هورمون های تیروییدی نقش مهمی در عملکرد طبیعی کبد ایفا می کنند و پرکاری تیرویید منجر به اختلال عملکرد کبد می گردد. هورمون های تیروییدی بر آنزیم های تولیدکننده ی اکسید نیتریک (Nitric oxide, NO) در بافت کبد تاثیر می گذارند. هدف از این مطالعه، تعیین آثار پرکاری تیرویید بر غلظت آنزیم های تولیدکننده ی اکسید نیتریک (NO synthase, NOS) شاملNOS عصبی (nNOS)، NOS قابل القاء (iNOS) و NOS اندوتلیال (eNOS) در کبد موش صحرایی نر می باشد.
مواد و روش ها12 سر موش صحرایی نر به دو گروه شاهد و پرکار تیرویید تقسیم شدند. پرکاری تیرویید با اضافه کردن 12 میلی گرم در لیتر لووتیروکسین به مدت 21 روز در آب آشامیدنی القا شد. غلظت متابولیت های اکسید نیتریک (نیترات+نیتریت یا NOx) در سرم و بافت کبد و همچنین غلظت پروتئین آنزیم های nNOS، iNOS و eNOS در کبد در انتهای مطالعه اندازه گیری شد.
یافته هادر گروه پرکاری تیرویید، غلظت سرمی تیروکسین تام و آزاد بالاتر و غلظت هورمون محرک تیروییدی پایین تر از گروه شاهد بود. غلظت سرمی و کبدی NOx در گروه پرکاری تیرویید به ترتیب 46 و70 درصد بالاتر از گروه شاهد بود. پرکاری تیرویید سبب کاهش غلظت پروتئین eNOS (31 درصد، 0/006=P) و افزایش غلظت پروتئین iNOS (26 درصد، 0/020=p) در کبد شد، اما تاثیری بر غلظت پروتئین nNOS نداشت.
نتیجه گیریپرکاری تیرویید سبب تغییر بیان آنزیم های تولیدکننده ی اکسید نیتریک در کبد می شود. نقص عملکرد اکسید نیتریک ممکن است در پاتوفیزیولوژی پرکاری تیرویید دخالت داشته باشد.

با توجه به اهمیت کنترل فشار خون، قند ناشتای پلاسما (FPS) و کلسترول- لیپوپروتیین کم چگالی (LDL-C) در کاهش عوارض واسکولار در بیماران دیابتی، در این مطالعه به بررسی روند تغییرات درصد افراد بالغ با دیابت شناخته شده که سطوح بالاتر از میزان مطلوب برای این عوامل خطر داشتند پرداختیم.
مواد و روش هااز داده های سه دوره ی مطالعه ی پایش عوامل خطر بیماری های غیرواگیر (SuRFNCD) در سال های 1386، 1390 و 1395 استفاده شد. شرکت کنندگان 4493 نفر از افراد 65-25 ساله بودند که تشخیص قبلی بیماری دیابت را اعلام کرده بودند. روند تغییرات طولی متغیرهای مورد بررسی با رگرسیون لجستیک برآورد شد.
یافته هاطی 1386 تا 1395، روند کاهشی معنی دار در درصد افراد با فشار خون بالا از 48/2 درصد به 40/4 درصد (47/3 درصد به 37/4 درصد در مردان و 53/0 درصد به 41/5 درصد در زنان) و کلسترول- لیپوپروتیین کم چگالی بالا از 75/4 درصد به 46/7 درصد (73/2 درصد به 45/7 درصد در مردان و 77/4 درصد به 48/3 درصد در زنان) در بالغین دیابتی دیده شد. روند قند ناشتای پلاسمای بالا تنها در میان زنان بطور معنادار کاهشی بوده است (51/8 درصد به 45/3 درصد).
نتیجه گیریعلی رغم بهبود قابل توجه در عوامل خطر قلبی عروقی در طی یک دهه گذشته، درصد افراد بالغ دیابتی با سطوح بالاتر از میزان مطلوب برای عوامل خطر قلبی عروقی فاصله زیادی با حد دلخواه دارد و بهبود بیش تر در سطوح این عوامل خطر منجر به کاهش عوارض عروقی در آینده خواهد شد.

