    مقدمه

     بیماری مزمن و التهابی سیستم عصبی است و پیشرفت بیماری با افزایش سیتوکاین‏های خاص ارتباط دارد. آنسفالومیلیت آلرژیک تجربی، به عنوان یک مدل حیوانی برای بیماری MS در نظر گرفته می‏شود و از آن برای ارزیابی روند بیماری و فرایند درمانی استفاده می‏گردد. با توجه به مطالعات انجام شده در زمینه ی فواید گیاه سیاه دانه، این مطالعه، با هدف بررسی تاثیر ضد التهابی سیاه دانه بر روی موش‏های صحرایی ماده ی مبتلا به آنسفالومیلیت آلرژیک تجربی انجام شد.

    روش ها

     در این مطالعه، تعداد 24 سر موش صحرایی ماده ی بالغ نژاد Lewis (با میانگین سن 8-6 هفته و وزن 220-200 گرم) در چهار گروه (6 = n) تقسیم شدند. گروه شاهد نرمال سالین و موش های صحرایی مبتلا به آنسفالومیلیت آلرژیک تجربی (Experimental autoimmune encephalomyelitis یا EAE) تحت درمان، 05/0 سی سی عصاره ی هیدروالکلی سیاه دانه را در دزهای (50، 100 و 200 میلی گرم/کیلوگرم وزن بدن) به صورت یک روز در میان به صورت تزریق داخل صفاقی دریافت کردند. عامل نکروز دهنده ی تومور آلفا (Tumor necrosis factor-α یا TNF-α)، پروتئین واکنشگر C (C-Reactive protein یا CRP)، نیترات و تغییرات وزن بدن روزانه ثبت گردید. بعد از پایان دوره ی 15 روزه، میزان تولید TNF-α و CRP با استفاده از روش Enzyme-linked immunosorbent assay (ELISA) و میزان نیترات توسط دستگاه High-performance liquid chromatography (HPLC) اندازه گیری شد. اطلاعات به دست آمده، با استفاده از آزمون های آماری One-way ANOVA، Mann-Whitney و آزمون Duncan مورد بررسی قرار گرفت.

    یافته ها

    سطوح سرمی TNF-α، CRP و نیترات سرم وابسته به دز در گروه های تیمار نسبت به گروه شاهد به صورت معنی داری کاهش نشان داد (001/0 > P).

    نتیجه گیری

    به دلیل این که وضعیت التهاب در روند بیماری MS نقش به سزایی دارد، اثرات ضد التهابی، آنتی‏اکسیدانی و محافظتی عصاره ی سیاه دانه در دز (200 میلی گرم/کیلوگرم وزن بدن) موجب کاهش عوامل التهابی و بهبود وضعیت التهابی در موش‏های صحرایی مبتلا به آنسفالومیلیت آلرژیک تجربی می‏گردد.

    مقدمه

     آنتاگونیست گیرنده ی اینترلوکین 1 (IL-1RA)، پروتئینی 153 آمینواسیدی (با وزن مولکولی 83/16 کیلودالتون) است. این پروتئین دارویی، با نام تجاری آناکینرا شناخته شده است و کاربرد موثری در درمان آرتریت روماتوئید دارد. این مطالعه، به منظور بررسی بیان پروتئین نوترکیب آنتاگونیست گیرنده ی اینترلوکین 1 در سویه های باکتری Escherichia coli (E. coli) انجام شد.

    روش ها

     بهینه سازی ژن IL-1 RA با استفاده از GenScript انجام شد و در پلاسمید pUC18 به عنوان وکتور کلونینگ، قرار گرفت. سپس، این پلاسمید با دو آنزیم محدود کننده ی NdeI و BamHI برش خورد. ژن IL-1RA از روی ژل تخلیص شد. ژن IL-1RA به داخل وکتور بیانی وارد شد. سازه ی کاست بیانی به داخل باکتری های E. coli Origami، E. coli BL21 و E. coli Rosetta با روش کلرید کلسیم (CaCl2</sub>) و شوک حرارتی منتقل شد.

    یافته ها

    تشخیص و تایید کلنی های منتقل شده با Colony polymerase chain reaction (Colony PCR) انجام شد. القای این ژن با به کار گیری Isopropyl β- d-1-thiogalactopyranoside (IPTG) انجام گرفت. بیان پروتئین با استفاده از روش های Western blotting و Sodium dodecyl sulfate-Polyacrylamide gel electrophoresis (SDS-PAGE) تایید شد و به وسیله ی رزین نیکل تخلیص گردید. آنالیز بیانی سوش های E. coli ترانسفورم شده تایید کرد که ورود سازه به داخل وکتور بیانی انجام شده است. وزن مولکولی پروتئین بیان شده، 83/16کیلو دالتون برآورد شد.

