رادیوتراپی قبل از ماستکتومی موجب افزایش شدت استرس و هورمون کورتیزول میگردد، با توجه به استقبال بیماران و پزشکان از روش های غیردارویی مدیریت استرس، مطالعه حاضر با هدف اثر طب فشاری گوش بر اضطراب و سطح هورمون کورتیزول قبل از ماستکتومی در زنان دریافت کننده رادیوتراپی انجام شد.
روش بررسیاین مطالعه کارآزمایی بالینی تصادفی شده در طی سالهای 98-1397 در بیمارستان امام رضا تبریز با مشارکت 66 (هر گروه 33 نفر) بیمار انجام شد. بیماران گروه مداخله از سه روز قبل از جراحی روزانه دو نوبت طب فشاری گوش را انجام می دادند (هر گوش هشت دقیقه)، سطح سرمی کورتیزول و اضطراب پنهان و آشکار (ابزار اشپیل برگر) قبل و بعد از مداخله اندازه گیری شد (برای گروه کنترل اقدامی انجام نشد) و با آزمون های آماری مناسب در نرمافزار SPSS20 مقایسه شدند. P کمتر از 05/0 معنادار تلقی شد.
یافته هامتغیرهای سطح اضطراب پنهان (41/6±22/43) و آشکار (52/5±50/49) و هورمون کورتیزول (95/18±15/156) در گروه مداخله با گروه کنترل هیچ اختلاف آماری معناداری نداشتند (05/0<P)؛ پس از مداخله شاهد تاثیرات مثبت طب فشاری گوش بر کاهش سطح اضطراب پنهان (91/4±09/33) و آشکار (01/5±18/32) و هورمون کورتیزول (80/17±59/115) در گروه مداخله بودیم (005/0>P) حال آنکه هیچ تغییر معناداری در متغیرهای ذکر شده در گروه کنترل مشاهده نشد (05/0<P).
نتیجه گیریطب فشاری گوش در بیماران مبتلا به سرطان پستان که دوره های رادیوتراپی را سپری نمودهاند و قبلا تحت استرس قرار گرفتند، قبل از جراحی ماستکتومی دارای اثرات مثبت و مفید در کاهش سطح استرس (آشکار و پنهان) و هورمون کورتیزول شده است.

E-cadherin در بیشتر بافت های نرمال اپیتلیال بیان می شود. از دست رفتن E-cadherin می تواند منجربه تمایز و تهاجم در انواع کارسینوما شود؛ لذا E-cadherin به عنوان یک سرکوبگر تومور در نظر گرفته می شود. از اینرو، هدف از پژوهش حاضر بررسی اثر اثر تمرینات تناوبی بر بیان ژن E-cadherin بهعنوان ژن سرکوبگر تومور در موش های مبتلا به سرطان پستان است.
روش بررسیبدین منظور 20 سر موش (Balb/c) ماده با میانگین وزن 8/0±17 گرم از موسسه پاستور خریداری و در آزمایشگاه حیوانات دانشگاه تربیت مدرس تحت شرایط کنترل شده نگهداری و به طور تصادفی به چهار گروه (استراحت-سرطان- استراحت، استراحت- سرطان- تمرین، تمرین- سرطان- استراحت و تمرین- سرطان- تمرین) تقسیم شدند؛ گروه های تمرینی پس از آشناسازی وارد شش هفته برنامه تمرینی تناوبی شدند، و پس از پایان دوره تمرینی، موش ها با تزریق سلول سرطان پستان از رده ی 4T1 توموری شدند و مجدد گرو ه های تمرینی وارد چهار هفته برنامه تمرینی تناوبی شدند. 48 ساعت پس از اجرای آخرین جلسه تمرینی موش ها قربانی شدند. بیان ژن E-cadherin، در بافت تومور به روش PCR Real-time بررسی شد. جهت تعیین معنی دار بودن تفاوت بین متغیرها از آزمون ناپارامتریک کروسکال والیس استفاده گردید.
یافته هانتایج تفاوت معنادار ژن E-cadherin را بین گروه های تمرین- سرطان-تمرین و استراحت- سرطان- استراحت نشان می دهد (p=0.03). همچنین این تفاوت معنادار بین گروههای تمرین- سرطان- استراحت و تمرین- سرطان-تمرین نیز وجود داشت (p=0.04).
نتیجه گیریبا توجه به نتایج مطالعه حاضر به نظر می رسد، تمرینات تناوبی می تواند به عنوان یک راهبرد غیر دارویی باعث تعدیل بیان ژن های سرکوبگر تومور، تهاجم توموری، متاستاز و التهاب سیستمیک شود و حتی روند پیشروی بیماری و متاستاز را معکوس کند.