مدل ارتقای سلامت پندر به عنوان چهارچوبی جهت تبیین رفتارهای سبک زندگی ارتقاءدهنده ی سلامت در افراد دارای بیماری های مزمن مطرح است. این مطالعه با هدف تعیین تاثیر آموزش بر اساس مدل ارتقای سلامت پندر بر سبک زندگی و ابعاد آن در مددجویان با زخم پای دیابتی انجام شده است.
مواد و روش هادر این مطالعه کارآزمایی بالینی که در سال 1398 انجام شد، 74 مددجو بر اساس معیارهای ورود به روش در دسترس انتخاب و به روش تصادفی بلوک بندی شده به دو گروه آزمون و شاهد تقسیم شدند. پرسش نامه ی مشخصات جمعیت شناختی و ابزار سبک زندگی ارتقاء دهنده سلامت توسط دو گروه تکمیل شد. تجزیه و تحلیل داده ها با استفاده از روش های آماری توصیفی و مقایسه ای مانند: آزمون های تی مستقل، زوجی و تحلیل کوواریانس انجام شد.
یافته هابعد از مداخله، میانگین نمره گروه آزمون در همه ابعاد سبک زندگی به طور معنی داری افزایش داشت (0/001<P). بررسی تغییرات دوگروه، 50 روز بعد از مداخله، نمایان گر افزایش در میانگین نمرات سبک زندگی و ابعاد آن در گروه آزمون در مقایسه با گروه شاهد بود (0/001>P).
نتیجه گیرینتایج به دست آمده از این مطالعه نشان می دهد که آموزش بر اساس مدل ارتقای سلامت پندر می تواند منجر به ارتقای سبک زندگی و ابعاد آن در افراد دارای زخم پای دیابتی شود و بنابراین برای ایجاد تغییر در سبک زندگی مددجویان با زخم پای دیابتی، می توان از این شیوه آموزشی استفاده نمود.

شیوع گلوکز ناشتای مختل (IFG) در جهان در حال افزایش است و تمام رده های سنی از جمله نوجوانان را دربرگرفته است. مطالعه ی حاضر جهت بررسی روند تغییرات گلوکز ناشتای مختل و علل آن در میان نوجوانان ایرانی در طول یک دهه انجام شد.
مواد و روش هاتعداد 2998 شرکت کننده بین سنین 19-11 سال در دوره 1 مطالعه (1380-1378، 1415 پسر و 1583 دختر) و تعداد 946 نفر در دوره 2 مطالعه (1393-1390، 477 پسر و 469 دختر) مورد بررسی قرار گرفتند. جهت بررسی نقش عوامل خطر بالقوه در ارتباط بین دوره مطالعه با شیوع IFG از مدل های رگرسیون لجستیک دوسطحی چند متغیره استفاده شد.
یافته هاشیوع IFG یک جهش دو برابری از 7% در دوره 1 مطالعه به 16/6% در دوره 2 مطالعه نشان داد. در میان عوامل خطر اصلاح ناپذیر، سن بالاتر و جنس مونث با خطر پایین تر IFG همراه بودند. در میان عوامل خطر اصلاح پذیر، در مدل کاملا تعدیل شده، اضافه وزن و چاقی کلی با افزایش خطر IFG همراه بودند در حالی که چاقی شکمی به عنوان یک عامل خطر مستقل شناسایی نشد. با این حال، تعدیل هم زمان برای چاقی کلی و شکمی نتوانست تاثیر ارتباط دوره مطالعه با افزایش شیوع IFG را کاهش دهد. (2/68-1/81) 2/20.
نتیجه گیریدر فاصله سال های 1393-1390 افزایش چشمگیری در شیوع IFG در میان نوجوانان مشاهده گردید و همچنین دوره مطالعه به عنوان یک عامل مستقل پیش بینی کننده شناخته شد. با این حال، تاثیر دیگر عوامل خطر بالقوه می بایست بررسی شوند.