    نتیجه گیری

    در این مطالعه، آنتاگونیست گیرنده ی اینترلوکین 1 انسانی در باکتری E. coli Origami با مقادیر بالا نسبت به سویه های دیگر E. coli، به طور موفقیت آمیزی تولید شد. باکتری E. coli Origami می تواند به عنوان میزبان مناسب در تولید IL-1RA نوترکیب انسانی مورد استفاده قرار گیرد و این فن آوری قابلیت بومی سازی دارد.

    مقدمه

    با توجه به شیوع بیماری های قلبی مادرزادی، مطالعه ی حاضر، با هدف بررسی تاثیر تمرین استقامتی و تزریق نانوذره ی نقره در دوران بارداری بر سطح تروپونین T سرمی مادر و شاخص هیستوپاتولوژیک جنین موش سوری انجام شد.

    روش ها

     در این مطالعه ی تجربی، 40 سر موش سوری ماده با میانگین وزنی 5 ± 30 گرم به طور تصادفی در چهار گروه 10تایی (تمرین استقامتی، تزریق نانوذره ی نقره، تمرین استقامتی همراه تزریق نانوذره ی نقره و شاهد) قرار گرفتند. گروه تمرین استقامتی، قبل از بارداری به مدت 11 هفته و هر هفته، 5 جلسه تمرین با شدت 65-30 درصد بیشینه ی توان، بین 50-5 دقیقه به دویدن روی تردمیل پرداختند. سپس، بارداری انجام و سه هفته ی دیگر، موش های باردار با شدت 60-20 درصد بیشینه ی توان روی تردمیل دویدند. گروه دوم، از روز دوم بارداری به میزان 400 میلی گرم/کیلوگرم وزن بدنشان به صورت درون صفاقی، نانوذره ی نقره را دریافت نمودند. گروه سوم، تمرین و تزریق را همانند دو گروه دیگر دریافت کردند. گروه شاهد فعالیتی نداشتند. در روز نوزدهم بارداری، خون گیری از مادر انجام و میزان تروپونین T سرمی خون مادر، اندازه گیری شد و برای ارزیابی تغییرات هیستوپاتولوژی، قلب جنین استخراج و به روش هماتوکسیلین- ائوزین رنگ آمیزی شد.

    یافته ها

    سطح تروپونین T در گروه های تجربی کاهش یافت (05/0 > P). در گروه نانوذره ی نقره و گروه تمرین استقامتی، اختلاف معنی داری با گروه شاهد دیده شد. کمترین میزان آسیب بافتی قلب جنین در گروه تمرین استقامتی و بیشترین میزان آسیب در گروه شاهد وجود داشت.

    نتیجه گیری

    با توجه به نتایج این مطالعه، تمرین استقامتی باعث کاهش سطح تروپونین T سرمی و کمترین میزان آسیب بافتی قلب جنین موش سوری می شود. همچنین، نانوذره ی نقره، باعث کاهش تروپونین و آسیب بافت قلبی جنین موش سوری می شود.

    مقدمه

     آسیب Syndesmosis مچ پا، از شایع ترین آسیب های مچ پا می باشد. این آسیب دیدگی، منجر به ناتوانی طولانی مدت از انجام فعالیت های فیزیکی می گردد. روش های مختلف جراحی، جهت درمان به کار رفته اند که هر یک با محدودیت ها، مزایا و معایبی همراه می باشند و یافته های مطالعات در این زمینه، با یکدیگر متناقض است. از این رو، مطالعه ی حاضر، با هدف مقایسه ی دو روش ثابت سازی نخ محکم (Tightrope) و پیچ (Tricortical-screw) انجام شد.

    روش ها

    مطالعه ی کارآزمایی بالینی تصادفی حاضر، بر روی 30 بیمار با آسیب Syndesmosis مچ پا در بیمارستان های الزهرا (س) و آیت اله کاشانی اصفهان در سال های 96-1393 انجام شد. بیماران به صورت تصادفی به دو گروه Tightrope و Tricortical-screw تقسیم شدند. دو گروه در پی گیری های 6، 12 و 18 ماهه از نظر محدوده ی حرکتی، شاخص American Orthopedic Foot and Ankle Society (AOFAS)، زمان لازم برای برگشت به کار و نیاز به جراحی مجدد، مورد ارزیابی و مقایسه قرار گرفتند.

    یافته ها

     بیماران دو گروه از نظر سن، جنس و شاخص توده ی بدنی (Body mass index یا BMI) تفاوت معنی داری نداشتند (050/0 < P)، اما مدت ریکاوری و نیاز به جراحی مجدد در گروه Tightrope کمتر بود (001/0 > P). در امتیاز AOFAS نیز گروه Tightrope از نظر درد، محدودیت حرکتی، Maximum walk و Walk surface نسبت به Tricortical-screw برتر بود (050/0 > P)، اما در مورد Flexion دورسال و پلانتار، Gait، حرکت Sagittal، حرکت Hind foot و عملکرد دو گروه تفاوتی نداشتند (050/0 < P).