سرطان پستان به عنوان یکی از شایع ترین علل مرگ و میر در میان زنان در نظر گرفته می شود. تشخیص زودهنگام سرطان پستان شانس زنده ماندن را افزایش می دهد. مطالعه حاضر جهت پیش بینی دقیق تر و تصمیم گیری موثرتر در درمان بیماران مبتلا به سرطان پستان صورت گرفته است.
روش بررسیمطالعه حاضر که از نوع کاربردی و توصیفی-تحلیلی بر اساس بهره گیری از روش های کامپیوتری است، جامعه ی هدف آن متشکل از 699 مورد بیماران مبتلا به سرطان پستان خوش خیم و بدخیم با 9 متغیر ورودی روی مجموعه داده های بیماری سرطان پستان پایگاه UCI انجام شده است، قبل از نرمال سازی داده ها از الگوریتم EM برای داده کاوی استفاده شده است. سپس از مدل ترکیب شبکه عصبی مبتنی بر ساختار پرسپترون چند لایه با الگوریتم بهینه سازی وال (WOA) برای پیش بینی بدخیمی تومور پستان استفاده شده است و دقت و پیش بینی آن مورد بررسی و مقایسه قرار گرفته است.
یافته هانتایج مطالعه حاضر نشان می دهد که پس از پیش پردازش مجموعه داده های بیماری، دقت الگوریتم پیشنهادی برای داده های آموزش و آزمون به ترتیب برابر با 6/99 و 99 بوده است و همچنین دقت پیش بینی مدل پیشنهادی برابر 4/99 به دست آمد که با مقایسه صورت گرفته نسبت به روش های مختلف یادگیری ماشین در مطالعات دیگر نتیجه خوبی می باشد.
نتیجه گیریبا توجه به اهمیت تشخیص زودهنگام بیماری سرطان پستان، یافته های این مطالعه می تواند به برنامه ریزان و ارائه کنندگان خدمات سلامت در برنامه های تشخیص به موقع این بیماری کمک شایانی نماید.

بروز سرطان پستان آسیب های جسمی و روانی بسیاری را برای بیمار و خانواده او و به ویژه همسر بیماران در پی دارد. بر این اساس، هدف مطالعه بررسی اضطراب، افسردگی و کیفیت زندگی همسر بیماران مبتلا به سرطان پستان، قبل و بعد از درمان می باشد.
روش بررسیپژوهش حاضر، یک مطالعه توصیفی است که در دو مقطع به فاصله حدود شش ماه در کلینیک تخصصی سرطان پستان جهاد دانشگاهی انجام شد. 142 نفر از همسران، به صورت نمونهگیری در دسترس انتخاب و در دو مرحله قبل و بعد از درمان به وسیله پرسشنامههای افسردگی بک، اضطراب بک و فرم کوتاه زمینه یابی سلامت مورد ارزیابی قرار گرفتند.
یافته هامیزان اضطراب و افسردگی شدید در نوبت دوم (بعد از درمان) کمتر از نوبت اول (قبل از درمان) بود. اضطراب (001/0>p) و افسردگی (002/0 >p) همسران پس از درمان کمتر شده بود. میزان رضایت جنسی قبل از درمان نیز بیش از زمان پس از درمان بود. همسران از نظر ابعاد کیفیت زندگی، در خرده مقیاس های مشکلات جسمانی، محدودیت سلامتی و محدودیت سلامتی هیجانی، در زمان قبل از درمان بهتر از بعد از درمان بیمار بودند و اکثریت همسران پس از درمان بیمار مشکلات جسمی بیشتری داشتند.
نتیجه گیریبراساس یافته های پژوهش مبنی بر سطوح متفاوت مشکلات جسمانی، اضطراب و افسردگی همسران در مراحل قبل و بعد از درمان بیمار، لازم است سیستم های بهداشتی درمانی، خدمات آموزشی و مداخلات رفتاری گسترده تری را در مراحل مختلف تشخیص و درمان بیماری، برای بیمار و خانواده بیمار ارایه نمایند.

