    مقدمه

     پژوهش حاضر با هدف بررسی تاثیر رویکرد روانی- اجتماعی بر میزان افسردگی و کنترل قند خون در بیماران مبتلا به دیابت نوع 2 و افسردگی انجام شد.

    روش ها

     این پژوهش به صورت کارآزمایی بالینی شاهد دار در جامعه ی آماری متشکل از بیماران مبتلا به دیابت نوع 2 دارای پرونده در مراکز جامع خدمات سلامت شهرستان اصفهان در سال 1397 انجام شد. در هر دو گروه، پیش از انجام مداخله، میزان افسردگی با استفاده از پرسش نامه ی افسردگی Beck فرم 21 سوالی (Beck depression inventory-II یا BDI-II) مورد بررسی قرار گرفت و در صورت کسب نمره ی 14 و بالاتر و با رضایت آگاهانه، بیماران به مطالعه وارد شدند. اندازه گیری وزن، قد و آزمایش های Fasting blood sugar (FBS) و Hemoglobin A1C (HbA1C) انجام شد. پس از آن، مداخله ی رویکرد روانی- اجتماعی بر روی 38 نفر از بیماران (گروه مورد) صورت پذیرفت. افراد گروه شاهد (41 نفر از بیماران) نیز خدمات و آموزش های معمول مراکز جامع خدمات سلامت را دریافت کردند. سپس، در تمام آزمودنی ها، آزمایش های FBS و HbA1C و آزمون افسردگی Beck انجام گرفت. نتایج به دست آمده، با استفاده از نرم افزار SPSS و آزمون های آماری Independent t، Paired t و 2</sup>χ مورد تجزیه و تحلیل قرار گرفت.

    یافته ها

     در مرحله ی پس آزمون، میزان HbA1c و سطح افسردگی در افراد گروه مورد در مقایسه با گروه شاهد به طور معنی داری کاهش یافت.

    نتیجه گیری

    بر اساس نتایج پژوهش حاضر، رویکرد روانی- اجتماعی، می تواند در کاهش افسردگی و کاهش سطح هموگلوبین گلیکوزیله ی خون (HbA1c) در بیماران مبتلا به دیابت افسرده موثر باشد.

    مقدمه

    دایسکشن آئورت، از یک پارگی در لایه ی اینتیما منشا می یرد و خون از داخل رگ وارد لایه ی مدیا می شود. با توجه به مرگ و میر بالای بیماری، شناسایی عوامل خطر ناشناخته، می تواند به افزایش طول عمر بیماران کمک نماید. هدف از انجام این مطالعه، بررسی ارتباط گروه های خونی با میزان مرگ و میر بیماران مبتلا به دایسکشن آئورت توراسیک بود.

    روش ها

     در این مطالعه ی مقطعی، 147 بیمار مبتلا به دایسکشن آئورت توراسیک که در یک بازه ی زمانی ده ساله بستری شده بودند، وارد مطالعه شدند. بیماران به دو گروه دارا و فاقد گروه خونی O تقسیم شدند. فقدان گروه خونی O (Non-O) شامل گروه های خونی A، B و AB بود. همچنین، از نظر محل دایسکشن، بیماران به دایسکشن پروگزیمال شامل آئورت صعودی و دیستال شامل آئورت نزولی تقسیم شدند. برای واکاوی متغیرهای کیفی، از آزمون آماری 2</sup>χ و آزمون Fisher's exact و برای واکاوی متغیرهای کمی از آزمون Independent t استفاده شد.

    یافته ها

     از مجموع 147 بیمار، 93 بیمار گروه خونی Non-O و 54 بیمار گروه خونی O داشتند. متوسط سنی افراد گروه Non-O، 04/16 ± 77/58 سال و میانگین سنی گروه O، 32/15 ± 30/59 سال بود. گروه خونی A و O بیشترین فراوانی را در بین جمعیت مورد مطالعه (7/36 درصد) داشت. 54 بیمار (7/36 درصد) در طی بستری در بیمارستان فوت نمودند. بیشترین خطر مرگ در بیماران با گروه خونی AB مشاهده گردید (52/3 = Odds ratio یا OR) و پس از آن، بیماران با گروه خونی A قرار داشتند (67/1 = OR). بیماران با گروه خونی O در معرض کمترین احتمال مرگ داخل بیمارستانی قرار داشتند (40/0 = OR).

    نتیجه گیری
    بیماران مبتلا به دایسکشن آئورت توراسیک که دارای گروه خونی O هستند، به احتمال زیاد مرگ بیمارستانی کمتری دارند.

    مقدمه

     کله سیستکتومی لاپاراسکوپیک، در حال حاضر روش استاندارد طلایی برای درمان سنگ کیسه ی صفرا می باشد. این مطالعه، با هدف بررسی تاثیر کلونیدین قبل از عمل بر روی درد شانه بعد از کله سیستکتومی لاپاراسکوپیک با بیهوشی عمومی انجام شد.

    روش ها

     در این مطالعه ی کارآزمایی بالینی تصادفی شده ی دو سو کور، 64 بیمار کاندیدای کله سیستکتومی لاپاراسکوپیک انتخابی مورد بررسی قرار گرفتند. بیماران به طور تصادفی در هر یک از دو گروه دریافت کننده ی کلونیدین (گروه مورد) و دارونما (گروه شاهد) قرار گرفتند. گروه شاهد، دارونما به طور خوراکی و گروه مورد، 150 میکروگرم کلونیدین خوراکی قبل از بیهوشی دریافت کردند. پیش دارو 90 دقیقه قبل از القای بیهوشی برای بیماران تجویز شد. میزان درد شانه بعد از عمل، بر اساس نمره دهی دیداری و میزان داروی ضد درد مصرفی در 24 ساعت اول مورد ارزیابی قرار گرفت.