آترواسکلروز بیماری است که با رسوب لیپید و لیپوپروتئین کم چگال (LDL) بر روی دیواره داخلی سرخرگ های با قطر متوسط و بزرگ مشخص می شود. هدف از این مطالعه، مقایسه ی اثر آنتی اسکلروتیک عصاره هیدروالکلی گل خرگوشک و پوست و آب لیموترش و داروی کیناکرین بر غلظت لیپیدهای خون، پروتئین واکنش گر C و سطح فعالیت آنزیم فسفولیپاز A2 در موش های صحرایی نر نژاد ویستار بود.
مواد و روش هادر این مطالعه، 25 سر موش صحرایی نر نژاد ویستار به صورت تصادفی به گروه های شاهد (5 سر) و تغذیه با رژیم پرچرب (20 سر) تقسیم شدند. پس از 2 ماه تغذیه با رژیم پرچرب، موش های مبتلا به هیپرکلسترولمی به 4 گروه شامل: دریافت کننده آب لیموترش به میزان 2/5 میلی لیتر به ازای کیلوگرم وزن بدن، پوست لیموترش به میزان 500 میلی گرم به ازای کیلوگرم وزن بدن ، عصاره هیدروالکلی گل خرگوشک به میزان 800 میلی گرم به ازای کیلوگرم وزن بدن و داروی کیناکرین به میزان 6 میلی گرم به ازای کیلوگرم وزن بدن، تقسیم و به مدت 1 ماه تحت تیمار قرار گرفتند. در پایان؛ تاثیر دارو و عصاره ها بر پروفایل لیپیدی، فسفولیپاز A2، پروتئین واکنش گر C و اینترلوکین 6 در موش های هیپرکلسترولمی مورد ارزیابی قرار گرفت.
یافته هاتغذیه با رژیم پرچرب سبب افزایش مقادیر تری گلیسرید، کلسترول تام و LDL وکاهش لیپوپروتئین پر چگال گردید و تجویز عصاره هیدروالکلی گل خرگوشک، پوست و آب لیموترش و همچنین داروی کیناکرین موجب بهبود وضعیت لیپیدها (0/05>p)، کاهش فعالیت فسفولیپاز A2 و پروتئین واکنش گر C (0/05>p) شد.
نتیجه گیریتیمار با عصاره هیدروالکلی گل خرگوشک، پوست و آب لیموترش و داروی کیناکرین هر یک به تنهایی می تواند در بهبود غلظت لیپیدها، پروتئین واکنش گر C و فعالیت فسفولیپاز A2موثر باشد.

مقدمه چاقی به عنوان یکی از مشکلات قرن حاضر از روند رو به رشدی برخوردار است. امروزه درصد زیادی از افراد اعم از مرد و زن در تمام سنین از معضل چاقی رنج می برند. در این میان ارتباط ابتلا به چاقی و بروز نقص در عملکرد جنسی مشخص گردیده است. چاقی به عنوان یکی از عوامل خطر در بروز بیماری های قلبی عروقی و متابولیکی می تواند با ایجاد اختلال در سیستم ایمونولوژیک و اندوکرین، اندوتلیوم عروق را متاثر سازد. تاثیر چاقی بر بروز نقص عملکرد اندوتلیال عروق و متعاقبا نقص در عملکرد جنسی مردان به خصوص اختلال در نعوظ کاملا مشخص گردیده است. همچنین ابتلا به چاقی با افزایش مقاومت به انسولین موجب تغییراتی مولکولی در عروق، به خصوص در زمینه کاهش دسترسی زیستی به نیتریک اکساید به عنوان منبسط کننده عروق می شود و سبب عدم کارآیی عروق مرتبط با آلت مردانه و کلیتوریس شده و منجر به نقص عملکرد جنسی در مردان و عدم رسیدن به اوج لذت جنسی (ارگاسم) در زنان می گردد. چاقی در زنان با تاثیر بر روی افزایش هورمون های جنسی و خطر بروز سندرم تخمدان پلی کیستیک و ایجاد تغییرات ناخوشایند در ظاهر فرد، همچون بروز آکنه و موی زاید، باعث کاهش مطلوبیت تصویر بدنی و منجر به نقص عملکرد جنسی در آنان می شود. تاثیر چاقی بر بروز نقص عملکرد جنسی از جنبه های ایمونولوژیک، اندوکرینولوژیک، سایکوژنیک، و آناتومیک در این مقاله مروری مورد بررسی قرار گرفته است.

