    مقدمه

    امروزه با توجه به افزایش جمعیت سالمندان جهان و روند رشد بیماری آلزایمر، بررسی مهار کننده های Acetyl cholinesterase (AChE) و Butyl cholinesterase (BChE) که تجزیه شدن آن ها در مغز از دلایل اصلی ایجاد این بیماری می باشد، امری ضروری به نظر می رسد. از آن جایی که سنتز و بررسی هر ترکیب جدید بسیار پرهزینه و زمان بر است، محققان سعی بر تخمین میزان فعالیت بیولوژیکی ترکیبات قبل از انجام فعالیت های آزمایشگاهی دارند. تا کنون روش های محاسباتی مختلفی پیشنهاد شده است که از مهم ترین آن ها می توان به بررسی کمی ارتباط ساختار- فعالیت اشاره نمود که بر اساس مدل سازی خطی و یا غیر خطی با استفاده از محاسبه ی توصیف گرهای ملکولی استوار می باشند. مطالعه ی حاضر با هدف شناسایی مدل مناسب جهت پیش بینی اثر بیولوژیکی ترکیبات مرتبط با مهار کننده های آنزیم های ترکیبات استیل کولین استراز و بوتیل کولین استراز با استفاده از مدل سازی Quantitative structure-activity relationship (QSAR) و با روش جنگل تصادفی انجام شد.

    روش ها:

    در راستای پیش بینی فعالیت دارویی ترکیبات AChE و BChE، روش خطی حداقل مربعات جزئی و روش غیر خطی جنگل تصادفی به کار گرفته شدند. به منظور دست یابی به نتایج دقیق تر و قابل اعتمادتر، در هر اجرا به صورت تصادفی، 80 درصد ترکیبات به عنوان نمونه ی آموزشی و سایر ترکیبات به عنوان نمونه ی آزمایشی جهت ساخت مدل و ارزیابی قدرت پیش بینی مدل استفاده شدند.

    یافته ها:

    با اعمال مدل غیر خطی جنگل تصادفی بر روی مهار کننده های استیل کولین استراز و بوتیل کولین استراز، دقت 89 درصد حاصل شد. در پایان، به منظور بررسی دقیق تر عملکرد مدل پیشنهادی، نتایج با خروجی حاصل از روش کیمینه ی خطای جزئی مقایسه گردید. نتایج نشان داد که روش جنگلی تصادفی غیر خطی نسبت به روش مربعات حداقل خطی، عملکرد قوی تری دارد.

    نتیجه گیری:

    مشاهدات بیانگر این است که روش غیر خطی جنگل تصادفی، می تواند در تخمین اثر بیولوژیکی ترکیبات استیل کولین استراز و بوتیل کولین استراز پیشنهادی توسط پزشکان و متخصصان شیمی دارویی بسیار موثر باشد. بنابراین، قبل از انجام آزمایش های حیوانی و انسانی ترکیب پیشنهادی، فعالیت بیولوژیکی این ترکیب به طور تقریبی 90 درصد برآورد می شود. بر اساس میزان فعالیت بیولوژیکی تخمین زده شده، می توان اظهار نظر کرد که «آیا ترکیب دارویی جدید با صرف هزینه و زمان، خطر تبدیل شده به یک داروی جدید را دارد یا خیر؟».

    مقدمه

    رسالت نظام آموزش عالی در حفظ و ارتقای سلامت جامعه شناخته شده است. از سویی، نقش عوامل اجتماعی موثر بر سلامت، بر وضعیت سلامت جامعه بسیار مورد تاکید و نیازمند مطالعات عمیق و وسیع است. در این راستا، مطالعه ی حاضر با هدف شناسایی جایگاه عوامل تعیین کننده ی اجتماعی سلامت در نظام آموزشی عالی انجام شد.

    روش ها:

    مطالعه ی حاضر، به صورت کیفی با مشارکت 20 صاحب نظر در جلسه ی بحث گروهی متمرکز و 8 صاحب نظر در مصاحبه ی نیمه ساختار یافته انجام شد. اطلاعات به روش تحلیل محتوای کیفی در قالب کدها، طبقه های فرعی و طبقه های اصلی گزارش گردید.