    نتیجه گیری

    طبق یافته های این مطالعه، در آسیب Syndesmosis مچ پا، استفاده از روش Tightrope با نتایج بهتری نسبت به روش Tricortical-screw همراه بود

    مقدمه

    بخشی از اختلالات بینایی به ویژه کم بینایی، ناشی از کارکرد ناقص عصب بینایی است. کودکان کم بینا، با عوارض و مشکلاتی از قبیل انزوای اجتماعی، بی علاقگی به فعالیت ها، افزایش وابستگی به دیگران و تاخیر تحول شناختی همراه می باشند. هدف از انجام این مطالعه، بررسی تاثیر مداخلات عصب- روان شناختی بر عملکرد دیداری در کودکان کم بینا در حوزه ی درک نور، تشخیص و درک رنگ با به کارگیری تمرینات توان بخشی ویژه با استفاده از سامانه ی محرک نوری (Optical stimulus system یا OSS) بود.

    روش ها

    در این پژوهش، ارزیابی عملکرد بینایی در پاسخ به تمرینات پیاده سازی شده بر مبنای متغیرهای وابسته به روش مورد منفرد بر روی 5 کودک کم بینا، به گونه ای طراحی شد که 4 جلسه ی سنجش خط پایه، 8 جلسه ی مداخله ی تمرینات توان بخشی با OSS و 4 جلسه ی پی گیری جهت بررسی پایانی اثر تمرینات، انجام شد و جهت ارزیابی، از محاسبه و میانگین درصد داده های غیر هم پوش (Percentage of non-overlapping data یا PND) و هم پوش (Percentage of overlapping data یا POD) برای متغیرها استفاده گردید.

    یافته ها

     میانگین نمرات POD و PND برای کلیه ی آزمودنی ها و برای همه ی متغیرها به ترتیب 5/92 درصد و 5/7 درصد و به طور متوسط نمرات در مرحله ی مداخله 84/14 درصد و در دوره ی پایایی، 73/13 درصد نسبت به خط پایه افزایش داشت.

    نتیجه گیری

    نتایج نشان دهنده ی بهبود هر 5 آزمودنی در عملکرد درک و تشخیص نور و رنگ بود.

    Background

    Multiple sclerosis (MS) is a chronic and inflammatory disease of the nervous system, and the progression of the disease is associated with an increase in specific cytokines. Experimental allergic encephalomyelitis is considered as an animal model for MS, and is used to assess the disease process and therapeutic process. According to studies on the benefits of Nigella sativa, the aim of this study was to evaluate the anti-inflammatory effect of Nigella sativa on female rats with experimental allergic encephalomyelitis.

    Methods

    In this study, 24 adult Lewis female rat (average age of 6 to 8 weeks, weighing 200-220 g) were selected in 4 groups of 6. The control mice received normal saline and the ones with experimental allergic encephalomyelitis received 0/05 cc of hydroalcoholic extract of Nigella sativa at doses of 50, 100, and 200 mg/kg body weight every other day as intraperitoneal injection. Symptoms of disease and body weight changes were recorded in daily bases. Tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), nitrate, and body weight changes were recorded daily. After the end of the period (15 days), the production of TNF-α and CRP was measured using the enzyme-linked immunosorbent assay (ELISA) and nitrate levels using high-performance liquid chromatography (HPLC). The collected data were analyzed using one-way ANOVA and Duncan tests.

    Findings

    Serum levels of TNF-α, CRP, and nitrate were significantly lower in treatment groups than the control group (P < 0.001).

    Conclusion

    Because inflammation plays an important role in the process of MS, the anti-inflammatory, antioxidant, and protective effects of the extract of dandelion (200 mg/kg body weight) reduce inflammatory factors and improve inflammatory status in rats with experimental allergic encephalomyelitis.

    Background

    Recombinant human interleukin-1 receptor antagonist (IL-1RA) is a protein with 153 amino acids and molecular weight of 16.83 kDa. This drug protein is known as Anakinra, and has an effective application in the treatment of rheumatoid arthritis. This study was conducted to examine the produce of the recombinant IL-1RA protein in Escherichia coli (E. coli) strains.

    Methods

    Codon optimization of IL-1RA gene was done using GenScript, and the gene was cloned in the pUC18 as cloning vector. Then, plasmid was cut by two restriction enzymes including NdeI and BamH1 enzymes. IL-1RA gene was purified from the agarose gel. IL-1RA gene was ligated into expression vector. The constructed expression cassette was transformed into E. coli BL21 (DE3) Origami (DE3) and Rosetta (DE3) using CaCl2</sub> and heat shock method.