این پژوهش با هدف مقایسه اثر بخشی درمان مبتنی بر پذیرش و تعهد انطباق یافته زوجی با زوج درمانی یکپارچه نگر بر تعارضات زناشویی زوجین دارای همسر مبتلا به سرطان پستان انجام گرفت.
روش بررسیدر این پژوهش از طرح نیمه آزمایشی از نوع پیش آزمون- پس آزمون (دو گروه آزمایش و یک گروه گواه) استفاده شد. با استفاده از نمونه گیری در دسترس200 نفر از بین زنان مبتلا به سرطان پستان و همسران آنان که در سال 1397 به مرکز آموزشی و درمانی بیمارستان الزهرا تبریز مراجعه کردند، انتخاب و 45 نفر از زنانی که نمرات بالایی در پرسشنامه تعارض زناشویی کسب کردند به عنوان نمونه پژوهش انتخاب و در سه گروه (دو گروه آزمایش و یک گروه کنترل) به صورت تصادفی جایگزین شدند. یکی از گروه های آزمایش طی 16 جلسه 90 دقیقه ای تحت درمان مبتنی بر پذیرش و تعهد انطباق یافته زوجی و گروه آزمایش دیگر تحت زوج درمانی یکپارچه نگر طی 16 جلسه قرار گرفت، اما گروه گواه مداخله ای دریافت نکرد. ابزارهای پژوهش شامل پرسشنامه تعارضات زناشویی بود. داده ها با استفاده از آزمون تحلیل کوواریانس مورد تجزیه و تحلیل قرار گرفت.
یافته هانتایج نشان داد درمان مبتنی بر پذیرش و تعهد انطباق یافته زوجی بر کاهش تعارض زناشویی موثر است (05/0>P). بر اساس نتایج، زوج درمانی یکپارچه نگر منجر به کاهش تعارض زناشویی زنان مبتلا به سرطان میگردد (05/0>P). همچنین نتایج نشان داد تفاوت معناداری بین اثر بخشی درمان مبتنی بر پذیرش و تعهد انطباق یافته زوجی با زوج درمانی یکپارچه نگر بر تعارضات زناشویی زوجین دارای همسر مبتلا به سرطان پستان وجود ندارد (05/0< P).
نتیجه گیریبا توجه به نتایج درمان مبتنی بر پذیرش و تعهد انطباق یافته زوجی و زوج درمانی یکپارچه نگر به عنوان یک درمان انتخابی برای حل مشکلات فردی و ارتباطی زوجین مبتلا به سرطان پستان به کار برده شود.

صفات شخصیتی، اضطراب، افسردگی و ناامیدی در زنان مبتلا به سرطان پستان موجب اثرات منفی بر کیفیت زندگی این بیماران می گردد که سهم اثر این عوامل به صورت دقیق تعیین نشده است؛ لذا هدف مطالعه حاضر بررسی تاثیر صفات شخصیتی، اضطراب، افسردگی و سطوح ناامیدی بر کیفیت زندگی بیماران مبتلا به سرطان پستان است.
روش بررسیدر این مطالعه مقطعی ارتباط صفات شخصیتی، اضطراب، افسردگی، ناامیدی و کیفیت زندگی 180 زن مبتلا به سرطان پستان (90 بیمار و 90 نفر سالم) بیمارستان امام رضا و طباطبایی شهر تبریز که به روش نمونه گیری در دسترس وارد مطالعه شدند، به کمک پرسشنامه های مقیاس ناامیدی بک، مقیاس اضطراب بک، مقیاس افسردگی بک و مقیاس کیفیت زندگی SF_36 (دارای روایی و پایایی قابل قبول) بررسی شد. از آزمونهای تی تست و ضریب همبستگی پیرسون برای تحلیل داده ها استفاده شد و میزان P کمتر از 05/0 معنی دار تلقی گردید.
یافته هاهمبستگی بین متغیرهای مختلف با کیفیت زندگی حاکی از ارتباط منفی بین روان رنجوری (13.00±5.11)، افسردگی (8.90±7.07)، ناامیدی (4.88±3.60) و اضطراب (13.96±10.20) و همچنین ارتباط مثبت برونگرایی (10.08±3.91) با تمامی ابعاد کیفیت زندگی بود. آنالیز همبستگی بین صفات شخصیتی با افسردگی، ناامیدی و اضطراب نشان میدهد که همبستگی مثبت بین مقیاس روان رنجوری با افسردگی، ناامیدی و اضطراب وجود دارد حال آنکه هبستگی منفی بین سایر ابعاد صفات شخصیتی (برونگروایی، روانپریشی و دروغگویی) و افسردگی، ناامیدی و اضطراب وجود دارد.
نتیجه گیریاضطراب، افسردگی و ناامیدی در بیماران مبتلا به سرطان موجب کاهش سطح کیفیت زندگی آنان شده است. بیمارانی که خصوصیت برونگرایی دارند، اضطراب و افسردگی کمتری دارند که منجر به بهتر شدن کیفیت زندگی آنها میشود، در حالی که بیماران این خصوصیت را ندارند ممکن است علایم اضطراب و افسردگی بالا و کیفیت پایین زندگی را نشان دهند.

