in 27.8, 41.0, and 31.2 percent of men, and in 28.5, 49.5, and 22.0 percent of women, respectively. The mean score of PSQI was 4.36 ± 2.38. WAI had significant relation with quality of sleep, and the quality of sleep had relation with age. There was not any significant relation between sex, site of work, and marital status with sleep quality and work ability.ConclusionSleep quality was relatively low in the staffs, and affected their work abilities; maybe both were established under other causes. However, more researches on causes of low sleep quality, and interventions by time, duration, and relaxation for prevention and treatment of it are suggested.</div>Keywords: Sleep, Work performance, Iran
