کمبود ویتامین D و ناباروری، دو معضل مهم سلامت در ایران است. برخی مطالعات نشان می دهد که ویتامین D بر روی هورمون ضد مولرین (AMH) و تعداد فولیکول های آنترال (AFC) به عنوان ذخیره تخمدان تاثیر می گذارد.
هدفهدف از این مطالعه بررسی تاثیر ویتامین D بر غلظت سرمی AMH/AFC است.
مواد و روش هادر این مطالعه 240 نفر از زنان نابارور که بین ماه های خرداد تا پایان آذر 1396 به واحد IVF یک بیمارستان دانشگاهی مراجعه کرده بودند وارد مطالعه شدند و خصوصیات جمعیت شناختی شرکت کنندگان، و همچنین سطح سرمی ویتامین D، AMH و معاینه اولتراسونیک AFC ثبت شدند.
نتایجدر نهایت، 287 زن نابارور مورد تجزیه و تحلیل قرار گرفتند با میانگین سنی 95/29 سال و انحراف معیار 73/4 سال (رنج سنی 18 تا 45 سال) و میانگین نمایه توده بدنی 41/4±11/25 کیلوگرم بر متر مربع بود. میانه AMH و ویتامین D به ترتیب 20/3 و 82/22 نانوگرم در میلی لیتر بود. با در نظر گرفتن کات آف 20 نانوگرم در میلی لیتر، 7/58% بیماران کمبود ویتامین D داشتند و آنالیز رگرسیون هیچ ارتباطی بین میزان AMH و ویتامین دی (161/0=P)، حتی پس از تعدیل متغیرهای اولیه (182/0=P) نشان نداد. در مجموع 120 بیمار AFC< 6 و 164 نفر ≥6 داشتند که بین گروه های دارای سطح طبیعی یا کمبود ویتامین D اختلاف آماری معنی داری نداشتند (133/0=P).
نتیجه گیریدر این مطالعه مقطعی، هیچ ارتباط معنی داری بین سطح سرمی ویتامین D و AMH یا AFC در زنان نابارور حتی پس از تعدیل متغیرهای پایه دیده نشد.

دیابت ملیتوس یک اختلال متابولیکی است که به علت نقص در ترشح انسولین یا ناتوانی بافت ها در پاسخ به انسولین ایجاد می شود. این بیماری می تواند منجر به آسیب بافت بیضه و کاهش کیفیت مایع منی شود. بنابراین می تواند منجر به ناباروری در مردان شود. درمان های گیاهی مختلف برای کنترل دیابت و عوارض آن استفاده می شود.
هدفهدف این مطالعه ارزیابی اثرات دیابت القا شده با استرپتوزوتوسین بر پارامترهای اسپرم و قابلیت لقاح آزمایشگاهی، و بررسی اثرات محافظتی عصاره متانولی Equisetum arvense در موش های سوری دیابتی بود.
مواد و روش ها24 قطعه موش سوری نر بالغ در چهار گروه کنترل-شم، دیابتی، دیابتی درمان شده با عصاره دم اسب با دوزmg/kg 250 و دیابتی درمان شده با عصاره دم اسب با دوزmg/kg 500 تقسیم شدند. پس از طی 45 روز، به منظور ارزیابی پارامترهای اسپرم (شامل قابلیت زنده ماندن، تعداد، تحرک، مورفولوژی و یکپارچگی DNA/کروماتین اسپرم) و قابلیت لقاح آزمایشگاهی، نمونه های اسپرم از دم اپی دیدیم جمع آوری شدند.
نتایجتحرک و قابلیت زنده ماندن اسپرم ها در گروه های درمان شده با عصاره در مقایسه با گروه دیابتی درمان نشده به طور قابل ملاحظه ای افزایش یافته بود (001/0 ≥p). کاهش تعداد اسپرم ها در گروه دیابتی در مقایسه با گروه های درمانی، از نظر آماری، معنی دار نبود. از طرفی، درصد اسپرم هایی با DNA آسیب دیده، درصد اسپرم هایی با هسته نابالغ و درصد اسپرم های نابالغ با مورفولوژی غیر طبیعی در گروه های درمانی نسبت به گروه دیابتی در سطح معنی داری کاهش یافته بود (001/0 ≥p). میزان لقاح آزمایشگاهی و درصد اووسیت های بارور شده که تا مرحله بلاستوسیست پیشرفت کرده بودند، در حیوانات درمان شده نسبت به گروه دیابتی در سطح معنی داری افزایش یافته بود (001/0 ≥p).
نتیجه گیریعصاره متانولی گیاه دم اسب اثرات مضر ناشی از دیابت بر کیفیت اسپرم و میزان لقاح آزمایشگاهی را مهار می کند که ممکن است به فعالیت های آنتی اکسیدانتی و هایپوگلایسمی در این گیاه مربوط باشد.