Radiation therapy before mastectomy increases the severity of stress and cortisol hormone. Because of the preference of patients and physicians for nonpharmacologic stress management methods, we conducted the present study with the aim of evaluating the effect of ear acupressure on anxiety and cortisol hormone levels in women receiving premastectomy radiotherapy.
MethodsThis randomized clinical trial was carried out on 66 patients (based on sample size formula) at Imam Reza Hospital in Tabriz during the years 2018-19. The intervention group was given acupressure, twice a day (8 minutes on each ear), for three days before surgery. Serum cortisol level and latent and manifest anxiety (the Spielberger State-Trait Anxiety Inventory) were measured before and after the intervention. Data were analyzed using SPSS 20 software. A P value less than 0.05 were considered significant.
ResultsThere was no significant difference in latent anxiety, manifest anxiety, or cortisol levels between the groups at baseline. Ear acupressure caused a significant decrease in latent anxiety (from 43.22 ± 6.41 to 33.09 ± 4.91), explicit anxiety (from 49.50 ± 5.52 to 32.18 ± 5.01), and plasma cortisol levels (from 156.15 ± 18.95 to 115.59 ± 17.80) (P < 0.005), while no significant changes were observed in the control group (P > 0.05).
ConclusionEar acupressure can reduce anxiety and stress (cortisol) levels in breast cancer patients undergoing radiotherapy before mastectomy.

E-cadherin is expressed in most normal epithelial tissues. Loss of E-cadherin can cause dedifferentiation and invasiveness in human carcinomas, leading E-cadherin to be classified as a tumor suppressor. Therefore, the aim of this study was to investigate the effect of interval training on the expression of tumor suppressor gene E-cadherin in breast cancer-bearing BALB/c mice.
MethodsTwenty female BALB/c mice were purchased from the Pasteur Institute and transferred to the Animal Laboratory of Tarbiat Modares University and randomly divided into four groups of training-cancer-rest, training-cancer-training, rest-cancer-rest, and rest-cancer-training. Cancer was induced by subcutaneous injection of 4T1 cell line. The mice performed an average-intensity interval training for 10 weeks, 5 days per week. Forty-eight hours after the last exercise session, the mice were sacrificed to measure the research variables. Gene expression was investigated using real-time PCR. The Kruskal-Wallis test was used to determine the statistical differences between groups.
ResultsThere was a significant difference in the E-cadherin gene expression between the training-cancer-training and rest-cancer-rest groups (p = 0.03). This difference was also observed between the training-cancer-training and training-cancer-rest groups (p = 0.04).
ConclusionInterval exercise training may influence the expression of major tumor suppressor genes and systemic inflammation involved in the development of metastasis and even reverse this process.