    یافته ها:

    یافته های بحث گروهی متمرکز و مصاحبه های نیمه ساختار یافته با صاحب نظران، 500 کد، 17 طبقه ی فرعی و 4 طبقه ی اصلی نظیر آموزش، سیاست گذاری، قانون گذاری و اجرا، مشارکت و پاسخ گویی بین بخشی و دولتی و ضعف حوزه های مرتبط با عوامل اجتماعی را نشان داد.

    نتیجه گیری:

    یافته های این مطالعه، نقش راهبردهای گسترده ی قابل استفاده توسط نظام آموزشی را برای معرفی و تبیین Social determinants of health (SDH) نشان داد. لازم است این یافته ها، توسط سیاست گذاران، برنامه ریزان و مجریان نظام آموزش برای کاربردی نمودن آموزش های نظام سلامت در راستای تمرکز مفید و بهره مندی مناسب در حوزه ی SDH استفاده شود.

    مقدمه

    فعالیت آنزیم مونوآمین اکسیداز A (MAOA) در اختلالات خلق و خو و فعالیت پایین همراه با رفتارهای پرخاشگرانه تغییر می یابد. ژن مونوآمین اکسیداز A که نقش مهمی در تخریب نوروترانسمیترهایی مانند سروتونین (نه اپی نفرین) و دوپامین بازی می کند، حاوی پلی مورفیسم هایی در ناحیه ی پروموتر خود است که بر کارایی رونویسی تاثیر می گذارد. همچنین، ژنوتیپ MA0A-VNTR با اعمال روان شناختی و سطح فعالیت بدنی مرتبط است. هدف از انجام مطالعه ی حاضر، تعیین تنوع ژنتیک و فراوانی آللی MAOA-VNTR در جمعیت جوان مشهدی و مقایسه ی آن با داده های مشابه از جمعیت های سایر کشورها بود.

    روش ها:

    از 124 داوطلب (شامل 56 مرد و 68 زن با میانگین سن 7/5 ± 3/24 سال) مشهدی نمونه ی بزاق تهیه شد. DNA از نمونه ی سلول های گونه استخراج شد و ژنوتیپ MAOA با استفاده از Polymerase chain reaction (PCR) با پرایمرهای اختصاصی ژن شناسایی شدند. پلی مورفیسم ها توسط PCR با استفاده از پرایمرهای Forward: 5′ACAGCCTGACCGTGGAGAAG-3′ و Revers: 5′GAACGTGACGCTCCATTCGGA-3′ شناسایی شدند و منجر به محصولات 321 جفت باز (آلل 3 تکرار)، 336 (آلل 5/3 تکرار)، 351 (آلل 4 تکرار) و 381 (آلل 5 تکرار) شد.

    یافته ها:

    از نتایج ترتیب توالی ناحیه ی پروموتر R5/3، R5/4 و R 5/5 تکرار با موفقیت به جای تکرارهای R3، R4 و R5 که توسط محققان قبلی معرفی شده بود، شناسایی شد. نتیجه ی ژنوتایپ MAOA-VNTR نشان داد که به ترتیب سه ژنوتیپ 5/4*5/4، 5/3*5/3 و 5/4*5/3، با حدود 77/46، 61/26 و 96/20 درصد و دو آلل 5/3 و 5/4 با 50/37 و 06/58 درصد شایع ترین آلل ها بودند. نتایج مطالعه ی حاضر در مقایسه با داده های دوازده جمعیت، تفاوت معنی داری بین فراوانی آلل با سایر جمعیت های عراقی، آفریقایی، نیوزلندی (با منشا اروپایی)، آلمانی، ایتالیایی، چینی و آمریکایی داشت (05/0 > P).

    نتیجه گیری:

    فراوانی آلل MAO-A بین گروه های نژادی متفاوت است و فراوانی آلل R5/4 در جمعیت مردان و زنان مطالعه ی حاضر بیشتر بود؛ در نتیجه، همان گونه که در مطالعات گذشته نیز تایید شده است، افراد دارای آلل R5/4 به دلیل فعالیت بالای رونویسی ژن MAOA از سطح فعالیت بدنی، افسردگی و انگیختگی پایین تری برخوردار بودند.

    Background

    Due to the growing population of the elderly and the increasing trend of Alzheimer's disease, evaluation of acetylcholinesterase (AChE) and butyl cholinesterase (BChE) inhibitors, as major causes of Alzheimer's disease, is essential. Since the synthesis and investigation of each new compound is very costly and time-consuming, computational modeling techniques have been used to estimate biological activity. Up to now, various computational methods have been proposed which one of the major approaches, quantitative structure activity relationship, is based on the linear and non-linear methods using calculating the independent molecular descriptors. This study aimed to improve the biological activity prediction of AChE and BChE inhibitors using nonlinear random forest algorithm.