استفاده از انتقال جنین فریز شده به عنوان درمان ناباروری در لقاح در خارج از بدن به صورت روزافزون در سراسر جهان رو به افزایش است. روش های متعددی برای آماده سازی آندومتر رحم به منظور انتقال جنین فریز شده مورد بررسی قرار گرفته است.
هدفهدف این مطالعه، بررسی پیامدهای بارداری حاصل از داروی Letrozole با آگونیست gonadotropin releasing hormone (GnRH) در جریان انتقال جنینی فریز شده می باشد.
مواد و روش هااین مطالعه ی کارآزمایی بالینی تصادفی شده بر روی 142 زن نابارور با سابقه ی شکست در انتقال جنین فریز شده انجام شد. شرکت کنندگان به صورت تصادفی در دو گروه 71 نفره شامل گروه آگونیست GnRH و گروه Letrozole تقسیم شدند. گروه GnRH 500 میلی گرم buserelin به همراه 4 میلی گرم استرادیول (که در صورت عدم رسیدن زخامت آندومتر به 5 میلی متر تا 6 میلی گرم افزایش یافت) دریافت کردند و گروه Letrozole 5 میلی گرم Letrozole به همراه 75 واحد هورمون تحریک کننده فولیکول نوترکیب دریافت کردند. برای هر شرکت کننده در گروه ها حداقل 2 جنین با کیفیت بالا انتقال داده شد. میزان بارداری، ثبت ضربان قلب جنین به عنوان پیامدهای مطالعه در نظر گرفته شد.
نتایجشرکت کنندگان در این مطالعه در ابتدای مطالعه خصوصیت های دموگرافیک یکسان داشتند. میانگین ضخامت در دو گروه Letrozole و گروه آگونیست GnRH به ترتیب 88/0±90/8 و 85/0±99/8 میلی متر گزارش شد (57/0=p). میزان تست مثبت BHCG و ثبت ضربان قلب جنین بین دو گروه تفاوت معناداری نداشت (05/0>P).
نتیجه گیریاستفاده از Letrozole و آگونیست GnRH پیامدهای مشابهی را در جریان انتقال جنین فریز شده به همراه داشت.

سندروم آشرمن (AS) یک اختلال نادر باروری است که به علت آندومتریت تهاجمی و یا کورتاژ مکرر سبب تخریب آندومتر می شود.
هدفمطالعه حاضر با هدف بررسی اثربخشی استفاده از پلاسمای غنی از پلاکت (PRP) در کاهش میزان عود چسبندگی داخل رحمی پس از هیستروسکوپی انجام شد.
مواد و روش هادر مطالعه کنترل شده غیر تصادفی، زنان 20 تا 45 ساله مبتلا به سندرم آشرمن تشخیص داده شده با سونو هیستروگرافی، سونوگرافی 3D، هیستروسکوپی و uterosalpingography در دوره زمانی بین ماه مه 2018 تا سپتامبر 2018 مورد مطالعه قرار گرفتند. 30 بیمار در دو گروه مورد و کنترل تقسیم شدند. پس از هیستروسکوپی، کاتتر فولی در دو گروه وارد حفره رحمی شد. پس از دو روز کاتتر فولی خارج شد و یک میلی لیتر PRP به داخل حفره رحم در گروه مورد تلقیح شد و هیچ تلقیح در گروه کنترل اعمال نشد. گروه های مورد مطالعه 8 تا 10 هفته پس از مداخله به منظور ارزیابی چسبندگی داخل رحمی با استفاده از نمره ASRM مورد بررسی قرار گرفتند. این مطالعه در مرکز ثبت کارآزمایی بالینی ایران تحت کد IRCT20110509006420N17 ثبت شد.
نتایجنتایج ما نشان داد که در الگوی قاعدگی هر دو گروه مورد مطالعه قبل و بعد از درمان اختلاف معنی داری وجود ندارد. همچنین مرحله چسبندگی داخل رحمی هر دو گروه مورد مطالعه قبل و بعد از درمان مشابه بود. مدت زمان خونریزی قاعدگی هر دو گروه مورد مطالعه قبل و بعد از درمان نیز مشابه بود.
نتیجه گیریپلاسمای غنی ازپلاکت نمی تواند الگوی قاعدگی را کاهش دهد و چسبندگی داخل رحمی پس از جراحی را که به وسیله هیستروسکوپی بررسی می شود، را کاهش دهد.