Thyroid hormones play an important role in normal function of liver and hyperthyroidism can cause liver dysfunction. Thyroid hormones affect nitric oxide (NO)-producing enzymes in liver. The aim of this study is to investigate effects of hyperthyroidism on protein levels of three isoforms of NO synthase (NOS) enzymes, including endothelial NOS (eNOS), inducible NOS (iNOS), and neuronal NOS (nNOS) in the liver of male rats.
Materials and MethodsTwelve male Wistar rats were divided into the hyperthyroid and the control groups. Hyperthyroidism was induced by administration of 12 mg/L of levothyroxine in the drinking water of rats for a period of 21 days. NO metabolites (i.e. nitrate+nitrite or NOx) concentrations in the serum and liver as well as iNOS, eNOS, and nNOS protein levels in the liver tissue were measured on day 21.
ResultsSerum levels of free thyroxine and total thyroxine were significantly higher, whereas that of the thyroid stimulating hormone (TSH) was significantly lower in rats with hyperthyroidism. Compared to controls, NOx levels were significantly higher in the serum (46%) and liver tissue (70%) of rats with hyperthyroidism. Hyperthyroidism decreased protein levels of eNOS (31%, P=0.006) and increased protein levels of iNOS (26%, P=0.020) but had no effect on nNOS in the liver.
ConclusionHyperthyroidism changed levels of NO-producing enzymes liver, indicating that dysfunction of NO system may contribute to the pathophysiology of hyperthyroidism.

Control of blood pressure, fasting plasma sugar (FPS) and low-density lipoprotein-cholesterol (LDL-C) reduces to vascular complications of diabetes. In this study we determined the prevalence of diabetic individuals with uncontrolled blood pressure, FPS and LDL-C.
Materials and MethodsWe used data from three national Surveys of Risk Factors of Non-Communicable Disease (SuRFNCD) conducted in 2007, 2011 and 2016. Participants were 4493 adults, aged 25-65 years, who self-reported a previous diagnosis of diabetes. Logistic regression was used to determine p-value for trends of prevalence rates.
ResultsFrom 2007 to 2016, significant reductions in the prevalence of individuals with high blood pressure from 48.2 % to 40.4 % (47.3 % to 37.4 % in men and 53.0 % to 41.5 % in women) and also in those with high LDL-C levels from 75.4 % to 46.7 % (73.2 % to 45.7 % in men and 77.4 % to 48.3 % in women) were observed. Only women showed a significant decrease in the prevalence of those with high FPS.
ConclusionDespite significant improvements during the last decade, achieving goals of these risk factors remains suboptimal among Iranian adults with diagnosed diabetes. Considerable further improvement is needed to control and reduce future vascular complications.

The Pender’s health promotion model is a framework used for determining health-promoting lifestyle behaviors in people with chronic diseases. This study aims to investigate the effect of educational intervention based on the Pender’s health promotion model on the lifestyles of patients with diabetic foot ulcers.
Materials and MethodsIn this clinical trial conducted in 2019, 74 patients were selected based on inclusion criteria by convenience sampling and allocated into two groups, the intervention and control, using the randomized block method. Both groups completed the demographic characteristics- and the Health-Promoting Lifestyle Profile Questionnaires. Data obtained were analyzed using SPSS 16, using descriptive & comparative statistical methods, including independent t-test, paired t-test and ANCOVA.
ResultsAfter the intervention, a significant increase was observed in the average score of the intervention group in all lifestyle dimensions (P<0.001). Comparing the changes in the average scores of lifestyle and its dimensions in both groups 50 days after the intervention, an increase was observed in the average scores of lifestyle and its dimensions in the intervention group, compared to those in the control group (P<0.001).
ConclusionEducational intervention based on the Pender’s health promotion model can promote the lifestyles of patients with diabetic foot ulcers, results of which can be used in educational interventions aimed at patients with diabetic foot ulcers to change their lifestyles.