    یافته ها

    در ریکاوری بعد از عمل، میانگین نمره ی درد در گروه شاهد نسبت به گروه مورد بیشتر بود و از نظر آماری، تفاوت معنی داری داشت (03/0 = P). نمره ی درد در 6، 12 و 24 ساعت بعد از عمل نیز در گروه مورد کمتر بود، اما این تفاوت از نظر آماری متفاوت نبود (05/0 < P). میزان مصرف داروی ضد درد در گروه مورد در 24 ساعت بعد از عمل به طور معنی داری کمتر بود (02/0 = P). تغییرات همودینامیک ناشی از لارنگوسکوپی در گروه مورد بر گروه شاهد برتری نداشت و دو گروه یکسان بودند، اما میزان کاهش ضربان قلب در زمان قبل از عمل در گروه مورد بیشتر بود (02/0 = P).

    نتیجه گیری

    استفاده ی دز تنهای 150 میکروگرم کلونیدین 90 دقیقه قبل ازالقای بیهوشی برای کله سیستکتومی لاپاراسکوپیک به طور قابل مقایسه ای نمره ی درد شانه بعد از عمل را در ریکاوری کاهش می دهد.

    Background

    The present study aimed to investigate the effect of psychosocial approach on depression and blood glucose control in patients with type 2 diabetes mellitus and depression.

    Methods

    This was a controlled clinical trial study in patients with type 2 diabetes mellitus, who had records in comprehensive health care centers of Isfahan City, Iran, during the years 2018-2019. In both groups before the intervention, depression was assessed using standard Beck Depression Inventory (BDI-II) questionnaire, and if beck score is above 14 and informed consent was signed, the patients were included in the study. Fating blood sugar (FBS) and blood glycosylated hemoglobin (HbA1C) levels, weight, and height was measured. Then, psychosocial approach intervention was applied on 38 patients in intervention group; and the control group (41 patients) received routine health care services trainings. 3 months after the end of intervention, FBS, HBA1C, and BDI were assessed again. The results were analyzed using independent t, paired t, and chi-square tests via SPSS software.

    Findings

    In posttest phase, HbA1c level and level of depression significantly decreased in the intervention group compared to the control group.

    Conclusion

     Based on the results of this study, psychosocial approach can be effective in reducing depression and lowering HbA1c level in patients with type 2 diabetes mellitus and depression.

    Background

     Acute aortic dissection is one of the catastrophic cardiovascular events with high in-hospital mortality. We aimed to assess the distribution of ABO blood groups in patients with acute aortic dissection, and to identify any association between their blood groups and in-hospital mortality.

    Methods

    In cross-sectional study, 147 patients with confirmed acute aortic dissection by computed tomography angiography were included. According to the type of their blood groups, patients were divided into non-O (including A, B, and AB blood groups) and O blood groups. According to the tearing location, patient were divided into proximal (ascending aorta) and distal (descending aorta) dissection groups. The data were statistically analyzed using chi-square, Fisher's exact, and independent t tests.

    Findings

    The mean age of patients was 58.77 ± 16.04 and 59.3 ± 15.32 years in non-O and O blood groups, respectively. From 147 patients with thoracic aortic dissection, 93 patients had non-O and 54 patients had O blood groups. A and O blood groups were seen more frequently among the patients. 54 patients (36.7%) died during their hospital course. Blood groups of AB [odds ratio (OR) = 3.52] and A (OR = 1.67) had stronger correlation with in-hospital mortality. Patients with blood group O had better in-hospital survival (OR = 0.40).

    Conclusion

     It seems that patients with aortic dissection and O blood group have a better in-hospital survival than patients with non-O blood groups.

    Background

    Laparoscopic cholecystectomy is now the gold standard for the treatment of gallbladder disease. The study evaluated the clinical preoperative effects of oral administration of clonidine on shoulder pain after surgery in patients undergoing laparoscopic cholecystectomy with general anesthesia.

    Methods

    64 patients, scheduled for elective laparoscopic cholecystectomy, were recruited for this prospective randomized double-blind comparative study. They were randomly allotted to either placebo or clonidine group. Patients of the placebo group (n = 32) were premedicated with placebo, while those in the clonidine group (n = 32) were premedicated with oral clonidine 150 micrograms prior to anesthesia. The premedication was given 90 minutes before the anticipated time of induction of anesthesia. Postoperative shoulder pain was recorded based on visual analog scale (VAS), and cumulative analgesic requirement was reported in 24 hours.

    Findings

    The postoperative VAS scores of shoulder pain were significantly less in clonidine group compared with placebo group at recovery period (P = 0.03). VAS scores were lower in clonidine group compared with the placebo group at 6, 12, and 24 hours postoperatively, but the differences were not significant (P > 0.05). Analgesic consumption was significantly less in clonidine group during 24 hours postoperatively (P = 0.02). Clonidine was not superior to placebo for attenuation of the hemodynamic responses to laryngoscopy and laparoscopy, but it increased the incidence of preoperative bradycardia (P = 0.02).

    Conclusion

     A single oral dose of 150 micrograms clonidine, 90 minutes before induction of anesthesia for laparoscopic cholecystectomy, significantly reduces postoperative shoulder pain only at recovery period.