ناباروری یک وضعیت شایعی است که حداقل 15% از زوج ها را در سطح جهان درگیر می کند و عوامل مردانه مسول حدود نصف این موارد می باشد. در کامرون در حدود 20 تا 40% زوجین درگیر این مسئله هستند. به هرحال خصوصیات اسپرم در مردان نابارور هنوز در نواحی بهداشتی کامرون برای مدیریت بهتر مسئله بررسی نشده است.
هدفمطالعه حاضر برای بررسی پروفیل اسپرم و فاکتورهای اجتماعی در مردان مراجعه کننده به سرویس های ارولوژی در Dschang Health District طراحی شد.
مواد و روش هایک مطالعه گذشته نگر در 10 سال گذشته در Dschang Health District انجام شد. نتایج آزمایش اسپرم بیماران با استفاده از نرم افزار Epi Info بررسی و به عنوان اسپرموگرام کمی و کیفی حاصل از نتایج بیان شده توسط متخصصان بالینی و وضعیت اجتماعی بیماران ارائه شد.
نتایجاز تعداد کل 379 بیمار 91/83% آنها اسپرموگرام غیر طبیعی داشتند. بیماران بزرگتر از 50 سال بیشترین گروه درگیر بودند. در رابطه با شغل بیماران به طور کلی شغل 51/52% بیماران مشخص بود و از بین این گروه هر چند درصد کشاورزان کمتر از بقیه بود 31/9% ولی این گروه بالاترین درصد اسپرموگرام غیرطبیعی را داشتند 74/94%.
نتیجه گیریاین مطالعه نشان داد که آسیب به اسپرم اصلی ترین علت ناباروری در مردان در Dschang Health District است. این مطالعه همچنین نشان داد که کشاورزان بیشتر در معرض ناباروری هستند که این موضوع می تواند به علت تماس طولانی مدت با حشره کش ها باشد. این نتایج اهمیت بررسی سمیت حشره کش ها بر روی باروری را نشان می دهد.

نقش پاپیلوما ویروس انسانی به عنوان یک عفونت تنها یا متعدد در زنان حامله می تواند مرتبط با تعیین زمان پیشرفت یا پسرفت ضایعات سرویکس باشد.
هدفدر این مطالعه اولیه ما شیوع پاپیلوما ویروس انسانی را بعنوان یک عفونت تنها یا متعدد در زنان حامله شمال شرقی مکزیک تعیین کردیم.
مواد و روش هانمونه 31 زن حامله و 62 زن غیر حامله از ژانویه تا نوامبر 2015 در UMAE-23 of the Instituto Mexicano del Seguro Social (IMSS) مکزیک آزمایش شد. نمونه های ترشحات سرویکوواژینال جهت تشخیص DNA پاپیلوما ویروس انسانی با روش INNO-LiPA تست شد و عفونت پاپیلوما ویروس انسانی بعنوان عفونت منفرد یا متعدد مورد مطالعه قرار گرفت. شرکت کنندگان یک پرسشنامه در باره خصوصیات اجتماعی، ژنیکولوژی، حاملگی و رفتارهای جنسی را پر کردند.
نتایجمتوسط سن خانم های حامله 7/25±8/4 سال بود و متوسط سن حاملگی 6±1 ماه در زمان مطالعه بود. در ارتباط با سن، تعداد حاملگی، تاریخچه استعمال دخانیات و استفاده از قرص های ضد حاملگی خوراکی هیچ تفاوتی بین دو گروه مشاهده نشد. عفونت با پاپیلوما ویروس انسانی در گروه زنان حامله 7/2 برابر گروه کنترل بود (35% در مقابل 13%). در کل 78% خانم های حامله ای که از نظر عفونت مثبت بودند عفونت منفرد داشتند در مقابل 28% خانم های غیر حامله.
نتیجه گیریشیوع بالاتری از عفونت منفرد پاپیلوما ویروس در خانم های حامله مکزیکی مشاهده شد. پیگیری این امر از نظر بررسی پیشرفت یا پسرفت عفونت مهم است.