    Methods

    In order to predict the biological activity of AChE and BChE compounds, linear partial least squares and nonlinear random forest algorithms were used. To obtain more accurate and reliable results, 80% of the compounds were randomly used as a training sample, and the rest as a test sample, to construct the model and evaluate the predictive power of the model.

    Findings

    By applying nonlinear random forest model on AChE and BChE inhibitors, the accuracy of 89% was achieved. Finally, in order to examine more accurately the performance of the proposed model, the results were compared with the results obtained from the minimum partial error method, and the nonlinear random forest method had stronger performance than linear least squares method.

    Conclusion

    The observations indicated that the nonlinear random forest method could be very effective in predicting the biological activity of AChE and BChE compounds proposed by physicians and pharmaceutical chemists. Therefore, before animal and human testing of the proposed compound, the biological activity of the compound was estimated approximately to be 90%. Based on the estimated biological activity, it can be argued that a new drug combination, at the expense of time and money, has the potential of becoming a new drug or not.

    Background

    The higher medical education system’s mission is to maintain and improve the health in society. Furthermore, the roles of social determinants of health in health situation of society have been emphasized, and require in-depth and expansive studies. Therefore, the current study aimed to identify the position of social determinants of health in higher medical sciences education system.

    Methods

    This study was conducted using qualitative approach with 20 experts in focus group discussion, and 8 experts in semi-structured interviews. The gathered information was reported using qualitative content analysis approach using codes, subcategories, and main categories.

    Findings

    The findings of focus group discussion and semi-structured interviews with experts resulted in 500 codes, 17 subcategories, and 4 main categories of education, policymaking, legislation, and implementation, participation and accountability in public and private sectors, and weakness in areas related to social determinants of health.

    Conclusion

    The findings of this study play a strategic role in medical education system for identification and expansion of social determinants of health. These findings can be used by policymakers, planners, and executives of education system to apply the teachings provided in medical education system, and create a more focused and productive system in the area of social determinants of health.

    Background

    Monoamine oxidase A (MAOA) enzyme activity is changed in mood disorders, as well as lower activity associated with criminal and aggressive behaviors. The MAOA gene, which plays a vital role in degradation of neurotransmitters such as serotonin, norepinephrine, and dopamine, </em>contains a polymorphism in its promoter region that affects transcriptional efficiency. Moreover, MAOA-VNTR genotype has been related with both physical activity level and psychological measures. The aim of the current study was to determine the genetic variation and allelic frequency of the MAOA-VNTR in young population, and to compare the result with similar data from other populations.

    Methods

    Salvia samples were obtained from 124 volunteers in Mashhad, Iran (age: 24.3 ± 5.7 years, 55 men and 67 women). DNA was extracted from the cheek cell, and the MAOA genotype was identified using polymerase chain reaction (PCR) with gene specific primers. The polymorphism was detected by PCR using primers: Forward: 5′ACAGCCTGACCGTGGAGAAG-3′ and Revers: 5′GAACGTGACGCTCCATTCGGA-3′, and resulted in following products: 321 (3-repeat allele), 336 (3.5-repeat allele), 351 (4-repeat allele), and 381 (5-repeat allele).

    Findings

    From the sequencing results, 3.5R, 4.5R and 5.5R repeats were successfully identified instead of the 3R, 4R, 5R repeats which were identified by previous researchers. The result of the MAOA-VNTR genotyping revealed that the three common genotype were 4.5*4.5, 3.5*3.5, and 3.5*4.5 with frequency of 46.77%, 26.61%, and 20.96%, respectively, and the two common alleles were 4.5R and 3.5R with frequency of 37.50% and 58.06% respectively. The result of our study, compared with data of twelve populations, showed a significant difference between allele frequencies of Iranian samples with Iraqi, Afrikaner, New Zealand (European origin), German, Italian, Chinese and American populations (P < 0.05).

    Conclusion

    The frequency of MAOA allele is different between racial groups, and the frequency of the 4.5R allele is greater in the men and women populations of our study. As a result, as previously confirmed, individuals with the 4.5R allele have lower levels of physical activity, depression, and arousal due to high MAOA gene transcription activity.