Breast cancer is the most prevalent cause of cancer mortality among women. Early diagnosis of breast cancer gives patients greater survival time. The present study aims to provide an algorithm for more accurate prediction and more effective decision-making in the treatment of patients with breast cancer.
MethodsThe present study was applied, descriptive-analytical, based on the use of computerized methods. We obtained 699 independent records containing nine clinical variables from the UCI machine learning. The EM algorithm was used to analyze the data before normalizing them. Following that, a combination of neural network model based on multilayer perceptron structure with the Whale Optimization Algorithm (WOA) was used to predict the breast tumor malignancy.
ResultsAfter preprocessing the disease data set and reducing data dimensions, the accuracy of the proposed algorithm for training and testing data was 99.6% and 99%, respectively. The prediction accuracy of the proposed model was 99.4%, which would be a satisfying result compared to different methods of machine learning in other studies.
ConclusionConsidering the importance of early diagnosis of breast cancer, the results of this study may have highly useful implications for health care providers and planners so as to achieve the early diagnosis of the disease.

The incidence of breast cancer poses a variety of physical and psychological stresses to the patients and their families, especially husbands. The aim of this study was to evaluate anxiety, depression, and quality of life of husbands of breast cancer patients before and after cancer treatment.
MethodsIn this descriptive study, we recruited husbands of breast cancer patients (n = 142) under treatment at the Breast Cancer Clinic of the ACECR. The participants completed the Beck Depression Inventory, Beck Anxiety Inventory, and SF-36 before and after completion of their wives’ treatments.
ResultsThe husbands had significantly lower anxiety (p < 0.001) and depression (p < 0.002) following their wives’ treatment However, their sexual satisfaction had declined at the posttreatment period. The mean scores on three dimensions of quality of life, i.e., physical functioning, general health perception, and mental health, were also declined after the treatment. Most of the husbands had more physical problems after the treatment of their spouses.
ConclusionIt is necessary for health and medical organizations to provide extensive educational services and behavioral interventions to the patients and their families at different stages of diagnosis and treatment.

The aim of this study was to determine the efficacy of acceptance and commitment therapy adopted for couples with integrative couple therapy in improving marital conflicts of couples with breast cancer.
MethodsIn this semi-experimental study, a pre-post design was used. Using available sampling, we approached 200 couples facing breast cancer referred to the educational and therapeutic center of Alzahra Hospital in Tabriz and enrolled 45 couples with the highest scores on the Marital Conflict Questionnaire in the study. They were randomly divided into three groups: two experimental groups and one control group. One of the experimental groups received 8 consecutive 90-minute sessions of acceptance and commitment therapy adopted for couples, and the other experimental group received 16 sessions of integrative couple therapy. The control group did not receive any intervention. The research tool was the Marital Conflict Questionnaire. Data were analyzed using analysis of covariance.
ResultsThe treatment based on acceptance and commitment therapy was effective in reducing marital conflict (P < 0.05). Integrative couple therapy, too, reduced marital conflict in women with cancer (P < 0.05). There was no significant difference between the two treatment modalities (P > 0.05).
ConclusionBoth acceptance and commitment therapy and integrative couple therapy are effective approaches to solving couples’ individual and communicative problems.

Personality traits, anxiety, depression, and hopelessness negatively affect quality of life in women with breast cancer. However, the contribution of each of these factors is not precisely determined. Therefore, the aim of this study was to investigate the relationships among personality traits, anxiety, depression, hopelessness, and quality of life in patients with breast cancer.
MethodsIn this cross-sectional study, 180 women with breast cancer (90 patients and 90 healthy individuals) were recruited from Imam Reza and Tabatabaee hospitals in Tabriz. Data were collected using the Beck Hopelessness Scale, Beck Anxiety Inventory, Beck Depression Inventory, and Short Form Health Survey (SF-36) (with acceptable validity and reliability). T tests and Pearson’s correlation coefficients were used for data analysis, and P < 0.05 was considered significant.
ResultsQuality of life was negatively correlated with neuroticism, depression, despair, and anxiety and positively correlated with extroversion. Also, there was a positive correlation between neuroticism and depression, hopelessness, and anxiety, while other personality traits were negatively correlated with depression, hopelessness, and anxiety.
ConclusionAnxiety, depression, and hopelessness reduce quality of life of patients with advanced cancer. Patients with extraversion have less anxiety and depression, which leads to a better quality of life, while more nervous patients may show signs of anxiety and depression and low quality of life.