یک میوم سرویکال حجیم نادر در یک زن جوان بعنوان یک کابوس برای متخصصین زنان زایمان مطرح است از این نظر که نیاز به تکنیک چالش زایی برای میومکتومی دارد. بعلاوه در 2014 اداره دارو و غذای آمریکا استفاده از تکنیک قطعه قطعه کردن را در میومکتومی لاپاراسکوپیک ممنوع کرده است به دلیل خطر بدخیمی و عفونت در مقایسه با جراحی.
مورد:
این گزارش تجربه ما را در ارتباط با یک مورد میوم سرویکال حجیم قدامی به وزن 473 گرم در یک زن جوان بدون سابقه حاملگی بیان می کند که با موفقیت تحت میومکتومی لاپاروتومیک قرار گرفت. بعد از تشخیص بیماری توسط MRI یک کاتتریزاسیون مجاری ادرای قبل عمل انجام شد. هسته میوما به روش Victor Bonney که از پیشروان میومکتومی است بیرون آورده شد البته همراه با چند مرحله اضافی ساده. ما ازgonadotropin-releasing hormone قبل عمل و همچنین تزریق حین عمل vasopressin یا لیگیشن عروق رحمی استفاده نکردیم. بعد از 6 ماه پیگیری MRI نشاندهنده یک کانال سرویکس بدون ضایعه در وضعیت خط وسط بدون وجود شواهدی از فیبروید بود.
نتیجه گیریبر اساس تجربه ما و مرور مطالعات مرتبط و همچنین ممنوعیت قطعه قطعه کردن میوم در حین لاپاراسکوپیک میومکتومی توسط اداره دارو و غذای آمریکا در حال حاضر میومکتومی لاپاروتومیک برای یک میوم سرویکال حجیم نقش حیاتی را در حفظ باروری دارد. ما حروف اختصاری MUSIC را بعنوان یک راهنمای مفید پیشنهاد می دهیم:MUSIC: M (preoperative MRI), U (prophylactic Ureteric catheterization), S (Shell out the myoma following Bonney's principles i.e. start-up and stay intracapsular), I (Immediate suction to clarify dead space) and C (Close the cavity by spiraling stitch).

Vitamin D deficiency and infertility are two important health problems in Iran. Some studies suggest that vitamin D may influence Anti-Müllerian hormone (AMH) and antral follicle count (AFC) as an ovarian reserve.
ObjectiveThe present study aimed to investigate the impact of vitamin D on AMH serum concentrations/AFC.
Materials and Methodsthree hundred and five infertile women referred to the IVF Unit of Yas hospital, between July and December 2017, were enrolled in this cross-sectional study. The demographic characteristics of the participants, as well as the serum levels of vitamin D, AMH, and ultrasonic examination of AFC were recorded.
ResultsFinally, 287 infertile women were included in the analysis with a mean age of 29.95 ± 4.73 yr (18-45 yr) and a mean Body mass indexof 25.11 ± 4.41 kg/m2. The median AMH and vitamin D levels were 3.20 and 22.82 ng/ml, respectively. Considering the cut-off level of 20 ng/ml, 58.7% were vitamin D deficient. Regression analysis showed no association between AMH and vitamin D levels (p = 0.161), even after adjusting for baseline variables (p = 0.182). A total of 120 patients had an AFC < 6 and 164 ≥ 6, which was not statistically different between the groups with normal level or deficient vitamin D (p = 0.133).
ConclusionThe present cross-sectional study showed no significant association between serum levels of vitamin D and AMH or AFC in infertile women, even after adjusting for baseline variables.