The prevalence of impaired fasting glucose (IFG) is increasing worldwide, affecting people in all age groups, including adolescents. The objective of this study was to explore changes in IFG prevalence during a decade long follow-up in adolescent participants of a population-based cohort study from Iran.
Materials and MethodsWe included 2998 participants, aged 11-19 in study period 1 (1999-2005; 1415 boys, 1583 girls) and 946 participants in study period 2 (2011-2014; 477 boys, 469 girls). Multivariate binary logistic regression models were performed to examine the role of susceptible risk factors in the relationship between study periods with IFG.
ResultsA two-fold increase was observed in the prevalence of IFG, from 7% in study period 1 to 16.6% in study period 2. Regarding non-modifiable risk factors, older age and female gender were associated with lower risk for IFG. Among modifiable risk factors in the fully adjusted model, being overweight and obese were significantly associated with IFG, whereas central obesity was not found to be an independent risk factor. Yet, adjusting for general and central obesity could not attenuate the increase in IFG over time.
Conclusionthe prevalence of IFG among Iranian adolescents increased significantly and study period remained a significant factor after all adjustments. We propose the role of other potential risk factors be further investigated.

Atherosclerosis is a disease of the veins characterized by the deposition of low-density lipoprotein (LDL) cholesterol and total cholesterol on the inner wall of medium and large diameter arteries. The aim of this study was to compare the anti-sclerotic effect of hydroalcoholic extract of verbascum Thapsus, lemon juice, lemon peel and quinacrine on blood lipid concentration, C-reactive protein (CRP) and phospholipase A2 (PLA2) enzyme activity in male Wistar rats.
Materials and MethodsIn this study, 25 male wistar rats were randomly divided into the control (n=5) and high fat diet groups (n=20). After 2 months of a high fat diet, rats with hypercholesterolemia were divided into 4 groups: receiving 2.5 ml/kg body weight lemon juice, 500 mg/kg body weight, lemon peel, 800 mg/kg body weight hydroalcoholic extract of verbascum thapsus and 6 mg/kg body weight Quinacrine and treated for one month. Finally, the effects of the drug and extracts on lipid profiles, PLA2 and CRP were evaluated in hypercholesterolemic rats.
ResultsThe high-fat diet increased triglycerides, total cholesterol, and LDL levels and decreased high-density lipoprotein. Administration of hydroalcoholic extract of verbascum thapsus, lemon peel and juiceand quinacrine improved lipid status and decreased phospholipaseA2, CRP activity (P<0.05).
ConclusionTreatment with hydroalcoholic extract of verbascum thapsus, lemon peel and juice and quinacrine can improve lipid concentrations, CRP and phospholipaseA2 activity.

Introduction Obesity, one of the major growing problems of the present century is reaching pandemic proportions. Today, a large percentage of men and women of all ages suffer from obesity. The relationship between obesity and its effect on sexual dysfunction through different mechanisms has been documented. Obesity, a risk factor for cardiovascular and metabolic diseases with impaired immune system and endocrine function through its effect on the vascular endothelium, can play a major role in the development of vascular defects in the reproductive system, causing sexual dysfunction especially in men. The mechanism of the effect of obesity on vascular epithelium is the origin of vascular endothelial dysfunction and consequently sexual dysfunction of men, viz. erectile dysfunction which has been well established. The association between obesity and increased insulin resistance has also led to molecular changes in the vasculature, especially in the context of reduced bioavailability of nitric oxide as a vasodilator, leading to dysfunction of the penis and clitoris, leading to sexual dysfunction in men and not achieving orgasm in women. In women, the role of obesity affects the development of sex hormones and the incidence of polycystic ovary syndrome and causes unpleasant changes in the appearance of the person such as acne and hair loss in women, affecting the body image and resulting in sexual dysfunction. The impact of obesity on sexual dysfunction from the immunological, endocrinological, psychogenic, and anatomical aspects has been reviewed in this study.