    Findings

    Identification and confirmation of transformed colonies was performed using colony polymerase chain reaction (PCR). Induction of this gene was done with isopropyl β- d-1-thiogalactopyranoside (IPTG). The protein expression was analyzed using sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) and western blotting techniques, and it purified by Ni nickel resin. Expression analysis of transformed E. coli strains confirmed that gene integrated into expression host. Molecular weight of expressed protein was estimated to be 16.83 kDa.

    Conclusion

     In this study, Human IL-1RA was successfully produced in E. coli Origami with high quantity other than the rest of E. coli strains. Therefore, E. coli BL21 Origami (DE3) can be used as the suitable host for production of recombinant IL-1RA, and this technology has a potential for localization.

    Background

    Due to the prevalence of congenital heart diseases, this study was performed to evaluate the effect of endurance training and silver nanoparticle injection on maternal serum troponin T level and fetal histopathological index in the mice.

    Methods

    In this experimental study, 40 female laboratory mice with the average weight of 30 ± 5 g were randomly divided in four equal groups of endurance training, silver nanoparticle injection, endurance training together with silver nanoparticle injection, and the control. The endurance-training group did exercises by running on a treadmill before pregnancy for 11 weeks, 5 sessions per week (with the intensity of 30-65 percent of the maximum capability of the mice) from 5 to 50 minutes. After becoming pregnant, these mice continued exercising by running on the treadmill for 3 more weeks with the intensity of 20 to 60 percent of the maximum capability of the mice. The second group received silver nanoparticles intraperitoneally from the second day of pregnancy at a dose of 400 mg per kilogram body weight. The third group had the exercise and received the injections similar to the other two groups. The control group had no activities. Maternal blood sampling was taken on the 19th</sup> day of pregnancy, as well as the evaluation of histopathological changes and staining with hematoxylin-eosin. Then, the maternal serum troponin T levels were measured.

    Findings

    Troponin T levels decreased in the experimental groups (P < 0.05). There was a significant difference between silver nanoparticles and endurance training group. The least amount of fetal heart tissue damage was seen in endurance training group, and the highest damage was observed in the control group.

    Conclusion

     Endurance training reduces the serum troponin T level, and minimizes the tissue damage to fetal heart tissue in laboratory mice. Silver nanoparticles also reduce troponin T level and the damage to fetal heart tissue of mice.

    Background

    Ankle syndesmosis injury is among the common ankle injuries. This injury results in a long-term inability to perform physical activities. Various surgical techniques have been experimented that each accompanies with limitations, advantages, and disadvantages, and controversial outcomes have been presented. In the current study, two techniques of tightrope and tricortial-screw fixation were compared.

    Methods

    The current randomized clinical trial study was conducted on 30 patients with ankle syndesmosis injury in the Alzahra and Kashani hospitals, Isfahan, Iran, during the years 2014-17. The patients were randomly divided into two groups of tightrope and tricortical-screw treatments. The two groups were followed and compared considering the range of motion, American Orthopedic Foot and Ankle Society (AOFAS) score, required time for return to work, and need for reoperation within 6, 12, and 18 months.

    Findings

    Patients of the two groups were not different regarding age, gender, and body mass index (BMI) (P > 0.050); but the recovery time and need for reoperation were significantly less in tightrope group (P < 0.001). According to AOFAS score, tightrope group was better considering pain, activity limitation, maximum walk, and walk surface (P < 0.050); but they were not statistically different regarding plantar and dorsiflexion, gait, sagittal motion, hind foot motion, and function (P > 0.050).

    Conclusion

     Based on the findings of the current study, tightrope fixation technique was accompanied with better outcomes in comparison to tricortical-screw fixation in ankle syndesmosis injury.

    Background

    Optic nerve lesions cause a group of visual impairment and especially low vision. Children with this defect have problems such as social isolation, lack of commitment to activities, increased dependence on others, and delayed cognitive development. In this research, we aimed to study the effect of neuropsychological interventions by using an optical stimulus system (OSS) (in the other words, applying special rehabilitation exercises) on visual performance, perception and recognition of light and color in children with low vision.

    Methods

    In this study, visual acuity assessment was performed in response to the exercises based on the dependent variables of the single subject method on 5 children with low vision. First, 4 sessions of their baseline were measured, and then, 8 intervention session was conducted with the optical stimulator of rehabilitation exercises. Follow-up sessions were conducted to assess the effect of the exercises, and the mean of the percentage of non-overlapping data (PND) and percentage of overlapping data (POD) for variables were used to evaluate the effect of the exercises.

    Findings

    Mean POD and PND scores for all subjects and for all variables were 92.5% and 7.5%, respectively. The average scores in the intervention phase increased by 14.84%, and the scores in the follow-up period were 13.73% higher than the baseline.

    Conclusion

    The results showed improvement in the performance of all 5 children in the perception and recognition of light and color.