Diabetes mellitus is a metabolic disorder characterized by impaired insulin secretion or the inability of tissues to respond to insulin. This disease can damage the testis and reduce semen quality. Therefore, it can impair the potential for male fertility. Different herbal therapeutic treatments have been used to control diabetes and its complications.
ObjectiveThis study aimed to evaluate the effects of streptozotocin-induced diabetes on sperm and in vitro fertilization (IVF) potential and investigate the protective effects of Equisetum arvense methanolic extract on diabetic mice.
Materials and MethodsTwenty-four adult male mice were divided into four groups: control-sham, diabetic, diabetic + Equisetum extract (250 mg/kg), and diabetic + Equisetum extract (500 mg/kg). After 45 days, sperm samples were collected from the cauda epididymis to evaluate the characteristics of sperm (including viability, count, motility, morphology and chromatin/DNA integrity of sperm) and the IVF potential.
ResultsSperm motility and viability were increased remarkably (p≤ 0.001) in the treated groups compared with the non-treated diabetic group. The decrease in sperm count in the diabetic group compared with the treated groups was not significant. Moreover, the percentage of sperm with DNA damage, nuclear immaturity, and abnormal morphology was decreased significantly (p≤ 0.001) in the treated groups compared with the diabetic group. The treated animals exhibited remarkably higher fertilization rates and a higher percentage of fertilized oocytes that developed toward the blastocyst stage compared with the non-treated diabetic group (p≤ 0.001).
ConclusionThe methanolic extract of the Equisetum arvense inhibited diabetes-induced detrimental effects on sperm quality and fertilization rate, which may have been associated with hypoglycemic and antioxidative activities in this plant.

The use of frozen embryo transfer (FET) is increasing worldwide in the treatment of infertility by in vitro fertilization. Different methods of endometrial preparation for FET have been suggested.
ObjectiveThe aim of this study was to compare the pregnancy outcomes after treatment with letrozole and those after treatment with the combination of gonadotropin-releasing hormone (GnRH) agonist and estradiol in FET.
Materials and MethodsThis randomized controlled trial study was conducted on 142 infertile women with a history of previous FET failure. Participants were randomly assigned to two groups (n = 71 each). The GnRH group received 500 µg of buserelin plus 4mg estradiol (which increased to 8 mg if endometrial thickness was less than 5 mm), and the letrozole group received 5 mg of letrozole plus 75 IU of recombinant human follicle-stimulating hormone). At least two high-quality embryos were transferred to each subject in both groups. The outcome measures were clinical pregnancy rate and fetal heart rate detection.
ResultsSubjects in the study groups had similar demographic characteristics and baseline clinical condition. Mean endometrial thickness in the letrozole and GnRH agonist groups were 8.90 ± 0.88 mm and 8.99 ± 0.85 mm, respectively (p = 0.57). The number of positive results of the beta human chorionic gonadotropin test and detection of fetal heartbeat were not significantly different between the groups (p > 0.05).
ConclusionThe administration of letrozole and GnRH may produce similar pregnancy outcomes in FET.

Asherman’s syndrome (AS) is a rare reproductive abnormality, resulting in endometrial collapse due to aggressive or recurrent endometritis and/or curettage.
ObjectiveWe aimed to assess the effectiveness of using platelet-rich plasma (PRP) to lower the recurrence rate of intrauterine adhesions (IUAs) following hysteroscopy.
Materials and MethodsIn this non-randomized clinical trial, women aged 20-45 years with AS diagnosed by sonohysterography, 3D sonography, hysteroscopy, or uterosalpingography between May 2018 and September 2018 were included. Participants (n = 30) were divided into case and control groups. Following hysteroscopic adhesiolysis, a Foley catheter was placed into the uterine cavity in all women. After two days, the catheter was removed, and 1-mL PRP was injected into the uterine cavity of women in the PRP (case) group, while the control received no PRP. All controls and subjects underwent diagnostic hysteroscopy 8-10 weeks following the intervention to assess the IUAs according to the American Society for Reproductive Medicine scoring system.
ResultsOur results did not reveal any significant difference in the menstrual pattern of either the control or test groups before or after treatment (p = 0.2). Moreover, the IUA stage in both studied groups before and after treatment was similar (p = 0.2). The duration of menstrual bleeding in both studied groups before and after treatment was also similar.
ConclusionPRP cannot change the menstrual pattern or development of postsurgical AS, as evaluated by follow-up hysteroscopy.

Infertility is a common condition affecting at least 15% of couples worldwide, and male factors are involved in about half of this prevalence rate. In Cameroon, about 20%-40% of couples are the victims. However, the sperm characteristics of infertile men are yet to be described in the health districts in Cameroon for better management of male infertility.
ObjectiveThe present study was designed to assess the sperm profile and related sociodemographic factors of men attending the urology services at the Dschang Health District.
Materials and MethodsIt consisted of a 10 yr retrospective study carried out in the Dschang Health District. The results of patients’ semen analysis (SA) were computed using Epi Info software and expressed as qualitative and quantitative spermogram state as described by the clinician and sociodemographic features of those patients.
ResultsOut of the 379 patients studied, 83.91% had abnormal spermogram. Patients older than 50 yr were the most affected when grouped into age categories. With regard to patient’s profession, 52.51% had specified their profession and from that group, although farmers (9.31%) represented the lowest size category, they were the most affected with 94.74% having abnormal spermogram.
ConclusionThis study indicates that the sperm damage is the major cause of male infertility in the Dschang Health District. It also shows that farmers are the most affected category and it could be linked to the long-term exposure to pesticides. These results call for the assessment of the reproductive toxicity of locally used pesticides.

The role of human papillomavirus (HPV) as single or multiple infections in pregnant women would be relevant to determine the time to progression and/or the time to regression of cervical lesions.
ObjectiveIn this preliminary study, we determined the prevalence of HPV as single or multiple infections in pregnant women from Northeastern Mexico.
Materials and MethodsSamples from 31 pregnant and 62 nonpregnant women were examined between January 2015 and November 2015 at UMAE-23 of the Instituto Mexicano del Seguro Social (IMSS). The samples of cervicovaginal exudate were obtained for HPV DNA detection using the INNO-LiPA test, and HPV infections were analyzed as single or multiple infections. Participants completed a questionnaire on sociodemographic, gynecological, obstetric, and sexual behavior characteristics.
ResultsThe mean age of the pregnant women was 25.7 ± 4.8 yr, with an average time of pregnancy of 6 ± 1 months at the time of the study. With respect to age, parity, smoking history, or oral contraceptive use no statistically significant differences between the two studied groups was observed. The HPV infection was 2.7 times higher in pregnant women (35%) than in the control group (13%). In total, 78% of the pregnant women who were HPV-positive presented with single infections compared with 28% of the nonpregnant women.
ConclusionA higher prevalence of HPV as a single infection was found in this sample of pregnant Mexican women. Follow-up is necessary to evaluate the persistence or regression of the infection.

A huge cervical myoma (rare) in a young woman is a nightmare of every gynecologist owing to the associated technical challenges in performing a myomectomy. Moreover, the 2014 US Food and Drug Administration prohibited power morcellation during laparoscopic myomectomy due to the inadvertent spread of occult malignancy and an increased risk of iatrogenic parasitic leiomyoma negatively affected the overall rate of a minimally invasive surgery.
Case:
This report described our experience with a case of a huge anterior cervical myoma (473 gr) in a young nulligravida woman who successfully underwent laparotomic myomectomy. After an initial diagnosis by Magnetic resonance imaging (MRI), we performed preoperative ureteric catheterization. The myoma was enucleated following the footsteps of Victor Bonney, the pioneer of myomectomy, combined with simple additional steps. We did not use preoperative gonadotropin-releasing hormone analog, intraoperative vasopressin injection, or uterine artery ligation. A 6-month follow-up MRI revealed an intact cervical canal in midline position with no evidence of residual fibroid.
ConclusionBased on our experience, the review of the relevant literature, and the US Food and Drug Administration's prohibition of power morcellation during laparoscopic myomectomy, a laparotomic myomectomy for a huge cervical myoma still plays a vital role in fertility preservation. We propose the mnemonic "MUSIC" as a helpful guide for a consistent strategy: M (preoperative MRI), U (prophylactic ureteric catheterization), S (shell out the myoma following Bonney's principles i.e. start-up and stay intracapsular), I (immediate suction to clarify dead space) and C (close the cavity by spiraling stitch).
